Necessidade de reabordagem cirúrgica após tratamento de fraturas mandibulares por fixação interna rígida / Necessity of surgical retreatment in mandibular fractures after treatment by rigid internal fixation

As fraturas de mandíbula são freqüentes e o seu tratamento é por meio de fixação interna rígida. Complicações podem ocorrer após o tratamento das fraturas mandibulares levando a necessidade de reabordagem cirúrgica, havendo poucos estudos a esse respeito na literatura. A proposta deste estudo retrospectivo foi avaliar as características, os possíveis fatores de risco e os tipos de tratamento realizado em pacientes que necessitaram de reabordagem cirúrgica de fraturas de mandíbula tratadas com fixação interna rígida (FIR). Dentre 364 pacientes tratados por fraturas de mandíbula com FIR, houve 17 pacientes (4,7%) que necessitaram de reabordagem cirúrgica, tendo sido incluídos três pacientes provenientes de outros serviços, totalizando 20 casos com necessidade de nova cirurgia. Houve predomínio do gênero masculino, com idade média de 31,4 anos, sendo freqüentes o tabagismo e o etilismo. Foram freqüentes fraturas múltiplas e cominutivas nas regiões de corpo e ângulo mandibular, dente no traço e exposição intraoral da fratura. O tempo de espera para primeira cirurgia foi alto e o acesso extraoral e o sistema de fixação menos rígido 2.0 mm foram freqüentes. As complicações mais comuns foram dor, infecção e mobilidade anormal. Nas culturas bacterianas houve predomínio do Staphylococcus aureus e a imagem mais freqüente foi de reabsorção óssea difusa, seguida por parafuso solto, seqüestro ósseo, traço de fratura visível, fixação solta e placa fraturada. A reabordagem cirúrgica ocorreu em média de 7,5 meses após a primeira cirurgia e constou de remoção dos meios de fixação associada ou não a nova fixação ou ainda a remoção de seqüestro ósseo, sendo que apenas um caso necessitou de refratura. Histologicamente houve predomínio de osteomielite crônica. Os diagnósticos em ordem decrescente foram infecção, pseudoartrose, osteomielite e placa exposta, sendo que muitos pacientes tiveram mais de um diagnóstico. Foi destacada a freqüência de tabagismo e etilismo, fraturas múltiplas e cominutivas na região de corpo e ângulo mandibular, dente no traço, exposição intraoral, tempo de espera alto e acesso extraoral predispondo complicações das fraturas mandibulares e exames de imagem de reabsorção óssea, fixação e parafusos solto e seqüestro ósseo e diagnóstico histológico de osteomielite como característica dos casos requerendo nova cirurgia. / Mandibular fractures are frequent and their treatment is through rigid internal fixation (RIF). Complications can occur after treatment of the mandibular fractures which may require a new surgical procedure, and there are a few studies about that in the literature. The purpose of this retrospective study was to evaluate the characteristics, possible risk factors, and the kinds of treatment did in patients which needed another surgery after treatment of mandibular fracture with RIF. From 364 patients with mandibular fractures treated by RIF, there were 17 patients (4.7%) with need of a new surgery, and 3 patients coming from another city were included, comprising a total of 20 patients who needed a new surgery. There was predominance of the male gender, with a mean age of 31.4 years, being frequent smoking and alcohol abuse. Multiple and comminuted fractures on the body and angle sites, teeth in the fracture line, and intraorally exposed fractures were frequent. Delay time to the first surgery was high, and extraoral approaches and system 2.0mm were predominant. The most common complications were pain, infection and abnormal mobility. In the bacterial culture there was predominance of Staphylococcus aureus, and the most frequent radiographic images were of diffuse bone resorption, loosening of screws, bone sequestration, fracture line visible, loose fixation, and fractured plate. A new surgery occurred with a mean of 7.5 months after the first intervention and comprised plate and screws removal associated or not to a new fixation or bone sequestra removal, and only a case the fracture needed to be osteotomized. Histologically there was predominance of chronic osteomyelitis. The diagnoses in decreasing order were infection, nonunion, osteomyelitis and exposed plate, although many patients had more than one diagnosis. It was evidenced the frequency of smoking and alcohol abuse, multiple and comminuted fracture on the body and angle regions, teeth in the fracture line, intraoral fracture exposition, high delay time and extraoral approaches predisposing complications of the mandibular fractures, and images showing bony resorption, loose hardware and bone sequestra, as well as histological diagnosis of osteomyelitis as characteristic of the cases requiring a new surgery.

