The facility is situated in South Bohemia and is also part of South Bohemia hospitals association. Majority owner is South Bohemia Region. In this region are located another seven hospitals witch may be considered as competitors. Analysis will verify internal as well as external environment of organization. Based on these findings, the current status, strenghts and weaknesses, opportunities and potential threats will be determined strategy.

They affect patient's safety from the long-term point of view.1 The great part of medication errors occurs on the level of drug administration.2 In healthcare facilities mostly nurses are responsible for medication administration.3 AIM To obtain and evaluate medication errors during drug administration by nurses in a healthcare facility. METHODICS A prospective observational study was conducted in the facility Hamzova léčebna for children and adults focusing on medical rehabilitation. Data collection was performed in May 2019 three consecutive days on each ward, where drugs were administrated by nurse. Direct observation was done in morning, noon and evening drug administrations. Obtained data were recorded into prepared form and were compared with physician's medication order on the patient's record. Management of the Faculty of Humanities, Charles University considers opening a pre-school child care facility in a new seat of the faculty. The intention of this diploma work is to map and analyze legal forms of pre-school child care, needs of parents - employees and students of the faculty, and financial sources from which such facility could be funded; and then on the base of the analysis to design a solution suitable for the faculty. The solution has a form of a project plan, which, when approved, could be worked up and realized. Data needed for the project plan were gathered via analysis of documents, i.e. legal rules and web pages, and a questionnaire among employees and students of the faculty. According to the survey and in the case of available sufficient financial coverage, it would be ideal to establish at the faculty a facility with two children groups, each consisting of 15 children and a special children area of a 12 children capacity.

Perceptions of primary health care facility managers towards the integration of mental health into primary health care : a study of the Tshwane District, Gauteng Province

Thesis (M.A. (Clinical Psychology)) -- University of Limpopo, 2020 / The integration of mental health care (MHC) into primary health care (PHC) has been identified as a practical intervention to: increase accessibility to mental health care; reduce stigma and discrimination against people living with mental illnesses; improve the management of chronic mental illness; and, to reduce the burden of comorbidity of mental illnesses with other chronic illnesses. In the South African context, integrating MHC into PHC also seeks to respond to numerous legislative reforms, with the aim of providing comprehensive health care, particularly to previously disadvantaged populations. The aim of the present study was to explore the perceptions of facility managers in the Tshwane District (Gauteng Province) towards the integration of mental health into PHC. Fifteen participants from the Tshwane district facilities participated in the study. The participants were selected through a non-probability purposive sampling method. Data was collected through in-depth interviews using a semi-structured questionnaire, and analysed using the thematic coding approach. Significant findings suggested that the major hindrances to the realisation of the policy objectives may be due to: the lack of rehabilitation and psychotherapeutic services; insufficient skill and knowledge of mental health on the part of staff; insufficient or unsuitable practice space in the facilities; and, poor cooperation between South African Police Services, Emergency Medical Services and Primary Health Care. Inter-facility communication, district implementation support and policy knowledge has notably increased over the years and were deemed to be amongst the biggest enablers.

