Nutritional assessment of agricultural migrant workers in southern Brazil

With the urbanization phenomenon, a population of unskilled migrant workers commonly known as Boia- Frias has rapidly grown up in slums on the peripheries of Brazilian cities. This study was carried put to assess the food habits and nutritional status of 100 Boia-Fria families of Vila Recreio, a slum area on the edge of Ribeirao Preto, S.P., Brazil, using dietary, anthropometric, and biochemical investigations. Qualitatively, the Boia-Fria diet was monotonous and simple, consisting basically of polished rice, beans, white bread, and coffee with sugar. In general, the foods which were lacking were: milk products, meats, fish, eggs, poultry, non-refined grain products, and fruits and vegetables rich in vitamins A and C. Foods of low nutritional value such as starchy gruels, sugar-water, herb tea, coffee with sugar and soft drinks were commonly used as weaning foods. Although dietary practices of pregnant and lactating women were poor, breastfeeding was still practiced by most mothers. According to 24-hour dietary recall data, conditions existed which were conducive to the development of nutritional problems, especially with respect to calcium, vitamin A, riboflavin, niacin, vitamin C, and iron, of the nutrients tested and with respect to quantitative intake of food. Biochemical data confirmed the presence of early malnutrition, pre-clinical in nature for about 257o of the population with respect to vitamin A, carotene, and iron. Plasma cholesterol, total lipid and vitamin E values were found to be normal. Anthropometric examinations revealed clear signs of clinical undernutrition among men and women as well as some degree of obesity among women. Child mortality data provided evidence of some advanced clinical malnutrition among children. Basic causes of malnutrition among the Boia-Frias included the following ecological factors: recent urbanization; housing, sanitation, and environmental conditions, associated with serious infection problems; poverty; illiteracy; and an ignorance of what constitutes good nutrition. Recommendations for intervention and "long-range" nutrition programs to minimize the harsh effects of poverty and upheaval on these migrant workers of Brazil were suggested. / Land and Food Systems, Faculty of / Graduate

