This research looks at using Inclusive Design as mean to bridge the age-based digital divide, by conducting Action research in a Local Council in the UK. Research on the digital divide looks at the inequality in technology access between different demographical characteristics. While this is very useful to highlight access inequality, it fails to address the steps required to bridge the gap. Some research even suggested that the digital divide is bridged and that research in this field is irrelevant. This is due to the narrow focus of some researchers about the digital divide. The focus should be not on whether older people possess Information and Communications Technology (ICT) products; it should be on how they use their products and what we can do to improve the person-technology fit. On the other hand, inclusive design has been a main driver to ensure that buildings and products do not prohibit people with disabilities from utilising the building or product. However, there is limited research about the extension of inclusive design practices in Information Systems (IS), particularly interface design and system design. Most of the current design guidelines and best practices do not provide a holistic view of the inclusive design process, thus leaving the designers on their own in terms of producing an easy interface for users, by just merely implementing the accessibility guidelines provided by the W3C to ensure inclusivity of the website or online service. Assuming that inclusivity is reached only by including screen readers and magnified modes to prospective users. The aim of the research is to provide a clear picture to discover the main challenges to Inclusive Design as well as provide a holistic and thorough inclusive design metric that allows designers to ensure that their proposed website or online service is inclusive of users from different age groups and capabilities. This design methodology can customisable according to the website or online service requirements, highlighting the important features to ensure inclusivity. Using Action research as methodology to investigate and improve design practices in a local council. The research employed a mixture of research methods to understand and intervene in the creation of an inclusive e-service during the implementation of the Care Act 2014 in the Local Council. First, semi structured interviews were conducted to provide insights of the changes that the Care Act 2014 which brought upon social care and the way that social care services are being provided in the county. Then, observations were performed during the design sessions of the online services in the Local Council to see how the design process is being carried out to ensure that the website is inclusive of all prospective users. Finally, when the website is fully developed it was tested on various users with from different age groups using the inclusivity index developed as part of the evaluation phase of the design methodology. In addition, interviews with older adults were conducted to understand the issues that make them using ICT difficult so the needs of these users were documented to help in the design process later on. The research indents to advance knowledge about inclusive design from practical and theoretical perspective. Practically, this research will help web designers and organisations looking to cater online services to as many people as possible and design methodology that fosters inclusivity. Moreover, this will help older adults to use online service by themselves, thus ensure that they can remain independent and exercise control over their needs from the government or otherwise. Theoretically, this research will help to shed the light on the digital divide and the ways to bridge this divide. By creating an inclusive design methodology that can fundamentally address the causes of the divide rather than just simply patch existing design methodologies.

L’implication de la proprotéine convertase PACE4 dans le cancer du sein / The functions of the proprotein convertase PACE4 in breast cancerPanet, François January 2017 (has links)
Mondialement, le cancer du sein est celui le plus fréquent et mortel chez les femmes. Malgré un programme de dépistage national et de nouveaux traitements, le taux de rechute demeure haut. Ceci illustre le besoin de nouvelles thérapies. Même dans un pays industrialisé comme le Canada, ce cancer est toujours le deuxième plus mortel chez les femmes. Nos travaux précédents ont identifié PACE4, un membre de la famille des proprotéines convertases (PCs), comme étant une nouvelle cible thérapeutique dans le cancer de la prostate. Dans cette étude, nous avons vérifié si PACE4 pourrait aussi être une cible dans le cancer du sein. À l’aide d’échantillons clinique de cancer du sein, nous avons premièrement démontré que PACE4 est spécifiquement surexprimé dans le sous-type moléculaire positif aux récepteurs hormonaux. Par la suite, nous avons sélectionné une lignée cellulaire pour poursuivre nos expériences. Nous avons choisi la lignée de cancer du sein ZR-75-1 parce qu’elle provient du sous-type positif aux récepteurs hormonaux et qu’elle exprime PACE4. En culture cellulaire, nous avons comparé la croissance de cellules ZR-75-1 modifiées qui sous-exprimaient furine, PACE4 ou PC7. Lors de ces expériences, PACE4 était la seule PC importante pour la prolifération cellulaire. L’importance de PACE4 pour la croissance tumorale a aussi été confirmée in vivo à l’aide d’un modèle de xénogreffes de cellules ZR-75-1 chez des souris immunosupprimées. Les inhibiteurs peptidiques spécifiques à PACE4 C23 