First, this dissertation utilizes a statutory analysis to examine all 50 states policies for admitting and managing such individuals in long-term care. Second, the study aims to explain why some states may enact a LTC/SO policy by examining state characteristics, policy and political affiliation. Third, the study sets out to better understand if facility-level characteristics matter to the acceptance of persons on the sex offender registry. And will discuss findings from semi-structured interviews of LTCF administrators of their decision-making processes, and their attitudes towards company policy. The findings of all three levels of analyses are presented and future research is discussed. Bakgrund Det finns en ökad risk hos äldre personer att drabbas av åldersförändringar som kan medföra behov av utökad omvårdnad. En flytt till ett vård- och omsorgsboende kan vara ett alternativ för många äldre, med möjlighet till vård och omsorg dygnet runt. Flytten kan innebära en stor förändring inom individen. Sjuksköterskans ansvar är att identifiera risker och behov för att främja välbefinnande varpå det är av vikt att sjuksköterskan medvetandegör de äldres upplevelser i samband med flytt för att vidta åtgärder vid behov. Syfte Syftet med föreliggande litteraturöversikt är att beskriva äldre personers upplevelser i samband med flytt från det egna hemmet till ett vård- och omsorgsboende. Metod Arbetet utfördes som en strukturerad litteraturöversikt med artiklar hämtade från databaserna CINAHL, PubMed samt PsycINFO. Resultatet analyserades med hjälp av innehållsanalys. Resultat Resultatet frambringade två huvudkategorier; Beslutet att flytta och Anpassningen. I samband med beslutet upplevdes delaktighet som bidragande till känsla av lättnad och trygghet medan motsatsen ledde till stress och maktlöshet. Vid anpassningen upplevdes regler och rutiner som frustrerande och kontrollerande. Möjligheten att bibehålla relationer, upprätthålla sin självbild och fortsätta med tidigare aktiviteter var viktigt för välbefinnandet. Slutsats Upplevelsen hos en äldre person i samband med flytt till ett vård- och omsorgsboende har visat sig bero på en rad påverkbara faktorer. Faktorer som sjuksköterskan kan medvetandegöra och med hjälp av omvårdnadsprocessen bidra till en positiv transition för den äldre i samband med flytten. / Background There is an increased risk in older persons of being affected by age-related changes that may entail the need for extended care. A move to a residential care facility can be an option for many elderly persons, with the possibility of round-the-clock care. There location can lead to a major change within the individual. The nurse's responsibility is to identify risks and needs to promote well-being, where upon it is important that the nurse raises awareness of the elderly's experiences when moving in order to implement measures if necessary. Aim The purpose of the present literature review is to describe older persons ́ experiences when moving from their own home to a residential care facility. Method This study was carried out as a structured literature review with articles from the databases CINAHL, PubMed and PsycINFO. The results were analyzed using a content analysis. Results The result produced two main categories; The decision to move and Adaptation. In relation with the decision, the older person experienced participation as contributing to feeling of relief and security, while the opposite led to stress and powerlessness. During adaptation, rules and routines were perceived as frustrating and controlling. The ability to maintain relationships, one's self-image and continue with previous activities was important for well-being. Conclusions The experience of an elderly person when moving to a residential care facility has been shown to depend on several influenceable factors. The nurse should raise awareness of the factors and with the help of the nursing process contribute to a positive transition for the elderly when moving.

Emergency Powers: Addressing the Crisis of Homelessness in a Canadian City

Urban homelessness is one of the most blatant forms of social exclusion in advanced capitalist societies. In Canada, the United States, and the United Kingdom, homelessness has become particularly entrenched due to systemic forces such as global economic recession, the restructuring of regional economies, the dismantlement of the welfare state, and the erosion of affordable housing that together have pushed more people towards economic marginality and housing insecurity. Despite years of advocacy and 'high profile' government investments, homelessness has persisted as an intransigent social problem. A central purpose of this doctoral thesis is to provide some insight into how homelessness is being 'managed' in one Canadian city. The papers gathered together in this dissertation are based on three years of ethnographic fieldwork (conducted between 2006 and 2009) in a medium-sized, de-industrializing city in the province of Ontario. The specific focus of the thesis is the emergency shelter system. Over the past ten years, a number of new social service models have emerged in response to rising rates of homelessness. As these have been adopted homeless shelter systems in many Canadian cities have undergone significant reconfigurations. This thesis focuses on some of the new 'management spaces' that are emerging in this unfolding policy context. The first paper explores the experiences of voluntary sector organizations and local state authorities. The second paper explores the experiential dimensions of a unique service environment providing emergency shelter and social services. Résumé : La dénutrition subséquente à la dysphagie chez les résidents en centre d’hébergement et de soins de longue durée (CHSLD) est un problème majeur en raison de ses conséquences délétères, de sa prévalence élevée et de ses multiples facteurs de risque. La principale intervention nutritionnelle chez cette clientèle est de faciliter l’alimentation avec un régime de textures adaptées (ex. : purée, hachée). Cependant, des taux élevés de dénutrition sont constatés, malgré les interventions. La production d’aliments à textures adaptées, souvent réalisée en institution, comporte de nombreux défis (qualités organoleptiques, valeur nutritive, textures) alors que les produits commerciaux peuvent présenter plusieurs avantages. Les aliments Épikura© se distinguent avec des valeurs nutritives standardisées, des textures mesurées et un aspect visuel amélioré, en plus d’une efficacité clinique montrée lors d’une étude pilote. Le manque de données probantes dans le domaine a inspiré l’idée d’évaluer l’effet des aliments Épikura© sur les apports nutritionnels et le poids de résidents dysphagiques en CHSLD. Pour ce faire, nous avons réalisé un essai clinique randomisé. Les participants ont reçu, de façon aléatoire, les aliments Épikura© (groupe expérimental) ou les aliments à textures adaptées institutionnels (groupe témoin) pendant 20 semaines. Dû à des contraintes organisationnelles et à la fragilité de la clientèle, la taille d’échantillon optimale (n calculé = 48) n’a pu être atteinte (n réel = 17). Une augmentation significative des apports énergétiques, glucidiques et lipidiques (p < 0,05) a tout de même été observée dans le groupe expérimental (n = 7), en comparaison avec le groupe témoin (n = 10). Les participants des deux groupes ont conservé un poids stable, toutefois une tendance à la baisse a été observée dans le groupe témoin. Nos résultats montrent que les aliments Épikura© ont le potentiel d’améliorer les apports énergétiques et nutritionnels chez les résidents dysphagiques en CHSLD. De plus, l’étude souligne les défis méthodologiques reliés à la recherche en milieu clinique et d’hébergement et met en évidence les perspectives de la recherche dans le domaine des aliments à textures adaptées. / Abstract : Malnutrition resulting from dysphagia among older adults living in a residential longterm care (LTC) facility is a major concern as regards its high prevalence, multiple risk factors and severe consequences. The main intervention among this clientele is to provide a texture-modified diet in order to facilitate feeding. However, despite interventions, high prevalence of malnutrition is observed. In-house production of texture-modified foods is common and challenging (organoleptic qualities, nutritional value, texture) whereas commercial products can provide several advantages. Epikura© foods distinguished itself by its standardized nutritional values, rheological measures and improved visual aspect, in addition to a clinical efficacy shown in a previous pilot study. The goal of this study is to assess the effect of Epikura© foods on nutritional intakes and weight of dysphagic residents of LTC facility. A randomized clinical trial was conducted where participants were randomly allocated to receive either Epikura© foods (experimental group) or in-house texture-modified foods (control group) during 20 weeks. Due to organizational constraints and the frailty of this clientele, optimal sample size (calculated n = 48) could not be reached (actual n = 17). Significant increase in energy, carbohydrates and lipids intakes (p < 0.05) has nevertheless been oberved in the experimental group (n = 7), in comparison with the control group (n = 10). Participants of both groups maintain a stable weight, although the control group experienced a downward trend. Our results showed that Epikura© foods have the potential to improve nutritional intakes among dysphagic residents of LTC facility. Moreover, this study highlights methodological challenges of conducting research in a clinical setting and emphasizes new research opportunities in the area of texture-modified foods.

