O método é a pesquisaação, apoiada em encontros denominados journalclub, ou clube de leitura, meio empregado para a coleta de dados com a participação de enfermeiros em posição gerencial. Tem a finalidade de fomentar discussões e sensibilização para pesquisa científica em um processo educativo sobre gerenciamento baseado em evidências. A técnica de grupo focal foi utilizada para apreender, na perspectiva dos participantes, o significado do processo educativo e as possibilidades de transformação da prática gerencial. Os encontros foram gravados e os discursos transcritos e codificados com ajuda do NVivo9 software© QRS International. Os dados foram analisados considerando o diagrama proposto para a pesquisa-ação: Planejamento, Desenvolvimento, Descrição e Avaliação do Processo Educativo adotando-se a análise de conteúdo conforme referencial de Graneheim e Lundman. Os resultados revelaram os temas que interferem no Gerenciamento Baseado em Evidências. No tema, Aspectos referentes à organização, os subtemas foram: planejamento, processo de trabalho, estrutura e gestão de pessoas. No tema, Aspectos referentes à equipe, foram desvelados os subtemas: autonomia, comportamento/atitude, sensibilização e problemas sociais. Este estudo mostrou a abrangência de temas que o clube de leitura como estratégia pode abordar em relação ao gerenciamento e o quanto o embasamento científico é importante no cotidiano dos gerentes de enfermagem para melhorar a qualidade da assistência e a capacidade do gerenciamento baseado em evidências. / ABSTRACT The purpose of this project is to develop a learning environment to discuss the issues based on management concept of hospital device management systems. The method is an action research, through a strategy of meetings called Journal Club, a methodology used for data collection with a participation of nurses in managerial position, aiming to discussions and sensitization for scientific research in an educational process about evidence-based management. The focus group technique was used to understand from the perspective of the participants the meaning of the educational process and the possibilities of transformation of the managerial practice. The meetings were recorded and the speeches were transcribed and decoded with the help of NVivo 9 QRS International software. The data were analyzed considering the proposed diagram for action research: Planning, Development, Delimitation and Evaluation of the Educational Process, adopting a content analysis according to Graneheim and Lundman. The results revealed the issues that interfere with evidence-based management. In the theme, Aspects related to the organization, the sub-themes were: planning, work process, structure and people management. The theme, Aspects referring to the team, revealed the sub-themes: autonomy, behavior / attitude, sensitization and social problems. This study showed the scope of topics that the reading club as a strategy can address in relation to management and how much the scientific background is important in the daily routine of nursing managers to improve the quality of care and the capacity of evidence-based management.

Ensino clínico sob o paradígma da prática embasada em evidências: integração da teoria e prática para a prevenção da úlcera por pressão / Clinical teaching under the evidence-based practice paradigm: integration between theory and practice for pressure ulcer prevention

