Processus de validation d’une base de données haute résolution dans une unité de soins intensifs pédiatriques

Objectif : Notre objectif était d’évaluer la qualité des données de la base de données haute résolution (BDHR) implantée dans l’unité de soins intensifs pédiatriques (USIP) de l’Hôpital Sainte-Justine (HSJ). Type d’étude : Un rapport descriptif et une analyse d’une étude prospective de validation d’une BDHR. Environnement : Une USIP de 32 lits, adaptée aux soins médicaux, chirurgicaux et cardiaques dans un centre tertiaire mère-enfant du Canada. Population : Tous les patients admis à l’USIP et ayant un monitorage d’au moins 1 signe vital par un moniteur cardio-respiratoire. Mesures et résultats principaux : Entre juin 2017 et août 2018, les données de 295 jours de patients ont été enregistrées à partir des appareils médicaux et 4465 données ont été filmées et comparées aux données correspondantes dans la BDHR de l’USIP de l’HSJ. Les analyses statistiques ont démontré en général une bonne corrélation, une excellente fiabilité et un bon agrément. Les graphiques de Bland-Altman ont aussi démontré l’exactitude et la précision entre les données récoltées et les données filmées selon les limites d’agrément cliniquement significatives préalablement définies. Conclusions : Cette étude de validation exécutée sur un échantillon représentatif a démontré que la qualité des données était globalement excellente. / Objective: Our objective was to evaluate the data quality of our high-resolution electronic database (HRDB) implemented in the pediatric intensive care unit (PICU) of HSJ. Design: A descriptive report and analysis of a prospective validation of a HRDB. Setting: A 32 beds pediatric medical, surgical and cardiac PICU in a tertiary care free-standing maternal-child health center in Canada. Population: All patients admitted to the PICU with at least one vital sign recorded using a cardiorespiratory monitor connected to the central monitoring station. Measurements and Main Results: Between June 2017 and August 2018, data from 295 patient days were recorded from medical devices and 4,645 data points were video recorded and compared to the corresponding data collected in the HSJ-PICU HRDB. Statistical analysis showed excellent overall correlation, agreement and reliability. Bland-Altman analysis showed excellent accuracy and precision between recorded and collected data within clinically significant pre-defined limits of agreement. Conclusions: This prospective validation study performed on a representative sample showed excellent overall data quality.

Apprendre ensemble : attitudes des étudiants en médecine envers la pratique collaborative suite à un cours d’éducation interprofessionnelle impliquant un patient-formateur

Les cours d’éducation interprofessionnelle (IPE) permettent de cultiver la pratique collaborative, composante intégrante et contemporaine de la prestation optimale des soins aux patients. Ce projet de recherche a pour but d’explorer si un cours d’IPE utilisant, entre autre, un atelier interprogramme et la rétroaction d’un tandem composé d’un patient partenaire à profil formateur et d’un professeur, influence les attitudes des étudiants en médecine envers l’IPE, ainsi que le développement des habiletés de communication et de collaboration. Quarante-six de 250 étudiants en troisième année de médecine ayant participé au cours ont rempli le questionnaire Readiness of Health Care Students for InterProfessional Learning Scale. Près de 90% de ces étudiants démontrent une attitude positive envers l’IPE. L’analyse qualitative de plus de 1500 commentaires écrits de 223 étudiants en médecine à partir d’un questionnaire d’évaluation du cours a démontré que ceux-ci perçoivent une amélioration de leurs habiletés de communication et de collaboration, une confiance accrue en leurs compétences et globalement une meilleure compréhension des rôles et responsabilités des professionnels. Des facteurs influençant leurs attitudes et croyances, ainsi que les barrières à l’IPE émergent de l’analyse, tels que l’importance d’un respect mutuel et de l’abolition des préjugés envers les autres professions pour la réussite de l’IPE. En participant à ce cours d’IPE, les étudiants de l’Université de Montréal ont une opportunité exceptionnelle de s’initier à la pratique collaborative en partenariat de soins avec le patient. Les barrières à la participation des étudiants de médecine à la recherche pédagogique sont discutées et des pistes de solution pour rehausser leur intérêt dans le futur sont présentées. / Interprofessional education (IPE) courses help cultivate collaborative practice, an essential aspect of contemporary health care delivery that improves patient care. The goal of this study is to explore if an IPE course, using an interprogram workshop and retroaction by a tandem comprised of a patient-partner with an instructor profile and of a teacher, an innovative approach initiated by the Université de Montréal, influences medical students’ attitudes towards IPE and allows developing communication and collaborative skills. Forty-six of the two hundred and fifty medical students participating in the course filled out the Readiness of Health Care Students for InterProfessional Learning Scale. A positive attitude towards IPE is found in the vast majority of students («teamwork and collaboration» section: 88.9±5.8% of student answers = agree/completely agree). Qualitative analysis of more than 1500 written comments made by students showed that students perceived an improvement in their communication and collaboration skills, as well as their confidence in those skills. Overall, they had better understanding of professional roles and responsibilities. Factors influencing their attitudes and beliefs, as well as barriers to IPE, emerged from the analysis; the importance of respect and abolishing prejudices in order to succeed at IPE were raised. By participating to this IPE course, medical students of Université de Montréal have an exceptional opportunity to familiarize themselves with health care involving partnership with patients and collaborative practice. A discussion of barriers for the participation of medical students in pedagogical research and possible solutions to enhance their interest in the future are provided.

