Previous issue date: 2011 / IEC - Instituto Evandro Chagas / CNPq - Conselho Nacional de Desenvolvimento Científico e Tecnológico / A tese aqui apresentada trata-se do primeiro estudo em nível mundial que pesquisa e caracteriza a resposta imune citocínica em infecções humanas pelo Orthobunyavirus Oropuche. Como metodologia para o alcance dos objetivos aqui apresentados foi utilizado um total de 320 amostras de soros humanos, onde 60
destas foram provenientes de Banco de Sangue (Controle negativo) e 260 foram obtidas mediante dois surtos do Vírus Oropouche nos Estados do Pará e Amapá (Brasil), sendo estas últimas divididas em oito subgrupos para obtenção dos dados
com exatidão. Nas amostras coletadas foram realizadas análises dos dados clínicos/sintomatologia através dos prontuários, dados sorológicos através da titulação de anticorpos por Inibição da Hemaglutinação (IgM/IgG) e detecção do nível de citocinas plasmáticas por citometria de fluxo a qual permitiu a descrição técnica da dosagem de citocinas possibilitando ainda a análise de frequência de baixos e altos produtores de citocina. Os dados obtidos permitiram observar as variáveis e o comportamento das assinaturas de citocinas expressas pelos pacientes mediante a
confirmação sorológica do vírus, bem como o comportamento destes analitos séricos quando da presença de sintomas específicos como febre, calafrios, cefaléia e
tontura, permitindo assim que se chegasse à conclusão que a) existe um padrão na síntese de citocinas pró-inflamatórias e reguladoras; b) observa-se um balanço no perfil da resposta imune entre citocinas pró-inflamatórias (Th1) e moduladoras
(Th17); c) a infecção pelo Vírus Oropouche altera a produção das citocinas nos indivíduos; d) os resultados mostram também que ao comparar os indivíduos Não respondedores com os Respondedores precoces, houve aumento da IL-1β e diminuição da IL-12; Não respondedores com Respondedores tardios, houve
diminuição da IL-8, e aumento da IFN-α, IL-23 e IL-17; Não respondedores comparados com Respondedores precoces ocorreram o aumento de IL-4 e IFN-; Já quando comparado Respondedores precoces e respondedores tardios houve diminuição de IFN-α e IL-6; Respondedores precoces de forma geral apresentaram diminuição da IL-10 e Respondedores tardios apresentaram aumento da IL-5; e) Os resultados mostram ainda a expressão de IL-5 em pacientes que manifestaram os sintomas específicos para a infecção pelo Oropouche (febre, calafrios, cefaléia e tontura), sugerindo este sinal estar associado diretamente à patogênese do vírus; f) há a necessidade da complementação desta pesquisa com mais estudos como àqueles relacionados com a expressão de quimiocinas. / This thesis is the first global study that researches and analyzes the immune
response of cytokine in human infections by Orthobunyavirus Oropuche virus. The
study used 320 samples of human serum. Sixty were from the Blood Bank (negative
control) and 260 were obtained from two outbreaks of the Oropouche virus in the
State of Pará and Amapá (Brazil). The latter was divided into 8 subgroups for better
data accuracy. The collected samples were analyzed for clinical data/symptoms with
serologic testing by titration of antibodies by the hemagglutination inhibition (IgM/IgG)
and the detection cytokines plasma levels by flow cytometry. This allowed for the
technical description of cytokine. The data obtained allowed for the observation of the
characteristics and the behavior of the cytokines signatures expressed by patients by
the presence or not of the virus. This also allowed for the observation of changes to
serum through the presence of specific symptoms such as fever, chills, headache
and dizziness. This led to the following conclusions a) there is a pattern in the
synthesis of pro-inflammatory and regulatory cytokines; b) there is a balance in the
profile of the immune response between pro-inflammatory cytokines (Th1) and
modulators (Th17); c) an infection by the Oropouche virus alters the production of
cytokines in individuals; d) the results also show that whem comparing individuals no
responders with early responders, there was an increase of IL-1β and decreased IL-
12; no responders with late responders, there was a decrease of IL-8, and increased
IFN-α, IL-23 and IL-17; No responders occurred early responders compared with the
increase IL-4 and IFN-g; However, when compared early responders and late
responders, decreased IFN-α and IL-6; early responders generally showed a
decrease in IL-10 and late responders showed an increase in IL-5; e) The results
also show the expression of IL-5 in patients who showed symptoms specific for
Oropouche infection (fever, chills, headache and dizziness), suggesting this signal to
be directly associated with pathogenesis of the virus; f) there is a need to
complement this research with more studies such as those related to the expression
of chemokines.

