中醫總額預算制度之研究—以共有資源管理的觀點 / A Study of the Chinese Medicine’s Global Budget in Taiwan: The Perspective of Common-Pool Resources Management

從實務面來看,總額支付制度(global budget)的施行絕非僅止於管制成本的手段,還必須觀照該管理的制度設計。畢竟制度不但能夠彙集各種資源,同時也成為資源使用的指標,影響行動者間的策略選項。另外,針對政策實施後的監控與評估檢討,也是修正制度與提升品質的反饋歷程。然而,學界與實務界在總額支付制度設計原則的研究卻十分有限,究其原因,重要的是主要集中於制度在實際的應用與效果,缺乏以一套適用的「方法」(methodology)來審視。誠如Elinor Ostrom所言,「政策分析家的工具箱有各式各樣的工具,但還缺乏一種具體明確的集體行動理論。」,共有資源管理的例子到處都有,但是分析家卻沒有很好的在理論上總結他們。他藉由制度分析與經驗實例,衍生出來的「制度設計原則」(Design principles),研究各種共有資源管理的問題:從不同類型的資源系統如灌溉系統、捕魚等自然資源或網路資源、財政預算等、到公共財制度研究等,都已經獲得豐碩的成果。本文的主要目標,就是在既有的理論基礎上,討論應用制度設計原則來進行「總額預算評估」的可能內容,並提出政策建議與後續研究的可能內容提出建議。 本文主要分為:(一)導論(二)文獻回顧與理論基礎(三)研究設計(四)中醫總額支付制度的演變(伍)中醫總額的自主管理(六)結論。希冀對學界或實務界在健保改革評估未來的發展,能有所啟發與助益。 / From the practice, global budget is not just the means to control cost, must also take the design of management into consideration. The institution not only can gather various kinds of resources, but also become the indexes, and influence the tactics among the persons who take action to select. Since a global budget married to fee-for-service payments directly analogous to a common-pool resources, we use a framework of design principles to explore the difference of outcomes of management, and to discuss the policy implications about institutional design and financial Incentives. Sincere as what Elinor Ostrom said, ' the policy analyst's box has many kinds of tools , but still lack a kind of concrete and clear collective action theory. ', example there are resource management in common everywhere, but it is the getting better to summarize they in theory. In addition, the “self-governing institutions” designed by each local medical association must be flexibly adapted to the context in which it is applied. To foster the cooperation among physicians, the medical associations of local level should also play the role to provide the institutional setting for collective participatory decision making. The medical associations of national level ought to change the financial incentives facing the physicians to induce the rational patterns of medical behavior so that the allocate efficiency of medical resource and effectiveness of global- budget management can be improved.

Ce travail questionne les contraintes et les déclencheurs des pratiques du cosmétique en Chine et la conception du corps des Chinois. Il s'agit, d'une part, d'analyser diachroniquement les effets de générations influencés par les produits cosmétiques disponibles sur le marché et les normes sociales en vigueur quant aux soins du corps à différentes époques. Il s’agit, d'autre part, de mettre en évidence les effets de cycle de vie et de les considérer comme autant de déclencheurs, de leviers, influant sur la pratique des soins du corps et du maquillage. Enfin, il s'agit, synchroniquement, de suivre l'itinéraire des produits cosmétiques chez les femmes de milieu aisé : depuis la prise d'information avant l'achat, la mobilité, l'acquisition, l'usage et le rangement jusqu'au devenir du produit. Dans un deuxième temps, l'enquête menée auprès de dermatologues exerçant la médecine traditionnelle chinoise et la médecine moderne occidentale a d’abord pour but d’examiner si les dermatologues sont des prescripteurs de produits cosmétiques en Chine, puis d’approfondir l’analyse de la conception du corps et de la maladie chez les médecins exerçant la médecine traditionnelle. Ce travail nous permet de comprendre que les femmes chinoises réinterprètent les produits cosmétiques modernes et occidentaux selon leurs propres conceptions, quotidiennes et chinoises, du corps. / This work questions the constraints and triggers of cosmetic practices in China and Chinese people body conception. It consists in, firstly, studying diachronically the effects of generations that are influenced by cosmetic products on the market and the social norms of body care at different times. Secondly, the effects of life cycle are analyzed as triggers, levers, important to body care and makeup practices. Finally, synchronously, it aims to follow the itinerary of cosmetics in wealthy women families: from the information before the purchase decision, mobility, acquisition, use, storage to the fate of the product. In the next step, the survey with dermatologists of traditional Chinese medicine and modern Western medicine help to check if dermatologists are prescribers of cosmetics in China and dig further analyze the conception of the body and disease among the doctors applying traditional Chinese medicine. This work allows us to understand that Chinese women reinterpret the modern Western cosmetics by their Chinese body conception in everyday life.