et Multi-Leu (ML) ont aussi été testés en culture cellulaire où ils étaient en mesure de diminuer la prolifération des cellules ZR-75-1 et MCF-7. Nous avons aussi démontré, à l’aide d’un peptide ML radiomarqué, que l’expression de PACE4 est nécessaire pour que les peptides inhibiteurs pénètrent dans la cellule. De plus, le peptide C23, lorsqu’administré systémiquement à des souris immunosupprimées portant des tumeurs de cancer du sein, était en mesure de diminuer la croissance tumorale. De manière intéressante, les tumeurs sous-exprimant PACE4 et celles traitées avec le peptide C23 démontraient un ralentissement de la croissance tumorale similaire. De plus, lorsque ces tumeurs ont été analysées en immunohistochimie, elles possédaient une diminution du marqueur de prolifération Ki67 et une augmentation de cellules positives aux marqueurs d’arrêt du cycle cellulaire p27KIP1 et p21Waf1/Cip1 comparativement aux tumeurs contrôles. Pour conclure, la sous-expression de PACE4 et l’administration systémique d’un inhibiteur de PACE4 en culture cellulaire ainsi que dans un modèle de xénogreffe résultent en une diminution de la prolifération néoplasique. Nos résultats suggèrent que PACE4 est une cible intéressante dans le cancer du sein positif aux récepteurs hormonaux. / Abstract: Breast cancer is the most frequent and deadly malignancy in women worldwide. Despite national screening prog rams combined with new treatments relapse rate remain high and new therapies are needed. Even in industrialized country like Canada, it is still the second most deadly cancer in women. From previous work, we identified PACE4, a member of the proprotein convertases (PCs) family of endoproteases, as a novel therapeutic target in prostate cancer. In the present study, we examined whether PACE4 could also be a potential target in breast cancer. In clinical samples of breast adenocarcinoma, we observed a specific overexpression of PACE4 in the estrogen-receptor-positive (ER+) subtype. We therefore looked for a breast cancer cell line model which would be representative and thus focused on ZR-75-1 since it both expresses PACE4 and is estrogen-receptor-positive. We compared stable knockdowns of furin, PACE4 and PC7 in the estrogen-receptor-positive cell line ZR-75-1 to evaluate their respective contribution to cell growth and tumor progression. PACE4 was found to be the only PC displaying a decrease in cell growth. The impact of PACE4 on tumor growth was also confirmed in vivo with xenograft of the ZR-75-1 cell line in athymic nude mice. PACE4 specific peptide-based inhibitors (C23 and Multi-Leu) were tested and shown to decrease prolife ration of ZR-75-1 cells in cell-based assays. We also confirmed that PACE4 expression was necessary for its specific peptide-based inhibitors to penetrates in the cell using a radiolabeled Multi-Leu peptide. Moreover, the systemic administration of C23 had a potent effect on tumor growth on xenografts of the ZR-75-1 cell line. Interestingly, PACE4-silencing and systemic administration of the PACE4 inhibitor C23 resulted in similar slowed tumor growth in vivo. Furthermore, these tumors demonstrated lower Ki67 proliferative indices with increased cell quiescence assessed with p27 KIP1 and p21 Waf1/Cip1 biomarkers. To conclude, PACE4-silencing and systemic administration of a PACE4 inhibitor result in hindered tumor progression and slower cellular proliferation. Our results suggest that PACE4 is a promising target for estrogen-receptor-positive breast cancer.

A GPU parallel approach improving the density of patch based multi-view stereo reconstructionHaines, Benjamin A. January 2016 (has links)
Multi-view stereo is the process of recreating three-dimensional data from a set of two or more images of a scene. The ability to acquire 3D data from 2D images is a core concept in computer vision with wide-ranging applications throughout areas such as 3D printing, robotics, recognition, navigation and a vast number of other fields. While 3D reconstruction has been increasingly well studied over the past decades, it is only with the recent evolution of CPU and GPU technologies that practical implementations, able to accurately, robustly and efficiently capture 3D data of photographed objects have begun to emerge. Whilst current research has been shown to perform well under specific circumstances and for a subset of objects, there are still many practical and implementary issues that remain an open problem for these techniques. Most notably, the ability to robustly reconstruct objects from sparse image sets or objects with low texture. Alongside a review of algorithms within the multi-view field, the work proposed in this thesis outlines a massively parallel patch based multi-view stereo pipeline for static scene recovery. By utilising advances in GPU technology, a particle swarm algorithm implemented on the GPU forms the basis for improving the density of patch-based methods. The novelty of such an approach removes the reliance on feature matching and gradient descent to better account for the optimisation of patches within textureless regions, for which current methods struggle. An enhancement to the photo-consistency matching metric, which is used to evaluate the optimisation of each patch, is then defined. Specifically targeting the shortcomings of the photo-consistency metric when used inside a particle swarm optimisation, increasing its effectiveness over textureless areas. Finally, a multi-resolution reconstruction system based on a wavelet framework is presented to further improve upon the robustness of reconstruction over low textured regions.

