INTRODUÇÃO: O presente trabalho teve como objetivo elaborar análise custoefetividade do esquema de quimiorradioterapia com cisplatina (estratégia 2) comparado ao tratamento radioterápico (estratégia 1) para pacientes portadores de CECCP localmente avançado não elegíveis para tratamento cirúrgico. MÉTODOS: levantamos dados prospectivos de 33 pacientes na estratégia 2 e dados retrospectivos de 29 pacientes tratados no HC-FMUSP e Hospital A.C. Camargo, (estratégia 1). Consideramos a tabela de reembolso do Sistema Único de Saúde (perspectiva SUS) e custos do HC-FMUSP com honorários profissionais, medicamentos, demais insumos e depreciação de equipamentos (perspectiva Institucional). A medida de efetividade foi 1 ano de vida ganho, livre de progressão da doença (SLPD). Calculamos a Razão Incremental Custo Efetividade (RICE). RESULTADOS: 31% dos pacientes da estratégia 1 e 58% na estratégia 2) tiveram 1 ano de SLPD. Na perspectiva SUS o custo total por paciente na estratégia 1 foi de R$ 2.798,52 e R$ 4.938,11 na estratégia 2. Na perspectiva institucional os custos foram R$ 26.798,52 e R$ 5.040,79, respectivamente. A RICE na perspectiva SUS foi de R$ 7.924,00 reais por ano de vida ganho e R$ 8.912,71 na perspectiva institucional. CONCLUSÃO: nas duas perspectivas a estratégia 2 se mostrou custo-efetiva, sendo o custo incremental considerado aceitável, segundo diretrizes do Banco Mundial. / INTRODUCTION: The present study aims to conduct a cost-effectiveness analysis comparing chemoradiotherapy with cisplatine and radiotherapy alone, to treat inoperative advanced head and neck cancer. METHODS: we collected data from 29 patients in a prospective study on chemoradiotherapy with cisplatin, conducted at Hospital das Clínicas HC-FMUSP,(strategy 2). For strategy 1, we collected retrospective data of 33 patients treated with radiotherapy at HC-FMUSP and Hospital A.C. Camargo. We considered only direct costs (personnel, drugs, material and equipment depreciation). We considered, the National Health Service (SUS) reimbursement parameters as the National Security System perspective, and HC-FMUSP costs as the institutional perspectives. We measured effectiveness as one year of diseasefree life gained. We collected costs and effectiveness data and calculated the cost-effectiveness incremental ratio ICER, which expresses additional costs per life year gained, in strategy 2, compared to strategy 1 RESULTS: 31.0% of the patients treated in strategy 1 lived more than 12 months, without disease progression, compared to 58.0% of the patients in strategy 2. According to SUS perspective, the total cost per patient in strategy 1) is R$ 2.798,52 and R$ 4.938,11 in strategy 2. Considering the institutional perspective, total costs are R$ 2.634,36, and R$ 5.040,79 respectively. In SUS perspective, the ICER ratio of strategy 2 compared to 1 is R$ 7.924,00 per lifes year gained. In the institutional perspective, ICER is R$ 8.912,71. We conducted a one way sensitivity analysis to verify our calculations. CONCLUSION: Chemoradioterapy with cisplatin proved more cost-effective than radiotherapy. Using the World Bank guidelines, wich considers the countries GDP per capita an acceptable cost per additional year of life (R$ 12.491,00 in 2006), the incremental cost of both is acceptable.

Cette thèse se concentre sur la compréhension et l'analyse de l'efficacité technique des banques islamiques et conventionnelles. La thèse est divisée en deux parties : dans la première partie, l'étude fournit des illustrations théoriques utiles pour une comparaison détaillée entre les banques conventionnelles et islamiques, couvrant les différences en terme du système bancaire, la structure organisationnelle, la création monétaire, la structure du bilan, le rôle de la banque centrale et leurs impacts sur les cycles économiques. La deuxième partie se focalise sur le calcul des scores d'efficacité technique, pour essayer de comprendre la répartition des scores sous trois hypothèses principales : Constant Return to Scale (CRS), Variable Return to Scale (VRS), et Scale Efficiency (SE). La deuxième partie comprend également une analyse de régression pour explorer les différentes variables financières qui affectent l'efficacité technique, et qui expliquent la différence entre les scores des deux catégories de banques. L'échantillon de cette étude contient 1 425 banques commerciales et 85 banques islamiques tirées de la base Bankscope. La méthode utilisée dans la première partie combine la Data Envelopment Analysis (DEA) et l'Analyse Descriptive, tandis que la deuxième partie est réalisée selon la régression Tobit. Les résultats de cette étude montrent que les scores de type Scale Efficiency sont préférables pour représenter l’efficacité, par rapport aux scores CRS et VRS. L'étude montre également que les banques islamiques sont statistiquement plus efficaces que les banques conventionnelles. De plus, l'étude a révélé que le rendement de l’actif moyen, le ratio prêt net / actif total, la marge nette d'intérêts et le ratio des capitaux propres par rapport au total des actifs ont une incidence importante sur le rendement. / This thesis focuses on two parts: providing theoretical understanding and investigation of technical efficiency score in Islamic and conventional banks. This study is divided into two parts. In the first part, the study provides useful theoretical illustrations regarding the comparison between conventional banks and Islamic banks in greater details, covering the difference in banking scheme, organization structure, money creation, balance sheet structure, the function of a central bank, and the difference on impact to economic system and economic cycles. The second part is the calculation of technical efficiency score to understand how the distribution of score across banks under three primary assumptions: Constant Return to Scale, Variable return to Scale and Scale Efficiency. The second part involves regression analysis in understanding financial variables affecting technical efficiency and how different the score between Islamic and conventional banks. The sample of this study is 1,425 commercial banks and 85 Islamic banks (Data downloaded from Bankscope Database). The method used in the first part is Data Envelopment Analysis and Descriptive Analysis, while the method used in the second part is Tobit regression. The findings in this study shows that Scale Efficiency score is better to represent efficiency score compared to Variable Return to Scale and Constant Return to Scale. The study also shows that Islamic banks are statistically more efficient than conventional banks. Moreover, the study found that return on average asset, net loan to total asset ratio, net interest margin, and equity to total asset ratio significantly affect the efficiency score.