Au Canada, la proportion de la population âgée de 65 ans et plus a augmenté depuis 1980. Bien que la dénutrition ne soit pas inévitable avec le vieillissement, certains changements et facteurs physiopathologiques, environnementaux et psycho socio-économiques peuvent entraîner une détérioration des choix alimentaires et donc, de la qualité de vie des aînés [1]. Plusieurs études font le lien entre l’état nutritionnel et la morbidité ainsi qu’avec les capacités fonctionnelles [2]. Ces observations expliquent l'intérêt de la prévention et du traitement de la dénutrition afin d’optimiser la prise alimentaire et un meilleur état de santé de cette population. Objectifs :1) Explorer les barrières individuelles et socio-environnementales, réelles et perçues, qui peuvent mener à la détérioration des choix et de la qualité alimentaires et entraîner une dénutrition chez les personnes âgées vivant à domicile. 2) Examiner la distribution de ces facteurs dans la population à l’étude. 3) Étudier la relation entre ces facteurs afin de dresser un portrait plus éclairé des déterminants négatifs de l’alimentation chez les adultes âgés pour mieux comprendre les barrières à la prise alimentaire saine. Méthodologie : Il s'agit d'une analyse secondaire réalisée à partir des données recueillies auprès des participants (n=1 602), âgés entre 67 et 84 ans,de l’Étude longitudinale québécoise sur la nutrition et le vieillissement réussi (NuAge) débutée en 2003 et dont le suivi était prévu sur cinq ans [3]. Le but principal de NuAge était de déterminer le rôle de la nutrition dans l’accomplissement d’un vieillissement réussi. Les données comprennent des mesures socio-démographiques, nutritionnelles, fonctionnelles, sociales de même que biologiques et médicales. À partir d'un modèle théorique des déterminants de la prise alimentaire chez la population âgée, ces données ont été mises en lien avec la qualité alimentaire. Cette dernière a été déterminée selon l’adaptation canadienne de l’indice d’alimentation saine (C-HEI), calculé à partir des données alimentaires et nutritionnelles obtenues par le questionnaire de fréquence alimentaire administré aux participants au T1, soit à leur entrée dans l'étude. Résultats : Les barrières qui pourraient freiner la qualité alimentaire des femmes incluent un statut affectif fragile et un fonctionnement social limité. Ce qui ressort, comme étant des barrières au C-HEI chez les hommes, est un revenu perçu comme étant insuffisant pour satisfaire les besoins, le port de prothèses dentaires et le fait de manger souvent au restaurant. Étonnamment, le nombre d’attitudes positives relatives à l’alimentation et un score plus élevé de la composante mentale du SF-36 prédisent un C-HEI plus faible. La nature des réponses auto rapportées pourrait expliquer ces résultats. Conclusion : Les résultats de cette recherche permettent de mieux comprendre les barrières d’une saine alimentation au sein d’une population bien-portante. Il est souhaité que les résultats contribueront au développement d’interventions efficaces ciblant les personnes âgées pour favoriser un apport nutritionnel et un état de santé optimal. / In Canada, the proportion of the population aged 65 and over has increased since 1980. Although under nutrition is not inevitable with aging, some changes and physiological factors, environmental and psycho-socio-economic conditions may cause deterioration in food choices and in the quality of life of seniors [1]. Several studies show the link between nutritional status and morbidity as well as functional capacity [2]. These observations justify the importance of prevention and treatment of under nutrition so as to optimize food intake and improve the health of this population. Objectives : 1) Exploring individual and socio-environmental barriers, whether it is real or perceived that may lead to deterioration of choices and quality food and cause under nutrition in the elderly living at home. 2) Examine the connection of these factors in the population being studied. 3) Examine the relationship between these factors so as to be knowledgeable about the negative determinants of nutrition in older adults to better understand the barriers to a healthy food intake. Methodology: This is a secondary analysis conducted using data collected from participants (n = 1 602), aged between 67 and 84 years in the Quebec’s Longitudinal Study on Nutrition and successful aging (NuAge) that began in 2003 with follow-ups planned over the following five years [3]. The main goal of NuAge was to determine the role of nutrition in the achievement of successful aging. The data include nutritional, functional, social, medical and biological measurements. From a theoretical model of determinants of food intake in the elderly population, these data have been linked to food quality. This was determined by the Canadian adaptation of the Healthy Eating Index (C-HEI) calculated from data of food and nutrition obtained by food frequency questionnaire administered to participants at T1 or their entry into the study. Results : The barriers that might inhibit food quality in women include a fragile emotional status and limited social functioning. What emerges as barriers of the C-HEI among men is an income considered insufficient to meet one’s needs, the use of dentures and eating at restaurant repeatedly. Surprisingly, the number of positive attitudes about nutrition and a higher score of the mental component of SF-36 are predictors of a lower C-HEI. The nature of self-reported responses could explain these results. Conclusion : The results of this research allow the better understanding of the barriers to healthy eating within a healthy community-dwelling elderly population. It is hoped that the results will contribute to the development of effective interventions targeting the older population to promote nutritional and optimal health.

An evaluation of the validity and reliability of the Healthy Athletes health promotion questionaire used to assess the dietary intake of Delaware County Special Olympics athletes

The purpose of this correlational study was to evaluate the validity and reliability of the Healthy Athlete® Health Promotion questionnaire as an instrument to assess the true dietary habits of Delaware County, Indiana, Special Olympics Athletes. A total of 35 Delaware County Special Olympics athletes completed this study. The athletes’ true dietary habits were estimated using a three-day food record and the Caregiver Questionnaire (CQ). Results indicated the Healthy Athletes Software (HAS) nutrition questions lacked statistical strength in both reliability and validity. The test-retest indicated only one-third of the questions were identified as reliable (Kappa ranged from 0.347 to 0.773; r ranged from 0.356 to 0.794). When compared to the standard, only three of the 15 food items on the HAS questionnaire had a significant relationship to the standard (r coefficients ranged from 0.458 to .777). In contrast, 11 of the 15 food items on the CQ were highly correlated with the three-day food record. The results of this study indicate the need for improvement to make to the HAS nutrition questions more reliable and valid in the assessment of Special Olympics athletes’ dietary habits / Department of Family and Consumer Sciences

A nutritional and socio-economic study of Philippi farm children and their mothers during November 1986