Desenvolvimento de um algoritmo para um sistema dinâmico representante de um mecanismo de prótese de joelho

A falta de um membro, devido a doenças, más-formações ou traumas, impacta enormemente na vida de um indivíduo. Para que o mesmo possa realizar Atividades de Vida Diária (AVD), sem drásticas limitações, faz-se necessário o desenvolvimento de próteses e cientes. Para auxiliar no projeto de tais produtos, este trabalho visa criar e solucionar um modelo de sistema dinâmico capaz de simular o comportamento de joelhos prostéticos. A m de alcançar tal objetivo, foi estudada a biomecânica da marcha humana, além dos tipos de amputações existentes e seus impactos na mesma. Buscou-se também estudar os principais componentes de próteses existentes atualmente no mercado, além de suas geometrias básicas. Após de nir-se um modelo simpli cado que representasse os componentes protéticos e as partes remanescentes do corpo, foram buscadas métodos de solução disponíveis para tal sistema. Baseado nos estudos encontrados em uma revisão bibliográ ca, foi de nida uma metodologia de solução numérica de sistemas multi-corpos. Durante o trabalho, surgiu a necessidade de representar forças de contato entre corpos rígidos, e foram incluídas soluções de atuais teorias do assunto. Essas metodologias foram então empregadas para a construção de um algoritmo capaz de solucionar o sistema proposto. Depois de resolvido o sistema, os valores encontrados para as variáveis cinéticas foram comparados com valores obtidos por uma simulação numérica utilizando dois programas computacionais comercias, cujas fundamentações teóricas utilizam diferentes métodos de solução. A comparação das diferenças entre os resultados apontou similaridade entre o algoritmo e os programas utilizados, mas com a necessidade de validação do método através de ensaio mecânico. Ao nal foram sugeridas ideias para trabalhos futuros. / The lack of a member due to diseases, malformations or trauma, greatly impacts the life of an individual. In order to perform Activities of Daily Living (ADL) without drastic limitations, it is necessary to develop e cient prosthetic devices. To assist the design of such products, this work aims to create and solve a dynamic system model to simulate the behavior of prosthetic knees. In order to achieve this goal, the biomechanics of human gait was studied, as well as the existing types of amputation and their impact on human gait. The main components of existing prosthetic devices, currently on the market, were identi ed, as well as its basic geometries. After de ning a simpli ed model able to incorporate the prosthetic components and the individual remaining body parts, an available methodology for the solution of such a system was prospected. Based on the studies found in the currently literature of multibodysystems, a numerical solution methodology for has been set. During the work, the need to represent contact between parts occurred, leading to the implementation of currently theories of contact forces for rigid bodies. These methodologies were then used to construct an algorithm for solving the proposed system. After the system has been solve, the values found for the kinetic variables were compared with those obtained by numerical simulations using two commercial softwares, whose theoretical foundation used di erent solution methodology. Comparing the di erences in results of these simulations, it was revealed a similar behavior between the algorithm and the commercial programs, but with the need of an experimental test, for truly validation of the method. At the end of this work, ideas for future improvement were suggested.