Introdução: A equipe de Enfermagem em hospitais tem sido exposta a ambientes de trabalho estressantes, submetidos, muitas vezes, à condições de trabalho precárias, com baixa qualidade de vida. A auriculoterapia chinesa apresentou eficácia em estudo preliminar para redução de estresse com um protocolo escolhido com base na Medicina Tradicional Chinesa(MTC) e este Ensaio Clínico se propôs a avaliar a eficácia e o alcance da técnica quando é aplicada com e sem protocolo fechado. Objetivos: Descrever e investigar os níveis de estresse da equipe de Enfermagem do Hospital Samaritano; Comparar a eficácia da auriculoterapia chinesa realizada com e sem protocolo, descrever os principais diagnósticos de MTC para estresse e a eficácia dos pontos escolhidos. Material e Método: Na primeira fase, 484 profissionais responderam a um questionário de dados sócio-demográficos e à Lista de Sintomas de Stress de Vasconcellos(LSS). Foram incluídos aqueles com pontuação entre 37 a 119 pontos (médio e alto estresse). Randomizaram-se 213 pessoas em 3 grupos (controle, grupo protocolo e grupo sem protocolo); 175 finalizaram o tratamento de 12 sessões de auriculoterapia com agulha semipermanente, duas vezes por semana, por seis semanas. O protocolo de pontos utilizado foi: ponto Rim, Tronco Cerebral, Shenmen e Yang do Fígado 1 e 2. Os instrumentos de Coleta de dados no Ensaio Clínico foram a LSS, o Inventário de Sintomas de Stress da Lipp (ISSL), o Instrumento de Qualidade de Vida (SF36v2), uma Ficha de diagnósticos de MTC. O período de coleta foi de novembro de 2011 a julho de 2012 e sete acupunturistas participaram do estudo. A pesquisa foi aprovada pelo Comitê de Ética em Pesquisa da EEUSP e do Hospital. Resultados: Na primeira fase, a pontuação média de estresse de 484 profissionais foi de 45,92 (nível médio). Os que apresentaram níveis mais elevados de estresse foram: as enfermeiras (p=0,012), profissionais do turno da manhã (p=0,022) e sujeitos com doenças auto referidas (p=0,001). Na segunda fase, os dois grupos de intervenção obtiveram diferenças estatísticas significativas quando comparadas ao grupo controle (p<0,05), com efeito superior para o grupo sem protocolo segundo a LSS no pós-tratamento e follow-up. Quanto ao ISSL, houve melhores resultados para o grupo sem protocolo para a fase de resistência/quase exaustão e domínio psicológico da fase de alerta. Quanto ao SF36v2, houve diferença estatística (p<0,05) somente para o grupo sem protocolo quanto ao domínio físico no follow-up. No domínio mental, ambos os grupos de intervenção obtiveram resultados positivos(p<0,05), com ligeira superioridade para o grupo sem protocolo quanto ao índice d de Cohen. Os principais diagnósticos de MTC foram: Estagnação de Qi e Xue nos meridianos tendino-musculares, Calor de Estômago e Subida de Yang do Fígado, Estagnação de Qi do Fígado, Distúrbio de Shen, Deficiência de Yin do Rim, Deficiência de Qi e Xue do Baço-Pâncreas. Os pontos mais utilizados foram os cinco pontos do protocolo somados a Estômago, Baço e pontos de dor. Conclusão: o grupo sem protocolo obteve melhores resultados para redução de estresse e melhoria de qualidade de vida, demonstrando que a auriculoterapia chinesa quando feita de forma individualizada / Introduction: Hospitals Nursing Team have been exposed to highly stressful work environment and submitted many times to precarious work conditions and thus low quality of life. Chinese auricular therapy has presented efficiency in a preliminary study to reduce stress by a selected protocol based on Chinese Traditional Medicine (MTC). This Clinical Trial is proposed to evaluate the effectiveness and reach of the technique whenever this is applied with or without a closed protocol. Objectives: Describe and investigate the stress levels of Samaritano Hospitals Nursing Team; Compare the effectiveness of Chinese Auricular Therapy performed with or without protocol; Describe the main diagnosis of MTC for stress and efficiency of the selected points. Material and Method: On the first phase, 484 professionals answered to a questionnaire containing demographic social data and to the Vasconcelos List of Stress Symptoms (LSS). Those ones with score between 37 and 119 points (average and high stress) were included. 213 people were randomized in 3 groups (control, protocol group and group without protocol); 175 ended a 12-session auricular therapy treatment with semi-permanent ear needles twice a week for six weeks. The scoring protocol used was: Kidney point, Brain stern point, Shenmen and Yang of the Liver 1 and 2. The instruments for the data collection in the Clinical Trial were LSS, Lipp Stress Symptoms Inventory (ISSL), Instrument of Life Quality (SF36v2), MTC diagnosis chart. The period of data collection was between November 2011 and July 2012 and seven acupuncturists participated in the trial. This study was approved by the School and Hospital Ethical Committee on Clinical Trial. Results: On the first phase, the average scoring of stress was of 45,92 (average level of stress). The professionals that presented higher stress levels were: nurses (p=0,012), morning shift professionals (p=0,022) and subjects with self-referred diseases (p=0,001). On the second phase, the two intervention groups presented differences once compared with the control group (p<0,05), with higher effect in the group without protocol according to LSS in the after treatment and follow-up. Regarding ISSL, there were better results to without protocol group for resistance/quasi-exhaustion phase and physical domain of alert phase. In relation to SF36v2, there was statistics difference (p<0,05) only for the group without protocol in connection with physical dominance during follow-up. In mental dominance, both of intervention groups presented positive results (p<0,05), with an slight superior effect for the group without protocol as per Cohens index. The main MTC diagnosis was: Qi stagnation and Xue in the tendino-muscular meridians, Stomach Heat and Rising of Yang of the Liver, Qi stagnation of the Liver, Shen Disturbs, Kidney Yin Deficiency, Qi deficiency and Xue of Spleen-Pancreas. The mostly used points were the five ones of the protocol plus Stomach, Spleen and pain points. Conclusion: the group without protocol presented the best results for stress reduction and improvement of life quality, demonstrating that Chinese auricular therapy made in a personalized way broaden the reach of the technique.

