Esse grupo é caracterizado por alterações qualitativas das interações sociais recíprocas e modalidades de comunicação e por um repertório de interesses e atividades restrito, estereotipado e repetitivo. Embora já reconhecido que estes pacientes apresentam prejuízo no mínimo das três áreas citadas acima, estudos que avaliam o desempenho funcional global , ainda não foram realizados para a identificação e compreensão das peculiaridades de cada condição. Objetivos: Quantificar o desempenho funcional global e a necessidade de assistência do cuidador no autismo infantil (AI) e na síndrome de Asperger (SA); compará-los com os dados normativos de crianças brasileiras e entre os dois grupos; verificar se o teste utilizado (PEDI) pode ser utilizado como um método auxiliar no diagnóstico do AI e SA. Métodos: Estudo transversal, composto por 52 crianças na faixa etária de 3 a 8 anos, diagnosticadas com AI (n=26) e SA (n=26). Utilizado o Inventário de Avaliação Pediátrica de Incapacidade (PEDI) – versão brasileira, através de entrevista estruturada com o cuidador e observação direta. Resultados: Crianças com AI e SA obtiveram escores significativamente menores do que o esperado para a normalidade. Crianças com SA apresentaram função social significativamente melhor do que crianças com AI. No entanto, no AI os escores foram significativamente melhores do que na SA em atividades relacionadas ao autocuidado e mobilidade, necessitando de menor nível de assistência. Conclusão: Crianças com AI e SA possuem déficits em toda a esfera do desenvolvimento comparado a crianças normais. Embora pacientes com SA sejam melhores na interação social que autistas típicos, estes apresentam déficits mais acentuados em tarefas básicas como autocuidado e mobilidade, recebendo maior auxílio nestas áreas do que crianças com AI. As particularidades observadas devem auxiliar na construção do diagnóstico, além de sugerirem foco diferenciado no processo de reabilitação. / Introduction: Childhood autism and Asperger’s syndrome belong to a family of neurodevelopmental disorders called Pervasive Development Disorders. This group is characterized by qualitative abnormalities in reciprocal social interactions and modes of communication as well as a restricted repertoire of stereotyped, repetitive interests and activities. While it is recognized that such patients exhibit impairment in at least three of the aforementioned areas, studies assessing overall functional performance have not been carried out for the identification and comprehension of the peculiarities of each condition. Objectives: The aims of the present study were to 1) quantify the overall functional performance and need for caregiver assistance in childhood autism (CA) and Asperger’s syndrome (AS), 2) compare the findings between groups and to normative data from Brazilian children and 3) determine whether the PEDI test can be used as an auxiliary method for the diagnosis of CA and AS. Methods: A cross-sectional study was carried out involving 52 children between three and eight years of age diagnosed with either CA (n=26) or SA (n=26). The Brazilian version of the Pediatric Evaluation of Disability Inventory (PEDI) was administered in the form of a structured interview with the caregiver and direct observation. Results: The children with CA and AS achieved significantly lower scores than that expected for normality. The children with AS had a significantly better social function than those with CA. However, those with CA achieved significantly better scores than those with AS on activities related to self-care and mobility, requiring less assistance. Conclusion: Children with CA and AS exhibit deficits in all aspects of development in comparison to normal children. While patients with AS are better at social interaction than typical autistic children, they exhibit greater deficits with regard to basic tasks, such as self-care and mobility, requiring greater assistance than children with CA. No. of bitstreams: 0 Previous issue date: 2009-02-18 / A inteligência é a capacidade que o ser humano tem de se adaptar ao meio.As habilidades cognitivas são utilizadas para uma inserção social mais efetiva.O objetivo da