A úlcera por pressão, considerada um evento adverso, compromete a segurança do paciente sob os cuidados dos profissionais de saúde. Uma das formas de proporcionar segurança é por meio da prática baseada em evidência que pode ser implementada com estudantes pelo uso de metodologias ativas de ensino- aprendizagem. O objetivo geral do estudo foi propor uma metodologia de ensino para estudantes do curso de Enfermagem da Universidade Estadual de Londrina, pela avaliação do risco de desenvolvimento da úlcera por pressão em pacientes internados em uma unidade médico-cirúrgica e pela identificação das prescrições de enfermagem para a prevenção nos registros em prontuários. A pesquisa foi aprovada pelo Comitê de Ética e Pesquisa institucional e realizou-se utilizando métodos quantitativos e qualitativos, com delineamento descritivo, ocorrendo em três fases. A primeira foi um workshop sobre prática baseada em evidência. A segunda foi desenvolvida pelos estudantes e pesquisadora e a coleta de dados foi realizada com 21 pacientes hospitalizados em unidade médico-cirúrgica durante 15 dias sequenciais, em que avaliou-se o risco para úlcera por pressão por meio da Escala de Braden e os registros das prescrições de enfermagem relacionados ao uso das evidências para prevenção de úlcera por pressão. A terceira foi desenvolvida com os estudantes para investigar o significado da experiência com a metodologia de ensino. Os 21 pacientes avaliados tinham a média de idade de 44,76 anos e diagnósticos médicos mais frequentes de doenças do sistema osteomuscular e do tecido conjuntivo (42,86%). No primeiro dia de avaliação, considerando a escore total da Escala de Braden, 17 pacientes (80,95%) tinham risco entre moderado e muito alto. Na inspeção da pele foram identificadas 25 úlceras por pressão, sendo a localização mais frequente na região sacral (44%) e as classificações predominantes foram no estágio I e II. Na análise dos prontuários, não foram identificadas anotações dos enfermeiros referentes à avaliação do risco e nem ao uso da Escala de Braden. As prescrições de enfermagem mais frequentes para a prevenção da úlcera foram mudança de decúbito, uso de coxins, proteção contra a umidade e uso de cremes hidratantes, e a menos frequente foi a manutenção da cabeceira até 30 graus. Não foi encontrada associação entre as medidas de prevenção prescritas e os subescores da escala de Braden. O significado das experiências de oito estudantes que participaram da pesquisa foi investigado pela análise de conteúdo das falas obtidas nas discussões em grupo e identificaram-se duas categorias: aprendendo sobre o tema úlcera por pressão e conhecendo a realidade da instituição e da enfermagem sobre a úlcera por pressão. Concluiu-se que os estudantes relacionaram a teoria à prática, identificaram as competências do enfermeiro na avaliação do paciente com risco para úlcera por pressão, aprimoraram o conhecimento nos estágios da úlcera por pressão e identificaram a Escala de Braden como suporte para avaliação de risco para a úlcera por pressão. Os estudantes, ainda, identificaram a falta de cuidados pela equipe de enfermagem e a importância da presença da família e perceberam a necessidade da prática segura em relação à úlcera por pressão / Pressure ulcers, considered an adverse event, compromise the safety of patients under the care of health professionals. One way to provide safety is through evidence-based practice, which can be implemented with students through the use of active teaching-learning methods. The general study objective was to propose a teaching method for students from the Nursing program at the Universidade Estadual de Londrina, to assess the risk of pressure ulcer development in patients hospitalized at a medical-surgical unit and to identify the nursing prescriptions for this prevention in the records in patient files. Approval for the research was obtained from the institutional Research Ethics Committee and quantitative and qualitative methods were applied, with a descriptive design, involving three phases. The first was a workshop on evidence-based practice. The second was developed by the students and researcher and the data were collected with 21 patients, hospitalized at the medical-surgical unit for 15 continuous days, in which the risk of pressure ulcer was assessed through the Braden Scale and the records of the nursing prescriptions related to the use of evidences to prevent pressure ulcers. The third was developed with the students to investigate the meaning of the experience with the teaching method. The 21 patients assessed had a mean age of 44.76 years and the most frequent medical diagnoses were diseases of the musculoskeletal system and connective tissue (42.86%). On the first assessment day, considering the total score on the Braden Scale, 17 patients (80.95%) had a moderate to very high risk. In the skin inspection, 25 pressure ulcers were identified, with the sacral region as the most frequent location (44%) and stages I and II as the predominant classifications. In the analysis of the files, no nursing notes were identified with regard to the risk assessment, nor related to the use of the Braden Scale. The most frequent nursing prescriptions to prevent ulcers were decubitus change, use of cushions, protection against humidity and use of hydrating creams, and the least frequent was the maintenance of the headrest at 30 degrees. No association was found between the prescribed prevention measures and the subscores of the Braden scale. The meaning of the experience of the eight students who participated in the research was investigated by means of the content analysis of the statements obtained in the group discussions and two categories were identified: learning about the theme pressure ulcer and getting to know the reality of the institution and nursing with regard to pressure ulcer. In conclusion, the students related the theory with the practice, identified the nurses\' competences in the assessment of patients with risk of pressure ulcers, improved the knowledge on the pressure ulcer stages and identified the Braden Scale to support the risk assessment for pressure ulcer. The students also identified the lack of care by the nursing team and the importance of the family\'s presence and perceived the need for safe practice with regard to pressure ulcers

