An Examination of Awareness of Over-the-Counter Nonsteroidal Anti-Inflammatory Drugs and Adverse Events

<p> The elderly population is among the fastest growing populations in the United States. Finding and consuming medications safely and effectively are challenging endeavors for this population. Nonsteroidal anti-inflammatory drugs (NSAIDs) are a widely consumed class of medications among the elderly population, with 70% of individuals consuming over-the-counter (OTC) NSAIDs once a week and 34% using them daily. The purpose of this quantitative study was to determine whether (a) patients are aware of the risks associated with the consumption of NSAIDs, and (b) there are differences in awareness based upon specific demographic characteristics and levels of patient-physician communication. The health belief model (HBM) was used to interpret the results. The HBM is a social cognition framework that takes into account different perceptions, namely, perceived susceptibility of acquiring a health condition, perceived severity of the condition and its consequences, perceived barriers to engaging in the recommended behavior, perceived benefits of engaging in the recommended behavior, and perceived costs of engaging in the recommended behavior. Multiple linear regression was used to analyze the data. The results, which were based upon a cross-sectional survey of 124 participants, showed that the participants' awareness of adverse events associated with NSAIDs use was not associated with sociodemographic variables, rates of consumption, or patient-physician communication. The findings will give the key stakeholders more insight into the issue of preventable adverse events that might lead to the establishment of more safety programs and informatics structural systems to monitor the consumption of OTC NSAIDs and improve lines of communication to protect the elderly population.</p>

Rehabilitation Therapy Services For Older Long–Stay Clients in the Ontario Home Care System

BACKGROUND Rehabilitation therapies are effective for older persons in home-based settings, and have the potential to save money for the health system, while also improving the quality of life for older adults who may otherwise be hospitalized or institutionalized. Although there is evidence that home-based rehabilitation can improve functional outcomes in older adults, research has shown that many older home care clients do not receive the rehabilitation services they need. Despite the home care sector’s increasing importance within Ontario’s health care system, we have a limited understanding of the population that currently utilizes these services and how these services are allocated in the province. This dissertation project aims to enhance the understanding of this domain using a large provincial data repository of home care client information (RAI-HC information system). METHODS Using the Andersen-Newman Framework to guide this research from a conceptual standpoint, and combining it with the CRoss Industry Standard Process for Data Mining (CRISP-DM) as an organizational framework, this dissertation focuses on examining data collected on older long-stay home care clients. Prior to the data mining modeling procedures, knowledge of the rehabilitation services in home care was developed through a series of semi-structured interviews with key informants. The results of this qualitative study were then used to inform quantitative analyses that included creating rehabilitation service user profiles using the K-means clustering algorithm, and the development of predictive models of rehabilitation service provision using a Random forest algorithm and multilevel models. RESULTS Older home care clients who receive occupational therapy and physiotherapy in the Ontario Home Care System form a complex and heterogeneous client population. These services are often provided to clients following an acute event, yet many older adults who could benefit from therapy services for functional improvement and maintenance are not provided services due to limited resources. K-means clustering analyses resulted in the creation of seven profiles of rehab service users illustrating the multidimensional diversity of the service user population. Predictive models were able to identify client characteristics that are commonly associated with service provision. These models confirmed the large amount of regional variation found across the province and highlighted the differences between factors that lead to occupational therapy and physiotherapy service provision. CONCLUSIONS Using multiple methods to systematically examine rehabilitation services for long-stay clients, new insights into the current user population and the client characteristics related to service provision were obtained. Future research activities should focus on ways to use the regularly collected standardized data to identify older long-stay home care clients who would benefit most from the rehabilitation therapy services provided by the provincial home care system.

