Microfluidic bead-based methods for DNA analysis

With the completion of the human genome sequencing project, attention is currently shifting toward understanding how genetic variation, such as single nucleotide polymorphism (SNP), leads to disease. To identify, understand, and control biological mechanisms of living organisms, the enormous amounts of accumulated sequence information must be coupled to faster, cheaper, and more powerful technologies for DNA, RNA, and protein analysis. One approach is the miniaturization of analytical methods through the application of microfluidics, which involves the manipulation of fluids in micrometer-sized channels. Advances in microfluidic chip technology are expected to play a major role in the development of cost-effective and rapid DNA analysis methods. This thesis presents microfluidic approaches for different DNA genotyping assays. The overall goal is to combine the potential of the microfluidic lab-on-a-chip concept with biochemistry to develop and improve current methods for SNP genotyping. Three genotyping assays using miniaturized microfluidic approaches are addressed. The first two assays are based on primer extension by DNA polymerase. A microfluidic device consisting of a flow-through filter chamber for handling beads with nanoliter liquid volumes was used in these studies. The first assay involved an allelespecific extension strategy. The microfluidic approach took advantage of the different reaction kinetics of matched and mismatched configurations at the 3’-ends of a primer/template complex. The second assay consisted of adapting pyrosequencing technology, a bioluminometric DNA sequencing assay based on sequencing-bysynthesis, to a microfluidic flow-through platform. Base-by-base sequencing was performed in a microfluidic device to obtain accurate SNP scoring data on nanoliter volumes. This thesis also presents the applications of monolayer of beads immobilized by microcontact printing for chip-based DNA analysis. Single-base incorporation could be detected with pyrosequencing chemistry on these monolayers. The third assay developed is based on a hybridization technology termed Dynamic Allele-Specific Hybridization (DASH). In this approach, monolayered beads containing DNA duplexes were randomly immobilized on the surface of a microheater chip. DNA melting-curve analysis was performed by dynamically heating the chip while simultaneously monitoring the DNA denaturation profile to determine the genotype. Multiplexing based on single-bead analysis was achieved at heating rates more than 20 times faster than conventional DASH provides. / QC 20101008

Coordenação de Aperfeiçoamento de Pessoal de Nível Superior / A adoção do plano dietético Dietary Approaches to Stop Hypertension (DASH) tem sido enfatizado na população hipertensa como uma importante estratégia no controle dos níveis pressóricos elevados. O objetivo deste estudo foi analisar o consumo alimentar de macro e micronutrientes em pacientes hipertensos, em especial do sódio, cálcio, potássio e magnésio, e comparar com as recomendações dietéticas contidas no plano DASH. Estudo transversal envolvendo uma amostra de conveniência composta por 113 hipertensos entre 40 e 69 anos. A medida da pressão arterial (PA) foi determinada com aparelho eletrônico devidamente calibrado e a avaliação dietética obtida pelo questionário de freqüência do consumo alimentar. Os alimentos ingeridos foram convertidos em porções e distribuídos em diferentes grupos alimentares. A faixa de porções recomendadas pelo plano DASH foi determinada com base na média das necessidades energéticas desta amostra permitindo assim uma posterior comparação com o hábito alimentar dos hipertensos, utilizando um escore de pontos com pontuação máxima de 9 pontos. A amostra foi dividida em três grupos: grupo B que obteve 2,5 