Trata-se de estudo de abordagem qualitativa, do tipo exploratório e descritivo, que possuí como fio condutor a complexidade de Edgar Morin. Os participantes do estudo foram sete enfermeiros docentes do curso de graduação em Enfermagem do Centro Universitário Franciscano, localizado na região central do Rio Grande do Sul, os quais participam do Grupo de Estudos e Pesquisa em Empreendedorismo Social da Enfermagem e Saúde da referida instituição. A coleta de dados, realizada no mês de novembro de 2011, baseou-se na técnica de grupo focal, a partir de três encontros, nos quais foram desenvolvidos momentos de sensibilização, de leitura (textos selecionados), de reflexão e discussão conjunta e de fechamentos/acordos. Cada encontro foi norteado por temas específicos. No primeiro encontro foi apresentado um texto sobre a Complexidade, para maior aproximação com o tema, seguido de uma discussão a respeito do cuidado de enfermagem. No segundo encontro foram apresentados e discutidos os sete princípios da Complexidade de Edgar Morin. E, no terceiro encontro discutiu-se acerca do cuidado do enfermeiro e suas especificidades em relação ao cuidado dos demais profissionais. Para a análise dos dados utilizou-se a análise textual discursiva, da qual emergiram duas grandes categorias: cuidado de enfermagem complexo como construção singular; divergências e convergências entre o cuidado do enfermeiro e o cuidado dos demais profissionais, seguidas de suas respectivas subcategorias. Evidenciou-se, que o cuidado de enfermagem complexo como construção singular vai além de um cuidado técnico-prescritivo, pontual e linear e que o enfermeiro desenvolve um cuidado ampliado, diferentemente do cuidado dos demais profissionais, além de ser ele o articulador e integrador do cuidado. Conclui-se, que o cuidado de enfermagem não pode ser concebido como ação reducionista, mas como construção singular, que envolve interações, reflexões e autoconhecimento, além do enfermeiro precisar ser capaz de desenvolver um cuidado ampliado, contextualizado e focado nas necessidades dos seres humanos. / This study aimed at knowing what nursing care means to nurse educators and what differentiates nurse’s care from other professionals’ care, in the perspective of complexity. This is a qualitative, exploratory and descriptive study, which has Edgar Morin’s complexity paradigm as its guiding principle. The participants of the study were seven nurse educators from the undergraduate course in Nursing of Franciscan University Center, located in central Rio Grande do Sul. They are members of the Group of Study and Research in Social Entrepreneurship of Nursing and Health of the aforementioned institution. Data collection, carried out in November 2011, was based in the focus group technique. It was accomplished in three meetings, which created moments for awareness, reading (of selected texts), group reflection and discussion, and conclusions/agreements. Each meeting was guided by particular topics. In the first meeting a text about Complexity was introduced, for a better awareness of the topic, and followed by a discussion about nursing care. In the second meeting the seven principles of Edgar Morin’s Complexity Theory were introduced and discussed. Finally, the third meeting discussed the nurse’s care and its specificities in relation to other professionals’ care. Discursive text analysis was used for data analysis, and two big categories emerged: complex nursing care as a unique construction, and divergences and convergences between nurse’s care and other professional’s care, followed by their respective sub-categories. Results evinced that nursing care as unique construction goes beyond a technical-prescriptive, punctual and linear care. It was also made clear that nurses develop an expanded care that is different from the punctual care of other professionals – moreover, nurses are the articulators and integrators of this care. The study concludes that nursing care cannot be conceived as a reductionist action, but as a unique construction, which involves interactions, reflections and self-knowledge, besides the fact that nurses need to be capable of developing an expanded care which is contextualized and focused on the needs of human beings. / Este estudio ha objetivado conocer el significado del cuidado de enfermería para enfermeros docentes y conocer lo que diferencia el cuidado del enfermero del cuidado de los demás profesionales, en la perspectiva de la complejidad. Se trata de un estudio cualitativo, exploratorio y descriptivo, que tiene como guía la complejidad de Edgar Morin. Los participantes del estudio fueron siete enfermeros docentes del curso de graduación en Enfermería del Centro Universitario Franciscano, ubicado en la región central del Rio Grande do Sul, los cuales participan del grupo de estudio e investigación en Emprendimiento Social de la Enfermería y Salud de la institución. La recolecta de los datos, fue realizada en el mes de noviembre de 2011, se basó en la técnica de grupos focales, a partir de tres encuentros, en los cuales fueron desarrollados momentos de sensibilización, de lectura (textos seleccionados), de reflexión y debate y de cierres/acuerdos. Cada encuentro fue basado por temas específicos. En el primer encuentro fue presentado un texto sobre la complejidad, para mayor aproximación con el tema, seguido de una discusión sobre el cuidado en enfermería. En la según encuentro fueron presentados y discutidos los siete principios de la complejidad de Edgar Morin. Y en el tercer encuentro se discutió sobre el cuidado del enfermero y sus especificidades en relación al cuidado de los demás profesionales. Para el análisis de los datos se utilizó el análisis textual discursivo, donde surgieron dos grandes categorías: cuidado de enfermería complejo como construcción singular; divergencias y convergencias entre el cuidado del enfermero y el cuidado de los demás profesionales, seguidas de sus respectivas subcategorías. Se evidenció, que el cuidado de enfermería complejo como construcción singular va allende un cuidado técnico prescriptivo, puntual y lineal y que el enfermero desarrolla un cuidado amplio, distinto del cuidado de los demás profesionales, siendo el enfermero el articulador e integrador del cuidado. Se concluye que el cuidado de enfermería no puede ser concebido como acción reduccionista, más como construcción singular, que implica interacciones, reflexiones y conocimiento propio, allende el enfermero tener capacidad de desarrollar un cuidado amplio, contextualizado y centrado en las necesidades de los seres humanos.

