An assessment of the effectiveness of primary health care services in addressing HIV/AIDS by providing anti-retroviral treatment : the case of Du Noon clinic in the Western health sub-district of the city of Cape Town

Thesis (MPA (Public Management and Planning))--University of Stellenbosch, 2010. / ENGLISH ABSTRACT: The accessibility of anti-retroviral drugs to patients and families affected by HIV and AIDS, and the affordability of these drugs, have been challenges to the Du Noon community in the Cape Peninsula. The aim of the study was to assess the effectiveness of primary health care services in addressing HIV/AIDS in the light of these challenges. The focus was on patients registered on the ARV programme and who were receiving treatment at Du Noon Clinic. Interviews were conducted with 15 groups of 10 patients each using a patient questionnaire. During these interviews qualitative and quantitative data were gathered and secondary data was used for quantitative analysis. The results that the data analysis yielded are in keeping with the hypothesis that the HIV/AIDS programme is effective in meeting the needs of the HIV/AIDS patients of Du Noon. After content analysis of qualitative data, two themes related to patient satisfaction emerged: positive and negative feelings that were categorised as satisfied and not satisfied with the service. Most often noted was the feeling of satisfaction with the services rendered at the clinic and that the staff were helpful. The staff rendering the service were also satisfied with the kind of service offered to the patients, but were dissatisfied with the allocation of resources. An increase in enrolment figures of patients was noted in the statistical analysis for the period 2004-2008 with 1,018 patients registered. The statistics illustrate the linear tendency in the enrolment of patients, which indicated the accessibility and affordability of the service. / AFRIKAANSE OPSOMMING: Geredelike toegang tot en die bekostigbaarheid van anti-retrivorale middels (ARM’s) vir pasiënte en families wat deur MIV en VIGS aangetas is, is ‘n uitdaging vir die Du Noon-gemeenskap in die Kaapse Skiereiland. Die doel van die studie was om die doeltreffendheid van primêre gesondheidsorgdienste te bepaal wanneer MIV/VIGS aangespreek word. Die fokus is op geregistreerde pasiënte wat die ARM-program volg en behandeling by die Du Noon Kliniek ontvang. Met behulp van ‘n pasiëntevraelys was onderhoude met 15 groepe van 10 pasiënte elk gevoer. Tydens hierdie onderhoude is kwalitatiewe data versamel en vir kwantitatiewe analise was sekondêre data aangewend. Die resultate wat uit die data analise verkry was, strook met die hipotese dat die MIV/VIGS-program doeltreffend is om die behoeftes van die pasiënte en die gemeenskap van Du Noon aan te spreek. Nadat ‘n inhoudsanalise van die kwalitatiewe data onderneem was, het twee temas rakende positiewe en negatiewe gevoelens – gekategoriseer as tevrede en nie tevrede nie – ten opsigte van die gelewerde diens na vore getree. Veral die gevoel van tevredenheid teenoor die diens gelewer by die kliniek en die personeel as behulpsaam, is opgemerk. Die personeel wat die diens lewer, was ook tevrede met die diens wat aan die pasiënte gelewer word, maar was ontevrede oor die toekenning van hulpbronne. By die statistiese analise is ‘n toename in die inskrywingsgetalle deur pasiënte waargeneem. Toename in inskrywingsgetalle deur pasiënte is gemerk in statistiese analise van 2004 - 2008, met 1,018 pasiënte geregistreer. Die statistiek het die lineêre tendens toegelig ten aansien van die inskrywing van pasiënte wat die toeganklikheid en bekostigbaarheid van die diens uitbeeld.

Formats for storytelling by caregivers for sharing knowledge in home-based health care

