Síntese, purificação, caracterização e análise da atividade antinociceptiva de peptídeos opióides

Este trabalho tem como objetivo a predição e a síntese de peptídeos que tenha atividade antinociceptiva e hipotensora. A predição da estrutura primária dos peptídeos foi realizada com base em trabalhos anteriores do grupo, os quais focam na prospecção e caracterização de peptídeos bioativos, assim como no conhecimento disponível na literatura acerca das propriedades biológicas apresentadas por domínios protéicos específicos. Os peptídeos foram sintetizados por meio de síntese química em fase sólida, utilizando a estratégia Fmoc, seguida da purificação em cromatografia líquida de alta eficiência. O grau de pureza e a confirmação da estrutura primária dos peptídeos sintetizados foram determinados por espectrometria de massa, Maldi e ESI. Os peptídeos foram testados em camundongos , por via intraperitoneal com intuito de avaliar sua possível atividade antinociceptiva, realizaram-se então os testes da placa quente ou Hot Plate e teste de retirada da cauda ou Tail Flick. Os testes in vivo demonstraram que os peptídeos sintéticos PSLEM 11011 e 11012 apresentaram atividade antinociceptiva via receptores opióides com perfil de atividade mais tardio e duradouro em relação a morfina. Eles apresentaram atividade nos dois modelos de testes propostos, podem ter atravessado a barreira hematoencefálica e possuem ação prolongada. ______________________________________________________________________________ ABSTRACT / To prospect of peptides with biological activity is important from the point of view of biotechnology as a starting point in many fields of research as the development of new drugs and the production of genetically modified plants. This study aims to predict and synthesize peptides that have antinociceptive and hypotensive activities. The prediction of the primary structure of the peptides was based on previous work of the group, which focuses on the prospection and characterization of bioactive peptidesas well as in the knowledge available in the literature about the biological properties of specific protein domains. The peptides were synthesized by solid-phase chemical synthesis using Fmoc strategy followed by purification on high performance liquid chromatography. The purity and confirmation of the primary structure of the synthesized peptides were determined by mass spectrometry, MALDI and ESI.The peptides were injected in mice intraperitoneally in order to assess its possible antinociceptive activity through Hot Plate and Tail Flick assays.The in vivo tests showed that synthetic peptides PSLEM 11011 and 11012 have antinociceptive activity via opioid receptors with delayed response and longer activity compared to morphine. The peptides showed activity in both test models proposed in this work, may have crossed the blood-brain barrier and presenting high bioavailability.

It is known that the prognosis of this tumor is directly related to its staging, with more advanced stages responsible for lower survival and impairment in the patient's quality of life, mainly due to pain complaints. Opioids are widely used to minimize pain at the oncology clinic. In the literature there are conflicting results regarding the use of opioid drugs and their action on tumor cells and the process of angiogenesis in the tumor micro-environment. Therefore, this study aimed to evaluate the expression of mu (μ), Kappa (κ) opioid receptors, correlating with prognostic factors such as markers of angiogenesis (VEGF), cell proliferation (Ki-67) and cell death (TUNEL) in Biopsies of tumors with T4 staging, with or without metastatic process, stratified for the Luminal A and Luminal B molecular subtype, diagnosed and operated at Haroldo Juaçaba Hospital (Ceará Cancer Institute) from 2011 to 2016. A total of 38 biopsy specimens from Luminal A and Luminal B breast cancer patients with T4 tumor size, with and without metastatic process, diagnosed, treated and operated at the abovementioned Hospital were included in the study. Demographic data and tumor characteristics, including the use of opioids and the presence of disease recurrence, were also a reason for evaluation and comparison. In addition, in the present study, the expression, by immunohistochemistry, of the μ and κ opioid markers, VEGF, Ki-67 and TUNEL in the breast biopsy samples by the Tissue Microarray (TMA) technique was evaluated. Our results demonstrated that: 1. It was found that there is a significant predominance of ki67 immunoblotting in lymph nodes in patients who used opioids when compared to patients who did not use opioids in the clinical follow-up (p = 0.011); 2. It was evidenced that patients of the Luminal A molecular