Aspectos cicatriciais do reparo das porções gastrocnêmias do tendão calcâneo envelopados com poli ácido lático-trimetileno carbonato em coelhos / Healing patterns related to the reconstruction of the gastrocnemic part of the Achilles tendon wrapped by a poly lactic acid trimethylene carbonate membrane on rabbits

Júnior, José Carlos Garcia 13 December 2017 (has links)
This study assessed the effectiveness of a new absorbable membrane, that presents mechanical and chemical features more suitable to tendons, in rabbit tendons. Before the animal model assessments a mechanical study of the membrane was carried out demonstrating that the minimal capability for elastic deformation of the membrane was more than 100%. All rabbits underwent to tenotomy and reconstruction of the right gastrocnius tendons, thereafter they were randomly divided in tendon wrapped by the membrane and control groups. Extraction was performed in the following periods of time: seven, 14 and 28 days. Assessments used macroscopy, histology, objective collagen assessment by using polarized light and Image-J® program in mean of gray values(mgv), sulphated glycosaminoglycans, genetic expression of proteoglycans. In the macroscopic 14 and 21-day assessments the membrane group presented less adherences p=0.02 and p=0.03 respectively. The modified Watkins classification: 14,67±0,42 membrane and 12,67±0,56 without membrane p=0,03 for 14 days; 19,88±0,83 membrane and 17,25±0,62 without membrane p=0,02 for 28 days. The type III collagen were 17,97±1,83 membrane and 12,63±1,07 without membrane p=0,029 for 14 days. For type I collagen were 2,41±0,33 membrane and 1,31±0,18 without membrane p=0,01 for 14 days and 7,30±0,63 memebrane and 2,92±0,32 without membrane p < 0,0001 for 28 days. The glycosaminoglycans were measured in 3 tendon portions, distal, central and proximal, by using ug/mg of dry tissue. In seven days just the distal part presented statistical differences 0,80±0,04 membrane and 0,38±0,04 without membrane, for 14 days no differences were found for Chondroitin-Sulphate. For Dermatan-Sulphate the central part of the tendon 0,42±0,09 and 1,29±0,67 p=0,02, for 14 days no differences were found. There was high variability for beta-actin and GAPDH for the samples in 28 days with inconclusive results that may mean no variability in gene expression of GAGs at this time period. Results as mentioned above demonstrated that the wrapped tendons by the new membrane presented acceleration in the healing processes for gastrocnemius tendons of New Zealand Rabbits

Efeito da aplicação do ultrassom terapêutico durante 4 e 5 minutos por área do transdutor no processo de reparação de tendão de ratos / Effect of application times 3, 4 and 5 minutes ERA of therapeutic ultrasound in tendon injury of calcaneal rats

Farcic, Thiago Saikali 29 April 2016 (has links)
Conclui-se que o UST, aplicado no tempo de 5 minutos por área de radiação efetiva, apresentou a melhor dose-resposta quanto à organização das fibras colágenas no reparo tecidual de tendões de ratos / The aim of this study was to evaluate the effect of application times 3, 4 and 5 minutes ERA of therapeutic ultrasound in the organization of the collagen fibers in rat calcaneal tendon injury. Forty male Wistar rats were used, of which 32 underwent complete tenotomy of the calcaneal tendon and were divided into 5 groups: GC without tenotomy and treatment; GT tenotomy with and without treatment; UST3, UST4 UST5 and submitted to tenotomy treated with therapeutic ultrasound at times 3, 4 and 5 minutes per effective radiating area respectively. The animals were submitted to the first application of therapy US tenotomy 24 hours after surgery. Ultrasonic irradiation was applied with the following parameters: 1 MHz, pulsed mode at 20% duty cycle (2ms transmission / 8 ms interval), frequency 100 Hz, 0.5 W / cm² intensity and ERA 0.5 cm². The application was performed 1x / day. The animals were sacrificed after the 10th treatment session, on the 12th postoperative day. The tendons were surgically removed for analysis of the organization of the collagen fibers through birefringence method (optical delay - OR). The collagen fibers showed better aggregation and organization in group UST3, UST4 and UST5 when compared to the GT (p <0.05) and UST5 showed better response in the intergroup comparison. We conclude that the UST, applied in time of 5 minutes for effective radiation area, presented the best dose-response as the organization of the collagen fibers in tissue repair of rat tendons

Evaluation of tissue repair process in rats Achilles tendon after tendinopathy induced by collagenase: effect of low-level laser and anti-inflammatory drugs.

