CoordenaÃÃo de AperfeiÃoamento de Pessoal de NÃvel Superior / Estudos recentes tem apontado um efeito deletÃrio da hiperglicemia persistente em pacientes submetidos a cirurgia cardÃaca. O controle glicemico estrito abaixo de 110 mg/dl com insulina tem demonstrado exercer um efeito antiinflamatÃrio em pacientes crÃticos,melhorando o prognostico da doenÃa. A glutamina, um aminoÃcido condicionalmente essencial durante o estresse, promove a utilizacao de glicose e aumenta a sensibilidade a insulina em pacientes criticos. Objetivou-se, no presente trabalho, estudar os efeitos de doses nutracÃuticas de L-alanil-glutamina (L-Ala-Gln) administrada no perÃodo prÃ-operatÃrio, associada ao uso intra e pÃs-operatÃrio da infusÃo de insulina. Vinte e dois pacientes adultos (idade media: 63,4 anos), candidatos a cirurgia de revascularizacao do miocÃrdio com circulaÃÃo extracorporea (CEC), foram randomizados em 2 grupos: Grupo salina (GS) e Grupo L-Ala-Gln (GG). Receberam por via endovenosa uma infusÃo de 1.000 ml de soluÃÃo salina (GS) ou 250ml de uma soluÃÃo de L-Ala-Gln a 20% diluÃda em 750 ml de soro fisiolÃgico, nas 3 horas que precederam a intervenÃÃo cirÃrgica. A insulina foi administrada ininterruptamente, durante a realizacao do procedimento cirÃrgico, a ambos os grupos. Amostras de sangue foram coletadas antes e apos a infusÃo da soluÃÃo salina ou da L-Ala-Gln [tempo basal (T-0) e pre-operatorio (T-1) respectivamente]; antes e apos a circulaÃÃo extracorporea (CEC) e no final do perÃodo operatÃrio (T-2); e 12 e 24 horas no perÃodo pÃs-operatÃrio (T3). Foram analisadas as seguintes variÃveis: Insulina, peptÃdeo C, creatinofosfoquinase (CPK), lactato desidrogenase (LDH), ProteÃna C reativa ultra-sensÃvel (PCR), IL-1, IL-6, IL-10, TN-, metabolitos (lactato, piruvato, acetoacetato e 3-hidroxibutirato), substancias reativas ao acido tiobarbiturico (TBARS), glutationa reduzida (GSH) e estado antioxidante total (TAS). AtravÃs da punÃÃo digital, amostras de sangue capilar foram coletadas, para analise da glicemia, nos periodos T-0, T-1, a cada hora durante a cirurgia (T-2) e a cada 2 horas apos a cirurgia (T-3). Apesar da administraÃÃo exÃgena de insulina durante a cirurgia ter sido constante em ambos os grupos, nos pacientes do grupo GG, a insulinemia permaneceu em concentraÃÃo semelhante em todos os perÃodos, enquanto que no grupo GS elevou-se em T-2 e T-3 comparado ao valor basal (T-1) (31.9+/-28.8 Versus 6.56+/5.4, p=0.013). O aumento da concentraÃÃo plasmÃtica de glucose foi significativamente menor nos pacientes tratados com L-Ala-Gln no perÃodo intra quando comparado ao grupo controle (129.9+/-15.2 versus 158.6+/-18.6, p= 0.003). NÃo se observaram alteraÃÃes significantes nas concentraÃÃes sericas das demais variÃveis estudadas entre os dois grupos. Conclui-se que a administraÃÃo prÃ-operatÃrio de doses nutraceuticas de L-Ala-Gln em pacientes submetidos a cirurgia cardÃaca com revascularizacao do miocÃrdio com CEC induz uma menor elevaÃÃo da glicemia concomitante a menor elevaÃÃo da insulinemia, o que ressalta o uso potencial da administracao prÃ-operatÃrio desse dipeptideo para atingir um melhor controle glicemico e melhorar a sensibilidade a insulina. / Recent studies have pointed out the negative effects of persistent hyperglycemia in cardiac surgery patients. Tight glucose control below 110 mg/dL with insulin has been shown to exert anti-inflammatory effects in critically ill patients. Glutamine, a conditionally essential amino acid during stress, has been shown to promote glucose utilization and to increase insulin sensitivity in trauma patients. The present study aimed to access the effects of nutraceutical doses of L-alanyl-glutamine preoperatively in a prospective, randomized, controlled, double-blind study. Twenty-two elective patients (63 Â 8 years) with coronary artery disease scheduled for coronary artery bypass grafting were randomly assigned to receive either saline 1000 ml (Group Saline, n=11) or L-alanyl-glutamine 20% (250ml) in saline to final volume 1000 ml (Group L-alanyl-glutamine, n=11). The infusions were started 3 h prior to the operative procedure and lasted 3 hours. Blood samples were collected 3h before [Basal (T-0)], just before the beginning of the surgical procedure [Preoperative (T-1)], at the onset and at the end of the extra-corporeal perfusion (T-2), and at the end of the surgical procedure (Intraoperative). Additional samples were colleted 12 and 24 h later (Postoperative). The following variables were analysed: Insulin, peptide C, creatine phosphokinase (CPK), lactate deshydrogenase (LDH), high sensibility protein C (PCR), IL-1, IL-6, 1L-10, TNF-α, metabolites (lactate, pyruvate, acetoacetate, 3-hidroxibutyrate), reactive substances to thiobarbituric acid (TBARS), reduced glutathione (GSH) and total antioxidant state (TAS). Glucose level were analysed throught capillary blood sample obtained by digital puncture in the periods T-0, T-1, every hour during surgery (T-2) and every two hours after surgery (T-3). Despite the exogenous insulin infusion rate to be constant in both groups during the operation, insulinemia was kept at similar concentrations at all periods studied in patients who received the dipeptide. On the other hand, control patients increased their insulinemia during the intra (T-2) and post-operative period (T-3) as compared to basal values (T-0) (31.9+/-28.8 Versus 6.56+/-5.4, p=0.013). Glucose increasing plasma concentrations were significantly decreased in L-alanyl-glutamine treated patients during the intraoperative period as compared to control patients (129.9+/-15.2 versus 158.6+/-18.6, p= 0.003). No significant difference were observed between the two groups on plasma concentration of others variables studied. In conclusion, the infusion of nutraceutical doses of L-alanyl-glutamine prior to operative period in patients submitted to coronary artery bypass grafting with extracorporeal perfusion, reduce the glicemic surge compared to control patients, without increasing insulin plasma concentrations as compared to their basal values This prospective study highlights the potential use of preoperative L-alanyl-glutamine administration in heart surgery patients to attain glucose control and to improve insulin sensitivity

