Patientens upplevelse av att få information från sjuksköterskor angående egenvård vid venösa bensår

Syfte med denna litteraturstudie var att beskriva och sammanställa patientens upplevelse av att få information från sjuksköterskor angående egenvård vid venösa bensår . Artiklarna söktes på databaserna Medline och Cinahl. Sökorden som användes var leg ulcer, information, experience, self-care. Resultatet inkluderade elva artiklar där åtta hade kvalitativ ansats, två hade kvantitativ ansats och en hade både kvalitativ och kvantitativ ansats. Patienternas upplevelser av att få information varierade. En del var positiva till informationen på grund av sitt förtroende för sjuksköterskan. Relationen spelade stor roll i deras bedömning av informationen. En del patienter upplevde förvirring av att få information på grund av motstridig information från olika sjuksköterskor. Upplevelsen påverkades inte av hälso- och sjukvårdens system och patienterna beskrev lika hög tillfredställelse av informationen vid vård i hemmet som på avdelning. När det gällde kunskapssökning ville patienter ha mer kunskap och de tyckte den nuvarande informationen var otydlig och oklar. Det tydde på att patienternas upplevelser av information från sjuksköterskor angående egenvård påverkade deras inställning till egenvård. Det är därför viktigt att sjuksköterskorna ska veta hur de kan förmedla informationen till patienterna och informationens betydelse i samband med egenvård. / The Aim of this study was to describe and summarize the patient's experience in receiving information from nurses about self-care for venous leg ulcers.Articles were searched on Medline and Cinahl with the keywords leg ulcer, information, experience and self-care. The results included eleven articles, eight with a qualitative approach, two with a quantitative approach, and one with both qualitative and quantitative approach. Patients' experiences in receiving information varied. Some were positive about the information because of the confidence to the nurse. Relationships played a large role in their assessment of the information. Some patients experienced confusion of obtaining information because of conflicting information from different nurses. The experience was not affected by the health care system and the patients described as high satisfaction of the informationin home as the ward. With regard to search of knowledge the patients wanted to have more knowledge and they thought the current information was vague and unclear.It is suggested that patients' perceptions of information and in receiving information from nurses about self-care affected their  self-care behavior.  Therefore it is important that nurses know how to pass on information to patients and the importance of information in self-care.

Bakgrund: Venösa bensår är ett växande problem som orsakar smärta, begränsad rörlighet och minskad livskvalitet. Genom att främja egenvård till patienter med venösa bensår kan sårläkning förbättras. Egenvård är en hälso- och sjukvårdsåtgärd som legitimerad yrkesutövare har bedömt att personen kan utöva själv eller med hjälp av någon annan. Sjuksköterskors nyckelroll är att främja och optimera följsamhet av egenvård. Patientdelaktighet leder till att målen med vård och behandling uppfylls bättre. Syfte: Syftet med denna studie var att belysa distriktssköterskors erfarenheter av att främja egenvård hos patienter med venösa bensår. Metod: Data analyserades genom kvalitativ innehållsanalys med en induktiv ansats. Tio intervjuer med distriktssköterskor genomfördes. Resultat: Ur analysen av datamaterialet framträdde fyra kategorier med elva underkategorier; Involvera patienten i behandlingen, Skapa samsyn till förändring, Samarbeta kring patienten och Beakta patientens bristande medverkan. Konklusion: Studiens resultat visade att egenvårdsråd främjas till patienter med venösa bensår men att distriktssköterskorna saknade gemensamma riktlinjer. Genom att distriktssköterskorna tar sig tid och främjar egenvård kan patienten få ökad insikt till varför egenvård behövs för att främja sårläkning. / Background: Leg ulcers are a growing problem causing pain, limited mobility and reduced quality of life. Through promotion of self-care for patients with venous leg ulcers, wound healing can be improved. Self-care is a health measure that qualified professionals can decide a person can exert him or herself or with assistance from others. Nurses’ key role is to foster and optimize adherence to self-care. Patient participation leads to better fulfillment of the goals for the care and treatment. Purpose: The purpose of this study was to elucidate the district nurses’ experiences of promoting self-care in patients with venous leg ulcers. Method: Data were analyzed using qualitative content analysis and an inductive approach. Ten interviews with district nurses were conducted. Results: Analysis of the data produced four categories with eleven sub-categories; Involve the patient in treatment, Create consensus for change, Collaborate with the patient and Observe the patient's lack of participation. Conclusion: Our results showed that self-care is promoting to patients with venous leg ulcers, but that the district nurses lacked common guidelines. The district nurses can by taking the time to promote self-care advice give the patient a greater understanding why self-care is needed to promote wound healing.