A community based survey to determine the nutritional status using anthropometric methods, of children and mothers and to assess certain socio-economic factors was carried out during November and December 1986 in Philippi, a predominantly vegetable farming area. Twenty-one (43%) of the 49 vegetable farms which were included in the study were selected by stratified random sampling. All children in the 0-6 year age group and their parents on the selected farms were included in the sample which consisted of 129 children and 212 parents. Questionnaires were administered and subjects' weights and heights were measured. It was found that 47% of children were below the National Centre for Health Statistics 5th percentile weight for age and 58% were under the 5th percentile height for age. There were 47 child deaths out of 279 births over a six-year period, thirty-four of which had occurred in the first year of life. Twenty eight percent of children had a birth weight of less than 2.5 kg. The number of cases of tuberculosis (10%) and severe diarrhoea (31%) contracted within the first year of life is unacceptably high. Twenty four percent of mothers were malnourished in terms of body mass index and fifty five percent of mothers were illiterate. Fifty nine percent of mothers were regular farm workers who worked for an average of 10.2 hours per day and a wage of R0.44 per hour. Grossly inadequate facilities existed for the care of children while the mothers worked. The average amount spent on food was insufficient to maintain normal nutrition. Although 91.5% of mothers breast fed their children and fed for a mean duration of 13.4 months, breast feeding was not exclusive. Supplementary feeding was introduced on average at 3 months of age. The long working hours of working mothers made it difficult for them to breast feed their children satisfactorily. Acceptance of the family planning services was high and immunisation cover was good. These statistics reflect the effects of extreme poverty and neglect of a community which is totally reliant on its employers for its livelihood. The parents poor educational level, coupled with their meagre financial and other physical resources gives them and their children little opportunity to improve their station in life and leaves them open to the scrounge of malnutrition and disease. Similarly, the dearth of educational and other child care facilities will ensure that this disastrous trend continues. A multidisciplinary approach, including active community involvement of both farmers and workers, to the many physical and social problems is urgently needed if the unacceptable human suffering is to be stopped.

Diet Quality and Evening Snacking in Relation to Sleep Duration and Quality among Women with Young Children: A Dissertation

Background: Mothers’ diets impact their health and the health of their children, but diet quality is suboptimal among women with young children. Evening snacking among women with young children, especially consumption of high-calorie, high-carbohydrate snacks, may impact overall diet quality and glucose metabolism. Short sleep duration and poor sleep quality may be potential risk factors. We examined whether sleep duration and poor sleep quality were associated with diet quality and evening snacking among women with young children. Methods: Data were from the National Health and Nutrition Examination Survey (NHANES) 2005-2012, nationally representative cross-sectional surveys of noninstitutionalized U.S. population. Eligible participants were non-pregnant women aged 20-44 years within 5 years of childbirth who completed two 24-hour dietary recalls and completed questions on sleep duration and quality. Results: Among US women with young children, sleep duration was not associated with diet quality. However, overall sleep quality was associated with poorer diet quality. Short sleep duration was not associated with the consumption of neither evening snacks, nor energy intake from or nutrient consumption of evening snacks. Conclusion: The findings of this dissertation provide information useful for informing the direction of future research of dietary quality and eating behaviors of U.S. women with young children. Studies are needed to explore whether improvement in sleep quality may improve diet quality among women with young children, which has the potential to improve both maternal and children’s health. Research may be better focused on identifying other psychosocial and behavioral risk factors for unhealthy dietary behaviors among US women with young children.

Attitudes and barriers to healthy eating amongst adolescent girls in Durban, KwaZulu-Natal

Dietary intake is a strong determinate of the health of an individual. Healthy eating is an important prevention to non-communicable diseases. If behaviour is to be changed insight into what the attitudes and barriers are to eating healthily amongst adolescents is imperative. This can provide knowledge for future prevention campaigns for healthy eating to prevent overweight and obesity. The purpose of the study was to quantitatively describe what attitudes adolescent females have towards healthy eating and what potential barriers omits them from adopting healthy eating behaviours. Data collection was done using structured questionnaires. Girls (N=73) from 3 public all girls’ high schools within Durban, KZN, participated in this study. The findings revealed that overall adolescent girls have a positive attitude to healthy eating. A lack of time, the foods sold in the school shop and the conveniences of less healthy food have been identified as barriers to healthy eating. / Health Studies / M. A. (Public Health)

Consumo alimentar de ácidos graxos em gestantes com insuficiência placentária / Food intake of fatty acids in pregnant women with placental insufficiency