Cortisol, resposta glicêmica e secreção de insulina em equinos clinicamente sadios submetidos a sessões de hipóxia normobárica / Serum cortisol, glycemic response and insulin secretion in healthy horses exposed to normobaric hypoxia sessions

Coordenação de Aperfeiçoamento de Pessoal de Nível Superior / Many studies pointed out to the effects of acute or chronic hypoxia in order to study the adaptation in response to high altitudes. Recently, some studies have tried to unravel the effects of exposure to intermittent hypoxia (IH), to explore the therapeutic value to patients and potential influence on the performance of athletes. Same studies show that exposure to acute or chronic hypoxia can influence the metabolism of glucose and related hormones, however, the results are still inconsistent. In horses affected with polysaccharide storage myopathy, in which horses show enhanced glucose uptake by muscle cells and increase insulin sensitivity may be the target of future studies for the potential use of IH in veterinary medicine. The aim of this study is to evaluate the effect of normobaric intermittent hypoxia in serum cortisol, blood glucose levels and insulin secretion in non-exercised horses. Healthy adult horses (n=8) were selected after physical examination and underwent an intravenous glucose tolerance test (0.5gr/kg) (IVGTT). Blood samples (n=11) were collected between 0 and 180 minutes after glucose administration to evaluate glycemic response and insulin secretion. Then, they were exposed to 1-hour hypoxia sessions twice a day, reaching a total of 43 hypoxia sessions. From first to last session the [O2] was gradually decreased from 17% to 12%. The heart rate and the samples to measure serum cortisol were collected before and every 15 minutes during the sessions 1, 4, 6, 8, 10, 14, 22 and 43. The IVGTT was repeated at the end of the study. The serum cortisol levels were higher in session 1 (S1) (17% O2) when compared to the levels of S10 (13% O2) and S43 (12% O2) (P<0.05). The glycemic response was not significantly altered in resting horses submitted to normobaric hypoxia sessions from 17% to 12% [O2], however, required higher insulin secretion, suggesting a decrease in sensitivity to this hormone. The decline in serum cortisol and heart rate suggests an adaption to the stimulus of hypoxia. IH is a resource that provides a wide area for further studies in horses, both as a therapeutic tool, about the potential benefits on athletic performance. / No passado a maioria das pesquisas apontou para os efeitos da hipóxia aguda ou crônica no intuito de estudar a adaptação em resposta às altas altitudes. Mais recentemente, alguns estudos têm buscado desvendar os efeitos da exposição à hipóxia intermitente (HI), objetivando explorar o valor terapêutico em pacientes e a influência potencial no desempenho de atletas. Muitos estudos demonstram que a exposição à hipóxia aguda ou crônica pode influenciar no metabolismo da glicose e hormônios relacionados, entretanto, os resultados ainda são inconsistentes, talvez devido aos vários protocolos usados até o momento. Em equinos, doenças como a miopatia por acúmulo de polissacarídeos, na qual os cavalos apresentam aumento na captação da glicose pelas células musculares e aumento da sensibilidade à insulina, podem ser alvo de futuros estudos para o uso potencial da HI em medicina veterinária. O objetivo do presente estudo foi avaliar o efeito da hipóxia normobárica intermitente nos níveis séricos de cortisol, resposta glicêmica e secreção de insulina de cavalos não exercitados. Cavalos adultos sadios (n=8) foram selecionados após exame físico e submetidos ao teste de tolerância à glicose intravenosa (0,5g/kg) (TTGIV). Amostras de sangue (n=11) foram coletadas entre 0 e 180 minutos após a administração de glicose para avaliar a resposta glicêmica e a secreção de insulina. Os animais foram então expostos a sessões de uma hora de hipóxia duas vezes ao dia, totalizando 43 sessões. Da primeira à última sessão o percentual de O2 foi gradativamente diminuído de 17 a 12%. A freqüência cardíaca e as amostras sanguíneas para a determinação do cortisol sérico foram coletadas antes e a cada 15 minutos durante as sessões 1, 4, 6, 8, 10, 14, 22 e 43. A TTGIV foi repetida ao final do estudo. Os níveis de cortisol sérico foram maiores na sessão 1 (S1) (17% O2) quando comparados aos níveis da S10 (13% O2) e da S43 (12% O2)(P<0,05). A resposta glicêmica não foi significativamente alterada em eqüinos não exercitados submetidos a sessões de hipóxia normobárica de 17% a 12% de O2, entretanto, foi necessária uma maior secreção de insulina, que sugere diminuição na sensibilidade a este hormônio. O declínio nos níveis de cortisol sérico e na freqüência cardíaca sugere uma adaptação ao estímulo da hipóxia. A HI é um recurso que proporciona uma ampla área para novos estudos em cavalos, tanto como ferramenta terapêutica, quanto aos possíveis benefícios no desempenho atlético.

