Performing school nursing : narratives of providing support to children and young people

Background: Child and adolescent mental health is an important public health issue within the UK. Providing support to young people, to help them cope with everyday life, is a key aspect of the school nurse’s role. Yet there is a paucity of published research within the UK and internationally about how this support is provided. Methodology: Using a narrative inquiry approach, presented as a performative text, this study set out to address the following research question, ‘How do school nurses provide support to young people?’ Stories were gathered from eleven school nurses to explore their experiences of providing support to young people using purposive sampling. The stories were analysed using an adapted version of the interpretivist-interactionist model (Savin-Baden, 2004). Poetic re-presentations were used to tell the stories of individual school nurses; an approach seen to be a novel in school nursing research. Using Soja’s (1996) spatiality theory as a framework the stories were analysed collectively, to explore different spaces used when providing support to young people. Findings: This study extends school nursing current literature about what it means to provide support. The importance of regular support and building trusting relationships is identified. Yet challenges exist in terms of the amount of emotional investment required by the nurses, as well as a lack of workforce capacity and organisational demands. It provides an original contribution to the body of school nursing knowledge by using an approach new in school nursing research, and distinguishing different and new spaces in which they perform to provide support to young people. Recommendations: Further research is necessary to gather stories from young people themselves. Additional support and training is recommended to enhance school nurses’ knowledge and skills in providing support. Findings should be conveyed to commissioners to provide insight into the school nurses’ role.

Celiaki – en social sjukdom som kräver stöd från sjuksköterskan : En allmän litteraturstudie / Celiac disease- a social disease that requires support from the nurse : A general literature study

Bakgrund: Celiaki är en autoimmun sjukdom som påverkar två delar av tunntarmen. En autoimmun reaktion påbörjas vid intag av gluten (gliadin) som främst finns i sädesslagen vete, korn och råg. De sociala situationer som personer med Celiaki upplever är komplexa. Sjuksköterskans stöd till personer med Celiaki är viktigt för att bidra till en ökad livskvalité och kan även stödja till att bibehålla, främja och återfå hälsa. Syfte: Var att belysa personer med Celiakis behov av stöd ur ett omvårdnadsperspektiv. Metod: Studien genomfördes som en allmän litteratur studie och åtta resultatartiklar framkom. Resultatartiklarna granskades, bearbetades och sammanställdes till tre teman. Resultat: De teman som framkom var: Sjuksköterskans stöd- Att vara en del av ett sammanhang, Sjuksköterskans roll- Att dela erfarenheter, Sjuksköterskans betydelse- Att ge stöd för personer med Celiaki. Det framkom från resultatet att personer med Celiaki påverkades både psykiskt och socialt och därmed i behov av stöd. Konklusion: Från resultatet framkom det att personer med Celiaki är i behov av stöd. En okunskap om sjukdomen från allmänheten påverkar personer med Celiaki i sociala sammahang. / Background: Celiac disease is an autoimmune disease that affects two parts of the small intestine. An autoimmune reaction is initiated by consuming gluten (gliadin) which is mainly found in wheat, barley and rye. The social situation that people with Celiac disease experience is complex. The nurse's support for people with Celiac disease is important to contribute to an increased quality of life and can also help maintain, promote and regain health. Purpose: To highlight people with Celiac disease's need for support from a nursing perspective. Method: The study is conducted as a general literature study and eight result articles appeared. The eight results articles was reviewed, processed and compiled into three themes. Results: The themes that emerged was: The nurse's support- To be part of a context, The nurse's role- To share experiences, The nurse's importance of providing support for people with Celiac disease. It emerged from the results that people with Celiac disease were affected both mentally and socially and thus are in need of support. Conclusion: The results showed that people with Celiac disease are in need of support. The lack of knowledge by the general public of the disease affects people with Celiac disease in the social context

Unga kvinnors uppelvelser av att få bröstcancerdiagnos : En litteraturöversikt

Bakgrund: Bröstcancer är vanligaste typen av cancer bland kvinnor i världen samt orsakar flest dödsfall av cancerformerna. Drabbas av bröstcancer har väsentlig påverkan på unga kvinnors liv, behandling kräver specifik kompetens gällande sexualitet, fertilitet samt graviditet. Antonovsky beskriver att ”KASAM”- känsla av sammanhang, bidrar till ökad förmåga att hantera svårigheter som det kan innebära att få diagnos och hantera sjukdom som bröstcancer. Hög grad av KASAM kan hjälpa kvinnor att hantera och acceptera livssituationen.Syftet är att beskriva unga kvinnors upplevelser av att få bröstcancerdiagnos i samband med diagnostik och behandling. Metod: Litteraturöversikt med induktiv ansats baserad på 12 vetenskapliga kvalitativa artiklar som analyserades enligt Fribergs (2017) fem steg. För artikelsökningen användes databaserna Cinahl och MedLine. Resultat bestod av tre huvudkategorier och åtta underkategorier som sammanställdes.Huvudkategorierna är att få bröstcancerdiagnos (upplevda känslor, återgå till vardagen, acceptans och balans), förändrad kropp (kroppsuppfattning och identitet, sexuell hälsa, fertilitet och graviditet) och trygghet genom stöd (närståendes stöd, sjuksköterskors stöd). Slutsats: Enligt denna litteraturöversikt blev slutsatsen att bröstcancer påverkar den unga kvinnans känsloliv, vardag, kroppsuppfattning, identitet, sexuell hälsa, fertilitet samt graviditet. Närstående och sjuksköterskor kan vara det huvudsakliga stödet och hjälpa kvinnor att bekämpa bröstcancer. / Title: Young women's experiences of being diagnosed with breast cancer. A literature review.   Background: Breast cancer is most common type of cancer among women in the world and causes the most deaths from the cancers. Suffering from breast cancer has a significant impact on the lives of young women. Young women's treatment requires specific skills regarding sexuality, fertility and pregnancy. Antonovsky describes that "SOC" - sense of coherence, contributes to an increased ability to deal with difficulties that it may mean to be diagnosed and manage a disease as breast cancer. Strong SOC can help women manage and accept the life situation. The aim is to describe young women's experiences of being diagnosed with breast cancer associated with diagnosis and treatment. Method: Literature review with inductive approach based on 12 qualitative articles that were analyzed with Friberg's (2017) five steps. Cinahl and MedLine were used for the article search. The result consists of three head categories and eight subcategories. The main categories are getting breast cancer diagnosis (perceived emotions, return to everyday life, acceptance and balance), changed body (body perception and identity, sexual health, fertility and pregnancy) and security through support (relatives 'support, nurses' support). Conclusion: According to this literature review, the conclusion was that breast cancer affects young woman's emotional life, everyday life, body image, identity, sexual health, fertility and pregnancy. Close relative and nurses can be the main support and help women fight breast cancer.