Dados epidemiológicos têm demonstrado crescente envolvimento da mulher idosa na epidemia de aids, esta constatação foi a motivação para a realização do presente estudo. Trata-se de uma pesquisa qualitativa, que teve como referencial teórico a Antropologia Médica; para o tratamento dos dados adotou-se o Discurso do Sujeito Coletivo. Os objetivos do estudo foram: verificar a percepção sobre a epidemia de aids por mulheres com idade igual ou superior a 50 anos, moradoras em uma comunidade de baixa renda no Município de São Paulo; verificar sua percepção de risco à infecção pelo HIV, e se adotam ações preventivas da infecção. Como critérios de inclusão consideraram-se: mulheres com idade igual ou superior a 50 anos e tempo de residência no local do estudo superior a um ano. Os dados foram coletados por meio de entrevista com questões sobre caracterização da mulher e com uma pergunta norteadora. Foram entrevistadas 13 mulheres, que mostraram ter como características comuns: baixa escolaridade, reduzida renda familiar e religiosidade. O tratamento dos dados resultou na construção de 12 discursos narrados na primeira pessoa, nos quais se verifica que as mulheres do grupo cultural estudado apresentam lacunas no conhecimento a respeito da aids e do cotidiano do indivíduo infectado pelo HIV. Atribuem significado religioso à epidemia; não se vêem sob risco de infecção pelo HIV por via sexual, principalmente devido à confiança na fidelidade do parceiro; negam o uso do preservativo nas relações sexuais, embora temam a aids por identificarem-na como uma doença incurável. Valorizam a necessidade de adoção de medidas preventivas à infecção pelo HIV, porém, pelos jovens, considerados mais susceptíveis. A análise dos dados, segundo o referencial teórico adotado, mostrou que a aids, para o grupo de mulheres estudado, não se constitui em preocupação, quando a abordagem recai na transmissão sexual em si própria, fato que alerta para a necessidade de planejamento de ações educativas culturalmente embasadas sobre a infecção pelo HIV nesse segmento. / Epidemic data has been demonstrating the senior woman\'s growing involvement in epidemic aids, this seen, it suited as motivation for the present study\'s realization. It\'s over a qualitative research, which had as theoretical reference the Medical Anthropology; for the data treatment the Collective Subject\'s Speech was adopted. The study objectives were: to verify the perception of 50-year-old women or older on epidemic aids, residents in a community of low income in the Municipal district of São Paulo; to verify their risk perception to the HIV infection, also if preventive actions are adopted. As inclusion criteria were considered: 50 year-old women or older along with time of residence in the place of study superior to one year. The data was collected through subjects\' interview over the woman\'s characterization and with a leading question. 13 women were interviewed that showed to have as common characteristics: low school education, reduced family income and religiosity. The treatment of the data resulted in the construction of 12 first person narrated speeches, which verify that women from the studied cultural group present gaps in the knowledge regarding aids and the HIV infected individual\'s everyday. They attribute religious meaning to the epidemic; not seeing each other under infection risk for HIV under sexual way, mainly due to trust in the partner\'s fidelity; they deny the use of preservatives in their sexual relationships, although they fear aids for identifying it as an incurable disease. They value the need to adopt preventive measures to the HIV infection, however, for the youths, considered more susceptible. The data analysis, according to the adopted theoretical reference, showed that aids, for the studied women\'s group, is not constituted in concern, when the approach relapses in the sexual transmission itself, fact that alerts to the need of educational action planning culturally based on the infection by HIV in that segment.

The main factors which influence these social and health facilities during the planning of social and health services are a demographic prognosis, the social structure of its inhabitants, the informal support and solidarity within the family, and also the geographic profile of the region, urbanization, human settlement and the extent of municipality. The planning of social and health services has to follow modern principles in social services which are widely used in Western Europe. The goal of this thesis is to undertake research in the capacity and quality of health and social services and the use of these services by seniors in the region. The next goal is to see if modern European principles are applied in the social and health services in the region. This research was carried out though the analysis of documents and methods of questioning. This research has shown that there is a lack of institutional social and health services in the region. Also modern European principles have not been applied in the social and health services.