Previous issue date: 2010 / O uso de plantas medicinais para tratamento dos males da saúde está disseminado não só nas áreas rurais como também nas áreas urbanas do território brasileiro. O significado econômico destas plantas medicinais, usadas no atendimento das necessidades básicas de saúde, para as economias domésticas da parte socialmente vulnerável da população, ainda é pouco investigado. Muitas espécies vegetais são comercializadas na cidade de Belém por erveiros em mercados ou feiras livres da cidade. Este trabalho tem como objetivo analisar em que medida a inserção do uso de Plantas Medicinais minimiza os custos do tratamento de agravos atendidos pela Assistência Farmacêutica na Atenção Básica á saúde para o usuário do Sistema de Saúde. Para se atingir esse objetivo, foi realizado um estudo, de natureza exploratória, por meio de aplicação de questionários, utilizando o método de amostragem, definida por critério não probabilístico, no bairro do Jurunas com apoio na Unidade de Saúde Radional II, localizado na Região Metropolitana de Belém no Pará. Foi realizado um levantamento de informações referente ao foco deste estudo no Centro de Referência de Tratamento Natural (CRTN), localizado na cidade de Macapá, que já utiliza as plantas medicinais como recurso terapêutico. No resultado da pesquisa de campo no bairro do Jurunas foi verificado que aproximadamente 50% dos entrevistados praticam a automedicação com plantas medicinais, relacionadas na RDC nº 10/ANVISA, sendo necessária, entretanto, orientação correta para seu cultivo e emprego terapêutico. A comparação entre os gastos com medicamentos sintéticos no tratamento dos sintomas de cinco doenças que ocorrem com frequência no bairro do Jurunas e aqueles resultantes do uso de plantas medicinais, para os mesmos sintomas, revela uma redução no gasto para tratamentos feitos à base de plantas medicinais. Em conclusão, pode-se inferir que esta redução representa uma economia de recursos que o usuário e o sistema deixam de despender quando utilizam plantas medicinais. / The use of medicinal plants for the treatment of the ailments of health is disseminated not only in the rural areas but also in the urban areas of the Brazilian territory. The economic meaning of these medicinal plants, used in the attendance of the basic health needs, for the home economics of the socially vulnerable part of the population, is still poorly investigated. Many plant’s species are commercialized in the city of Belem by herbalists in markets and open fairs of the city. This article has as objective analyze in what extent the insertion of the use of medicinal plant minimizes the costs of the treatments of injuries treated by the Assistencia Farmaceutica na Atencao Basica (Pharmaceutical Assistance in the Basic Attention). To achieve this objective, it was realized a study, of exploratory nature, by means of application of questionnaires, using the method of sampling, defined by non-probabilistic criteria, in the district of Jurunas, with support in the Unidade de Saude Radional II (Health Unit Radional II), located in the metropolitan region of Belem, Para. It was made a survey of informations regarding the focus of this study in the Centro de Referencia de Tratamento Natural (Natural Treatment Reference Center - CRTN), located in the city of Macapa, that already utilizes the medicinal plants as a therapeutic resource. In the result of the field research in the district of Jurunas it was verified that approximately 50% of the interviewed practice the self-medication with medicinal plants, listed in the RDC nº 10/ANVISA, been needed, however, correct orientation for its growth and therapeutic use. The comparison between expenses with synthetic medicines in the treatment of the symptoms of five diseases that frequently occur in district of Jurunas and those resultants of the use of medicinal plants, for the same symptoms, reveals a reduction in the expenses for treatments based on medicinal plants. In conclusion, it can be inferred that this reduction represents a saving of resources that the user and the system leave to expend when utilizes the medicinal plants.

Previous issue date: 2011 / A doença de Chagas aguda (DCA) é endêmica na Amazônia Brasileira sendo a via oral a principal forma de transmissão com surtos familiares ou multifamiliares. Esta via independe da colonização de triatomíneos no domicílio e a ocorrência é regular com média de 100 casos/ano e 5% de óbitos. Apresenta distribuição espaço-temporal bem definida, colocando a enfermidade como emergência de importância em saúde pública nos estados do Pará, Amapá e Amazonas. A presença de mamíferos e triatomíneos silvestres, infectados naturalmente com o c e habitando distintos ecótopos terrestres e arbóreos, mantém um intenso ciclo enzoótico em toda a Amazônia. Perfis moleculares de linhagens de T. cruzi na região estão associados a hospedeiros mamíferos (incluindo o homem), triatomíneos, ecótopos e manifestações clínicas. Foram estudados quatro surtos de DCA ocorridos nos Municípios de Barcarena, Belém e Cachoeira do Arari no Estado do Pará e em Santana, no Estado do Amapá e abordados os aspectos epidemiológicos (parasitológico e sorológico manifestações clínicas, reservatórios e triatomíneos silvestres associados aos surtos). Foi investigado também em São Luís, Estado do Maranhão, o ciclo domiciliar e silvestre do T. cruzi, porém sem a ocorrência de casos de DCA. O estudo incluiu também a genotipagem molecular de T. cruzi pelo gene de mini-exon dos isolados (homem, mamíferos e triatomíneos silvestres) associados aos diferentes ciclos de transmissão. O diagnóstico