Sjuksköterskans stöd till personer med psykiska funktionsnedsättningar i utövandet av fysisk aktivitet och dess hälsoeffekter Litteraturstudie / The nurse’s support to people with mental disabilities in the practice of physical activity and health effects A literature review

Det har forskats mycket om fysisk aktivitets positiva effekter på människokroppen rent fysiskt. Tidigare har det inte varit en naturlig del av behandlingen inom psykiatrin vilket gjorde författarna nyfikna på hur det kan lyftas fram i vården av personer med psykiska funktionsnedsättningar. Syfte: Syftet med litteraturstudien var att beskriva hälsoeffekterna av fysisk aktivitet för personer med psykiska funktionsnedsättningar och sjuksköterskans stöd genom handledning. Metod: Litteraturstudie. Resultat: Under sammanställningen av resultatet framkom att sjuksköterskor kan ge stöd genom Motiverande samtal (MI) som var av central betydelse. Då patienten själv hörsammas och bygger upp sina mål som ska vara mätbara tillsammans med utbildad vårdpersonal, men även genom stöd i patienternas egen utbildning och kunskap av den fysiska aktivitetens positiva inverkan på psykisk ohälsa. Slutsats: Sjuksköterskans metod att stödja och motivera personer med psykiska funktionsnedsättningar till att utöva fysisk aktivitet visade sig vara flera. Då det var viktigt med personcentrarad metod, var viktigt att de behärskade och kände till olika vägar att kunna motivera personer med psykiska funktionsnedsättningar. Andra betydelsefulla tillvägagångssätt var god vårdrelation, individualiserade träningsprogram, samt stöd och utbildning i de positiva hälsoeffekterna som fysisk aktivitet gav. / It has been researched a lot of physical activity beneficial effects on the human body physically. Previously, there has been a natural part of treatment in psychiatry, which made the authors curious about how it can be highlighted in the care of people with mental disabilities. Aim: The purpose of this study was to describe the health benefits of physical activity for people with mental disabilities and nurse support through tutoring. Methodology: During the compilation of the results revealed that nurses can provide support through the Motivational Interviewing (MI) was of central importance. When the patient heard and build up their goals to be measurable with trained medical staff, but also through support of patient self-education and knowledge of the physical activity's positive impact on mental health. Conclusion: The nurse's approach to support and motivate people with mental disabilities to exercise physical activity was found to be several. Then it was important to the ruled and knew the different ways to motivate people with mental disabilities. Other important approaches were good care relationship, individualized exercise programs, and support and training in the health benefits that physical activity gave.

Examining the perceived nurses' support for self management among type 2 diabetes mellitus patients in Botswana

Patients’ perceptions of health care and support is a key determinant in self-efficacy and active participation in the management of chronic conditions. Nurses play a significant role in Diabetes Self-Management Education by providing clients with tools, empowerment and knowledge to self-manage their condition. The purpose of this study was to examine and describe diabetes patients’ perceptions of nurses’ support for self-management in Botswana. The aim was to improve clients’ skills in self-management and to strengthen diabetes health care management. This study was carried out at the Diabetes Clinic of Excellence, in the city of Francistown, the second largest city in Botswana. The study population comprised of Type 2 Diabetes patients registered at the Diabetes Clinic for their follow-up. All were aged 18 years and above. Three hundred and fifty-four (354) patients participated in the study. The study used a non-experimental, descriptive, quantitative design. Probability sampling method was used to recruit diabetes patients from the selected clinic. Data were were collected using a structured, researcher developed, questionnaire mostly in face-to-face interviews, a few participants completed the questionnaire. The Quantitative data analysis included descriptive and inferential statistics using SPSS (Statistical Package for Social Science Software (version 25). Spearman rho was used to determine statistical correlation between patients’ perceptions and their self-management practices. The findings showed that generally, patients’ perceptions of professional support was positive regarding most of the constructs measured. However, there were areas that showed less satisfaction with the support such as foot-care, risk control, and ability to identify signs of low and high blood sugar level and carrying of Identification Band (ID). Perceptions of nurses’ motivational behaviour showed varied responses. Patients’ self-care activities were sub-optimal and showed some variations which tended to correspond with their perceptions of professionals support. The Spearman's correlation results ranged from strong, moderate, and weak positive correlation. A few demographic variables showed some impact on self-care activities. Based on the findings, the study concludes that professional support through DSME and DSMS, self-management and patients’ perceptions of care play a significant role in diabetes management. / Health Studies / M.A. (Nursing Science)
