Virtual morality in the helping professions: Simulated action and resilience

Yes / Recent advances in virtual technologies have allowed the investigation of simulated moral actions in aversive moral dilemmas. Previous studies have employed diverse populations to explore these actions, with little research considering the significance of occupation on moral decision‐making. For the first time, in this study we have investigated simulated moral actions in virtual reality made by professionally trained paramedics and fire service incident commanders who are frequently faced with and must respond to moral dilemmas. We found that specially trained individuals showed distinct empathic and related personality trait scores and that these declined with years of experience working in the profession. Supporting the theory that these professionals develop resilience in moral conflict, reduced emotional arousal was observed during virtual simulations of a distressing dilemma. Furthermore, trained professionals demonstrated less regret following the execution of a moral action in virtual reality when compared to untrained control populations. We showed that, contrary to previous research, trained individuals made the same moral judgements and moral actions as untrained individuals, though showing less arousal and regret. In the face of increasing concerns regarding empathy decline in health care professionals, we suggest that the nature of this decline is complex and likely reflects the development of a necessary emotional resilience to distressing events.

Ethnography in Dementia Care Research: observations on ability and capacity

No / This case outlines the rationale and methods used when carrying out ethnographic fieldwork in a care home environment with research participants who were living with a dementia diagnosis. Although concerns had been raised at ethics approval about the use of such methods – visual ethnography in particular – we found that there were ethical benefits for the participants whose capacity for research participation, and for social participation generally, was, in every case, higher than anticipated at the outset. By comparison we found that formal methods for assessing ability to give informed consent often appeared to create excess disability, and to exacerbate ill-being for people with dementia. The case draws on specific examples to show how issues related to methods and to ethical conduct of research are frequently intertwined, and should be considered together rather than in isolation.

This thesis focuses on the dilemmas linked to the future of the affected sibling and the ensuing arguments, which in the future may help to siblings without disabilities in their decisions on the future sibling with a disability. The introduction defines basic fundamental concepts: combined disability, mental retardation and associated with the most common combinations of disability. Subsequently, the work is focused on the complexity and pitfalls associated with caring for people with multiple disabilities. On the sibling relationship, which can have a big impact on the future of siblings with disabilities, but also on the possible reasons causing disability and dilemmas associated with it based on the future care of siblings with disabilities. Furthermore dissertation discusses the possible variants of care that might siblings without disabilities to facilitate their decision-making and assist in the eventual home care. In the next part closer look at the particular dilemmas, with whom, according to interviews siblings without disabilities might face in the future. Individual dilemmas was determined on the basis of the research probe that howled implemented method of qualitative research through interviews with siblings of people with multiple disabilities. Towards the end of the thesis is to analyze the chosen dilemma, which is subject to the conclusion obsáhlému analysis from the perspective of two ethical theories - utilitarianism and deontology.

Detta arbete redogör för effekterna tillgången eller avsaknaden av omfattande karaktärsinformation samt information om beslutens direkta konsekvenser har på spelarens beslutsprocess i narrativa prohibition dilemman. Då det finns avsaknad av konsensus om dess effekter i ämnesområdet och en frånvaro av objektiva studier för att undersöka de aktuella ståndpunkterna, är studien nödvändig för att förbättra kunskapen inom området och för att generera objektiva resultat av dess effekter på spelare. För att undersöka problemformuleringen skapades två likadana textbaserade spel, där skillnaden mellan dem var att den ena gav spelaren omfattande karaktärsinformation och information om beslutens direkta konsekvenser i spelet narrativa prohibition dilemman, medan den andra artefakten saknade den informationen. Respektive artefakt testades på en separat testgrupp för att generera resultat om dess respektive effekt på testgruppen som spelade den. Resultaten var blandade i de olika testgrupperna. Det gick dock att finna tendenser i resultaten i de olika grupperna. När spelare inte hade omfattande karaktärsinformation samt information om beslutens konsekvenser, var de mer benägna att välja de beslut de kände var rätt utifrån deras egna värderingar när de inte kunde härleda beslutens konsekvenser. När de kunde härleda beslutens konsekvenser valde de ofta det val vilket gav dem den mest fördelaktiga utkomsten. När spelaren hade tillgång omfattande karaktärsinformation samt information om beslutens direkta konsekvenser motiverades deras beslut av att väga fördelarna och nackdelarna av respektive beslutsalternativ mot varandra, utifrån en strategisk och/eller moralisk basis, där de tog det alternativ som gav dem den mest optimala utkomsten. I fall där spelarna hade en emotionell investering i besluten valde de oftast oavsett mängden tillgänglig information vid beslutet att välja det beslut som kändes rätt. Något som även framkom i studien var att bättre resultat för att besvara problemformuleringen och explorativt undersöka ämnesområdet, skulle vara att göra ett liknande test av artefakterna på grupper av definierade spelartyper. Det skulle ge en bättre översikt av respektive utgångspunkts påverkan på specifika spelartyper och generera tydligare samt mer användbara resultat att besvara problemformuleringen med.