Introduction: Majority of type I diabetes (type I DM) patients do not reach satisfactory levels of compensation regardless of the advances made in the available treatment. One of the basic pillars of successful type I DM treatment is thorough education. Objective: The aim of this thesis was to describe changes in glycosylated haemoglobin (HbA1c) in patients that completed a four day long educational program. Method: The retrospective analysis evaluated 40 patients with type I DM (age 32 y.o. ± 13, HbA1c before the program 67,1 mmol/mol ± 11,75, diagnosed with DM for 12,5 years ± 7,01). HbA1c was measured before the educational program and then again in a period of 3, 6, 12 and 24 months after the program completion. The program provided the patients with classes focusing on carb counting, flexible insulin dosing, effective ways to manage hypoglycemia and physical activity. Statistical data were obtained by non-parametric tests. (Kruskal-Wallis, ANOVA-repeated measures). Results: Three months after the program completion, a significant drop in HbA1 levels could be observed (67,1 mmol/mol ± 11,75 vs. 60,2 mmol/mol ± 9,52; p=0,0093). Impacto do consumo de pães integrais na resposta glicêmica de voluntários saudáveis / Impact of consuming whole meal breads in the glycaemic response of healthy volunteers

Pães integrais são alimentos de consumo habitual da população brasileira, porém há poucas informações a respeito da resposta glicêmica pós-prandial, O presente trabalho teve como objetivo avaliar a resposta glicêmica produzida, em indivíduos saudáveis, após o consumo de pães de fôrma rotulados como integrais. Oito pães de fôrma integrais de três categorias (clássico, light e com grãos) foram avaliados, após o consumo de porcão equivalente a 25 g de carboidratos disponíveis, através do índice glicêmico (IG) e carga glicêmica (CG). Os voluntários (n=15) compareceram ao laboratório em jejum (10 a 12 horas), pela manhã, em onze ocasiões (três dias para o consumo do pão controle e um dia para cada tipo de pão de fôrma integral). A glicemia foi determinada em jejum (t=0) e após o consumo de cada pão nos tempos: 15; 30; 45; 60; 90 e 120 minutos. A curva de resposta glicêmica, a área sob a curva (ASC) e o cálculo do IG e CG para cada um dos pães foram realizados. Considerando a glicose como referência, os pães integrais clássicos (n=2) apresentaram alto IG (71 %); os light (n=2), IG baixo (50 %) e médio (58 %) e; os com grãos (n=4), IG baixo (44 e 49 %) e médio (57 e 60 %). Os pães de fôrma light e com grãos apresentaram IG menor que os do tipo clássico (p<0,05), os quais apresentaram IG igual ao pão francês (controle). Como foi consumida a mesma quantidade de carboidratos disponíveis, a menor proporção de açúcar solúvel na categoria light parece ter sido o fator que induziu ao menor IG observado. Em relação à CG, um pão de fôrma integral light (CG=10) e outro com grãos (CG=7) foram classificados como baixa CG; os demais pães integrais (n=6) foram classificados como média CG (11 a 16). Todos os pães integrais apresentaram CG inferior a do pão controle (CG=18) (p<0,05) e entre os integrais novamente os da categoria light e com grãos foram os que apresentaram menor CG. Assim, foi observada variação na resposta glicêmica após o consumo de pães de fôrma integrais, sendo que a redução no conteúdo de açúcares solúveis, para os pães light, e a adição de grãos integrais, nos pães com grãos, favoreceram menor elevação da resposta glicêmica pós-prandial. / Whole meal breads are part of the habitual daily diet of the Brazilian population, but there is little information on the postprandial glycaemic response. The aim of this work was to evaluate the glycaemic response produced, in healthy volunteers, following the consumption of breads labeled as whole meal. Eight whole meal breads of three different categories (classic, light and grains) were evaluated, after the consumption of a portion containing approximately 25 g of available carbohydrates, using the glycaemic index (GI) and glycaemic load (GL). The subjects (n=15) attended to the laboratory after an overnight fasting (10 to 12 hours), in eleven different occasions (three days for the consumption of the control bread and a day for each whole meal bread). In every occasion, a portion of bread containing 25 g of available carbohydrate was consumed. Capillary blood samples were taken immediately before (t=0) and 15, 30, 45, 60, 90 and 120 minutes after the consumption of test breads. The glycaemic response curve, area under the curve (AUC), GI and GL for each bread were obtained. Considering glucose as reference, the classic breads (n=2) had high GI (71 %); the light (n=2), low (50 %) and medium (58 %) GI; and grains (n=4), low (44 and 49 %) and medium (57 and 60 %) GI. The light and grain breads had lower GI than the classic (p<0,05), which presented GI similar to white bread (control). As the same amount of available carbohydrates was consumed, the reduced proportion of soluble sugars in the light category breads seems to be a factor that induced the lower GI observed. In relation to the GL, one light bread (GL=10) and a grain bread (GL=7) were classified as low GL; the other whole meal breads (n=6) were classified as medium GL (11 to 16). All whole meal breads had lower GL than the control bread (GL=18) (p<0,05), and among the whole meal breads the ones in both light and grain categories presented the lower GL. Therefore, it was possible to observe variation on the glycaemic responses following the consumption of whole meal breads, the reduction in soluble sugar content, in the light breads, and the addition of whole grains, in the grain breads, favored lower elevation in the postprandial glycaemic response.