Dental Students' Perceptions of Dental Hygienists' Education and Scope of Practice

Oral health is strongly correlated to systemic health in the United States. Dental professionals play a significant role in addressing the oral health and prevention of disease for populations in the United States. Because many areas in the United States lack access to dental care, the federal government considers dental hygienists part of the solution. It is now paying attention to restrictive dental hygiene scope of practice rules in each state. State dental boards, comprised mainly of dentists, make these rules based on their perception of dental hygienists' lack of education. This study explored fourth-year dental students' perceptions of the dental hygienist's education and scope of practice. This research found that while dental students felt that the dental hygienist would be a positive addition to their future practices, they did not wholly understand the rigors of dental hygiene education. In addition, dental schools may need to educate on the differences between direct and general supervision.

Innovation in Health Science Education: An Experiential Learning Program

The SARS-CoV-2 pandemic disrupted health professions education on a number of different levels. Many health professions and pre-health professions students lost access to real-world clinical experience which has lead to disruptions in the healthcare workforce pipeline. At the University of Montana a diverse group of health professions educators designed an innovative experiential learning program, called Griz Health, that would allow UM students to engage in healthcare experiences while helping the campus with COVID-19 response. Because of the overwhelmingly positive response from students and community members that participated in this program, the Griz Health program was shifted from a volunteer response program to a year-long course. Students in the Griz Health course will work in small, interprofessional teams to engage in the innovation process to tackle local healthcare issues in underserved communities.

An Evaluation of the Quality Assurance Plan at East Tennessee State University’s Dental Hygiene Program

Quality assurance in healthcare is fundamental in ensuring the achievement of desired outcomes for patients. In 2011 a quality assurance plan was created at the East Tennessee State University (ETSU) dental hygiene program in order to meet accreditation standards. The purpose of this study was to evaluate this plan in order to determine its effectiveness in improving the quality of patient care indicators. One hundred fifty patient charts were selected and audited. Deficiencies were counted in the categories of assessment, treatment, documentation, referral, caries management, perio management, patient education, and follow-up. Research findings were varied; however, external variables with the potential to affect the study’s results were identified. Furthermore, statistical process control procedures indicated that the quality assurance program was effective or had the potential to be effective. Although further research is warranted, this study could be used to improve quality assurance practices at the ETSU dental hygiene program.