parasitológico foi confirmado em 63 pacientes com a seguinte sensibilidade nos testes aplicados: 41,3% (26/63) pela gota espessa; 58,7% (37/63) no QBC; 79,4% (50/63) no xenodiagnóstico e 61,9% (39/63) na hemocultura. A sorologia de 2648 pessoas por hemaglutinação indireta (HAI) foi de 3,05% (81/2648) e imunofluorescência indireta IFI apresentaram respectivamente resultados de e 2,49% (66/2648) para IgG e 2,37 (63/2648) para IgM. Os resultados em São Luís foram todos negativos. Foram capturados 24 mamíferos, 13 Didelphis marsupialis, 1 Marmosa cinerea, 5 Philander opossum, 3 Metachirus nudicaudatus, 1 Oryzomys macconnelli, 1 Oecomys bicolor e 433 R. rattus. A taxa de infecção para T. cruzi foi de 7,14% (29/404). Um total de 3279 triatomíneos foi capturado sendo: Triatoma rubrofasciata (n=3008), com taxa de infecção (TI) de 30.46%, (39/128), Rhodnius robustus (n=137), com TI de 76% (79/104), R. pictipes (n=94), TI de 56,9% (49/86%), E. mucronatus (n=6) e P. geniculatus (n=12) com TI de 50% e as demais espécies sem infecção R. neglectus (n=5) e P. lignarius (n=6). As palmeiras foram os principais ecótopos dos triatomíneos silvestres. O urucurizeiro (S. martiana) apresentava infestação de 47,41% (101/213) dos triatomíneos; o “inajazeiro” (Maximiliana regia) 35,21% (75/213); o “babaçueiro” (Orbgnya. speciosa) 5,16% (11/213); o “dendezeiro” (Eleas melanoccoca) 1,87% (4/213) e a “bacabeira” (Oenocarpus bacaba) 10,32% (22/213). Para a genotipagem foram obtidos 46 isolados de tripanossomas de origem humana, 31 isolamentos de mamíferos silvestres e 74 amostras de triatomíneos. Todos os isolados foram caracterizados como da linhagem TcI de T. cruzi. Todos os casos humanos no Pará foram caracterizados como positivos por exame parasitológico. Nem todos os casos de Santana, Amapá, apresentaram casos parasitológicos positivos, pela demora do diagnóstico, mesmo assim estes foram definidos como DCA. Exames como xenodiagnóstico, hemocultura e o QBC® foram mais sensíveis do que a gota espessa. A sorologia por HAI e IFI (IgG e IgM) tiveram excelente sensibilidade para detectar os casos agudos em tempos distintos de infecção. O achado de mamíferos (D. marsupilais) e triatomíneos silvestres (R. pictipes e P. geniculatus) infectados com consideráveis taxas de infecção para T. cruzi no entorno das residências dos pacientes sustentam a importância destes hospedeiros associados à transmissão da DCA. Apesar de na Amazônia circularem vários genótipos de T. cruzi nos diferentes hospedeiros, neste trabalho foi identificada somente a linhagem TCI de T. cruzi, a mais predominante na Região. Em São Luís, Maranhão, embora sem registro de casos de DCA apresenta um ciclo domiciliar associados ao rato doméstico e o triatomíneo da espécie T. rubrofasciata, e um ciclo silvestre mantido por didelfídeos. Nos dois ciclos circulam a linhagem TCI de T. cruzi. Estudos com marcadores de maior resolução com isolados de T. cruzi regionais podem ajudar a esclarecer os ciclos de transmissão, as rotas de contaminação e os hospedeiros envolvidos em casos de DCA na Amazônia. / The acute Chagas disease (ACD) is endemic in the Brazilian Amazon, and its main
transmission route is oral, throughout family and multi-family outbreaks. This route is independent from
the colonization of triatomine bugs in dwellings and its occurrence is regular, with mean rates of 100 cases per year and a lethality rate of 5%. The disease has a well-defined spatio-temporal distribution, which makes it a relevant public health concern in the states of Pará, Amapá and Amazonas. The existence of wild mammals and triatomine bugs naturally infected with Trypanosoma cruzi inhabiting different terrestrial and arboreal ecotopes maintains an intense enzootic cycle all over the Amazon region. Molecular profiles of T. Cruzi lineages in the region are associated with mammal reservoirs (including humans), triatomine bugs, ecotopes and clinical manifestations. We analyzed four ACD outbreaks in the municipalities of Barcarena, Belém and Cachoeira do Arari on Pará State, and Santana on Amapá State, based on epidemiologic features (laboratory, parasitological and serological diagnoses, clinical manifestations and the reservoirs and wild triatomines related to the outbreaks). We also investigated the domestic and wild transmission cycles of T. cruzi in São Luis on Maranhão State, without the occurrence of ACD cases. This study comprised molecular genotyping of T. cruzi on the mini-exon gene of the isolates associated with both transmission cycles (humans, mammals and wild
triatomine vectors). Parasitological diagnosis was confirmed in 63 patients with the following sensitivity
rates: 41.3% (26/63) for the thick blood film method; 58.7% (37/63) for QBC; 79.4% (50/63) for
xenodiagnosis; and 61.9% (39/63) for blood culture. The serological diagnosis of 2648 individuals by
indirect hemagglutination assay (IHA) was of 3.05% (81/2648), whereas the results of the indirect
immunofluorescence test (IIF) were 2.49% (66/2648) for IgG and 2.37% (63/2648) for IgM. All tests
carried out in São Luís were negative. A total of 24 mammals, 13 Didelphis marsupialis, 1 Marmosa
cinerea, 5 Philander opossum, 3 Metachirus nudicaudatus, 1 Oryzomys macconnelli, 1 Oecomys bicolor and 433 R. rattus were captured. The infection rate for T. cruzi was of 7.14% (29/404). A total of 3279
triatomine bugs were captured: T. rubrofasciata (n=3008) and infection rate (IR) of 30.46%, (39/128)
and R. robustus (n=137) IR of 76% (79/104), R. pictipes (n=94), IR of 56.9% (49/86%) E. mucronatus
(n=6) and P. geniculatus (n=12) IR of 50% and the other non-infected species R. neglectus (n=5) and P.
