Avaliação cardiorrespiratória e analgésica da ropivacaína isolada e associada ao fentanil ou ao tramadol, administrados pela via peridural em cães

A ropivacaína é um fármaco relativamente novo, ainda pouco utilizado em Veterinária. O fentanil é um opióide agonista e o tramadol é um opióide de ação mista. Neste experimento, oito cães foram tranqüilizados com acepromazina, submetidos à peridural com um dos protocolos a seguir: GR (ropivacaína), GRF (ropivacaína + fentanil), GRT (ropivacaína + tramadol), em volume total de 0,25 mL/kg. Durante o procedimento foram avaliados e comparados os seguintes parâmetros vitais (FC, f, temperatura retal, pressão arterial, e gasometria do sangue arterial), os bloqueios sensitivo e motor (latência e duração de ação), o grau de sedação, e a ocorrência de possíveis efeitos indesejáveis advindos da administração de ropivacaína isolada ou em associação. A diminuição mais intensa na FC ocorreu nos grupos GRF e GRT, e ocorreu hipotermia significativa nos animais do GRF. Todos os grupos apresentaram sedação, sendo severa nos grupos GRF e GRT. De maneira geral, o período de recuperação foi mais curto nos animais do grupo GRT do que nos demais. O GRT também foi o que apresentou bloqueio mais cranial. Foram observadas bradicardia, hipotermia e síndrome de Shiff- Sherrington no período trans-anestésico em animais de todos os grupos. Decorridas 24 horas de período pós-anestésico, não foram evidenciados efeitos indesejáveis, em nenhum dos grupos. GRF foi o grupo com maior duração de anestesia e analgesia, GRT apresentou a menor duração de anestesia com analgesia intermediária, e GR apresentou duração intermediária, com menor analgesia. / Peridural anesthesia is broadly applied in the Veterinary field, using the isolated local anesthetic or in combination with opiates capable to increase the analgesic effect. The ropivacaine is a relatively new drug, not much used in the Veterinary field yet. The fentanil is an agonist opiate and tramadol is a mixed action opiate. In this experiment, eight dogs were sedated with acepromazine and subjected to the epidural anesthesia with one of the following protocols: GR (ropivacaine), GRF (ropivacaine + fentanyl), GRT (ropivacaine + tramadol), in 0,25mL/Kg of total volume. During the procedure, following vital signs were evaluated and compared (heart rate, respiratory rate, rectal temperature, blood pressure and gasometry of arterial blood), the sensory and motor blockade (latency and length of action), level of sedation and the occurrence of possible side effects due to administration of ropivacaine individually or in combination with other drugs. The highest decrease in the heart rate occurred in the following GRF and GRT and also significant hypothermia in animals of GRF. All groups presented sedation, even severe in the period of recovering was shorter in the animals belonging to GRT than in others. The GRT was also the one that presented the most cranial block. Bradycardia, hypothermia and Shiff- Sherrington syndrom were observed in the transanesthetic period in animals belonging to all of the groups. Twenty-four hours after the postanesthetic period, no side effects were observed, in none of the groups. The GRF was the one with higher length of anesthesia and analgesia, GRT presented the length of anesthesia with intermediate analgesia and, GR group presented intermediate length, with lower analgesia.