It is estimated that by 2050 has to be more than 2 billion people over the age of 60 years in the world. Brazil follows the change this, since 2020 it is expected that the Brazilian population has about 29 million seniors. Given the importance of developing research focusing on this audience, in this study the objective was to describe the health status of elderly residents in the municipalities of the environs South and North Region Integrated Development of the Federal District and Surrounding Areas - RIDE-DF between 2010 and 2012. for the study was used a descriptive epidemiological study, cross the household survey population based in the municipalities belonging to the surrounding south and north. For data collection was performed participant definition by selection with equal likelihood held in three stages, at the end of the stages were included those who had more than 60 years. Analysis using software SPSS 19.0 and Excel 2010. This study was approved by the Ethics Committee on human and animal medical research at the University of Brasilia under the FR 342534. protocol Search by 112 people aged over 60 years. The results show that most of the participants are female, aged between 60 and 74 years, both women and men. 53.1% of women are widows and 72.9% of men are married or living with a partner. As for the work 58.9% reported being retired or receiving pensions. When asked about their health self-assessment results were positive where 41.9% as very good and good. With regard to health conditions, 51.8% of the elderly are hypertensive, 24.1% reported back problems 22.3% high cholesterol, also addressing the morbidities 13.7% of seniors report having been diagnosed with diabetes , however reports do not seek health services for treatment. From the results obtained in this study, it is clear that this increase in life expectancy of the elderly is a challenge for the management of the three spheres of government. Therefore, it is important to implant specific services for the elderly population not only focused on chronic diseases, but also for the development and maintenance of functional capacity of the individual thus enhancing their autonomy, as well as disease prevention and / or complications. / O processo de envelhecimento vem se acelerando no Brasil e no mundo. Estima-se que até 2050 tenha-se mais de 2 bilhões de pessoas com idade superior a 60 anos no mundo. O Brasil acompanha essa mudança, visto que até 2020 espera-se que a população brasileira tenha cerca de 29 milhões de idosos. Diante da importância de se desenvolver pesquisas tendo como foco este público, neste estudo o objetivo foi de descrever as condições de saúde dos idosos residentes nos munícipios dos Entornos Sul e Norte da Região Integrada de Desenvolvimento do Distrito Federal e Entorno - RIDE-DF entre os anos de 2010 e 2012. Para realização do estudo foi utilizado um estudo epidemiológico descritivo, transversal do tipo inquérito domiciliar de base populacional nos municípios pertencentes ao entorno sul e norte. Para a coleta de dados foi realizada a definição do participante mediante seleção com equiprobabilidade realizada em três estágios, ao final das etapas foram incluídos aqueles que tinham mais de 60 anos. Análise realizada por meio de software SPSS 19.0 e Excel 2010. Este estudo foi avaliado e aprovado pelo Comitê de ética em pesquisa médica humana e animal da Universidade de Brasília sob o protocolo FR 342534. Participaram da pesquisa 112 pessoas, com idade igual ou superior a 60 anos. Os resultados demonstram que a maior parte dos participantes é do sexo feminino, idade entre 60 e 74 anos, tanto mulheres quanto homens. 53,1% das mulheres são viúvas e 72,9% dos homens são casados ou vivem com companheira. Quanto ao trabalho 58,9% referiram estar aposentados ou recebendo pensões. Ao serem questionados quanto a auto avaliação de sua saúde os resultados foram positivos onde 41,9% consideram muito boa e boa. No que se refere às condições de saúde, 51,8% dos idosos são hipertensos, 24,1% referiram problemas de coluna 22,3% colesterol alto, ainda abordando as morbidades referidas 13,7% dos idosos relatam terem sido diagnosticados com diabetes, porem informa não procurarem os serviços de saúde para tratamento. A partir dos resultados obtidos neste estudo, percebe-se que esse aumento na expectativa de vida dos idosos é um desafio para a gestão das três esferas de governo. Por isso, é importante que se implante serviços específicos para a população idosa não só voltada para as doenças crônicas, mas também para o desenvolvimento e manutenção da capacidade funcional do indivíduo valorizando assim a sua autonomia, além da prevenção de agravos e/ou complicações.

Avaliação da atenção às condições crônicas em idosos: hipertensão arterial sistêmica e diabetes mellitus como condições traçadoras / Evaluation of care for chronic conditions in elderly patients: hypertension and diabetes mellitus as outlining conditions