presente tese é identificar as habilidades cognitivas promotoras de uma melhor inserção social da criança com Autismo Infantil e Síndrome de Asperger em dois momentos de avaliação,com intervalo de doze meses.Vinte e uma crianças,com idade entre 3 e 12anos e diagnóstico de Autismo Infantil e Síndrome de Asperger,foram avaliadas e suas respectivas mães foram entrevistadas.As crianças estavam inseridas em contexto educacional e/ou sendo atendidas em instituição especializada em Autismo.Doze meses após a primeira avaliação,as mesmas crianças, participaram da segunda avaliação.Em entrevista individual as mães forneceram os dados sobre habilidades adaptativas respondendo a Vineland-VABS(Sparrow,Balla,Cichetti,1984)e sobre características comportamentais respondendo o ABC/ICA(Krug, Arick, Almond, 1993).Cada criança foi avaliada individualmente,por psicóloga experiente na aplicação de testes psicológicos e treinada no teste de nível intelectual Stanford-Binet Intelligence Scale 4ed(Thorndike, Hagen, Sattler,1986).Este instrumento avalia o desempenho intelectual de crianças e fornece uma estimativa do nível cognitivo geral e das habilidades cognitivas nas quatro áreas avaliadas:Raciocínio Verbal, Raciocínio Visual-Abstrato,Raciocínio Quantitativo e Memória de Curto Prazo.Os resultados mostraram que crianças com Autismo Infantil e Síndrome de Asperger apresentaram desempenho inferior no teste de avaliação cognitiva Stanford-Binet(Thorndike et al,1986)quando comparado ao desempenho de crianças e adolescentes inseridos em escola regular(Marteleto,Chiari,Perissinoto,2009).Nas duas avaliações habilidades cognitivas específicas se apresentaram altamente correlacionadas com domínios adaptativos específicos das crianças com AI e SA. Na primeira avaliação as Áreas e Total do Stanford-Binet se correlacionaram com os Domínios de Comunicação, Habilidades de Vida Diária, Socialização e Total da Vineland p<0,05).Doze meses depois houve correlação entre as Áreas e Total do Stanford-Binet com os Domínios Comunicação,Habilidades de Vida Diária,Socialização,Habilidades Motoras e Total da Vineland(p< 0,05).No decorrer de um ano, as habilidades cognitivas envolvidas no Raciocínio Quantitativo e Memória de Curto Prazo promoveram uma melhor inserção social das crianças com AI e SA. / The objective of this dissertation is to identify the cognitive abilities that promote a better child social insertion with Autism and Asperger in a 12 month period. A total of 21 children between 3 and 12 years old, diagnosed as Autism and Asperger, were assessed and their mothers were interviewed. Children were included in regular schools or attending special Autistic schools. A period of 12 months after the first assessment, the same children took part in the second stage of this research. In individual interviews the mothers provided the data about adaptive abilities and behavior characteristics giving answers to the Vineland Adaptive Behavior Scales – VABS (Sparrow, Balla, Cichetti, 1984) and the Autism Behavior Checklist – ABC/ICA (Krug, Arick, Almond, 1993). Each child was individually assessed, by an experienced psychologist, trained in the Stanford-Binet Intelligence Scale 4th Edition (Thorndike, Hagen, Sattler, 1986). This instrument estimates the general cognitive level and the pattern of the cognitive abilities in the four assessed areas: - verbal reasoning, visual abstract reasoning, quantitative reasoning, and short-term memory. The results showed that Autism and Asperger children were presented a worse performance than that of children in regular schools. The two assessments demonstrated that specific cognitive abilities and specific adaptive domains presented high correlations. In the first assessment, the Stanford Binet areas and total were correlated with the Communication domains, Daily life abilities, Socialization and Total score of the Vineland Scale (p < 0,05). One year later, there was a correlation between the Stanford Binet areas and total and the Communication domains, Daily life abilities, Socialization, Motor abilities and total score of the Vineland Scale (p < 0,05). Vivências da paternidade em homens que são pais de um filho com diagnóstico de autismo

