Objetivos: comparar pacientes com estenose do canal lombar com indicação de cirurgia e um grupo controle, assintomáticos para esta doença, levando em consideração imagens de ressonância magnética (RM) e o questionário de Oswestry. Material e Método: Vinte e três pacientes com estenose do canal lombar foram comparados com um grupo controle de 17 indivíduos. Todos foram submetidos a RM e responderam ao questionário de Oswestry. Medidas qualitativas foram descritas segundo os grupos com uso de frequências absolutas e relativas e, verificada a existência de associação dessas medidas com uso do teste exato de Fisher. Medidas quantitativas foram descritas e comparadas entre os grupos com uso do teste Mann-Whitney. O teste de Spearman foi utilizado para avaliar a correlação entre os grupos. Resultados: Lombalgia foi a queixa mais frequente nos dois grupos. O índice de Oswestry mostrou média de percentual de invalidez de 45,69% no Grupo Estenose e 11,60% no Grupo Controle. A RM mostrou que a área de secção transversa do saco dural, o diâmetro do canal e a avaliação dos forames e recesso lateral estavam alterados em ambos os grupos. Conclusões: Não houve relação entre o grau de estenose do canal lombar mensurado em exame de RM com o índice de Oswestry em ambos os grupos / Abstract: Study design: Transversal Case-control, diagnostic study. Objectives: To verify the relationship between the degree of lumbar spinal canal stenosis (LCS), as seen in magnetic resonance imaging (MRI), and the severity of disability as seen in the Oswestry Index, in patients with LCS compared to controls without a diagnosed LCS. Methods: Twenty-three patients with a previous diagnosis of LCS were compared with a control group of 17 volunteers. All participants underwent MRI and answered the Oswestry questionnaire. Qualitative data were described according to the groups with the use of absolute and relative frequencies, and the association of these measures was checked by using the Fisher exact test. Quantitative measures were described and compared between groups using the Mann-Whitney test. Spearman's test was used as well. Results: Low back pain was the most frequent complaint in both groups. The Oswestry index showed average percentage of disability of 45.69% in patients with stenosis and 11.60% in the control group. MRI revealed that the dural sac cross-sectional area (DCSA), the diameter of the canal and the evaluation of lateral recesses and foramina were equally changed in both groups. Objetivos: avaliar a prevalência das algias na coluna vertebral, identificar possíveis fatores de risco na gestação e avaliar a efetividade do método do Hatha Yoga para reduzir a PLBP e PGP relacionadas à gravidez. Métodos: a dor na coluna vertebral foi identificada através de estudo de corte transversal com 245 gestantes em atenção pré-natal selecionadas em Unidades Básicas de Saúde do município de Paulínia (Estado de São Paulo), através de entrevistas usando-se questionário estruturado pelos autores. Também foram convidadas 60 grávidas com PLBP e/ou PGP, para participar de um ensaio clínico prospectivo randomizado. Participaram gestantes com idade de 14 a 40 anos, idade gestacional de 4 a 40 semanas, no estudo de prevalência, e de 12 a 32 semanas para o ensaio clínico. Foram excuídas as grávidas submetidas a algum tipo de intervenção para o tratamento destas algias. As pacientes foram divididas em dois grupos: o Grupo YOGA (GY), que praticou exercícios orientados pelo método, e o Grupo Orientação Postural (GOP), que realizou orientações posturais padronizadas contidas em folheto preparado por fisioterapeutas. As gestantes foram acompanhadas por dez semanas, no período de junho 2009 a junho 2011. Foram utilizados a Escala Analógica Visual (EAV) para medir a intensidade da dor e os testes de provocação de dores lombar e pélvica posterior para verificação da presença da dor. Para análise estatística foram utilizados analise descritiva por medidas de posição e tendência central, frequência absoluta e relativa, testes de Mann-Whitney, Qui-quadrado de Pearson, Mc Nemar, Wilcoxon pareado, Análise de covariância, Regressão logística binária múltipla e Método Stepwise. Resultados: a prevalência de dor nas costas foi de 78,8%, sendo a região lombopélvica referida por 69%. A intensidade da dor foi relatada por 35,9% para a dor moderada e 32,7% para a dor