Hepatite C em pacientes portadores de insuficiência renal crônica em hemodiálise : aspectos histológicos, bioquímicos e virológicos

Existem poucos estudos na literatura, e aqueles que avaliaram histologia hepática apresentam resultados contraditórios. Objetivos: O objetivo desse estudo foi comparar os aspectos clínicos, bioquímicos e de histologia hepática dos pacientes em hemodiálise com aqueles com função renal normal. Material e Métodos: Foi realizado um estudo caso-controle com 36 pacientes em hemodiálise e 37 pacientes com função renal normal. Os dois grupos apresentavam pesquisa do RNA do Vírus da hepatite C e foram emparelhados com relação a sexo, idade e tempo estimado de infecção. Além disso, foi realizado um estudo prospectivo para avaliar a taxa de progressão da fibrose hepática. Nos pacientes portadores de IRC em hemodiálise que não foram submetidos ao transplante renal num período de pelo menos 03 anos foi realizado uma segunda biópsia hepática e comparado com os achados histológicos da primeira. Resultados: Os pacientes infectados pelo VHC em hemodiálise apresentaram menores níveis de enzima hepática (alanina aminotransferase) e menor carga viral que os pacientes com função renal normal. A presença de qualquer grau de fibrose hepática pela classificação de Metavir foi significativamente mais freqüente nos pacientes com função renal normal (73%) quando comparados com aqueles em hemodiálise (47.2%; p<0.025). Da mesma forma, o grau de atividade inflamatória foi mais intenso nos pacientes com função renal normal (59.5%) do que naqueles em hemodiálise (27.7%; p=0.003). Na análise multivariada, a presença de atividade inflamatória grave com o único fator de risco para a ocorrência de fibrose hepática (p<0.001) e o risco de inflamação grave (Metavir graus 2-4) foi quatro vezes menor nos pacientes em hemodiálise (OR=0.23, p<0.004). Em nove pacientes em hemodiálise que permaneceram em hemodiálise, uma segunda biópsia foi realizada com intervalo médio de 66 meses (40-88). Houve progressão de pelo menos um ponto na escala de fibrose do sistema Metavir em três pacientes, com taxa de progressão direta de 0.15/ano. Conclusões: A menor atividade inflamatória tecidual hepática e bioquímica observada nos pacientes renais crônicos sugere que a hemodiálise e a uremia podem atuar como fator protetor na doença causada pelo vírus da hepatite C. ___________________________________________________________________________________ ABSTRACT / Background: The severity of liver disease among hepatitis C patients on hemodialysis is controversial. Objective: The aim of this study was to compare the clinical, biochemical and liver histological characteristics of HCV in hemodialysis patients with those with normal renal function. Methods: A case-control study was carried out with 36 HCV patients on hemodialysis and 37 HCV patients with normal renal function matched for gender, age at infection, and estimated time of infection. Moreover, a prospective study was performed to compare two liver biopsies obtained at an interval of at least three years in hemodialysis patients who did not receive a kidney transplant. Results: HCV patients on hemodialysis had lower levels of alanina aminotransferase and lower viral load. Hepatic fibrosis was significantly higher in the patients with normal renal function (73%) than in hemodialysis patients (47,2% p<0.025) as well as inflammatory activity (control group 59.5% vs. hemodialysis patients 27.7%, p=0.003). In addition, the risk of tissue inflammation was four times lower in hemodialysis patients (OR=0.23, p<0.004) and severe inflammatory activity on biopsy was the only independent risk factor for fibrosis (p<0.001). In two liver biopsies performed at a mean interval of 66 months (40-88) in nine hemodialysis patients, fibrosis progressed 1 point (Metavir scale) only in three patients (mean direct progression rate 0.15/year). Conclusion: The lower biochemical and inflammatory activities observed in hemodialysis patients suggest that hemodialysis and uremia may have a protective role against progression of the disease caused by HCV.