Effektivitet av sekukinumab vid behandling av medelsvår och svår plack-psoriasis : En litteraturstudie / Efficacy of secukinumab in the treatment of moderate and severe plaque psoriasisSoligor, Olena January 2022 (has links)
Introduktion: Psoriasis är en kronisk inflammatorisk systemsjukdom som drabbar främst hud och leder. På grund av mycket snabbt tillväxt av hudceller ger psoriasis utslag på huden. En av de vanligaste typerna av psoriasis är plack-psoriasis som kännetecknas av större än 0,5 cm i diameter runda fjällande hudutslag som kliar. Mekanismen som ligger bakom plack-psoriasis medieras med hjälp av T-celler och dendritiska celler som är en viktig del av immunsystemet. Stimulering av dessa immunceller leder till frisättning av följande proinflammatoriska cytokiner såsom IL-17, IFN-γ, TNF samt IL-22. Dessa cytokiner utövar sin effekt på keratinocyter genom att öka psoriatisk inflammation. Flera biologiska läkemedel utvecklas nu för behandling av plack-psoriasis. I detta arbete har fokus lagts på två av dem, sekukinumab som är en IL-17A hämmare och ustekinumab, som är en IL-12 och IL-23 hämmare. Syfte: Syftet med arbetet är att undersöka effektivitet av sekukinumab vid behandling av plack-psoriasis. Vidare undersöks även hur behandling av plack-psoriasis med sekukinumab skiljer sig från behandling med ustekinumab gällande minskning av plack-psoriasis. Metod: Detta arbete är en litteraturstudie som inkluderar fem vetenskapliga artiklar som baserades på randomiserade kontrollerade studier som sponsrades av läkemedelsföretag. Resultat: Resultatet av de fem studier visar att behandling med hjälp av sekukinumab 300 mg och 150 mg var mer effektiv än behandling med ustekinumab 45/90 mg, enligt PASI 75, PASI 90 och PASI 100. Diskussion: Alla fem studierna var dubbel-blinda randomiserade kontrollerade studier vilket leder till säkra och tillförlitliga resultat. Däremot var samtliga studier sponsrade av läkemedelsföretag vilket kan leda till att positiv effekt beskrivs i större utsträckning för att kunna sälja sin produkt. Slutsats: Sekukinumab 300 mg och 150 mg är signifikant mer effektiv än placebo och signifikant mer effektiv än ustekinumab 45/90 mg vid behandling av medelsvår och svår plack-psoriasis. / Introduction: Psoriasis is a chronic inflammatory systemic disease that mainly affects the skin and joints. Due to the very rapid growth of skin cells, psoriasis causes rashes on the skin. One of the most common types of psoriasis is plaque psoriasis which is characterized by larger than 0.5 cm in diameter round scaly skin rash that itches. The mechanism behind plaque psoriasis is mediated by T cells and dendritic cells, which are an important part of the immune system. Stimulation of these immune cells leads to the release of the following proinflammatory cytokines such as IL-17, IFN-γ, TNF and IL-22. These cytokines exert their effect on keratinocytes by increasing psoriatic inflammation. Several biological drugs are now being developed for the treatment of plaque psoriasis. In this study, the focus has been on two of them, secukinumab which is an IL-17A inhibitor and ustekinumab, which is an IL-12 and IL-23 inhibitor. Purpose: The purpose of this study is to investigate the efficacy of secukinumab in the treatment of plaque psoriasis. Furthermore, it is also investigated if the treatment of plaque psoriasis with secukinumab differs from treatment with ustekinumab regarding reduction of plaque psoriasis. Method: This work is a literature study that includes five scientific articles based on randomized controlled trials sponsored by pharmaceutical companies. Results: The results of the five studies show that treatment with secukinumab 300 mg and 150 mg was more effective than treatment with ustekinumab 45/90 mg, according to PASI 75, PASI 90 and PASI 100. Discussion: All five studies were double-blind randomized controlled trials leading to safe and reliable results. On the other hand, all studies were sponsored by pharmaceutical companies, which may lead to a positive effect being described to a greater extent in order to be able to sell their product. Conclusion: Secukinumab 300 mg and 150 mg are significantly more effective than placebo and significantly more effective than ustekinumab 45/90 mg in the treatment of moderate to severe plaque psoriasis.

Statinbehandling av äldre för primär- och sekundärprevention i Region Stockholm / Statin Treatment for Primary and Secondary Prevention in Elderly Patients in Region StockholmKalantaripour, Camelia January 2020 (has links)
Background: Aging is a major risk factor for atherosclerotic cardiovascular diseases (CVD) and death. There is limited evidence on benefits of statin therapy for primary prevention in elderly patients, while there is strong evidence for its use in secondary prevention. The knowledge of prescribing patterns of statins to elderly is limited. Aim: To provide an overview of the use of statins in primary and secondary prevention for patients ≥ 75 years in Region Stockholm in 2019. Methods: A cross-sectional study based on the regional health database VAL containing all diagnoses and dispensed prescription drugs for all 174 950 inhabitants ≥ 75 years old in Region Stockholm. Prevalence and incidence were analysed by sex, age, cardiovascular risk, prescriber category, substance and the intensity of treatment. Results: Of the elderly population, 25-43% were treated with statins, and 2-4% were initiated on statins during 2019. Men, individuals <85 years and those with higher cardiovascular risk were treated to a higher extent. Simvastatin was prescribed primarily to prevalent users and atorvastatin to incident users. A majority were treated with moderate-intensity dosages and fewer women received high intensity treatment. General practitioners accounted for a majority of all prescriptions and they prescribed mainly low and moderate intensity dosages. Conclusions: Statins are widely prescribed in elderly. It is likely that there is undertreatment among high-risk patients (especially women and elderly 85+ years) and overtreatment among patients with low-risk for CVD. Physicians seem to consider patients’ cardiovascular risk when deciding to initiate statin treatment for elderly.

Yes / Introduction The Community Ageing Research 75+ Study (CARE75+) is a longitudinal cohort study collecting an extensive range of health, social and economic data, with a focus on frailty, independence and quality of life in older age. CARE75+ is the first international experimental frailty research cohort designed using Trial within Cohorts (TwiCs) methodology, to align applied epidemiological research with clinical trial evaluation of interventions to improve the health and well-being of older people living with frailty.