El presente documento se centra en el análisis de la eficiencia técnica de la infraestructura vial en las regiones de Perú, durante el periodo comprendido entre 2007 y 2018. La metodología se compone por dos etapas.
La primera tiene por objetivo determinar un índice que recoja la eficiencia del gasto público en infraestructura vial, sobre la base del análisis de eficiencia estocástica. Mediante el cual se encuentra que, un mayor gasto a nivel regional no necesariamente se refleja en una mejora en la infraestructura vial. Así, existen regiones con un menor monto destinado al gasto en carreteras, pero que cuentan con un mejor desempeño en la ejecución. En la segunda etapa, se busca asociar este indicador de eficiencia con el crecimiento de las actividades económicas a nivel regional, mediante la aplicación de Método Generalizado de Momentos. Los resultados muestran que el índice de eficiencia explica en gran parte el crecimiento de las actividades económicas. / The literature shows that the gap in road infrastructure can represent a limitation to economic growth, especially at the regional level. This article analyzes the technical efficiency of the road infrastructure in the 24 regions of Peru for the period between 2007-2018. The methodology consists of two stages. The first one aims to determine an index that collects the efficiency of public spending on road infrastructure, based on the Stochastic Efficiency Analysis. For the construction of the efficiency index, the following are identified as outputs: (i) Total National Length, (ii) Total Departmental Length and (iii) Total District Length. The main input used was spending on regional road infrastructure. The results suggest that higher spending at the regional level does not imply an improvement of the roads. There are regions with a lower amount earmarked for public spending on road infrastructure, but that have a better performance in execution. In the second stage, we seek to associate this efficiency indicator with the growth of economic activities at the regional level, through the application of Generalized Least Squares. The dependent variable was the domestic product and the explanatory variables are made up of variables such as the efficiency index, electricity consumption, hectares worked, educated EAP, mobile service lines, among others. The results confirm that the efficiency index largely explains the growth of economic activities. / Trabajo de investigación

The provision of local public goods and demographic changeMontén, Anna 15 May 2012 (has links)
The main contribution of this thesis is a comprehensive analysis of the influence of changes in the population structure on local communities, in particular with respect to the provision of publicly provided goods. The focus is placed on the consequences of two of the major processes of demographic change, namely aging and shrinking. The three main chapters of this contribution consider the effects at the local level from both a theoretical and an empirical perspective. The first model focuses on the influence of population aging on the provision of local publicly provided goods, when the young population may relocate. When aging advances, gerontocracies and social planners substitute publicly provided goods aimed at the mobile young for publicly provided goods for the elderly. However, due to fiscal competition, gerontocracies will provide even more of the publicly provided good for the young than the social planner. The second model considers in a two-period setting, the interaction of a shrinking population when the investments made by the previous generation are long lived. The laissez-faire and welfare maximizing outcomes are computed for two cases; first with no costs of upkeep and second for the case when costs of upkeep accrue. A comparison of the solutions shows that public provision for the first generation is inefficiently low in laissez-faire when there are no costs of upkeep. However, if costs of upkeep accrue, the laissez-faire outcome for the intergenerational publicly provided good may be too high. Chapter four contains an empirical analysis. In a two-stage analysis the efficiency of the provision of child care services in municipalities is evaluated in the German State of Saxony. First, the results of the Data Envelopment Analysis (DEA) show substantial efficiency differences; the median municipality is up to 28% inefficient. In a second stage bootstrapped truncated regression, determinants of the inefficiency are identified. Explanatory variables such as an uncompensated mayor or a larger share of over 65-year-olds significantly increase inefficiency.