Ropivacaína isolada ou associada à morfina, butorfanol ou tramadol pela via peridural em cadelas para realização de ovariosalpingohisterectomia

O presente trabalho teve como objetivo investigar a utilização da ropivacaína isolada ou em associação a diferentes opióides, na anestesia peridural de cadelas submetidas à ovariosalpingohisterectomia (OSH) eletiva. Participaram do estudo duplamente encoberto 32 cadelas sadias, adultas, de diferentes raças, pesando entre seis e 15 kg e pré-medicadas com acepromazina (0,05mg/kg, IM) associada ao midazolam (0,2mg/kg, IM), distribuídas em quatro grupos distintos: Grupo 1: ropivacaína: 0,3 mL/kg; Grupo 2: ropivacaína + morfina (0,1 mg/kg); Grupo 3: ropivacaína + butorfanol (0,1 mg/kg); e Grupo 4: ropivacaína + tramadol (0,5 mg/kg) administrados pela via peridural. Em cada momento experimental foram mensurados: freqüência cardíaca; freqüência respiratória; pressão arterial sistólica; temperatura retal; pressão parcial dos gases sangüíneos (arterial); pH sangüíneo; além da avaliação não-paramétrica do grau de sedação, grau de sangramento e de relaxamento muscular seguindo tabelas de escores. Os dados foram submetidos à ANOVA e comparados pelos testes de Kruskal-Wallis, Friedman, Dunn e Tukey (p< 0,05). Concluiu-se que a utilização da ropivacaína isolada ou associada à morfina, ao butorfanol ou ao tramadol pela via peridural não promoveu depressão cardiorrespiratória ou alterações hemodinâmicas significativas, sendo que a ropivacaína associada ao butorfanol permitiu a realização de OSH em cadelas. / The use of epidural local anesthesia has been reaching great emphasis for the last years, overcoat with the opioids using. This research ained the use of ropivacaine with or without association the different opioids, for epidural anesthesia biches submitted the elective ovariosalpingohisterectomy (OSH). 32 bitches tool part is this double-blind study, adult, different breed, weighing between 6 and 15kg and pré-medicated with acepromazine (0.05mg/kg, IM) associated to the midazolam (0.2mg/kg, IM), distributed in for different groups: Group 1: ropivacaine: 0.3 mL/kg; Group 2: ropivacaine + morphine (0.1 mg/kg); Group 3: ropivacaine + butorphanol (0.1 mg/kg); and Group 4: ropivacaine + tramadol (0.5 mg/kg) administered epidural. The following parameters were studied: heart frequency; breathing frequency; systolic arterial pressure; rectal temperature; blood gas partial pressures (arterial); blood pH; besides non-parametric of sedation grade, bleeding grade and muscular relaxation following tables scores. The results were submitted by ANOVA and compared by Kruskal-Wallis, Friedman, Dunn and Tukey test (p< 0.05). It was conclude that the use of only ropivacaine or associated with morphine, with butorphanol or tramadol for the epidural administration didn't promote depression cardiorrespiratory or significant hemodinamycs alterations and the ropivacaine associated to the butorphanol allowed OSH in bitches accomplishment.

Infusão intratecal de opióides para tratamento de dor crônica não decorrente de câncer / Intrathecal infusion of drugs for treatment of chronic nonmalignant pain

A infusão intratecal de fármacos analgésicos é método considerado útil no tratamento da dor decorrente do câncer. Entretanto, estudos sobre eficácia no tratamento prolongado da dor crônica não decorrente de câncer são escassos. Este trabalho objetivou analisar prospectivamente o resultado do tratamento de 80 doentes com dor crônica não decorrente de câncer com infusão intratecal de morfina. Os resultados foram avaliados quanto à intensidade, características e etiologias da dor, qualidade de vida e complicações dos procedimentos; 42 doentes eram do sexo masculino, a média das idades foi de 48,4 anos e a duração média da condição álgica foi de 53 meses. A dor decorreu de mielopatia em 26,3% dos doentes, de síndrome dolorosa miofascial em 6,3%, de síndrome dolorosa pós-laminectomia em 23,8%, de síndrome complexa de dor regional em 8,8%, de síndrome fibromiálgica em 13,8% e de neuralgia pós herpética em 5,0%. Apresentavam dor neuropática 49 (61,2%), nociceptiva 19 (23,8%) e mista 12 (15%) pacientes. Foram implantadas 62 