Methods and analysis Prospective cohort study using a TwiCs design. One thousand community-dwelling older people (≥75 years) will be recruited from UK general practices. Nursing home residents, those with an estimated life expectancy of 3 months or less and people receiving palliative care will be excluded. Data collection assessments will be face to face in the person’s home at baseline, 6 months, 12 months, 24 months and 48 months, including assessments of frailty, cognition, mood, health-related quality of life, comorbidity, medications, resilience, loneliness, pain and self-efficacy. A modified protocol for follow-up by telephone or web based will be offered at 6 months. Consent will be sought for data linkage and invitations to additional studies, including intervention studies using the TwiCs design. A blood sample biobank will be established for future basic science studies.
Ethics and dissemination CARE75+ was approved by the NRES Committee Yorkshire and the Humber—Bradford Leeds 10 October 2014 (14/YH/1120). Formal written consent is sought if an individual is willing to participate and has capacity to provide informed consent. Consultee assent is sought if an individual lacks capacity.
Study results will be disseminated in peer-reviewed scientific journals and scientific conferences. Key study results will be summarised and disseminated to all study participants via newsletters, local older people’s publications and local engagement events. Abstract
With the growing proportion of older people and increasing use of drugs in this population, it is important to study how drugs affect oral health among older people. The aim of this thesis was to study whether sedative load, which represents cumulative exposure to drugs with sedative properties, is associated with oral health among community-dwelling older people. The focus was on a dry mouth, oral health behavior, dental caries, and infection in the periodontium. In addition, sedative load and anticholinergic burden were compared.
The present study population was a subpopulation from an intervention study “Geriatric Multidisciplinary Strategy for Good Care of the Elderly”. The study population consisted of 159 community-dwelling, dentate, and non-smoking people aged 75 or older from the city of Kuopio, Finland. Data were collected with interviews, geriatric assessments, and clinical oral examinations. Sedative load was determined by using a previously published method.
The study showed that participants with a sedative load were more likely to have dental caries, but not periodontitis, when compared with participants without a sedative load. Sedative load was associated with decreased stimulated salivary secretion and less strongly with unstimulated salivary secretion but not with xerostomia. The results also showed that sedative load was associated with poor or insufficient oral health behavior. Anticholinergic burden was associated with low unstimulated salivary secretion and xerostomia, but not with low stimulated salivary secretion.
In conclusion, cumulative exposure to drugs with sedative properties was associated with insufficient oral self-care and poor oral health. The results from this study emphasize the fact that older people using drugs with sedative properties require thorough prophylaxis measures and regular dental check-ups because of their high risk of having poor oral health. / Tiivistelmä
Väestön iäkkäiden henkilöiden osuuden kasvaessa ja heidän lääkkeiden käytön lisääntyessä on tärkeää tutkia, miten lääkkeet vaikuttavat ikääntyneiden suun terveyteen. Tutkimuksen tarkoituksena oli selvittää lääkityksestä aiheutuvan sedatiivikuorman (sedative load) vaikutuksia suun terveyteen kotona asuvilla iäkkäillä henkilöillä. Eritoten tutkimuskohteena oli sedatiivikuorman yhteys kuivaan suuhun, suun terveyskäyttäytymiseen, kariekseen sekä hampaiden tukikudosten sairauteen. Lisäksi tutkittiin antikolinergisen kuorman yhteyttä kuivaan suuhun ja tuloksia verrattiin sedatiivikuormaan.
Tutkimuspopulaatio oli osa geriatrista Hyvän Hoidon Strategia -interventiotutkimusta (HHS). Populaatio koostui 159 kotona asuvasta 75-vuotiaasta tai sitä vanhemmasta hampaallisesta, ei-tupakoivasta kuopiolaisesta. Tutkimusmateriaali kerättiin haastattelulla, geriatrisella tutkimuksella sekä kliinisellä suun tutkimuksella. Sedatiivikuorma laskettiin käyttäen aiemmin kehitettyä mallia, jossa määritetään kokonaislääkityksestä aiheutuva sedatiivikuorma.
Tutkimus osoitti, että osallistujilla, joilla oli sedatiivikuormaa, oli keskimäärin enemmän kariesta muttei hampaiden tukikudoksen sairautta verrattuna henkilöihin, joilla ei ollut sedatiivikuormaa. Tulokset osoittivat myös, että sedatiivikuorma oli yhteydessä alentuneeseen stimuloidun syljeneritykseen ja vähemmässä määrin alentuneeseen leposyljeneritykseen, mutta ei kuivan suun tunteeseen. Antikolinerginen kuorma oli yhteydessä alentuneeseen leposyljeneritykseen ja kuivan suun tunteeseen, mutta ei alentuneeseen stimuloidun syljeneritykseen. Sedatiivikuorma oli yhteydessä puutteelliseen suun terveyskäyttäytymiseen, kuten vähäiseen hammastahnan käyttöön ja suureen plakkimäärään.
Johtopäätöksenä voidaan todeta, että altistuminen väsyttäville lääkkeille on yhteydessä puutteelliseen omahoitoon ja huonoon suun terveyteen. Tämän tutkimuksen tulokset korostavat väsyttäviä lääkkeitä käyttävien ikääntyneiden tarvitsevan perusteellisia ennaltaehkäiseviä toimia sekä säännöllisiä suun tutkimuksia, koska heillä on suurentunut riski huonoon suun terveyteen.