Fatores como o genótipo do vírus, a carga viral e características do hospedeiro estão envolvidos com o sucesso da terapia. Os fatores do hospedeiro, especialmente, os imunológicos e genéticos parecem ter um papel importante no resultado do tratamento da hepatite C. Algumas proteínas como a citocina TGF-β1 tem sido associada com as complicações decorrentes de produção excessiva de fibrose, associada com condições inflamatórias crônicas. Objetivo: O objetivo deste estudo foi avaliar a associação de polimorfismos genéticos da citocina TGF-β1 e o estágio da fibrose na hepatite crônica C. Métodos: Este é um estudo de caso-controle não pareado, onde foram avaliados pacientes monoinfectados com vírus da hepatite C (VHC) tratados com terapia combinada com interferon convencional e/ou peguilado associado a ribavirina, os quais foram divididos em dois grupos, de acordo com os estágios de fibrose: fibrose hepática leve a moderada < F2 e fibrose hepática avançada F3 e F4 e um grupo controle de uma amostra histórica de doadores de sangue. Sendo realizada a genotipagem da TGF-β1 pelo método PCR-SSP (Kit One Lambda). Resultados: Foram encontradas diferenças estatisticamente significantes entre o grupo de indivíduos com hepatite crônica C e doadores de sangue. Houve maior frequência do alelo G (p= 0,0021) nos indivíduos com hepatite crônica C como também observamos maior frequência do genótipo GG (p=0,0029) entre os indivíduos com hepatite crônica C. Conclusões: Os dados obtidos sugerem que os polimorfismos do gene TGFB1 nos códons 10 e 25 não estão associados com a fibrose hepática em indivíduos com hepatite crônica C e que o polimorfismo do gene TGFB1 no códon 25 tem maior frequência nos indivíduos com hepatite crônica C quando comparados com indivíduos sadios, podendo ser um marcador de susceptibilidade a infecção pela hepatite crônica C.

Depressão, função cognitiva e qualidade de vida em pacientes com hepatite crônica C: efeitos do tratamento antiviral / Depression, cognitive function and quality of life in patients with chronic hepatitis C: effects of antiviral treatment

Introdução: Tanto o vírus da hepatite crônica C (VHC) quanto o tratamento antiviral estão associados a transtornos do humor, piora da função cognitiva e da qualidade de vida. Objetivos: avaliar os pacientes com hepatite crônica pelo VHC antes, durante e após o tratamento antiviral em relação aos seguintes aspectos: depressão maior e sintomas depressivos, função cognitiva e qualidade de vida. Métodos: estudo clínico, prospectivo, cujos critérios de inclusão foram: idade 18 a 70 anos; clinicamente compensados. Critérios de exclusão: uso ativo de substâncias psicoativas ilícitas; confusão mental; encefalopatia hepática e recusa em participar do estudo ou em receber terapia antiviral. Utilizamos os seguintes instrumentos: 1) Diagnóstico de depressão maior: entrevista estruturada Primary Care Evaluation of Mental Disorders (PRIME-MD); 2) Sintomas depressivos: o Beck Depression Inventory (BDI). 3) Avaliação da qualidade de vida: questionários SF-36 e Liver Disease Quality of Life Questionnaire instrument (LDQOL). 4) Avaliação neuropsicológica: a) funções executivas - atenção seletiva e inibição (Stroop Teste Versão Victoria), atenção sustentada e alternada (Trail Making Test - partes A e B), fluência verbal fonológica (FAS) e fluência verbal semântica ou categórica. b) aprendizagem verbal e memória de longo-prazo (Hopkins Verbal Learning Test HVLT-R); c) memória de curtoprazo e memória operacional (Dígitos ordem direta e Dígitos de ordem inversa); d) teste de QI estimado. Os testes foram aplicados previamente ao início do tratamento, durante e após 6 meses do término do tratamento. Resultados: dos 78 pacientes avaliados, 36 completaram a avaliação de humor e qualidade de vida e 34, a avaliação neuropsicológica. Os 21 pacientes que completaram o tratamento foram considerados para análise final (n=16 com resposta virológica