Concentrações séricas de 25 hidroxivitamina D em mulheres acima de 35 anos: prevalência de valores considerados como normais e fatores associados / Serum concentrations of 25 hydroxyvitamin D in women over 35 years: prevalence of measures considered as normal and associated factors

Introdução: Atualmente o estudo da vitamina D é motivo de especial atenção pelos profissionais da área da saúde, não só pelos já consagrados benefícios à saúde óssea, mas também por inúmeros outros, como a melhora da função cognitiva, imunológica, depressão e até redução no risco de doenças cardiovasculares e câncer. Ademais, com o aumento da longevidade, a hipovitaminose D - de alta prevalência em idosos e mulheres após a menopausa - ganha importância na saúde pública. Diante deste cenário, torna-se necessário a realização de estudos que avaliem a prevalência de hipovitaminose D e os fatores a ela associados. Objetivo: estimar, em mulheres acima de 35 anos residentes em Pindamonhangaba, SP, as concentrações séricas de 25 hidroxivitamina D e segmentar as prevalências em dois valores: acima de 20 e acima de 30 ng/mL; identificar os possíveis fatores associados a esses dois limites propostos. Método: Em amostragem aleatória estratificada foram selecionadas 1200 mulheres de 35 a 74 anos, cadastradas no Programa de Saúde da Família da cidade de Pindamonhangaba; em 681 delas foram realizadas coletas sanguíneas para determinação da vitamina D. A variável dependente estudada foi a concentração sérica de 25(OH)D e, as independentes foram pressão arterial, circunferência abdominal e IMC depressão, ansiedade, estresse, insônia, hábito de tonar sol e suplementação de vitamina D referidos, além da determinação das concentrações séricas de cortisol, glicose, colesterol total/frações e triglicerídeos. Resultados: Da população estudada, 73,4% tinham concentrações de 25(OH)D >=20ng/mL e 34,8%, >=30ng/mL. O uso suplementos de vitamina D e o habito de tomar sol associaram-se positivamente às concentrações séricas de 25(OH)D >=30ng/mL (OR=1,58, IC95%: 1,08-2,33 e OR=1,28, IC95%: 1,03-1,58, respectivamente), mas apenas o uso de suplementos de vitamina D mostrou associação com as medidas >=20ng/mL (OR=1,20, IC95%: 1,02-1,40). Ter depressão leve associou-se positivamente a 25(OH)D >=30ng/mL (OR=1,41, IC95%: 1,12-1,79), mas nenhuma associação foi notada com medidas >=20ng/mL. As medidas de 25(OH)D >=30 e 20ng/mL associaram-se negativamente às medidas séricas de glicose >=100 mg/dL (OR=0,72, IC95% 0,58- 0,90 e OR=0,90, IC95% 0,82-0,99, respectivamente). As medidas séricas de HDL <=50mg/dL e triglicérides >=150mg/dL associaram-se negativamente às medidas de 25(OH)D >=30ng/mL (OR=0,79, IC95% 0,63-0,99 e OR=0,77, IC95% 0,62-0,97, respectivamente). Houve associação negativa entre as medidas >=30ng/mL e o diagnóstico de síndrome metabólica, tanto pelo ATPIII (OR 0,72, IC95%: 0,58-0,90) quanto pelo IDF (OR=0,73, IC95%: 0,60-0,91), mas nenhuma associação com as medidas >=20ng/mL. Conclusão: A hipovitaminose D é altamente prevalente na população de mulheres acima de 35 anos de idade. As medidas séricas >=20ng/mL associaram-se negativamente a glicemia >=100mg/dL. As medidas >=30mg/dL associaram-se negativamente a glicemia >=100mg/dL, triglicérides >=150mg/dL, HDL <= 50mg/dL e diagnostico de síndrome metabólica e, positivamente à depressão. / Introduction: Currently the study of vitamin D is a special concern of the health professionals, not only for the already established benefits to bone health, but also for innumerable others, such as the improvement of cognitive, immunological function, depression and even reduction in the risk of cardiovascular diseases and cancer. Moreover, with increasing longevity, vitamin D deficiency - highly prevalent in the elderly and postmenopausal women - gains importance in public health. In view of the aforementioned reasons, it is necessary to conduct studies that evaluate the prevalence of hypovitaminosis D and its associated factors. Objective: to estimate the prevalence of having 25hidroxivitamino D over 20 and 30 ng/mL and identify its associated factors in climacteric women resident in Pindamonhangaba, SP, Brazil. Method: In randomized stratified sampling, 1,200 women aged 35-74 years enrolled in the Pindamonhangaba\'s Family Health Program were selected; in 681 of them, blood samples were taken for clinical analysis. The dependent variable studied was the serum concentration of 25(OH)D and the independent variables were blood pressure, waist circumference and BMI depression, anxiety, stress, insomnia, sunbathing and vitamin D supplementation, as well as determination of serum cortisol, glucose, total cholesterol and fractions and triglycerides levels. Results: Of the study population, 73.4% had 25(OH)D >=20ng/mL and 34.8% >=30ng/ml. Vitamin D supplements use and sunbathing were both positively associated with 25(OH)D >= 30ng/mL (OR=1.58; 95%CI:1.08-2.33 and OR =1.28; 95%CI:1.03-1.58, respectively), but only in the use of vitamin D supplements the association with 25(OH)D >=20ng/ml was found (OR=1.20, 95%CI: 1.02- 1.40). Mild depression was positively associated with 25(OH)D >=30ng/mL (OR=1.41, 95%CI: 1.12-1.79), but no association was noted with measurements >=20ng/mL. Both 25(OH)D >=30 and >=20ng/mL were negatively associated with blood glucose >=100mg/dL (OR=0.72, 95%CI 