Tratase de parte do Estudo SABE Saúde, Bem Estar e Envelhecimento, de delineamento longitudinal de base populacional que entrevistou 2143 pessoas de 60 anos e mais em 2000, no município de São Paulo, e reentrevistou 1115 delas em 2006. O objetivo do presente estudo foi avaliar a atenção às condições crônicas, utilizando a Hipertensão Arterial Sistêmica e o Diabetes Mellitus como condições traçadoras. Para isso, identificouse o uso e acesso aos serviços de saúde e as práticas de controle dessas doenças. Além disso, observouse a associação do uso de serviços com a ocorrência de desfechos desfavoráveis e compararamse indicadores de atenção à saúde com relação à posse de plano de saúde ou não. Foram utilizados testes estatísticos de regressão logística múltipla. Observouse uma prevalência de Hipertensão Arterial Sistêmica de 53,1 por cento e de Diabetes Mellitus do tipo 2 de 16,8 por cento em 2000, com incidência de autorreferência acumulada no período, de 30,0 por cento e 8,0 por cento respectivamente. O uso de três ou mais consultas por idosos hipertensos e/ou diabéticos em 2006 foi de 80,0 por cento e identificouse associação com os fatores de necessidade e com posse de plano de saúde, indicando desigualdades de acordo com o modelo de Andersen. A cobertura de planos de saúde entre os hipertensos e/ou diabéticos foi de 48,9 por cento em 2006, mantido no período. Foi referida dificuldade em usar serviços de saúde por 28,6 por cento dos hipertensos e/ou diabéticos em 2006, a maior parte relacionada à qualidade percebida dos serviços. Quem tem plano de saúde mostrou menor dificuldade de acesso, menor tempo de espera para agendamento e para ser atendido no serviço e maior satisfação com o uso. No entanto, o uso de serviços para controle foi maior entre os que não referiram posse de plano de saúde. Houve ampliação do acesso medicamentoso no período para ambas as doenças, em particular no setor público, com 70,5 por cento dos diabéticos e 88,4 por cento dos hipertensos usando medicação específica em 2006. Não se encontrou associação entre uso de serviços ambulatoriais nem com AVC, nem com perda de capacidade funcional sendo que, houve associação de posse de plano de saúde apenas com dificuldades em AIVD. Por outro lado, usar serviços ambulatoriais com maior frequência, mostrouse protetor ao risco de morrer. A taxa de mortalidade foi de 48 por mil para os hipertensos e 59,2 por mil para os diabéticos. Em conclusão, as condições crônicas estudadas têm forte impacto no uso de serviços, mas estes mostram pouca influência nos desfechos e sugerem desigualdades no acesso e na qualidade da atenção / This is part of the SABE Study Health, Welfare and Aging, a longitudinal study, which interviewed 2143 people, aged 60 and older in 2000, in São Paulo, and reinterviewed 1115 people, in 2006. The aim of this study was to evaluate the care for chronic conditions using Hypertension and Diabetes Mellitus as tracer conditions. For this, use of and access to health services and control practices were identified. Furthermore, association between use of health services and the occurrence of unfavorable outcomes was observed and, indicators of health care between public health and health insurance were compared. Logistic regression was used for multivariate analysis. The prevalence of hypertension was 53.1 per cent and type 2 Diabetes Mellitus was 16.8 per cent in 2000 and the incidence of selfreference accumulated in the period was 30.0 per cent and 8.0 per cent respectively. The use of three or more visits by hypertensive and / or diabetics in 2006 was 80.0 per cent. It was possible to identify an association between health services use and the factors of need and health insurance, indicating inequalities, according to the model of Andersen. The percent of the health insurance of hypertension and / or diabetes was 48.9 per cent in 2006. Difficulty in health services use was referred by 28.6 per cent of hypertensive and / or diabetics in 2006 mostly related to quality of services. Those who have health plan haves less difficulty of access, less waiting time for scheduling and to be serviced and increased satisfaction with use. However, the services use for disease control was higher among those who did not have health insurance and the access to drugs was increased, particularly in the public sector, with 70.5 per cent of diabetics and 88.4 per cent of the patients using drugs in 2006. There was no association between health services use and the incidence of stroke and loss of functional capacity, but the last one was associated with health insurance. On the other hand, an increased use of health services is related with a lower risk of death. Mortality rate was 48 per thousand for hypertensive and 59.2 per thousand for diabetics. In conclusion, the chronic conditions studied have a strong impact on the use of services, but they showed little influence on outcomes and suggest inequalities in access and quality of care

