Ambulant (secondary) health care is the chain of health care, where specialized (secondary level) consultations of doctors specialists are rendered. In Lithuanian national health system we can mark three models (types) of institutions which render ambulant health care services, depending on organization of services and type of ownership. These institutions are: municipality clinics, rendering primary and secondary health care services (mixed institutions), municipality centres of primary health care (municipality institutions of primary health care) and private institutions of primary health care (independent contractors). This research is a way to name problems, which emerge seeking to control mixed institutions, rendering (secondary level) health care services. The aim of work: to analyse control problems of rendering ambulant health care services and propose possible ways of solutions. Methodology: to analyse ambulant services, rendered by secondary level doctors otorinolaringolists in the period 2006-01-01 – 2006-01-31, equivalence of these services to law acts and validity of tender to discharge them from the budget of compulsory health insurance allocation. Informational system data SVEIDRA of State patient fund and primary medicine documentary were analysed. L'objet de cette recherche urbanistique se situe à l'intersection de deux grandes dynamiques : la production officielle de l'espace public dans la ville contemporaine et le développement de l'activité commerciale dans les rues. Il s'agit d'une coproduction de l'espace public de facto. Cette thèse, en se construisant à partir d'études de différents types de terrain à Nankin (Chine), souhaite lancer une réflexion sur l'image et l'usage, la régulation et l'inventivité du quotidien. Elle cherche à démontrer les dimensions cachées du commerce de rue et son impact dans la vie quotidienne des habitants, car sa valeur sociale a souvent été ignorée, voire méprisée, dans la régulation de l'espace public ou dans la mise en place de projets urbains / The core topic of this urban research is situated at the crossing of two important dynamics: the official production of public space in the contemporary city and the development of commercial activities in the streets. Blowin' in the wind: encountering wind at fire lookouts in the Canadian Rocky Mountains

Weather, how we tangibly engage with climate in our everyday lives, is a central underpinning to life in Canada and around the world. This thesis investigates relating to weather through a focused exploration of wind in the everyday lives of fire lookout observers in the Canadian Rocky Mountains. Stitching together approaches from anthropology, phenomenology and mountain meteorology, it brings to bear insights on coexisting with weather changes through an understanding of lived mountain climates. Perched atop the front ranges of the Alberta Rocky Mountains are located a string of mountain fire lookouts. Tasked with discerning and detecting smoke plumes that may signal the start of a wildfire, lookout observers, who inhabit these remote lookout places for five to six months of the year, are attentive to the wind’s effect on visibility, its role in wildfire processes, and as a force to contend with in their daily lives on the lookout. Through participant observation, interviews and photo elicitation, I draw on fire lookout observers’ past and present experiences of wind, and its role in larger weather processes. With many lookout observers returning to their posts season after season, the breadth and depth of their experience stretches over three decades. Over the course of a summer’s fieldwork, I hiked in, and at times lived with, lookout observers. Walking, as a contemplative research practice, continued beyond the field and into analysis, engaging in a process I call ambulant listening as an alternative to transcribing interviews verbatim. This involved walking and listening to interviews multiple times, with notes later drawn out visually using mind maps. Through this process, I learned that wind stirs up much more than simply considering air in motion. Entwined in a variety of multi-sensory engagements, wind touches on broader themes of awareness, encounter and wonder that emerge as weather consciousness. This study offers a rare lens into a way of life that has been increasingly shuttered across Canada and around the world, while at the same time exemplifying ways of being and knowing weather inherent to coexisting with increasingly uncertain and unpredictable weather patterns in the midst of climate change. / Graduate / 0326, 0334,0314,0344 / kristen.walsh@hotmail.com

Specifika edukačního procesu u pacientů podstupujících plánovaný operační výkon ruky / Specifics of the educational process in patients undergoing elective hand surgery.

The object of this work is to chart an educational process of ambulant patients undergoing a planned arm surgery due to carpal tunnel syndrome or trigger finger and creating a functional educational material that leads to a change of knowledge and skills of those patients and that can be used by nurses during their work. The three questions to achieve the objectives were: 1) How does the educational process of the patient look like before arm surgery? 2) How is the patient educated on post-operative regime? 3) How are the educational activities implemented in patients undergoing arm surgery? The research project was aimed to the qualitative investigation. Generally the research survey was structured into two phases, the results are summarized in the practical part. The first phase was focused on observation of professional object and depth semi-structured interviews with patients who underwent an arm surgery were made. This survey was supplemented by direct structured observation of nurses in the process of educational activities. Six nurses were altogether involved in the survey. In the second phase, the educational material based on the obtained results, was made for the nurses to use during their work. This material includes educational cards and maps of care instructions for given diseases. Effectivnes of this material was subsequently verified by more semi-structured interviews with six patiens. The results of the survey showed that in the workplace where the respondents underwent the surgery, the educatinal process was singificantly eliminated and for its realization the nurses did not have satisfactory conditions, especially time and spatil conditions. During the educatinal process the nurses were using monologues teaching methods. Demonstration and practical methods were missing. Nurses were not using teaching aids that can make the educatinal process more effective. The results of this work can elucidate the educational reality in surgeries and focus attention on this sector, often neglected in expert literature.

