Living with a Stoma After Surgical Resection for Rectal Cancer: An Interpretive Phenomenological Study to Understand How Stomas Can Affect Physical Activity

The benefits of physical activity for cancer survivors are numerous and well-substantiated. However, it is unclear how to accommodate specific cancer treatments, such as a stoma as part of treatment for rectal cancer, in order to be physically active. To bridge this gap, this study undertook an interpretive phenomenological analysis to explore the experience of 15 rectal cancer survivors' engagement in physical activity with a stoma. Overall, three themes were found: 1) understanding the drive and the motivation to be physically active, 2) despite being motivated to be physically active with a stoma it can be challenging, and 3) how to be physically active with a stoma: lessons learned. The findings suggest rectal cancer survivors with a stoma require motivational support and enhanced competence and self-efficacy in order to engage in greater physical activity. Incorporating these skills into current information and supportive services targeting rectal cancer survivors with a stoma may help promote physical activity in this population.

Att vara självständig men inte oberoende : en intervjustudie om upplevelse av information och stöd hos personer med stomi

Sammanfattning Syftet med studien var att beskriva hur stomiopererade personer upplever stöd och informa­tion, relaterat till skötseln av stomin. Studien hade en kvalitativ ansats med beskrivande design. Tio personer med stomi från två hälsocentraler i Mellansverige deltog i studien. Data samlades in via intervjuer med semi­strukturerade frågor och analyserades med kvalitativ innehållsanalys. Resultatet redovisades utifrån temat Att vara självständig men inte oberoende och sex kategorier: Att få information som är individuellt anpassad, Att personer i omgivningen kan och känner till, Att ha tillgång till kompetent personal, Att inte få veta eller förstå, Att känna ensamhet samt Att känna sig osäker. Temat beskrev att personer med stomi strävade efter att bli självständiga i sin egenvård av stomin men att de samtidigt hade behov av omtanke, kunskap och förståelse i sin nära omgivning. Individuellt anpassad information var av stor betydelse och det uppskat­tades att bli bemött som en medmänniska av kompetent och tillgänglig personal. Några hade stöd av familj och vänner medan andra kände sig väldigt ensamma. Ibland var det svårt att förstå informationen och det ledde till svårigheter. Den första tiden i hemmet kände sig samt­liga väldigt osäkra att utföra sin egenvård av stomin och stödet och informationen upplevdes bris­tande. Kontinuerligt stöd, information och uppföljning i hemmet skulle underlätta tillvaron och rehabiliteringen för personer med stomi

Upplevelser av att leva med tarmstomi : En litteraturstudie / Experiences of living with an intestinal stoma

Background: Intestinal diseases that can cause a person to receive a stoma may include colorectal cancer, Crohn's disease and Ulcerative colitis. Aim: The aim of the study was to describe experiences of living with intestinal stoma. Method: A qualitative literature study in which eleven scientific articles have been analyzed according to Fribergs (2006a) analysis model. Result: The result was based on two categories which are experiences of altered body image, and experienced changes in social life. The result showed that patients with stoma felt that their body image changed and it was hard to accept the altered body image and thereby self image. Difficulties in controlling emissions, odor and leakage contributed to the experience of loss of control with limited social life as a result. The stoma also resulted in a change in everyday life which required careful planning. The sexual activity was affected due to altered body image. Despite the constraints of everyday life support, a functioning self care and acceptance contributed to a better quality of life. Conclusion: It can be concluded it's clear from the result that it is important for the patient to receive accurate information both before and after ostomy to get an adequate picture of what it means to live with a stoma. By supporting the patient and customizing information and creating a patient-centered care quality of life can significantly improve.

Det förändrade livet : En litteraturstudie om vuxna personers upplevelser av att leva med stomi

Bakgrund: Det finns ungefär 25000 stomiopererade personer i Sverige. Stomi innebär att tarmens naturliga tömningsväg ändras genom en kirurgisk anläggning av öppning på buken. Det finns olika typer av stomi och orsaken till stomibildning är oftast inflammatoriska tarmsjukdomar eller cancer. Syfte: Att beskriva vuxna personers upplevelser av att leva med stomi samt att beskriva vilka datainsamlingsmetoder som har använts i de utvalda artiklarna. Metod: En beskrivande litteraturstudie som byggt på 14 vetenskapliga artiklar, varav tretton med kvalitativ ansats och en med kvantitativ ansats. Huvudresultat: Resultatet av föreliggande studie visar att personers liv förändrades genomgripande på grund av stomi. Patientutbildning och information samt sjuksköterskans bemötande och kompetens kring området behöver förbättras. Många kämpade för att återställa ett normalt liv trots stomi. Utifrån den metodologiska frågeställningen framkom att de studierna med kvalitativ ansasts hade intervjuer som datainsamlingsmetod och den kvantitativa studien samlade in data via enkät. Slutsats: Stomibildning innebär en genomgripande livsförändring för personer då alla aspekter av livet påverkas. Trots stomi kämpade många för att återställa ett normalt liv. Patientutbildning och information samt sjuksköterskansarbete behöver förbättras. Föreliggande studie ger en djupare förståelse för personer som har stomi och kunskapen kan gynna sjuksköterskans arbete och därmed höja vårdkvaliteten. / Background: There are about 25,000 ostomy-operated people in Sweden. An ostomy is a surgically created opening on the abdomen for the discharge of body wastes. There are different types of ostomies and the cause of ostomy formation is usually inflammatory bowel disease or cancer. Purpose: To describe adults’ experiences of living with an ostomy and to describe which data collection methods have been used in the selected articles. Method: A descriptive literature study based on 14 scientific articles, of which thirteen articles have qualitative approach and one article has quantitative approach. Main result: The result of the present study shows that the people’s lives have changed profoundly due to ostomy. Many struggled to restore a normal life despite ostomy. Patient education and information as well as nurses’ attitude and knowledge need to be improved. Based on the methodological question, it was found that the studies with qualitative approach had interviews as data collection methods and the quantitative study collected data by using questionnaire. Conclusion: Ostomy formation means a profound change of life for people when all aspects of life are affected. Many struggled to restore a normal life. Patient education and information and nurses work need to be improved. The present study provides a deeper understanding of people who have ostomy and the knowledge can benefit nurses’ work and thus raise the quality of care.