Thesis submitted in fulfilment of the requirements for the degree Master of Technology: Design in the Faculty of Informatics and Design at the Cape Peninsula University of Technology, 2012 / Home-Based Health Care (HBHC) is the provision of health services and social assistance to individuals in their homes. It is an indispensable service to communities in need; presently in South Africa, these health care services are provided by local non-profit and non-governmental organizations. It is mainly targeted at poverty-stricken and under resourced communities and is an important aspect of healthcare in South Africa. A lot of communities in South Africa are still under-resourced and live in impoverished conditions. This is reflected in poor living standards characterized by unemployment, uneasy access to basic amenities, unequal distribution of wealth, poor medical facilities in rural communities, etc. The prevalence of the HIV/AIDS endemic has also worsened the situation. However, the South African government has constantly supported initiatives to enhance better healthcare in rural communities. This research presents a case study of caregivers working in Du Noon, an informal settlement in Western Cape, South Africa. Du Noon is overpopulated and home to lots of foreigners who have migrated in search of better living standards thus stretching an already over-burdened community. Naturally, this development supports poor health practices which encourage the spread of diseases within the community. Health facilities do not sufficiently cater for the ever-increasing healthcare demands and as such lots of people depend on NGOs such as SALT (Sharing Abundant Life Together) who through the help of caregivers and community workers have been providing for some of the healthcare needs of the Du Noon community since 2000. Findings reveal that although caregivers in this community have lots of stories to share about their work practices, they do not have a platform to share these stories. This research focuses on identifying processes involved in capturing these stories and moderating them into educative and entertaining visual contents for the purpose of sharing the stories within the community. Using qualitative research methods, empirical and field study provides the needed data as well as understanding of the multi-dynamism of this community. Data analysis is done through interpretive methods for the purpose of understanding the data collected and analysed during the research process. Finally, as a possible research contribution, the research concludes by examining how other technology can enhance the storytelling experience using interaction design methods that enables caregivers participate in the entire design process. Suggestions are also given as to direction for further research within the subject area.

A Study of Dvořak¡¦s Four Symphonic Poems on Erben¡¦s Ballad Kytice

In the 1890s, Antonin Dvořak created five symphonic poems, respectively The Water Goblin¡BThe Noon Witch¡BThe Golden Spinning Wheel¡BThe Wild Dove¡BA Hero's Song. The first four poems originated from Erben¡¦s prosody anthology, Dvořak present the personality of the roles by using Czech rhyme and characters features, and successfully promote the development of plot by adding the realism sound effects with poems content. According to the unique narrative poem, it presents the original by using succeed verse to verse faithfully. In addition to the introduction and conclusion in this study, it contains three main chapters. The Chapter 1 is discussed in detail for ¡§the creation course and nationality about Dvořak¡¨, ¡§Symphonic poem of the 19th century¡¨, ¡§narrative poem and ballade¡¨ and four symphonic poems according to Erben anthology respectively. Chapter 2 focuses on the careers and brainchild of Erben, and the writing concept about Kytice. The chapter 3 is base on the created years of the four symphonic poems to know the narratives of Dvořak by investigating the creative concept and the process of composition.

Path Planning Algorithms for Multiple Heterogeneous Vehicles

Unmanned aerial vehicles (UAVs) are becoming increasingly popular for surveillance in civil and military applications. Vehicles built for this purpose vary in their sensing capabilities, speed and maneuverability. It is therefore natural to assume that a team of UAVs given the mission of visiting a set of targets would include vehicles with differing capabilities. This paper addresses the problem of assigning each vehicle a sequence of targets to visit such that the mission is completed with the least "cost" possible given that the team of vehicles is heterogeneous. In order to simplify the problem the capabilities of each vehicle are modeled as cost to travel from one target to another. In other words, if a vehicle is particularly suited to visit a certain target, the cost for that vehicle to visit that target is low compared to the other vehicles in the team. After applying this simplification, the problem can be posed as an instance of the combinatorial problem called the Heterogeneous Travelling Salesman Problem (HTSP). This paper presents a transformation of a Heterogenous, Multiple Depot, Multiple Traveling Salesman Problem (HMDMTSP) into a single, Asymmetric, Traveling Salesman Problem (ATSP). As a result, algorithms available for the single salesman problem can be used to solve the HMDMTSP. To show the effectiveness of the transformation, the well known Lin-Kernighan-Helsgaun heuristic was applied to the transformed ATSP. Computational results show that good quality solutions can be obtained for the HMDMTSP relatively fast. Additional complications to the sequencing problem come in the form of precedence constraints which prescribe a partial order in which nodes must be visited. In this context the sequencing problem was studied seperately using the Linear Program (LP) relaxation of a Mixed Integer Linear Program (MILP) formulation of the combinatorial problem known as the "Precedence Constrained Asymmetric Travelling Salesman Problem" (PCATSP).