severa. A dor lombopélvica foi 11,16 mais frequente para o período noturno e 4,03 para o vespertino e a postura corporal em pé foi 2,9, comparativamente às que não sentiam dor . Observou-se que a mediana do escore de dor referido pelas gestantes foi menor no GY (p< 0,0058) quando comparada ao GOP. Houve diminuição da reatividade nos testes de provocação de dor lombar em relação aos testes de dor pélvica posterior. Observou-se a redução gradativa da intensidade da dor durante as dez sessões do Hatha Yoga programadas (p< 0,024). Conclusão: A prevalência de dor na coluna vertebral foi de 78,8% e a região lombopélvica foi a mais referida. O modelo de regressão logística apontou que os períodos noturnos, vespertinos e a posição corporal em pé são fatores preditivos associados para a dor lombopélvica durante a gestação. O método do Yoga foi mais efetivo para a diminuição da intensidade da PLBP e PGP quando comparado ao grupo de mulheres submetidas às orientações posturais / Abstract: Introduction: The back pain are symptoms that affect eight out of ten pregnant Brazilian women, and the use of Yoga for treatment and care have been considered. Objectives: To evaluate the prevalence of pain in the spine, to identify possible risk factors in Brazilian pregnant women and evaluate the effectiveness of the method Hatha Yoga to reduce pregnancy-related low back pain (PLBP) and pregnancy related pelvic girdle pain (PGP). Methods: pain in the spine was identified through cross-sectional study with 245 pregnant women in prenatal care selected in the Primary Healthcare Services in Paulínia (São Paulo State Brazil). Interviews using a structured questionnaire was used. Also were invited 60 pregnant with PLBP and / or PGP, to participate in a prospective randomized clinical trial, age 14 to 40 years and 4 to 40 gestacional age for prevelence and 12 to 32 gestacional age for the clinical trial. We excluded pregnant women who were being subjected to some type of intervention to treat these pains. The women were divided into two groups: Yoga Group (YG), who practiced exercises guided by the method, and the Postural Orientation Group (POG) that performed, postural standardized guidelines contained in booklet prepared by physical therapists. The women were followed for ten weeks, from June 2009 Summary to June 2011. We used the Visual Analogue Scale (VAS) to measure the intensity of pain and provocation tests to verify the presence of the low back pain and posterior pelvic pain. Statistical analysis included measures of central tendency and position, absolute and relative frequency. Mann-Whitney, Chi-square, McNemar, Wilcoxon Tests, analysis of covariance, Multiple binary logistic regression and stepwise method .Results: The prevalence of back pain was 78.8% and the lumbopelvic region was reported by 69%. The pain intensity was 35.9% for moderate and 32.7% for severe pain. The lumbopelvic pain was more prevalent 11.16 during night time, 4.03 during evening and standing posture was found 2.9 more frequent compared to those who felt no pain. Was also observed that the median pain score was lower in YG (p <0.0058) when compared to the POG. There was relief of the pain during low back pain provocation tests in relation to posterior pelvic pain tests. There was a gradual reduction of pain intensity during the ten sessions scheduled of Hatha Yoga (p <0.024). Conclusion: The prevalence of pain in the spine was 78.8% and lumbopelvic region was the most reported. The logistic regression model showed that the night-time, evening and standing position are predictive factors associated to lumbopelvic pain during pregnancy. Prévalence, mesures et méthodes alternatives de traitement de la douleur dorsale chez les adolescents ayant une scoliose idiopathique

