Ska jag göra det eller inte? : En kvalitativ intervjustudie om ambulanssjuksköterskors upplevelser av att göra en orosanmälan av barn

Bakgrund:Under de senaste decennierna har medvetenheten om barnmisshandel samt utvecklingen av förebyggande politik kring detta ökat. Ambulanssjuksköterskor har en unik möjlighet att upptäcka dessa barn som misstänks fara illa då de är först på plats vid ett akut sjukdomstillstånd och kan se tecken i hemmiljön som kan leda till en orosanmälan. Det kan leda till ett minskat lidande för barnen men för att det ska kunna ske bör en medvetenhet om vad en orosanmälan innebär finnas. Syfte:Syftet med studien var att beskriva ambulanssjuksköterskors upplevelser av att göra en orosanmälan av barn som far illa.   Metod:En studie med tio stycken kvalitativa intervjuer gjordes. En öppen fråga med möjlighet till följdfrågor ställdes utifrån en intervjuguide. Materialet analyserades på en latent nivå utifrån Lundman och Hällgren Graneheims kvalitativa innehållsanalys.   Resultat:Resultatet redovisas utifrån tre huvudkategorier; Misstankar väcks, Påverkan på orosanmälansamt Samarbete och Öppenhet. Av dessa kategorier framkom sju underkategorier som beskrev informanternas upplevelser. Det framkom att brist på kunskap angående identifieringen av barn samt en okunskap kring vad ens egenansvar innebär, finns. Rädslan för repressalier kan ses som en anledning till att orosanmälan inte görs.   Slutsats:Utbildning i ämnet barn som far illa är av stor vikt för att kunna identifiera de utsatta barnen. En rädsla för repressalier kan vara anledningen till att orosanmälningar inte görs men skulle kunna öka om möjlighet till anonymitet i utförandet finns. Mer utbildning och stöttning i ämnet behövs. / Background: In recent decades, awareness of child abuse and the development of preventive policies has increased. Ambulance nurse have a unique opportunity to identify children who may be in a vulnerable situation as they are the first at the scene in the event of acute illness and can pick up signs in the home environment which could lead to suspicions of neglect being reported.This may result in less suffering for the children concerned, but in order for this to happen, it is important to be aware of the implications of making a report. Purpose:The purpose of the study was to describe the ambulance nurses’ experiences of reporting suspicions of child neglect. Method:The study is based on ten qualitative interviews. One open question with the possibility of follow-up questions were posed, based on an interview guide. The material was analyzed on a latent level based on Lundman and Hällgren Graneheim's qualitative content analysis Result: The results are reported based on three main categories: Suspicions arise, Impingement of reporting child neglect andCooperation and Transparency.Seven subcategories, that best described the informants experiences, were then selected. It was found that lack of knowledge regarding the identification of children as well as an ignorance of what one's own responsibility implies exists. The fear of reprisals can be seen as a reason for not reporting on suspicion of child neglect. Conclusion: Education in the subject child neglect are important for identifying the exposed children. A fear of reprisals may be the reason why no report on child neglect is made but could increase, if there is an opportunity for anonymity in the implementation. More education and support on the subject is needed.

'This shared parenting we do is difficult to get your head around' : experiences of parents and carers during their child's first year at a residential therapuetic special school : a qualitative study

This research examined the impact on parents and carers of having a child placed at a residential special school for abused, neglected and traumatised primary aged children. The school is developing its work with families and carers, because if relationships at home are improved, children are more likely to benefit from the placement at Stowbury. Although it is the child who is referred, establishing a good working relationship with parents and carers is vital in helping them with their child’s return home. The study focused on parental experiences of the child’s first year at the school, using interviews at the start of placement and then twelve months later. The data were analysed using a comparative thematic analysis at two time points and a secondary narrative analysis. Researcher reflexivity is used and where appropriate the findings are discussed from a psychoanalytic perspective. The analysis found that during the first year the children make positive changes. Birth parents were helped with their parenting, particularly the (re)establishment of parental boundaries. In contrast, some foster carers found it difficult to share the parenting role, and tensions between home and school were identified. Some parents and carers found it difficult to reflect on their role and relationship with their child. In general, parents and carers expected their child to be able to go to mainstream school when they left Stowbury, and after one year some realised that was unlikely. The study concludes that the experience of foster carers could be improved if they were helped to increase their ability to reflect on their parenting role. In addition, sharing the parenting of this group of children can cause tension between parents, carers and staff. There are training implications for the professionals involved. This research makes an original contribution to knowledge about the psychodynamics that develop in the relationships between the parents, carers and staff looking after abused children in a residential school.

