Women's Selection and Evaluation of Obstetric Hospitals: A Survey of the Northern Sydney Area.

A study of women's views of maternity services in the Northern Sydney Area Health Service was conducted as a result of the changing patterns of use of the Area's 7 obstetric hospitals. 340 primiparous women living in the Northern Sydney Area who had given birth in the previous six months were approached in Early Childhood Health Centres and asked to complete a survey exploring the factors influencing their choice of obstetric hospital, postnatal length of stay in hospital, and overall satisfaction with their choice of hospital. Of the 315 eligible women, 312 (99%) consented to participate and 297 (94%) completed the survey. Overall, reputation of the hospital and quality of nursing care were the most frequent reasons given for choice of hospital and there was some evidence that women selected different hospitals for distinct reasons. Women's postnatal length of stay ranged from less than 1 day to 11 days with an average of 5.3 days. Private patients stayed an average of 1 day longer than public patients, after adjusting for delivery type and pregnancy induced hypertension. There was little evidence that women in the Northern Sydney Area Health Service desire a shorter postnatal stay with the majority of women reporting they were satisfied with their length of stay. Overall, women displayed high levels of satisfaction with their choice of hospital; at least 90% of women attending all hospitals except one reported that they would choose the same hospital for the birth of another baby. This study provides valuable information, based on the experiences of the service users, to help guide the Northern Sydney Area Health Service in the provision of its maternity services to ensure they meet the changing needs of women and their families.

Förlossningsrädsla under graviditet : Betydande bakgrundsfaktorer- Förekomst- Önskemål om förlossningssätt- Kvinnors kontakt med klinik för förlossningsrädda

Syftet med studien var att undersöka förekomst av gravida kvinnors upplevelse av förlossningsrädsla i relation till deras sociodemografiska, psykiatriska och obstetriska bakgrund samt önskat förlossningssätt. Metod. Förstföderskor och omföderskor som skrevs in på barnmorskemottagning tillfrågades om deltagande i studien och fick besvara två frågeformulär under graviditeten. Resultat. Totalt 776 kvinnor svarade på båda frågeformulären och svarsfrekvensen för de flesta frågor som användes till studien var över 92 %. I tidig graviditet upplevdes förlossningsrädsla av 12 % av kvinnorna i studien och av 15,5 % i sen graviditet. Nästan hälften av de med tidig upplevelse av förlossningsrädsla upplevde i slutet på graviditeten ingen eller liten rädsla. Upplevd förlossningsrädsla var vanligare bland kvinnor som tidigare varit gravida, omföderskor med erfarenhet av kejsarsnitt, kvinnor födda utanför Sverige och kvinnor som tidigare haft kontakt med psykiater och kurator. Kvinnor med upplevd förlossningsrädsla var mer benägna att önska kejsarsnitt än de som upplevde ingen eller liten rädsla. Av kvinnor med upplevd förlossningsrädsla hade omkring en fjärdedel under graviditeten varit i kontakt med Klaramottagningen, en mottagning med specialisering på förlossningsrädsla och en femtedel av dessa kvinnor rapporterade mot slutet av graviditeten ingen eller liten rädsla. Kontakt med Klaramottagningen hade även kvinnor som i tidig graviditet rapporterade ingen eller liten rädsla. Av dessa kvinnor rapporterade 45,8 % att de upplevde förlossningsrädsla i slutet på graviditeten. Konklusion. Former för hur identifiering, fördelning av vårdens resurser och utformning av stödet till gravida kvinnor och framförallt till de med upplevd förlossningsrädsla bäst sker kan utvecklas ytterligare.

It has as main objective to analyze morbidity in pregnant women as a factor associated with the occurrence of preterm births in São Luís, MA. This is an analytical cross-sectional study from the database of a research that investigates the causes of preterm birth and consequences of perinatal factors in child health. The sample consisted of pregnant women who delivered their babies in public or private institutions in 2010 in São Luís, MA. We used the chi-square test to assess the association between morbidity and preterm birth. Among the 5064 pregnant women participants, most were aged between 20 and 34 years (73.66%), was mixed/mulatto/cabocla (67.53%), schooling 9-11 years of education (57.93%) , lived in a consensual union (59.17%), and lived with 1-4 people at home (68.65%). Among the morbidities, there was a high prevalence of anemia (47.06%), urinary tract infection (26.17%), hypertension (16.34%), and lower prevalence of diabetes (2.10%), rubella (0.06%), toxoplasmosis (0.77%) and syphilis (0.65%). However, only hypertension (p < 0.001) showed to be associated with the occurrence of preterm births. We found a prevalence of 13.39% of preterm births. We conclude, be much higher prevalence of preterm births and morbidity in pregnant women in Sao Luis, MA, mainly anemia, urinary tract infection and hypertension, the latter being associated with preterm birth. Needing therefore effective measures for quality of care in pregnancy and childbirth in Sao Luis, MA. / As morbidades presentes em gestantes durante o ciclo gravídico puerperal causam sérios danos à integridade da saúde destas e de seus conceptos, podendo evoluir com a morte de ambos ou ainda com sérios problemas como o nascimento prétermo. Tem-se como objetivo principal analisar as morbidades em gestantes como fator associado à ocorrência de nascimentos pré-termos em São Luís-MA. Trata-se de um estudo transversal analítico, a partir do banco de dados de uma pesquisa que investiga as causas do nascimento pré-termo e consequências dos fatores perinatais na saúde da criança. A amostra foi constituída por gestantes que tiveram partos em instituições públicas ou privadas, em 2010, em São Luís-MA. Utilizou-se o teste Quiquadrado para verificar a associação entre as morbidades e o nascimento pré-termo. Dentre as 5.064 gestantes participantes, a maioria tinha idade entre 20 e 34 anos (73,66%), era parda/mulata/cabocla (67,53%), com escolaridade de 9 a 11 anos de estudo (57,93%), vivia em união consensual (59,17%), e convivia com 1 a 4 pessoas no domicílio (68,65%). Quanto às morbidades, observou-se elevada prevalência de anemia (47,06%), infecção urinária (26,17%), hipertensão arterial (16,34%), e baixa prevalência de diabetes (2,10%), rubéola (0,06%), toxoplasmose (0,77%) e sífilis (0,65%). Entretanto, somente a hipertensão (p < 0,001) apresentou-se associada à ocorrência de nascimentos pré-termo. Encontrou-se uma prevalência de 13,39% de nascimentos pré-termos. Conclui-se, ser elevada a prevalência tanto de nascimentos pré-termos como de morbidades em gestantes de São Luís-MA, principalmente, anemia, infecção urinária e hipertensão, sendo esta última associada ao nascimento pré-termo. Necessitando, portanto, de medidas efetivas na qualidade da assistência ao ciclo gravídico puerperal em São Luís-MA.