La scoliose idiopathique de l’adolescence constitue la déformation tridimensionnelle de la colonne vertébrale la plus couramment rencontrée chez les adolescents. Cette condition a fait l’objet de plusieurs études quant à ses facteurs étiologiques, ses facteurs de risque de progression et ses approches thérapeutiques. Les études démontrent que l’étiologie de cette pathologie serait multifactorielle, regroupant entre autres des causes génétiques, hormonales, mécaniques et neuromusculaires. Malgré le fait qu’il soit difficile de prédire quelles sont les déformations qui progresseront, certains facteurs tels que l’âge et le sexe du patient, l’âge du début de la ménarche, le type et la sévérité de la déformation ainsi que la maturité osseuse (signe de Risser) ont fait l’objet de nombreuses études et représentent par le fait même des critères de référence utilisés pour estimer cette probabilité. La scoliose idiopathique a souvent été considérée comme une condition non douloureuse, faisant en sorte que peu d’études se sont arrêtées à évaluer les douleurs au dos chez les patients affligés de cette pathologie. On retrouve donc, au cours des vingt dernières années, seulement une dizaine d’études qui ont spécifiquement évalué ce phénomène. Parmi ces études, on note une grande hétérogénéité des facteurs d’inclusion, des définitions des douleurs au dos, de la période de prévalence et des instruments de mesure utilisés, rendant difficile la détermination de cette prévalence. Considérant la relation entre les douleurs exprimées chez les adolescents et celles chez les adultes, ainsi que les coûts sociétaux associés à ces douleurs, il serait important d’obtenir un portrait plus exact de l’ampleur de la douleur chez cette clientèle. ii Les objectifs généraux de cette thèse visaient à (objectif 1) mettre à jour les connaissances relatives à la prévalence des douleurs au dos chez les adolescents avec une scoliose idiopathique (SI), à (objectif 2) vérifier s’il existe une relation entre la sévérité de la déformation, sa localisation et la présence de douleurs au dos chez ces patients, à (objectif 3) valider à nouveau l’aspect psychométrique du questionnaire SRS-22fv et enfin à (objectif 4) explorer une possible prise en charge alternative des patients avec scoliose idiopathique de l’adolescence (SIA) exprimant des rachialgies. La première étude rétrospective que nous avons effectuée a permis d’évaluer de quelle façon les douleurs exprimées par plus de 300 patients étaient rapportées dans les dossiers et d’établir la prévalence de ces douleurs chez ces derniers. De plus, cette étude a évalué le type de prise en charge recommandée lorsque des douleurs étaient mentionnées au dossier. Cette étude a permis de confirmer que près de 50 % des patients avec SIA exprimaient de la douleur au dos et que la majorité de ces derniers (80 %) n’avaient aucune prise en charge de celle-ci. La deuxième étude prospective que nous avons réalisée a évalué les douleurs au dos chez plus de 500 adolescents avec scoliose idiopathique de l’adolescence. Plus de 68 % de ces patients rapportaient la présence de douleurs au dos. Pour les régions thoraciques et lombaires, les douleurs étaient positivement associées avec la sévérité de la scoliose, alors que le port du corset avait un effet modérateur sur la douleur. La troisième étude incluse dans la présente thèse visait à (objectif 3) vérifier la validité de contenu et de construit de la version canadienne-française du questionnaire SRS- 22, soit le SRS-22fv, rempli par les patients recrutés lors de la deuxième étude. Ce iii questionnaire est couramment rempli par les patients scoliotiques. Notre étude a bénéficié d’un échantillon de 352 patients qui ont entièrement répondu au questionnaire SRS-22fv. L’analyse découlant de notre étude nous a menés à produire une version abrégée du SRS-22 contenant 18 éléments, avec une meilleure consistance interne ainsi qu’une variance explicative supérieure, soit 63,3 % pour le SRS-18fv, comparativement à 47,4 % pour le SRS-22fv. La dernière étude que nous avons menée (objectif 4) met en lumière une option de prise en charge alternative des patients avec scoliose exprimant ou non des douleurs au dos. En effet, les manipulations vertébrales font partie des options de traitements couramment utilisées chez les adolescents sains. Une grande incertitude demeure toutefois quant à l’utilisation de cette approche thérapeutique chez les patients avec scoliose idiopathique de l’adolescence. Cette étude a permis de confirmer qu’il y a un manque de données probantes dans ce domaine. Les résultats de cette thèse permettent de mieux comprendre le phénomène de la douleur au dos chez les adolescents avec scoliose idiopathique. Ces douleurs semblent plus prévalentes qu’initialement estimées, et il s’avère qu’un lien semble probable entre l’intensité des douleurs ressenties et la localisation de la courbe, ainsi qu’avec la sévérité de la scoliose chez ces