Depuis de nombreux siècles, médecin et patient sont unis sous le prisme de la confiance, principe qui a connu des vicissitudes face à la judiciarisation de la relation de soins et la mutation de l’exercice médical. L’essor du tourisme ainsi que la création du premier hôpital transfrontalier participent à ce phénomène, et montrent que désormais le patient n’hésite plus à franchir les frontières pour recevoir des soins de qualité. C’est dans un contexte d’étroite collaboration entre la France et l’Espagne que s’inscrit l’étude des relations contractuelles et extracontractuelles dans le secteur médical. Elle met en avant un enchevêtrement de jeu d’acteurs entre établissements publics et privés de santé, professionnels de soins, compagnies d’assurance de prestations de services sanitaire et patient. Il est donc difficile d’identifier la nature des relations qui les unit et le régime d’indemnisation applicable. L’impact des droits des patients et la recherche de la qualité des soins dans les deux pays frontaliers démystifient l’aspect contractuel de la relation et amènent à une nouvelle lecture en faveur d’une qualification unitaire. L’approche institutionnelle doit être privilégiée, caractérisée par l’intromission constante des législateurs français et espagnols, sur l’exécution des soins mais également sur l’indemnisation des dommages médicaux, mais à quel degré ? / Since the early ages, doctors and patients are bonded together under the lens of trust, principle that has suffered tribulations faced with the increasing litigiousness in the caring relationship and the professional medical practice changes.The rise of tourism as well as the creation of the first transborder hospital play an important role in this phenomenon, and show that from now on the patient does not hesitate to cross the borders in order to benefit from high quality medical care. The study of contractual and extra-contractual relationships in the medical field falls within a context of close collaboration between France and Spain. It reveals an entanglement in the interactions between public and private health care facilities, healthcare professionals, health care services insurance providers and the patient.Thus, it is difficult to identify the nature of the relationships federating them, as well as the practicable compensation system. The patients’ rights impact and the search for high quality medical care in both transborder countries explain the contractual aspect of the relationship and bring in a new point of view in favour of unitary qualification. The institutional approach must be given priority through the constant insertion of French and Spanish legislators, regarding the accomplishment of medical care as well as the medical damages compensation, but to which degree?