Following the study of the current status of electrical discharge machining in the engineering company, measures for assurance and increase of accuracy of wire-cut EDM machines are proposed.

An exploration of improvements to semi-supervised fuzzy c-means clustering for real-world biomedical dataLai, Daphne Teck Ching January 2014 (has links)
This thesis explores various detailed improvements to semi-supervised learning (using labelled data to guide clustering or classification of unlabelled data) with fuzzy c-means clustering (a ‘soft’ clustering technique which allows data patterns to be assigned to multiple clusters using membership values), with the primary aim of creating a semi-supervised fuzzy clustering algorithm that shows good performance on real-world data. Hence, there are two main objectives in this work. The first objective is to explore novel technical improvements to semi-supervised Fuzzy c-means (ssFCM) that can address the problem of initialisation sensitivity and can improve results. The second objective is to apply the developed algorithm on real biomedical data, such as the Nottingham Tenovus Breast Cancer (NTBC) dataset, to create an automatic methodology for identifying stable subgroups which have been previously elicited semi-manually. Investigations were conducted into detailed improvements to the ss-FCM algorithm framework, including a range of distance metrics, initialisation and feature selection techniques and scaling parameter values. These methodologies were tested on different data sources to demonstrate their generalisation properties. Evaluation results between methodologies were compared to determine suitable techniques on various University of California, Irvine (UCI) benchmark datasets. Results were promising, suggesting that initialisation techniques, feature selection and scaling parameter adjustment can increase ssFCM performance. Based on these investigations, a novel ssFCM framework was developed, applied to the NTBC dataset, and various statistical and biological evaluations were conducted. This demonstrated highly significant improvement in agreement with previous classifications, with solutions that are biologically useful and clinically relevant in comparison with Sorias study [141]. On comparison with the latest NTBC study by Green et al. [63], similar clinical results have been observed, confirming stability of the subgroups. Two main contributions to knowledge have been made in this work. Firstly, the ssFCM framework has been improved through various technical refinements, which may be used together or separately. Secondly, the NTBC dataset has been successfully automatically clustered (in a single algorithm) into clinical sub-groups which had previously been elucidated semi-manually. While results are very promising, it is important to note that fully, detailed validation of the framework has only been carried out on the NTBC dataset, and so there is limit on the general conclusions that may be drawn. Future studies include applying the framework on other biomedical datasets and applying distance metric learning into ssFCM. In conclusion, an enhanced ssFCM framework has been proposed, and has been demonstrated to have highly significant improved accuracy on the NTBC dataset.

Ο γενικός σκοπός της παρούσας μελέτης ήταν να προτείνει μια νέα μέθοδο για την πλήρη περιγραφή των χαρακτηριστικών ποιότητας εικόνας και την βελτιστοποίηση της Τομογραφίας Εκπομπής Ποζιτρονίων (PET) αναπτύσσοντας ένα μοντέλο Monte Carlo.
Υλικά και Μέθοδοι: Η μέθοδος αναπτύχθηκε χρησιμοποιώντας τον κώδικα Monte Carlo (GEANT4 Application For Tomographic Emissions) του GEANT4 με το πακέτο λογισμικού GATE που αναπτύχθηκε από την Open-GATE collaboration. Για την ανακατασκευή των εικόνων χρησιμοποιήθηκε το λογισμικό STIR και για τη λήψη των δεδομένων της ανακατασκευής, από το GATE, χρησιμοποιήθηκε μια συστοιχία από 12 επεξεργαστές Dual Core Intel (R) Xeon (TM) 3.00GHz (Supermicro SuperServer 6015B-UR/NTR, Αγγλία). Ο σαρωτής PET που προσομοιώθηκε σε αυτή τη μελέτη ήταν ο General Electric Discovery-ST (ΗΠΑ). Το μοντέλο GATE πιστοποιήθηκε μέσω της σύγκρισης των αποτελεσμάτων που ελήφθησαν με δημοσιευμένα αποτελέσματα (Bettinardi et al 2004, Mawlawi et al 2004) τα οποία ακολουθούν το πρωτόκολλο της NEMA-NU-2 2001. Οι εικόνες ανακατασκευάστηκαν με τρεις μεθόδους, αρχικά με τη 2D φιλτραρισμένη οπισθοπροβολή (FBP2D), στη συνέχεια με τη μέθοδο της 3D φιλτραρισμένης οπισθοπροβολής (FPB3DRP), των Kinahan και Rogers, και τέλος με χρήση επαναληπτικών αλγορίθμων (MLE-OSMAPOSL). Αρχικά έγινε προσδιορισμός της Συνάρτησης Μεταφοράς Διαμόρφωσης (MTF) για την αξιολόγηση της ποιότητας εικόνας συστημάτων PET. Η αξιολόγηση έγινε μέσω της προσομοίωσης μιας νέας επίπεδης πηγής Λεπτού Χρωματογραφικού Φιλμ (Thin Layer Chromatography-TLC) το οποίο αποτελείται από ένα στρώμα Διοξειδίου του Πυριτίου και υποστρώματα φύλλου Αλουμινίου, εμβαπτισμένο σε 18F-FDG με ενεργότητα 44.4MBq.