bombas de acionamento digital para infusão em bolo e 18 bombas de infusão contínua (gás) ou programável. As médias das intensidades da dor reduziram-se de 9,5 para 4,6 segundo a escala visual analógica (EVA) ao final do acompanhamento que variou de 18 a 98 meses (média = 46,7 meses); houve melhora significativa da dor nos doentes com dor neuropática (p < 0,001), nociceptiva (p < 0,001) ou mista (p = 0,005). Apesar da melhora da qualidade de vida de acordo com SF-36 (30,8 para 49,6) e nas dimensões do Questionário \"Treatment of Pain Survey\" (TOPS), não houve alteração na capacidade objetiva para o trabalho. Não houve diferença significativa entre infusão contínua e em bolo quanto à melhora da dor (p = 0,597). O consumo de morfina estabilizou-se após o sexto mês de tratamento na maioria dos casos. Não houve diferença significativa quanto à melhora em relação à localização da extremidade do cateter subaracnóideo (p = 0,227). Ocorreu agravamento da dor de 4,9 para 8,9 (p < 0,001) durante o período de uso de medicação placebo. Alguns efeitos adversos ocorreram inicialmente e geralmente foram toleráveis. Conclui-se que a infusão intratecal de opióides é método adequado e seguro para o tratamento da dor crônica rebelde não decorrente do câncer. / Implantable pumps for intrathecal delivery of opiates are efficient for treatment of cancer pain. However, studies of nonmalignant pain with long term follow-up are few. The present study use prospective analysis of the result of the long term treatment of 80 patients presenting nonmalignant pain with intrathecal infusion of morphine. The nature and etiology of the pain, quantitative and qualitative expressions of pain and the quality of the life before and at the end of the treatment and complications of procedures were evaluated; were male 42 (52%) patients, the average of the ages was 48.4 years and the mean duration of previous pain, 53 months. Pain was due to mielopathy in 26.3% of the cases, myofascial pain syndrome in 6.3%, failed back pain in 23.8%, complex regional pain syndrome in 8.8%, fibromyalgia in 13.8% and post-herpetic neuralgia in 5.0%. Presented as neuropathic pain 49 (61.2%) patients, as nociceptive pain 19 (23.8%) patients and as mixed pain 12 (15%) patients. In 62 patients pumps for self-administration bolus of morphine was implanted and in 18 constant-flow(gas) or programable pumps. The mean intensity of pain according the visual analogical scale (VAS) reduced from 9.5 to 4.6 at the end of 46.7 months (18 to 98 months) mean follow-up; there was significant improvement of the results in neuropathic(p < 0.001), nociceptive(p < 0.001) and mixed pain(p = 0.005). There was improvement of the quality of life measured by SF-36(30.8 to 49.6) and in all dimensions of the Questionnaire \"Treatment of Pain Survey\" (TOPS), except in working capacity. There was no significant difference of the results for patients treated with bolus or constant flow pumps (p = 0.597). The daily dose of morphine became constant after six month of treatment in the majority of the cases. The position of the tip of the cateter did not influenced improvement in pain intensity (p = 0.277). Patients treated with placebo had increasing of pain intensity from 4.9 to 8.9 according the VAS (p < 0,001). Side effects were more frequent at the beginning of the treatment and few were intolerable. Concluded that intrathecal infusion of morphine is a suitable and safe method for treatment of chronic nonmalignant pain.

Efeitos cardiorrespiratórios e motilidade gastrointestinal da infusão contínua de butorfanol em equinos anestesiados pelo isofluorano

