O fonoaudiólogo e os pais: uma parceria para o desenvolvimento da comunicação de crianças do espectro do autismo / Speech therapists and parents: a linked to the development of children\'s communication on the autism spectrum

Introdução: a linguagem representa um aspecto fundamental para a vida em sociedade. No entanto, o desenvolvimento sociocomunicativo está atrelado à forma como os pais percebem seus filhos, às atribuições de significados sobre as manifestações da criança e ao lugar em que essa criança ocupa no núcleo familiar. Em indivíduos com transtornos do desenvolvimento esses aspectos são igualmente preponderantes. As crianças com Transtorno do Espectro do Autismo (TEA) têm dificuldades em entender e usar a linguagem, especialmente em situações sociais. Assim, devido à complexidade do desenvolvimento comunicativo, é essencial incluir e dar suporte comunicativo aos pais e/ ou cuidadores de crianças com TEA, ampliando as condições e possibilidades psicossociais. Objetivo: verificar os benefícios de um Programa de Orientações sobre Comunicação para Cuidadores (POCC) de crianças com TEA analisando seus efeitos na percepção dos cuidadores quanto ao perfil funcional da comunicação destas crianças e quanto à percepção das dificuldades comunicativas. Método: Participaram desta pesquisa cuidadores de 62 crianças com diagnóstico de TEA, divididos em três grupos de intervenções. Todas as intervenções envolviam um programa de cinco sessões de orientações mensais pré-estabelecidas, visando fornecer informações sobre o desenvolvimento da comunicação e incentivar atividades práticas de comunicação no cotidiano. No primeiro grupo, os cuidadores receberam o programa de orientações em grupo e as crianças tratamento fonoaudiológico individual. No segundo, os cuidadores receberam o mesmo programa de orientações, mas de forma individual, e seus filhos terapia individual. O terceiro grupo, composto por cuidadores de crianças que aguardavam atendimento fonoaudiológico em lista de espera, recebeu as orientações em grupo. Em todos os grupos foram investigadas as percepções dos cuidadores quanto a competência funcional de comunicação das crianças conforme o Perfil Funcional da Comunicação-Checklist (PFC-C). Além disso buscou-se compreender a percepção de dificuldades comunicativas dos mesmos quanto ao Questionário sobre Dificuldades Comunicativas (QDC) percebidas por pais e/ou cuidadores de crianças com TEA. Os questionários foram aplicados em três momentos: marco zero e intervalos de cinco e de oito meses. Os participantes responderam um questionário de satisfação ao final do segundo intervalo, para avaliação do programa de orientações. Resultados: No PFC-C os pais relataram um aumento da ocorrência dos meios gestual, vocal e verbal em todos os grupos, para expressar as funções comunicativas interpessoais, exceto no grupo de cuidadores que recebeu o programa de orientações em grupo e as crianças tratamento fonoaudiológico individual. Nas funções comunicativas não interpessoais notou-se uma diminuição na ocorrência do meio comunicativo gestual, um aumento no meio verbal, sem diferença estatística entre os grupos. Quanto ao meio vocal, não houve diferença ao longo do tempo. No QDC observou-se a diminuição das dificuldades comunicativas dos cuidadores, sem diferença entre os grupos. O questionário de satisfação demonstrou altas pontuações quanto à realização do programa para todos os grupos. Conclusões: o Programa de Orientações sobre Comunicação para Cuidadores de crianças com TEA apresentado parece contribuir para o entendimento do processo comunicativo em diferentes situações, associados ou não ao tratamento fonoaudiológico, notando a diminuição de dificuldades comunicativas e aprimorando a percepção sobre a funcionalidade da comunicação / Introduction: language represents a fundamental aspect for life in society. However, sociocommunicative development is linked to the way parents perceive their children, the attributions of meanings about the child\'s manifestations, and the place where the child occupies in the family nucleus. In individuals with developmental disorders these aspects are equally prevalent. Children with Autism Spectrum Disorder (ASD) have difficulty understanding and using language, especially in social situations. Thus, due to the complexity of communicative development, it is essential to include and provide communicative support to parents and / or caregivers of children with ASD, expanding psychosocial conditions and possibilities. Objective: to verify the benefits of a Caregivers Communicative Orientation Program (CCOP) of children with ASD, analyzing their effects on the perception of caregivers as to the communication profile of these children and the perception of communicative difficulties. Method: Caregivers of 62 children diagnosed with ASD participated in this study, divided into three groups of interventions. All interventions include a program with five pre-established monthly orientation sessions to provide information on the development of communication and encourage practical communication activities in daily life. In the first group, caregivers received the group orientation program and the children received individual speechlanguage therapy. In the second, caregivers received the same program guidelines, but individually, and