A órtese é um dispositivo aplicado à superfície externa do corpo para: controlar o alinhamento biomecânico, corrigir ou acomodar a deformidade, proteger e suportar uma lesão, auxiliar na reabilitação, reduzir a dor, aumentar a mobilidade e a independência. Nos últimos anos, o desenvolvimento de órteses usando manufatura aditiva tem se incrementado tanto para membros superiores como inferiores devido a sua capacidade de reposicionar o membro de maneira mais precisa e personalizada. Este trabalho teve como objetivo desenvolver uma metodologia para projetar e fabricar uma órtese de subluxação de ombro utilizando escaneamento corporal e manufatura aditiva. O usuário foi submetido ao escaneamento 3D para obtenção do modelo digital sólido em formato 3D, operações elementares espaciais foram aplicadas ao modelo escaneado do usuário para a obtenção da órtese a fim de gerar uma silhueta tridimensional adequada ao ombro/tórax do paciente. Finalmente, a silhueta foi evoluída a órtese pela agregação de espessura, furos e contornos. Uma vez obtida a órtese, foi impressa, ajustada e testada no paciente. Uma primeira versão foi proposta em função do relato do paciente quanto à adaptação do ombro e conforto, e outras duas versões evolutivas foram desenvolvidas e avançaram para outros aspectos importantes como a redução de peso, graus de liberdade nos ombros saudávei... Orthotics is a device applied to the external surface of the body to control biomechanical alignment, correct or accommodate the deformity, protect and support an injury, assist in rehabilitation, reduce pain, increase mobility and independence. In recent years, the development of orthosis using additive manufacturing has been increasing for both upper and lower limbs because of their ability to more accurately and custom reposition the limb. This work aimed to develop a methodology to design and manufacture a shoulder subluxation orthosis using body scanning and additive manufacturing. The user underwent 3D scanning to obtain the solid digital model in 3D format, spatial elemental operations were applied to the user's scanned model to obtain the orthosis to generate a three-dimensional silhouette suitable for the patient's shoulder / chest. Finally, the silhouette was evolved into orthosis by aggregating thickness, holes, and contours. Once the orthosis was obtained, it was printed, adjusted and tested on the patient. A first version was proposed based on the patient's report on shoulder adaptation and comfort, and two other evolutionary versions were developed and advanced to other important aspects such as weight reduction, degrees of freedom in healthy shoulders and comfort gain. MRDV / Institute for Rural Development / Traditional medicine still plays a bigger role in people dealing with health and disease. Research shows that in some Asian and African countries, traditional medicine forms the primary mode of health care for 80% of the population. This is actively promoted in South Africa and is used by at least 70% of the country’s population. This study investigated the contributory factors leading mental health care users to use a combination of traditional and modern medicine in rural areas. The study was conducted using qualitative research methods. The population of this study were MHCUs, Mental health practitioners and traditional healers. Purposive sampling was used to select participants. Data was analyzed through a computer programme called Atlas TI. The results revealed the following as contributory factors leading mental health care users to use modern and traditional medicine in rural areas : cultural factor , belief in witch craft , influence by significant others (family members and friends), poor PHC intervention and treatment method. Recommendations to the health care practitioners and policy makers around the use of traditional and modern medicine was made based on the findings.