A Meta-Study of Filicide: A Reconceptualization of Child Deaths by Parents

abstract: Filicide, the killing of a child by a parent, is the focus of this meta-study. In the United States, the total number of nonaccidental deaths of children at the hands of a parent is unknown. Five children a day under the age of five die from fatal abuse and neglect (U.S. Advisory Board on Child Abuse and Neglect, 1995). This number is a conservative estimate and does not include children kill by means other than abuse and neglect. Regardless of the number, this author views each filicide as a sentinel event for the United States and the world. A sentinel event is an unexpected occurrence involving death and signals the need for immediate investigation and response. The perspectives of social constructionism and role theory frame this meta-study. The author explored six questions of the extant filicide research: What is the research knowledge on filicide? How is filicide constructed in the research discourse and what is the context of this research? Is filicide constructed as a social problem? Can the use of role theory advance our understanding of filicide? Are there common themes in the filicide research findings? Is there disagreement in the research? What is missing, assumed, or overlooked in the research? The sample consisted of 66 international studies of parents (i.e., genetic, step, foster, person in role of parent) who killed their child(ren) from 1969 to 2009. Major findings include "meta-categories" of filicide research, risk factors, salient themes, and new conceptualization of filicide based on role theory. Individual, social, and structural variables to identify and prevent filicide are presented. Role pedagoga v problematice CAN / The role of the teacher in the issue of CAN

The aim of this thesis was to monitor the school community's awareness about the issue Syndrome CAN and the ability and willingness of teachers to solve such situations. The theoretical part was focused on the characteristics of the CAN syndrome, its forms, manifestations and causes. This part was also given to the legislation and prevention. The research was attended by a total of 148 teachers of kindergartens and elementary schools situated in South Bohemia and Highland. Four hypotheses were defined for the research purposes, three of them: 1/"the educational community admits that the availability of information about the syndrome CAN should be improved",2/"if the teachers knew about incidents of violence, neglect or abuse of a child they would be willing to solve this situation and not 3/"the teachers are not fully confident that they could recognize the symptoms of the syndrome CAN", were confirmed. The hypothesis that the teachers know how to deal with the situation when the violence, neglect or child abuse is revealed wasn´t confirmed. According to the research the teachers are not convinced or do not know how to deal with the situation when they are confronted with abused or neglected child. The data were statistically processed and evaluated.