subtype present a significant increase in the chances of presenting a 51-75% T cell count in the tumor when compared to patients defined as Luminal B subtype (p = 0.020, OR = 21.000; CI = 1,613-273,340); 3. It was possible to show that patients with T4 staging who presented recurrence (7.2 ± 3.7) demonstrated a decrease in the histoscore expression of the Opioid κ in the lymph node location when related to the patients who did not present clinical evolution associated with relapse (10.8 ± 2.2) (p = 0.033) and 4. The correlation analyzes between the opioid μ and κ markers, VEGF, Ki-67 and TUNEL demonstrate that these findings probably act on the same cascade of events in the CA of T4 stage breast. Our study demonstrated that the use of opioids is directly associated with the increase in ki-67 expression and the prevalence of relapses, which showed lower expression for the opioid kappa receptor. In addition, there was no significant difference in the biological behavior of luminal A and luminal B tumors and the immunoexpression of opioid receptors were independent of angiogenesis receptors. In summary, in luminal A and luminal B tumors the use of opioids is directly associated with a worse prognosis, due to reduction of opioid kappa receptor expression. / O câncer de mama representa uma doença de grande preocupação, devido sua alta incidência e elevada mortalidade sendo, atualmente, considerado como a segunda maior causa de morte no mundo. Sabe-se que o prognóstico deste tumor está diretamente relacionado ao seu estadiamento, sendo estágios mais avançados responsáveis pela menor sobrevida e prejuízo na qualidade de vida da paciente, devido principalmente a dor. Os opióides são bastante utilizados para minimizar a dor na clínica oncológica. Contudo, na literatura observam-se resultados conflitantes em relação a utilização de fármacos opióides e sua ação nas células tumorais e no processo de angiogênese no microambiente tumoral, ainda há muitas controvérsias. Portanto, esse estudo propôs avaliar a expressão dos receptores opióides Mu (μ), Kappa (κ), correlacionando com fatores prognósticos tais como os marcadores de angiogênese (VEGF), de proliferação celular (Ki-67) e morte celular (TUNEL) em biópsias de tumores mamários com estadiamento T4, com ou sem processo metastático, estratificados quanto ao subtipo molecular Luminal A e Luminal B, diagnosticados e operados no Hospital Haroldo Juaçaba (Instituto do Câncer do Ceará) no período de 2011 a 2016. Um total de 39 amostras de biópsias de pacientes com câncer de mama padrões Luminal A e Luminal B, com tamanho do tumor T4, com e sem processo metastático, diagnosticados, tratados e operados no Hospital supracitado foram inseridos no estudo. Dados demográficos e características tumorais, incluindo o uso de opióides e a presença de recidiva de doença, foram também motivo de avaliação e de comparação. Além disso, no presente estudo, foi avaliada a expressão, através de imunohistoquímica, dos marcadores μ e κ opióides, VEGF, Ki-67 e TUNEL nas amostras de biópsia de mama através da técnica de Tissue Microarray (TMA). Nossos resultados demonstraram que: 1. verificou-se que há um predomínio significativo de imunomarcações de ki67 nos linfonodos em pacientes que utilizaram opióides quando comparados às pacientes que não utilizaram opióides no seguimento clínico (p=0,011); 2. foi evidenciado que pacientes do subtipo molecular Luminal A apresentam um aumento significativo de chances de apresentar 51-75% de marcação de morte celular tecidual (TUNEL) no tumor quando comparadas as pacientes definidas como subtipo Luminal B (p=0.020; OR=21.000; IC=1.613-273.340); 3. foi possível evidenciar que as pacientes com estadiamento do tipo T4 que apresentaram recidiva (7,2±3,7) demonstraram uma diminuição de expressão do histoscore do κ Opióide, na localização linfonodal, quando relacionado às pacientes que não apresentaram evolução clínica associada à recidiva (10,8±2,2) (p=0.033) e 4. as análises de correlação entre os marcadores μ e κ opióides, VEGF, Ki-67 e TUNEL demonstram que estes achados, provavelmente, atuam em uma mesma cascata de eventos no CA de mama com estadiamento T4. O presente estudo demonstrou que o uso de opioides está diretamente associado ao aumento da expressão de Ki-67 e da prevalência de não recidivar, as quais mostraram menor expressão para o receptor kappa opioide. Adicionalmente, não houve diferença significante entre os tumores luminal A e luminal B e a imunoexpressão dos receptores opioides foram independentes dos receptores de angiogênese. Em síntese, nos tumores luminal A e luminal B o uso de opioides está diretamente associado a um pior prognóstico, por redução da expressão do receptor kappa opioide.