Background: Ostomy surgery refers to surgical procedures that reroute the elimination process of the bowel and alters the usual form of elimination. There are different reasons why a person gets a ostomy, depending on the reason for the surgery. The number of persons living with ostomys in Sweden is unknown. However, the reaction to intestinal diversion surgery can be a devastating experience. Living with a ostomy for a longer period may affect the individual. Nurses are important members of the health care team and have a significant role in caring for patients with ostomy. Aim: The aim was to describe experiences living with a permanent ostomy. Method: A qualitative litterature-based design was used in the study. CINAHL and PubMed databases were used to search qualitative articles. Articles were analyzed and reviewed by the five-step method in Friberg. Results: The result is based on ten qualitative articles. After making the analysis three main themes and nine sub-themes were identified. The three main themes were: the altered body appearance, the changing of daily life and interference with anticipated return to normal. The sub- themes were: fear, anxiety, disgrace and irritated skin, body image, acceptance, independence, daily activities, diet, impact on relationships, supporting, loneliness and isolation. Conclusion: Having a ostomy is a major event and it affects personal self-concept and restricts their lives in various ways. Nurses need to provide education, support advice and referral for specialist help if required to promote health and reduce suffering.

Diversas vezes, como artifício de cura de algumas doenças do aparelho intestinal ou do trato urinário, um procedimento cirúrgico nomeado por estomia é uma técnica médica utilizada para manter o paciente em vida. Este procedimento acarreta em uma série de questões físicas e psicológicas para o estomizado, cuja falta de informação e de produtos modernos disponíveis no mercado podem agravar a situação. Através destas constatações, surgiu a intenção de criar produtos de moda, no segmento moda praia, para mulheres que passaram pela cirurgia em questão. Mas o intuito de desenvolvimento de tais produtos fez surgir, ainda, a constatação de outro problema: a carência de um método específico da moda que levasse em consideração as necessidades reais do usuário em questão. Enquanto as metodologias de design de produto têm focado no usuário, os métodos de projeto de moda costumam ignorar a pesquisa e as necessidades humanas, identificando o desenvolvimento com o estilo de quem o projeta. A possibilidade da constatação de necessidades reais do usuário, em projetos de moda, pode ser um caminho promissor se forem considerados os princípios do design. Sendo assim, o presente estudo teve como objetivo geral: desenvolver um método adaptado, embasado em métodos já consagrados do design e da moda, destinado para a criação de produtos de moda que possam atender às necessidades reais do usuário. Neste caso, para a criação de produtos no segmento moda praia que contemplem os anseios de mulheres com estoma. A criação e a aplicação da referida adaptação propiciou o desenvolvimento facilitado dos produtos pretendidos. Pretende-se que o método gerado seja válido para projetos de moda que, assim como este, possuam uma problemática bem definida e uma verificação de necessidade do usuário, assim como acontece nos projetos de design. / In the curing process of many diseases of the intestinal or urinary system, several times a surgical procedure known as ostomy is the only way to keep the patient alive. This procedure includes a series of physical and psychological issues, whose the lack of information and modern products available on the market can aggravate the situation. Through these findings, the intention was to create fashion products, in the beachwear segment, for women with stoma. But the development of these products led to the discovery of another problem: the lack of a specific fashion method that takes into account the real needs of the user. While product design methodologies have focused on the user, fashion design methods have ignored research and human needs. So, the development is identified in the style of the designer. The possibility of verifying the user's real needs in fashion projects can be a promising way if the designer considers the principles of design. Therefore, the present study aimed to develop an adapted method, based on methods of design and fashion, aimed at the creation of fashion products that can meet the real needs of the user - in this case, for the creation of products in the beachwear segment that contemplate the desires of women with stoma. The creation and implementation of this adaptation has facilitated the further development of products. It was intended that the method created be valid for fashion projects that, like this one, have a well-defined problem and a user need, as well as in the design projects.