Embora sua robustez e facilidade de manutenção tenham conservado, em linhas gerais, a disposição e projeto dos componentes, novos requerimentos de carga e velocidade dos trens vêm mudando a forma como os projetistas enxergam o truque. Especial atenção tem sido dada ao modelo matemático da cunha de atrito, a peça fundida que é responsável pelo amortecimento dos vagões. A cunha de atrito promove a dissipação da energia mecânica por meio de contato de atrito seco com outros componentes do vagão. Devido às altas forças normais desenvolvidas nas superfícies de contato com características não suaves e, em geral, não lineares de atrito, as equações que regem o movimento da suspensão tornam-se de resolução difícil e surgem fenômenos como o de adesão escorregamento e o comportamento caótico típico de osciladores auto excitados. O presente trabalho tem como objetivo propor o uso de algoritmos de solução de problemas de complementaridade linear para resolver as forças de contato entre os corpos, visando a aprofundar a discussão sobre os modelos adotados para a cunha de atrito. Os resultados obtidos mostram que é possível modelar as forças de contato desse sistema utilizando um problema de complementaridade linear e que essa abordagem é, sob certas condições, mais eficiente do que o método das penalidades, normalmente aplicado para a resolução de problemas de contato / Abstract: For about 150 years now, three-piece trucks have been the standard axis' and suspensions' subframe used in freight railroad cars. The toughness and low maintenance costs of this system worked to maintain its basic design almost unchanged, but new requirements for loads and speed for freight cars are changing the way designers see the three-piece truck. Among the many interesting components of the three-piece truck, the friction wedge is getting some attention. The friction wedge is the main damping element in three-piece trucks and acts to dissipate mechanical energy via highly stiff contacts with friction. Due to the non-smooth and non-linear nature of frictional efforts, the equations of motion of the three-piece trucks become very awkward to deal with. Interesting phenomena of stick-slip, bifurcations, and limit cycle, typical of friction oscillators appear to some extent under normal operation. This work's main objective is to propose a new approach based on complementarity problems, used to solve for contact forces, to further extend the discussion on wedge dampers models. São apresentadas as bases teóricas para esta nova modelação, justificadas através do equacionamento geral do escoamento fluido em condutos forçados. Os resultados obtidos pelo MDIR são comparados com os resultados obtidos pelo programa EPANET que utiliza o método gradiente. Discussões a respeito da importância da incorporação do efeito de inércia são apresentadas através de um estudo de casos, no caso de modelação hidráulica, e para análises de qualidade decorrentes desta. / Abstract: This research describes the stages which aim the use of the original structure proposed by Todini and Pilati in the so called gradient method (GM), used in the analysis in steady state of pipe networks, in the formulation of an inertial rigid dynamic model (IRDM) to analyse slow transients in this type of installation. The theoretical bases are presented for this new method, justified by the general equation of fluids flow in pipes networks. The results obtained by the MDIR are compared to the results obtained in the EPANET program, which uses the gradient method. Análise experimental estática e dinâmica da rigidez de ligações viga-pilar de concreto pré-moldado / Static and dynamic experimental analysis of precast concrete beam-to-column connection stiffness