Anestesia total intravenosa em cães pela administração de propofol e butorfanol em infusão contínua: hemodinâmica, eletrocardiografia e índice biespectral

Para tal, foram utilizados 20 cães adultos hígidos, sem raça definida. Os animais foram distribuídos em dois grupos de igual número (GP e GB) e induzidos à anestesia com propofoi, por via intravenosa, na dose de 10 mg/kg. Após a intubação orotraqueal, instituiu-se ventilação controlada com oxigênio a 60% e fluxo de. 30 mL/kg/min em circuito fechado, com volume controlado. A manutenção anestésica foi realizada com a administração de 0,6 mg/kg/min de propofol. O grupo butorfanol (GB) recebeu, por via intravenosa, bolus de butorfanol 0,1 mg/kg seguido de infusão contínua do fármaco dose de 40 mg/kg/h. O grupo placebo (GP) recebeu somente NaCI 0,9% em bolus, seguido de infusão contínua, em volume idêntico ao do GB. A taxa de infusão foi ajustada considerando-se o volume calculado/hora para ambos os grupos, por meio de bomba de infusão. As mensurações das variáveis foram realizadas imediatamente antes da aplicação do opióide ou da solução de NaCI a 0,9% (MO). Novas mensurações foram realizadas em intervalos de 15 minutos, após a administração do butorfanol ou da salina, por um período de 75 minutos (M15, M30, M45, M60 e M75). O método estatístico utilizado foi análise de variância (ANOVA), seguida pelo Teste Tukey (P<0,05). A administração de butorfanol resultou em aumento dos intervalos QRS (ms), RR (ms) e da freqüência respiratória (1), e em redução da resistência periférica total (RPT), das pressões arteriais sistólica (PAS), diastólica (PAD) e média (PAM), do débito cardíaco (DC) e da temperatura corporal central (T DC). Os resultados permitem concluir que a associação proporciona anestesia segura para pacientes com suporte ventilatório / The aim of this study was to evaluate the hemodynamic and electrocardiography effects and also the bispectral index of the total intravenous anesthesia with propofol and butorphanol in continuous infusion in dogs. For that purpose, twenty adult healthy crossbreed dogs were used. The animais were equally allocated intü twogroups (GP and GB) and induced to the anesthesia with intravenoüs propofol (10 mg/kg). After orotracheal intubation, ventilation controlled was instituted with 6-0% oxygen and flow of 30 mL/kglmin in a c10sed system, withcontmlled volume. The anesthetic maintenance was accomplished with propofol (0.6 mg/kg/min). The butorphanol group (GB) it received, for intravenous road, bolus of butorphanol 0.1 mg/kg foilowing by continuous infusion (40 ) mglkg/h). The placebo group (GP) it received only NaCI 0.9% in bolus, following by continuous infusion, in identical volume to of the GB. The infusion rate was adjusted according to the calculated volume/hour for both groups through an infusion bomb. Baseline measurements were performed before opioid or saline administration (MO). New measurements were obtained every 15 minutes after the butorphanol or placebo administration (M15, M30, M45, M60 and M75). Statistical analysis was carried out using variance analysis (ANOVA) followed by the Tukey Test (P<0.05). Administration of butorphanol resulted in an increase of QRS interval (ms), of RR interval (ms) and respiratory rate (f), decrease of total peripheric resistance (TPR), systolic arterial pressure (SAP), diastolic arterial pressure (DAP), mean arterial pressure (MAP), cardiac output (CO) and body central temperature (T DC). The results allow us to conclude that association provide safe anesthesia for patients with ventilatory support

Estudo demográfico sobre as condutas de avaliação e tratamento da dor dos médicos veterinários brasileiros no período perioperatório de grandes e pequenos animais /

Para tal, é necessário que os profissionais saibam reconhecer e tratar a dor de forma adequada em animais. Objetivou-se correlacionar diversos dados demográficos para obter o perfil do médico veterinário brasileiro de grandes e pequenos animais. O questionário da pesquisa foi composto por: dados pessoais, utilização de fármacos analgésicos, analgesia, conduta no alívio da dor, uso de analgésicos em diversos procedimentos de grandes e pequenos animais, avaliação da dor e atualizações. A estatística foi realizada pelo programa SAS for Windows versão 9.1.3 com estatística descritiva com análise de frequência. Para as comparações simples foi utilizado o teste de qui-quadrado (x2). Foram obtidos 1.298 questionários de pequenos animais e 713 de grandes. Mulheres e profissionais graduados havia menos de dez anos conferiram maiores escores de dor que homens e profissionais formados havia mais de dez