Rossi, Rafael Paolo 23 August 2011 (has links)
Introduction: Achilles tendinopathy is characterized by presence of classical signs of inflammatory response such as edema, pain, and structural changes in tissue. In this study we evaluated the repair process and tissue reorganization of rats tendons submitted to collagenase induced tendinopathy, being posteriorly treated with low-level laser therapy, steroidal anti-inflammatory and non-steroidal anti-inflammatory. Materials and Methods: Female Wistar rats were submitted to transcutaneous collagenase injection (100<font face=\"Symbol\">mg/tendon) at paws peritendinous site and posteriorly divided and treated in following groups: laser (3J), potassium diclofenac (1.1mg/kg), and dexamethason (0.02mg/kg). Histomorphological analyses were made including findings score, tissue collagen amount and collagen fibers aggregation/organization levels. Results and Discussion: Collagenase produced enhancement of inflammatory cells, edema and hemorrhage. Laser therapy showed be the better option between tested treatments, decreasing inflammatory response, increasing collagen concentration and mantaining collagen fibers organization.

Intra and inter-rater reliability of triceps surae morphological and mechanical properties in healthy subjects

Sonda, Francesca Chaida January 2018 (has links)
The Achilles tendon and the plantar flexor muscles play a key role in daily life activities such as raising from a chair, during gait and jump. Changes in mechanical overload can lead to both beneficial adaptations in the mechanical and morphological properties of the triceps surae muscle-tendon unit, such as after systemic exercise, as well as after the loss of structure and functionality after total Achilles tendon rupture and aging. The evaluation of the triceps surae structural and mechanical properties is fundamental so that losses in the health condition can be identified, as well as the improvements resulting from the recovery process of the musculoskeletal structures during a rehabilitation program. Therefore, it is fundamental that the methodology used to evaluate the structural and mechanical properties of the triceps surae muscle-tendon unit is reliable, and that the results are not influenced by the evaluator (high inter-rater reliability) or by the day and time of evaluation (high intra-rater reliability). However, there is still a lack of evidence in the literature regarding the psychometric variables related to the evaluation of the mechanical and morphological properties of the Achilles tendon and the plantar flexors muscular architecture in healthy subjects. In this perspective, the present dissertation presents three studies in order to achieve the following objectives: (Study 1) To verify the intra- and inter-rater reliability of the plantar flexors muscle architecture in healthy individuals. (Study 2) To verify intra- and inter-rater rater Achilles tendon morphology evaluation in healthy individuals. (Study 3) To verify intra and inter-rater reliability of the triceps surae isometric strength measurements The three studies were performed at the Neuromuscular Plasticity Sector of LAPEX of ESEFID-UFRGS. All participants were evaluated in their triceps surae muscle architecture (fascicle length, pennation angle and muscle thickness) and the Achilles tendon (cross-sectional area, tendon length and free tendon length) parameters by means of skeletal muscle ultrasonography. Maximal isometric voluntary contractions were performed on an isokinetic dynamometer. The evaluations occurred in 2 different moments: day 1 and day 2 by evaluator 1 (7-days interval between collections), and on the second day by 2 additional evaluators (2 and 3) in a successive way. Data analysis was performed in SPSS v. 20.0, by means of descriptive statistics (mean, SD - standard deviation) and inferential analysis (ICC - Intraclass Correlation Coefficient - SEM - Standard Error between Measures = DP√ (1 - ICC) MDC – Minimum Detectable Change = 1.96*EPM), and confidence interval analysis. In study 1, we verified a high intra- and inter-rater correlation for muscle thickness, pennation angle and fascicle length (ICC> 75), and low SEM and MDC values. In study 2, we found a high intra- and inter-rater correlation for total tendon length and cross-sectional area (ICC> 75). However, for the free tendon variable, the inter-rater correlation was low (ICC: 0.580). Comparing the evaluators' experience, evaluator 1 and 2 obtained a ICC of 0.910, whereas evaluators 1 and 3 had an ICC of 0.340, while between evaluators 2 and 3 the ICC was 0.280 These very low correlations for the free tendon variable can be explained by the evaluators' experience and by the training of the measurements. In study 3, we verified a high intra- and inter-rater correlation for isometric torque values of the plantar flexor muscles (ICC> 75), and low SEM and MDC values. Based on the above evidences, we can conclude that there is a high intra-rater and inter-rater reliability for muscle architecture, tendon structure and plantar flexor strength. In addition, our data can be used as a data base for the healthy condition for the comparison with patients with Achilles tendon injury. Our methodology can also be used in patients’ evaluation. However, some improvements in the used techniques are necessary to improve reliability and further reduce measurement errors.