There has been an attempt to keep the legitimacy of the source of the text in the translations into the Portuguese language, made by the author of the thesis. Thematic excerpts of dào, yīnyángqì, wǔxíng of the The Yellow Emperor s Inner Classic have been taken and contrasted to the European versions by means of comparisons that have been given systematized commentaries. Because both textualities are products of the fecundity and intelligibility of two distinct cultures, ancient Chinese and current European cultures, there is reference to the study of the problems of the cultural differences with the dimensions that have implications in the Philosophical, Linguistic and aesthetics scopes. A conceptual elaboration has been performed in order to explain the cultural phenomena involved in the transformation and in the translocation of the hypotext into the hypertext. The cultural differences have been made concrete based on Montgomery s cultural diffusion theories, Bastide s acculturation, globalization, Holton s hybridization, Lackner s hybridization and neo-figurism, Julien s cultural fecundity, Genette and Kristeva s intertextuality. The result of the research has led to the conclusion that the translation into Portuguese sought to preserve the authenticity of the original book s cultural and textual fecundity. The translocation of the original text from ancient China to current Europe has received modifications in cultural, philosophical, semantic-linguistic and aesthetic dimensions that took place in the translations of the ancient polysemic sinograms of taoism. These were decontextualized in the entrance of the received texts in another culture, the European culture, which is distinct from the former. It conduces to phrasing and recurrent significations, which are familiar in the receiving Western culture that overpowers the ancient Chinese, belonging to the ancestry of the Yellow Emperor / O presente trabalho científico investiga objeto composto do texto original de medicina chinesa antiga do taoismo, em chinês arcaico, o Clássico Interno do Imperador Amarelo e as traduções europeias em inglês, português, de Veith; em francês de Ung e Chamfrault. Procurou-se manter a legitimidade do texto de origem na tradução para o idioma português feito pelo autor da tese. Foram extraídos excertos temáticos do dào, yīnyángqì, wǔxíng do Clássico Interno comparado às versões europeias por recursos de cotejos que receberam comentários sistematizados. Sendo que, ambas as textualidades são produtos da fecundidade da inteligibilidade de duas culturas distintas, a da chinesa antiga e a europeia atual. Por essa razão remeteu-se ao estudo dos problemas das diferenças culturais com as dimensões que implicam no âmbito filosófico, linguístico e estético. Realizou-se a elaboração conceitual para se explicitar os fenômenos culturais envolvidos na transformação na transladação do hipertexto ao hipotexto. Foram feitas concretização das diferenças culturais fundamentando-se nas teorias da difusão cultural de Montgomery, aculturação de Bastide, globalização, polarização, hibridação de Holton, hibridação e neo-figurismo de Lacnker, fecundidade cultural de Jullien, intertextualidade de Genette e Kristeva. O resultado da pesquisa permitiu concluir que a tradução ao português procurou preservar a autenticidade da fecundidade cultural e textual do livro de origem. Effects of endocrine disruptors and traditional Chine medicine on the development of zebrafish / Effets des perturbateurs endocriniens et de la médecine traditionnelle chinoise sur le développement du poisson-zèbre