anos, porém a duração do tratamento não diferiu entre os gêneros. Os opioides mais utilizados para a analgesia foram tramadol (79%) e morfina (50,5%), em cães e gatos, e butorfanol (43,4%) e tramadol (39%) em grandes animais. Os efeitos adversos mais reportados dos opioides em gatos foram depressão respiratória e excitação. Em cães os principais efeitos adversos assinalados foram depressão respiratória e êmese. Para grandes animais, as preocupações com o uso desses fármacos foram: risco de excitação e síndrome cólica equina. Mais de 50% dos veterinários não utilizava opioides em bovinos. Os anti-inflamatórios não esteroidais (AINEs) mais escolhidos para pequenos animais foram meloxicam (81%) e cetoprofeno (70,4%), e flunixin meglumine (83,2%) e cetoprofeno (67,6%) em grandes animais. According to that, the professionals should know how to recognize and treat pain in animals. The aim of this study was to correlate the demographic data of the Brazilian veterinarians, with the use of analgesics, the factors that affected the decision on the use of analgesia, attitudes, knowledge and methods of obtaining information on the evaluation and treatment of pain in animals. The questionnaire was composed of demographics, personal data, use of analgesics in general and specific procedures, analgesia, attitudes in the assessment and relief of pain and types of information in the area. The descriptive statistics with frequency analysis was performed using SAS for Windows 9.1.3. Chi-square (x2) for simple comparisons test was used. Questionnaires were obtained from 1298 small and 713 large animal veterinarians. Women and veterinarians graduated for less than ten years attributed higher pain scores than men, and veterinarians graduated for over ten years, but the frequency and duration of analgesic treatment did not differ between gender. The most commonly used opioid for analgesia of small animals were tramadol (79%) and morphine (50.5%) for dogs and cats, and butorphanol (43.4%) and tramadol (39%) for large animals. The most important side effects of opioids in small animals were respiratory depression and excitement, for cats and emesis in dogs and excitement and colic syndrome in 4 horses. NSAIDs of choice for small animals were meloxicam (81%) and ketoprofen (70.4%) and for large animals, flunixin meglumine (83.2%) and ketoprofen (67.6%). Side effects of NSAIDs most frequently reported for all species were gastric changes and nephrotoxicity. Administração repetida de morfina em ratos infantes : efeitos em curto, médio e longo prazos nas atividades nociceptiva, comportamental e ectonucleotidásicas

O estudo do sistema opióide no neonato tem sido freqüentemente pesquisado. Os neonatos rotineiramente experimentam dor aguda e crônica causada por procedimentos invasivos em UTI pediátrica e freqüentemente não recebem analgesia adequada. Entretanto, o uso da analgesia opióide vem aumentando nas últimas décadas em consequência das mudanças e avanços na compreensão, na identificação e no tratamento da dor em crianças. Entretanto, ainda existe uma carência em estudos de seus efeitos específicos sobre uma administração em longo prazo nestes pacientes. Adicionalmente, alguns estudos têm demonstrado que a exposição às drogas logo após o nascimento pode ter implicações duradouras no sistema nervoso, e que a exposição à morfina pode influenciar no desenvolvimento e na função de alguns sistemas do neurotransmissores. Existem múltiplos sistemas espinhais que modulam a neurotransmissão nociceptiva no corno dorsal. Propõe-se que a adenosina extracelular esteja envolvida no controle fisiológico da dor a nível espinal e na ação antinociceptiva opióide. Por outro lado, o ATP é um neurotransmissor liberado dos terminais nervosos sensoriais aferentes para atuar na circuitaria central da dor. Os nucleotídeos extracelulares podem ser hidrolisados por E-NTPDase e por 5’nucleotidase. Esta cascata enzimática apresenta dupla função: remover o sinal do ATP e gerar um segundo, adenosina. Esta dissertação teve como objetivos: investigar o efeito da administração repetida da morfina de P8 até P14 nas respostas nociceptiva, inflamatória, comportamental e na atividade das ectonucleotidases em medula espinhal e córtex de ratos em P16, P30 e P60. Além disso, avaliar o efeito de uma segunda exposição repetida de morfina por 7 dias em P80 na resposta nociceptiva, inflamatória e comportamental. Os animais que receberam a morfina por sete dias em P14 não desenvolveram tolerância, porém, quando avaliados em P80, demonstraram maior analgesia da morfina (administração aguda), e após 7 dias de administração diária de morfina (P86), desenvolveram o clássico fenômeno da tolerância. No teste da formalina, observou-se aumento na resposta nociceptiva em P30 e em P60, mas não em P16. No desempenho no campo aberto os animais apresentaram alteração de alguns comportamentos, tais como aumento de grooming em P16, e aumento de rearing e locomoção em P30. Em P88, o grupo que recebeu a morfina diariamente (de P80 até P86) apresentou aumento de rearing. Nas atividades das E-NTPDases, observou-se em P16 um aumento na atividade de hidrólise do ATP em sinaptossomas de córtex cerebral, e uma diminuição na atividade de hidrólise do ADP em sinaptossomas de medula espinhal. Nas demais idades avaliadas (P30 e P60) não formas observadas diferenças nas atividades de hidrólises dos núcleotídeos de ambas as estruturas analisadas. O limiar nociceptivo mais elevado observado no teste da formalina pode ser devido a neuroplasticidade na circuitaria nociceptiva, tais como em neurotransmissores excitatórios em nível espinhal. As alterações comportamentais observadas sugerem que a exposição à morfina, logo após o nascimento, pode desencadear, a curto e longo prazo, fenômenos tais como sensibilização comportamental. Em conclusão, estes resultados indicam que a exposição à morfina durante a segunda semana de vida pode promover aumento na resposta nociceptiva e alterações comportamentais que perderam ao longo da vida. Os efeitos observados após exposição repetida à morfina em animais podem colaborar com o entendimento de conseqüências clínicas decorrentes da administração em longo prazo de opióides. Adicionalmente, estes resultados sugerem a necessidade do desenvolvimento de pesquisas que abordem diferentes sistemas de neurotransmissores objetivando neutralizar tais adaptações comportamentais induzidas pela morfina. Os resultados nas atividades de E-NTPDases destacam a importância do sistema purinérgico de ratos infantes submetidos à exposição repetida da morfina. Essas alterações enzimáticas podem constituir um dos mecanismos responsáveis pelos efeitos secundários da retirada opióide. / The study of the opioid system in the newborn has been frequently researched. The newborns routinely experiencing acute and chronic pain caused by invasive procedures in pediatric ICU and often do not receive adequate analgesia. However, the use of opioid analgesia has been increasing in recent decades as a consequence of changes and advances in the understanding, in the identification, and treatment of pain in children. However, there is still a lack of knowledge on their specific effects in a long-term administration for this population of patients. Additionally, some studies have shown that exposure to drugs soon after birth may have lasting implications in the nervous system, and that exposure to morphine can influence the development and function of some systems of neurotransmitters. There are multiple systems that modulate the spinal nociceptive neurotransmission in the dorsal horn. It has been proposed that the extracellular adenosine is involved in the physiological control of pain in spinal and action antinociceptiva opioid. Moreover, the ATP is a neurotransmitter released from sensory afferent nerve terminals to act in the central circuitaria of pain. The extracellular nucleotides can be hydrolysates by E-NTPDase and by 5’nucleotidase. This enzyme cascade presents dual function: remove the signal from the ATP and generating a second, adenosine. This dissertation had the following objectives: to investigate the effect of repeated administration of morphine from P8 to P14 in nociceptive responses, inflammatory, and behavioral activity ectonucleotidases in the cortex and spinal cord of rats in P16, P30, P60. Moreover, to evaluate the effect of one second repeated morphine exposition per 7 days in P80 in the nociceptive, inflammatory and behaviour response. Animals that received the morphine for seven days in P14 did not develop tolerance, but when evaluated in P80, showed greater the morphine analgesia (acute administration), and after 7 days of daily administration of morphine (P86), developed the classic phenomenon of tolerance. In the formalin test, it was observed increase in response to nociceptive P30 and P60 but not on P16. In performing in the open field the animals showed some change in behavior, such as increased grooming in P16, and increase of rearing and locomotion in P30. At P88, the group that received the morphine daily (from P80 to P86) showed increase of rearing. On the ENTPDases activities were observed at P16 increase in the activity of hydrolysis of ATP in sinaptossomas of cerebral cortex, and a decrease in the activity of ADP in the