sustentada RVS e n=5, sem RVS). Quanto ao transtorno de humor, dentre os pacientes com RVS, observamos depressão maior em 9,5% antes do tratamento e 52,4% durante (p=0,012). Ao término do tratamento, a frequência reduziu-se a 6,3 % (p=1,0). Em relação aos sintomas depressivos, encontramos uma frequência de 19,1%, 62,0% e 25,0% antes, durante e após o tratamento, respectivamente. Diferença estatisticamente significante foi observada nas frequências antes e durante o tratamento (p=0,016). Dentre os 5 pacientes que cursaram sem RVS, um apresentou critério para depressão maior e 2 para sintomas depressivos. Quanto à função cognitiva, houve melhora no domínio da atenção seletiva (p=0,004) durante o tratamento. Ao seu término, houve melhora significante nos domínios: memória episódica verbal imediata (p=0,014), memória episódica verbal tardia (p=0,024), atenção seletiva (p=0,001) e fluência verbal fonológica (p=0,030). Comparando-se os pacientes com e sem RVS, observamos melhora significante nos domínios: memória episódica verbal imediata (p=0,045) e memória episódica verbal tardia (p=0,040). Em relação à qualidade de vida, houve piora significante da autopercepção em todos os domínios avaliados ao longo do tratamento, exceto no estado geral de saúde, no questionário SF-36. No questionário LDQOL, observou-se piora ao longo do tratamento nos domínios sintomas (p=0,000), efeitos da doença hepática (p=0,007), concentração (p=0,008), questões sociais (p=0,002) e função sexual (p=0,010). Ao término do tratamento, observou-se melhora significante nos domínios efeitos da doença hepática (p=0,024), preocupação com a doença (p=0,018) e estigma da doença hepática (p=0,008). Comparando-se os pacientes com e sem RVS, observamos melhora significante no domínio efeitos da doença hepática (p=0,005) naqueles pacientes com RVS. Regarding depressive symptoms, we found a frequency of 19.1%, 62.0% and 25.0% before, during and after treatment, respectively. Significant differences in frequency before and during treatment were observed (p=0.016). Among the 5 patients who did not achieve SVR, one fulfilled diagnostic criteria for major depression and 2 for depressive symptoms. Regarding cognitive function, there was an improvement in the field of selective attention (p=0.004) during treatment. Upon termination of treatment, there was a significant improvement in these domains: immediate verbal episodic memory (p=0.014), delayed verbal episodic memory (p=0.024), selective attention (p=0.001) and phonological verbal fluency (p=0.030). Comparing patients with and without SVR, we observed a significant improvement in these domains: immediate verbal episodic memory (p=0.045) and delayed verbal episodic memory (p=0.040). Com o uso desses agentes taxas de Resposta Virológica Sustentada mais elevadas foram obtidas, mas também maior gravidade de eventos adversos. No Brasil, não há dados a respeito do impacto do telaprevir sobre a qualidade de vida relacionada à saúde (QVRS) em portadores de hepatite crônica C submetidos à terapia antiviral. Objetivo: Comparar os escores de QVRS em pacientes com hepatite crônica C submetidos à terapia antiviral dupla e tripla e analisar os possíveis fatores que afetam a QVRS. Casuísitca e método: Estudo do tipo observacional com inclusão prospectiva dos dados. Foram incluídos pacientes com Hepatite Crônica C submetidos a terapia antiviral com idade entre 18 e 75 anos acompanhados no HU-UFJF, no período de março de 2012 a julho de 2014. Foram excluídos coinfectados pelo HIV, VHB e portadores de doença renal crônica. Os pacientes em terapia dupla foram tratados com interferon peguilado alfa 2a ou alfa 2b (INF) associado à ribavirina (RIB) enquanto os pacientes do grupo terapia tripla receberam INF, RIB e telaprevir (TEL) nas primeiras 12 semanas seguido da terapia dupla. A classificação histológica utilizada foi a de Metavir. O Short Form 36 (SF-36) e Chronic Liver Diseases