Foram utilizados doze equinos adultos, distribuídos em dois grupos dois grupos, butorfanol (GB) e controle (GC). Os animais foram pré-medicados com xilazina, induzidos com quetamina associada ao midazolam e mantidos com isofluorano. Ao GB foi administrado butorfanol (0, 025 mg/kg IV) em bolus seguido de por infusão continua do fármaco (13µg/kg/h). As observações das variáveis foram feitas no momento basal e a cada 15 minutos por 75 minutos. Foi constatada redução na FC no M45 e M75 no GC e no M45 e M60 no GB comparados ao M0. A PAS, no GC, aumentou no M30 e M45 comparado ao M0. Já a PAD e PAM aumentaram em ambos os grupos quando comparadas ao valor basal. Houve redução da motilidade gastrointestinal por 60 minutos no GB. Foi observada elevação do cortisol plasmático em M60 e T30 em relação ao M0 em ambos os grupos. Os resultados obtidos permitiram concluir que a infusão contínua de butorfanol em equinos anestesiados pelo isofluorano promove discreta redução na motilidade gastrointestinal / The aim of this study was to evaluate the blood gases, cardiorespiratory effects, quality of recovery, changes in intestinal motility and plasma cotisol in isoflurane-anesthetized horses associated to continuous rate infusion of butorphanol and submitted to orchiectomy. Twelve adult horses distributed into two groups (BG) and control group (CG). The animals were premedicated with xylazine, induced with ketamine combined with midazolam and maintained with isoflurane. In the BG, butorphanol (0.025 mg-1 kg-1 IV) was administered followed by continuous rate infusion (13µg-1kg-1hour) of the opiod. Observations of the variables were carried out at baseline and every 15 minutes during 75 minutes. Reduction in HR was observed in M45 and M75 in the CG and M45 and M60 in BG compared to M0. The SAP increased in the CG at M30 and M45 compared to M0 whereas DAP and MAP increased in both groups compared to baseline. PaO2 and RT decreased over time in both groups. There was a reduction of gastrointestinal motility by 60 minutes in GB. And plasma cortisol was higher in M60 and T30 than M0 in both groups. The results allow conclude that the continuous rate infusion of butorphanol in horses anesthetized with isoflurane a modest intestinal motility reduction

Repercussão da substituição da infusão venosa de fentanil por metadona enteral sobre o tempo de desmame da ventilação mecânica em pacientes graves internados em unidades de terapia intensiva de adultos / Effect of substitution of intravenous infusion of fentanyl by enteral methadone on the time of weaning from mechanical ventilation in critically ill patients in intensive care units for adults

INTRODUÇÃO:Pacientes em ventilação mecânica (VM) são freqüentemente submetidos ao uso prolongado e/ou a doses elevadas de opióides, que durante a retirada podem causar abstinência dificultando o desmame da VM. Objetivo: testar a hipótese de que a introdução da metadona enteral na fase de desmame da sedação e analgesia em pacientes adultos graves sob VM diminui o tempo de desmame da VM. MÉTODOS: Estudo prospectivo, randomizado, controlado e duplo-cego, realizado entre abril de 2005 e outubro de 2009, em quatro Unidades de Terapia Intensiva (UTIs) de adultos de Joinville, SC. Foram randomizados 75 pacientes que apresentavam critérios para desmame da VM e estavam em uso de fentanil por mais de cinco dias consecutivos ou infusão ³ 5 g/kg/h por 12 horas. Os pacientes foram randomizados em dois grupos: Grupo Metadona (GM) e Grupo Controle (GC). Nas primeiras 24 horas após a inclusão os dois grupos receberam 80% da dose original do fentanil. Ao GM administrou-se metadona via enteral (10 mg cada 6 horas), e ao GC administrou-se placebo via enteral. Após as primeiras 24 horas acrescentou-se infusão intravenosa de solução salina (placebo) no GM, enquanto o GC recebeu infusão de solução intravenosa de fentanil. Em ambos os grupos, a solução venosa foi reduzida em 20% a cada 24 horas. Episódios de intolerância à retirada de opióide foram medicados com suplementação de opióide. Os grupos foram comparados entre si avaliando-se o tempo de desmame da VM, tempo de VM, permanência na UTI e permanência hospitalar. RESULTADOS: Dos 75 pacientes randomizados, sete foram excluídos e 68 foram analisados: 37 no GM e 31 no GC. Entre o início do desmame e a extubação, observou-se maior probabilidade de antecipação da extubação no GM, porém a diferença não foi significativa (Hazard Ratio: 1,52 (IC 95% 0,87 a 2,64; p = 0,11). Analisando-se o intervalo entre a randomização e o quinto dia do desmame da VM, a probabilidade de sucesso de desmame foi significativamente maior no GM (Hazard Ratio: 2,64 (IC 95%: 1,22 a 5,69; p < 0,02). Dentre os 54 pacientes que completaram o desmame da VM (29 do GM e 25 