The Need for Play and Creativity in Children’s Rehabilitation Process: a Field Study in the Philippines

De förhållanden som många filippinska barn lever under stämmer inte överens med de rättigheter de har under barnkonventionen. Extrem fattigdom och utbredd korruption gör att människor lever under svåra livsförhållanden och de värst utsatta är barnen. Den filippinska staten varken respekterar, skyddar eller uppfyller dess skyldigheter under internationell lag. Därför måste andra aktörer, som olika NGOs, ta på sig ansvaret för att barnens behov ska bli uppfyllda. Många barn från försummande och utnyttjande situationer är i behov av rehabilitering.Denna uppsats är baserad på en fältstudie med fokus på det arbete som två NGOs på Filippinerna utför för att ge barn tillbaka deras förlorade barndom. Syftet är att väcka uppmärksamhet kring de behov som barn har för deras utveckling och välbefinnande, speciellt behovet av lek och kreativitet, två behov som tenderar att komma i skymundan i arbetet för barnets bästa. / The situation of children in the Philippines is not in accordance with the rights stipulated under the CRC. The extreme poverty and widespread corruption in the country creates an abusive and neglectful environment for people, with children being the most severely affected. The Philippine state does not appear to respect, protect and fulfil its obligations under international law. Other agents, such as NGOs therefore assume responsibility to fulfil the needs of children. Many children are maltreated and in need of rehabilitation.This thesis is based on a field study focusing on the work performed by two NGOs in the Philippines that help children renew their lost childhood. The aim is to draw attention to the needs the child has in their development and wellbeing, especially the need for play and creativity. Two needs that tend to get overlooked in the assessment of the best interest of the child.

A Developmental Cascade Model of Maltreatment, Delay Discounting, and Health Behaviors across Adolescence and Young Adulthood

Maltreatment is a pervasive global problem known to have cascading consequences that persist long after exposure subsides (Masten and Cicchetti, 2010). Maltreatment is often co-occurring, involving exposure to multiple types. Cumulative maltreatment, or exposure to multiple types of neglect and abuse, is proposed to be of critical importance for developmental psychopathology. However, a cumulative approach to studying maltreatment provides little insight into the developmental processes whereby it exerts its effects on health. Thus, we employed both a cumulative approach and a multidimensional approach to facilitate our comprehensive understanding of maltreatment experiences related to behavioral development. Given the high prevalence of maltreatment, it is important to cultivate a greater understanding of the processes linking maltreatment and health and to identify developmental periods of vulnerability to its deleterious effects. The present study uses a longitudinal design and a multidimensional approach to examine the effects of maltreatment on delay discounting and health-promoting and health-demoting behaviors during adolescence and across the transition from adolescence to young adulthood. The study sample includes 167 adolescents (aged 13–14 at Time 1; 53% male) who participated across 5 time points over 6 years. At Time 5, adolescents provided retrospective reports of their exposure to maltreatment during adolescence across ages 13–18. Delay discounting, substance use, and body mass index (BMI) were assessed at each time point. We used a developmental cascade model with autoregressive, cross-lagged, and cross-sectional associations to examine the longitudinal multivariate change processes and indirect effects from maltreatment exposure during adolescence to delay discounting and health-promoting and health-demoting behaviors during adolescence and across the transition to young adulthood. Our results indicate that cumulative maltreatment affects health-demoting behavior via its effects on delay discounting and that maltreatment of omission but not commission drives this effect. Furthermore, the findings identify adolescents exposed to maltreatment of omission as being especially vulnerable to marijuana use via elevated delay discounting. Identifying mediating processes linking maltreatment exposure to health-promoting and health-demoting behaviors may be instrumental for preventing deleterious developmental cascades and interrupting related health problems during adolescence and across the transition from adolescence to young adulthood. / Doctor of Philosophy / Maltreatment is a pervasive global problem that casts a long shadow, often involving exposure to multiple types of abuse or neglect. Syftet med examensarbetet var att undersöka hur enhetschefer inom äldreomsorgen uppfattar möjligheterna för att uppmärksamma och förebygga vanvård och försummelse hos äldre personer som mottar hemtjänstinsatser i ordinärt boende. Metoden involverar en kvalitativ design med semistrukturerade intervjuer utifrån ett bekvämlighetsurval av enhetschefer inom hemtjänsten. Datainsamlingen inkluderade öppna frågor om möjligheter och resurser för att upptäcka vanvård och försummelse. Reflexiv tematisk analys användes för att analysera datamaterialet. Metoden bidrar till en djupare förståelse av enhetschefernas perspektiv på att förebygga vanvård hos äldre personer. Resultatet visade att en ökad kontinuitet mellan personal och den äldre personen i vardagen ökar förutsättningen att upptäcka vanvård och försummelse, kontinuerlig och kompetenshöjande utbildning krävs för att ge personalen rätt förutsättningar i sin yrkesroll att motverka och förebygga vanvård och försummelse och att samverkan mellan olika aktörer är en förutsättning för att identifiera och motverka vanvård och försummelse mot äldre personer. / The purpose of the undergraduate thesis was to explore home care unit managers' possibilities of identifying and preventing elder abuse and neglect. The study employs a qualitative design with semi-structured interviews in a convenience sample of unit managers from different municipalities and with different experiences. Reflexive thematic analysis was used to analyze data contributing to a deeper understanding of unit managers' perspectives on preventing elder abuse. Results found that increased continuity between staff and the older person in everyday life increases the possibilities to detect elder abuse and neglect. Continuous and competence-enhancing training is required to provide staff with tools in their professional role to counteract and prevent elder abuse and neglect. Additionally, collaboration between different stakeholders is essential for identifying and combating abuse and neglect of older adults.