Les problèmes de développement induits par les perturbateurs endocriniens (PE) sont actuellement peu étudiés, alors qu’une exposition précoce peut entraîner plus tard des problèmes permanents. Les lignées transgéniques de poisson zèbre (Danio rerio) avec une expression tissu-spécifique de la GFP sont des outils utiles pour identifier les organes affectés par un composé donné. Nous avons utilisé 7 lignées transgéniques pour visualiser in vivo si 6 PEs connus et 3 médicaments pouvaient avoir des effets sur le développement du poisson zèbre. Ce crible a révélé que 4 produits chimiques ont des effets sur 4 organes différents. Le tétrabromobisphénol-A, ainsi que le diclofénac, la trichostatine A et l'acide valproïque) perturbent le développement du système vasculaire. De plus, les inhibiteurs de HDAC trichostatine A et acide valproïque inhibent le développement du pancréas et induisent des retards de développement dans le foie et dans les dents pharyngiennes.La médecine chinoise traditionnelle (TCM) est un élément de la médecine moderne. Cependant, nous savons peu de choses sur les activités biologiques des composés TCM au cours du développement. Nous avons étudié les effets de 3 plantes et de 5 composés sur l'embryogenèse du poisson zèbre. Des extraits aqueux de Astragalus membranaceus et Akebia quinata provoquent des retards du développement. Nous avons aussi constaté que le développement vasculaire a été affecté à différents niveaux par Salvia miltiorrhiza et 3 de ses composants principaux, soit utilisés seuls ou mélangés entre eux.Nos résultats montrent que les PEs et les TCM peuvent causer des problèmes lors de l'embryogenèse. Ils montrent également que le poisson zèbre est un outil puissant pour le criblage rapide in vivo de petites molécules et de leurs effets sur le développement. Ce travail nous permet d'établir un parallèle entre EDC et TCMs, qui peuvent agir sur des cibles similaires, tels que les récepteurs nucléaires. / Development problems induced by endocrine disruptors (EDCs) are currently understudied. However, early exposure to EDCs may lead to deleterious and permanent problems in later lifetime. Zebrafish (Danio rerio) transgenic lines with tissue-specific expression of GFP are useful tools to identify the organs affected by a given compound. We have used 7 transgenic lines to visualize in vivo whether 6 known EDCs and 3 other pharmaceuticals can alter organogenesis during development of zebrafish. This screen revealed that 4 chemicals have effects on 4 different organs. The EDC tetrabromobisphenol-A, as well as the tested medicines (diclofenac, trichostatin A and valproic acid) disrupt vascular system development in zebrafish embryo. Moreover, HDAC inhibitors trichostatin A and valproic acid inhibit both endocrine and exocrine pancreas development. Developmental delays were also induced by trichostatin A and valproic acid in the liver and in the pharyngeal teeth. Traditional Chinese medicines (TCMs) are important components of modern medicine. However we know little about the biological activities of TCMs compounds during development. We used zebrafish embryos to study the effects of 3 plants and 5 of their major compounds on the development. We observed that zebrafish embryogenesis was delayed by water extracts from Astragalus membranaceus and Akebia quinata. We also found that the vascular development was affected at different levels by Salvia miltiorrhiza water extracts and by its 3 major components either used alone or mixed together.Our results show that EDCs and TCMs can cause problems during zebrafish embryogenesis. They also show that zebrafish is a powerful tool for rapid in vivo screening of small molecules and their effects on development. This work also enables us to draw a parallel between EDC and some TCMS, which may act on similar targets, such as nuclear receptors.