Utilizou-se como método de investigação a abordagem qualitativa optando pela fenomenologia, embasado no referencial teórico filosófico de Martin Heidegger. A pesquisa foi realizada de setembro de 2017 a março de 2018, com 10 mães de filhos acometidos por Acidente Vascular Encefálico devido a Doença Falciforme, e que fazem parte da Associação de Pessoas com Doença Falciforme de uma cidade da Zona da Mata Mineira. Para alcançar o objetivo do estudo foi utilizada uma entrevista fenomenológica, por meio da técnica de entrevista aberta e, para a captação das participantes utilizamos a técnica de snowball sampling (“bola de neve”). Ao indagarmos o ente, imerso na sua cotidianidade, fomos construindo as unidades de significação, nas quais vieram à tona significados comuns e diferentes da vivência destas mães frente aos questionamentos que eram apresentados. A compreensão vaga e mediana permitiu a elaboração do fio condutor indo em direção a hermenêutica. Para a mulher o vivido do cuidado com o filho significou relembrar o momento da revelação do diagnóstico da doença, o que lhes causou grande impacto. Sua participação como principal cuidadora exigiu tempo, dedicação e predisposição para prestar o cuidado, o que gerou cansaço. A fé em Deus deu-lhes força para que continuassem a cuidar dos filhos e se mostrou uma estratégia de enfrentamento dos problemas que surgiram, em que Deus não permite que falte nada pois tudo vem Dele, sem o qual não conseguem viver. As sequelas do Acidente Vascular Encefálico foram motivos de tristeza, sofrimento e dificuldade em aceitar e lidar com esta nova realidade. Relataram contar com o apoio da família e dos amigos nos cuidados com o filho, e no enfrentamento desta situação. Nesse vivido apareceu o relato de terem buscado educar os filhos de tal modo a terem uma vida mais próxima do normal possível. / The objective of this study was to reveal the lived experience of mothers whose chil- dren were affected by stroke due to sickle cell disease. The qualitative approach opting for phenomenology, based on the theoretical philosophical reference of Martin Heide- gger, was used as research method. The research was carried out from September 2017 to March 2018, with 10 mothers of children suffering from SCD due to Sickle Cell Disease, and who are part of the Association of People with Sickle Cell Disease in a city in the Zona da Mata Mineira. In order to reach the objective of the study, a pheno- menological interview was used, through the technique of open interview, and for the capture of the participants we used the technique of snowball sampling ("snowball"). When we inquired about the entity, immersed in its daily life, we were constructing the units of signification, in which common and different meanings of the experience of these mothers came to light on the questions that were presented. The vague and median understanding allowed the elaboration of the guiding thread towards herme- neutics. For the woman, the experience of caring for the child meant reminding the moment when the diagnosis of the disease was revealed, which caused them great impact. Her participation as the main caregiver required time, dedication and predis- position to provide care, which generated fatigue. Faith in God gave them strength to continue to care for their children and was a strategy to deal with the problems that have arisen, in which God does not allow anything to be lacking because everything comes from Him, without which they can not live. The sequelae of the Stroke were reasons for sadness, suffering and difficulty in accepting and dealing with this new reality. They reported having the support of family and friends in caring for their child, and in coping with this situation. In this lived appeared the report of having sought to educate the children in such a way to have a life closer to the normal possible.

