Burnout among young physicians and its association with physicians’ wishes to leave: results of a survey in Saxony, Germany

Background: Concerns about burnout, and its consequences, among German physicians are rising. However, data on burnout among German physicians are scarce. Also, a suspected association between burnout and German physicians’ wishes to leave remains to be studied. Therefore, the extent of burnout, and the association between burnout and wishes to leave clinical practice or to go abroad for clinical work was studied in a sample of young physicians in Saxony. Methods: In a cross-sectional survey, all physicians ≤40 years and registered with the State Chamber of Physicians of Saxony, Germany (n = 5956) received a paper-pencil questionnaire inquiring about socio-demographics, job satisfaction, and wishes to leave clinical practice or to go abroad for clinical work. Response rate was 40 % (n = 2357). Burnout was measured with the German version of the Maslach Burnout Inventory - Human Services Survey (MBI) consisting of the subscales emotional exhaustion (feeling emotionally drained), depersonalization (feelings of cynicsm) and personal accomplishment (feelings of personal achievement in job). Variables associated with burnout, and the association between burnout and wishes to leave were assessed in multivariate logistic regression analyses. Results: For emotional exhaustion participants reached a mean of 21.3 [standard deviation = 9.74], for depersonalization a mean of 9.9 [5.92], and for personal accomplishment a mean of 36.3 [6.77]. Men exhibited significantly higher depersonalization than women (11.3 [6.11] versus 9 [5.62], p < 0.001). Eleven percent of participants showed a high degree of burnout on all subscales, while 35 % did not show a high degree of burnout on any subscale. Confirming that one would become a physician again, and higher satisfaction with the components \"work environment\" and \"humaneness\", were associated with a lower chance for a high degree of burnout on all subscales. Higher emotional exhaustion and lower personal accomplishment were associated with an increased chance of wishing to leave clinical practice. Higher emotional exhaustion and higher depersonalization were associated with an increased chance of wishing to go abroad for clinical work. Conclusions: Preventing physician burnout may not only benefit the affected individual. It may also benefit the health care system by potentially preventing physicians from leaving clinical practice or from going abroad for clinical work.

Metod: Denna studie är utformad ur en kvantitativ metod med ett deduktivt synsätt. Datainsamlingen utgick från en webbaserad enkätundersökning som skickades ut till 3126 verksamma fastighetsmäklare i Europa. Analysen av datan utfördes i statistikprogrammet “JASP”, där primärt bivariata korrelationsanalyser utfördes för att finna signifikanta samband mellan de olika variablerna. Denna information kunde sedan användas för att redovisa och tolka resultatet. Resultat &amp; slutsats: Utifrån den analyserade datan visade undersökningen att fastighetsmäklarnas personlighetsegenskaper påverkade individens nivå av utbrändhet. Det anträffade positiva signifikanta samband mellan egenskapen neuroticism och utbrändhet. Personlighetsegenskaperna extraversion och conscientiousness visade däremot negativa samband med utbrändhetssyndromen. Undersökningen visade dessutom att det inte endast var personlighetsegenskaperna som påverkade välmående. Trivseln på arbetsplatsen hade även det en stor betydelse för den enskilde individens välmående. Examensarbetets bidrag: Denna studie bidrar teoretiskt med en klarare uppfattning kring utbrändhetsdimensionerna utifrån personlighetsegenskaperna inom Big five. Likaså sambandet till välmående, arbetstillfredsställelse och individens tanke om att lämna arbetsplatsen. Detta praktiska bidrag kan hjälpa individen att förstå dess sårbarhet och utmaningar för att i praktiken kunna förebygga utbrändhetssyndrom. Det hjälper dessutom chefer och ledare som bedriver fastighetsmäklarkontor i Europa att få en inblick i vilka anställda som föreligger större risk att utsättas för utbrändhetssyndrom samt är i behov av extra stöttning. Cheferna och ledarna kan därmed anpassa arbetsmiljön och förhållandena efter individens behov för att bidra till ett bättre välmående på arbetsplatsen och minska personalomsättningen. Förslag till fortsatt forskning: Denna studies population omfattar fastighetsmäklare i Europa. Result &amp; Conclusion: Based on the analyzed data, the study showed that the real estate agents personality traits affected how exposed the individual was to burnout. There were strongly positive significant associations between the trait neuroticism and burnout, while the personality traits extraversion and conscientiousness showed negative associations with burnout syndrome. The study also showed that it was not only the personality traits that affected the well-being of the individual. The comfort of the workplace, however, also has agreat importance for well-being. Contribution of the thesis: This study theoretically contributes with a clearer understanding of the dimensions of burnout based on the personality traits through Big five. Likewise, the connection to well-being, job satisfaction and the individual's thoughts of leaving the workplace. This practical contribution can help the individual to understand its vulnerability and challenges in order to be able to prevent burnout syndrome in practice. It can also help leaders and managers who run real estate agencies in Europe, to gain an insight into which employees are at greater risk of being exposed to burnout syndrome and the employees that are in need of extra support. The leaders and managers can thus adapt the work environmentand conditions to the individual's needs in order to contribute to a better workplace and reduce turnover intention. Suggestions for further research: The population of this study includes real estate agents in Europe. An interesting contribution to science would have been to examine other specific demographic areas and then compare these against each other in order to see if the working methods in the different areas would be better or worse from a burnout perspective.