A Survey of the Implementation and Usage of Electronic Dental Records and Digital Radiographs in Private Dental Practices in Mississippi

Implementation of electronic health records by the Health Information Technology for Economic and Clinical Health has led to the implementation of electronic dental records (EDRs) and digital radiography in dental offices. The purpose of this study was to determine the state of the implementation and usage of EDRs and digital radiographs by the private general and pediatric dental practices in Mississippi as well as reasons why the dental practices are not moving forward with the advanced technology. A survey was emailed to 712 dental practices: 116 responded (16% response rate), and 104 consented to participate (89.66%). Results indicated dental practices in Mississippi using EDRs was 46.07%, EDRs with paper records was 42.70%, and only paper records was 11.24%. Results indicated dental practices using digital radiography was 76.40%, conventional radiography was 13.48%, and both was 10.11%. Common reasons for not advancing were cost, insufficient training, computer/software issues, and “too old.”

The effects of social-comparative feedback during motor skill acquisition in highly-motivated learners: Applications to medical education

Social-comparative feedback (i.e., providing a learner with information regarding his/her performance relative to a group average) has been shown to influence a learner’s psychological and behavioural outcomes during motor skill acquisition (Avila, Chiviacowsky, Wulf, & Lewthwaite, 2012; Eliasz, 2012; Lewthwaite & Wulf, 2010; McKay, Lewthwaite, & Wulf, 2012; Stoate, Wulf, & Lewthwaite, 2012; Wulf, Chiviacowsky, & Cardozo, 2014; Wulf, Chiviacowsky, & Lewthwaite, 2010, 2012; Wulf & Lewthwaite, 2016). This research indicates that motor skill acquisition is facilitated when learners believe they are performing better than the average, regardless of their actual performance. It has been suggested (Wulf & Lewthwaite, 2016) that a better-than-average mindset enhances psychological factors such as self-efficacy and motivation and in turn, actual behaviour. However, there is also evidence to suggest that self-efficacy (having state-like properties) and motivation (having both state and trait-like properties) are related in terms of their affective influence on learning (Bandura, 1997; Schunk, 1990, 1991, 1995) but the relationship between the two constructs and its subsequent outcomes remain unclear. Even though individual differences in motivation have been suggested to influence self-efficacy beliefs, they have been largely ignored in this line of research. There is also evidence to suggest that learners possessing high levels of motivation (whether that may be at a trait or state level) may not interpret feedback in the same manner (Aronson, 1992; Festinger, 1957; Frey, 1986; Harmon-Jones, 2012; Harmon-Jones & Harmon-Jones, 2002; Harmon-Jones, Harmon-Jones, Fearn, Sigelman, & Johnson, 2008; Harmon-Jones & Mills, 1999; Harmon-Jones, Schmeichel, Inzlicht, & Harmon- Jones, 2011; Steele, 1988). Therefore, the goal of this dissertation is use both theoretical and applied perspectives to examine the degree to which social- comparative feedback affects psychological and behavioural outcomes in highly- motivated learners (e.g., medical trainees) learning procedural skills. Independent of actual performance, we provided manipulated feedback information to novice pre-clerkship medical trainees while they were learning motor skills to suggest that they were performing better or worse than the average. The first study used a basic sequential key-press learning task (Eliasz, 2012) and a basic suturing task to explore the role of social-comparative feedback in medical trainees and tested whether features of the task were important (i.e., basic laboratory task or technical skill task) during the interpretation of this feedback. The second study used the same experimental paradigm to extend our results to a relevant medical education context (i.e., medical trainees learning basic suturing techniques). The final study examined whether the credibility of the feedback provider (i.e., expert versus peer) played a role in how social-comparative feedback was being internalized by novice medical trainees. Our initial study demonstrated that, compared to those receiving positive or no social-comparative feedback, medical trainees receiving negative social- comparative feedback during motor skill acquisition had significant difficulties in learning both the laboratory and technical skill task. These findings suggested that compared to other learners, novice medical trainees (a subset of highly-motivated learners), responded differently to social-comparative feedback. The second study replicated this pattern and revealed that medical trainees receiving below-average feedback during technical skill acquisition experienced significant detriments to their performance, learning and self-efficacy. Our final study found that regardless of the feedback source (hypothetical expert versus another peer), the experience of receiving negative social-comparative feedback impacted self- reported psychological measures and the immediate performance of a basic surgical technique. This dissertation provides, to the best of our knowledge, the first demonstration that medical trainees, a subset of highly-motivated learners, interpret social-comparative feedback differently than other learners studied in the literature. More specifically, receiving positive social-comparative feedback did not facilitate the learning process as found in previous studies with non-medical learners, while the delivery of negative social-comparative feedback, irrespective of task or feedback provider, was psychologically and behaviourally detrimental to novice medical trainees learning motor skills. / Dissertation / Doctor of Philosophy (PhD) / This dissertation includes three original studies designed to examine the effects of social-comparative feedback during skill acquisition in highly- motivated learners (e.g., medical trainees). Regardless of actual task performance, novice medical trainees who were provided with feedback during the learning process indicating that they were performing worse than the group average, experienced significant detriments to their psychological and behavioural outcomes. This effect was present regardless of the task being learned (i.e., key- pressing or suturing) or who was delivering the feedback (i.e., a hypothetical ‘expert’ or ‘peer’). Receiving better-than-average feedback did not result in any additional psychological and behavioural benefits. Contrary to the research with non-medical students, where “you are above-average” social-comparative feedback facilitates learning and “you are below-average” social-comparative feedback is no different than a control condition, these studies suggest that the experience of receiving below-average feedback during the learning process can be detrimental for highly-motivated novice learners. These findings are important to consider in both the context of feedback delivery and remediation as they provide evidence that novice medical trainees, regardless of the task and feedback provider, experience difficulty in receiving information that they are performing relatively poorly compared to their peers.