Recognizing that the late period of Nietzsche's philosophy is marked by the affirmative proposal of a revaluation of all values, this study is dedicated to understand the journey crossed by the character in relation to the noon as the beginning of the character s task and the great noon as a celebration of the incorporation of the eternal recurrence of the same and the final consummation of the revaluation of all values. In light of this, the first section seeks to demonstrate the Nietzschean critique of Western culture, marked by nihilism and death of God such as it appears in the Twilight of the Idols under the heading How the 'true world' finally became the fable . Following this thought, we seek to understand the noon time while the total elimination of dualities and also the man's reconciliation with the nature. From this point the task of Zarathustra begins. Then, analyzing the works of the second period of Nietzsche's philosophy, the following section reconstructs the ground from which Zarathustra and the noon s philosophy develop themselves. This undertaking is intended to make known the character and his task from the context of the revaluation of all the values. Finally, in the third and final section, after accompanying the trajectory of Zarathustra along the prologue of the homonymous work to get the event of noon, the research follows the character in the path of his going under [Untergang], showing thus its incorporation of the eternal recurrence and the consolidation of the unconditional affirmation of the amor fati existence. When dealing with Zarathustra maturation, that allows him from towards the celebration of the great noon, this dissertation ends by calling attention to the role that the disciples of Zoroaster, while philosophers of the future, play in the realization of the revaluation of all the values. / Trata-se de uma investigação acerca do papel desempenhado pela personagem de Zaratustra no contexto da obra madura de Nietzsche, sobretudo no que se refere ao meio-dia e grande meio-dia. Por reconhecer que o período tardio da filosofia nietzschiana é marcado pela proposta afirmativa de uma transvaloração de todos os valores, o presente estudo dedica-se a compreender o itinerário percorrido pela personagem frente ao meio-dia enquanto início da tarefa da personagem e o grande meio-dia enquanto celebração que consuma sua incorporação do eterno retorno do mesmo e a efetivação definitiva da transvaloração de todos os valores. Em vista disso, a primeira seção busca evidenciar a crítica nietzschiana à cultura ocidental, marcada pelo niilismo e a morte de Deus tal qual ela se apresenta no Crepúsculo dos ídolos sob o título Como o mundo verdadeiro finalmente se tornou fábula . Shootin Up the Past: Terministic Frontiers in Angle of Repose and High Noon

The West has long been an important geographic and symbolic space for the United States. In the 19th and 20th centuries that space became the subject of numerous popular works of fiction, first in print and later in the cinema. These texts eventually formed a specialized genre, the Western, which had its own conventions, styles, and themes. Wallace Stegner's Angle of Repose and Fred Zinnemann's High Noon, both seminal western texts from the mid-twentieth century, seek to reinterpret those conventions. While the Western is often characterized as a genre of violent masculinity and rugged individualism, these two texts employ conventional Western motifs in an effort to articulate a metafictional criticism of those ideas. Ultimately, they posit a reality in which traditional portrayals of the West lead to alienation, while also advocating an escape from that alienation.