Considerando-se a falta de um instrumento de avaliação de qualidade de vida específico para pessoas estomizadas no Brasil, verificado após revisão bibliográfica de estudos sobre o tema em nosso meio, o objetivo deste estudo foi realizar a adaptação cultural e validar o instrumento City of Hope Quality of Life Ostomy Questionnaire (COH-QOL-OQ), para a língua portuguesa no Brasil. Após contato com a autora do instrumento original e obtenção de sua autorização para o desenvolvimento do processo de adaptação cultural e validação, o projeto recebeu também autorização do Comitê de Ética em Pesquisa da Secretaria Municipal de Saúde de Porto Alegre/RS. É um estudo do tipo metodológico, cujas etapas para adaptação cultural foram baseadas em Beaton et al (2002), modificado conforme preconizado e utilizado no Instituto Municipal d´ Investigación Médica de Barcelona (IMIM). Assim o processo de adaptação cultural, que caracterizou a primeira parte deste estudo, seguiu os seguintes passos: tradução, avaliação por comitê de juízes, grupo focal com pessoas estomizadas e retrotradução. A versão retrotraduzida foi aprovada pela autora do instrumento original, Dra. Márcia Grant e essa versão foi aplicada a 215 pessoas estomizadas, em três municípios do estado do Rio Grande do Sul, para avaliação de algumas de suas propriedades psicométricas. A consistência interna do instrumento, analisada por meio do coeficiente alfa de Cronbach (> 0,70), resultou nos escores 0,92 para QV Total e 0,82, 0,86, 0,83, 0,79 para os domínios BES, BEP, BEF, BEE, respectivamente. A validade de conteúdo foi avaliada por meio do nível de concordância entre os juízes, obtendo-se 70% A validade de critério concorrente, avaliada por meio da correlação (Pearson e Spearman) entre o item 16 do domínio BEP e os domínios do COH-QOL-OQ, verificando-se correlações entre 0,26 e 0,66. Quanto à validade de construto convergente por meio da correlação entre os domínios do COH-QOLOQ e do WHOQOL-Abreviado (coeficientes de correlação de Pearson e Spearman > 30) - os resultados variaram de 0,41 a 0,77. Finalmente, a validade de construto discriminante, em que se utilizaram os testes de Mann-Whitney, Kruskal-Wallis, tstudent e ANOVA, obteve-se discriminância na comparação para o tempo de estomizado, o caráter da estomia,, a situação conjugal, a prática da religião. Os resultados permitiram concluir que o COH-QOL-OQ teve desempenho satisfatório, atestando sua confiabilidade e mostrando-se válido para medir a QV de pessoas estomizadas. / One considering the lack of an evaluation tool of specific quality of life for ostomized people in Brazil, verified after a bibliographical review of studies about the theme in our field, the goal of this study was to carry out the cultural adaptation and to valid the instrument City of Hope Quality of Life Ostomy Questionnaire (COH-QOL-OQ). After contacting the author of the original instrument and obtaining her permission, though it was an instrument of free use and developing of the cultural adaptation process, as shown in the site of the City of Hope National Medical Center (COHNMC), the project has also received authorization from the Committee of ethics in researches of the Municipal Health Secretary of Porto Alegre/RS. It is a study of methodological kind that was carried out based on the steps for cultural adaptation of Guillemin, Bombardier and Beaton adapted for Beaton et al and yet adjusted by the steps followed by the Municipal Institute of Medical Investigation of Barcelona (IMIM). Therefore, the process of cultural adaptation, that has described the first phase of this study, followed the following steps: translations, evaluation by a committee of judges, focal group with ostomized people and retro translation. The retro translated version was approved by the author of the original instrument, Dra. Marcia Grant and such version was clinically applied to 215 ostomized people in three towns of the State of Rio Grande do Sul for evaluation of the psychometric properties of measure. Reliability was analyzed through intern consistency given by coefficient alfa of Cronbach ( > 70%), that resulted in scores 0,92 for QV Total and 0,82, 0,86, 0,83, 0,79 for dominions BES, BEP, BEF, BEE, respectively. One has also evaluated the validity of the content with acceptance of the level of agreement among the judges around 70%. The validity of the opponent criteria was evaluated by the correlation among the item 16 of BEP dominion and the dominions of the COH-QOL-OQ and the validity of the convergent construct through the correlation among the dominions of the COH-QOL-OQ and of WHOQOL- Abbreviated (coefficients of correlation of Pearson and Spearman > 30). The results obtained were correlations between 0,26 to 0,66 for the opponent validity and 0,41 to 0,77 for the convergent validity. The validity of the discriminatory construct was evaluated by tests of Mann- Whitney, Kruskal-Wallis, t-student and ANOVA and there was discrimination when the dominions scores were compared with time of ostomized, sort of ostomy, marital status, religion practice. Concerning the type of ostonomy, income, education, work, religion and age there were no discrimination. One is able to conclude that the COH-QOL-OQ had a good performance, certifying its reliability and showing itself valid to measure the QV construct of ostomized people.