adolescents. / Adolescent idiopathic scoliosis is a three-dimensional spinal deformity. This pathology is the most common spinal deviation encountered in adolescents and has been thoroughly researched over the past few decades regarding its aetiological factors, its risk of progression and management. It is now commonly accepted that the aetiology of adolescent idiopathic scoliosis is multifactorial and includes, among other things, genetic, hormonal, mechanic, and neuromuscular factors. Though predicting which deviation will progress remain challenging, certain factors such as the age of the patient at presentation, its gender, bone maturity (Risser sign), menarchal status, and the severity of the spinal deformity are now widely accepted as factors that may provide some guidance to estimate this probability. Adolescent idiopathic scoliosis has often been considered as a non-painful pathology, and this is perhaps why researchers have failed to assess back pain in this population more systematically. Only a few studies have been published over the last two decades on this subject. Among those studies, inclusion criteria are widely heterogeneous, as is the working definition of back pain, how the prevalence period was estimated, and which outcome measure was relied upon to assess back pain in this population. Because of this, the determination of back pain prevalence in scoliosis patients remains challenging. Considering the relationship between adolescent and adult spinal pain, and the high societal cost associated with this condition, it would be appropriate to obtain a clearer picture of this condition in this population. v The primary objectives of this thesis were to (objectif 1) update the current knowledge of back pain prevalence in adolescent idiopathic scoliosis patients, to (objectif 2) ascertain if a relationship was present between the severity and localisation of the scoliosis and the back pain, too (objectif 3) reassess the psychometric properties of the SRS-22fv questionnaire, and to (objectif 4) explore an alternative back pain management for those patients. The first retrospective study included in this thesis assessed back pain prevalence in 300 adolescent idiopathic scoliosis patients. It also looked at how pain was reported and if pain management was recommended for patients reporting back pain. This study confirmed that back pain was a prevalent condition in this population and that in the vast majority of those reporting back pain, there was no kind of management recommended. The second prospective study assessed back pain prevalence in more than 500 adolescent idiopathic scoliosis patients. Point prevalence of back pain was close to 68 %. For the thoracic and lumbar regions, the pain was positively associated with the severity of the scoliosis and bracing, prescribed to stabilise or prevent the curve progression, had a protective effect. Data collected in the previous study were employed to validate the most commonly utilised outcome measure in adolescent idiopathic scoliosis, the SRS-22. The SRS-22 is a measurement instrument used to ascertain the quality of life and pain in scoliotic patients. The third article includes a report on analyses done regarding the validation of the SRS- 22fv. Complete data were available for 352 adolescents. The study demonstrated that a briefer version (18 items) had a better internal consistency and explained a greater proportion of the variance thus 63,3 % instead of 47,4 % for the SRS-22fv. vi The last study included in this thesis looked at an alternative treatment for the management scoliotic patients with or without back pain. This modality of spinal manipulation is a treatment that healthy adolescents commonly utilised. However, it was found that the rate of utilisation remains unclear within adolescent idiopathic scoliosis patients. A literature review revealed the need to have better-designed studies to assess the efficacy of spinal manipulation in this population adequately. The acquired knowledge throughout this thesis leads us to a better comprehension and understanding of back pain in adolescent idiopathic scoliosis patients. Back pain appears to be more prevalent than initially estimated and a relationship seems to be present between the level of pain intensity and the scoliosis localisation and severity in patients suffering from this pathology.