No / As part of the multidisciplinary safeguarding team, children’s nurses should have a clear understanding of child protection processes and how they have sometimes lacked a systematic approach. Methods of detecting threats to patient safety in high-risk clinical care can also be employed to analyse child protection processes. This article outlines one tool, known as failure mode and effects analysis, which can be used in a framework that takes into account human factors that can influence an individual’s performance. It discusses how this tool can be used to identify and reduce the potential for failure in the serious case review process, in particular by not attributing blame.

Il NM è una disturbo del movimento spontaneo caratterizzato da un sottoutilizzo dell’arto controlesionale in assenza di un deficit primario che migliora con il comando verbale. Per la sue caratteristiche è difficile da evocare in ambito clini-co e il suo riconoscimento si basa sull'osservazione dei sintomi. Le lesioni associate al NM sono diverse e i suoi mecca-nismi sono sconosciuti. La compromissione selettiva del movimento spontaneo suggerisce un coinvolgimento del si-stema motorio mediale. La ricerca ha lo scopo di studiare il NM con un nuovo metodo quantitativo, basato su actigra-fia, e di esplorare le basi neuroanatomiche. Due accelerometri erano posti su entrambi i polsi per 24 ore. 31 soggetti sani e 38 cerebrolesi, 6 con MN sono stati reclutati. In due casi abbiamo esplorato le lesioni. E’stato validato il nuovo indice di asimmetria AR24h. I MN mostravano un comportamento asimmetrico, simile agli emiplegici e diverso dai sani e dai pazienti non emiplegici. I pazienti mostravano una lesione del cingolo e putamen, parti del sistema motorio mediale, importante per le azioni volontarie. I risultati che l’actigrafia differenziale nel quantifica il movimento spontaneo e valuta il NM. Putamen e il cingolo causano una disfunzione del sistema motorio mediale e induce NM. / The MN is a movement disorder characterized by spontaneous underutilized contralesional limb in the absence of a primary deficit that improves with the verbal command. For its characteristics it is difficult to evoke in the clinical set-ting and its recognition is based on observation of symptoms. The lesions associated with NM are different and its mechanisms are unknown. The selective impairment of spontaneous movement suggests the involvement of the medi-al motor system. Research has the aim of studying the MN with a new quantitative method, based on the actigraphy, and to explore the neuroanatomical bases. Two accelerometers were placed on both wrists for 24 hours. Sog-31 jets and 38 brain-healthy, with 6 MN were recruited. In two cases we analysis lesions. It has been validated new asymmetry index AR24h. The MN showed an asymmetric behavior, similar to and different from the healthy and hemiplegic pa-tients not hemiplegic. Patients showed a lesion of the cingulate and putamen, parts of the medial motor system im-portant for voluntary actions. The differential actigraphy quantifies the spontaneous movement and evaluates the NM. Putamen and the track cause dysfunction of the motor system and causes medial NM.