Η αξιολόγηση της MTF έγινε μέσω των ανακατασκευασμένων εικόνων της επίπεδης πηγής κάνοντας χρήση του λογισμικού STIR. Η αξιολόγηση της MTF πραγματοποιήθηκε επίσης:
α) σε τρεις διαστάσεις (3D), τοποθετώντας την επίπεδη πηγή σε οριζόντια και σε κάθετη κατεύθυνση (διαστάσεις πηγής οριζόντια και κάθετα 5x10 εκατοστά),
β) με σάρωση πηγής μεγαλύτερου πλάτους, ανακατασκευάζοντας την εγκάρσια τομή της (διαστάσεις 18x10cm) και
γ) από ανακατασκευασμένες εικόνες σημειακής πηγής.
Να τονίσουμε εδώ ότι η παρούσα μελέτη είχε ως στόχο να συγκρίνει την προτεινόμενη μέθοδο (αξιολόγηση της MTF μέσω επίπεδης πηγής) με την πιο παραδοσιακή τεχνική που βασίζεται σε σημειακές πηγές. Στη συνέχεια έγινε προσδιορισμός της Ανιχνευτικής Κβαντικής Αποδοτικότητας (DQE) για την εκτίμηση της συνολικής απόδοσης του συστήματος PET, μέσω υπολογισμού της Συνάρτησης Μεταφοράς Διαμόρφωσης και του Κανονικοποιημένου Φάσματος Ισχύος Θορύβου (NNPS). Οι καμπύλες της MTF υπολογίστηκαν από την ανακατασκευή των εγκάρσιων τομών της επίπεδης πηγής (1 MBq), ενώ το NNPS υπολογίστηκε από τις αντίστοιχες στεφανιαίες τομές. Οι εικόνες ανακατασκευάστηκαν εφαρμόζοντας επαναληπτικούς αλγόριθμους (MLE-OSMAPOSL), χρησιμοποιώντας διάφορα υποσύνολα των προβολών (subsets) (3 έως 21) και επαναλήψεων (iterations) (1 έως 20). Επιπλέον, η αξιολόγηση της DQE έγινε μέσω διερεύνησης της επίδρασης διαφόρων κρυστάλλων σπινθηριστών στην διακριτική ικανότητα (MTF) και τον θόρυβο (NNPS) των ανακατασκευασμένων εικόνων του PET. Σε αυτή την περίπτωση, ο αλγόριθμος ανακατασκευής της εικόνας MLE-OSMAPOSL, υλοποιήθηκε χρησιμοποιώντας 15 subsets και 3 iterations.
Αποτελέσματα και Συζήτηση: Τα αποτελέσματα της πιστοποίησης μέσω σύγκρισης με δημοσιευμένα αποτελέσματα είναι τα ακόλουθα: Η Διακριτική Ικανότητα (Spatial Resolution, SR) στο Πλήρες Εύρος στο Ήμιση του Μέγιστου (Full Width at Half Maximum - FWHM) βρέθηκε να έχει απόκλιση μικρότερη από 3,29% σε λειτουργία 2D και μικρότερη από 2,51% σε λειτουργία 3D, σε σχέση με δημοσιευμένα πειραματικά αποτελέσματα (Mawlawi et al 2004), αντιστοίχως. Οι τιμές 2D, για την Ευαισθησία (Sensitivity), το Ποσοστό Σκέδασης (Scatter Fraction-SF) και την Απόδοση του Ρυθμού Μέτρησης (Count-Rate), τα οποία ελήφθησαν ακολουθώντας το πρωτόκολλο της NEMA NU 2-2001, βρέθηκαν να διαφέρουν λιγότερο από 0,46%, 4,59% και 7,86%, αντίστοιχα με τα δημοσιευμένα πειραματικά αποτελέσματα (Mawlawi et al 2004).