Degenerative findings on MRI of the lumbar spine:prevalence, environmental determinants and association with low back symptoms

Abstract Earlier studies on lumbar degenerative imaging findings in magnetic resonance imaging (MRI) have been done mainly in adult populations and the associations of degenerative findings with low back pain (LBP) are controversial. Only a few studies have involved adolescents and young adults. Heritability has been acknowledged as the only explicit risk factor of disc degeneration (DD). This study investigated the prevalence and environmental determinant of lumbar degenerative findings in MRI and their association with low back symptoms among young adults. The data were based on two physical assessments, three questionnaires and one lumbar MRI that were executed on members of the Oulu Back Study (n=558), a subsample of Northern Finland Birth Cohort 1986, between 16 and 21 years of age. Prevalences of lumbar DD (54%), bulging (25%), protrusion (18%) and Schmorl’s node (17%) were high, whereas other degenerative findings were rare among young adults. Males had higher prevalence of DD and Schmorl’s nodes than females. DD and herniations were associated with low back symptoms. On the other hand, symptoms were present among subjects without DD or other abnormal findings on MRI. Of the environmental determinants, high body mass index and MRI-based obesity measurements of visceral adiposity were associated with lumbar DD among males. Waist circumference and smoking showed a comparable association with DD among males, but the level of physical activity was not associated with DD in either gender. Low back symptoms are more common among young adults with a higher degree of DD or presence of disc herniation. Smoking and overweight are associated with lumbar DD among young male adults. / Tiivistelmä Aikaisempia tutkimuksia magneettikuvantamisella (MK) todetuista lannerangan rappeumamuutoksista ja niiden yhteyksistä alaselkäkipuun on tehty lähinnä aikuisväestöllä ja tulokset ovat ristiriitaisia. Vain muutamia tutkimuksia on tehty alle 25-vuotiailla. Välilevyrappeuman mahdollisista riskitekijöistä vain perimästä on vahvaa näyttöä. Tässä tutkimuksessa tarkasteltiin MK:lla todettujen lannerangan rappeumamuutosten esiintyvyyttä, niihin vaikuttavia ympäristötekijöitä ja yhteyttä alaselkäoireisiin nuorilla aikuisilla. Tutkimuksen aineisto perustui kahteen kliiniseen tutkimukseen, kolmeen kyselyyn ja yhteen MK:een, jotka toteutettiin Pohjois-Suomen syntymäkohortti 1986:een kuuluville Oulun selkätutkimuksen koehenkilöille (n=558) 16-21 vuoden iässä. Lannerangan välilevyrappeumalla (54 %), välilevyn pullotuksilla (bulge; 25 %), sellaisilla välilevyn pullistumilla jotka eivät läpäisseet selkärangan takimmaista pitkittäissidettä (protruusio; 18 %) sekä päätelevyn läpi suuntautuvilla välilevyn pullistumilla (Schmorlin keräset; 17 %) oli korkea esiintyvyys nuorilla aikuisilla, kun taas muut kuvantamislöydökset olivat harvinaisia. Välilevyrappeuma ja Schmorlin keräset olivat yleisempiä miehillä. Sekä välilevyrappeuma että pullistumat olivat yhteydessä alaselkäoireisiin molemmilla sukupuolilla, mutta kaikilla oireisilla ei todettu poikkeavia löydöksiä MK:ssa. Ympäristötekijöistä korkea kehon painoindeksi ja MK:sta mitatut rasvan määrää mittaavat muuttujat olivat miehillä yhteydessä välilevyrappeumaan. Miehillä vyötärönympärys ja tupakointi olivat heikommin yhteydessä välilevyrappeumaan, kun taas liikunta-aktiivisuus ei ollut kummallakaan sukupuolella yhteydessä rappeumaan. Alaselkäoireet ovat yleisempiä nuorilla aikuisilla, joilla on vaikea-asteisempi välilevyrappeuma tai välilevyn pullistuma. Tupakointi ja ylipaino ovat yhteydessä lannerangan välilevyrappeumaan nuorilla aikuisilla miehillä.