a 4,0 pontos (n=34; 30%), grupo M entre 4,5 a 5,0 pontos (n=43; 38%) e grupo A que obteve 5,5 a 8,0 pontos (n=36; 32%). Não foram observadas diferenças significativas na média da PA sistólica (14024 vs 13823 vs 13515 mmHg) e diastólica (8617 vs 8813 vs 8410 mmHg). Apesar do grupo A consumir mais proteínas e gordura monoinsaturada, foi detectado um excesso pronunciado na ingestão de gordura saturada, colesterol e das calorias totais, por este grupo de pacientes. No que tange a ingestão dos micronutrientes, o grupo A apresentou consumo significativamente maior de cálcio, potássio e magnésio, refletido pela maior ingestão de vegetais e frutas em comparação aos grupos B e M. A média de ingestão do sódio intrínseco foi significativamente maior no grupo A (4,12,0 vs 3,11,1 vs 2,71,1 g/dia). Foram detectadas, apenas no grupo A, correlações entre PA sistólica e o percentual de proteína (r = -0,5; p=0,002) e PA sistólica e o percentual de carboidrato (r = 0,4; p=0,02). Apenas um terço dos hipertensos avaliados apresentaram padrão alimentar mais concordante com o plano DASH e com maior ingestão de proteínas, gordura monoinsaturada, fibras, cálcio, potássio e magnésio. Entretanto, o consumo mais elevado de sódio, gordura saturada, colesterol e das calorias totais por este grupo de pacientes poderia restringir uma maior queda dos níveis pressóricos elevados. / The adoption of the Dietary Approaches to Stop Hypertension (DASH) plan has been emphasized in the hypertension population as an important strategy in the control of the raised blood pressure (BP). The objective of this study was to analyze the alimentary consumption of macro and micronutrients in hypertensive patients, especially of sodium, calcium, potassium and magnesium, and to compare with the dietary recommendations contained in DASH plan. Transversal study was conducted in a convenience sample of 113 hypertensive patients, between 40 and 69 years. The BP measurement was determined with calibrated electronic device and the dietary assessment was performed trough the semiquantitative food frequency questionnaire. For comparison with DASH plan, ingested foods were converted into portions and the magnitude of the portions recommended for DASH plan was determined on the basis of the mean energy requirements of this sample. Thereafter, cut-off points were established for food groups, the maximum number of points that a patient could reach was 9. The sample was divided in tertils: Group L with 2.5 4.0 points (n=34; 30%), Group I with 4.5 5.0 points (n=43; 38%) and Group H with 5.5 8.0 points (n=36; 32%). Systolic and diastolic BP were not significantly different among the groups (14024 vs 13823 vs 13515 mmHg/ 8617 vs 8813 vs 8410 mmHg). Group H consumed more proteins and monoinsaturated fat, a pronounced excess in the saturated fat, cholesterol and total calories intake was also detected in this group of patients. Regarding micronutrients intake, group H presented significantly higher consumption of calcium, potassium and magnesium, reflected for higher vegetable and fruits intake in comparison to the groups L and I. The average of intrinsic sodium intake was significantly higher in the group H (4.12.0 vs 3.11.1 vs 2.71.1 g/day). Negative correlation between the systolic BP and dietary protein percentage (r=-0.5, p=0.002) and a positive correlation between the systolic BP and the carbohydrate percentage (r=0.4, p=0.02) was verified only in group H. The present study showed that only one third of the hypertensive patients presented alimentary pattern more consistent with DASH plan, showing greater intake of protein, monounsaturated fat, fiber, calcium, potassium and magnesium. However, pronounced sodium, saturated fat and cholesterol intake and excess of total calories in these hypertensive patients could restrict a greater control of BP.