The study is composed of three articles. The first was written in the model of an essay theme and discusses, with support in psychoanalysis, the complexity of parenting a child with disabilities in contemporary society. After there are two empirical articles produced with the intention of presenting and discussing the results of a qualitative study, whose general objective was to understand the paternal experiences of men who are parents of a child diagnosed with autism. They were collected from a biopsychosocial understanding, in which we have tried to dialogue with the authors of the psychoanalytic and systemic approaches, in order to understand the many variables involved in phenomenon studied, avoiding reductionism. Study participants were 11 parents who were interviewed with semi-structured interviews. The answers were examined through qualitative content analysis of Bardin (1977). The results, presented in the first empirical paper revealed that the paternity of a child with autism is a complex and challenging experience. The impact of the diagnosis of autism in the lives of parents is intense, they point the desire to be a better father than their fathers, but on the other hand, feel difficulty in exercising the paternity of the child who presents requirements derived from the specific clinical presentation in autism. The feelings frequently are related to a deep sense of powerlessness, overload of responsibilities, loneliness, sadness, and fear of dying and leaving the child helpless as to the needs of care, affection and material resources. Parents stand out as important for the pursuit of parenthood, the need to be patient, information and financial resources.Among the factors that complicate the paternity of this child, parents indicated the more regressive symptoms and, in particular the commitment in the ability to interact social as well as the lack of suitable institutions and little family support. The data presented in the second empirical paper, show that the confirmation of the diagnosis of autism in the child determines changes and losses in social, marital and professional life. Parents attributed the damages, in the three studied dimensions, to the frailty or lack of social support, which also can be related to the expansion of the feelings of loneliness and powerlessness. / Nesta tese, aborda-se o tema da paternidade em homens que tem um filho com diagnóstico de autismo. O estudo está composto por três artigos. O primeiro deles foi escrito no modelo de um ensaio temático, e discute, com apoio na psicanálise, a complexidade da parentalidade de um filho com deficiência na contemporaneidade. Após há dois artigos empíricos produzidos com intuito de apresentar e discutir os resultados de uma pesquisa qualitativa, cujo objetivo geral foi conhecer as vivências paternas de homens que são pais de filho com diagnóstico de autismo. Os dados foram trabalhados a partir de uma compreensão biopsicossocial, na qual se buscou dialogar com autores da abordagem psicanalítica e da sistêmica, com o intuito de compreender as diversas variáveis envolvidas no fenômeno estudado, evitando o reducionismo. Participaram do estudo 11 pais, que foram entrevistados com base em roteiro de entrevista semiestruturado. As respostas foram examinadas por meio da análise de conteúdo qualitativo de Bardin (1977). Os resultados, apresentados no primeiro artigo empírico, revelaram que a paternidade de um filho com autismo é uma experiência complexa e desafiadora. O impacto do diagnóstico de autismo na vida dos pais é intenso, eles apontam o desejo do pai de ser melhor do que o seu, mas, por outro lado, sentem dificuldades no exercício da paternidade do filho que apresenta necessidades especificas oriundas do quadro clínico presente no autismo. Os sentimentos mais frequentes estão relacionados a uma profunda sensação de impotência, sobrecarga de responsabilidades, solidão, tristeza, além de medo de morrer e deixar o filho desamparado quanto às necessidades de cuidado, afeto e recursos materiais.Os pais destacam como importantes para o exercício dessa paternidade, a paciência, informação e recursos financeiros. Entre os fatores que dificultam a paternidade desse filho, os pais apontaram os sintomas mais regressivos e, em especial, o comprometimento na habilidade de interação social, bem como a falta de instituições especializadas e o pouco apoio familiar. Os dados, apresentados no segundo artigo empírico, revelam que a confirmação do diagnóstico de autismo no filho determina mudanças e prejuízos na vida social, conjugal e profissional. Os pais atribuem os danos, nas três dimensões estudadas, à fragilidade ou inexistência de suporte social, o que, também, pode ser relacionado à ampliação dos sentimentos de solidão e impotência.