A model of facilitative communication for support of general hospital nurses, nursing mentally ill people

D.Cur. / The impressive growth in the extent and range of psychiatric services provided by general hospitals in South Africa, creates stress among nurses employed in these settings which manifests its self in the negative attitudes displayed towards mentally ill people, refusal of dual diagnosis patients transferred from medical surgical units and poor intra-institutional relationships between nurses in medical-surgical units and those who are in the psychiatric unit. This has led to the research study aimed at describing a model for support to assist general hospital nurses mobilize appropriate resources in the process of nursing mentally ill people. The research methodology followed the research model in nursing proposed by Botes (1995). A theory generative, qualitative, explorative, descriptive and contextual design was followed. The research methods were dealt with in four steps of theory generation in the following manner: Step 1: Concept analysis: This step was dealt with in two phases which are concept identification and concept definition. During concept identification, a qualitative research strategy which is explorative, descriptive and contextual was used. This was attained through field research conducted in an urban general hospital. A sample of twelve professional was selected from a population of 800 professional nurses employed in a general hospital using a purposive sampling technique. This sample size was determined by saturation of data in themes. Both semi-structured individual phenomenological interviews and observations were used as methods of data collection. The field work was conducted without any preset theoretical framework of reference by using "bracketing" and "intuiting". Giorgi's (1986) method of descriptive data analysis was used. After data analysis, the results were reflected within the Nursing for the Whole Person Theory. Four themes emerged from the results of the study which were:1) the experience with the perception of the mentally ill people, 2) the experience with interpersonal communication patterns, 3) nurses' experience of violence and lastly, 4) the experience of inappropriate patient behaviours. It became clear that the experience of nursing mentally ill people was negative and affected the social, psychological and the physical dimensions of nurses. The results were validated through literature control. The major concept of model was identified as "facilitation of communication". The concept was analysed thoroughly by looking at the dictionary and subject usage. The defining attributes were identified and synthesised through a definition. The other related concepts were identified and classified using a survey list of Dickoff, James & Wiedenbach (1968:430). Step 2: Step 2 dealt with the creation of interrelationship statements between concepts identified in step 1, so that concepts were able to stand in relation to one another. Step 3: dealt with the description of the model using strategies proposed Chin & Kramer (1991). Step 4: dealt with the description of guidelines for model operationalization in practice, education and research. The evaluation of model operationalization will be carried out in future research. M.Cur. (Psychiatric Nursing) / A diagnosis of cancer causes certain fears to arise in a patient, as well as among his family, activating intense emotional responses in everyone concerned. Examples of such emotions are anger. self-reproach, gUilt, heartache, helplessness and mourning. An acceptance of the circumstances does not necessarily occur, and the phases of the mourning process may be continually interchanging. As a result of all these emotions experienced by the patient with cancer as well as his family, mental discomfort is likely to occur. The family has to adapt its usual mechanism for handling internal discomfort, in order to include the pressure and demands to which they will be exposed as a result of the cancer and treatment thereof. Family cohesion will not necessarily be advanced as a result of this heightened pressure, and additional pressure may be placed on relationships within the family. Members of a family often find it difficult to discuss openly their fears, worries and feelings. for the simple reason that they are scared of becoming upset themselves, or alternately upsetting the rest of the family. Holding themselves back in this way may lead to a feeling of being isolated, which in turn leads to feelings of uncertainty being experienced. In this way the members of a family may find themselves becoming less and less involved with each other and with the patient's treatment, all as a result of not being able to talk about their fears and other intense emotions and therefore not being able to realise all these feelings. As a result of the family's uncertainty concerning their handling of the situation, they experience a loss of control and a change in their ability to handle their situation. These factors contribute to their experience of mental discomfort. Mental discomfort is not visible to others, therefore it is very rarely treated by the multiprofessional team. If the members of the team notice the existence of such discomfort, they will still emphasise the medical treatment, since they themselves may be uncertain as to any effective treatment for such a family. The registered nurse who is in continuous contact with the patient and his family, therefore has the challenge to support the family who suffers from mental discomfort in order to lessen their suffering and facilitate their striving towards achieving wholeness once more, whether in hospital, at home or in a community centre. There are several methods of interaction available for the treatment of such a family. Mental discomfort can be reduced by psychiatric nursing accompaniment. The purpose of this study was to explore and describe the folloWing: the contribution made by psychiatric nursing accompaniment through the medium of a psychiatric nursing specialist, as regards lessening the mental discomfort of the family of a patient being treated in an oncology unit or at home. A further purpose was to set certain guidelines for the registered nurse, concerning her support of the family of a patient in an oncology unit or at home, thereby lessening their mental discomfort. An exploratory, descriptive, contextual study was done on a family with one member who has been diagnosed with cancer, and who received treatment in an oncological unit and at home. M.Cur. / This research study is on the acceptability of clinical community nursing skills in mobile health services. It is aimed at highlighting whether the skills of community nurses working in mobile health services at a certain area in KwaZulu-Natal are accepted by the community they serve. This was of concern to the researcher because it was discovered that a significant number of people in the community in this area flock to the hospitals, private doctors and traditional healers when there are mobile services which travel to various communities, in order to attend to the health needs of their residents. Interviews were conducted with the patients attending the mobile health services to obtain their views, especially on the community nursing skills. Observations were also conducted after the interviews by the researcher on the skills of the community nurses in the mobile health services. The observations were conducted to verify the results of the interviews. Findings from both data collection methods revealed that some community nursing skills are acceptable and others are unacceptable. Guidelines were formulated to render the mobile health services more user friendly.