Venous hypertension in the lower limb, occurring from a variety of causes, leads to microcirculatory abnormalities which may permit the formation of a chronic ulcer (Zimmet, 1998). Venous leg ulcers are a cause of much debility, social isolation, depression, fears of loss of employment and income (Royal College of Nursing Institute, 1998). The mainstay of treatment at present is compression bandaging to reduce the venous pressure in the lower limb. Eighty percent of venous leg ulcers will heal within eight to twelve weeks with compression bandaging (Thomas, 1997). Homoeopathy is a system of medicine based on natural laws of healing, and recognizes the biophysical energy of an individual (also known as the vital force) as that which provides the normal protective defence mechanisms of the body, and is disturbed in disease, producing symptoms on the physical, mental and emotional levels (Vithoulkas, 1980). Many plants are known to have wound-healing properties (Curtis and Fraser, 2003). Calendula officinalis and Hypericum perforatum were considered the best plants in a homoeopathic lotion to be used on chronic venous leg ulcers, as they cover all the common problems associated with this type of wound, providing pain relief and anti-microbial activity (Lawless, 1995). In this study, homoeopathic mother tinctures of the herbs were used to make the lotion for topical application to the wounds.

Cursam com alta taxa de recorrência, retardo na cicatrização e maior probabilidade de tornarem-se crônicas. Os mecanismos desencadeantes envolvem episódios hemolíticos e vaso-oclusivos, e mais recentemente, foram apontados polimorfismos em genes regulatórios da via do TGF-β como contribuintes deste processo. Polimorfismos nos genes que regulam esta via mostram-se alvos moleculares promissores na elucidação da fisiopatologia das UMs. Com isso, nosso objetivo foi investigar a relação de polimorfismos nos genes SMAD7 e SMURF1 na ocorrência de UMs em pacientes com AF. O estudo foi realizado em duas coortes independentes, composta por 331 pacientes de Pernambuco (131 com UM e 200 sem UM) e 197 pacientes do estado do Rio de Janeiro (108 com UM e 89 sem UM). As genotipagens dos polimorfismos SMAD7 C>T (rs736839) e SMURF1 C>G) (rs219825) foram realizadas por PCR em tempo real. Empregando o modelo de análise recessivo, foi encontrada associação entre os homozigotos variantes (TT; GG) e a maior ocorrência de UM nos pacientes das duas coortes avaliadas, para SMAD7 e SMURF1 em Pernambuco (P=0.001 e P=0.050) e Rio de Janeiro (P=0.029 e P=0.006). Ademais, avaliando a coorte de Pernambuco os pacientes com genótipo TT apresentaram uma maior taxa de desenvolvimento das UMs (70%) em relação aos com genótipos CC + CT (45%) (P<0,0001) para a SMAD7, e os pacientes com genótipo GG para SMURF1 apresentaram uma maior taxa de desenvolvimento das UMs (65%) em relação aos genótipos CC+CG (46%) (P=0.009). Sumariamente, nossos resultados mostram que os polimorfismos estudados estão envolvidos na ocorrência das UMs nas coortes de pacientes com anemia falciforme, mostrando-se como potenciais moduladores fenotípicos na doença falciforme. / Leg ulcers are one of the most common clinical manifestations of sickle cell anemia (SCA). They present a high rate of reoccurrence, delayed wound healing and a higher probability of becoming chronical. The triggering mechanisms involve hemolytic and vaso-occlusive episodes, and polymorphisms in regulatory genes of the TGF-β pathway were identified recently as contributors of this process. Polymorphisms in genes that regulate this pathway are considered as promising molecular targets in the elucidation of the pathophysiology of leg ulcers. Therefore, our aim was to investigate the relationship of the polymorphisms in SMAD7 and SMURF1 genes with the occurrence of leg ulcers in patients with SCA. The study was conducted in two independent cohorts consisting of 331 patients from Pernambuco (131 with leg ulcer and 200 without leg ulcer) and 197 patients from Rio de Janeiro (108 with leg ulcer and 89 without leg ulcer). The genotyping of SMAD7 C>T (rs736839) and SMURF1 C> G (rs219825) were performed by real time PCR. Using the recessive model of analysis, we found an association between homozygous variants (CC; GG) and the higher incidence of leg ulcers in patients from the two evaluated cohorts for SMAD7 and SMURF1 in Pernambuco (P = 0.001 and P = 0.050) and Rio de Janeiro (P = 0.029 and P = 0.006). Moreover, assessing the cohort of Pernambuco, patients with CC genotype had a higher rate of leg ulcers development (70%), compared to those with CC + CT genotypes (45%) (P <0.0001) for SMAD7, and patients with GG genotype for SMURF1 showed a higher rate of development of leg ulcers (65%) compared to CC + CG genotype (46%) (P = 0.009). In summary, our results demonstrated that the studied polymorphisms are involved in the occurrence of leg ulcers in cohorts of patients with sickle cell anemia, showing up as potential phenotypic modulators of sickle cell disease.