lignarius (n=6). Palm trees were the main ecotopes for the wild triatomine bugs. S. martiana was infested with 47.41% (101/213) of the triatomines; Maximiliana regia, 35,21% (75/213); Orbgnya speciosa, 5.16% (11/213); Eleas melanoccoca), 1.87% (4/213); and Oenocarpus bacaba, 10.32% (22/213). Genotyping was carried out using 46 isolates of trypanosomes obtained from humans, 31 from wild mammals and 74 from samples of triatomine bugs. All isolates were characterized as belonging to the Tcl lineage. All human cases in Pará were characterized as positive by parasitological testing. Not all the cases in Santana were tested positive because of the delay on diagnosis, but they were defined as positive. Xenodiagnosis, blood culture and QBC® were more sensitive than the thick blood film. Serological examinations by IHA and IIF (IgG and IgM) presented an optimal sensitivity to detect acute
cases in different moments of infection. Mammals (D. marsupialis) and wild triatomine bugs (R. pictipes
and P. geniculatus) infected with high infection rates of T. cruzi in the patients’ peridomicile area account for the importance of these reservoirs in the transmission cycle of the ACD, and are associated with its transmission. Even though several genotypes of T. cruzi circulate in the Amazon Region, only the Tcl lineage was identified in the patients, mammals and triatomines investigated in this study. In São Luís, in spite of these having no record of human cases of ACD, it has a domestic cycle associated with the black rat and the triatomine bugs of the species T. rubrofasciata, as well as a sylvatic cycle associated with didelphids. The Tcl cycles circulate in both cycles. Studies with isolates of local T. cruzi using markers with a higher definition might help clarify the transmission cycles, transmission routes and the reservoirs involved in cases of ACD in the Amazon Region.

Previous issue date: 2009 / Amostras sanguineas de 188 touros bubalinos, criados em sistema extensivo em areas de varzea, sem controle sanitario e reprodutivo eficiente, com idade de dois a 15 anos,
das racas Murrah, Mediterraneo e seus mesticos, foram coletadas em tubos vacutainer
esterilizados de 5 mL e avaliadas quanto a presenca de anticorpos para HBV-1, atraves
do teste de soroneutrlizacao. Dos 188 touros examinados, somente de 51 foi possivel
obter amostras de semen, as quais foram submetidas a analise de RT-PCR. Informacoes
acerca da idade, raca e escore de condicao corporal (ECC) dos animais foram
observadas. A estatistica descritiva foi aplicada atraves da distribuicao proporcional e
determinacao dos quadris e medianas de variaveis categoricas. O estudo foi realizado
em animais de oito propriedades, localizadas nos estados do Para e Amapa, regiao Norte
do Brasil, encontrando-se distribuidos da seguinte forma: nos municipios de Itaubal do
Piriri (N=59), Cutias do Araguari (N=33), Tartarugalzinho (N=14) e Bailique, distrito
de Macapa (N=21), totalizando 127 animais no Amapa. Os 73 restantes localizavam-se
no estado do Para nos municipios de Soure (N=38) e Muana (N=23), Ilha do Marajo. A
analise estatistica constou da aplicacao do teste Binomial para comparar a prevalencia
do HBV-1 nas propriedades examinadas nos municipios, e comparar a prevalencia entre
as classes de raca, escore corporal e idade. Para avaliar a distribuicao da titulacao de
anticorpos para HBV-1, entre Amapa e Marajo, foi utilizado o teste estatistico de Mann-
Whitney. Todos os procedimentos foram executados pelo programa BioEstat 5, com
nivel de significancia α=0.05. Em todas as propriedades estudadas havia animais
positivos, e 82,4% das 188 amostras foram sorologicamente positivas. Entre as racas
mediterraneo e Murrah e seus mesticos, houve diferenca real de prevalencia somente
entre as racas Mediterraneo e Murrah (p=0,0004). Houve influencia do ECC na
prevalencia de touros sorologicamente positivos, sendo que os de ECC=3 apresentaram
maior prevalencia que os de ECC < 3 (p=0.009). A prevalencia conforme a idade
mostrou que existe uma diferenca significativa (p<0.0001) entre os animais com dois
anos e os touros com mais de dois anos de idade, havendo um incremento gradativo das
taxas de infeccao com o avancar da idade. A prevalencia no estado do Amapa foi
significativamente maior que no Para. As propriedades localizadas nos municipios de
Cutias do Araguari, Tartarugalzinho e Bailique (Macapa), apresentram 100% de
prevalencia. Foi observado que a titulacao minima ocorreu na maioria dos animais no
Marajo, entretanto em somente em 4.8% do rebanho procedente do Amapa teve baixas
titulacoes. Todas as 51 amostras de semen apresentaram-se negativas no teste da RTPCR.