M.Cur. / During the time as an infection control professional, there was close contact with patients with Acquired Immunodeficiency Syndrome (AIDS). During this contact period many needs regarding health support were identified. Discrimination and the testing of patients for Human Immunodificiency virus anti·bodies (HIV) without their permission, were the main problem areas. The purpose of the study was to determine the nature and extent of comprehensive health care services for patients with AIDS. The research design is a contextual, exploring and descriptive study. This study was done in the Northern half of economic development area H where 16 health care services were identified. These include clinics, community hospitals, regional- and academic hospitals and the local authority clinic. Private hospitals were not included in the study. Criteria for the nature and extent of comprehensive health care services were based on literature studies. Data was collected by means of structured interviews with nursing professionals in the employ of identified services and patients making use of those services. The results of the research showed various needs in the nature and extent of the comprehensive health care services of the patients with AIDS In order to improve the health care services, recommendations were made and guidelines given to nursing professionals. Recommendations were also made regarding other aspects of the multi·disciplinary team approach. Based upon this study other topics were identified to enable the health care services to provide health care to the patient with AIDS.

Facilitating the mental health of adolescents exposed to violence by group nursing therapy

(Psychiatric Nursing) / The objectives of this study were to describe group nursing therapy as a resource utilized by the psychiatric nurse as a way of facilitating the adolescent's mobilization of resources to cope with the violence in their everyday life and to describe guidelines for group nursing therapy to facilitate the promotion, maintenance and restoration of these adolescent's mental health. The situation of violence makes the adolescent susceptible to mental health problems. The adolescent perceive various degrees of violence in their homes, the community and at school. A combination of these violence related situations predisposes the adolescents to developing unhealthy ways of coping with the effects of violence. The number of adolescents referred to psychiatric community services has increased at an alarming rate. The main reasons for their referral was their poor scholastic performance; they were generally labelled as "a lost generation" and also as being mentally ill. The research was based on Nursing for the Whole Person Theory and the functional approach to nursing research was assumed. This implies that the goal of the research was aimed at improving the quality of patient care. The research design was descriptive and qualitative in nature, which was also contextual. The descriptive single case study method was utilized. Reliability was ensured by using as many steps as possible to operationalize the case study - and the research was conducted as if someone were always looking over the researcher's shoulder. Validity control measures were applied to counteract criticism by ensuring that a sufficient operational set of measures was developed and objective judgements were used to collect data by way of coding transcribed audiotaped material from all the sessions of the group nursing therapy...

Psychiatric nurses' communication with psychiatric patients

(Psychiatric Nursing) / The psychiatric nurse, as a member of the multiprofessional mental health team, utilises a goal directed approach to assist the psychiatric patient to mobilise resources to promote, restore and maintain his mental health as an integral part of his quest for wholeness. This goal directed approach is the nursing process which comprises assessment, planning, implementing and evaluation. All four steps of the nursing process and the nurse's interaction wi th the patient are dependent upon therapeutic communication between the nurse and the patient to elicit the necessary information so as to be able to formulate the nursing diagnosis, nursing actions and the patients' outcomes. Therapeutic communication remains important as the core of all nurse-patient interactions. Lack of therapeutic communication with the patient can cause conflict in the patient's internal and external environments since he will be unable to communicate his needs and problems and this will delay the mental health promotion, restoration and maintenance phases. Currently much attention is paid to the pharmacological treatment of the patient as more and more sophisticated psychotropic drugs are produced, and yet the other aspect of the patient's treatment which is equally important is neglected, namely his communication during hospitalisation, especially with the psychiatric nurse as she is the person in direct contact with him and should spend most of her time interacting with him.