Os maus-tratos na infância têm se revelado um foco de preocupação no meio científico devido às descobertas das graves conseqüências a curto e longo prazo no desenvolvimento da criança. No meio político e social, no entanto, essa questão, e suas graves conseqüências, parece caminhar ainda muito lentamente na direção do estabelecimento de políticas públicas efetivas. A escassez de estatísticas e a ausência de um conhecimento mais aprofundado das diversas situações de maus-tratos fazem com que a problemática receba uma atenção pouca adequada e especializada. A literatura científica aponta a provável existência de um grande número de casos que não se chega a conhecer, para além dos números divulgados por órgãos oficiais de proteção. Com o objetivo de estimar de forma mais precisa o número de casos de maus-tratos domésticos na cidade de Ribeirão Preto, o presente estudo buscou mensurar a prevalência de maus-tratos, por amostragem, em crianças matriculadas de 1ª a 4ª série do ensino fundamental nos estabelecimentos educacionais da rede pública e particular, a partir de informações obtidas junto ao setor da educação. A investigação adotou uma abordagem quantitativa/descritiva, de caráter epidemiológico. O instrumento utilizado foi a artilha Epidemiológica, já testada em nossa realidade em estudo semelhante, em que se focou a faixa etária de 0 a 6 anos. Esta foi elaborada para abordar os profissionais da educação e, no presente, foi utilizada junto a professores responsáveis por salas de 1ª a 4ª série do ensino fundamental do sistema público e particular do município. Além do número de casos e de suas características, a Cartilha também permite investigar os fatores de risco associados e os indicadores comportamentais/emocionais das crianças assinaladas. Respeitando-se o princípio da aleatoriedade, procedeu-se a um sorteio das escolas, por região da cidade, tendo chegado a um número de 151 professores entrevistados responsáveis por 3.885 crianças. Os resultados indicaram uma prevalência de 3,9% que calculada para a população varia entre 3,3% e 4,6% (IC=95%). Os tipos de maus-tratos assinalados, mais freqüentemente, pelos professores foram Maltrato Emocional, Abandono Emocional e Falta de Controle Parental. O Desemprego, (32%), dificuldades econômicas graves (26%) e baixa escolaridade (26%) foram os fatores de risco mais freqüentemente assinalados nas famílias. 75% das crianças assinaladas têm problemas escolares, parecem não ter interesse em aprender (62%) e parecem ter baixa auto-estima (57%). Ainda, os professores relataram que, pelo menos 72% dos casos assinalados não eram conhecidos dos órgãos oficiais de proteção. Isso significa que em cada sala de aula existe pelo menos uma criança que está vivendo uma situação adversa em casa sem receber qualquer tipo de acompanhamento, confirmando as indicações da literatura quanto ao fato de os dados oficiais serem a ponta do iceberg. Além disso, as crianças assinaladas já estão apresentando conseqüências desenvolvimentais importantes. A negligência como tipo mais freqüente merece ser melhor compreendida em estudos posteriores, devido aos graves danos que provoca às crianças. Em relação aos fatores de risco, os professores assinalaram mais freqüentemente o fato de os adultos das famílias estarem desempregados, passando por dificuldades econômicas e de possuírem baixa escolaridade. Child maltreatment has become a focus of concern in scientific literature mainly because of the discoveries of the serious outcomes to child development. In social and politician circles, however, this issue and its serious consequences, seems to walk slowly in the direction of the establishment of effective politics. The lack of statistics and the absence of knowledge about the diverse situations of child maltreatment permits that the problematic still receives insignificant adequate and specialized attention. Scientific literature points the probable existence of a great number of cases that is not known despite the numbers showed by protection agencies. The objective of this study is to estimate the prevalence of maltreatment in children (from 7 to 10 years old) in the city of Ribeirão Preto. A representative sample was used gathering children from public and private schools. Teachers were interviewed answering The Cartilha Epidemiológica already tested in our reality in a similar study for children from 0 to 6 years old. Besides the number of cases and its characteristics, the Cartilha allows investigating the risk factors associated to the child maltreatment and behavioral/emotional problems of the children refereed by teachers. A descriptive/quantitative approach was used to analyze data. 151 teachers who were responsible for 3885 children answered the Cartilha. The results pointed a prevalence of 3,9% (3,3% and 4,6% ,IC=95%). The most frequently types of child maltreatment were: ?Emotional Maltreatment, Emotional Abandonment and Lack of Parental Supervision. The unemployment, (32%), serious economic difficulties (26%) and low level of education (26%) had been the risk factors more frequently in the childrens families. With respect to the behavioral/emocional problems, 75% of children had school problems, 62% had no interest in learning and 57% had low self-esteem. Teachers pointed as well that at least 72% of the maltreated children didn`t receive any attention of protective services. This means that in each classroom exist at least on child suffering form maltreatment without receive any help. These results also confirm the indications of literature with respect to the fact that official data is just the tip of iceberg. Moreover, all these children are presenting important desenvolvimentais outcomes. The negligence as the most frequent form of maltreatment deserves to be better understood in posterior studies. In relation to the risk factors, teachers frequently pointed the unemployment of the adults of the families passing thru economic difficulties and low rates of education. However, is important to consider the fact that teachers have insignificant knowledge about the families denoting an enormous distance between family and school that should be reduced.