Coordenação de Aperfeiçoamento de Pessoal de Nível Superior / A presente dissertação se insere no campo de estudo sobre o trabalho informal, tendo como universo empírico de investigação a experiência de trabalho dos trabalhadores ambulantes integrantes do Programa Renda Alternativa, em Rio das Ostras/RJ. Portanto, a pesquisa foi desdobrada em três dimensões: 1) levantamento e estudo bibliográfico; 2) levantamento de dados sobre a formação sócio-histórica da cidade de Rio das Ostras; e, 3) trabalho de campo com ambulantes. Por meio desses dispositivos, a pesquisa buscou compreender os laços entre informalidade do trabalho e dinâmica capitalista, por meio da tradição de estudos das Ciências Sociais nessa área. Seguidamente, a pesquisa se ocupou do estudo de outras investigações sobre trabalho ambulante visando conhecer e ampliar o escopo de análise das peculiaridades desse tipo de trabalho no contexto de reprodução social da chamada superpopulação relativa. Esse universo de questões teóricas, analíticas e empíricas foi confrontado com as especificidades do contexto socioeconômico da cidade de Rio das Ostras, recentemente recomposta como um dos elos da cadeia produtiva do petróleo. Outro eixo da investigação de campo foi conhecer o trabalho, o que foi realizado por meio de entrevistas semi-estruturadas com os trabalhadores ambulantes do município, buscando perceber a relação do trabalhador com o trabalho ao longo de sua vida, as marcas das conjunturas socioeconômicas nas trajetórias e o papel da família e do associativismo na ocupação ambulante. Os achados da pesquisa mostram que o trabalho ambulante acomete gerações de trabalhadores como consequência do desenvolvimento desigual e dependente do capitalismo que limita a oferta de emprego. Somado a isso temos os processos de transformações socioeconômicas das últimas décadas que intensificam as formas de precarização do trabalho potencializando a chamada informalidade do trabalho. Esse processo se intensifica na região em estudo através dos impactos trazidos pelo intenso incremento das indústrias de petróleo e gás que não absorvem a força de trabalho disponível no local e atraída, engendrando uma densa superpopulação relativa para o capital, no município. A experiência de trabalho dos ambulantes que participaram da pesquisa mostra que a mesma é entrecortada por formas laborativas variadas, baixa qualificação profissional, além de dependência dos familiares, de ações públicas do Estado e do associativismo. A expropriação do direito ao emprego empurra os trabalhadores para experiências laborativas inseguras, incertas e sem proteção social. A repetição dessa história entre familiares amplia a percepção da perenidade da expropriação social entre gerações de trabalhadores. / This work belongs to the field of study on informal work, as an empirical universe of research work experience of mobile workers, members of the "Alternative Income Program" in the Rio das Ostras/RJ. Therefore, the survey was split into three dimensions: 1) survey and literature research; 2) survey data on the socio - historical development of the city of the Rio das Ostras; and 3) field work with vendors. Through these devices, the research sought to understand the links between informality of labor and capitalist dynamics, through the tradition of studies in the area of Social Sciences. Next, the research dealt with the study of other investigations into street work aiming at understanding and broaden the scope of analysis of the peculiarities of this type of work in the context of social reproduction of the call relative surplus population. This universe of theoretical, analytical and empirical issues was confronted with the specifics of the socioeconomic context of the city of Rio das Ostras recently remade as one of the production chain oil. Another aspect of the field investigation was to understand the work, which was conducted through semi - structured interviews with itinerant workers of the municipality interviews, trying to discern the relationship of the worker to work throughout his life, the marks of socioeconomic situations in trajectories and the role of families and associations in street occupation. The research findings show that the itinerant work affects generations of workers as a result of unequal and dependent capitalism that limits the jobs development. Added to this we have the processes of socio-economic transformations of recent decades intensified forms of precarious work enhancing the call labor informality. This process intensifies in the study area through the impacts brought by the intense increase in oil and gas industries that do not absorb the workforce on site and attracted, engendering a dense relative surplus population to capital in the municipality. The work experience of walking in the survey shows that the same is intersected by various work-related forms, low-skilled, and dependence on family members, the public actions of the state and associations. The expropriation of the right to employment pushes workers into insecure, uncertain and no social security, work-related experiences. The repetition of this story between families enhances perception of continuity between generations expropriation of social workers.