The purpose of this thesis is to investigate how international development organizations are working to communicate campaigns and photographs from one cultural context to another. Additionally, will challenges in making campaigns which are both ethical appropriate and engaging be highlighted and discussed in relation to today’s impatient media landscape where globalization and development are dominated by economic interests. This research follows the international development cooperation We Effect and explore their whole media production process while making external communication from the work in Kenya to the target group in Sweden. The researcher has done interviews with decision makers at the head office in Stockholm, regional communicators in Nairobi, independent photographers and farmers in the fields of Kenya, visible in We Effect’s campaigns in Sweden. Additionally, ethnographical observations and diary notes contribute to answer the question how international organizations are planning, creating and distributing ethical and engaging media about development organizations long-term development work. In theoretical discussions, anchored in concepts about global culture, cosmopolitanism and how to mediate distant others, together with previous literature by Lilie Chouliaraki, Roger Silverstone and Stuart Hall, will this research state that there are several challenges in communicating messages from one cultural context to another. The distance, both geographical and mental, makes it challenging for the media producers and spectators to understand the same message; this research states that both the media producers and spectators’ interpretations of photographs and messages are dependent on their cultural background.

O presente estudo teve por objetivos conhecer a percepção de enfermeiros sobre dilemas éticos existentes na assistência de enfermagem a pacientes terminais no contexto da UTI de um hospital geral do município de São Paulo e o que é considerado para a tomada de decisão frente a esses dilemas. O estudo foi realizado numa abordagem qualitativa, sendo utilizada a análise de conteúdo proposta por Bardin. Foram realizadas entrevistas com dez enfermeiros atuantes na UTI. Após análise, emergiram as seguintes categorias e subcategorias: Categoria 1– Fatores geradores de dilemas. Subcategorias - valores e crenças, responsabilidade profissional e a relação com a família do paciente terminal; Categoria 2- Fatores que influenciam a tomada de decisão Subcategorias - o esclarecimento da família e do paciente terminal, a postura profissional de enfermeiros e médicos, as dificuldades referidas por enfermeiros, valores e princípios dos profissionais; Categoria 3- Participação no processo de tomada de decisão. Subcategorias - o paciente terminal, a família do paciente terminal, a equipe médica e os enfermeiros.; categoria 4- Interferências na assistência de enfermagem ao paciente terminal; Subcategorias - a organização do cuidado e os valores pessoais; categoria 5- Considerações sobre a terminalidade e sentimentos relacionados ao paciente terminal e categoria 6 - Constatação da obstinação terapêutica. O estudo mostrou que os enfermeiros deparam-se com dilemas éticos decorrentes da internação de pacientes terminais em UTI gerados por diversos fatores: a diversidade de valores; a presença dos pacientes terminais na UTI; os princípios religiosos; os valores pessoais; as incertezas a respeito da terminalidade do paciente e dos limites de intervenção para prolongar a vida; a responsabilidade profissional relacionada à sobrecarga de tarefas; a discordância de tomadas de decisão; a necessidade de priorizar o atendimento entre pacientes terminais e outros com possibilidades terapêuticas; a não aceitação do processo de morte pela família do paciente e a falta de esclarecimento sobre o seu quadro clínico. Foi constatado que os enfermeiros não participam ativamente das tomadas de decisão relacionadas às condutas com o paciente terminal, mas consideram importante a participação da família, o respeito pelo desejo do paciente e a participação de outros profissionais nesse processo. Além disso, para tomar decisão frente aos dilemas éticos, ele leva em conta os seus valores, a ética profissional, a empatia e o diálogo com os colegas. / The aims of the present study are twofold: to know the nurses perception about the existing ethical dilemmas during nursing assistance to terminal patients in the intensive care unit (ICU) context of a general hospital in São Paulo city, and what is considered in the decision-making when facing those dilemmas. The study was carried out with a qualitative approach, utilizing the content analysis proposed by Bardin. Interviews were made with ten practicing nurses in the ICU. After analysis, there emerged the following categories and subcategories: Category 1– Dilemma-generating factors. Subcategories - values and beliefs, professional responsibility, and the relation with terminal patient’s family; Category 2- Factors that influence the decision-making. Subcategories - the explanations to the family and to the terminal patient, the professional posture of nurses and doctors, the difficulties referred to by nurses, and the values and principles of these professionals; Category 3- Participation on process of decision-making. Subcategories - the terminal patient, the family of the terminal patient, the medical team, and the nurses; Category 4- Interferences in nursing support to the terminal patient; Subcategories - care organization and personal values; Category 5- Considerations on the terminality and feelings related to the terminal patient, and category 6 – therapeutic obstinacy verification. The study showed that the nurses face ethical dilemmas resulting from the terminal patient’s admission in the ICU generated by several factors: diversity of values; presence of terminal patients in the ICU; religious principles; personal values; uncertainties regarding the terminality of the patient and the limits of intervention to prolong life; the professional responsibility related to the overload of tasks; decision-taking discord; the necessity to prioritize care between terminal patient and others with therapeutical possibilities; non-acceptance of the death process by the patient’s family, and the absence of explanations about their clinical status. It was verified that the nurses do not participate actively in the decisions for dealing with the terminal patient, but they consider important the family participation, the respect for the patient’s wishes and the participation of other professionals in the process. In addition to that, they take into account their values, professional ethics, the empathy, and the dialogue with colleagues when taking decisions in the face of ethical dilemmas.