The goal of this thesis is to describe the historical development of child care in the Czech Republic and in the world. It was never easy for children in the adult world. At the origination, the leader of the tribe decided about their lives. In the past murders of little girls were common, men could fight and protect the tribe or nation better. Boys were often killed by enemies so there was no descendant left. It was not until the acceptance of Christianity in 16th century that it had changed. It was still not a peaceful childhood, but there were no child murders. On the other hand, a lot of childen ended up in monasteries without parents. Nowadays at least children in the developed world should have a real childhood. Unfortunately there is still a considerable amount of adverse cases. Another goal was to familiarize the reader with CAN Syndrome. In 1962 the term "Beaten Child Syndrome" was used, later the name CAN Syndrome became common. This syndrome was first described in the USA, where they noticed children have injuries their parents cannot make clear. Categories included in CAN Syndrome were established by the Council of Europe in 1992. Estudamos neste trabalho o comportamento da impureza oxigênio em rede cristalina de silício, usando primeiramente o método INDO (lntermediate Neglect of Differential Overlap) com parametrização espectroscópica e incluímos os efeitos de muitos elétrons via Método CI-(Interacão de Configuração). Fazemos uma análise sobre as possíveis origens do posicionamento não-central (off-center) dessa impureza, - se decorrente de uma distorsão Pseudo Jahn-Teller: - se originária de um CR Reconstrução Química. Quando o átomo de Oxigênio é deslocado ao longo da direção [100] ocupa um sitio de mínimo absoluto. O oxigênio introduz no gap de banda um orbital anti-ligante de simetria a1 totalmente ocupado, situação semelhante à dos anti-sitios em materiais semi-condutores III-V. Estudamos também o comportamento dessa impureza quando a mesma é deslocada ao longo da direção [111], verificando que é criada uma posição de Instabilidade mas, os auto-valores têm comportamento não esperado para orbitais anti-ligantes. / In this work we studied the Oxygen impurity in Silicon. The calculations were performed using a semi-empirical method (INDO/S) with Configuration Interaction to account for many-electron effects. We analyse the origin of the off-center position for substitutional oxygen. Pseudo Jahn-Teller distortion or Chemical Rebonding effects. The oxygen introduces an a1 anti-bonding state in the forbidden band-gap similar to anti-site defects in III-V materials. To answer the question about distortion, we also studied oxygen dislocation in the (111) direction and we obtained a metastable state but his eigen-value has a non-expected behavior for anti-bonding orbitals.