Gender and Self-Care Behaviors in the Burnout of Mental Health Professionals

Difficulties experienced at work can cause feelings of burnout that become prolonged and intensified without acts of self-care. The intense nature of mental health workers' jobs may make them, more vulnerable to burnout than other professionals. Because mental health professionals' mental and emotional wellness can significantly affect their work, adequate self-care is critical to both their well-being and that of their clients. Previous researchers have investigated the self-care behaviors of mental health professionals, but little was known about how gender affected the use of these behaviors in burnout prevention among mental health professionals. The purpose of this quantitative study was to examine the relationship between self-care behaviors and burnout among a sample of 325 mental health professionals working in New York. Differences in the ways male and female mental health professionals practiced self-care behaviors were also investigated. Burnout and gender role theories were used as the theoretical framework. Study instruments included the MBI-HSS and the Brief COPE. Multiple regression analysis and independent sample t tests were employed to analyze survey data. Analysis revealed levels of self-care behaviors were significantly predictive of depersonalization, emotional exhaustion, and reduced personal accomplishment. Gender differences in self-care behaviors were indicated for substance use, self-blame, depersonalization, emotional exhaustion, and reduced personal accomplishment. Since burnout is a significant problem for many mental health professionals, understanding how self-care affects burnout is critical to promoting behavioral changes among these professionals. Self-care among mental health professionals may improve their professional and personal lives.

Psicólogo / En este trabajo se investiga la validez factorial del MBI (Maslach Burnout Inventory) que intenta medir el síndrome de Burnout. El colectivo lo conformaron 98 profesores dirigentes sindicales del Colegio de Profesores A.G. Se realizó un análisis de Componentes Principales y rotación ortogonal de tipo Varimax, el cual presentó 6 factores. En el primer componente se agrupan los ítems de Agotamiento Emocional, en el segundo los sugeridos para Despersonalización. No obstante, los descritos para Realización Personal se fragmentaron en 4 factores. La distribución de los ítems es congruente en forma parcial a los del manual y se propone una solución factorial de dos dimensiones, esto es Agotamiento Emocional y Despersonalización. Así mismo, se sugiere revisar el ítem 21, dado que en este estudio se encuentra evidencia para su eliminación. En conclusión, este trabajo plantea que la solución bifactorial permitiría evaluar el síndrome de BO con un buen nivel de validez y confiabilidad