0.58-0.90 and OR=0.90, 95%CI 0.82-0.99, respectively). Serum HDL <= 50mg/dL and triglycerides >=150mg/dL were negatively associated with 25(OH)D >=30ng/mL (OR=0.79, 95%CI 0.63-0.99 and OR=0.77, 95%CI 0.62-0.97, respectively). A negative association was present between 25(OH)D >=30 ng/mL and metabolic syndrome by ATPIII (OR= 0.72, 95%CI: 0.58-0.90) and IDF criteria (OR=0.73, 95%CI: 0.60-0.91), but there was no association with 25(OH)D >=20ng/mL. Conclusion: Hypovitaminosis D is highly prevalent in study population. Introdução: Atualmente o estudo da vitamina D é motivo de especial atenção pelos profissionais da área da saúde, não só pelos já consagrados benefícios à saúde óssea, mas também por inúmeros outros, como a melhora da função cognitiva, imunológica, depressão e até redução no risco de doenças cardiovasculares e câncer. Ademais, com o aumento da longevidade, a hipovitaminose D - de alta prevalência em idosos e mulheres após a menopausa - ganha importância na saúde pública. Diante deste cenário, torna-se necessário a realização de estudos que avaliem a prevalência de hipovitaminose D e os fatores a ela associados. Objetivo: estimar, em mulheres acima de 35 anos residentes em Pindamonhangaba, SP, as concentrações séricas de 25 hidroxivitamina D e segmentar as prevalências em dois valores: acima de 20 e acima de 30 ng/mL; identificar os possíveis fatores associados a esses dois limites propostos. Método: Em amostragem aleatória estratificada foram selecionadas 1200 mulheres de 35 a 74 anos, cadastradas no Programa de Saúde da Família da cidade de Pindamonhangaba; em 681 delas foram realizadas coletas sanguíneas para determinação da vitamina D. A variável dependente estudada foi a concentração sérica de 25(OH)D e, as independentes foram pressão arterial, circunferência abdominal e IMC depressão, ansiedade, estresse, insônia, hábito de tonar sol e suplementação de vitamina D referidos, além da determinação das concentrações séricas de cortisol, glicose, colesterol total/frações e triglicerídeos. Resultados: Da população estudada, 73,4% tinham concentrações de 25(OH)D >=20ng/mL e 34,8%, >=30ng/mL. O uso suplementos de vitamina D e o habito de tomar sol associaram-se positivamente às concentrações séricas de 25(OH)D >=30ng/mL (OR=1,58, IC95%: 1,08-2,33 e OR=1,28, IC95%: 1,03-1,58, respectivamente), mas apenas o uso de suplementos de vitamina D mostrou associação com as medidas >=20ng/mL (OR=1,20, IC95%: 1,02-1,40). Ter depressão leve associou-se positivamente a 25(OH)D >=30ng/mL (OR=1,41, IC95%: 1,12-1,79), mas nenhuma associação foi notada com medidas >=20ng/mL. As medidas de 25(OH)D >=30 e 20ng/mL associaram-se negativamente às medidas séricas de glicose >=100 mg/dL (OR=0,72, IC95% 0,58- 0,90 e OR=0,90, IC95% 0,82-0,99, respectivamente). As medidas séricas de HDL <=50mg/dL e triglicérides >=150mg/dL associaram-se negativamente às medidas de 25(OH)D >=30ng/mL (OR=0,79, IC95% 0,63-0,99 e OR=0,77, IC95% 0,62-0,97, respectivamente). Houve associação negativa entre as medidas >=30ng/mL e o diagnóstico de síndrome metabólica, tanto pelo ATPIII (OR 0,72, IC95%: 0,58-0,90) quanto pelo IDF (OR=0,73, IC95%: 0,60-0,91), mas nenhuma associação com as medidas >=20ng/mL. Conclusão: A hipovitaminose D é altamente prevalente na população de mulheres acima de 35 anos de idade. As medidas séricas >=20ng/mL associaram-se negativamente a glicemia >=100mg/dL. As medidas >=30mg/dL associaram-se negativamente a glicemia >=100mg/dL, triglicérides >=150mg/dL, HDL <= 50mg/dL e diagnostico de síndrome metabólica e, positivamente à depressão. / Introduction: Currently the study of vitamin D is a special concern of the health professionals, not only for the already established benefits to bone health, but also for innumerable others, such as the improvement of cognitive, immunological function, depression and even reduction in the risk of cardiovascular diseases and cancer. Moreover, with increasing longevity, vitamin D deficiency - highly prevalent in the elderly and postmenopausal women - gains importance in public health. In view of the aforementioned reasons, it is necessary to conduct studies that evaluate the prevalence of hypovitaminosis D and its associated factors. Objective: to estimate the prevalence of having 25hidroxivitamino D over 20 and 30 ng/mL and identify its associated factors in climacteric women resident in Pindamonhangaba, SP, Brazil. Method: In randomized stratified sampling, 1,200 women aged 35-74 years enrolled in the Pindamonhangaba\'s Family Health Program were selected; in 681 of them, blood samples were taken for clinical analysis. The dependent variable studied was the serum concentration of 25(OH)D and the independent variables were blood pressure, waist circumference and BMI depression, anxiety, stress, insomnia, sunbathing and vitamin D supplementation, as well as determination of serum cortisol, glucose, total cholesterol and fractions and triglycerides levels. Results: Of the study population, 73.4% had 25(OH)D >=20ng/mL and 34.8% >=30ng/ml. Vitamin D supplements use and sunbathing were both positively associated with 25(OH)D >= 30ng/mL (OR=1.58; 95%CI:1.08-2.33 and OR =1.28; 95%CI:1.03-1.58, respectively), but only in the use of vitamin D supplements the association with 25(OH)D >=20ng/ml was found (OR=1.20, 95%CI: 1.02- 1.40). Mild depression was