do GC), o tempo de desmame da VM foi significativamente menor no GM (Hazard Ratio: 2.06; IC 95% 1.17 a 3.63; p < 0.004). Não houve diferença entre os grupos com relação ao tempo de VM, permanência na UTI e permanência hospitalar. CONCLUSÃO: a introdução da metadona enteral na fase de desmame da sedação e analgesia de pacientes adultos graves sob VM resultou na diminuição do tempo de desmame da VM / INTRODUCTION: Patients on mechanical ventilation (MV) are often subjected to prolonged use and / or high doses of opioids, which when removed can cause withdrawal syndrome and to difficult weaning from MV. Objective: to test the hypothesis that the introduction of enteral methadone in weaning from sedation and analgesia in critically ill adult patients on MV decreases the time of weaning from MV. METHODS: Prospective, randomized, controlled, double-blind trial, conducted between April 2005 and October 2009 in ICUs of four hospitals in Joinville, SC. We randomized 75 patients who met the criteria for weaning from MV and were using fentanyl for more than 5 consecutive days or infusion ³ 5 g/ kg / h for 12 hours. Patients were randomized into two groups: Methadone group (MG) and Control Group (CG). At first 24 hours both groups received 80% of the original dose of fentanyl and received, additionally, enteral methadone (10mg qid) or enteral placebo. After the first 24 hours, MG: received enteral methadone (10mg qid) and intravenous placebo. CG received enteral placebo and intravenous fentanyl. In both groups, the blinded intravenous solutions were reduced by 20% of the original dose, every 24 hours. Episodes of intolerance opioid withdrawal were treated with supplemental opioid. The groups were compared by evaluating the time of weaning from MV, duration of MV, ICU and hospital stay.RESULTS: Of 75 randomized patients, 7 were excluded and 68 were analyzed: 37 at MG and 31 in CG. Between the beggining of weaning and extubation, there was a greater probability of anticipation of extubation in the MG, but the difference was not significant. (Hazard Ratio: 1,52 (IC 95% 0,87 a 2,64; p = 0,11). INTRODUÇÃO:Pacientes em ventilação mecânica (VM) são freqüentemente submetidos ao uso prolongado e/ou a doses elevadas de opióides, que durante a retirada podem causar abstinência dificultando o desmame da VM. Objetivo: testar a hipótese de que a introdução da metadona enteral na fase de desmame da sedação e analgesia em pacientes adultos graves sob VM diminui o tempo de desmame da VM. MÉTODOS: Estudo prospectivo, randomizado, controlado e duplo-cego, realizado entre abril de 2005 e outubro de 2009, em quatro Unidades de Terapia Intensiva (UTIs) de adultos de Joinville, SC. Foram randomizados 75 pacientes que apresentavam critérios para desmame da VM e estavam em uso de fentanil por mais de cinco dias consecutivos ou infusão ³ 5 g/kg/h por 12 horas. Os pacientes foram randomizados em dois grupos: Grupo Metadona (GM) e Grupo Controle (GC). Nas primeiras 24 horas após a inclusão os dois grupos receberam 80% da dose original do fentanil. Ao GM administrou-se metadona via enteral (10 mg cada 6 horas), e ao GC administrou-se placebo via enteral. Após as primeiras 24 horas acrescentou-se infusão intravenosa de solução salina (placebo) no GM, enquanto o GC recebeu infusão de solução intravenosa de fentanil. Em ambos os grupos, a solução venosa foi reduzida em 20% a cada 24 horas. Episódios de intolerância à retirada de opióide foram medicados com suplementação de opióide. Os grupos foram comparados entre si avaliando-se o tempo de desmame da VM, tempo de VM, permanência na UTI e permanência hospitalar. RESULTADOS: Dos 75 pacientes randomizados, sete foram excluídos e 68 foram analisados: 37 no GM e 31 no GC. Entre o início do desmame e a extubação, observou-se maior probabilidade de antecipação da extubação no GM, porém a diferença não foi significativa (Hazard Ratio: 1,52 (IC 95% 0,87 a 2,64; p = 0,11). Analisando-se o intervalo entre a randomização e o quinto dia do desmame da VM, a probabilidade de sucesso de desmame foi significativamente maior no GM (Hazard Ratio: 2,64 (IC 95%: 1,22 a 5,69; p < 0,02). Dentre os 54 pacientes que completaram o desmame