Concluimos assim que a prevalencia da IBR e altissima em touros bubalinos
criados extensivamente no estado do Amapa e Para, Brasil. / Blood samples of 188 buffalo bulls, reared in extensive systems in pasture of lowland,
without effective control and reproductive and health, with two to 15 years old, Murrah,
Mediterranean breeds and crossbreeds, were collected in sterile Vacutainer tubes, 5 mL
and analyzed for the presence of antibodies to HBV-1, through the test virus
neutralization. Of the 188 bulls examined, only 51 were obtained from semen of
samples, which were submitted to analysis RT-PCR. Information about age, race and
body condition score (BCS) of the animals were observed. The descriptive statistics was
applied through the proportional distribution and determination of the hips and medians
for categorical variables. The study was conducted on eight properties, located in the
states of Para and Amapa, northern Brazil, were distributed as follows: 127 animals in
the municipalities Itaubal of Piriri (N = 59), Agouti Araguari (N = 33), Tartarugalzinho
(N = 14) and Bailique, district of Macapa (N = 21) located in the state of Amapa. The
remaining 73 located in the state of Para in the municipalities of Soure (N = 38) and
Muana (N = 23), Marajo of Island. Statistical analysis consisted of applying the
Binomial test to compare the prevalence of HBV-1 in the properties examined in the
cities, and compare the prevalence between the classes of race, age and body score. To
assess the distribution of titers of antibodies to HBV-1, between Amapa and Marajo, we
used the statistical test of Mann-Whitney. All procedures were performed by the
program BioEstat 5, with significance level α= 0.05. All studied properties had
seropositive animals, and 82.4% of 188 samples were serologically positive. Between
the Mediterranean and Murrah breeds and crossbreeds, there was only real difference in
prevalence between the Mediterranean and Murrah breeds (p = 0.0004). There was
influence of BCS in the serologically positive bulls prevalence. The BCS = 3 had the
highest prevalence of ECC <3 (p = 0.009). The prevalence according to age showed a
significant difference (p <0.0001) between animals with two years and the bulls with
more than two years of age, with a gradual increase in rates of infection with age. The
prevalence in the state of Amapa was significantly higher than in Para. The properties
located in the municipalities of the Cutias do Araguari, Tartarugalzinho and Bailique
(Macapa), had a 100% prevalence. It was observed that the titration was minimal in
most animals in Marajo, but only 4.8% of the Amapa’s herds was founded low titers.
All 51 samples of semen, were negative in the RT-PCR test. We conclude that the
prevalence of IBR is high in buffalo bulls bred extensively in the state of Para and
Amapa, Brazil.

Previous issue date: 2010 / O estado do Amapá apresenta-se com 70 % dos seus 14.281.458,5 ha de extensão transformados em áreas protegidas, entres as várias categorias de Unidades de Conservação e terras indígenas, por sua vez, muitas dessas áreas adentras as águas litorâneas do atlântico, ora pelos seus limites, ora pelos seus entornos (área circundante), tornando conflitante a atividade pesqueira na linha da costa amapaense. O Acordo de Pesca assinado em 2007 entre pescadores do município do Oiapoque e gerencia do Parque Nacional do Cabo Orange (ICMBIO) busca o controle das pescarias pela frota do Oiapoque e diminuir conflito na faixa de 20 km em águas marinhas, que pertencem ao seu limite e área circundante. Este estudo investigou as características da pesca na região do estuário do rio Oiapoque, faixa marinha do Parque e sua área circundante. A metodologia contemplou entrevistas formais e informais de vários atores do setor pesqueiro no Oiapoque, monitoramento de 488 desembarques e viagens aos locais de pescarias. Foram obtidas as CPUE’s em função do rendimento por dia de pesca por pescador (Kg/dia/pesca para cada tipo de embarcação). Foram mapeados os pesqueiros considerados importantes e tradicionalmente explorados pela frota do Oiapoque, dentro dos limites do Parque Nacional do Cabo Orange e em sua área circundante. Os pescadores do município realizam pescarias de subsistência, artesanal de menor e maior escala. Por sua vez, são as pescarias artesanais de menor escala que predominam na área do parque particularmente as embarcações do tipo “barco de pequeno porte” (BPP), que se destacaram em número, cerca de 60 % das cadastradas na colônia. O volume de pescado desembarcado no município do Oiapoque foi 766 toneladas no ano de 2008. São principalmente alvo das pescarias a corvina (Cynoscion virescens) e a pescada branca (Plagioscion spp), desta capturada de pescarias de menor escala, principalmente em embarcações de capacidade até 1 tonelada do tipo barco motorizado (BOM). A produção é comercializada diretamente dos pescadores para intermediários (balanceiros) que por sua vez, vendem para o mercado local, os dois grandes centros consumidores do Amapá (Macapá e Santana) e para outros estados (Pará, São Paulo etc). / The state of Amapá comes with 70% of their 14.281.458,5 there is of extension transformed in protected areas, enter the several categories of Units of Conservation and indigenous lands, for his/her time, many of those areas penetrate the coastal waters of the Atlantic, some times for their limits, other times for their surrounding area, turning conflicting the fishing activity in the line of the coast of Amapá. The Agreement of Fishing signed in 2007 among fishermen of the municipal district of Oiapoque and it manages of Cabo Orange National Park (ICMBIO) it looks for the control of the fisheries for the fleet of Oiapoque and to reduce conflict in the strip of 20 km in sea waters, that you/they belong to his/her limit and surrounding area. This study investigated the characteristics of the fishing in the area of the estuary of the river Oiapoque, sea strip of the Park and his/her surrounding area. The methodology contemplated several actors' of the fishing section formal and informal interviews in Oiapoque, 488 disembarkation attendance and trips to the places of fisheries. They were obtained CPUE's in function of the income a day of fishing for fisherman (Kg/day/fishing for each boat type). The fishing ones were mapped considered important and traditionally explored by the fleet of Oiapoque, inside of the limits of Cabo Orange National Park and in his/her surrounding area. The fishermen of the municipal district accomplish subsistence fisheries, craft of minor and larger scale. For his/her time, they are the craft fisheries of smaller scale than they prevail in the area of the park particularly the embarkations of the type "boat of small load" (BPP), that you/they stood out in number, about 60% of the registered in the colony. The fish volume disembarked in the municipal district of Oiapoque was 766 tons in the year of 2008. They are mainly objective of the fisheries the corvina (Cynoscion virescens) and fished her white (Plagioscion spp), of this captured of fisheries of smaller scale, mainly in capacity embarkations to 1 ton of the type motorized boat (BOM). T HTLV 1/2 humanas 1 e 2 é ampla, porém existem áreas de maior
endemicidade e também particularidades de acordo com o tipo de HTLV. O
HTLV-1 apresenta maior soroprevalência no sudoeste do Japão, no Caribe, na
América Central, nas diferentes regiões da América do Sul e nas porções
centrais e ocidentais da África e Melanésia. Enquanto o HTLV-2 parece
acometer grupos populacionais distintos, como as populações nativas de
indígenas das Américas do Norte, Central e Sul, pigmeus da África Central,
mongóis na Ásia e também usuários de drogas injetáveis. O trabalho realizado
teve como objetivo descrever a epidemiologia molecular do HTLV em três
populações distintas do estado do Amapá, que foram: pacientes HIV/AIDS
infectado, população afro-descendente e finalmente indivíduos atendidos no
Laboratório Central de Saúde Pública do Amapá (LACEN-AP), encaminhados
para diagnóstico de HTLV. As amostras foram avaliadas para a presença do
vírus por métodos sorológicos (ELISA e Western blot) e moleculares
(amplificação gênica e caracterização de segmentos das regiões pX e env pela
análise de polimorfismo de fragmentos de restrição por ação de endonuclease.