Bakgrund: I Sverige lever närmare en procent av befolkningen med venösa bensår. Såren uppstår på grund av venös insufficiens. Venösa bensår är ett stort folkhälsoproblem i hela världen. Personer som har ett stillasittande levnadssätt och är överviktiga har en högre risk än genomsnittsbefolkningen att utveckla venösa bensår. Såren är svårläkta och drabbar främst äldre personer. Sjuksköterskan har det yttersta ansvaret för patientens omvårdnad. Syfte: Syftet var att belysa personers upplevelser av att leva med venösa bensår. Metod: En litteraturöversikt har använts som metod. Ur databaserna PubMed och CINAHL Complete hämtades tio stycken vetenskapliga artiklar som grundar sig på forskning enligt kvalitativ metod. Resultat: Tre huvudteman och tre underteman identifierades. Det första huvudtemat var Smärta och dess hantering. Det andra huvudtemat var Påfrestningar till följd av odör och exsudat. Det tredje huvudtemat var Ett förändrat liv och har följande underteman; Beroende, Behandling och läkning samt Kunskapsbrist. Diskussion: I metoddiskussionen diskuterades litteraturöversiktens styrkor och svagheter. Resultatfynd associeras till vårdkontext och diskuteras i relation till bakgrund, ny forskning samt Katie Erikssons teori där lidande och vårdprocessen fokuseras. / Background: Nearly one percent of the Swedish population lives with venous leg ulcers. Venous leg ulceration is caused by venous insufficiency. This is a big public health problem worldwide. People who live their lives sedentary and are overweight suffers a greater risk of evolving venous leg ulcers. These types of ulcers are slow-healing and mostly affects elderly people.  The responsibility for patient care is the nurse. Aim: The aim of the study was to enlighten people’s experiences of living with venous leg ulcers. Method: A literature review was used as method. PubMed and CINAHL Complete were used as databases. Ten scientific qualitative articles were found. Results: Three main topics and three subtopics were identified. The first main topic was Pain and its management. The second main topic was Strains following odour and exudate. The third main topic was A different life with following subtopics; Dependence, Treatment and healing and Knowledge deficit. Discussion: The strengths and weaknesses of the literature review is discussed in the method discussion. The results are discussed in nursing context and is related to background, new research and Katie Eriksson’s theory with suffering and care process.

Teste alérgico de contato nos portadores de úlceras crônicas de membros inferiores