Traditional Chinese Medicine extracts exert angiogenic and protective effects towards human endothelial progenitor cells: from cellular function to molecular pathway

Despite intense research efforts, the repair of large bone defects is still not satisfactory and remains a major challenge in Orthopaedic Surgery. In this context bone tissue engineering has emerged as a promising strategy. However, one of the fundamental principles underlying tissue engineering approaches is that newly formed tissue must maintain sufficient vascularization to support its growth. Thus an active blood vessel network is an essential pre-requisite for scaffold constructs to integrate within existing host tissue. Currently, great efforts are made to address this problem employing transplantation of vascular cells and loading of appropriate biological factors. Endothelial progenitor cells (EPCs) are a heterogeneous subpopulation of bone marrow mononuclear progenitor cells with potential for differentiation to the endothelial lineage and thus vasculogenic capacity. However, clinical studies reported that with the increase of age, increased susceptibility to apoptosis and accelerated senescence may contribute to the numerical and functional impairments observed in EPCs, which may lead to a reduced angiogenic capacity and an increased risk of vascular disease. Hence attention has increasingly been paid to enhance mobilization and differentiation of EPCs for therapeutic purposes. A large body of evidence indicates that in Traditional Chinese Medicine (TCM) a plethora of herbs and herbal extracts are effective in the treatment of vascular diseases such as chronic wounds, diabetic retinopathy and rheumatoid arthritis. Thus, it seems rational to explore these medicinal plants as potential sources of novel angiomodulatory factors. In this thesis we demonstrated that treatment with TCM herbal extracts promote cell growth, cell migration, cell-matrix and capillary-like tube formation of BM-EPCs. Among these TCM extracts, Salidroside (SAL) and Icariin (ICAR) incubation increased VEGF and nitric oxide secretion, which in turn mediated the enhancement of angiogenic differentiation of BM-EPCs. A mechanic evaluation provided evidence that SAL stimulates the phosphorylation of Akt, mammalian target of rapamycin (mTOR) and ribosomal protein S6 kinase (p70S6K), as well as phosphorylated ERK1/2, which is associated with the cell migration and tube formation. Furthermore, a pilot in vivo study showed that SAL has the potential to enhance bone formation in a murine femoral critical-size bone defects model. Another new finding of the present study is that hydrogen peroxide (H2O2)-induced cytotoxicity is counteracted by TCM extracts. We found that SAL, Salvianolic acid B (SalB) and ICAR significantly abrogated H2O2-induced cell apoptosis, reduced the intracellular level of reactive oxygen species (ROS) and nicotinamide adenine dinucleotide phosphate-oxidase (NADPH) expression, and restored the mitochondrial membrane potential of BM-EPCs. Our data suggest that this protective effect of SalB is mediated by the activation of mTOR, p70S6K, 4EBP1, and by the suppression of MKK3/6-p38 MAPK-ATF2 and ERK1/2 signaling pathways after H2O2 stress. In addition, the investigation also demonstrates that ICAR owns the ability to inhibit apoptotic and autophagic programmed cell death via restoring the loss of mTOR and attenuation of ATF2 activity upon oxidative stress. Based on the outcomes of the present work, we propose SAL, SalB and ICAR as novel proanigiogenic and cytoprotective therapeutic agents with potential applications in the fields of systemic and site-specific tissue regeneration including ischaemic disease and extended musculoskeletal tissue defects.