Energy Measurements of High Performance Computing Systems: From Instrumentation to AnalysisIlsche, Thomas 31 July 2020 (has links)
Energy efficiency is a major criterion for computing in general and High Performance Computing in particular. When optimizing for energy efficiency, it is essential to measure the underlying metric: energy consumption. To fully leverage energy measurements, their quality needs to be well-understood. To that end, this thesis provides a rigorous evaluation of various energy measurement techniques. I demonstrate how the deliberate selection of instrumentation points, sensors, and analog processing schemes can enhance the temporal and spatial resolution while preserving a well-known accuracy. Further, I evaluate a scalable energy measurement solution for production HPC systems and address its shortcomings.
Such high-resolution and large-scale measurements present challenges regarding the management of large volumes of generated metric data. I address these challenges with a scalable infrastructure for collecting, storing, and analyzing metric data. With this infrastructure, I also introduce a novel persistent storage scheme for metric time series data, which allows efficient queries for aggregate timelines.
To ensure that it satisfies the demanding requirements for scalable power measurements, I conduct an extensive performance evaluation and describe a productive deployment of the infrastructure.
Finally, I describe different approaches and practical examples of analyses based on energy measurement data. In particular, I focus on the combination of energy measurements and application performance traces. However, interweaving fine-grained power recordings and application events requires accurately synchronized timestamps on both sides. To overcome this obstacle, I develop a resilient and automated technique for time synchronization, which utilizes crosscorrelation of a specifically influenced power measurement signal. Ultimately, this careful combination of sophisticated energy measurements and application performance traces yields a detailed insight into application and system energy efficiency at full-scale HPC systems and down to millisecond-range regions.:1 Introduction

Although the German framework of incentive-based energy regulation appears to be well-defined
because it draws from international experience and eliminates several present drawbacks existing with
revenue-cap regulation doubts of industry and politicians concerning the application of benchmarking
techniques and more importantly, the future industry structure, remain. The regulation scheme is based
on experience with energy sector reforms in the US due to it being a precursor in this regard, e.g. by
introducing incentive-based regulation as early as 1994 and publishing the relevant data in a very
detailed way.
This thesis therefore brings together the two issues of certain efficiency measurement problems and
the industry restructuring in Germany and the US in order to contribute to the current discussion of
robust benchmarking and to provide political implications related to the industry’s structure.
Therefore, the application of DEA and SFA constitutes the heart of the whole study. These techniques
are applied on German electricity DSOs as well as on data for US electricity DSOs and US gas TSOs.
The application to US data can then be used to derive information about business strategies and their
success in a sector that is being restructured. The US experience will lead to relevant German policy
implications with respect to the future structure of the energy industry in Germany. / Der deutsche Regulierungsansatz erscheint im Allgemeinen als ausgewogen, da er zum einen internationale Erfahrungen mit einbezieht und zum anderen bestehenden Schwächen der Erlös-Obergrenzenregulierung berücksichtigt. Dennoch bestehen Zweifel seitens Industrie- und Politvertretern bezüglich der Anwendung von Benchmarking-Techniken und noch bedeutsamer der zukünftigen Industriestruktur. The Role of User Interface Design in a Digital Document ReaderPham, Nam, Zhao, Yurou January 2018 (has links)
User Interface Design is the center concept of this thesis. It was used throughout this thesis in order to remedy the design issue of a product called Loredge. Loredge was a digital document reader that contained a Library page with a very simple design. The ultimate goal was to create design suggestions that provides users with a lot of convenience in navigating items for Loredge’s Library page. The solution was proposed by introducing the design process and the concept of Human-Centered Design, which were the essentials in compliance with Loredge users’ desires. The end results demonstrated some interesting facts about users that researchers did not think about at the beginning, which was very helpful to recognize what users want the most for themselves. Despite some predictable answers, the outcome indeed gave the problem a potential solution. The solution was to introduce categories/groups to users and design for menus and buttons that helps user filter out the items they want to navigate. This was implemented to digital mock-ups using a wireframing tool called Axure. The mock-ups could be found within this degree project. / Användargränssnitt är centrumkonceptet för denna avhandling. Det användes under hela denna avhandling för att avhjälpa designproblemet av en produkt som heter Loredge. Loredge var en digital dokumentläsare som innehöll en bibliotekssida med en mycket enkel design. Det ultimata målet var att skapa designförslag som ger användarna mycket bekvämlighet i navigeringsdokument för Loredges bibliotekssida. Lösningen föreslogs genom att introducera designprocessen och konceptet Human-Centered Design, vilka var nödvändiga i enlighet med Loredge-användarnas önskemål. Slutresultatet visade några intressanta fakta om användare som forskare inte tänkte på i början, vilket var till stor hjälp för att känna igen vad användarna vill ha mest för sig själva. Trots vissa förutsägbara svar gav utfallet verkligen problemet en potentiell lösning. Lösningen var att introducera kategorier / grupper till användare och design för menyer och knappar som hjälper användaren att filtrera ut de dokument som de vill navigera. Detta implementerades för digital mock-ups med hjälp av ett wireframing verktyg som heter Axure. Mock-ups kunde hittas inom detta examensarbete.