Questionnarie (CLDQ), questionários de avaliação de QVRS, foram aplicados no: pré-tratamento e nas semanas 4, 12 e 16. A comparação entre os grupos foi realizada utilizando o test-t (t). Os fatores preditivos foram calculados através da regressão linear múltipla. Valores de P < 0,05 foram considerados significantes. Resultados: Foram incluídos 32 pacientes, dos quais 17 em terapia dupla e 15 em terapia tripla. O sexo feminino foi o predominante (59%) e a média de idade observada foi 54,9 ± 15,85 anos. A maioria dos pacientes foram genótipo 1 (75,0%) e cirróticos (34%). No pré-tratamento, a média do componente físico (51,5 ± 8,40 vs. 53,8 ± 6,50; p=0,413) e mental (60,1 ± 3,80 vs. 55,1 ± 9,11; p = 0,067) resumido (CFR e CMR) foi semelhante entre o grupo terapia dupla e tripla. Após o início do tratamento (semana 4 e 12), a média do CFR e CMR foi pior na terapia tripla do que na dupla (p<0,05). Após a retirada do TEL, as médias se aproximaram, o mesmo esta ocorrendo com o escore total do CLDQ. Conclusão: Em portadores de hepatite crônica C, a terapia tripla foi associada de modo independente a redução dos escores de QVRS tanto pelo SF-36 quanto pelo CLDQ, sendo o momento mais crítico a 12ª semana de terapia. Outros fatores associados à redução dos índices de qualidade de vida foram ansiedade e depressão no prétratamento, status empregatício e raça. / Introduction: In Brazil, patients with chronic HCV genotype 1 with advanced fibrosis are treated with pegylated interferon, ribavirin and direct antiviral action telaprevir and boceprevir. With the use of these agents higher SVR rates were obtained, but also increased severity of adverse events. In Brazil, there are no data regarding the impact of telaprevir on HRQL in patients with chronic hepatitis C undergoing antiviral therapy. Objective: To compare the scores of HRQL in patients with chronic hepatitis C undergoing antiviral therapy double and triple. Casuistic and method: Study of observational with prospective inclusion of data. Patients with Chronic Hepatitis C undergoing antiviral therapy between 18 and 75 years followed at HU-UFJF, from March 2012 to July 2014. Exclusion criteria were co-infected with HIV, HBV and patients with chronic kidney disease. Patients on dual therapy were treated with pegylated interferon alpha 2a or alpha 2b (INF) in combination with ribavirin (RIB) whereas patients in the triple therapy group received INF, RIB and telaprevir (TEL) in the first 12 weeks followed by dual therapy. The histological classification was used to Metavir. The Short Form 36 (SF-36) and Chronic Liver Diseases Questionnaire (CLDQ), the evaluation of HRQL questionnaires were applied in: baseline and at weeks 4, 12 and 16. The comparison between groups was performed using the tests: test -t (t). Predictive were calculated by multiple linear regression. P values <0.05 were considered significant. Results: Were include 32 patients, 17 of whom were enrolled in dual therapy and 15 on triple therapy. The female sex was predominant (59%) and the average age was 54.9 ± 15.85 years. Most patients was genotype 1 patients (75.0%) and cirrhotic patients (34%). At baseline, the physical component (51.5 ± 8.40 vs. 53.8 ± 6.50; p = 0.413) and mental (60.1 ± 3.80 vs. 55.1 ± 9.11; p = 0.067) summarized (PCS and MCS) was similar between the dual and triple therapy group. After the initiation of treatment (Week 4 and 12) the scores of PCS and MCS, were worse in the triple therapy than double (p <0.05). After removal of the drug TEL, the HRQL improved in the two therapies. The same occurs with the total score of CLDQ. Conclusion: Patients with chronic hepatitis C, triple therapy was independently associated with reduced HRQOL scores both the SF-36 as the CLDQ, the most critical time to 12 weeks of therapy. Other factors that are associated with reduced quality of life indices are anxiety and depression at baseline, employment status and race.