Ακολούθως, οι αντίστοιχες τιμές σε λειτουργία 3D βρέθηκαν να διαφέρουν λιγότερο από 1,62%, 2,85% και 9,13%, αντίστοιχα, με τα δημοσιευμένα πειραματικά αποτελέσματα (Mawlawi et al 2004). Η ευαισθησία επιπλέον εκτιμήθηκε χωρίς την παρουσία υλικού εξασθένησης, προσομοιώνοντας απευθείας μια ιδανική πηγή. Οι διαφορές που προέκυψαν μεταξύ της ιδανικής πηγής και της μεθοδολογίας κατά NEMA-NU-2 2001 κυμαίνονται από 0,04% έως 0,82% (ακτινική θέση R = 0cm) σε λειτουργία 2D και από 0,52% έως 0,67% σε λειτουργία 3D (ακτινικές θέσεις R = 10cm). Συνεπώς κάνοντας χρήση αυτής της μεθόδου, η ευαισθησία μπορεί να προσδιοριστεί με πιο απλοποιημένη και γρήγορη διαδικασία. Οι τιμές του Ρυθμού Μέτρησης Ισοδύναμου Θορύβου (Noise Equivalent Count Rate-NECR) που προέκυψαν ήταν 94.31kcps σε 2D και 66.9kcps σε 3D στα 70 και 15kBq/mL αντίστοιχα, σε σύγκριση με τα δημοσιευμένα αποτελέσματα τα οποία ήταν 94.08kcps σε 2D και 70.88kcps σε 3D στα 54.6kBq/mL και 14kBq/mL αντίστοιχα. Οι τιμές για την ποιότητα εικόνας βρέθηκαν σε εξαιρετική συμφωνία με τα δημοσιευμένα αποτελέσματα. Αφού ολοκληρώθηκε η πιστοποίηση του μοντέλου έγινε υπολογισμός της MTF. Οι τιμές MTF που προέκυψαν από την ανακατασκευή με τον αλγόριθμο FBP2D βρέθηκαν σε εξαιρετική συμφωνία με αυτές που προέκυψαν από την ανακατασκευή με τον αλγόριθμο FBP3DRP, ενώ οι αντίστοιχες τιμές μέσω του αλγόριθμου MLE-OSMAPOSL ήταν σε όλο το φάσμα των χωρικών συχνοτήτων υψηλότερες σε σχέση με τους αλγόριθμους οπισθοπροβολής (FBP). Η προσομοίωση της μεγάλης επίπεδης πηγής με αλγορίθμους οπισθοπροβολής έδειξε ότι οι τιμές της MTF ελαττώνονται σταδιακά από το κέντρο προς τα άκρα του οπτικού πεδίου (Field Of View-FOV). H MTF, της κάθετης τομής, διέφερε ελάχιστα σε σχέση με την οριζόντια. Η σύγκριση της επίπεδης πηγής με τη σημειακή πηγή έδειξε ότι η πρώτη είναι λιγότερη ευαίσθητη στο θόρυβο (SD = 0,0031 και 0,0203, αντίστοιχα). Η αξιολόγηση της DQE μέσω επαναληπτικών αλγορίθμων MLE-OSMAPOSL έδειξε ότι η απόδοση του συστήματος βελτιώνεται όσο αυξάνεται ο αριθμός των επαναλήψεων μέχρι μια μέγιστη τιμή (12 iterations) και παρέμενε αναλλοίωτη από εκεί και έπειτα.
Επιπλέον, η μεταβολή του αριθμού των subsets δεν είχε επίδραση στην MTF, για ίσο αριθμό επαναλήψεων. Αντίστοιχα τα επίπεδα θορύβου (NNPS) μειώθηκαν με την αύξηση του αριθμού των iterations και των subsets. Με βάση τα προηγούμενα οι τιμές της DQE επηρεάστηκαν τόσο από την MTF όσο και από το NNPS και βρέθηκαν να αυξάνουν με την αντίστοιχη αύξηση του αριθμού των iterations και των subsets. Τέλος, ο ανιχνευτής PET στον οποίο τοποθετήθηκαν κρύσταλλοι LuAP, παρείχε τις βέλτιστες τιμές MTF σε οπισθοπροβολή 2D και 3D ενώ η αντίστοιχη διαμόρφωση με κρυστάλλους BGO παρείχε τις βέλτιστες τιμές MTF μετά την ανακατασκευή με MLE-OSMAPOSL.
Αντίστοιχα, ο ανιχνευτής με κρυστάλλους BGO είχε τα χαμηλότερα επίπεδα θορύβου και τις υψηλότερες τιμές DQE μετά από την εφαρμογή όλων των αλγόριθμων ανακατασκευής.
Συμπερασματικά: Η παρούσα μελέτη έδειξε ότι η συνολική απόδοση συστημάτων PET μπορεί να χαρακτηριστεί πλήρως, να βελτιωθεί περαιτέρω και να γίνει πιο απλή με τη διερεύνηση των διαφόρων στοιχείων της αλυσίδας απεικόνισης μέσω μεθόδων Monte Carlo.
Η μέθοδος αξιολόγησης ανιχνευτών ΡΕΤ, βασιζόμενη σε επίπεδη πηγή TLC, απαιτεί υλικά που είναι συνηθισμένα στο κλινικό περιβάλλον, μπορεί να εφαρμοστεί πειραματικά και να χρησιμοποιηθεί στην κλινική πράξη. Σε αυτή τη μελέτη χρησιμοποιήθηκε για την εκτίμηση και βελτιστοποίηση της ποιότητας εικόνας, αλλά μπορεί να είναι επίσης χρήσιμη στον τομέα της έρευνας για περαιτέρω ανάπτυξη συστημάτων ΡΕΤ και SPECT, μέσω προσομοιώσεων με το πακέτο λογισμικού GATE.