Low back pain and associated factors in adolescence:a cohort study

Abstract Low back pain (LBP) is a common condition already in adolescence and seems to predict future symptoms. Most of the previous longitudinal studies on LBP have been conducted in adult populations. Among adolescents, LBP has been linked most convincingly to psychosocial factors and smoking. We are not aware of any previous studies on clustering of potential risk factors in adolescent LBP. The main objective of this study was to analyze the potential associations of mainly modifiable psychosocial and lifestyle factors with LBP in adolescence. The factors evaluated were overweight, smoking, physical workload, family’s socioeconomic status, and the co-occurrence of psychosocial and lifestyle factors. The present study used the database of the 16- to 18-year-old adolescent subcohort, Oulu Back Study (n=1,987), of the Northern Finland Birth Cohort 1986. Moreover, we evaluated whether the associations differ depending on the presence of LBP symptoms at baseline of the two-year follow-up period. LBP was common among cohort adolescents as, on average, every second adolescent reported LBP, girls reporting more symptoms. As a whole, more associations were found among girls than boys. Overweight was associated with new LBP in the follow-up, but the association was not strong. Prolonged habitual smoking was associated with LBP, particularly with symptoms at baseline and follow-up. High exposure to awkward trunk postures and an overall physically demanding job were especially associated with new LBP in the follow-up. Family’s socioeconomic status was not consistently associated with LBP. At 16 years, four latent class clusters of psychosocial and lifestyle factors were found in both genders. Clusters characterized by behavioural problems were associated with LBP among both genders. Especially girls with the combination of emotional and behavioural problems were at the greatest risk of seeking care for new LBP. The results of this study indicate that LBP in adolescence is associated with both psychosocial and lifestyle factors. The information obtained may be utilized for improving preventive measures and individual care for adolescent LBP. / Tiivistelmä Alaselkäkipu on yleistä jo nuorilla, ja varhain koetut oireet ennustavat myöhempiä selkäkipuoireita. Suurin osa alaselkäkivun seurantatutkimuksista on tehty aikuisväestöllä. Nuorten alaselkäkivun on esitetty vakuuttavimmin liittyvän psykososiaalisiin ongelmiin ja tupakointiin. Alaselkäkipuun liittyvien tekijöiden mahdollista kasautumista ei ole nuorilla tutkittu aiemmin. Tämän väitöstutkimuksen tarkoituksena oli selvittää, ovatko pääasiassa muunneltavissa olevat psykososiaaliset ja elämäntapatekijät yhteydessä nuoruudessa esiintyvään alaselkäkipuun. Arvioitavina tekijöinä olivat ylipaino, tupakointi, fyysinen työkuormitus, sosioekonomiset tekijät, sekä psykososiaalisten ja elämäntapatekijöiden kasautuminen. Tutkimusaineisto koostui Pohjois-Suomen syntymäkohortti 1986:n osaotoksesta (Oulun selkätutkimus), johon kuului 1987 16–18-vuotiasta nuorta. Mahdollisia yhteyksiä arvioitiin erikseen sen mukaan, raportoivatko nuoret seurannan alussa alaselkäkipuoireita vai ei. Alaselkäkipu oli nuorilla yleistä, ja tytöillä yleisempää kuin pojilla. Keskimäärin joka toinen nuori raportoi oireita. Tutkittujen tekijöiden ja alaselkäkivun välisiä yhteyksiä todettiin selvemmin tytöillä. Ylipaino ennusti seurannassa ilmaantuvaa alaselkäkipua, mutta yhteys ei ollut vahva. Säännöllinen tupakointi oli yhteydessä alaselkäkipuun ja etenkin toistuviin oireisiin. Tutkituista työkuormitustekijöistä etenkin hankalat työasennot ja fyysisesti monella tapaa raskaaksi luokiteltava työ altistivat seurannassa uudelle alaselkäkivulle. Sosioekonomiset tekijät eivät olleet selkeästi yhteydessä alaselkäkipuun. Latenttien luokkien analyysi tunnisti seurannan alkuvaiheessa neljä erilaista ryhmää (klusteria) psykososiaalisten ja elämäntapatekijöiden perusteella molemmilla sukupuolilla. Sekä tytöillä että pojilla alaselkäkipua esiintyi erityisesti niissä ryhmissä, joissa esiintyi runsaasti käyttäytymishäiriöitä. Tunne-elämän ja käyttäytymisen ongelmat ennustivat alaselkäkivun vuoksi hoitoon hakeutumista tytöillä. Tulosten perusteella nuorten alaselkäkipuun liittyy sekä psykososiaalisia että elämäntapatekijöitä. Tuloksia voidaan hyödyntää alaselkäkivun ennaltaehkäisyn ja nuorten yksilöllisen hoidon suunnittelussa.