This thesis starts from the observation that, while it is common for commentators to divide postmodern fiction into two general fields – one experimental and anti-mimetic, the other cautiously mimetic, there remains a fairly significant field of postmodern texts that use largely mimetic approaches but represent worlds that are categorically distinct from actuality. This third group is even more pronounced if popular culture and “commercial” fiction, in particular sf and fantasy, are taken into account. Additionally, the third category has the interesting characteristic that the texts within this group very often generate unusual loyalty among its fans. Based on a renewed investigation of the main genre critics in postmodern fiction, the first chapter suggests a tripartite division of postmodern fiction, into formalist, metamimetic, and transreferetial texts. These are provisionally circumscribed by their reference worlds: formalist fiction attempts to derail its own capacity for presenting a world; metamimetic fiction presents mediated versions of worlds closely reminiscent of actuality; and transreferential fiction sets its narrative in worlds that are experienced as such, but are clearly distinct from actuality. If transreferential fiction deals with alternate worlds, it also very often relies on the reader’s immersion in the fictional world to provide unique, often subversive, fictional experiences. This process can be identified as the exploration of the fictional world, and it is very often guided so as to be experienced as a virtual reality of sorts. If transreferential texts are experienced as interactive in this sense, it is likely that they convey experiences and insights in ways different from either of the other two strands of postmodern fiction. In order to investigate the interactive experience provided by these texts, an extended conceptual and analytical set is proposed, rooted primarily in Ricoeurian hermeutics and possible-worlds theory. These two main theoretical approaches approximately correspond to the temporal and the spatial dimensions of texts, respectively. Much of the power of these texts rooted in the care they take to guide the reader through their fictional worlds and the experiences offered by the narrative, often at the hand of fictioninternal ‘guides’. These theoretical approaches are supplement by sf theoretical research and by Aleid Fokkema’s study of postmodern character. Chapters 3, 4, and 5 apply the theoretical toolset to three paradigmatic transreferential texts: sf New Wave author M John Harrison’s Viriconium sequence; Gibson’s Sprawl trilogy; and Jeff Noon’s Vurt and Pollen, texts that have much in common with cyberpunk but which make much more extensive use of formalist techniques. Each chapter has a slightly different main focus, matching the text in question, respectively: aesthetic parameters and worldcreation strategies of transreferential fiction; close “guidance” of the reader and extrapolation; and virtual reality and identity games. The final chapter presents the findings from the research conducted in the initial study. The findings stem from the central insight that transreferential texts deploy a powerful suit of mimetic strategies to maximise immersion, but simultaneously introduce a variety of interactive strategies. Transreferential fiction balances immersion against interactivity, often by selectively maximising the mimesis of some elements while allowing others to be presented through formalist strategies, which requires a reading mode that is simultaneously immersive and open to challenging propositions. A significant implication of this for critical studies – both literary and sf – is that the Barthesian formalist reading model is insufficient to deal with transreferential texts. Rather, texts like these demand a layered reading approach which facilitates immersion on a first reading and supplements it critically on a second. This thesis deals with George Soros foundation network, specifically with the Soros centers for contemporary art, their support of the art in post-soviet countries and the following transformation. One of the important tasks was support for artists in the form of grants. Attention is focused on one specific case and that is the gallery Atelier NOON, which was based in Prague in Smichov in 1999 - 2002. It was an unconventional combination of the art studio, that belong to the artist Petra Nováková Ondreičková, and exhibition space. The gallery is shown in context of contemporary art scene from those times, and it is also trying to show similar unconventional exhibition spaces. La représentation de la collectivité dans la mise en acte du chant choral sénégalais chez les Sérères noon de Saint Pierre Julien Eymard de Koudiadiène

Portant sur les modalités d’appropriation de la pratique chorale occidentale en milieu catholique chez les Sérères noon de Saint Pierre Julien Eymard de Koudiadiène, au Sénégal, ce mémoire tente de voir pourquoi, et surtout comment les différents syncrétismes agissent sur divers symbolismes sénégalais. Il vise plus particulièrement à comprendre toute l’importance de l’action de la collectivité dans la mise en acte du répertoire choral, et à comprendre comment l’improvisation spontanée en ensemble se construit en situation de performance dans ce qui est appelé ici la plurivocalité linéaire. Ce mémoire vise dans cette foulée à décrypter le sens autour de cette pratique spécifique du chant choral et à cerner comment le processus d’acculturation agit sur l’identité culturelle des Sérères noon. Finalement, il vise à dégager les affects en situation de performance de la chorale qui semble détenir un rôle central pour les membres de la paroisse Saint Pierre Julien Eymard de Koudiadiène, et à comprendre et mettre en valeur une pratique musicale et son contexte d’insertion social jusqu’alors peu étudiés de façon systématique. / On the modalities of appropriation of Western choral practice among Catholic Sérères noon of Saint Pierre Julien Eymard of Koudiadiène, Senegal, this dissertation tries to demonstrate why and how different syncretisms act on various Senegalese symbolisms. It aims to understand more particularly the importance of the community in the mise en acte of the choral repertoire, and how spontaneous improvisation is built in performance situation in what is called here the plurivocalité linéaire. This dissertation is in this vein to decipher the meaning of that specific practice of choral singing and to point how the acculturation process acts on the cultural identity of Sérères noon. Finally, it aims to identify affects in performance situation of the choral that seems to have a central role for members of the parish of Saint Pierre Julien Eymard, and understand and enhance a musical practice and its social context of integration hitherto little studied systematically.