Sabe-se que o vírus da imunodeficiência humana (HIV) pode modificar a história natural da infecção pelo HCV, acelerando a progressão da fibrose hepática e conseqüente evolução para cirrose. O tratamento com interferon-alfa pode levar à melhora da histologia hepática, reduzindo a inflamação e fibrose, principalmente nos pacientes que apresentam resposta virológica sustentada. O impacto do tratamento sobre a evolução histológica em pacientes não-respondedores ao tratamento da hepatite C apresenta resultados controversos. Objetivos: Avaliar, nos pacientes com hepatite C crônica, co-infectados pelo HIV, o impacto do tratamento da hepatite C, sobre as modificações de parâmetros indicativos de fibrose e atividade inflamatória, em biópsias de fígado realizadas antes e após o tratamento da hepatite C. Métodos: Foram estudados 26 pacientes co-infectados pelo HCV e HIV-1, submetidos à biópsia hepática antes e, em média, 25,2 meses após o término do tratamento da hepatite C. Fragmentos da biópsia hepática foram comparados, antes e após o tratamento, em relação aos seguintes parâmetros: Índice de Atividade Histológica (HAI) e o grau de Fibrose (Knodell); intensidade do depósito de colágeno (coloração pelo picrosirius) e o grau de ativação das células estreladas (marcação com alfa-actina de músculo liso). Os índices destas variáveis histológicas, pós e pré-tratamento, foram relacionadas com o resultado do teste qualitativo RNA HCV (RT-PCR-AMPLICOR?) na 24a semana de tratamento, com a duração do tratamento da hepatite C e com o intervalo de tempo entre a interrupção do tratamento e a biópsia-controle. Foi avaliada a relação de possíveis fatores prognósticos de resposta virológica com a resposta histológica e foram realizadas correlações dos parâmetros histológicos entre si. Resultados: Os parâmetros histológicos avaliados de forma global foram semelhantes nas biópsias pré e pós-tratamento. As razões pós/pré-tratamento, em todos os parâmetros avaliados nas biópsias hepáticas, diminuíram de forma significativa nos pacientes que apresentaram pesquisa negativa para o RNA HCV na 24ª semana de tratamento. Os parâmetros histológicos foram semelhantes nos grupos com duração de tratamentos diferentes (? 24 semanas e > 24 semanas), exceto as CEH células ?-positivas, que teve melhora no grupo com tratamento mais prolongado. O intervalo de tempo entre o final do tratamento e a biópsia-controle não influenciou a melhora histológica. Na análise de regressão logística, houve associação da melhora dos parâmetros histológicos com o resultado negativo do RNA HCV na 24ª semana de tratamento. Houve correlação significativa entre todos os parâmetros histológicos avaliados. Conclusões: Os pacientes avaliados em conjunto tiveram seus parâmetros histológicos nas biópsias pré e pós-tratamento com valores semelhantes, porém quando se definiu melhora histológica, como a manutenção, ou melhora dos parâmetros histológicos avaliados individualmente, verificou-se que uma substancial parcela da população estudada apresentou melhora histológica. O efeito benéfico do tratamento parece se relacionar com o controle da viremia do HCV durante o tratamento e se verifica também em pacientes que não sustentaram a resposta virológica após a suspensão dos medicamentos. Os achados fazem supor que a redução da viremia do HCV teria como conseqüência a diminuição da inflamação e fibrose hepáticas, inclusive com diminuição do estado de ativação das células estreladas hepáticas, mesmo nos casos em que não houve resposta virológica sustentada. Os resultados do presente estudo são sugestivos de que o tratamento da hepatite C pode modificar a história natural da evolução da hepatite crônica nos pacientes co-infectados pelo HIV. / Human immunodeficiency virus (HIV) is known to be able to modify the natural history of HCV infection, accelerating the progression of hepatic fibrosis and the consequent evolution to cirrhosis. Treatment with interferon-alpha can lead to improved hepatic histology, reducing inflammation and fibrosis, especially in patients who present a sustained virologic response. The impact of treatment on histological evolution in patients who do not respond to treatment of hepatitis C is a controversial matter. Objectives: To evaluate in patients with chronic hepatitis C co-infected with HIV the impact of treatment of hepatitis C on the modifications of the parameters indicative of fibrosis and of inflammatory activity in liver biopsies obtained before and after treatment of hepatitis C. Methods: Twenty-six patients co-infected with HCV and HIV-1 were submitted to a liver biopsy before