Em estruturas de concreto pré-moldado, as ligações usualmente apresentam comportamento semi-rígido, apesar desse tipo de vinculação não ser sempre considerado no projeto e análise das estruturas. Ensaios dinâmicos para a determinação da rigidez da ligação apresentam a vantagem de serem não-destrutivos e a possibilidade de serem utilizados na verificação das condições reais de vínculo ou para a avaliação da necessidade de um eventual reforço estrutural. Este trabalho apresenta o desenvolvimento de um método para a análise da deformabilidade de estruturas de concreto baseado em ensaios dinâmicos, focalizando a semi-rigidez das ligações de elementos pré-moldados. Os objetos de estudo específicos são as ligações viga-pilar com capacidade de transmissão de forças e momentos. Em uma etapa preliminar desta pesquisa, realizaram-se ensaios-piloto em diferentes modelos de estruturas de concreto e de aço, em tamanho real e reduzido, com diferentes condições de contorno, para avaliar os instrumentos, equipamentos e sensores disponíveis. A partir da verificação da metodologia experimental proposta, realizou-se um estudo comparativo dos resultados obtidos com ensaios estáticos e dinâmicos em um modelo reduzido de pórtico de concreto pré-moldado, utilizando métodos diretos na obtenção da rigidez da ligação entre os elementos. Na confecção das ligações viga-pilar, foram empregados dispositivos usuais, tais como: dente e consolo, chumbador, almofada de apoio e preenchimento com graute e, posteriormente, aplicados reforços por inserção de laminado de PRFC inseridos no concreto de cobrimento, que permitiu o incremento da resistência e da rigidez à flexão das ligações. A metodologia experimental para determinar a rigidez da ligação viga-pilar envolveu procedimentos diretos, com ensaios dinâmicos de vibração forçada e sinais medidos por acelerômetros, e ensaios estáticos usuais em que se utilizam transdutores de deslocamento e clinômetros. Os resultados dos ensaios com os modelos de concreto, corroborados pelos resultados dos modelos metálicos, demonstram a validade da utilização da metodologia experimental dinâmica para determinação direta da rigidez de ligações. As conclusões obtidas aplicam-se diretamente ao projeto das estruturas e à prática da investigação estrutural, dentro do ambiente acadêmico e da indústria. / Usually precast concrete connections are considered as perfect joints (pinned or rigid), however the real behavior can be intermediate what is called semi-rigid. The use of dynamic tests is an advantage to obtain stiffness values without damaging the structure and can be used to assess the real boundary conditions or verify the need for structural strengthening. This research aims to develop a dynamic experimental procedure to determine the beam-to-column connection stiffness of precast concrete structures directly from accelerometer measured data. The preliminary tests were carried on specimens of real size or reduced scale, as well as concrete or steel connection specimens with different boundary conditions. The experimental tests were conducted in a reduced scale concrete frame composed of precast columns and beam. The frame was tested through dynamic and static methods in order to obtain the beam-to-column stiffness value directly. The beam was connected to the columns by pinned connections, formed by support cushions and dowels. In addition, the connections were strengthened by embedding CFRP strips in the lateral concrete cover, according to NSM technique (Near-Surface Mounted). Experimental static and dynamic tests were done at different loading steps. The results obtained with concrete and steel models show good agreement between static and dynamic tests.

Studujeme nespočetné struktury, které splňují exstension property vzhledem k nějaké Fraïssé třídě C. Takovým strukturám říkáme Fraïssé-like struktury. Tyto struktury nejsou většinou jednoznačně určeny. Je známo, že pokud existuje Katětov funktor pro C, pak existují Fraïssé-like struktury libovolné kardinality s bohatou grupou automorfismů. Ukážeme, že v případě třídy všech konečných grafů a všech konečných metrických prostorů existuje Fraïssé-like struktura, která má kardinalitu ℵ1 a její grupa automorfismů je triviální. Dále zodpovíme otázku z W. Kubi's, D. Mašulovi'c, Katětov functors, to appear in Applied Categorical Structures tak, že nalezneme Fraïssé třídu bez Katětova funktoru. 1

Modèles statistiques réduits de la croissance cardiaque, du mouvement et de la circulation sanguine : application à la tétralogie de Fallot / Reduced-order statistical models of cardiac growth, motion and blood flow : application to the tetralogy of Fallot heart