Estado nutricional e sua associação com força muscular, flexibilidade e equilíbrio de idosos residentes no município de São Paulo / Nutritional status and its association with muscle strength, flexibility and balance or elderly or the city or São Paulo

Objetivo - Avaliar o estado nutricional (EN) e sua associação com força muscular de membros inferiores (FMI) e de preensão manual (FPM), flexibilidade (FLX) e equilíbrio (EQ) de idosos do município de São Paulo. Delineamento - Estudo epidemiológico transversal, de base populacional e domiciliar, parte de estudo multicêntrico (Pesquisa SABE), coordenado pela Organização Pan-Americana de Saúde. Casuística - A Pesquisa SABE abrangeu 2143 idosos (&#8805; 60 anos), de ambos os sexos, no período de janeiro/2000 a março/2001, selecionados por amostragem probabilística; sendo que 1894 (88,4%), participaram deste estudo. Métodos - Os idosos foram estratificados segundo sexo e grupo etário (60-69, 70-79 e &#8805; 80 anos) e o EN foi avaliado de acordo com Índice de Massa Corporal (IMC): baixo peso (IMC &#8804; 23kg/m2) e obesidade (IMC &#8805; 28kg/m2). A FMI e a FLX foram avaliados por tempo, a FPM por dinamômetro e o EQ por 4 medidas estáticas. As associações foram verificadas por análise de sobrevida, segundo Cox (FMI e FLX) e por quiquadrado (FPM e EQ). Resultados - Verificou-se maior prevalência de obesidade em mulheres e de baixo peso em homens. Houve aumento na freqüência de baixo peso e redução da obesidade nos grupos etários mais avançados. Observaram-se associações estatisticamente significativas entre FLX e EQ e mulheres obesas; e entre FPM e baixo peso em homens e mulheres. Conclusão: A inadequação nutricional foi constatada em ambos os sexos e todos os grupos etários. As associações entre EN e capacidade funcional mostraram-se específicas para sexo, grupo etário e teste utilizado. / Objective - To evaluate the nutritional status (NS) and its association with handgrip strength (HS), lower limb muscle strength (LMS), flexibility (FLX) and balance (BL) of the elderly of the city of São Paulo. Design - Cross-sectional epidemiological study, in a population and household basis, is part of a multicenter study (the SABE Survey), which was coordinated by the Pan-American Health Organization. Setting - The SABE survey reached 2143 elderly (&#8805; 60 yrs old), from both genders, from january/2000 to march/2001, who were selected by probabilistic sampling; out of those, 1894 (88.4%) took part in this study. Methods - The elderly were separated by gender and age group (60-69, 70-79 e &#8805; 80 yrs old) for the tests, and the NS was evaluated according to the Body Mass Index (BMI): underweight (BMI &#8804; 23kg/m2) and obesity (BMI &#8805; 28kg/m2). The LMS and the FLX were measured by timing, the HS by a dynamometer and the BL by four static measurements. The associations were verified through Cox Regression analysis (LMS e FLX) and by chi square (HS e BL). Results - A larger prevalence of obesity was verified in women and of underweight in men. There was an increase in the underweight frequency and a reduction of the obesity level in the oldest age group. There were associations (statistically significant) between FLX and BL and obese women; and HS and underweight (men and women). Conclusions: The nutritional inadequacy was verified in both genders and all age groups. The associations between nutritional status and functional capacity were proved to be specific to each gender, age group and test.