and, on average, 25.2 months after the end of treatment of hepatitis C. Fragments of the liver biopsy were compared before and after treatment regarding the following parameters: histological activity index (HAI) and grade of fibrosis (Knodell); intensity of collagen deposition (picrosirius staining), and grade of stellate cell activation (labeling with smooth muscle alpha-actin). The post- and pretreatment ratios of these histological variables were related to the result of the quantitative HCV-RNA test (RT-PCR-AMPLICOR?) during the 24th week of treatment, to the duration of treatment of hepatitis C and to the time interval between the discontinuation of treatment and the control biopsy. The relationship between possible factors of the virologic response and the histological response was evaluated and correlations between the various histological parameters were calculated. Results: The histological parameters evaluated in a global manner were similar in the pre- and post-treatment biopsies. The post-pretreatment ratios for all parameters evaluated in the liver biopsies were significantly reduced in patients who were negative for HCV RNA during the 24th week of treatment. The histological parameters were similar in the groups with different durations of treatment (? 24 weeks and > 24 weeks), except for ?-positive stellate hepatic cells, which were improved in the group with a more prolonged treatment. The time interval between the end of treatment and the control biopsy did not affect the histological improvement. In logistic regression analysis, the improvement of the histological parameters was found to be associated with a negative result of HCV RNA during the 24th week of treatment. There was a significant correlation between all histological parameters evaluated. Conclusions: When evaluated as a whole, the patients showed similar values for their histological parameters in the pre- and post-treatment biopsies. However, when histological improvement was defined as the maintenance or improvement of the histological parameters evaluated individually, a substantial part of the study population was found to present histological improvement. The beneficial effect of treatment seemed to be related to the control of HCV viremia during treatment and was also observed in patients who did not sustain the virologic response after the discontinuation of the medications. These findings lead us to assume that the reduction of HCV viremia causes a reduction of hepatic inflammation and fibrosis, as well as a reduction of the state of activation of stellate hepatic cells even in cases in which there is no sustained virologic response. The results of the present study suggest that treatment of hepatitis C can modify the natural history of the course of chronic hepatitis in patients co-infected with HIV.

A mortalidade associada à hepatite A aguda pode estar particularmente elevada em pacientes com pré-existência de hepatite crônica causada pelo VHC. Por esta razão, a imunização ativa com vacinas contra o VHA e o VHB vem a ser obrigatória nesta população, e consequentemente esta sorologia deve ser determinada. Objetivos: O objetivo deste trabalho foi avaliar a prevalência de marcadores sorológicos da hepatite A e hepatite B em 1.000 pacientes com infecção crônica pelo VHC atendidos no Ambulatório de Hepatites da Divisão de Gastroenterologia e Hepatologia Clínica do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo. Resultados: O anti-VHA IgG foi positivo em 923 de 1000 pacientes (92,3%). Quando estratificados por idade, o anti-VHA IgG foi encontrado em 61% dos pacientes entre 20 e 29 anos, 70% entre 30 e 39 anos, 85% entre 40 e 49 anos, 94% entre 50 e 59 anos e 99% nos pacientes com mais de 60 anos . O anti-HBc total foi positivo em 244 pacientes (24%). Estratificados por idade, em 4,3% dos pacientes entre 20 e 29 anos, 17% entre 30e 39 anos, 21% entre 40 e 49 anos, 24% entre 50 e 59 anos, e 28% dos pacientes com mais de 60 anos. Dos 244 pacientes anti-HBc IgG positivos, 0,8% são HBsAg positivo, 8,5% anti-HBc IgG isolado e 16% anti-HBs positivo. Conclusões: A prevalência de anti-VHA IgG nod nossos pacientes com hepatite C crônica foi semelhante à da população geral no município de São Paulo. Of the 244 anti-HBc IgG positive patients, 0.8% were also HBsAg positive, 8.5% were anti-HBc IgG isolated and 16% were also anti-HBs positive. Conclusions: The prevalence of anti-HAV IgG was similar to the general population in the city of São Paulo. However, anti-HBc IgG was higher in our chronic HCV patients, when compared historically to the general population of western countries, suggesting similar risk factors for HBV and HCV acquisition, so emphasizing the importance of immunization programs in this population