positively associated with 25(OH)D >=30ng/mL (OR=1.41, 95%CI: 1.12-1.79), but no association was noted with measurements >=20ng/mL. Both 25(OH)D >=30 and >=20ng/mL were negatively associated with blood glucose >=100mg/dL (OR=0.72, 95%CI 0.58-0.90 and OR=0.90, 95%CI 0.82-0.99, respectively). Serum HDL <= 50mg/dL and triglycerides >=150mg/dL were negatively associated with 25(OH)D >=30ng/mL (OR=0.79, 95%CI 0.63-0.99 and OR=0.77, 95%CI 0.62-0.97, respectively). A negative association was present between 25(OH)D >=30 ng/mL and metabolic syndrome by ATPIII (OR= 0.72, 95%CI: 0.58-0.90) and IDF criteria (OR=0.73, 95%CI: 0.60-0.91), but there was no association with 25(OH)D >=20ng/mL. Conclusion: Hypovitaminosis D is highly prevalent in study population. Serum 25(OH)D >=20mg/dL was negatively associated with glycemia >=100mg/dL, and the 30mg/dL limit was negatively associated with glycemia >=100mg/dL, triglycerides >=150mg/dL, HDL <=50mg/dL and metabolic syndrome and positively associated to depression. Serum measurements >=20ng/mL were not significantly associated with the investigated factors.

Programa de educação em diabetes mellitus focado na automonitorização da glicemia capilar: estudo quase experimental / Diabetes mellitus education program focused on the selfmonitoring of capillary glycemia: a quasi-experimental study

Trata-se de uma pesquisa quase experimental que objetivou avaliar as contribuições de um programa educativo, focado na automonitorização da glicemia capilar, para as pessoas com diabetes mellitus tipo 2, desenvolvida em unidade ambulatorial de um hospital de nível terciário do interior paulista, em amostra de 25 pessoas com diabetes mellitus tipo 2. O programa educativo teve duração de 12 meses, estruturado com cinco encontros, dos quais dois foram individuais e utilizados para realizar a coleta de dados, nos dois tempos do estudo, que ocorreram antes e após o programa educativo, respectivamente, e três na modalidade grupal, nos quais foram desenvolvidas as intervenções educativas por meio de ferramentas visuais e interativas, com ênfase no conceito de autoeficácia da Teoria Social Cognitiva. Todos os participantes receberam sete reforços educativos por contato telefônico, sobre os temas discutidos nos grupos. Compuseram o estudo as variáveis sociodemográficas, clínicas, antropométricas, exames laboratoriais, automonitorização da glicemia capilar, autocuidado, conhecimento sobre a doença e autoeficácia. Os dados de caracterização da amostra foram apresentados por meio da estatística descritiva, e as análises entre os tempos, antes e após a participação do programa educativo, foram realizadas por meio dos testes de Wilcoxon e McNemar, com o nível de significância estatística 5%. Na caracterização da amostra, houve predomínio do sexo masculino 14 (56,0%), adultos idosos com média de 60,16 (DP=8,58) anos de idade, tempo médio de diagnóstico de 17,28 (DP=8,22) anos. O exame laboratorial referente à glicemia de jejum aumentou no T12, e a hemoglobina glicada manteve o mesmo valor médio, após o programa educativo. Na automonitorização da glicemia capilar, houve resultados indicativos de melhora dos parâmetros, após a participação no programa educativo, para os itens referentes ao conhecimento do valor da glicemia pós-prandial (p=0,0039), interpretação dos resultados de glicemia capilar com as refeições e medicamentos (p=0,0156) e reconhecimento de \"fraqueza\" como um sintoma da hiperglicemia (p=0,0386). A variável de autocuidado com a doença apresentou resultado negativo no item sobre a prática de exercício físico específico, ao diminuir o valor médio após o programa educativo (p=0,0445). O escore médio de conhecimento sobre a doença aumentou após o programa educativo (p>0,05). Para a autoeficácia, a média total da escala diminuiu após o programa educativo, no T0 a média foi de 1,67 (DP=0,72) e no T12 1,55 (DP=0,51). O programa educativo contribuiu, de maneira positiva, para as variáveis da automonitorização da glicemia capilar, autocuidado com o diabetes mellitus e conhecimento sobre a doença. Observou-se aumento no valor do escore médio em seis itens da autoeficácia no T12. Os resultados mostraram contribuições positivas para as pessoas na realização da automonitorização da glicemia capilar, como forma de manter o controle da doença / This is a quasi-experimental study that aimed to evaluate the contributions of an educational program focused on the self-monitoring of capillary glycemia for people with type 2 diabetes mellitus developed in an outpatient unit of a tertiary-level hospital in the interior of São Paulo in a sample of 25 people with type 2 diabetes mellitus. The educational program lasted 12 months, structured with five meetings. Of the five meetings, two were individual and used to collect data, in the two study times, which occurred before and after the educational program, respectively; and three meetings in the group modality, in which the educational interventions were developed by means of visual and interactive tools, with emphasis on the concept of self-efficacy of Social Cognitive Theory. All the participants received seven educational reinforcements by telephonic contact, about topics discussed in the groups. The study was composed these variables: of sociodemographic, clinical, anthropometrics, laboratory tests, selfmonitoring