Dilemma situations are illustrated by case studies. The selected dilemma is subjected to an analysis using the ethical theory of I. Kant. The thesis seeks to thoroughly reflect the principles of I. Kant's ethical theory, which can be helpful in solving this dilemma that arises when working with a family in which child violence is present.

The Man Behinf the Mask: A Principal's Search For a Moral Leaderhip Purpose

The purpose of this autoethnographic narrative inquiry was for the researcher to describe and explain how he discovered, constructed, and refined his sense of moral purpose as a principal during his seven-year tenure at Orange Pines Middle School. He inductively analyzed and reflected primarily on self-authored texts tied to critical professional ethical dilemmas so as to discover emergent themes, patterns, insights, and epiphanies in the development of his persona as a morally directed school leader. He then analyzed and reflected on how he applied those defined values in interactions with groups of teachers to design and implement elements of school reform. He re-created these critical events through descriptive vignettes in which he captured personal and social implications of the experiences using Clandinin and Connelly's model of three-dimensional narrative space. In this study the researcher probed especially problematic ethical dilemmas he experienced while working as principal. He viewed the events through the multidimensional ethical frameworks of care, critique and justice of Starratt; the ethic of community described by Furman; and the ethic of the profession, posited by Shapiro and Stefkovich. Included is a discussion of moral purpose by Fullan and Sergiovanni, ethics by Begley, Senge, and others, leadership theories, and perspectives regarding interpersonal conflicts between principals and their staff. The researcher found the ethics of care, justice, critique, community, and the profession provided a useful framework for his professional reflections. He was able to describe and capture the tensions within the dilemmas through the specific language utilized by Starratt, Furman, and Shapiro and Stefkovich to analyze and understand the issues packed within each dilemma. Through the application of these frameworks he determined that his moral purpose has been to approach the position of school leadership with a combination of compassion and justice, in order to establish a collaborative and synergistic school community that works for the greater good of students. The study calls for more autoethnographic research into the dilemmas administrators teachers face in their daily practice, arguing that the best way to improve public education in this era of intense scrutiny and accountability is through the qualitative analysis of individual cases. The author places his particular constructivist approach to autoethnographic narrative inquiry within the broader philosophical background of qualitative research. This study contributes to the literature by showing focused insights into how representative ethical conflicts and dilemmas school leaders face during their daily practice can shape and guide their moral pursuit of effective school reform. It also shows ways that theoretical knowledge can inform professional practice.