Orientação da atenção em pacientes portadores de tumor do lobo parietal / Orientation of attention in patients with parietal lobe tumor

Atenção consiste em conjunto de processos que leva à seleção ou priorização no processamento de certas categorias de informação, em detrimento de outras, possibilitando um processamento mais eficiente do que seria possível caso o sistema nervoso processasse os estímulos presentes no ambiente simultaneamente. A atenção participa da maioria das funções cognitivas humanas. Depende, portanto, de região cerebral com privilégios anátomo-fisiológicos, como o lobo parietal, cujo padrão de conectividade (áreas unimodais, córtex pré-motor, colículo superior, giro cingulado, giro parahipocampal, insula, córtex orbitofrontal) possibilita a integração sensório-motora e cognitiva necessária à atenção. Desde o início da década de 80 estudos sobre o lobo parietal tem sugerido uma reavaliação de suas funções, modificando a percepção comum de que esteja relacionado exclusivamente a desempenhar funções espaciais, incluindo uma suposta especialização do lobo parietal direito na distribuição da atenção no espaço. No entanto, a base fisiológica para a especialização do lobo parietal na orientação atencional é mal compreendida pelas seguintes razões: 1- lesão unilateral do lobo parietal direito determina deficiência de processamento atencional em relação ao hemiespaço contralateral, e raramente ocorre após lesão do lobo parietal esquerdo; 2- o processamento das informações pelo sistema visual humano varia sensivelmente em relação aos campos visuais, e a metodologia dos testes atencionais tradicionais não considera que o desempenho dos voluntários pode ser limitado pela visibilidade dos estímulos; a localização dos estímulos tem sido avaliada independentemente da disposição dos alvos no campo visual. Nossa proposta é padronizar as condições de estimulação no teste de Posner para orientação da atenção, considerando os limiares específicos de cada voluntário e avaliar as deficiências de orientação da atenção endógena e exógena nos planos horizontal, vertical e diagonal, em pacientes portadores de dano nos lobos parietais direito e esquerdo, secundário a neoplasia, e compará-las entre si e com voluntários saudáveis. Desse modo poderíamos contribuir para o conhecimento sobre as bases neurais da atenção e para o desenvolvimento de estratégias eficazes e individualizadas de reabilitação / Attention consists of processes that lead to selection or prioritization in processing certain categories of information over others, allowing more efficient processing than would be possible if the nervous system had to process the stimuli in the environment simultaneously. Attention integrates most of human cognitive functions. Therefore it depends on specific brain regions with anatomical and physiological privileges, such as the parietal lobe, which pattern of connectivity (unimodal areas, premotor cortex, superior colliculus, cingulate gyrus, parahippocampal gyrus, insula, orbitofrontal cortex) enables sensorimotor and cognitive integration required for attention. Since early of 1980´s the common perception that parietal lobe is related solely to performance of spatial tasks, including a supposed specialization of the right parietal lobe for the distribution of attention in space is changing. The physiological basis for the specialization of the parietal lobe in orienting of attention is poorly understood for the following reasons: 1 - unilateral lesion of the right parietal lobe attentional determines disability in attention processing of information which comes from the contralateral space, and rarely occurs after injury of the left parietal lobe 2 - the processing of information by the human visual system varies considerably in relation to visual fields, and traditional attentional testing methodology does not consider that performance of the volunteers may be limited by the visibility of the stimuli; the location of the stimuli has been evaluated independently on the targets position in the visual field. Our proposal is to standardize the conditions of stimulation in the Posner test for orienting of attention, considering the specific visual thresholds of each subject and evaluate exogenous and endogenous orientation of attention deficiencies in horizontal, vertical and diagonal plans, in patients with damage to the right and left parietal lobes, secondary to brain tumor, and compare them among themselves and with healthy volunteers. Thereby this approach could contribute for the knowledge about the neural bases of attention and therefore help to develop effective strategies for rehabilitation

En förlorad barndom : En kvalitativ studie baserad på självbiografier med fokus på individers upplevelse av trauma och dess konsekvenser / A lost childhood