da VM (29 do GM e 25 do GC), o tempo de desmame da VM foi significativamente menor no GM (Hazard Ratio: 2.06; IC 95% 1.17 a 3.63; p < 0.004). Não houve diferença entre os grupos com relação ao tempo de VM, permanência na UTI e permanência hospitalar. CONCLUSÃO: a introdução da metadona enteral na fase de desmame da sedação e analgesia de pacientes adultos graves sob VM resultou na diminuição do tempo de desmame da VM / INTRODUCTION: Patients on mechanical ventilation (MV) are often subjected to prolonged use and / or high doses of opioids, which when removed can cause withdrawal syndrome and to difficult weaning from MV. Objective: to test the hypothesis that the introduction of enteral methadone in weaning from sedation and analgesia in critically ill adult patients on MV decreases the time of weaning from MV. METHODS: Prospective, randomized, controlled, double-blind trial, conducted between April 2005 and October 2009 in ICUs of four hospitals in Joinville, SC. We randomized 75 patients who met the criteria for weaning from MV and were using fentanyl for more than 5 consecutive days or infusion ³ 5 g/ kg / h for 12 hours. Patients were randomized into two groups: Methadone group (MG) and Control Group (CG). At first 24 hours both groups received 80% of the original dose of fentanyl and received, additionally, enteral methadone (10mg qid) or enteral placebo. After the first 24 hours, MG: received enteral methadone (10mg qid) and intravenous placebo. CG received enteral placebo and intravenous fentanyl. In both groups, the blinded intravenous solutions were reduced by 20% of the original dose, every 24 hours. Episodes of intolerance opioid withdrawal were treated with supplemental opioid. The groups were compared by evaluating the time of weaning from MV, duration of MV, ICU and hospital stay.RESULTS: Of 75 randomized patients, 7 were excluded and 68 were analyzed: 37 at MG and 31 in CG. Between the beggining of weaning and extubation, there was a greater probability of anticipation of extubation in the MG, but the difference was not significant. Embora existam muitos estudos clínicos avaliando analgésicos e o controle da dor em cães, poucos são realizados em animais com dor do câncer e submetidos a procedimento cirúrgico para ressecção da neoplasia como a maxilectomia e mandibulectomia. Este estudo clínico foi realizado de forma prospectiva, comparativa, aleatória e de maneira simples cego com o propósito de avaliar a eficácia analgésica de diferentes tratamentos no período pós-operatório em cães submetidos à maxilectomia ou mandibulectomia. Foram utilizados no estudo 42 cães com neoplasia oral. Todos os animais foram prémedicados com acepromazina (0,05mg/kg) associado à meperidina (2mg/kg) por via intramuscular e a anestesia foi induzida com propofol por via iv na dose suficiente realizar a intubação (2.3-6.5mg/kg). O isoflurano foi utilizado para a manutenção da anestesia. Trinta minutos antes do fim do procedimento cirúrgico, os cães foram distribuídos aleatoriamente em um dos 5 diferentes grupos para analgesia pósoperatória: tramadol 2mg/kg (Tra), codeína 2mg/kg (Co), cetoprofeno 2mg/kg (Ce), tramadol 2mg/kg associado ao cetoprofeno 2mg/kg (TraCe) ou codeína 2mg/kg associado ao cetoprofeno 2mg/kg (CoCe), por via subcutânea. A freqüência cardíaca (FC) e respiratória (FR), pressão arterial sistólica (PAS), pressão arterial diastólica (PAD) e pressão arterial média (PAM), glicose sanguínea, cortisol e interleucina-6 (IL- 6) e grau de sedação foram verificados até 24 horas, e grau de analgesia foi verificado por até 120 horas do início da administração do analgésico, ou seja, os respectivos tratamentos foram mantidos por 5 dias da seguinte forma: tramadol ou codeína a cada 8 horas e cetoprofeno a cada 24 horas por via oral (MBL, M1, M2, M3, M4, M5, M24, M48, M72, M96 e M120). O resgate analgésico foi realizado nos animais que apresentaram escore de dor 4 em qualquer momento do estudo (dipirona 25mg/kg e morfina 0,1mg/kg). A análise estatística foi realizada por meio do Kruskal-Wallis, Friedman para mensurações repetidas, ANOVA e teste 2. Os gráficos em boxplot ou diagrama em caixas representam a distribuição dos dados. Os