Lärare är en av de viktigaste skolrelaterade faktorerna angående elevers akademiska lärande. Akademiskt ledarskap inom lärarprofessionen har enligt tidigare forskning en väsentlig del i elevers skolresultat. Kunskaper om ledarskap, och dess innebörd, skapar behovet av att införskaffa mer kunskap om hur akademiskt ledarskap utbildas inom en idrottslärarutbildning. Studiens syfte var att undersöka vilka kunskaper om ledarskap, och dess innebörd, som lärarstudenter inom en idrottslärarutbildning på ett universitet i södra Sverige har införskaffat samt saknat under utbildningen. Kvalitativ intervjustudie var undersökningens metod med en tillhörande semistrukturerad intervjuguide. Sju deltagare intervjuades och efter intervjuernas genomförande utfördes en tematisk innehållsanalys av deltagarnas svar som genererade identifiering av teman samt kategorier som besvarade studiens syfte samt frågeställningar. Studien resulterade i att införskaffade kunskaper om ledarskap inom idrottslärarutbildningen innefattar professionella kunskaper, didaktik och friluftsliv. Studieresultaten om kunskaper inom ledarskap som saknas i utbildningen innefattar interpersonella kunskaper, intrapersonella kunskaper, bindning av kunskaper och engagemang. Studiens slutsats förklarar att kunskaper om ledarskap, och dess innebörd, behöver i framtida forskning fortsätta undersökas inom två olika nivåer: institutionell nivå och lärarmässig nivå. / Teachers are one of the most important school-related factors regarding students' academic learning. Academic leadership within the teaching profession has an essential part in students' school results according to previousresearch. Knowledgeofleadership,anditsmeaning,createstheneed to acquire more knowledge about how academic leadership is educated in a PE teacher education. This study's purpose was to investigate PE student teachers ́ experiences about what knowledges, and its meaning, of leadership has been acquired and is missing in the PE education. Qualitative interview study was the research method which includes a semi-structured interview guide and there were seven interviews conducted in this study. After the implementation of the interviews a thematic content analysis was conducted of the participants' responses that generated identification of themes and categories that answered the study's purpose and questions. The result of the study was that acquired knowledge in leadership within the PE teacher education are professional knowledge, didactics and outdoor education. It appeared in the results that knowledge in leadership that is missing in the PE teacher education are interpersonal skills, intrapersonal skills, binding of skills and dedication. The conclusion of this study explains that knowledge of leadership, and its meaning, needs to be included in future research within two different levels: institutional level and teaching level.