Swahili modern proverbs: the role of literary writers and social network users

Proverbs are one of the important oral literary genres in various cultures. Though in many societies and for a long time proverbs are regarded as succinct fixed artistic form, and authoritative which contain a general truth, wisdom and experience of the society and its creators are elders or anonymous, these characteristics are increasingly challenged today. This paper, therefore, intends to examine how Swahili literary writers and social network users participate in the creation and spread of Swahili modern proverbs in Tanzania. Data of this study were collected from Swahili literary works and websites. The findings reveal that the need to address and cope with today’s environment and change of worldview of the present generation are among the important factors to the emergence of the modern proverbs. It is also noted that modern Swahili proverbs are not only found among the Kiswahili literary writers and social network users, but also other people and avenues. / Methali ni moja ya utanzu muhimu wa fasihi simulizi katika tamaduni mbalimbali. Ingawa katika jamii nyingi na kwa muda mrefu methali zimekuwa zikichukuliwa kama usemi mfupi wa kisanaa, wenye mamlaka, ukweli, maarifa na tajiriba ya jamii, na watungaji wake ni wazee au hawajulikani, sifa hizi siku hizi zinazidi kudadisiwa. Makala hii, kwa hiyo, inalenga kuchunguza namna waandishi wa fasihi ya Kiswahili na watumiaji wa mitandao ya kijamii wanavyoshiriki katika uundaji na usambazaji wa methali za kileo za Kiswahili nchini Tanzania. Data za makala hii zilikusanywa kwa kupitia kazi mbalimbali za fasihi ya Kiswahili na kutembelea tovuti. Matokeo ya data yanaonesha kuwa haja ya kuakisi mazingira ya sasa na mabadiliko ya kimtazamo kwa kizazi cha leo ni miongoni mwa sababu zinazochangia katika kuibuka kwa methali za kileo. The theme of this study is to evaluate the effectiveness of the social services sheltered housing. Due to demographic developments, increasing demand for social services and increasing spending in the social sector, the effectiveness of social services will be an increasingly important topic. We can assume, that because of to these factors, there will be more pressure on social services to prove their results. This study talks about theoretical concepts of effectiveness and defines the concept of effectiveness, which could be applied in social services. The work summarizes the models of measuring effectiveness in selected countries, including the Czech Republic, and specifies the concept of effectiveness in the Carlsbad region, in which sheltered housing social services are provided. The work also defines the term "the need" of the users of the social services and presents models, that can identify those needs. The part, that determines the effectiveness of social services, is in the research section. This is the part, where identification of adverse social situations and identification of meeting the needs of the users, is specified. The aim of the thesis is to explore which attributes of expression and attributes of manifestation are commonly found in bibliographic records and compare them with the information needs of users. The research is focused on the Union catalog of the Czech Republic - CASLIN. In the theoretical part there are presented problems with attributes of expression and attributes of manifestation in model FRBR. In the research part there was performed content analysis of several bibliographic records owing to problems of attributes of expression and attributes of manifestation. In the research section there was made the analysis of information needs of users, too. As qualitative research methods were selected observation of participants and semi-structured interviews with respondents. The result of this thesis should be a comparison between reality in library catalogs and information needs of users.

An investigation into home and community based health care programmes in Zimbabwe : an analysis of the congruency of service users' needs and the programme goals

The past two decades have witnessed transition from in-patient hospital settings to Community and Home Based Care in Zimbabwe. This is because of an increase in the number of people developing chronic health conditions such as diabetes, hypertension and cancers, and thus needing longer treatment durations. The increase in chronic conditions has mainly been attributed to the increase in incidence and prevalence of Human Immunodeficiency Virus and Acquired Immune-deficiency Syndrome in Sub-Saharan Africa. Many Governments, including that of Zimbabwe, endorsed and promoted community and home based care to provide a continuum of care to the chronically ill. Despite the implementation of Community and Home Based Care programmes, no studies known to the researcher have been done to investigate the congruency of service users’ needs and the programme goals. Purpose: The purpose of the study is twofold; firstly to explore whether Community and Home Based Care interventions provide services that effectively meet users’ goals in a dynamic social environment and secondly, to develop guidelines for enhancing care provision as well as congruency of service users’ needs and programme goals. Methods: A mixed-method approach combining qualitative and quantitative data collection and analysis was used. Concurrent collection of data with equal status was placed on both qualitative and quantitative data. For qualitative data interviews and focus groups were used with home based care service users, family care givers, and health providers. Structured questionnaires were used for the quantitative data. Framework: Imogene King’s Interacting Systems Framework and Theory of Goal Attainment were adopted to guide the study. These were utilised because of the assumption that mutual agreement on goals for effective care can be achieved through nurse-client interactions and communication Research findings: Community and Home Based Care programme in Zimbabwe does not comprehensively meet the needs of its service users. Service users have diverse needs and goals depending on the specific condition and symptoms they experience. They perceive access to medication for symptom management and pain control, food and financial stability, as priority needs. The study found that nursing services provided in Community and Home Based Care generally align with the physical needs of service users and was deficient in addressing the socio-economic and psycho-social needs of service users. Conclusion: No single sector can achieve the users’ and the programme`s goals of quality health on its own. The goals may only be realised when other stakeholders and relevant sectors contribute to the Community and Home Based Care programme, which requires diverse resources. Previous research shows that people with severe disabilities often are neglected in the work of enhancing user participation. It is shown that even though professionals increase the possibility for user participation in their work, it is not necessary that the users receive it. This study aims to show how professionals working for people with disabilities applies user participation in their everyday work. We used a qualitative exploratory method and interviewed ten social workers within service administration and supportive housing, located at three different municipalities in the region of Stockholm. Using Shier’s participation model in combination with theory of recognition we got similar results as previous research. The social workers have a great intention in enhancing user’s participation in their work, though organizational structures stand in their way. That drives them to use what is known as “silent knowledge”, a combination of their personality, previous experience and their ingenuity.