their children individual therapy. The third group, comprised of caregivers of children who waited for speech-language therapy on the waiting list, received group orientation. In all groups, the perceptions of the caregivers regarding the functional competence of communication of the children according to the Functional Communication Profile -Checklist (FCP-C) were investigated. In addition, we sought to understand the perception of their communicative difficulties regarding the Questionnaire on Communicative Difficulties (QDC) perceived by parents and / or caregivers of children with ASD. The questionnaires were applied in three moments: baseline and intervals of five and eight months. The participants answered a satisfaction questionnaire at the end of the second interval, to evaluate the orientation program. Results: In the FCP-C the parents reported an increase in the occurrence of the gestual, vocal and verbal means in all the groups, to express the interpersonal communicative functions, except in the group of caregivers who received the group orientation program and the children speech-language therapy individual. In the non-interpersonal communicative functions, there was a decrease in the occurrence of the gestural communicative medium, an increase in the verbal environment, with no statistical difference between the groups. As for the vocal environment, there was no difference over time. The QDC observed the decrease in the communicative difficulties of the caregivers, with no difference between the groups. The satisfaction questionnaire showed high scores regarding the achievement of the program for all groups. Conclusions: The Caregivers Communicative Orientation Program of children with ASD seems to contribute to the understanding of the communicative process in different situations, associated or not with speech-language therapy, noting the reduction of communicative difficulties and improving the perception about communication functionality

Introduction and rationale: Clinical experience with infants presenting with feeding difficulties and increasing referrals of infants with colic led to this study. There is uncertainty whether the speechlanguage therapist should play a role in the management of this condition. Despite extensive clinical research into the multiple factors related to the condition, the etiology of infantile colic has not yet been established. Suck- swallow-breathing coordination (SSBC), which is key to successful feeding, has not yet been investigated as a factor in this condition. Goals: The goal of the study was to give a comprehensive description of the symptoms and the SSBC of a group of young infants with colic. Sub-goal 1 was to compile a list of symptoms based on a description by parents of infants with infantile colic, which could be used to compare symptoms of a group of infants with the condition to a group without the condition. Sub-goal 2 was to describe SSBC, in a group of infants with the condition, by conducting a clinical assessment. Sub-goal 3 was to compare SSBC in a group of infants with colic to a group without the condition of the same age. Method: A descriptive survey design and interview guide was used in Phase 1. The parents of 60 infants with colic participated in this study. A correlation research design (Phase 2) and the List of symptoms for Infantile Colic (compiled in Phase 1) as well as the Assessment Protocol for SSBC (compiled in Phase 2) were used. A new research group of 50 participants with infantile colic and a control group of 28 participants with the condition, whose ages correlated with those of the research group, was selected. An interview was conducted with the parents of all participants. SSBC was clinically assessed in all the participants. Results: Parental description of infantile colic resulted in a comprehensive list of 27 symptoms. Audible swallowing of air and a feeding duration of more than 20 minutes were described more by parents than found in the literature. Statistically significant differences were found when the postural control and SSBC of the research group were compared with the control group. The difficulties with SSBC differed across the age categories of the participants. Conclusion: The study indicated that infantile colic is related to a disturbance in postural control and components of SSBC, and infants with the condition have subtle feeding difficulties. INTRODUÇÃO: A Dislexia do Desenvolvimento é um transtorno específico da aprendizagem, caracterizado como um comprometimento apresentado pelo indivíduo ao realizar a leitura, mesmo com inteligência, motivação e instrução escolar adequadas. Possui base neurobiológica de origem genética. Embora a literatura disponibilize muitos estudos sobre a dislexia que enfocam a elaboração e o uso de instrumentos de avaliação da compreensão leitora, consciência fonológica, entre outros, ainda são pouco divulgados os programas de intervenção em compreensão de leitura para essa população. A Técnica de Cloze, que consiste na organização de um texto curto com lacunas, tem sido caracterizada como um método eficaz na mensuração e na remediação da compreensão leitora. OBJETIVOS: Elaborar um Programa Fonoaudiológico em Compreensão Leitora e Ortografia e verificar seus efeitos na compreensão de leitura de escolares com dislexia. MÉTODOS: Fizeram parte dessa pesquisa onze indivíduos diagnosticados com dislexia do desenvolvimento, entre 09 e 11 anos, desses, 8 eram meninos. O programa foi composto por 16 sessões divididas em quatro níveis de complexidade. Todas as sessões contaram com um texto adaptado com a Técnica de Cloze, atividades de compreensão leitora e ortografia. Os sujeitos foram submetidos ao pré e pós-teste, com a avaliação da compreensão leitora e da ortografia. RESULTADOS: Os achados demonstraram que os sujeitos apresentaram melhor desempenho de compreensão leitora no pós em comparação ao pré-teste, tanto na porcentagem de acertos no Cloze e nas perguntas de compreensão textual, quanto no nível de compreensão de leitura atingido. Além disso, foram observadas generalizações dos ganhos de compreensão leitora para outros contextos. CONCLUSÕES: O Programa Fonoaudiológico de Compreensão Leitora e Ortografia foi um instrumento relevante na remediação da compreensão de leitura dos participantes dessa pesquisa, por desenvolver melhora na compreensão leitora tanto de textos com Cloze quanto em perguntas de compreensão e por produzir leitores mais motivados. Além disso, concluiu-se que tanto a Técnica de Cloze quanto as perguntas de compreensão textual são instrumentos precisos para avaliar a compreensão de leitura dos indivíduos com dislexia. Esse estudo foi inovador por elaborar e verificar os efeitos de um programa específico de remediação para crianças com dislexia com uma proposta de atuação conjunta em compreensão de leitura de textos com a Técnica de Cloze, tarefas de perguntas e respostas e ortografia / INTRODUCTION: Developmental dyslexia is a specific learning disorder, typified as an impairment showed by the individual while reading, even with intelligence, motivation and proper school instruction. It has neurobiological basis with genetic inheritance. Although the literature provides several studies about dyslexia which focus on the elaboration and use of evaluation instruments of the reading comprehension, phonological awareness, and others, there is almost no published studies of reading comprehension intervention programs for this population. Cloze\"s Technique, which consists on the organization of a short text with blanks, is been characterized as an effective method on the measurement and remediation of reading comprehension. OBJECTIVES: Elaborate a Speech Therapy Program of Reading Comprehension and Orthography and verify its effects on reading comprehension of dyslexic students. METHODS: There were part of this research eleven individuals diagnosed with developmental dyslexia, in ages between 09 and 11 years old, which, 8 of them where boys. The program was comprised of 16 sessions split into four levels of complexity. All of the sessions relied on an adapted text with Cloze\"s Technique, activities of reading comprehension and orthography. The subjects underwent pre-tests and post-tests, with the evaluation of reading comprehension and orthography. RESULTS: Findings showed that the subjects had better performance on reading comprehension at the post-test in comparison to the pre-test, in the percentage of right answers on Cloze\"s and in the comprehension questions as well as in the level of reading comprehension. Apart from that, generalizations of the gains in reading comprehension could be observed in another contexts. CONCLUSIONS: The Speech Therapy Program of Reading Comprehension and Orthography was a relevant instrument in remediation of reading comprehension on the subjects of this study, by increasing reading comprehension in texts with Cloze´s Technique as well in the comprehension questions, and by producing more motivated readers. Besides, this study concluded that Cloze´s Technique and comprehension questions were precise instruments for evaluating reading comprehension of dyslexic children. This study was innovative because of the elaboration of a specific remediation program for children with dyslexia and the verification of its effects allied with a purpose of joint work among reading comprehension of texts using Cloze\"s Technique, questions and answers tasks and orthography

Diplomová práce zjišťuje možnosti využití textových materiálů, zejména učebnic, pro výuku češtiny jako cizího jazyka pro práci terapeutů pacientů s Vycházíme ze zkušeností některých terapeutů, kteří pomocí materiálů pro výuku češtiny jako cizího jazyka kompenzují nedostatek afaziologických materiálů. Teoretická část práce přináší vybraná specifika češtiny jako cizího jazyka a terapie afázií a ukazuje shodné rysy jazykového projevu těchto dvou typů mluvčích. Součástí je také analýza projevu afatickéh připravovaného korpusu českých afatiků a analýza projevu nerodilého mluvčího češtiny. Oba rozbory se opírají o analýzy afatických projevů, které zpracovali lingvističtí odborníci. Praktickou část tvoří kvalitativní výzkum formou polostrukturov českými klinickými logopedy, jehož hlavním cílem bylo zjistit, zda tito terapeuti učebnice češtiny pro cizince používají, zda jsou podle jejich názoru pro terapii afázií vhodné a co by se na takových materiálech mohlo případně upravit, ab afázií užitečné. Zjistili jsme, že užívání učebnic češtiny pro cizince není mezi dotazovanými klinickými logopedy rozšířené. Po určitých úpravách si ale dokáží představit, že by je ve své praxi využili. Ti, kteří s takovými učebnicemi zkušenost v využívají především pro pacienty v chronické fázi nebo pacienty, kteří trpí pouze