The aim of this study was to evaluate the effects of different treatment times of therapeutic ultrasound (US) on tendon injury healing. Forty male Wistar rats were selected (300 ± 45g) and 32, who underwent tenotomy of the Achilles tendon, were divided into five groups: Control, without tenotomy nor any treatment; tenotomy group, with tenotomy and without treatment; US groups (US1, US2, and US3), submitted to tenotomy and treated respectively with US for one, two, and three minutes per area of the transducer. The animals were sacrificed on the 12th post-operative day and the tendons were surgically removed for analyses of the collagen fiber organization by means of birefringence analyses, or optical retard. The collagen fibers showed better aggregation and organization in the US3 group, compared to the tenotomy group (p<0.05). The findings indicated that the US applied for three minutes per treated area improved the organization of the collagen fibers in the tendon repair of rats

Follow-up of Patients Treated with Sclerosing Therapy and/or Surgery for Achilles Tendinopathy

Hammar Clausen, Adrian January 2019 (has links)
Introduction: Achilles tendinopathy can be a chronic disabling condition. Sclerosing injections under ultrasonographic guidance is one method to treat Achilles tendinopathy. Initially good results have later been questioned. Surgery is another treatment option that has been questioned because of varying reported success rate. Aim: We aimed to assess patient- reported outcome in patients suffering from Achilles tendinopathy, treated with sclerosing injections and/or surgery during a 6 ½ year- period. Method: After review of patient records, the Self-reported foot and ankle score (SEFAS) together with an in-house satisfaction questionnaire were mailed to the patients. A SEFAS score of 48 represents normal foot/ankle function. Results: 97 patients (53 women, 44 men, 104 tendons) were included. 69 patients (41 women, 28 men, 75 tendons) returned the questionnaires. The SEFAS values (median and range) were 37.5 (13-48) in patients treated with sclerosing therapy, 42 (15-48) in patients treated surgically and 47 (19-48) in patients that received both treatments. A greater proportion of surgically treated patients were satisfied (90% vs 50%), experienced symptom improvement and were able to return to the previous level of activity. Complications following surgery were wound infections (n=3) and deep vein thrombosis (n=3), two with pulmonary embolism. Following sclerosing injection, there was one complete Achilles tendon rupture. Conclusion: Sclerosing injections seems to be a safe treatment and a positive outcome in 50% of patients might be sufficient to use this therapy in selected patients with Achilles tendinopathy. However, surgical treatments seem more effective but are associated with more severe complications.

Foundations of Deduction's Pedigree: A Non-Inferential Account

Seitz, Jeremy January 2009 (has links)
In this thesis I discuss the problems associated with the epistemological task of arriving at basic logical knowledge. This is knowledge that the primitive rules of inference we use in deductive reasoning are correct. Knowledge of correctness, like all knowledge, is available to us either as the product of inference, or it is available non-inferentially. Success in the campaign to justify the correctness of these rules is mired by opposing views on how to do this properly. Inferential justifications of rules of inference, which are based on reasons, lead to regressive or circular results. Non-inferential justifications, based on something other than reasons, at first do not seem to fare any better: without a basis for these justifications, they appear arbitrary and unfounded. The works of Boghossian and Dummett who argue for an inferentialist approach, and Hale who supports non-inferentialism are carefully examined in this thesis. I conclude by finding superiority in Hale's suggestion that a particular set of basic logical constants are indispensable to deductive reasoning. I suggest that we endorse a principle which states that rules are not premises, and are therefore to be excluded from expression as statements in a deductive argument. I argue that the quality of being indispensable is sufficient for a basic rule of deduction to be countenanced as default-justified, and therefore need not be expressed in argument. By a rule's evading expression in argument, it avoids circular reasoning in deductive arguments about its own correctness. Another important outcome that emerges from my research is the finding that non-inferential knowledge is ontologically prior to the inferential sort. This is because plausible inferential knowledge of basic logical constants shall always be justified by circular reasoning that already assumes the correctness of the rule to be vindicated. This initial assumption is tantamount to non-inferential knowledge, and therefore this latter is more primitive-in fact the only primitive-species of basic logical knowledge.

Upplevelse av fotfunktion : efter konservativt behandlad hälseneruptur

Svanbom, Anna, Wedebrand, Sofia January 2012 (has links)
Background: Previously research shows that people treated conservatively can have reduced foot function after Achilles tendon rupture. However, there are few qualitative studies in the subject, so this essay is written to provide a deeper understanding of how these people feel about their foot function one to two years after the injury. Purpose: Investigate how people perceive their foot function after conservative treatment one to two year after Achilles tendon rupture and if they have an avoidance behavior in their everyday lives. Method: Four people who suffered from Achilles tendon rupture participated in this qualitative study. The study was conducted with semi-structured interviews, which in turn were processed using qualitative content analysis. Results: The perceived foot function is today varied in terms of mobility, strength and pain according to the participants. They feel difference in their everyday life in comparison with before the injury. It appears that participants have avoidance behavior when it comes to the situation of injury. They expressed that the time course of development of foot function from the time of injury until today has been long-lasting but most of the participants mentions that there is still development of the foot function. Conclusion: The participants experience reduced foot function in varying degrees, who affect them in their everyday life, in situations including walking and staircase- walking. For this reason is it difficult to draw a collective conclusion of the participants experienced foot function. However, the situations where the injury occurred are avoided by the participants because of fear of getting a new rupture. Development of foot function is continuing, but is perceived as slow by many participants and they are not satisfied with this.