Sabe-se que o vírus da imunodeficiência humana (HIV) pode modificar a história natural da infecção pelo HCV, acelerando a progressão da fibrose hepática e conseqüente evolução para cirrose. O tratamento com interferon-alfa pode levar à melhora da histologia hepática, reduzindo a inflamação e fibrose, principalmente nos pacientes que apresentam resposta virológica sustentada. O impacto do tratamento sobre a evolução histológica em pacientes não-respondedores ao tratamento da hepatite C apresenta resultados controversos. Objetivos: Avaliar, nos pacientes com hepatite C crônica, co-infectados pelo HIV, o impacto do tratamento da hepatite C, sobre as modificações de parâmetros indicativos de fibrose e atividade inflamatória, em biópsias de fígado realizadas antes e após o tratamento da hepatite C. Métodos: Foram estudados 26 pacientes co-infectados pelo HCV e HIV-1, submetidos à biópsia hepática antes e, em média, 25,2 meses após o término do tratamento da hepatite C. Fragmentos da biópsia hepática foram comparados, antes e após o tratamento, em relação aos seguintes parâmetros: Índice de Atividade Histológica (HAI) e o grau de Fibrose (Knodell); intensidade do depósito de colágeno (coloração pelo picrosirius) e o grau de ativação das células estreladas (marcação com alfa-actina de músculo liso). Os índices destas variáveis histológicas, pós e pré-tratamento, foram relacionadas com o resultado do teste qualitativo RNA HCV (RT-PCR-AMPLICOR?) na 24a semana de tratamento, com a duração do tratamento da hepatite C e com o intervalo de tempo entre a interrupção do tratamento e a biópsia-controle. Foi avaliada a relação de possíveis fatores prognósticos de resposta virológica com a resposta histológica e foram realizadas correlações dos parâmetros histológicos entre si. Resultados: Os parâmetros histológicos avaliados de forma global foram semelhantes nas biópsias pré e pós-tratamento. As razões pós/pré-tratamento, em todos os parâmetros avaliados nas biópsias hepáticas, diminuíram de forma significativa nos pacientes que apresentaram pesquisa negativa para o RNA HCV na 24ª semana de tratamento. Os parâmetros histológicos foram semelhantes nos grupos com duração de tratamentos diferentes (? 24 semanas e > 24 semanas), exceto as CEH células ?-positivas, que teve melhora no grupo com tratamento mais prolongado. O intervalo de tempo entre o final do tratamento e a biópsia-controle não influenciou a melhora histológica. Na análise de regressão logística, houve associação da melhora dos parâmetros histológicos com o resultado negativo do RNA HCV na 24ª semana de tratamento. Houve correlação significativa entre todos os parâmetros histológicos avaliados. Conclusões: Os pacientes avaliados em conjunto tiveram seus parâmetros histológicos nas biópsias pré e pós-tratamento com valores semelhantes, porém quando se definiu melhora histológica, como a manutenção, ou melhora dos parâmetros histológicos avaliados individualmente, verificou-se que uma substancial parcela da população estudada apresentou melhora histológica. O efeito benéfico do tratamento parece se relacionar com o controle da viremia do HCV durante o tratamento e se verifica também em pacientes que não sustentaram a resposta virológica após a suspensão dos medicamentos. Os achados fazem supor que a redução da viremia do HCV teria como conseqüência a diminuição da inflamação e fibrose hepáticas, inclusive com diminuição do estado de ativação das células estreladas hepáticas, mesmo nos casos em que não houve resposta virológica sustentada. Os resultados do presente estudo são sugestivos de que o tratamento da hepatite C pode modificar a história natural da evolução da hepatite crônica nos pacientes co-infectados pelo HIV. / Human immunodeficiency virus (HIV) is known to be able to modify the natural history of HCV infection, accelerating the progression of hepatic fibrosis and the consequent evolution to cirrhosis. Treatment with interferon-alpha can lead to improved hepatic histology, reducing inflammation and fibrosis, especially in patients who present a sustained virologic response. The impact of treatment on histological evolution in patients who do not respond to treatment of hepatitis C is a controversial matter. Objectives: To evaluate in patients with chronic hepatitis C co-infected with HIV the impact of treatment of hepatitis C on the modifications of the parameters indicative of fibrosis and of inflammatory activity in liver biopsies