The aim of the study was to describe and analyse self-biographical narratives through a retroperspective, how three exposed individuals formulated the appearance and management of trauma during their childhood. The aim was further to describe the impact the trauma had in their childhood, due to the fact that they grew up with a disabled parent. To do so I had to get information from the individuals and chose to use autobiographies as empirical material. In order to implement the study I sought answers to four questions: The first question was how adults described experienced trauma and its origin during their childhood, that their parents had caused by them, depending whether the parents having a mental or social disable? The second question was how the adults handled their experience of trauma which occurred during their childhood? The third question for the study was which consequences the trauma had on the adults as children? The last question was which different factors of protection the individuals described during their childhood and how they related to these? The study showed that risk of the individuals suffering from trauma increased the longer the individual were exposed to violence and neglect. The severity of violence and neglect contributed to the origin and duration of trauma and caused problems that persisted in the children. The results also showed that children who experienced trauma, caused by their parents, depending their parents having a mental or social disable, had multiple consequences. The parents of the children often used physical or mental abuse against the children or neglected their basic needs of food and clothes. The children showed signs of distrust of others, problems with sleeping and often felt scared and ashamed of their parent’s behaviour, when the parents were sick. Furthermore the result of the study showed that the children tried to stop the violence from their parents by all means by trying to change their own behaviour. They did so because they thought they were to blame for the violence.

This thesis on the topic of criminal aspects of Child Abuse is trying comprehensively to introduce the issue of child abuse primarily in association with CAN Syndrome. Child abuse is very dangerous social phenomenon, mainly because the victim is child who is considered to be defenceless and trusting being. The Child Abuse and Neglect Syndrome has several forms of abuse and that is what makes it even harder to discover and early intercept not just for uninitiated society but even for specialists. The expert knowledge of this issue is still behind to where it should be. That also reflects on the legislation. Even though it is trying to deal with this problem there are still certain shortcomings, which should be improved. This thesis is divided into sex chapters. First chapter concentrates on the term CAN Syndrome itself and on his evolution. It also includes statistics on victims of different forms of CAN Syndrome. Second chapter contains single types of child abuse and forms in which they can be manifested and observed by the environment. Barns upplevelser av emotionella kränkningar och övergrepp inom barn- och ungdomsidrotten / Children´s experience of emotional abuse and maltreatment in children´s sports

Since 2009 the Swedish Sport Confederation has included a child rights perspective in their policy guidelines for child sport in Sweden. These guidelines states that all children shall be protected from all forms of physical or mental violence, injury or abuse, neglect or negligent treatment, maltreatment or exploitation. Despite this, studies show not only that emotional abuse and maltreatment occurs in children´s sport but that it is has a great effect on children. The aim of this study was to analyze the experiences and effects of emotional abuse and maltreatment among children athletes. Data were gathered through semi-structured interviews with nine children, 13-18 years old, from both team- and individual sports. The result shows that emotional abuse and maltreatment occurs between both athletes and between athletes and their coaches. It is the coherency and atmosphere in the group that is a vital factor for the sport to lead to positive feelings and experiences. When there is no strong coherency, the atmosphere is perceived as negative, destructive criticism between athletes increases or when destructive criticism comes from coaches, it leads to negative feelings and experiences. The most commonly form of emotional abuse and maltreatment was verbal abuse, lack of attention, neglect and non-verbal abuse. The result show effects such as decrease in performance, lower self-confidence and insecurity. It also shows effects associated with feelings such as irritation, inadequateness, unsuccessfulness and sadness. Emotional abuse and maltreatment occurs on practice, competitions as well as beside the field of play and is in the vast majority of situations linked to performance.

Predictability of Identity Voting Behaviour, Perceived Exclusion and Neglect, and the Paradox of Loyalty: A Case Study of a Conflict Involving the Ewe Group in the Volta Region of Ghana and the NDC-led Administrations

The Republic of Ghana is the legacy of the colonial amalgam of multiple, and previously distinct, ethnic homelands. The Trans-Volta Togoland became the Volta Region of Ghana following a Plebiscite in 1956. The dominant ethnic group in this region; the Ewe, has long maintained a claim of neglect of the Volta Region and the marginalization of its people in this postcolonial state. Protests in the street and at media houses ensued against the State. This qualitative case study explores the undercurrents of this conflict in the context of the Ewe group’s identity and their experiences of neglect and marginalization in the postcolonial state. The main objective of the study was to understand why the Ewe group has not revolted despite the perceptions of deprivation. This study focused on the Ewe group in the Volta Region of Ghana a as sub-colonial construct that has managed its perceptions of deprivation without revolting against the host State.