Denna vetenskapliga essä utgår från författarens personliga erfarenhet som boendestödjare i form av en gestaltning. Syftet med essän är att formulera boendestödjarens praktik utifrån gestaltningen samt med hjälp av Aristoteles Den Nikomachiska etiken samt Donald A. Schöns Educating the Reflective Practitioner reflektera över vilken kunskap som boendestödjare använder samt hur det är möjligt att lära sig densamma. Metoderna består av skrivande, fenomenologiskt tillvägagångssätt gällande gestaltningen och hermeneutiskt utforskande. I essän presenteras en personlig och historisk bakgrund samt ett forskningsurval rörande boendestöd. Därefter diskuterar författaren, utifrån Aristoteles teorier, kring syftet med boendestöd och de kunskapsformer som återfinns i den inledande gestaltningen. Författaren diskuterar vidare hur boendestödjaren lär sig dessa kunskaper med utgångspunkt ur Schöns teorier. Därefter diskuteras instruktioner i ett annat sammanhang, utifrån den konceptuella konstnären Sol LeWitt. Essän avslutas med att författaren bland annat konstaterar att den praktiska kunskapen är centrala inom boendestödsarbetet även om instruktioner kan uppmärksamma författaren om medelvägen. / This scientific essay starts with a literary composition from the author’s personal experience as a community mental health worker. The purpose of the essay is to articulate the practice of the community mental health worker starting with the literary composition and by utilizing Aristotle The Nicomachean Ethics and Donald A. Schön Educating the Reflective Practitioner reflect over what type of knowledge the mental health worker uses and how it is possible to attain that knowledge. The methods consist of writing in addition to a phenomenological procedure regarding the literary composition and hermeneutic exploring. A personal and historical background is presented, along with a selection of research regarding community mental health work. This is followed by the authors discussion, starting with the theory of Aristotle, regarding the purpose of community mental health work and the type of knowledge that is located in the literary composition. The author continues to discuss how the community mental health worker learns this knowledge with starting from the theory of Schön. After that instructions are discussed in another context, from the conceptual artist Sol LeWitt. The essay ends with a conclusion by the author that the practical knowledge is central within the community mental health work even if instructions can make the author attentive on the middle way.

Utilisation of information and communication technologies in public libraries at Ekurhuleni Metropolitan Municipality, South Africa

Public libraries, globally, are adopting the use of information and communication technologies (ICTs) in order to improve services to the users. ICTs in public libraries are enhancing services to the users by ensuring that services are available to them quickly and efficiently. ICTs in public libraries include computers and their Internet-based facilities, printers, photocopiers, laminators, faxes, telephones, televisions, compact discs, (CDs) and digital video discs (DVDs). This study focussed on the utilisation of ICTs in public libraries at Ekurhuleni Metropolitan Municipality (EMM). The general aim of the study was to assess the utilisation of ICTs at EMM public libraries by the users. Both qualitative and quantitative research approaches through a survey research method were deployed. The study targeted all public libraries at Ekurhuleni and within the libraries; library users and control librarians (librarians in charge of libraries at Ekurhuleni) were targeted. Control librarians participated in the study as key informants. The researcher needed to find out if they would corroborate what users said. There is a total of 43 public libraries at Ekurhuleni and eight were included in this study. These eight libraries were selected through stratified sampling. Out of 24033 registered users at Ekurhuleni libraries, 332 were randomly selected to take part in the study. All eight control librarians of participating libraries were included in the study. The total response rate for the targeted library users was 51% (170). The findings of this study indicate that although ICTs in EMM libraries are highly utilised, staff attitudes, cost of ICTs services, users’ lack of ICTs knowledge and skills, slowness of ICTs and power failure, make it difficult for users to maximise the full benefits offered by ICTs. The study recommended that the cost of ICTs services be reduced, customer care workshops and ICTs training be prioritised for staff, structured ICTs training programmes be developed for library users, users’ access time to Internet be extended, subscription to online databases be prioritised, Internet bandwidth be increased, uninterrupted power supply be installed in libraries and that positions be created for librarians specialising on ICTs. / Information Science / M.