of capillary glycemia, self-care, knowledge about the disease and selfefficacy. The characterization data of the sample were presented by means of descriptive statistics and the analyzes between the times before and after the participation of the educational program were held by means of the tests of Wilcoxon and McNemar, with the level of statistical significance 5%. In the characterization of the sample, there was a predominance of male 14 (56.0%), elderly adults with average of 60.16 (SD = 8.58) years old, average diagnosis time of 17,28 (SD = 8.22) years. The laboratory test for fasting glycemia increased in T12 and glycated hemoglobin maintained the same average value after the educational program. In the selfmonitoring of capillary glycemia, there were indicative results of improvement of the parameters, after participation in the educational program, for the items related to knowledge of the value of postprandial blood glucose (p = 0.0039), interpretation of capillary glycemia results with meals and medications (p = 0.0156), and recognition of \"weakness\" as a symptom of hyperglycemia (p = 0.0386). The self-care variable with the disease presented a negative result in the item on the practice of specific physical exercise, decreasing the average value after the educational program decreased (p = 0.0445). The average score of knowledge about the disease increased after the educational program (p> 0.05). For self-efficacy, the total average of the scale decreased after the educational program, at T0 the average was 1.67 (SD = 0.72) and at T12 1.55 (SD = 0.51). The educational program contributed positively to the variables of self-monitoring of capillary glycemia, self-care with diabetes mellitus and knowledge about the disease. There was an increase in the value of the average score in six items of self-efficacy in T12. The results showed positive contributions to people in the selfmonitoring of capillary glycemia as a way to maintain control of the disease

Evaluation des performances de systèmes d’assistance au contrôle pour la réanimation : Application au contrôle de la glycémie / Assessment of decision-making support systems performances in ICU : Application to glycaemic control

La diversité des maladies des patients de réanimation fait que l’environnement technologique de soins intensifs est composé de nombreux systèmes de monitorage de constantes physiologiques, permettant à l’équipe soignante de déterminer un traitement adapté au patient. De plus en plus de systèmes ont une fonction d’assistance à la prescription ou aux soins afin de réduire la charge de travail et mentale des infirmières exigées par certains traitements ou protocoles. Ces systèmes informatisés facilitent l’intégration du protocole et de l’information disponible dans les signaux et peuvent aboutir à des systèmes de complexité élevée. La question de l’évaluation de la qualité de la réalisation et de la balance bénéfice/risque pour les patients liée à l’usage de nouveaux systèmes d’assistance se pose alors. Un problème consiste à mener cette évaluation a priori dès la conception du protocole ou de l’algorithme. Ce travail de thèse donne un exemple de méthode pour un système d’assistance au contrôle de la glycémie des patients de réanimation : l’évaluation est menée depuis la conception de l’algorithme de contrôle jusqu’à une étude clinique de grande ampleur. L’origine du dispositif étudié vient de ce que les patients présentent souvent une hyperglycémie liée au stress à leur arrivée en réanimation (l’augmentation de l’insulino-résistance, l’administration de certaines de drogues ou la déficience en insuline inhibent la réponse physiologique à l’augmentation de la glycémie). Un problème vient alors de ce que, d’une part, l’hyperglycémie prolongée étant associée à une morbidité voire une mortalité accrue, contrôler la glycémie est bénéfique, et d’autre part réduire la glycémie fait courir le risque d’épisodes hypoglycémiques pendant le séjour en réanimation. Dans ce cas, optimiser la balance bénéfice/risque est encore un problème ouvert. Bien qu’il existe de nombreux travaux sur la modélisation de la pharmacodynamique du système glucose-insuline, peu de travaux exploitent ces modèles pour fournir un système de contrôle fonctionnel, testé et industrialisable. La thèse présente un système de contrôle glycémique ainsi qu’une méthode d’évaluation généralisable à d’autres systèmes, qui teste numériquement les performances techniques et cliniques de ce type de systèmes sur des patients virtuels. Les résultats d’une étude clinique réelle sont aussi présentés. / The variety of ICU patients diseases implies that technological environment of critical care is composed of many vital signs monitoring systems, allowing the medical team to determine appropriate treatment to the patient. More and more systems have a decision-making support function to reduce the mental and physical workload of nurses required for certain treatments or protocols. These computerized systems facilitate the integration of the protocol and the information available in the signals and can lead to systems of high complexity. The issue of assessing the performances and the benefit/risk balance for the patient related to the use of new support systems arises. The problem is to conduct this evaluation a priori, during