Multidimensional manual therapy model for managing patients with chronic non-specific low back pain

Low back pain (LBP) is regarded as a major health and economic problem in western industrialised countries even at this time in the twenty-first century. Researchers estimate that it has increased to affect about 45% of the population in 2011. This increase creates a major burden on the health care services, social structures and the economy in terms of absenteeism from work. CNSLBP is still poorly understood. Main reasons for the poor understanding of CNSLBP discussed in this study are the limited understanding of the effect of the spine as kinetic chain which includes the head and pelvic girdle and with its attachments to the scapulae . The process of development of ISMS dysfunction are discussed as a combination of abnormal spinal loading, soft and neural tissue plasticity that result in biomechanical malalignment, adaptive and maladaptive movement patterns, pain processing integrated with psychosocial factors that influence the biomechanical, pain processing and psychological responses are discussed as possible mechanisms in the development of CNSLBP. The researcher developed a multidimensional manual therapy model to manage patients with CNSLBP based on metacognitive reflection on her clinical reasoning over a period of 40 years as the research methodology. The metacognitive reflection has been performed within the interpretive paradigm The model that resulted from the metacognitive reflection is dialectic in nature because it entails the understanding of the patient‘s problem from an interpretive as well as from an empirico-analytical perspective. The model is conceptualised in three stages: Firstly the conceptualisation of the integrated spinal movement system (ISMS), to indicate that the spine, head, shoulder and pelvic girdles function as a closed kinematic chain. Secondly the process of the development of ISMS dysfunction as a major concept in the clinical picture of patients with CNSLBP is based on functional anatomy of the ISMS and the researcher‘s clinical observation in clinical practice. The researcher indicates how the development of ISMS dysfunction and characteristic adaptive behaviour are integrated components of the patient‘s complex heterogenic clinical picture. The underlying process for the development of ISMS dysfunction as a possible mechanism for CNSLBP is described as plasticity of soft and neural tissues (including the brain) which result in chronicity over time. Thirdly a multidimensional manual therapy model to manage patients with CNSLBP‘s heterogenic condition is discussed. The model indicates how the mechanisms underlying the development of ISMS dysfunction is addressed in a multidimensional approach to patient management. Finally the multidimensional manual therapy model is discussed in relation to other relevant intervention approaches. The model finally serves as a point of departure for planning and conducting appropriate research in basic and clinical sciences. Caractérisation mécanique des orthèses : Application aux ceintures de soutien lombaire dans le cadre de la lombalgie / Mechanical effects of orthosis : Application to lumbar belts for the treatment of low back pain

La lombalgie est une douleur de la région lombaire du rachis. Un des moyens communément utilisés pour soulager cette douleur est le port d’orthèses lombaires. Bien que l’efficacité de ces orthèses soit démontrée, leur mode d’action reste mal connu. L’objectif de ce projet est d’analyser ce mode d’action dans le cas de ceintures de soutien lombaire.En lien avec des médecins et un industriel, trois approches ont été mises en œuvre. Premièrement, une approche expérimentale a permis de déterminer les propriétés mécaniques des ceintures de soutien lombaire et d’évaluer la pression appliquée suivant ces propriétés. Secondairement, la pression appliquée par les ceintures a été mesurée par une approche clinique. Cette pression a été liée à la diminution de la douleur engendrée par les ceintures, au bien-être ressenti, à la modification de la posture et à la déformation de la ceinture. Troisièmement, une approche numérique a servi à montrer les paramètres influents sur le soulagement de la douleur.Un des modes d’action des ceintures de soutien lombaire, mis en évidence dans ce projet, est la pression appliquée sur le tronc qui engendre une variation des pressions abdominale et intradiscale, ainsi qu’une modification de la posture. La pression appliquée dépend, entre autres, des caractéristiques mécaniques des ceintures. Les résultats mettent en évidence l’importance de maitriser cette pression appliquée ainsi que la spécificité du patient sur les effets mécaniques des ceintures.Les différentes approches sont complémentaires ; elles sont des moyens de caractérisation des orthèses qui pourraient être utilisés pour d’autres orthèses et également aider au développement de produits. / Low back pain is a pain felt in the lumbar region of the spine. Lumbar orthotics might be used to relieve this pain. Efficacy of lumbar orthotics was proved, but mechanical effects remain unclear. Aim of this project if to analyze these mechanical effects for lumbar belts, a specific lumbar orthotic.In relation with clinicians and manufacturer, three different approaches were developed. First, experimental approach permits to determine mechanical properties of lumbar belts and to evaluate applied pressure related to these mechanical properties. Second, applied pressure by lumbar belts was measured using a clinical approach. This pressure has been related to the decrease in pain, to the wellbeing felt, to the modification of posture and to the lumbar belts strain. Third, numerical approach was used to demonstrate the influential parameters on pain relief.One of mechanical effects of lumbar belts, highlighted in this project, is the pressure applied on the trunk which causes change in abdominal and intradiscal pressure and posture. The applied pressure depends on the mechanical properties of the belts. Results demonstrate the importance of mastering the applied pressure and the specificity of the patient on the mechanical effects of belts. The different approaches are complementary; they are orthotics characterization tools that could be used for others orthotics and also helps in the development of new products.