A hipertensão é uma das principais causas de morbidade e mortalidade no Brasil. Os hipertensos muitas vezes apresentam perfil lipídico e glicidico desfavoráveis. A alimentação pode desempenhar um papel importante na redução da pressão arterial (PA) e no perfil lipídico e controle glicêmico desses pacientes. Avaliar o impacto de uma intervenção nutricional adaptada ao padrão alimentar brasileiro no controle dos níveis pressóricos e metabólico de pacientes hipertensos em acompanhamento em um serviço de atenção primária de saúde do município de São Luís do Maranhão. Metodologia: ensaio clínico randomizado utilizando uma dieta de baixo índice glicêmico combinada ao aumento do consumo de frutas, vegetais, grãos integrais e laticínios desnatados que são os princípios do Dietary Approach to Stop Hypertension (dieta DASH). Foram alocados randomicamente 206 pacientes hipertensos que foram acompanhados por 6 meses. O grupo controle (GC, n=101) recebeu aconselhamento padrão, focado na redução da ingestão de sal. Resultados: Dos 206 pacientes randomizados, 156 (37 homens, 119 mulheres) completaram o estudo. A idade média dos participantes foi de 60,1 (DP 12,9) anos. Após 6 meses, houve redução na média da pressão arterial sistólica (PAS) em 14,4 mmHg e na diastólica (PAD) de 9,7 mmHg no grupo experimental (GE), em comparação a 6,7 mmHg e 4,6 mmHg, respectivamente, no GC. Após o ajuste para mudança de peso corporal, PA na linha de base e idade, essas diferenças entre os grupos foram de aproximadamente 9,2 mmHg e 6,2 mmHg, respectivamente. Ocorreram tambem variações estatisticamente significantes na excreção urinária de sódio, reduzida em 43,4 mEq/24 h no GE, bem como o colesterol total (-46.6mg/dl) , LDL colesterol (-42.5mg/dl), triglicérides (-31.3mg/dl), glicemia de jejum (-9.6mg/dl ) e hemoglobina glicada (-0,1%). O consumo alimentar modificou-se no GE com aumento do consumo de vegetais, passando de 2,97 para 5,85 ; frutas (4,09-7,18); feijão (1,94-3,13) e peixes (1,80 para 2,74). Modificações importantes relacionadas à redução significativa de carboidratos, teor lipídico e carga glicêmica da dieta, foram observadas. Conclusão: Este estudo mostrou a viabilidade e a eficácia de uma abordagem dietética com base no padrão alimentar brasileiro, na redução da PA e parâmetros bioquímicos inadequados, podendo causar um grande impacto na saúde pública. / Hypertension is one of the leading causes of morbidity and mortality in Brazil. Objective: To evaluate the impact of nutritional intervention adapted to the Brazilian food in controlling blood pressure and metabolic monitoring in hypertensive patients from a primary care service in São Luís do Maranhão. Methodology: Randomized clinical trial using a low-glycemic index diet combined with increased consumption of fruits, vegetables, whole grains and nonfat dairy products which principles of the Dietary Approach to Stop Hypertension (DASH). We randomly assigned 206 patients with hypertension who were followed for 6 months. The control group (CG, n = 101) received standard counseling, focused on reducing salt intake. Results: Of the 206 patients randomized, 156 (37 men, 119 women) completed the study. The average age of participants was 60.1 (SD 12.9) years. After 6 months, a reduction in mean systolic blood pressure (SBP) by 14.4 mmHg and diastolic blood pressure (DBP) by 9.7 mmHg in the experimental group (EG), compared to 6.7 mmHg and 4.6 mmHg, respectively GC. After adjusting for change in body weight at baseline BP and age, these differences between groups were approximately 9,2 mmHg and 6,2 mmHg, respectively. Avec l'hétérogénéité des réseaux et la demande croissante de services multimédias, la Qualité d'Expérience (QoE) devient un facteur déterminant de réussite ou d’échec de ces services. Dans cette thèse, nous avons d’abord analysé l'impact des paramètres de qualité sur l'engagement de l'utilisateur, afin de comprendre l’effet des paramètres vidéo (temps de démarrage, qualité intrinsèque, taux de blocage) et la popularité du service sur l'engagement de l'utilisateur. Nos résultats ont montré que le taux de blocage et la popularité du service sont des paramètres critiques qui impactent fortement la satisfaction et l'engagement de l’utilisateur, tandis que, le temps de démarrage a moins d’impact. Ensuite, les approches subjectives de QoE comme le Mean Opinion Score (MOS) ont été traité dans cette thèse, où les utilisateurs sont appelés à donner une note de satisfaction en fonction des informations de contextes. Une analyse statistique des résultats obtenus à partir de tests utilisateurs a montré que le type de terminal et de contenu sont des paramètres qui impactent fortement le MOS. En plus, des modèles mathématiques d’estimation de QoE en fonction des informations de contextes ont été proposés. Enfin, deux techniques d’optimisations ont été proposées : Le MDASH (MOS Dynamic Adaptive Streaming over HTTP) pour l’optimisation vidéo de flux partageant les mêmes