Special Education Teacher Burnout: A Factor Analysis

The poor retention of special education teachers negatively impacts student academic outcomes. Special education teachers commonly cite burnout as a primary reason for leaving the field; however, there is a deficit of literature available to validate claims concerning special educators and their level of burnout. This study analyzed the psychometric properties of the Maslach Burnout Inventory: Educators' Survey using a sample of 349 special education teachers from schools across the nation (201 resource room special education teachers and 148 self-contained special education teachers). The Maslach Burnout Inventory: Educators' Survey measures three factors (e.g. subscales) emotional exhaustion, depersonalization, and personal accomplishment. A confirmatory factor analysis, an exploratory factor analysis, and a multi-group measurement invariance confirmatory factor analysis were conducted. The results showed measurement invariance between the two groups of teachers. During the exploratory factor analysis, a significant fourth factor, collaborative stress, emerged. These findings suggest the current factor structure of the Maslach Burnout Inventory: Educators' Inventory needs to be modified when measuring burnout among special education teachers. This includes the need to further explore how collaboration stress relates to special education teachers and how to implement formative collaboration practices to retain special education teachers.

Syftet med denna studie är därför att undersöka hur nivåerna av utbrändhet och känslan av sammanhang (KASAM) påverkar arbetstillfredsställelse bland fullständigt registrerade och yrkesmässigt aktiva fastighetsmäklare i Sverige. Metod: Studien utgår från en kvantitativ forskningsmetod där en tvärsnittsstudie genomförts med hjälp av en enkätundersökning som genererade totalt 404 svar. Studiens population består av fullständigt registrerade och yrkesmässigt aktiva fastighetsmäklare i Sverige. Den data som kom av enkätundersökningen analyserades i statistikprogrammet JASP och presenterades därefter i olika figurer och tabeller som återfinns i resultatet samt bilagor. Resultat och slutsats: Av studien framkom att respondenterna generellt upplever låga nivåer av utbrändhet samt höga nivåer av KASAM och arbetstillfredsställelse. Resultatet tydliggör ett samband innebärande att låga nivåer av utbrändhet korrelerar med högre nivåer av KASAM samt arbetstillfredsställelse. Examensarbetets bidrag: Av studien genererades ett generaliserbart resultat som bidrar med praktisk likväl som teoretisk kunskap om nivåerna av utbrändhet, KASAM och arbetstillfredsställelse hos populationen. Även kunskap om hur de tre teoretiska modellerna påverkar varandra bland de svenska fastighetsmäklarna presenteras. Förslag till fortsatt forskning: Utifrån studiens resultat och begränsningar ges ett antal förslag till vidare forskning relaterade till de teoretiska modellerna som appliceras i denna studie. Bland annat föreslås en longitudinell studie som undersöker utbrändhet, KASAM och arbetstillfredsställelse. Ytterligare förslag är att undersöka hur värdena skiljer sig åt geografiskt. Development of an awareness-based intervention to enhance quality of life in severe dementia: trial platform

Yes / Quality of residential care for people with severe dementia is in urgent need of improvement. One reason for this may be the assumption that people with severe dementia are unaware of what is happening to them. However, there is converging evidence to suggest that global assumptions of unawareness are inappropriate. This trial platform study aims to assist care staff in perceiving and responding to subtle signs of awareness and thus enhance their practice. Methods/Design: In Stage One, a measure of awareness in severe dementia will be developed. Two focus groups and an expert panel will contribute to item and scale development. In Stage Two observational data will be used to further develop the measure. Working in four care homes, we will recruit 40 individuals with severe dementia who have no, or very limited, verbal communication. Data on inter-rater reliability and frequency of all items and exploratory factor analysis will be used to identify items to be retained. Test-retest and inter-rater reliability for the new measure will be calculated. Correlations with scores for well-being and behaviour and with proxy ratings of quality of life will provide an indication of concurrent validity. In Stage Three the new measure will be used in a single blind cluster randomised trial. Eight care homes will participate, with 10 residents recruited in each giving a total sample of 80 people with severe dementia. Homes will be randomised to intervention or usual care conditions. In the intervention condition, staff will receive training in using the new measure and will undertake observations of designated residents. For residents with dementia, outcomes will be assessed in terms of change from baseline in scores for behaviour, well-being and quality of life. For care staff, outcomes will be assessed in terms of change from baseline in scores for attitudes, care practice, and well-being. Discussion: The results will inform the design of a larger-scale trial intended to provide definitive evidence about the benefits of increasing the sensitivity of care staff to signs of awareness in residents with severe dementia.