hydrolysis of sinaptossomas spinal cord. In other ages evaluated (P30 and P60) it was not observed differences on nucleotides hydrolysis activity from both structures.analyzed. The higher threshold nociception observed in the test of formalin can be due to neuroplasticidade in circuitaria nociceptive, such as neurotransmitters in excitatórios in spinal level. The behavioural changes suggest that exposure to morphine, soon after birth, can trigger in the short and long term, phenomena such as behavioral sensitization. In conclusion, these results indicate that exposure to morphine during the second week of life may promote increase in response nociceptive and behavioural changes that have lost their lifetime. These behavioural changes indicate the need for the evaluation of the clinical consequences of long-term opioid administration. Additionally, these results suggest the need for development of research that address different systems of neurotransmitters aiming neutralize the behavioral changes induced by opioid. AThe findings on the E-NTPDases activities highlight the importance of purinergic system of young rats submitted to the repeated morphine exposure. These alterations activities may constitute one of the mechanisms that mediate the development of some of the side effects, such as opioid withdrawal, associated with repeated morphine exposure in early life.

A ropivacaína é um fármaco relativamente novo, ainda pouco utilizado em Veterinária. O fentanil é um opióide agonista e o tramadol é um opióide de ação mista. Neste experimento, oito cães foram tranqüilizados com acepromazina, submetidos à peridural com um dos protocolos a seguir: GR (ropivacaína), GRF (ropivacaína + fentanil), GRT (ropivacaína + tramadol), em volume total de 0,25 mL/kg. Durante o procedimento foram avaliados e comparados os seguintes parâmetros vitais (FC, f, temperatura retal, pressão arterial, e gasometria do sangue arterial), os bloqueios sensitivo e motor (latência e duração de ação), o grau de sedação, e a ocorrência de possíveis efeitos indesejáveis advindos da administração de ropivacaína isolada ou em associação. A diminuição mais intensa na FC ocorreu nos grupos GRF e GRT, e ocorreu hipotermia significativa nos animais do GRF. Todos os grupos apresentaram sedação, sendo severa nos grupos GRF e GRT. De maneira geral, o período de recuperação foi mais curto nos animais do grupo GRT do que nos demais. O GRT também foi o que apresentou bloqueio mais cranial. Foram observadas bradicardia, hipotermia e síndrome de Shiff- Sherrington no período trans-anestésico em animais de todos os grupos. Decorridas 24 horas de período pós-anestésico, não foram evidenciados efeitos indesejáveis, em nenhum dos grupos. GRF foi o grupo com maior duração de anestesia e analgesia, GRT apresentou a menor duração de anestesia com analgesia intermediária, e GR apresentou duração intermediária, com menor analgesia. / Peridural anesthesia is broadly applied in the Veterinary field, using the isolated local anesthetic or in combination with opiates capable to increase the analgesic effect. The ropivacaine is a relatively new drug, not much used in the Veterinary field yet. The fentanil is an agonist opiate and tramadol is a mixed action opiate. In this experiment, eight dogs were sedated with acepromazine and subjected to the epidural anesthesia with one of the following protocols: GR (ropivacaine), GRF (ropivacaine + fentanyl), GRT (ropivacaine + tramadol), in 0,25mL/Kg of total volume. During the procedure, following vital signs were evaluated and compared (heart rate, respiratory rate, rectal temperature, blood pressure and gasometry of arterial blood), the sensory and motor blockade (latency and length of action), level of sedation and the occurrence of possible side effects due to administration of ropivacaine individually or in combination with other drugs. The highest decrease in the heart rate occurred in the following GRF and GRT and also significant hypothermia in animals of GRF. All groups presented sedation, even severe in the period of recovering was shorter in the animals belonging to GRT than in others. The GRT was also the one that presented the most cranial block. Bradycardia, hypothermia and Shiff- Sherrington syndrom were observed in the transanesthetic period in animals belonging to all of the groups. Twenty-four hours after the postanesthetic period, no side effects were observed, in none of the groups. The GRF was the one with higher length of anesthesia and analgesia, GRT presented the length of anesthesia with intermediate analgesia and, GR group presented intermediate length, with lower analgesia.