Objetivo: analisar o consumo alimentar de energia, macronutrientes e ácidos graxos, de gestantes com insuficiência placentária, comparando com gestantes sem esta complicação obstétrica. Métodos: Estudo prospectivo, transversal e caso-controle realizado no período de fevereiro de 2012 a setembro de 2013, que incluiu gestantes que preencheram os seguintes critérios: gestação com feto único e vivo; idade gestacional superior a 26 semanas completas; diagnóstico de insuficiência placentária caracterizada pelo exame de Doppler de artéria umbilical com índice de pulsatilidade acima do p95; morfologia fetal normal ao exame de ultrassonografia; ausência de diagnóstico de diabetes; não suplementação pré-natal com ácidos graxos. Foram adotados os seguintes critérios de exclusão: diagnóstico pós-natal de anomalia do recém-nascido. O estado nutricional da gestante foi avaliado pelo índice de massa corporal (IMC) e o consumo dietético foi investigado pela aplicação do questionário de frequência alimentar, analisado pelo programa Avanutri Revolution versão 4.0, pelo qual se obteve o consumo de energia, macronutrientes (carboidratos, proteínas e lipídios) e de ácidos graxos (saturados, poli-insaturados e monoinsaturados). Foram analisados os valores absolutos obtidos e a % do valor energético total (VET) da dieta. Resultados: Foram incluídas 21 gestantes no grupo com insuficiência placentária e 21 gestantes no grupo controle. Não se constatou diferença na mediana do IMC na comparação entre os grupos (grupo estudo=26,5 kg/m2, grupo controle=28,0kg/m2; P=0,563). Houve diferença significativa na comparação do grupo com insuficiência placentária com o grupo controle na análise do consumo alimentar de: energia (2002 kcal vs. 1515 kcal, p= 0,021). Com relação ao consumo de ácidos graxos, houve diferença significativa na comparação da % do VET entre os grupos com insuficiência placentária e controle: saturados (11,5% vs. 9,3%; p=0,043); poli saturados (2,7% vs. 3,6%; p=0,029); monoinsaturados (1,2 % vs. 2,1%; p= 0,005). Não foram encontradas diferenças significativas na qualidade da dieta entre os grupos quanto ao consumo avaliado de acordo com a % do VET: carboidratos (51,5% vs. 51,8%; p= 0,831); proteínas (15,3% vs. 16,1%; p= 0,458); lipídios totais (37,8% vs. 33,0%; p=0,831). Conclusão: Gestantes com o diagnóstico de insuficiência placentária relatam consumo alimentar diferente de gestantes que não apresentam esse diagnóstico, com dieta de ácidos graxos com qualidade inferior, notadamente com maior consumo de ácidos graxos saturados, e menor consumo de poli-insaturados e monoinsaturados, além de maior consumo de energia / Objective: To analyze the dietary intake of energy, macronutrients and fatty acids of pregnant women with placental insufficiency, and to compare with pregnant women without this obstetric complication Methods : A prospective, cross-sectional and case -control study from February 2012 to September 2013, which included women who met the following criteria: singleton pregnancy with fetus alive; above 26 weeks gestation; diagnosis of placental insufficiency characterized by umbilical artery Doppler presenting pulsatility index above the p95; normal fetal morphology at ultrasound, absence of diabetes, absence of prenatal supplementation with fatty acids. We used the following exclusion criteria: neonatal diagnosis of malformation. The maternal nutritional status was assessed by body mass index (BMI) and dietary intake was investigated by applying the food frequency questionnaire analyzed by the program Avanutri Revolution version 4.0 , which was obtained by the consumption of energy, macronutrients (carbohydrates, proteins and lipids) and fatty acids (saturated, polyunsaturated and monounsaturated). We analyzed the absolute values and the % of total energy value (TEV). Results: We included 21 pregnant women in the study group with placental insufficiency and 21 pregnant women in the control group. There was no difference in median BMI between the groups (study group = 26.5 kg/m2, control group = 28.0 kg/m2, P = 0.563). Significant difference was found when the group with placental insufficiency was compared with the control group in food consumption of energy (2002kcal vs. 1515 kcal, p = 0.021). With regard to the consumption of fatty acids, there was a significant difference in the percentages of daily energy intake between the group with placental insufficiency and control group: saturated fatty acids(11.5% vs. . 9.3% , p = 0.043), polyunsaturated fatty acids(2.7 % vs. 3.6% , p = 0.029), monounsaturated fatty acids(1.2% vs. 2.1% , p = 0.005). There were no significant differences in diet quality between the groups regarding the consumption evaluated according to the % of VET: carbohydrates ( 51.5 % vs. 51.8 %, p = 0.831 ), protein (15.3 % vs. 16.1 %, p = 0.458), total fat (37.8 % vs. 33.0 %, p = 0.831) . Conclusion: Pregnant women with the diagnosis of placental insufficiency reported food consumption other than pregnant women who do not have this diagnosis with lower quality of dietary fatty acids consumption, especially with higher intake of saturated fatty acids , and lower intake of polyunsaturated and monounsaturated fatty acids, and greater energy consumption