Las representaciones incluyen diversos elementos como sensaciones, pensamientos, percepciones, motivaciones, y elementos del contexto donde se encuentra la madre y acciones que se construyen a partir de la interacción con otro (Stern, 1997).

Dificuldades comunicativas percebidas por pais e/ou cuidadores de crianças do espectro do autismo: um questionário de levantamentos / Communication difficulties perceived by parents/caregivers of children of the autism spectrum: a survey questionnaire

As alterações comunicativas têm sido descritas em muitas pesquisas como sendo uma das primeiras preocupações dos pais de crianças do espectro do autismo, além de serem consideradas parâmetro de diagnóstico e indicações de intervenções terapêuticas. O objetivo deste estudo foi, a partir da percepção dos pais e/ou cuidadores, buscar compreender as variáveis relevantes quanto às dificuldades comunicativas percebidas por eles. Considera-se que a interpretação e a atitude dos pais frente aos comportamentos, não somente das crianças, mas também das outras pessoas, tenha influência no processo comunicativo da díade. O questionário envolve aspectos fundamentais para o relacionamento interpessoal, tanto no âmbito comunicativo quanto social. Ele é dividido em 24 questões fechadas que abrangem quatro domínios (impressão dos pais sobre si mesmos, em relação às outras pessoas, em relação a seus filhos e atitudes dos pais em relação a seus filhos) e uma questão aberta. Neste estudo o questionário foi aplicado a um grande grupo de pais e/ou cuidadores em diferentes regiões do Brasil e as variáveis sociodemográficas consideradas foram idade da criança, posição entre outros irmãos; idade e nível de escolaridade dos cuidadores. Os resultados foram estatisticamente analisados e indicaram que, de uma forma geral, os dados sociodemográficos não apresentaram relação significativa com as respostas obtidas no questionário. Em relação aos domínios, a impressão dos pais a respeito das reações das outras pessoas às manifestações de seus filhos originou a maior parte das dificuldades relatadas. Três questões específicas destacaram-se como aquelas em que houve um volume significativo de concordâncias: elas dizem respeito à dificuldade de comunicação das outras pessoas com a criança, hábitos de comunicação da díade e preocupação com o futuro e a falta de informações a respeito do tema. O tratamento dos dados possibilitou identificar as dificuldades recorrentes apontadas pelos cuidadores. Caberá ao fonoaudiólogo usar essas informações e, ao mesmo tempo, valorizar o há de individual e peculiar a cada família nos processos de orientação. / Communication disorders have been described in several studies as one of the first worries of autism spectrum children\'s parents and also as a diagnostic and therapeutic intervention parameter. The speech-language pathologist should use this information and, at the same time consider aspects that are individual and peculiar to each dyad during the orientation processes.

En la actualidad, se evidencia que cada vez es mayor la posibilidad de contar con algún estudiante de inclusión en las aulas de Educación Básica Regular, incluyendo en ello a quienes poseen Trastorno de Espectro Autista (TEA); no obstante, en muchos casos estos no se atienden de la manera más adecuada a causa de la deficiencia o carencia en la preparación de los docentes, principales encargados de fomentar a inclusión, y se tiende a recaer en la exclusión, segregación de estudiantes mal considerados como “retardantes” en el proceso de aprendizaje de sus pares, llegando a limitar las oportunidades y potencial de aprendizaje y desarrollo. De esto se desprende el objetivo general de la presente tesina, el cual es analizar la contribución de las prácticas educativas para fomentar la inclusión de estudiantes con TEA, y la metodología empleada es la del trabajo documental, la cual se desarrolla principalmente partiendo por conceptualizar y caracterizar propiamente el Trastorno de espectro autista, abarcando a qué hace referencia, su clasificación, proceso de diagnóstico, características y necesidades del estudiante que lo posee. Luego se procede a describir los factores que favorecen a la inclusión de los estudiantes con TEA, desarrollando inicialmente el marco legal y normativo de la inclusión de quienes lo poseen, la función y actores que intervienen para la inclusión y finalmente la intervención en el aula propiamente dicha. De dicho desarrollo se concluye, principalmente, que se debe dar un trabajo entre docentes, padres y el entorno, debido a que ello permite realizar un trabajo coordinado, que se complemente con miras a favorecer el desarrollo de los estudiantes con TEA, considerando sus características y necesidades particulares a cada caso.