Administração repetida de morfina em ratos infantes : efeitos em curto, médio e longo prazos nas atividades nociceptiva, comportamental e ectonucleotidásicas

O estudo do sistema opióide no neonato tem sido freqüentemente pesquisado. Os neonatos rotineiramente experimentam dor aguda e crônica causada por procedimentos invasivos em UTI pediátrica e freqüentemente não recebem analgesia adequada. Entretanto, o uso da analgesia opióide vem aumentando nas últimas décadas em consequência das mudanças e avanços na compreensão, na identificação e no tratamento da dor em crianças. Entretanto, ainda existe uma carência em estudos de seus efeitos específicos sobre uma administração em longo prazo nestes pacientes. Adicionalmente, alguns estudos têm demonstrado que a exposição às drogas logo após o nascimento pode ter implicações duradouras no sistema nervoso, e que a exposição à morfina pode influenciar no desenvolvimento e na função de alguns sistemas do neurotransmissores. Existem múltiplos sistemas espinhais que modulam a neurotransmissão nociceptiva no corno dorsal. Propõe-se que a adenosina extracelular esteja envolvida no controle fisiológico da dor a nível espinal e na ação antinociceptiva opióide. Por outro lado, o ATP é um neurotransmissor liberado dos terminais nervosos sensoriais aferentes para atuar na circuitaria central da dor. Os nucleotídeos extracelulares podem ser hidrolisados por E-NTPDase e por 5’nucleotidase. Esta cascata enzimática apresenta dupla função: remover o sinal do ATP e gerar um segundo, adenosina. Esta dissertação teve como objetivos: investigar o efeito da administração repetida da morfina de P8 até P14 nas respostas nociceptiva, inflamatória, comportamental e na atividade das ectonucleotidases em medula espinhal e córtex de ratos em P16, P30 e P60. Além disso, avaliar o efeito de uma segunda exposição repetida de morfina por 7 dias em P80 na resposta nociceptiva, inflamatória e comportamental. Os animais que receberam a morfina por sete dias em P14 não desenvolveram tolerância, porém, quando avaliados em P80, demonstraram maior analgesia da morfina (administração aguda), e após 7 dias de administração diária de morfina (P86), desenvolveram o clássico fenômeno da tolerância. No teste da formalina, observou-se aumento na resposta nociceptiva em P30 e em P60, mas não em P16. No desempenho no campo aberto os animais apresentaram alteração de alguns comportamentos, tais como aumento de grooming em P16, e aumento de rearing e locomoção em P30. Em P88, o grupo que recebeu a morfina diariamente (de P80 até P86) apresentou aumento de rearing. Nas atividades das E-NTPDases, observou-se em P16 um aumento na atividade de hidrólise do ATP em sinaptossomas de córtex cerebral, e uma diminuição na atividade de hidrólise do ADP em sinaptossomas de medula espinhal. As alterações comportamentais observadas sugerem que a exposição à morfina, logo após o nascimento, pode desencadear, a curto e longo prazo, fenômenos tais como sensibilização comportamental. Em conclusão, estes resultados indicam que a exposição à morfina durante a segunda semana de vida pode promover aumento na resposta nociceptiva e alterações comportamentais que perderam ao longo da vida. Os efeitos observados após exposição repetida à morfina em animais podem colaborar com o entendimento de conseqüências clínicas decorrentes da administração em longo prazo de opióides. Recentemente foi demonstrado que a testosterona apresenta um efeito protetor ao diminuir o risco de ratos desenvolverem dor na Articulação Temporomandibular (ATM), o que explica pelo menos em parte, a menor prevalência de dor no sexo masculino. No entanto, o mecanismo através do qual a testosterona induz o efeito protetor em machos não é conhecido. Assim, o objetivo deste trabalho foi investigar se o efeito protetor da testosterona é mediado pela ativação do sistema opióide endógeno no sistema nervoso central e quais os subtipos de receptores opióides estão envolvidos nesse efeito protetor sobre o desenvolvimento de dor na ATM em ratos. Para o procedimento experimental foram usados ratos machos Wistar (230-300 g), intactos, gonadectomizados (Gx) e sham gonadectomizados (sham Gx) e a injeção de formalina 0,5% na ATM foi usada como estímulo nociceptivo. Para testar o envolvimento de um mecanismo neural central dependente da ativação do sistema opióide, os animais receberam injeção de naloxona, antagonista não seletivo de receptores opióides, CTOP, Naltrindole e Nor-BNI, antagonistas opióides seletivos para os subtipos de receptores opióides mu (µ ), delta e capa (µ ), respectivamente na região do núcleo sensorial trigeminal, antes do teste da formalina na ATM. A administração da Naloxona (15?g) antagonista não seletivo ou a combinação dos antagonistas de receptor µ opióide CTOP (30µ g/10µ l) mais o de receptor µ opióide Nor- BNI (90µ g/10µ l) aumentou significativamente a nocicepção induzida pela formalina 0,5% na ATM em ratos sham Gx, mas não em ratos Gx (31.09%, n = 6 e 26,9%, n = 6; respectivamente) A resposta de ratos intactos a estes tratamentos foi semelhante à de ratos sham Gx. Em contraste, a administração de cada antagonista de receptores opióides sozinhos ou a combinação de CTOP (30µ g/10µ l) mais o antagonista do receptor delta opióide Naltrindole (90µ g/10µ l) ou de Nor-BNI (90µ g/10µ l) mais Naltrindole (90µ g/10µ l) não afetou a nocicepção induzida pela formalina 0,5% em ratos Gx, intactos e sham Gx. Estes resultados sugerem que o efeito protetor da testosterona no desenvolvimento da dor na ATM depende da liberação de opióides endógenos e a subseqüente ativação dos receptores opióides mu e capa no sistema nervoso central. Conclui-se que a ativação individual dos subtipos de receptores é insuficiente enquanto que a co-ativação dos receptores opióides µ e k é necessária para mediar o efeito protetor da testosterona / Abstract: Temporomandibular Joints (TMJ) dysfunctions are painful conditions involving the masticatory muscles and temporomandibular joint with higher prevalence, severity and duration in females. Recently it was shown that testosterone has a protective effect by reducing the risk of rat develop pain in TMJ, which explains at least in part, the lower prevalence of pain in males. However, the mechanism through which the testosterone induces protective effect in males is not known. Thus, the aim of this study was to investigate whether the protective effect of testosterone is mediated by activation of endogenous opioid system in the central nervous system and what subtypes of opioid receptors are involved in this protective effect on the development of TMJ pain in rats. For the experimental procedure Intact, gonadectomized and sham Wistar (230-300g) male rats were used and all experimental procedures were approved by the Ethics Committee in Animal Research at the UNICAMP. The TMJ injection of 0.5% formalin was used as a nociceptive stimulus. The nociceptive behavior was quantified for 45 minutes and used as a quantitative nociceptive behavior measure that was defined as the cumulative total number of seconds that the animal spent rubbing the orofacial region asymmetrically with the ipsilateral fore or hind paw plus the number of head flinches counted during the observation period. Administration of the opioid receptor antagonist naloxone 15 mg or the combination of the mu-opioid receptor antagonists CTOP (30µ g/10µ l) plus the kappa-opioid receptor antagonist nor-binaltorphimine (90µ g/10µ l), significantly increased TMJ 0.5% formalin-induced nociception in sham (31.09%, n=6 and 26.9%, n=6; respectively) but not in gonadectomized rats. The response of intact rats to these treatments was similar to that of sham rats. In contrast, the administration of each opioid receptor antagonist alone or the combination of CTOP (30µg/10µ l) plus the delta-opioid receptor antagonist Naltrindole (90µ g/10µ l) or of Nor-binaltorphimine (90µ g/10µ l) plus Naltrindole (90µ g/10µ l) did not affect TMJ 0.5% formalin-induced nociception in intact, sham and gonadectomized rats. These findings suggest that the protective effect of testosterone on TMJ pain development depends on the release of endogenous opioids and on the subsequent activation of mu and kappa opioid receptors in the central nervous system.