ressources, de manière à garantir une certaine QoE pour tous les utilisateurs. In this thesis, we first propose to analyze the impact of quality metrics on user engagement, in order to understand the effects of video metrics (video startup time, average bitrate, buffering ratio) and content popularity on user engagement. Our results show that video buffering and content popularity are critical parameters which strongly impacts the end-user’s satisfaction and user engagement, while the video startup time appears as less significant. On other hand, we consider subjective approaches such as the Mean Opinion Score (MOS) for evaluating QoE, in which users are required to give their assessment according to contextual information. A detailed statistical analysis of our study shows the existence of non-trivial parameters impacting MOS (the type of device and the content type). We propose mathematical models to develop functional relationships between the QoE and the context information which in turn permits us to estimate the QoE. A video content optimization technique called MDASH (for MOS Dynamic Adaptive Streaming over HTTP) is proposed, which improves the perceived QoE for different video sessions sharing the same local network, while taking QoE fairness among users as a leitmotiv. We also propose a utility-based approach for video delivery optimization, in which a global utility function is computed based on different constraints (e.g. target strategies coming from the actors of the delivery chain)

A Contribution to Video Streaming Quality and Energy Efficiency in Optical and Ethernet Networks

[ES] La distribución de contenidos multimedia, tanto a través de Internet como de redes gestionadas dedicadas exclusivamente a este fin, está teniendo una mayor cuota del tráfico total y crece cada día, especialmente el servicio de vídeo streaming. Por ello, ha surgido el interés de investigar para mantener y mejorar la Calidad de Servicio (QoS) en redes de acceso y backbone, y ofrecer Calidad de Experiencia (QoE) a los usuarios finales; mediante el ajuste de parámetros, configuraciones, protocolos y algoritmos a los requerimientos de la red en cuanto a la alta demanda de tráfico de vídeo. Lo anterior aprovechando al máximo los recursos de la red, tanto en el núcleo y las redes de acceso, como el ancho de banda y la capacidad total instalada, y al mismo tiempo, proponiendo alternativas de transporte eficientes para el tráfico de vídeo agregado sin incrementar los costos de los servicios; y alternativas para reducir los costos operativos y gastos de mantenimiento de manera eficiente, como por ejemplo la reducción del consumo de energía. En primer lugar, esta tesis aborda el estudio de propuestas de conmutación óptica en redes troncales y la distribución de contenidos multimedia, particularmente la propuesta de Red de Conmutación Óptica de Ráfagas (OBS) desde el punto de vista de la Ingeniería Telemática. Por lo tanto, se estudia la arquitectura, protocolos y algoritmos de estas redes para hacer aportes a la evaluación del tráfico de vídeo y alternativas para ofrecer QoS y QoE para el tráfico multimedia. En este sentido, este trabajo contribuye con propuestas para la evaluación de la calidad mediante simulación utilizando trazas de transmisiones de vídeo reales, y propuestas de algoritmos de scheduling y ensamblaje de ráfagas ópticas para ofrecer QoS y QoE al tráfico de vídeo en las redes OBS. En segundo lugar, se estudian los patrones de tráfico de vídeo a nivel de aplicación y de red para caracterizarlos y realizar estudios de eficiencia energética y ahorro de consumo energético en las tarjetas de red utilizadas por servidores de vídeo en streaming y clientes que implementan el estándar IEEE 802.3az Energy Efficient Ethernet (EEE), tanto en redes gestionadas, como IPTV que implementa transmisión de vídeo RTP, como en redes no gestionadas, como Internet, que implementa el estándar Dynamic Adaptive Streaming over HTTP (DASH) para servicios Over The Top (OTT). Para alcanzar este objetivo, se implementan testbeds y se llevan a cabo experimentos para concluir cómo el tráfico de vídeo podría beneficiarse de la eficiencia energética y lograr una reducción a gran escala en el consumo de energía si el estándar EEE se implementa a nivel mundial en las tarjetas Ethernet de los dispositivos involucrados en la distribución de vídeo. El tema integrador que se aborda en esta tesis es la distribución de vídeo digital. Por lo tanto, este trabajo también presenta un estudio sobre la codificación y adaptación de transmisión de vídeo en redes IP, presentando aspectos relacionados con la evaluación de la calidad de vídeo y la evaluación de la calidad de transmisión de la red. Por ello, se proponen e implementan metodologías y bancos de pruebas que involucran la programación y ejecución de simuladores, scripts, algoritmos, software y, en general, frameworks para realizar estudios de transmisiones de vídeo en redes de