Os resultados obtidos nas diferentes populações foram na população de
indivíduos infectados pelo HIV/AIDS, todas as amostras foram negativas, na
população afro-descendente, apenas uma amostra apresentou positividade na
sorologia pelo método de ELISA, porém foi negativa no Western blot e quando
submetida ao método molecular, não houve amplificação. No entanto, entre os
indivíduos encaminhados para diagnóstico de HTLV, 06 (seis) amostras foram positivas, e dessas, 05 (cinco) foram confirmadas por Western blot e pelo método molecular. O resultado molecular demonstrou a presença de HTLV-1. / The geographical distribution of the infection by human T-cell lymphotropic
virus 1/2 – HTLV 1 / 2 is extensive, nevertheless, there are areas that are more
endemic and have more particularities depending on the HTLV type. The HTLV
1 shows a bigger occurrence in south west of Japan, Caribbean, Central
America, in different regions of South America and in parts of Central, as well
as in western Africa, while HTLV-2, seems to assault distinct groups of people,
such as Indians native people from North, Central and South America, Pigmies
from Central Africa, Mongols in Asia as well as in infecting drug users. The
present work had as objective describing HTLV molecular epidemiology, in
three different populations the State of Amapá, they are: HIV/IDS positive
patients, Afro-descenig population and individuals assisted of Public Health
Center Laboratory of Amapá – LACEN-AP, directed for diagnosis of HTLV. The
samples were tested for the presence of virus using serological (ELISA and
Western blot) and molecular assays (gene amplification and restriction fragment
length polymorphism from pX and env for the analysis of polimorfismo o
restriction fragments for endonuclease action. The obtained results in different
populations are In the population of HIV infected people, all the samples were
negative; in the Afro-desceding population, only one sample was positive
confirmed by serological test (ELISA), but negative according to western blot
test and submitted to the molecular analysis, there wast not amplification.
However, among samples of individuals directed for diagnosis of HTLV, 06 (six)
were positive, 5(five) out of 6/them were confirmed by western blot test. The
molecular result demonstrated the presence of HTLV-1.

Previous issue date: 2007 / Uma das principais características do Vírus da imunodeficiência humana
(HIV) é sua grande diversidade genética. O presente trabalho tem como objetivo descrever a epidemiologia molecular do HIV-1 circulante na cidade Macapá, Amapá,
assim como os fatores associados à aquisição da infecção e correlacionar estes com os
subtipos virais encontrados e as informações epidemiológicas da população examinada. Amostras de sangue de 48 indivíduos portadores do HIV foram colhidas no Serviço de
Assistência Especializada (SAE) da cidade de Macapá, no período de junho de 2003 a junho de 2004. Após a extração do DNA, foi realizada a amplificação de 297 pb da protease (gene pro) por meio da técnica de Nested PCR, sendo seqüenciadas, posteriormente, para a determinação dos subtipos virais. Foram identificados os
subtipos B (93,7%) e F (6,3%) na cidade de Macapá, que são os mais prevalentes no
Brasil, não tendo sido observada associação entre os subtipos do HV-1 circulantes nesta
cidade e a preferência sexual. No entanto, uma parcela significativa da população
examinada possui um baixo grau de escolaridade, característica esta que reflete a maior
parcela população infectada pelo HIV no Brasil. / One of the main characteristics of Human immunodeficiency virus (HIV)
is its great genetic diversity. The present work has as main objective the description of
the molecular epidemiology of HIV-1 circulating in the city of Macapa, Amapa, as well
as the factors associated with the infection and to correlate these with the viral subtypes
found and the epidemiological data of the investigated population. Blood samples from
48 subjects bearers of the HIV were collected in the Service of Specialized Assistance
(SSA) in the city of Macapa, in the mean time of June 2003 to June 2004. After the
DNA extraction, was performed an amplification of 297 bp from the protease (gene pro)
y means of the Nested PCR assay, being sequenced, afterwards, for the determination of
the viral subtypes. Were identified the subtypes B (93.7%) and F (6.3%) in the city of
Macapa which are the most prevalent in Brazil, and was not observed an association
between the subtypes of HIV-1 circulating in this city with sexual preferences.
