A random sample of 365 students attending a university in northern Texas returned a mailed questionnaire measuring the students' knowledge of Acquired Immuno-Deficiency Syndrome (AIDS), their perceived susceptibility to AIDS, and their sexual practices during the last year. The students had high knowledge levels about AIDS in general. However, 70% did not worry about getting AIDS. Collectively, there was a display of concern until it became a personal issue. The students (59%) saw themselves as less likely than most people to get AIDS. Although 70% of this sexually active group did not use condoms, they still did not perceive themselves as susceptible to AIDS.

The Effects of Red Meat Consumption Within a Healthy Dietary Pattern on Cardiovascular Risk: the Importance of Gut Microbiota

<p>Accumulating evidence from randomized-controlled, full-feeding trials suggests neutral to beneficial effects of consuming lean and unprocessed red meat within a healthy dietary pattern on cardiovascular health. It is unclear how red meat interacts with the dietary pattern and the host in mediating its health effects. The gut microbiome provides a novel perspective in understanding the diet-host relationship for its abilities to metabolize dietary components including those within red meat and influence cardiovascular health. Prior to starting our study, we identified a lack of evidence in the English literature for the effects of consuming lean red meat, in unprocessed or processed forms, on gut microbiota in the context of a controlled healthy dietary pattern. Our findings presented in this dissertation demonstrate differential effects of consuming unprocessed versus processed red meat on gut microbiota. Consistent with previous research, we observed cardiovascular improvements induced by the healthy dietary pattern independent of lean red meat intakes among young adults with apparently healthy cardiovascular profiles. Importantly, with three intervention periods, we found consistent and reproducible changes in both gut microbiota and cardiovascular risk factors when repeatedly adopting and abandoning a healthy dietary pattern. Collectively, findings in this dissertation highlight the importance of gut microbiota in potentially mediating or responding to diet-induced cardiovascular improvements. Future research should investigate the changes in the function of gut microbiota induced by healthy dietary patterns containing red meat. Studies assessing the unique food matrix of processed versus unprocessed animal- or plant-based protein-rich foods are also warranted. Additionally, dietary strategies should focus on promoting healthy dietary modifications and enhancing dietary adherence for long-term cardiovascular benefits. </p>

Knowledge, attitudes & practices towards HIV/AIDS among former mineworkers of Transkei

Thesis (MPhil)--Stellenbosch University, 2005. / ENGLISH ABSTRACT: Mineworkers in South Africa are in abundance as mining is the main occupation especially in the former black homeland. These mineworkers retrenched or retire, and returned to their homeland. Transkei is a former black homeland where abundant retired mineworkers reside to their destiny. Migrancy, which for century has been such a conspicuous feature of the South African labour system, and thought to be a significant contributor to the spread of HIV/AIDS (Lurie M, 2000). The migrant labour system in Transkei region was attributed very extensively to contagious disease especially related with sexually transmitted diseases (STI) like HIV/AIDS. It is estimated that 2 million of the 5 million black mineworkers in South Africa at the time were migrant labourers. Majority of them are from the Transkei region. The ex-mineworkers in this study group are between 41 and 78 years of age (mean 55 years). They have serviced in the mines from 5 to 48 years (mean 20 years). All are married, and having children between 1 and 10 ((mean 6). Fifty seven percent ex-mineworkers did not answer about their number of sexpartners. Only 43% mentioned that they have one partner. Only 31% mineworkers were consuming alcohol. Of them 9% were consuming everyday, and 18% twice in a week. In this study, HIV/AIDS related knowledge, attitudes and behaviour of exmineworkers, living in the remote rural areas of Transkei region of Eastern Cape, were elicited by sending questionnaires to them. Only 19.9% have replied back. There is absolute lack of knowledge in 13.6%, and negative attitude in 27% of the ex-mineworkers. Risks of unsafe sex have been observed among 69%. Of them, 48% have mentioned that they have trust in their wives. Urinary tract symptoms were indicated by 18%, and HIV positivity was reported by 4.5% of mineworkers. There is suicidal tendency (life is not worth living) was found in 22.7% of ex-mineworkers, and a majority (16%) mentioned that it is related with their sickness. Majority of ex-mineworkers have good knowledge about HIV/AIDS, and positive attitude for the survivals. They have also been practicing sex without condoms, but they have provided reasoning to be a faithful to their partners. There is a high level of awareness and a positive attitude towards HIV/AIDS individuals. It was observed that there is less risk taking sexual behaviour among exmineworkers. / AFRIKAANSE OPSOMMING: 'n Groot gedeelte van Suid-Afrikaanse mynwerkers is uit die Transkei gebied afkomstig. Daar word konserwatief beraam dat tussen twee en vyf miljoen mynwerkers jaarliks tussen hulle werkplek en die myne migreer en dat hierdie migrasie ingrypende inplikasies vir die verspreiding van die MI-virus het. Die doel van hierdie studie is om die kennis, houding en gedrag van mynwerkers ten opsigte van MIV/Vigs by 'n steekproef van 198 voormalige mynwerkers in die Transkei te toets. Die inligting is deur middel van 'n vraelys en onderhoude gedoen en daar word tot die gevolgtrekking gekom dat die kennis van Vigs en die voorkoming daarvan besonder goed is by hierdie steekproef. Die implikasies van die bevindinge van die studie word uitgespel en voorstelle vir verdere studies word aan die hand gedoen.