telecomunicaciones, cubriendo el amplio espectro de las Comunicaciones Multimedia. Aquest estudi podria ser útil per emprendre altres estudis necessaris en la ràpida evolució tant de la demanda de serveis multimèdia com de les noves propostes de tècniques de distribució de continguts. / [EN] Multimedia content distribution, both over the Internet and over managed networks dedicated exclusively for this purpose, is having a greater share of total traffic and it is raising every day, especially the video streaming service. Therefore, research interest has arisen to maintain and improve the Quality of Service (QoS) in access and backbone networks, and to offer Quality of Experience (QoE) to end users, by adjusting parameters, configurations, protocols and algorithms to the network requirements regarding the video traffic high demand. The latter is carried out taking full advantage of the network resources at core and access networks such as bandwidth and total installed capacity, and at the same time, proposing efficient transportation alternatives for the aggregated video traffic without increasing services costs and alternatives to reduce operational and maintenance expenditures in an efficient manner, such as reducing the energy consumption. In the first place, this thesis deals with the study of optical switching proposals in backbone networks regarding multimedia content distribution, particularly studying the Optical Bursts Switching (OBS) Networks proposal from the Telematics Engineering point of view. Therefore, architecture, protocols and algorithms of these networks are studied to make contributions to the video traffic evaluation and alternatives to offer QoS and QoE for multimedia traffic. In this sense, this work contributes with proposals for the evaluation of quality through simulation using traces of real video transmissions, and optical burst assembly and scheduling algorithms proposals to offer QoS and QoE to video traffic in the OBS networks. Secondly, video traffic patterns at the application and network level are studied to characterize them and carry out studies on energy efficiency and energy consumption savings in the network cards used by streaming video servers and clients that implement the IEEE 802.3az Energy Efficient Ethernet (EEE) standard, both in managed networks, such as IPTV that implements RTP video streaming, and unmanaged networks, such as Internet which implements Dynamic Adaptive Streaming over HTTP (DASH) standard for Over The Top (OTT) services. To reach this goal, testbeds are implemented, and experiments are carried out to conclude how video traffic could benefit from the energy efficiency and achieve a large-scale reduction in energy consumption if the EEE standard is implemented word wide in the Ethernet cards of devices involved in video distribution. The integrating subject that is addressed in this thesis is the distribution of digital video. Therefore, this work also presents a study on the IP networks video transmission encoding and adaptation, presenting aspects regard the video quality evaluation and the network transmission quality assessment. Therefore, methodologies and testbeds are proposed and implemented that involves programming and execution of simulators, scripts, algorithms, software and, in general, frameworks to carry out studies for video transmissions in telecommunications networks, covering the broad spectrum of Multimedia Communications. Aerobic Exercise, Diet, and Neurocognition among Individuals with High Blood Pressure

<p>In addition to the adverse effects of high blood pressure (HBP) on cardiovascular disease, HBP is also associated with increased risk of stroke, dementia, and neurocognitive dysfunction. Although aerobic exercise and dietary modifications have been shown to reduce blood pressure, no studies have examined the effects of a combined aerobic exercise and dietary intervention on neurocognition among individuals with HBP, a group at elevated risk for neurocognitive dysfunction. As part of a larger investigation, the ENCORE study, this study examined the effects of dietary modification alone and combined with aerobic exercise on neurocognitive function among individuals with HBP. One hundred twenty five individuals with high normal blood pressure were randomized to an aerobic exercise and dietary modification group (DASH + WM), dietary modification alone (DASH-A), or a usual care control group. Participants completed a battery of neurocognitive tests assessing executive function and vigilance at baseline and again following the four month intervention. Following the intervention, participants in the DASH + WM and DASH-A groups exhibited modest improvements in neurocognitive function relative to controls, and these changes appeared to be mediated by improved cardiovascular fitness and weight loss. A combined aerobic exercise and dietary intervention improves neurocognitive function among individuals with HBP.</p> / Dissertation