Although, a significant part of the population examined has a low degree of scholarity,
characteristic which reflects the majority of the population infected with HIV in Brazil.

Previous issue date: 2011 / CAPES - Coordenação de Aperfeiçoamento de Pessoal de Nível Superior / O V.cholerae é um microorganismo autóctone do ambiente aquático e os sorogrupos O1 e O 139 estão ligados a pandemia e epidemia de cólera. Os V.cholerae não O1 e não O139 ou vibrios não aglutinantes (NAGs) estão envolvidos em casos isolados e surtos de diarréia semelhantes à cólera. No decorrer da sétima pandemia houve o surgimento de diversos isolados “El Tor atípicos”. Entre estes se encontra a variante bioquímica do V.cholerae O1 que não fermenta a sacarose no TCBS em 18 a 24 horas que é o tempo de incubação convencional. Neste trabalho foram estudados 138 isolados de V.cholerae O1 e não O1 não fermentador da
sacarose no TCBS de procedência clínica e ambiental, obtidos entre 1994 e 1995 na Amazônia Brasileira (Estados do Pará, Amapá e Amazonas). Avaliou-se a
fermentação da sacarose no TCBS e em caldo; o perfil de suscetibilidade a oito diferentes antimicrobianos em ágar difusão; a relação clonal entre os V.cholerae O1
e NAG clínicos e ambientais pelo PFGE e a presença de genes de virulência ctxAB e tcpA pela reação em cadeia da polimerase. Observou-se que as amostras de
V.cholerae não fermentaram a sacarose em 24 de incubação no ágar TCBS e em
caldo, 43% utilizaram a sacarose em 24 horas e 57% a fermentavam tardiamente
(tempo superior a 24 horas). Os isolados apresentaram baixo percentual de
resistência a antimicrobianos (8,7%) e nenhum caso de multiresistência. Em relação
aos genes de virulência, de um modo geral, os isolados de V.cholerae O1
apresentavam o tcpA e o ctxAB. Nos não O1 estes estavam ausentes, com exceção
de um isolado clínico não O1 (gene tcpA+). A análise do PFGE revelou pulsotipos
distintos entre os O1 e NAGs, embora dois destes últimos tenham apresentado
relação clonal com os O1 clínicos. Todos os O1 clínicos apresentaram relação clonal
com isolados de referência da sétima pandemia. / The V. cholerae is an autochthonous organism of the aquatic environment and
serogroups O1 and 139 are associated to the pandemic and epidemic cholera. The
V. cholerae non-O1 and non O139 or non-binders vibrios (NAGS) are involved in
isolated cases and outbreaks of cholera-like diarrhea. During the seventh pandemic
there was the emergence of several isolated “El Tor atypical”. Among these there is a
biochemical variant of V. cholerae O1 that does not ferment sucrose in TCBS in 18 to
24 hours which is the conventional incubation time. In this work, we studied 138
isolates of V. cholerae O1 and non O1 non-fermenter of sucrose on TCBS from
clinical and environmental origin, obtained between 1994 and 1995 in the Brazilian
Amazon (states of Pará, Amapá and Amazonas). We evaluated the fermentation of
sucrose in TCBS and broth; the susceptibility to eight different antimicrobials in agar
diffusion, the clonal relationship between V. cholerae O1 and NAG from clinical and
environmental origin by PFGE and the presence of virulence genes tcpA and ctxAB
by the polymerase chain reaction. It was observed that the samples of V. cholerae
did not ferment sucrose in 24 hours of incubation in TCBS agar and broth, 43% used
sucrose in 24 hours and 57% fermented it lately (more than 24 hours). The isolates
had a low percentage of antimicrobial resistance (8.7%) and no cases of multidrug
resistance. Regarding the virulence genes, in general, the isolates of V. cholerae O1
showed the ctxAB and tcpA. In the non-O1, these were absent, except for one
clinical isolate non-O1 (gene tcpA +). The PFGE analysis revealed pulsotypes
distinguished between O1 and nags, although two of the latter had presented the
clonal relationship to clinical O1. All O1 clinical isolates were clonally related to the
reference isolates from the seventh pandemic.