Technical and Vocational Education and Training (TVET) colleges have become institutions of choice in the Department of Higher Education and Training (DHET) attracting youth between 15 and 24 years of age. College campuses are an opportune space for mixing of students with Grade 10 equivalence up to students with post-matriculation level. Students who enrol at TVET colleges for the first time are vulnerable to Human Immunodeficiency Virus (HIV) and Acquired Immuno Deficiency Syndrome (AIDS) since they mingle with students of different ages and diverse experiences. Sometimes they can be taken advantage of as the environment is inviting for transactional sex practices, casual and multi-partner sexual relationships with benefactors / “blessers”, promising them the world as angel babies, sugar pups and tuition babies in the sugar bowl of dating scene, hence risky sexual behaviours make college first year students vulnerable to HIV and AIDS. A qualitative research approach was followed underpinned by constructivism as a research paradigm. The theoretical framework followed in this study was the Social Cognitive Theory (SCT). A purposive sampling technique was used and six students who had enrolled in a TVET college as first year students for National Certificate Vocational (NCV) level 2 and were sexually active and willing to talk about the risky sexual behaviours they engaged in, participated during focus group interviews. Three students selected from the six were interviewed individually. The thematic analysis approach was used to identify patterns and themes which were considered for major findings. Findings of the study revealed that TVET college students who are enrolled as first year students in the NCV programme engage themselves in risky sexual behaviours such as transactional sex, casual and multi-partner sexual relationships, not using condoms, putting themselves at risk of contracting HIV. Among factors that were found to be contributing to risky behaviours were that students are away from the supervision of parents, peer pressure, partying, poverty, sugar daddy and sugar mummy (cougar) / Ben 10 with dating life style. / Psychology of Education / M. Diversas evidências comprovam que a obesidade está associada a alterações estruturais e funcionais do coração em modelos humanos e animais. Outros estudos recentes também demonstram que a obesidade humana está associada com alterações na função e na estrutura vascular, especialmente em grandes e médias artérias. Estudos epidemiológicos têm confirmado que a obesidade é um fator de risco significativo para o aparecimento de proteinúria e de doença renal terminal em uma população normal. Com o objetivo de determinar as alterações morfológicas relacionadas ao remodelamento cardíaco, vascular e renal em um modelo experimental de obesidade induzida pelo glutamato monossódico (MSG) e os efeitos da metformina sobre estes achados, foram estudados 25 ratos divididos em cinco grupos: controle com 16 e 22 semanas (CON-16 e CON-22); obeso com 16 e 22 semanas (MSG-16 e MSG-22) e obeso + metformina (MET-22) 300mg/Kg/dia por via oral. A caracterização da resistência à insulina foi feita através da medida da insulina plasmática e cálculo do índice de HOMA-IR. As análises morfológicas e quantificação do colágeno miocárdico foram feitos pelo sistema de imagem Image Pro Plus analysis. A pressão arterial sistólica foi levemente maior no grupo MSG-22, adquirindo significância estatística quando comparada com o grupo MSG-16 (1222 vs 1082 mmHg, p<0,05). Por outro lado, o grupo MET-22 mostrou níveis mais baixos de pressão arterial (1181 mmHg), sem alcançar diferença significativa. No grupo de animais obesos, foi observado aumento na relação média-lumen com 16 semanas (39,93,7 vs 30,22,0 %, p<0,05) e com 22 semanas (39,81,3 vs 29,51,2%, p<0,05), que foi reduzida com o uso da metformina (31,50,9%). O depósito de colágeno na área perivascular no ventrículo esquerdo foi significativamente maior no grupo MSG-22 (1,390,06 vs 0,830,06 % no CON-22, p<0,01), sendo atenuado pela metformina (1,020,04%). No rim, a área seccional transversa das arteríolas intrarrenais foi semelhante entre os grupos (18,52,2 no CON-16; 19,93,7 no MSG-16; 18,93,1 no CON-22; 21,81,5 no MSG-22; 20,21,4 no MET-22). Foi observado aumento da área glomerular no grupo MSG-22 (141,34,5 vs 129,50,5 m2), mas sem significância estatística. Em conclusão, nos ratos com obesidade induzida pelo MSG, com resistência à insulina, as alterações cardíacas foram mais proeminentes do que as alterações renais. No coração foram observados sinais de remodelamento vascular hipertrófico nas pequenas artérias intramiocárdicas e evidências de fibrose miocárdica mais proeminente na área perivascular, alterações que foram, pelo menos parcialmente, atenuadas com o uso de metformina durante seis semanas, mostrando que esta droga pode ser benéfica na prevenção de complicações cardíacas, vasculares e renais associadas com a obesidade. / Many evidences show that obesity is associated to structural and functional changes in the heart of human and animal models. Recent studies also show that human obesity is associated with vascular structural and functional modifications, specially at large and medium-sized arteries. Epidemiological studies have confirmed that obesity is a significant risk factor for the development of proteinuria and end-stage renal disease in a normal population. With the objective to determinate morphological changes related to cardiac, vascular and renal remodeling in an experimental model of monosodium glutamate (MSG)-induced obesity and the effect of metformin at this finding. Twenty five rats were studied and divided into five groups: control with 16 e 22 weeks (CON-16 and CON-22); obese with 16 and 22 weeks (MSG-16 e MSG-22), and obese + metformin (MET-22) 300mg/Kg/day per oral. The characterization of insulin resistance was done through measurement of plasma insulin and calculation of HOMA-IR index. The morphological analysis and the quantification of myocardial collagen were carried out by Image Pro Plus analysis system. The systolic blood pressure was slightly higher in MSG-22 group, reaching statistical significance when compared to MSG-16 group (1222 vs 1082 mmHg, p<0.05). On the other hand, the MET-22 group demonstrated lower blood pressure levels (1181 mmHg), without reaching statistical difference. The obese animals presented increase in media-to-lumen ratio with 16 weeks (39.93.7 vs 30.22.0 %, p<0.05) and with 22 weeks (39.81.3 vs 29.51.2%, p<0.05), which was reduced with use of metformin (31.50.9%). The collagen deposition in perivascular area of left ventricle was significantly greater in MSG-22 group (1.390.06 vs 0.830.06 % in CON-22, p<0.01), and attenuated by metformin (1.020.04%). In the kidney, the media cross-sectional area of intrarenal arterioles was similar among the groups (18.52.2 in CON-16; 19.93.7 in MSG-16; 18.93.1 in CON-22; 21.81.5 in MSG-22; 20.21.4 in MET-22). An increase of glomerular area was observed in MSG-22 group (141.34.5 vs 129.50.5 m2), but without statistical significance. In conclusion, rats with MSG-induced obesity and insulin resistance presented more pronounced cardiac changes than renal alterations. In the heart, there were evidences of hypertrophic vascular remodeling were observed in intramyocardial small arteries and perivascular fibrosis. These findings were, at least partially, attenuated by metformin for six weeks, suggesting that this drug may be beneficial for prevention of cardiac, vascular and renal complications associated with obesity.