obtained before and after treatment of hepatitis C. Methods: Twenty-six patients co-infected with HCV and HIV-1 were submitted to a liver biopsy before and, on average, 25.2 months after the end of treatment of hepatitis C. Fragments of the liver biopsy were compared before and after treatment regarding the following parameters: histological activity index (HAI) and grade of fibrosis (Knodell); intensity of collagen deposition (picrosirius staining), and grade of stellate cell activation (labeling with smooth muscle alpha-actin). The post- and pretreatment ratios of these histological variables were related to the result of the quantitative HCV-RNA test (RT-PCR-AMPLICOR?) during the 24th week of treatment, to the duration of treatment of hepatitis C and to the time interval between the discontinuation of treatment and the control biopsy. The relationship between possible factors of the virologic response and the histological response was evaluated and correlations between the various histological parameters were calculated. Results: The histological parameters evaluated in a global manner were similar in the pre- and post-treatment biopsies. The post-pretreatment ratios for all parameters evaluated in the liver biopsies were significantly reduced in patients who were negative for HCV RNA during the 24th week of treatment. The histological parameters were similar in the groups with different durations of treatment (? 24 weeks and > 24 weeks), except for ?-positive stellate hepatic cells, which were improved in the group with a more prolonged treatment. The time interval between the end of treatment and the control biopsy did not affect the histological improvement. In logistic regression analysis, the improvement of the histological parameters was found to be associated with a negative result of HCV RNA during the 24th week of treatment. There was a significant correlation between all histological parameters evaluated. Conclusions: When evaluated as a whole, the patients showed similar values for their histological parameters in the pre- and post-treatment biopsies. However, when histological improvement was defined as the maintenance or improvement of the histological parameters evaluated individually, a substantial part of the study population was found to present histological improvement. The beneficial effect of treatment seemed to be related to the control of HCV viremia during treatment and was also observed in patients who did not sustain the virologic response after the discontinuation of the medications. These findings lead us to assume that the reduction of HCV viremia causes a reduction of hepatic inflammation and fibrosis, as well as a reduction of the state of activation of stellate hepatic cells even in cases in which there is no sustained virologic response. The results of the present study suggest that treatment of hepatitis C can modify the natural history of the course of chronic hepatitis in patients co-infected with HIV.

HOMA-IR superior a 2,5 foi considerado resistência insulínica. Resultados: RI foi observada em 27% dos pacientes e foi associada a idade, circunferência abdominal e índice de massa corpórea. Quando comparado a pacientes sem RI, aqueles com HOMA-IR superior a 2,5 apresentaram graus mais acentuados de fibrose hepática e atividade necroinflamatória, maiores níveis de aminotransferases e esteatose hepática mais freqüente. Conclusões: É comum a presença de RI em portadores de hepatite crônica C e esta tem como resultado a acentuação da fibrose hepática induzida pelo vírus da hepatite C. DESCRITORES: Hepatite crônica C, fibrose hepática, resistência insulínica, HOMA-IR / Objectives: To determine the prevalence of insulin resistance (IR) in patients nondiabetic with chronic hepatitis C and analyze the impact of insulin resistance on laboratory and histology parameters. Subjects and methods: The study included 82 patients and blood samples were collected for glucose, lipid profile, CRP, ferritin, ALT, AST, HOMA-IR, viral load and HCV genotype. HOMA-IR greater than 2.5 were considered insulin resistance. Results: Insulin resistance was observed in 27% of patients and was associated with age, waist circumference and body mass index. When compared with patients without RI those with insulin resistance had more pronounced stage of hepatic fibrosis and necroinflammatory activity, higher levels of aminotransferases and liver steatosis more frequently. Conclusions: It is often the presence of insulin resistance in patients with chronic hepatitis C and this has resulted in the exacerbation of liver fibrosis induced by HCV.