Cette thèse présente les travaux réalisés en vue de l’élaboration d’un modèle cardiaque associant croissance, mouvement et circulation sanguine pour permettre ensuite la construction d’un modèle patient à partir d’un modèle de population. Le premier axe de ce travail est la simulation de la croissance bi-ventriculaire. Un modèle existant de surface unique, calculé à l’aide de méthodes statistiques, a été généralisé à un modèle bi-ventriculaire puis appliqué à la tétralogie de Fallot (ToF). Le deuxième axe concerne la modélisation du mouvement cardiaque au niveau de la population. Un modèle d’ordre réduit basé sur un modèle Polyaffine et LogDemons a été proposé. Il simule la dynamique cardiaque avec peu de paramètres. Les paramètres de transformation sont analysés par des méthodes statistiques. Un modèle de mouvement moyen a été calculé pour représenter le mouvement standard de la population. Le troisième axe s'intéresse à la simulation de l’écoulement sanguin à l’échelle de la population. La complexité des simulations spécifiques à un patient a été réduite grâce à l’utilisation de méthodes d’analyse d’image, de dynamique des fluides numérique et de réduction d’ordre de modèle. La simulation du flux sanguin dans l’artère pulmonaire pour des patients ToF a permis de mieux comprendre l’impact du sang régurgité sur la pression et la vitesse. Étant donné nos contributions sur ces trois axes, nous sommes maintenant en bonne position pour élaborer le modèle couplé des contributions interdépendantes de la croissance, du mouvement et de l'écoulement sanguin. Ce modèle pourrait être utilisé afin d'aider la planification de la thérapie chez les patients atteints de maladies cardiaques. / This thesis presents work towards a coupled model of cardiac growth, motion, and blood flow to enable predictive patient-specific models to be built from a population-based model. The first axis of this work is to simulate bi-ventricular growth through aging. A previously proposed single surface model computed using statistical methods was extended to a bi-ventricular model and applied to Tetralogy of Fallot patients to model the complex evolution of the ventricles due to the pathology. The second axis concerns the development of a model to simulate cardiac motion at a population level. A reduced-order cardiac-specific motion model was proposed to simulate the motion dynamics with a small number of parameters using a Polyaffine and LogDemons based model. From the computed transformations, the parameters were analysed using statistical methods to obtain population-based measures of normality. A mean motion model was derived to represent the normal motion for a given population. The third axis is to develop a model of population-based flow dynamics. The complexity of patient-specific simulations was reduced by combining image analysis, computational fluid dynamics and model order reduction techniques. Blood flow through the pulmonary artery in Tetralogy of Fallot patients was simulated to better understand the impact of regurgitated blood on pressure and velocity. Given our contributions on these three axes, we are now in a good position to couple the models in order to capture the interrelated contributions of growth, motion and flow. Such a model could be used to aid in therapy planning and decision making for patients with heart disease.

Suivi d’objets dans des séquences d’images de scènes déformables : de l’importance des points d’intérêt et du maillage 2D / Objects tracking in video of non rigid scenes : importance of interest points and 2D mesh

Nous abordons le suivi d’objets dans des séquences d’images de scènes déformables selon deux axes de recherche. Il s’agit de déterminer les transformations d’un objet, d’une image à l’autre, lorsque celui-ci s’est éventuellement déformé ou déplacé et lorsque le point de vue de la caméra a éventuellement été modifié (déplacement, zoom...). Pour cela, nous nous sommes inspirés de l’algorithme de Jurie et Dhome qui permet de suivre un objet plan indéformable. D’une part, nous en améliorons les performances. D’autre part, nous le généralisons au cas d’objets déformables. Le premier axe de recherche consiste à améliorer les performances de l’algorithme de Jurie et Dhome en termes de précision et robustesse. Le suivi s’appuie sur un ensemble de points d’intérêt, dont dépendent fortement les performances. Ces points d’intérêt sont issus d’une sélection des points obtenus par des détecteurs reconnus, à savoir SIFT, KLT, SUSAN, HARRIS et MORAVEC. Nous avons étudié et mis en oeuvre, sur différentes classes d’images, des heuristiques de sélection fondées sur des approches statistique et algébrique. Nous montrons : – qu’il n’existe pas de détecteur universel, – que l’approche statistique est à privilégier dans tous les cas. Le second axe de recherche est une proposition d’un nouvel algorithme de suivi s’appuyant sur le maillage 2D des images de la séquence. Cet algorithme généralise celui de Jurie et Dhome aux scènes déformables. Il repose sur : – des transformations élémentaires (nodales) du maillage, directes et inverses, que nous avons caractérisées tant d’un point de vue géométrique qu’analytique, – l’utilisation des coordonnées barycentriques généralisées pour approcher la composition de deux transformations d’un maillage. Cet algorithme donne des résultats similaires à celui d’appariement hexagonal de Nakaya et Harashima tout en étant plus rapide. / We deal with object tracking in videos of non-rigid scenes with two main purposes. We aim at determining the transformations of an object, from one frame to the next, when it may be distorted or moved and when the camera focus may change (movement, zoom...). To do this, we were inspired by the Jurie and Dhome algorithm, which enables the tracking of plane rigid objects. On the one hand, we improve its performance. On the other hand, we generalize it to non-rigid objects. The first goal consists in improving the performance of the Jurie and Dhome algorithm, in terms of accuracy and robustness. The tracking is based on a set of interest points, which has a great effect on the algorithm’s performance. These interest points come from a selection among the points extracted with some common detectors: SIFT, KLT, SUSAN, HARRIS, and MORAVEC.With various pictures classes, we have studied and implemented some selection heuristics based on statistical or algebraic approaches. We show that : • there is no universal detector, • the statistical approach is the best in all cases. The second goal is a proposal of a new tracking algorithm based on a 2D mesh of the video frames. This algorithm generalizes the Jurie and Dhome one for non-rigid scenes. It is based on : • elementary (nodal), direct or inverse, mesh transformations that we geometrically and analytically characterize, • generalized barycentric coordinates to approximate the composition of two mesh transformations. This algorithm gives similar results to the hexagonal matching algorithm of Nakaya and Harashima while being faster.