This qualitative study, supported by the gerontology literature, employed the methodological resources of the analysis of oral reports and observations of activities developed by a group of elderly individuals. Volunteers were selected to participate in the study: five men (average age of 71.8 years) and five women (average age of 75.4 years). Socioeconomic data were collected using a questionnaire with closed questions. An open interview with a dialogue nature was then conducted. Data on information from the questionnaire were organized in a databank and later structured in the form of tables to facilitate the reader s understanding. Interviews were recorded on an audio recorder and in a field log; the topics were categorized in order for readers to be able to navigate better. Topics included the body that ages and the body that moves; beach environment; reflections on the body-nature relationship; body techniques; topophilia, health in the normal and pathological realms in the ageing process; as well as topics such as recreation and the history of the city of Santos / O presente estudo, realizado na cidade de Santos-SP, teve como objetivo compreender o significado do meio ambiente praiano para os idosos que assiduamente freqüentam este espaço natural para realizar atividades físicas. Caracterizado como um estudo qualitativo, a pesquisa, sustentada na literatura da gerontologia, utilizou como recursos metodológicos a análise de relatos orais e a observação das atividades desenvolvidas por um grupo de idosos. Foram selecionados, de forma voluntária, sujeitos de ambos os sexos, sendo cinco homens (média de idade de 71,8 anos) e cinco mulheres (média de idade de 75,4 anos). Inicialmente colheram-se dados sócio-econômicos, através de um questionário com perguntas fechadas e em seguida foi realizada uma entrevista aberta de caráter dialogal. Os dados referentes às informações do questionário fechado foram organizados em um banco de dados e posteriormente estruturados em forma de tabelas para melhor entendimento do leitor. As entrevistas, por sua vez, foram registradas por gravador e diário de campo sendo categorizadas em temas que o leitor poderá navegar. Objetivo - Avaliar o estado nutricional (EN) e sua associação com força muscular de membros inferiores (FMI) e de preensão manual (FPM), flexibilidade (FLX) e equilíbrio (EQ) de idosos do município de São Paulo. Delineamento - Estudo epidemiológico transversal, de base populacional e domiciliar, parte de estudo multicêntrico (Pesquisa SABE), coordenado pela Organização Pan-Americana de Saúde. Casuística - A Pesquisa SABE abrangeu 2143 idosos (&#8805; 60 anos), de ambos os sexos, no período de janeiro/2000 a março/2001, selecionados por amostragem probabilística; sendo que 1894 (88,4%), participaram deste estudo. Métodos - Os idosos foram estratificados segundo sexo e grupo etário (60-69, 70-79 e &#8805; 80 anos) e o EN foi avaliado de acordo com Índice de Massa Corporal (IMC): baixo peso (IMC &#8804; 23kg/m2) e obesidade (IMC &#8805; 28kg/m2). A FMI e a FLX foram avaliados por tempo, a FPM por dinamômetro e o EQ por 4 medidas estáticas. As associações foram verificadas por análise de sobrevida, segundo Cox (FMI e FLX) e por quiquadrado (FPM e EQ). Resultados - Verificou-se maior prevalência de obesidade em mulheres e de baixo peso em homens. Houve aumento na freqüência de baixo peso e redução da obesidade nos grupos etários mais avançados. Observaram-se associações estatisticamente significativas entre FLX e EQ e mulheres obesas; e entre FPM e baixo peso em homens e mulheres. Conclusão: A inadequação nutricional foi constatada em ambos os sexos e todos os grupos etários. As associações entre EN e capacidade funcional mostraram-se específicas para sexo, grupo etário e teste utilizado. / Objective - To evaluate the nutritional status (NS) and its association with handgrip strength (HS), lower limb muscle strength (LMS), flexibility (FLX) and balance (BL) of the elderly of the city of São Paulo. Design - Cross-sectional epidemiological study, in a population and household basis, is part of a multicenter study (the SABE Survey), which was coordinated by the Pan-American Health Organization. Setting - The SABE survey reached 2143 elderly (&#8805; 60 yrs old), from both genders, from january/2000 to march/2001, who were selected by probabilistic sampling; out of those, 1894 (88.4%) took part in this study. Methods - The elderly were separated by gender and age group (60-69, 70-79 e &#8805; 80 yrs old) for the tests, and the NS was evaluated according to the Body Mass Index (BMI): underweight (BMI &#8804; 23kg/m2) and obesity (BMI &#8805; 28kg/m2). The LMS and the FLX were measured by timing, the HS by a dynamometer and the BL by four static measurements. The associations were verified through Cox Regression analysis (LMS e FLX) and by chi square (HS e BL). Results - A larger prevalence of obesity was verified in women and of underweight in men. There was an increase in the underweight frequency and a reduction of the obesity level in the oldest age group. There were associations (statistically significant) between FLX and BL and obese women; and HS and underweight (men and women). Conclusions: The nutritional inadequacy was verified in both genders and all age groups. The associations between nutritional status and functional capacity were proved to be specific to each gender, age group and test.