the design process of the protocol or algorithm. This work provides an example of a method to support the control of blood glucose in the ICU system evaluation is conducted for the design of the control algorithm to a large-scale clinical study.The origin of the studied device comes from the fact that patients often experience hyperglycemia due to stress upon their arrival in the ICU (increased insulin resistance, administration of interacting drugs or insulin deficiency inhibit the physiological response to the blood sugar increase). A problem then is that, on the one hand, as sustained hyperglycemia is associated with an increased morbidity or mortality, controlling glycaemia reduces risks, but, on the other hand, reducing blood sugar exposes to the risk of hypoglycemia during the ICU stay. In this case, optimizing benefit/risk ratio is still an open problem.Although there are many studies on modeling the pharmacodynamics of glucose-insulin system, few works use these models to provide a functional, tested and industrialized control system. The thesis presents a glycemic control system and a generalized method of evaluation with other systems, which tests digitally technical and clinical performances of such systems on virtual patients. The results of a real clinical trial are also presented.

Differential Effects of Chronic Low Calorie Sweetener Consumption on Body Weight, Glycemia, and Ingestive Behavior

<p>Low calorie sweeteners (LCS) provide sweetness with little to no energy. Each sweetener has a unique chemical structure that possesses unique sensory and functional characteristics. While LCS are generally considered in aggregate, these unique chemical structures have potential implications for sensory, metabolic, and behavioral differences that may impact body weight and glycemia. Therefore, two, twelve-week experiments were conducted to determine the effect of chronic LCS consumption on body weight, glycemia, and ingestive behaviors. </p> <p>The first experiment investigated the differential effects of four LCS (saccharin, aspartame, rebaudioside A, and sucralose) and sucrose consumed for twelve weeks on body weight, glycemia, and ingestive behaviors among healthy adults with overweight or obesity (body mass index (BMI) between 25 and 40 kg/m<sup>2</sup>). In a parallel-arm design, 154 participants were randomly assigned to consume 1.25 to 1.75L of beverage sweetened with 1 of the 5 sweeteners daily for 12 weeks. Body weight was measured every two weeks; energy intake, energy expenditure, and appetite were assessed every 4 weeks; and glucose tolerance was measured at baseline and week 12. Every four weeks, participants completed 24-hour urine collections to determine study compliance via PABA excretion. Sucrose and saccharin consumption led to increased body weight across the 12-week intervention (Δ weight = +1.85 and +1.18kg, p ≤ 0.02) and did not differ from each other. While there was no significant change in body weight with consumption of the other LCS treatments compared to baseline, changes in weight in comparison to the sucrose treatment (sucrose – LCS) were significantly different for aspartame, rebA, and sucralose after 12 weeks (weight difference = 1.13, 1.25, 2.63kg, respectively; p ≤ 0.03). In addition, change in body weight at week 12 was significantly lower between sucralose and all other LCS (weight difference ≥ - 1.37 kg, p=0.008).</p> <p>The second experiment investigated the effect of daily aspartame ingestion on glycemia, body weight, and appetite. One hundred lean (BMI between 18 and 25 kg/m<sup>2</sup>) adults were randomly assigned to consume 0, 350, or 1050 mg aspartame/day for twelve weeks in a parallel-arm design. This experiment followed a similar protocol but measured body weight and blood pressure weekly and contained a 240-min glucose-tolerance test (OGTT) with measurements of selected hormones at baseline and week 12. Participants also collected 24-h urine samples every four weeks. There were no group differences for glucose, insulin, resting leptin, glucagon-like peptide 1, or gastric inhibitory peptide at baseline or week 12. There also were no effects of aspartame ingestion on appetite, body weight, or body composition. </p> <p>These trials demonstrate that all LCS contribute negligible energy but should not be aggregated because of their differing effects on body weight. Sucrose and saccharin consumption significantly increased body weight compared to aspartame, rebA, and sucralose. This differential change in body weight among LCS indicates individual LCS likely exert different physiological responses beyond the contribution of sweetness with negligible energy. Saccharin, rebA, sucralose, and aspartame (ingested at three doses) for twelve weeks had no effect on glycemia. These data do not support the view that LCS are problematic for the management of glycemia. If substantiated through additional testing, findings from this trial have implications for consumers, food industry, clinicians, and policy makers. Some LCS may not hold the anticipated beneficial effects on body weight (e.g., saccharin) and positive effects of one LCS (sucralose) may be attenuated if combined with select other LCS. Going forward it will be important to consider each LCS as a distinct entity with respect to its potential health effects.</p>