Previous issue date: 2007 / No gênero Staphylococcus, o S. aureus com resistência à oxacilina é sem duvida alguma o patógeno de maior importância, quando associado às infecções hospitalares, sendo responsável por elevadas taxas morbidade e mortalidade. Este estudo descreve a epidemiologia e perfil de sensibilidade de linhagens de S. aureus procedentes de hospitais públicos de Macapá-Amapá. Todos os isolados usados neste trabalho foram novamente reisolados através de métodos convencionais da microbiologia e sistemas automatizados. As amostras com resistência à oxacilina foram todas submetidas ao teste screening com a cefoxitina 30 mcg. O tratamento estatístico dos dados revelou que houve predominância de S. aureus no sexo masculino (62,8%), sendo a média de idade dos pacientes de 20 anos, entretanto, a maior ocorrência foi na faixa etária de 0 a 10 anos, o hospital de maior prevalência foi o Hospital da Criança e do Adolescente (54,7%). A prevalência das amostras isoladas nos hospitais foi de 3,8%. Do total de amostras isolada (n=105), 25 (23,8%) foram resistentes à oxacilina. Essas amostras apresentaram resistência cruzada à Gentamicina (80%); Sulfazotrim (72%), Tetraciclina (64%), Eritromicina (60%), Clindamicina (44%), Norfloxacino (44%) e Quinupristina/Dalfopristina (32%). A vancomicina apresentou 100% de sensibilidade. Apesar dos diversos estudos realizados no Brasil e no mundo mostrarem altos índices de resistência do S. aureus à oxacilina, nesta pesquisa os níveis de resistência da bactéria nos hospitais públicos de Macapá ainda podem ser considerados baixos. Contudo, os resultados revelam a necessidade de vigilância sistemática, visando o controle e prevenção da disseminação de linhagens resistentes deste patógeno associado com infecção hospitalar. / Amongst the Staphylococcus genus, the oxacillin-resistant Staphylococcus aureus is without doubts some the pathogen of bigger importance, when associated to the nosocominal infections, being responsible for high mortality and morbidity rates. This study describes the epidemiology and the sensitivity profile of the Staphylococcus aureus originated from public hospitals of Macapá-Amapá. All strains used in this study had been again isolated through manual and automatized methodology. The oxacilin-resistant samples had all been submitted to the screening test with cefoxitin 30 mcg. The statistical treatment of the data revealed the predominance of the masculine sex (62,8%), being of 20 years the average age of the patients. The biggest occurrence was in the age group of 0 to 10 years, the hospital of bigger prevalence was the Hospital of the Child and the Adolescent (54,7%). The prevalence of the isolated samples in the hosptals was 3,8%. 25 (23,8) of the total isolated samples (n=105) were resistant to oxacilin. These samples presented crossed resistnce to gentamycin (80%), sulfazotrim (72%), tetracyclin (64%), erythromycin (60%), clindamycin (44%), norfloxacin (44%) and quinupristin/dalfopristin (32%). The vancomycin presented 100% of sensitivity. Despite many studies done in Brazil and in the world show high rates of oxacillin-resistant S. aureus, in this research the resistance levels of the bacterium in the public hospitals of Macapá still can be considered low. However, the results reveal the need for systematic surveillance for the control and prevention of the spread of resistant strains of the pathogen associated with nosocomial infection.

Previous issue date: 2005 / No presente estudo a soroprevalência da infecção pelo Vírus da hepatite C (VHC) foi investigada em indivíduos portadores da infecção pelo Vírus da imunodeficiência humana 1 (HIV-1) da cidade de Macapá, Estado do Amapá, Brasil. Um total de 120
indivíduos infectados pelo HIV-1 foi testado para a presença de anti-VHC usando um ensaio imunoabsorvente ligado a enzima. Todos os pacientes envolvidos no presente estudo foram testados para a carga viral plasmática do HIV-1 e para os níveis de células
T CD4+ no momento do consentimento em fazer parte do estudo. Os pacientes responderam a um questionário epidemiológico no momento da coleta de sangue. Do total de 120 amostras testadas apenas sete (5,83%) foram soropositivas para anti-VHC. Quanto ao gênero, quatro (57,2%) e três (42,8%) eram homens e mulheres, respectivamente. A idade dos indivíduos soropositivos para anti-VHC variou de 21 a 40 anos entre os homens e de 21 a 60 entre as mulheres. Considerando o impacto da coinfecção
nos valores de carga viral plasmática do HIV-1 e na contágem de células T CD4+ não foram observadas diferenças significativas (Odds Ratio=2,7368, p=0,3624; Odds Ratio=1,7803, p=0,7764). O uso de drogas injetáveis e de piercing mostrou ser
um importante fator de risco para a co-infecção (p<0,05). Os resultados do presente estudo representam os primeiros achados acerca da co-infecção HIV-1/VHC no Estado do Amapá e demonstra a necessidade de um estudo continuado objetivando avaliar a
soroprevalência da infecção pelo VHC no Amapá e detectar o real impacto clinico da co-infecção HIV-1/VHC no paciente. / In the present study, the seroprevalence of Hepatitis C virus (HCV) infection was
investigated among Human immunodeficiency virus-infected subjects from the city of
Macapá, state of Amapá, Brazil. A total of 120 HIV-1-infected subjects were tested for
the presence of anti-HCV using an enzyme-linked immunosorbent assay. All the
patients enrolled in the present study were tested for plasma HIV-1 viral load and CD4+
T-cells at the time of acceptance to join the study. The patients responded to an
epidemiological questionnaire at the moment of blood collection. Seven out of 120
tested samples (5,83%) were seropositive for anti-VHC. Concerning gender,
four(57,2%) and three(42,8%) were male and female, respectively. The ages of HCV
seropositive subjects ranged from 21 to 40 years for males and from 21 to 60 for
females. Take into account the impact of co-infection on plasma HIV-1 viral load and
CD4+ T-cells quantification it was not observed significant differences (Odds
Ratio=2.7368, p=0.3624; Odds Ratio=1.7803, p=0.7764). The intravenous drug use and
piercing showed to be important risk factors to co-infection (p<0.05). The results of the
current study represent the first report about HIV-1/HCV co-infection in the state of
Amapá, and highlight the need for continuous study aiming to evaluate the
seroprevalence of HCV infection in Amapá and to detect the real clinical impact of
HIV-1/HCV co-infection on the patient.