O tratamento local é feito através do uso de curativos específicos e outros produtos tópicos. A utilização destes podem causar dermatite de contato alérgica, aumentando a morbidade relacionada à doença e retardo da cicatrização. Objetivos: Identificar a frequência e principais substâncias alergênicas relacionadas à dermatite de contato nos pacientes portadores de úlceras crônicas nos membros inferiores. Métodos: Estudo observacional, com desenho transversal, realizado em 64 pacientes com diagnóstico de úlcera crônica de membros inferiores ativa ou cicatrizada independente da etiologia. Em todos os participantes foi realizado o teste de contato com a bateria padrão e com os curativos e produtos frequentemente utilizados para tratamento destas úlceras. Foram avaliadas variáveis clínicas e as relacionadas ao teste de contato. Resultados: Eczema peri-úlcera estava presente em 50 pacientes (78,1%). O teste de contato padrão foi positivo em 28 participantes (43,8%), sendo o paraben mix, sulfato de níquel e lanolina as substâncias do teste padrão que mais positivaram. O teste de contato com os curativos e produtos foi positivo em 17 participantes (25,6%) e os principais alérgenos foram, em ordem decrescente: colagenase com cloranfenicol (16%), sulfadiazina de prata (13%) e bota de Unna (6%). Eczema peri-úlcera relacionou-se positivamente com o uso de colagenase e hidratantes. Conclusão: A positividade ao teste... (Resumo completo, clicar acesso eletrônico abaixo) / Abstract: Introduction: Chronic leg ulcers are very prevalent and the local treatment is done through the use of specific dressings and other topical products. The use of these can cause allergic contact dermatitis, increasing disease-related morbidity and delayed healing. Objective: To identify the frequency and the main allergenic substances related to contact dermatitis in patients with chronic leg ulcers. Methodology: Observational study, with transversal design, performed with 64 patients diagnosed with active or healed chronic leg ulcers regardless of etiology were included. Patch tests were performed with all the patients, including the standard battery and the dressings and products frequently used on the treatment. Clinical variables and those related to the patch test were evaluated. Results: Peri-wound eczema was present in 50 patients (78.1%). The standard patch test was positive in 28 (43.8%) participants, with paraben mix, nickel sulfate and lanolin being the most positive substances. The patch test with dressings and products was positive in 17 (25.6%) participants and the main allergens were, in decreasing order: collagenase with chloramphenicol (16%), silver sulfadiazine (13%) and Unna's boot (6%). Peri-wound eczema was positively related to the use of collagenase and moisturizers. Bakgrund: Venös insufficiens är en kronisk sjukdom som kan orsaka venösa bensår. Livssituationen för patienter med venösa bensår förändras när de tvingas förhålla sig till symtom från såret såsom smärta, klåda, sårvätska och odör, samt till en långvarig läkeprocess med besvärande kompressionsbehandling. Av kliniska riktlinjer framgår att sjuksköterskan skall omhänderta patienten ur ett helhetsperspektiv, men sjuksköterskor har i tidigare studier uppgivit att de saknar nödvändiga kunskaper för att möta personen bakom såret. Syfte: Att beskriva patienters upplevelser av att leva med venösa bensår. Metod: Studien utformades som en kvalitativ litteraturstudie. Resultat: Patienter med venösa bensår kan uppleva skam för rinnande sårvätska, skrymmande bandage och odör. Sårrelaterad smärta leder till fysiska begränsningar, som i kombination med skam, oro och osäkerhet ger patienterna en begränsad tillvaro. Den långa och osäkra läkeprocessen leder till känslor av hopplöshet, sorg och maktlöshet. Slutsats: Begränsningarna som patienterna upplever kan leda till social isolering och nedsatt fysisk aktivitet. Maktlöshet, sorg och hopplöshet kan leda till en sämre hälsa. Ovanstående konsekvenser av den förändrade livssituationen kan i sin tur påverka sårläkningen negativt. Sjuksköterskan kan ge stöd till patienten genom att använda humor, skapa mötesplatser och tillämpa empowermentstrategier. Klinisk betydelse: Patienter kan uppleva ett bättre bemötande när sjuksköterskan har en god förståelse för de besvär som såren orsakar. Förståelsen kan även vara till hjälp när sjuksköterskor planerar stödåtgärder till patientgruppen. / Background: Venous insufficiency is a chronic disease that can cause venous leg ulcers. Living conditions of patients with venous leg ulcers are changed when they need to adapt to wound symptoms such as pain, itching, exudate and odour, as well as a prolonged healing process with disturbing compression therapy. Clinical guidelines show that nurses should have a holistic approach to patient care, but in previous studies nurses have indicated lack of necessary skills to meet the person behind the wound. Purpose: To describe patients' experiences of living with venous leg ulcers. Method: The study was designed as a qualitative literature study. Results: Patients with venous leg ulcers may experience shame for running exudate, bulky bandages and odour. Wound pain leads to physical limitations, which in combination with shame, anxiety and uncertainty gives patients a restricted life. The long and uncertain healing process results in feelings of hopelessness, sadness and powerlessness. Conclusion: Limitations that patients experience may lead to social isolation and reduced physical activity. Powerlessness, sadness and hopelessness may lead to poor health. The above consequences of the changed life conditions may in turn influence wound healing. Nurses can provide patients support by using humour, creating meeting places and applying empowerment strategies. Clinical significance: Patients may experience better treatment when nurses show a good understanding of the problems that wounds cause. Understanding can also be helpful when nurses plan measures to support the patient group.

The aetiology and pathogenesis of tropical ulcer

Tropical ulcer is a very specific form of cutaneous ulceration. It occurs worldwide in most tropical and a number of subtropical areas. The disease occurs mainly in older children and young adults with children under the age of 5 and adults over 45 years rarely being affected. Ulcers occur most commonly on the lower leg but may occur on the upper limb. Although most ulcers normally heal slowly over many weeks or months, some ulcers may recur. Recognised complications include squamous cell carcinoma, gangrene and osteitis, although these are rare. A number of authors have reported on the disease and suggested diet, trauma and infection as aetiological factors for this condition. This survey was thus conducted to assess as many of these factors as possible. The study took place in 5 tropical areas, namely Zambia, Gambia, southern India, Fiji and Papua New Guinea. Consultations took place at hospitals, rural clinics, health centres and villages. Although many authors have suggested that the disease is related to malnutrition, few have objectively assessed the nutritional status of the patients and compared it with controls. Those studies which included objective assessments were limited to small areas and only investigated specific parameters. In order to investigate the immune response of the host to an anaerobic infection, the antibody levels to the organisms isolated from the ulcers were measured by an ELISA test. The local host response to an infection with a Fusobacterium species was assessed by the number of antibody secreting B-lymphocytes at the site of the ulcers. These parameters may play a role in the localisation of the ulcers and account for recurrent infections.