HIV, AIDS and gender issues in Indonesia : implications for policy : an application of complexity theory

The aim of the study was to offer solutions for the enhancement of Indonesia’s HIV and AIDS policy and to suggest future possibilities. In the process, the gendered nature of the epidemic was explored. In light of the relatively lower rates of employment among Indonesian women, this study also sought to gain insights into the possible reasons for many women appearing to be attached to domesticity. In the first phase of the study, interviews with stakeholders in HIV and AIDS prevention were conducted, followed by a Delphi exercise involving 23 HIV and AIDS experts. In the second phase, 28 women from various ethnicities were interviewed, including those in polygamous and contract marriages. The overall results were interpreted through the lens of complexity theory. Fewer than half of the proposed objectives were approved by the experts in the Delphi round. These were interventions mainly aimed at the risk groups while most objectives relating to education about HIV and AIDS and safer sex for the general public failed to obtain consensus. Reasons for the lack of consensus were differences in perceptions associated with human rights, moral reasoning, the unfeasibility of certain statements and personal conviction about the control of the epidemic. Emphasis on men’s and women’s innate characteristics; men’s role as breadwinner; women’s primary role as wife, mother and educator of their children; and unplanned pregnancies emerged as major themes from the qualitative phase. While the adat and Islam revival movements may have endorsed the ideals of the New Order state ideology, Javanese rituals regarded as violating Islam teachings were abandoned. Ignorance about safer sex and HIV and AIDS was also established. Interpretation of the results through the lens of complexity theory revealed that the national HIV and AIDS policy needs to encompass interventions for the general population, which would include comprehensive sex education in schools and media campaigns focusing on women. Masculinity ideals and HIV prevention: an analysis of perceptions among male graduates of the Tavern Intervention Programme (TIP) in Gauteng

In many South African communities, socially constructed masculinity norms that promote unequal gender relations and high risk sexual behaviour are key contributing factors to HIV transmission. Following a qualitative approach, using in-depth interviews, this study engaged graduates of the Tavern Intervention Programme (TIP) in Gauteng to explore and describe their perceptions of traditional and modern masculinities, as well as their experience of the TIP. The findings indicated that, while there are differences between traditional and modern men, several masculinity practices, such as unequal gender relations, inconsistent use of condoms, infrequent accessing of HIV testing opportunities and entitlement to multiple partners endure as potential barriers to HIV prevention. Additionally, peer groups reinforce and reward HIV risk behaviour among modern men. Participants did, however, report changes in perceptions and behaviour regarding gender relations and HIV prevention as a result of their participation in the TIP. This study concluded that the role the TIP played in providing these men with an environment where alternative masculinity behaviour could be explored and supported was of particular value in terms of changes in their perceptions of masculinities, gender relations and HIV prevention. / Sociology / M.