Esta investigación doctoral aborda la robótica asistencial (Assistive Robotics) como tema general, también llamada robótica social; específicamente el estudio de la interacción de la relación humano-robot. Con base en el estudio del estado del arte realizado, esta tesis se orientó hacia el siguiente problema de investigación: ¿es posible mejorar la efectividad terapéutica y de esta forma la calidad de vida de las personas con dificultades para comunicarse verbalmente debido a singularidades relacionadas con el habla y lenguaje; por medio del desarrollo de estrategias socio-terapéuticas que utilicen robots y mundos virtuales con frecuencias de exposición flexibles en comparación con la exposición programada utilizando únicamente robots en el mundo físico? El problema de investigación brinda una direccionalidad innovadora desde varias perspectivas científicas. Por un lado, la integración terapéutica de recursos en el mundo virtual así como en el mundo físico con robótica asistencial en colaboración con el ser humano para cumplir un objetivo de crecimiento personal. Además, la posibilidad de romper la barrera del tiempo controlado de exposición terapéutica por medio de la tecnología. Y, por otro lado, metodológicamente buscar un abordaje científico que demuestre causalidad y no sólo asociación por medios cualitativos; dado que se quiere saber si la efectividad terapéutica realmente puede incrementar como variable de respuesta. Por lo tanto, y como elemento de innovación adicional, esta investigación abordó el diseño de un prototipo tecnológico de programación robótica con emulación animada, que integra el uso de un avatar robótico virtual para facilitar la interacción social de personas que presentan dificultades de comunicación verbal relacionadas con el habla y lenguaje. El objetivo general de esta tesis se plantea como sigue: “Aportar al conocimiento científico sobre la interacción humano-robot con fines terapéuticos de comunicación verbal en el idioma castellano comparando el uso de robots en el mundo físico y virtual con flexibilidad de tiempos versus la interacción limitada a robots físicos por periodos de tiempo fijos para saber si es posible incrementar de forma relevante la efectividad terapéutica en términos de mejora de habilidades y tiempo invertido en terapia.” Esta investigación contribuye científicamente con la propuesta de un enfoque metodológico que busca obtener resultados basados en la evidencia experimental y no sólo en el análisis hermenéutico o el análisis léxico de datos cualitativos que constituye lo más frecuente en la investigación científica en este campo. En este sentido, el método exploratorio basado en datos cualitativos y abordajes epistemológicos subjetivistas pueden verse bien complementados con investigación positivista más orientada a la evidencia basada en resultados vinculados a la causalidad. Otra aportación de esta investigación está en el desarrollo tecnológico orientado hacia el uso de la experiencia de realidad virtual de personas con condiciones de limitación en habla o lenguaje en combinación con un robot físico. Esta es una forma innovadora de buscar la exposición continua y en tiempo real a los protocolos de terapia de habla sin supervisión física del terapeuta, teniendo en cuenta que las aplicaciones robóticas en mundos virtuales vinculados con el "mundo físico" no son frecuentes.

Developmental screening : predictors of follow-up adherence in primary health care

Background: Early identification and intervention for infants and young children with developmental delays may negate or minimise the negative effect of a disability on a child's development. Poor follow-up on early detection referrals, however, undermines the effectiveness of early intervention programmes. Objectives: To identify factors, including text message reminders, that influence follow-up adherence for early intervention after developmental screening in primary health care. A secondary objective surveyed reasons for follow-up default. Methods: The PEDS Tools was used to screen 247 high-risk children. A risk assessment questionnaire was completed with caregivers whose children were referred for speech-language and/or occupational therapy (n=106, 43%). A quasi-experimental study was employed to identify risk factors for defaulting on appointments. A thematic analysis of telephonic interviews was also employed to determine reasons for follow-up defaults. Results: Follow-up adherence was 17%. Participants who were divorced, widowed or never married were 2.88 times more likely to attend a follow-up appointment than those who were married or living together (95%, CI 0.97- 8.63). Text message reminders did not significantly improve follow-up. More than half (58%) of participants who defaulted on appointments could be reached for telephonic interviews. During the telephonic interviews 87% of participants did not report concern about their child's development. Reasons for defaulting were employment, logistical issues, other responsibilities and forgetfulness. Conclusions: Detecting possible developmental delays did not lead to acceptable follow-up adherence for early intervention services at primary health care levels.