Work-related low back pain among clinical nurses in Tanzania

Strategies to reducing the incidences of LBP in nurses have been previously implemented but with little outcomes. The purpose of this study was to explore the relationship between occupational risk factors and the prevalence of LBP in nurses at MOI, Tanzania. Self-assembling peptide hydrogel for intervertebral disc tissue engineering

The intervertebral disc (IVD), situated between adjoining vertebrae, consists of the gelatinous nucleus pulposus (NP) in the centre surrounded by the tougher annulus fibrosus (AF). Its main roles are to distribute loads and to act as joints. With aging, degenerative disc disease (DDD) occurs due to an imbalance in anabolic and catabolic events in the IVD, which results in a loss of function. Lower back pain (LBP) affects 84% of people at some point in their lifetime and is strongly associated with DDD. Current LBP treatments have limited long term efficacy and are symptomatic rather than curative. Cell-based therapies are regarded to hold great potential for the treatment of DDD as it has been hypothesised that they could regenerate the damaged tissue and alleviate LBP. A number of natural and synthetic biomaterials have been investigated as NP tissue engineering scaffolds with varying results. In this study, a self assembling peptide hydrogel (SAPH) was investigated for its potential as a cell carrier and/or scaffold for NP tissue engineering. SAPHs display the advantages of natural polymer hydrogels such as biocompatibility and biodegradability whilst combining the advantages of synthetic materials such as controlled structural and mechanical propertiesCharacterisation determined that the SAPH nanofibrous architecture had features that were of similar scale to extracellular matrix (ECM) components of the human NP. The mechanical properties of the SAPH could be optimised to closely match the native tissue. The system could shear thin and self-heal making the system ideally suited to delivery via minimally invasive procedure. The three dimensional (3D) culture of bovine NP cells (bNPCs) in the SAPH demonstrated that the NP phenotype could be restored after de-differentiation during monolayer culture. Gene expression results demonstrated that ‘traditional’ and ‘novel’ NP markers were highly expressed throughout in vitro culture. Cell viability was high, cell population remained stable and bNPCs adopted the characteristic rounded morphology of native NPCs. Finally, type II collagen and aggrecan, the main ECM components of the NP, were deposited with increasing production over culture period. Growth differentiation factor 6 (GDF-6) has been identified as the most promising current growth factor for inducing discogenic differentiation from human bone marrow mesenchymal stem cell (h-BMMSCs). After samples were stimulated with GDF-6, gene expression results confirmed that a NP-like phenotype could be induced with high expression of ‘traditional’ and ‘novel’ NP markers. Cell viability was high, cell population remained stable and NP associated ECM components were deposited with cells displaying a rounded morphology. Interestingly, when h-BMMSCs were cultured without GDF-6, it was strongly suggested that spontaneous discogenic differentiation occurred after culture in the SAPHs as ‘traditional’ and ‘novel’ NP markers were highly expressed, morphology was comparable to native NPCs and type II collagen and aggrecan were deposited extracellularly. If these findings were accurate then this is the first study to demonstrate that a NP-like phenotype could be induced from MSCs without use of an exogenous growth factor or a discogenic bioactive motif. Despite exciting and novel results, further work is required to confirm the potential of SAPHs for NP tissue engineering scaffolds.