Making sense of traditional Chinese medicine: a cognitive semantic approach

Cognitive linguists posit that language as a system of meaning is closely related to cognition and to the associated perceptual and physiological structures of the body. From the cognitive semantic viewpoint, cognitive processes underpin and motivate linguistic phenomena such as categorisation, polysemy, metaphor, metonymy and image schemas. The pedagogical implication of the cognitive semantic perspective is that understanding these cognitive motivations facilitates language learning. This dissertation uses an applied cognitive semantic approach to `make sense' of a traditional knowledge system, Traditional Chinese Medicine (TCM). TCM views human physiology as a holistic and dynamic system that exemplifies the same principles as the cosmos-environment. TCM models result in a categorisation of physiological phenomena based on a complex system of experiential and cosmological correspondences. I suggest that the holistic epistemology of cognitive linguistics is well suited to an understanding of these holistic models. From a pedagogical viewpoint, I argue that an analysis of the cognitive motivations which underpin TCM categorisations and the polysemy of some key TCM terms can help the student make sense of TCM as a meaningful system of thought and practice. Both the theoretical and applied approaches explored in this dissertation should have relevance to other traditional knowledge systems, particularly traditional medical systems. / Linguistics and Modern Languages / M.A. (Linguistics)

Blood constituents, as red blood cells (RBC) and plasma proteins, have been labeled with 99mTc. Natural and synthetic drugs can alter the labeling of these constituents. The aim of this work was to investigate the possibility of a Buzhong YiQi Wan extract to alter (i) the labeling of blood constituents with 99mTc, (ii) the RBC morphology, and (iii) osmotic fragility of RBC withdrawn from Wistar rats. The data showed that the BYQW extract (i) could affect labeling of blood constituintes with 99mTc, (ii) could affect the membrane integrity decreasing the osmotic resistance and (iii) could not alter the shape of RBC. Probably, these findings would be associated &#65279;with properties of the substances present in the aqueous extract of BYQW. Diversas drogas naturais ou sint?ticas s?o capazes de interferir na marca??o de estruturas sangu?neas com 99mTc, assim como na biodistribui??o de outros radiobiocomplexos. Os procedimentos relacionados com a medicina tradicional chinesa tamb?m v?m ganhando destaque em todo o mundo. O objetivo deste estudo foi investigar o efeito da possibilidade de altera??es pelo extrato de Buzhong Yi Qi Wan (f?rmula magistral chinesa) nos constituintes do sangue marcados com 99mTc, (i) na marca??o de hem?cias e prote?nas plasm?ticas, (ii) na morfologia, e (iii) na fragilidade osm?tica de hem?cias de ratos Wistar. Foi observado, diminui??o significativa (p<0,05) na marca??o dos constituites sangu?neos com 99mTc, n?o altera??o da morfologia das hem?cias e modifica??o da curva de fragilidade das hem?cias (p<0,05). Tradiční čínská medicína a současné ošetřovatelství / Tradicional Chinese medicine and current nursing

Traditional Chinese medicine is one of the oldest curative methods in the world. It is focused particularly on the support of a body to recover balance and harmony that have become impaired. Owing to the growing number of the Chinese and the Vietnamese minorities in the Czech Republic, the medical staff providing healthcare may meet members of these minorities for whom this medicine may be of a large importance both when being ill and when being healthy. The new conception of the Czech nursing is outlined to provide every individual, regardless of the race, with the care which is able to satisfy their bio-psycho-social-religious needs. The nursing staff must possess a basic knowledge of the traditional Chinese medicine so that they are able to satisfy the needs of these patients/clients better. The objective of this diploma work is to ascertain what health problems are treated by members of the Chinese and the Vietnamese minorities through the traditional Chinese medicine, and what curative methods are used most frequently by them. The empirical part was processed in the form of a qualitative research. The data collection technique was a semi-structured interview of selected informants of the Chinese and the Vietnamese nationalities living in the Czech Republic. Based on the interviews of individual minorities, case studies were established and used as the research base for elaboration of categorization tables and charts presenting the most important results of the research. The main results of the research include the ascertained heath problems which are treated by means of the Chinese medicine by members of both minorities, and the ascertainment what curative methods are used most frequently, and whether the members of both minorities wish to make use of the medicine also during hospitalization. Based on the results of the research, the information material was elaborated that may be of use for the nursing staff in healthcare facilities and for other specialized public as a brief summary of methods of the Chinese medicine and of their application by the members of the Chinese and the Vietnamese minorities.