Gegenstand dieser Arbeit ist die Simulation einer Wirkpaarung am Beispiel des innermaschinellen Transports von Stückgütern. Zur Schonung des Verarbeitungsgutes wird für das intermittierende Fördern ein neuartiger Bewegungsansatz betrachtet. Da sich der mit diesem Ansatz durchgeführte Prozess sehr sensitiv gegenüber den Eingangsparametern verhält, ist es notwendig, diesen zu simulieren, um somit eine stabile und robuste Bewegung zu synthetisieren. Als grundlegender Modellansatz wird die Diskrete Elemente Methode (DEM) gewählt, da diese es ermöglicht, den Kontakt von Starrkörpern realistisch abzubilden. Zur Umsetzung der Simulation wird die Modellbildung mit zwei unterschiedlichen Modellierungsumgebungen realisiert, die sich hinsichtlich der Umsetzung der DEM unterscheiden: Zum Einen mit Hilfe der kommerziellen Software MATLAB/Simulink und zum Anderen mit Hilfe der Open-Source-Software Woo DEM. Im Anschluss werden die damit erzeugten Modelle verifiziert und experimentell validiert, wodurch sich sowohl die Modellgüte als auch die Modelleignung ableitet. Dabei wird besonders auf die prozessentscheidende Rolle der Reibung eingegangen. Abschließend wird mit Hilfe des Vorzugsmodells an Hand zweier Beispiele die Modellanalyse vollzogen. Hierbei wird der neuartige Bewegungsansatz synthetisiert und simulativ hinsichtlich des Prozesserfolges überprüft. Dabei wird insbesondere auf die mit Hilfe des Modells quanitifizierbaren Prozessgrößen eingegangen. / The subject of this thesis deals with the simulation of an active unit demonstrated by the mechanical transport of pieced goods. To protect the processing goods, a novel motion approach for the intermittent transport is researched. Since the process performed with this approach is very sensitive to its input parameters, it is necessary to simulate it, with the aim to synthesize a stable and robust motion. The Discrete Element Method (DEM) is chosen as the basic model approach, because it allows the realistic representation of rigid body contacts. To implement the simulation the modelling is realised with two different modelling environments, which differ in the implementation of the DEM: Firstly, using the commercial software MATLAB/Simulink and secondly with the help of the open-source-software Woo DEM. Following that the generated models are verified and experimentally validated, whereby both the model goodness and the model suitability are derived. Special attention is dedicated to the role of the process relevant friction. Eventually, the model analysis is carried out with the help of two examples using the preferred model. Here, the novel motion approach is synthesized and verified by simulation in terms of process success. In particular it will address process variables, which are quantifiable because of the model.