Experiencia emocional en hermanos adolescentes de niños con trastorno del espectro autista

La presente investigación busca explorar la vivencia de los hermanos de niños con Trastorno del Espectro Autista durante una etapa tan crucial en la vida de cada persona como es la adolescencia. Para ello, se realizó un estudio de casos con un análisis cualitativo de los mismos. Se entrevistó a 4 adolescentes y a sus respectivos padres en un total de tres entrevistas, una con uno de los padres y dos con el adolescente. Para la recolección de información, se utilizó una ficha de datos sociodemográficos, una guía de entrevista y un instrumento de frases incompletas que fue aplicada durante la segunda entrevista con el adolescente. Los resultados muestran que los adolescentes tienden a sentir emociones de ira, de tristeza y de culpa, pero también de cariño, alegría y esperanza. Se plantea que una comunicación adecuada con los padres y una evolución favorable del hermano con TEA en las terapias favorece en el adolescente un manejo afectivo más saludable. / This research aims to explore the experience of siblings of children with ASD during such a crucial stage in the life of every person as is adolescence. This was a case study with a qualitative analysis of the same. Four teens were interviewed and their parents in a total of three interviews, one with a parent and two with the teenagers. For data collection was used, a sociodemographic data sheet, an interview guide and an instrument of incomplete sentences that was applied during the second interview with the teen. The results show that teenagers tend to feel emotions of anger, sadness and guilt, but also of love, happiness and hope. It is argued that adequate communication with parents and favorable evolution of the brother with ASD in therapy promotes healthier emotional management for the adolescent.

Estrés parental y estilos de afrontamiento en padres de niños con trastornos del espectro autista

El objetivo de esta investigación es describir y analizar la relación entre los niveles de estrés parental y los estilos/estrategias de afrontamiento en padres de niños y niñas con trastorno del espectro autista (TEA), así como su relación según ciertos datos sociodemográficos. La muestra estuvo conformada por 58 padres de familia cuyos hijos acudían a uno de los cinco módulos de una institución para el desarrollo infantil. Ellos completaron un cuestionario para medir el estrés parental (PSI-SF) y otro para identificar los estilos y estrategias de afrontamiento más empleados (COPE). Se encontraron altos niveles de estrés parental tanto en la escala total como en sus tres dominios. El dominio malestar paterno correlacionó de manera positiva con las estrategias supresión de actividades competentes, desenganche conductual y negación; mientras que la correlación fue negativa con la estrategia búsqueda de apoyo social por razones emocionales. Por otro lado, el dominio interacción disfuncional padres-hijo correlacionó positivamente con la estrategia desenganche conductual y de manera negativa con la estrategia aceptación. Finalmente, el dominio estrés derivado del cuidado del niño se observó una correlación positiva con la estrategia desenganche conductual y negación, pero una correlación negativa con búsqueda de apoyo social por razones instrumentales. Finalmente, se encontraron diferencias significativas tanto en los niveles de estrés parental como en los estilos de afrontamiento según el módulo al que asisten los participantes. Así quienes acuden a San Juan de Miraflores reportan menores niveles de estrés parental y emplean en mayor medida los estilos de afrontamiento orientados a la emoción y al problema al ser comparados con los otros cuatro módulos. / The objective of this research is to describe and analyze the relationship between parental stress levels and styles/strategies of coping in parents of children with autism spectrum disorder (ASD) and their relationship according to some sociodemographic data. The sample consisted of 58 parents whose children went to one of the five modules of an institution for child development. They completed a questionnaire to measure parental stress (PSI-SF) and another to identify the coping styles and strategies more employees (COPE). We found high levels of parental stress in both the total scale and its three domains. The parental distress domain correlated positively with suppression strategies relevant activities, behavioral disengagement and denial, while the correlation was negative with the strategy seeking social support for emotional reasons. On the other hand, the domain dysfunctional parent-child interaction positively correlated with behavioral disengagement strategy and negatively with the acceptance strategy. Finally, the domain related stress of child care was a positive correlation with behavioral disengagement and denial strategy, but negatively correlated with seeking social support for instrumental reasons. Finally, we found significant differences in the levels of parental stress and coping styles according to the module that the participants were attending. So those who go to San Juan de Miraflores reported lower levels of parental stress and further used coping styles focused on the emotion and the problem when compared with the other four modules.