Οι ανακατασκευασμένες εικόνες από το λογισμικό STIR μπορούν επίσης να χρησιμοποιηθούν για την εκτίμηση της κατανομής του ραδιοφαρμάκου, καθώς και την απευθείας λήψη της δοσιμετρικής κατανομής, με αντίστοιχο όφελος στους πυρηνικούς γιατρούς. / The overall purpose of this study was to propose a novel method for the complete image quality characterization and optimization of Positron Emission Tomography (PET) scanners with Monte-Carlo (MC) methods. Α model was developed using the Monte Carlo package of Geant4 Application for Tomographic Emission (GATE) and the software for tomographic image reconstruction (STIR) with cluster computing to obtain reconstructed images. The PET scanner used in this study was the General Electric Discovery-ST (US). The GATE model was validated by comparing results obtained in accordance with the National Electrical Manufacturers Association NEMA-NU-2-2001 protocol (Bettinardi et al 2004, Mawlawi et al 2004). Validation images were reconstructed with the commonly used 2D Filtered Back Projection (FBP2D) and the Kinahan and Rogers FPB3DRP Reprojection Algorithms. Image quality, in terms of the Modulation Transfer Function (MTF), was initially assessed with a novel plane source consisting of a Thin Layer Chromatography (TLC) plate, simulated by a layer of Silica gel on Aluminium foil substrates immersed in Fluorodeoxyglucose (18F-FDG) bath solution (44.4MBq). MTF was assessed from the evaluated STIR digital reconstructed images of the plane source. MTF was also assessed a) in three dimensions, in lines passing through the central axis of the PET scanner, by placing the plane source only horizontally and vertically (5x10cm), b) by scanning 18cm of the transaxial Field Of View (FOV) through the simulation of a horizontal large plane source (18x10cm) and c) by evaluating reconstructed point source images. Furthermore, the study aimed to compare the proposed method with the more traditional technique based on a line source. The complete image quality characterization was assessed in terms of the Detective Quantum Efficiency (DQE) by estimating the MTF and the Normalized Noise Power Spectrum (NNPS) of the 18F-FDG TLC plane source (1 MBq). MTFs curves were estimated from transverse reconstructed images of the plane source, whereas the NNPS data were estimated from the corresponding coronal images. Images were reconstructed by the Maximum Likelihood Estimation (MLE)-OSMAPOSL reprojection algorithm by using various subsets (3 to 21) and iterations (1 to 20). Additionally, the influence of different scintillating crystals on PET scanner’s image quality, in terms of the MTF, the NNPS and the DQE, was also investigated. In this case, OSMAPOSL image reconstruction was assessed by using 15 subsets and 3 iterations. The simulated spatial resolution in terms of Full Width at Half Maximum (FWHM) agreed with published data of Mawlawi et al (2004), within less than 3.29% in 2D and less than 2.51% in 3D with published data of others, respectively. The 2D values for the sensitivity, the scatter fraction and the count-rate were found to agree within less than 0.46%, 4.59% and 7.86%, respectively with corresponding published values. Accordingly, the corresponding 3D values were found to agree to less than 1.62%, 2.85% and 9.13%, respectively with Mawlawi et al (2004) published values. Sensitivity, which was also estimated in the absence of attenuation material by simulating an ideal source, showed differences between the extrapolated and the ideal source values (with and without attenuation) ranging in 2D from 0.04% to 0.82% (radial location R=0cm) in 2D and from 0.52% to 0.67% in 3D mode (radial locations R=10cm). By using this model, sensitivity can be obtained in a more simplified procedure. The simulated noise equivalent count rate was found to be 94.31kcps in 2D and 66.9kcps in 3D at 70 and 15kBq/mL respectively, compared to 94.08kcps in 2D and 70.88kcps in 3D at 54.6kBq/mL and 14kBq/mL respectively, from the published by others values. The simulated image quality was found in excellent agreement with these published values. The MTFs obtained using the FBP2D were in close agreement to those obtained by the FBP3DRP, whereas the MTFs of the OSMAPOSL show, in all cases, that higher frequencies are preserved than in the case of the FBP. FBP reconstructed images obtained from the large horizontal plane source showed that the MTF was found to degrade gradually as we move towards the edge of the FOV. The MTFs of the FBP images along the vertical direction were slightly lower than the corresponding horizontal ones. In addition, the plane source method is less prone to noise than the conventional line source method (sd=0.0031 and 0.0203, respectively). In the case of DQE investigation, MTF values assessed from the evaluated STIR digital reconstructed images, of the TLC based plane source, were found to improve as the number of iterations increased up to 12 and remain almost constant thereafter. Furthermore, variation in the number of subsets didn’t show any effect on the MTF, for equal number of iterations. The noise levels, in terms of the NNPS, in the reconstructed PET image, were found to decrease with the corresponding increase of both the number of iterations and subsets. DQE values were influenced by both MTF and NNPS and were found to increase with the corresponding increase in both number of iterations and subsets. Finally, the PET scanner configuration, incorporating LuAP crystals, provided the optimum MTF values in both 2D and 3DFBP whereas the corresponding configuration with BGO crystals was found with the higher MTF values after OSMAPOSL. The scanner incorporating BGO crystals were also found with the lowest noise levels and the highest DQE values after all image reconstruction algorithms. In conclusion, our study showed that the imaging performance of PET scanners can be fully characterized, further improved and simplified by investigation of the imaging chain components through MC methods. The simulated PET evaluation method, based on a TLC plane source, requires materials commonly found in a clinical environment and can be experimentally implemented and used in clinical practice. In this study it was used for the image quality assessment and optimization, but it can be also useful in research for the further development of PET and SPECT (Single Photon Emission Tomography) scanners though GATE simulations. Reconstructed images by STIR can be also used to obtain radiopharmaceutical distribution of images and direct dose maps, quite useful to nuclear medicine practitioners.