Introdução: A negligência é um fenômeno de incidência crescente na enfermagem contemporânea, uma omissão frente ao cuidado bastante naturalizada no cotidiano profissional, mas que constitui infração ética grave, pois viola os preceitos éticos e a própria essência da profissão. Este estudo abordou a negligência cometida pelos profissionais de enfermagem no exercício da função nos processos éticos julgados pelo COREN-SP entre 2001 e 2010 e nas falas dos profissionais condenados nos processos éticos de negligência com criança. Contemplou-se a negligência nas dimensões conceitual, semântica, jurídica e ético-profissional, com o intuito de elucidar seus contornos, distingui-la das infrações éticas análogas imperícia e imprudência e dimensionar a responsabilidade do profissional condenado por conduta negligente. Objetivos: Identificar e classificar os casos de negligência profissional julgados pelo COREN-SP no período de 2001 a 2010; analisar as justificativas presentes nos depoimentos dos profissionais condenados nos processos éticos de negligência com criança e reconstituir o sentido do ato negligente nos depoimentos desses profissionais. Métodos: Delineou-se como percurso metodológico a pesquisa bibliográfica exploratória da literatura especializada, realizada em duas etapas, e a pesquisa documental dos processos éticos nas abordagens quantitativa e qualitativa. A primeira etapa da pesquisa bibliográfica concentrou-se em publicações nacionais que abordaram a negligência sem definição de recorte temporal. Localizou-se um número reduzido de 13 estudos e constatou-se a inexistência de um conceito de negligência no campo da enfermagem, pressuposto do estudo. Na segunda etapa, ampliou-se a revisão das publicações nacionais para 19 estudos e procedeu-se a revisão dos 11 estudos internacionais encontrados, realizada subsequentemente à leitura dos processos. Utilizou-se como referencial teórico-filosófico as concepções de ética, deliberação e responsabilidade de Aristóteles e de Hans Jonas. Resultados: O estudo quantitativo abrangeu 482 processos éticos e forneceu dados para a composição de um primeiro retrato da negligência na enfermagem. Identificou-se 68 processos de negligência e 20 processos de negligência com criança, dos quais selecionou-se sete para o estudo qualitativo, realizado mediante a análise de conteúdo dos depoimentos, na modalidade de análise temática. Tal abordagem propiciou uma definição mais específica de ato negligente e subsidiou a formulação de seis categorias analíticas. Observou-se, nos depoimentos, a mesma naturalização da negligência presenciada no cotidiano profissional, dado que os profissionais não se referiram à conduta negligente como negativa em suas falas mesmo quando esta resultou em consequências fatais, tampouco expressaram qualquer manifestação de pesar pelo efeito lesivo do ato praticado, buscando sempre atenuá-lo por meio de estratégias discursivas. Conclusão: Concluiu-se que o ato negligente comporta sempre um componente de deliberação e de responsabilidade pessoal, o que permitiu definir negligência como o descumprimento deliberado do dever de atender a necessidade do cuidado e confirmar que, na qualidade de conduta antiética, a negligência efetivamente afronta a responsabilidade profissional que decorre do compromisso ético inerente à profissão / Introduction: Neglect is a phenomenon of growing incidence in contemporary nursing, comprising an omission regarding care, which is quite naturalized in professional daily life, but which constitutes a serious ethical breach, as it violates ethical precepts and the very essence of the profession. This study addressed neglect committed by nursing professionals in the exercise of their function in the ethical proceedings judged by COREN-SP between 2001 and 2010 and in the statements of professionals convicted in ethical proceedings of child neglect. Neglect in the conceptual, semantic, juridical and ethical-professional dimensions was contemplated, with the aim of elucidating its contours, distinguish it from analogous ethical infractions, malice and imprudence, and dimension the responsibility of professionals convicted for negligent conduct. Purpose: To identify and classify the cases of professional neglect judged by COREN-SP in the period from 2001 to 2010; analyze the justifications present in the testimonies of convicted professionals in the ethical proceedings of child neglect; and reconstitute the meaning of the negligent act in the testimonies of these professionals. Methods: The exploratory bibliographic research of the specialized literature, carried out in two stages, as well as the documentary research of the ethical proceedings in the quantitative and qualitative approaches were delineated as a methodological course. The first stage of the bibliographical research focused on Brazilian publications that addressed neglect without a definition of timeframes. A small number of 13 studies were located, and there was no concept of neglect in the field of nursing, which was the studys assumption. In the second stage, the review of the Brazilian publications for 19 studies was expanded, and the 11 international studies were reviewed following the reading of the proceedings. The conceptions of ethics, deliberation and responsibility of Aristotle and Hans Jonas were used as theoretical-philosophical references. Results: The quantitative study covered 482 ethical proceedings and provided data for the composition of an initial image of nursing neglect. A total of 68 neglect cases and 20 child neglect cases were identified, of which seven were selected for the qualitative study, based on the analysis of the content of the testimonies, in the thematic analysis mode. This approach provided a more specific definition of a negligent act and subsidized the formulation of six analytical categories. The testimonies showed the same naturalization of the neglect observed in the professional daily life, as the professionals did not refer to the negligent conduct as negative in their statements, even when this resulted in fatal consequences, nor did they express any manifestation of regret by the injurious effect of the act, always seeking to alleviate it through discursive strategies. Conclusion: It was concluded that the negligent act always involves a component of deliberation and personal responsibility, which enabled a definition of neglect as the deliberate failure to comply with the duty to provide the care needed and to confirm that, as an unethical conduct, neglect is an effective violation of professional responsibility, arising from the ethical commitment inherent in the profession.

Poverty and child neglect : subtypes of neglect and stress as a mediator

This study examined the association between poverty and child neglect. The existence of a general association has been established for some time. However, there is much debate, and little detailed research, on the specific processes that create this association. This study focused on the form of neglect that involves the most health risk for children—physical neglect. It was hypothesized that poverty should increase the likelihood of a specific type of physical neglect, neglect of safety and basic needs, occurring more than other types. Using official child protective services data from a national data set three types of physical neglect were examined: abandonment, lack of safety or basic needs, and inadequate supervision. Hypothesis 1 was that poverty increases the odds of safety/basic needs neglect more than it influences the odds of either abandonment neglect or inadequate supervision neglect, controlling for prior neglect. Hypothesis 2 focused on a test of whether the link between poverty and physical neglect is not direct, but is instead mediated by caregiver stress. Three waves of longitudinal data were used for this test to establish causal time order between poverty and stress, and between stress and physical neglect. This study analyzed data from the Longitudinal Studies of Child Abuse and Neglect (LONGSCAN), Assessments 0-3 from the National Data Archive on Child Abuse and Neglect at Cornell University. The data was analyzed using multinominal logistic regression for both models. The results did not confirm Hypothesis 1, though the analysis was limited somewhat due to low frequencies of some physical neglect types in certain age groups. Hypothesis 2 was confirmed showing that the effect of poverty on physical neglect was completely mediated by caregiver stress for the abandonment and safety/basic needs types of physical neglect. Implications of the results for research on the effects of poverty on child neglect, and for preventing child neglect are discussed. / Graduation date: 2012