valores com p<0,05 foram considerados significantes. Não houve diferença entre os grupos de tratamento com relação ao peso, tempo de cirurgia, tempo para extubação, FC, FR, PAS, PAD e PAM, cortisol e IL-6 séricos, e escore de dor pela escala de análise descritiva. A concentração da glicose sanguínea aumentou de forma significante com relação aos valores basais no grupo Tra (M5= 96±14), Co (M1= 120±66 e M3=96±21), Ce (M5= 105±22) e CoCe (M3=104±16). Aumento do escore de dor foi observado no M2 do grupo Tra em relação a MBL e M1 a M5 do grupo Co em relação a M120 (p<0,05), contudo a média do escore não foi maior que 2,7. Baixo grau de sedação ainda foi observado no grupo CoCe no M24 (0,1±0,4 p<0,001) com relação ao M1. O número de resgate foi baixo, totalizando 19 administrações. No grupo Ce houve maior necessidade de resgate analgésico. Com base nos resultados obtidos, pôde-se concluir que os grupos de tratamento analgésico promoveram controle da dor pós-operatória de boa qualidade na maioria dos cães do estudo e com baixa incidência de efeitos adversos, podendo ser indicados no controle da dor em procedimentos de maxilectomia e mandibulectomia. / Although there are many clinical studies evaluating analgesics and pain control in dogs, very few were carried out in animals with cancer pain, and submitted to oncologic surgery with tumor resections such as maxillectomy and mandibulectomy. This clinical, prospective, randomized, simple blinded study was performed with the purpose of evaluating analgesic efficacy of different treatments in the postoperative period in dogs submitted to maxillectomy or mandibulectomy. Forty-two client-owned dogs with oral tumor were used in the study. Dogs were premedicated with acepromazine (0.05mg kg-1) and meperidine (2mg kg-1) by the intramuscular route and anesthesia was induced with intravenous propofol in a dose sufficient to allow intubation (2.3-6.5mg-1). Isoflurane was used for maintenance of anesthesia. Thirty minutes prior to the end of surgery, dogs were randomly allocated in one of 5 different groups for postoperative analgesia: tramadol 2mg kg-1 (Tra), codeine 2mg kg-1 (Co), ketoprofen 2mg kg-1 (Ke), tramadol 2mg kg-1 + ketoprofen 2mg kg-1 (TraKe) or codeine 2mg kg-1 + ketoprofen 2mg kg-1 (CoKe), subcutaneously. Heart (HR) and respiratory (RR) rates, systolic (SBP), median (MBP) and diastolic (DBP) blood pressures, blood glucose, serum cortisol and interleukin-6 (IL-6) and degree of sedation were recorded for 24 hours, and degree of analgesia were evaluated until 120 hours of the start of analgesic administration (MBL, M1, M2, M3, M4, M5, M24, M48, M72, M96 and M120), being that treatments were maintained for 5 days as follows: codeine or tramadol every 8 hours and the ketoprofen every 24 hours orally. Analgesic rescue was delivered to animals with pain scores equal or superior to 4 at any time of the study (dypirone 25mg -1 and morphine 0,1mg-1). Statistical analyses were performed by means of the Kruskal-Wallis, Friedmann for repeated measures, ANOVA and 2 tests. Graphics boxplot or box diagrams represents dates of distribution. Values of p<0.05 were considered significant. There were no differences between groups related to weight, surgical time, extubation time, HR, RR, SBP, MBP, DBP, serum cortisol and IL-6, and pain score by Descriptive Scale (DS). Blood glucose concentrations were significantly increased in relation to baseline, in groups Tra (M5= 96±14), Co (M1= 120±66 e M3=96±21), Ke (M5= 105±22) and CoKe (M3=104±16). Increase of pain score was observed in M2 of group Tra in relation to baseline, and M1 to M5 of group Co in relation to M120 (p<0,05), however the average score was not higher than 2.7. Low level of sedation was also observed in group CoKe in M24 (0.1 ± 0.4 - p <0.001) compared to M1.. The number of rescue was low, totaling 19 administrations. Ke group required more analgesic rescue. So, it can be conclude that treatment analgesic groups promoted a good quality pain control of postoperative in most of the dogs in the study and with low incidence of side effects, could be indicated in the control of the pain in procedures of maxillectomy and mandibulectomy.
