Effekter av Överbeläggningar på Akutmottagningar: Sjuksköterskans Upplevelse : En litteraturöversikt

Sjuksköterskor har en central roll i att tillgodose patientens vårdbehov. Det primära målet inom hälso- och sjukvården är att erbjuda en säker och god vård till alla landets invånare. Det finns tydliga kopplingar mellan överbeläggningar på akutmottagningar och stress hos sjuksköterskor. När sjuksköterskorna påverkas av den arbetsrelaterade stressen så kan det begränsa förmågan att erbjuda högkvalitativ vård och patientsäkerhet. Därför är syftet med studien att undersöka effekten av överbeläggningar inom akutsjukvård ur sjuksköterskans perspektiv. Studiens metod är en litteraturöversikt som omfattar tolv artiklar. Resultatet består av tre huvudkategorier och sex subkategorier som tillsammans belyser olika faktorer som kan påverka sjuksköterskors upplevelser av stress och dess påverkan på sjuksköterskans arbete samt psykiska mående. Resultatet belyser att överbeläggningar är ett globalt dilemma inom vården och påverkar både sjuksköterskor samt patienter. Överbeläggningar och arbetsrelaterad stress bidrar till svårigheter för sjuksköterskorna att tillhandahålla god och holistisk vård. Förekomsten av överbeläggningar bidrar till bristande resurser, samarbete och kommunikation inom teamet, vilket bidrar till försämrad vårdkvalitet och patientsäkerhet. Grundat på litteraturstudiens resultat framkommer att stressfaktorer på överbelagda akutmottagningar är bristande utrustning, bemanning och kompetens. Dessa faktorer bidrar till en minskad patientsäkerhet och hotar integriteten i vården, vilket för sjuksköterskorna leder till att fler lämnar yrket. Genom att tillhandahålla bättre förutsättningar och organisatoriska förändringar kan samhället bidra till bättre vårdmiljö och därav öka patientsäkerheten. / Nurses play a pivotal role in meeting patients' healthcare needs. The primary goal in healthcare is to offer safe and high-quality care to all residents of the country. There are clear links between overcrowding in emergency departments and stress among nurses. Whether nurses are affected by work-related stress can significantly impact their ability to provide high-quality care and influence patient safety. Hence, the purpose of this study is to examine the effect of overcrowding in the emergency department from the perspective of the nurse. The study's methodology involves a literature review comprising twelve articles. The results encompass three main categories and six subcategories that collectively highlight various factors influencing nurses' experiences of stress and its impact on their work and mental well-being. The findings illuminate that overcrowding is a global healthcare dilemma affecting both nurses and patients. Overcrowding and work-related stress contribute to challenges for nurses in delivering comprehensive and excellent care. The presence of overcrowding leads to inadequate resources, teamwork, and communication within the team, consequently diminishing the quality of care and patient safety. Based on the literature study results, it is discussed that stressors in overcrowded emergency departments include insufficient equipment, staffing, and expertise. These factors contribute to decreased patient safety and jeopardize the integrity of healthcare, resulting in more nurses leaving the profession. By providing better conditions and implementing organizational changes, society can contribute to a better healthcare environment and thereby enhance patient safety.

Addressing low matriculation pass rates in the Eastern Cape province : an education management perspective

In South Africa, the Eastern Cape Province (ECP) has shown the lowest matriculation pass rates for most of 1994 to 2002. This has led to great academic and media focus on the managerial and leadership role of school principals, which became the inspiration for this education management study. The researcher investigated a school principal's efficiency and effectiveness in a school system, and the detrimental impact on learner academic achievement should these vital skills be lacking. Thus, the role of the school principals at schools with low matriculation pass rates was probed to reveal shortcomings in the level of their commitment, and how these could be rectified. The researcher completed a qualitative research study at four schools with low matriculation pass rates in the Eastern Cape Province. The empirical study confirmed that learners at many schools with low matriculation pass rates are subjected to poor infrastructure and overcrowding. Furthermore, the school principals did not create school atmospheres conducive to teaching and learning, and neglected to foster open communication between the school stakeholders. The study found that the school principal's management and leadership role was not based on the belief that all learners are worthy of being taught under the best possible educational conditions. The teachers are required to promote learner academic interests, made possible through efficient classroom management, to sustain teacher quality, and to ensure learner academic achievement, yet they are subjected to poor working environments and ill-disciplined learners. This study nevertheless emphasises that all school stakeholders are accountable for low matriculation pass rates, but that it is the responsibility of the school principals to institute corrective measures. It highlights that parents can enhance learner academic progress through their active involvement in their children's learning process and the provision of home environments conducive to learning. There is a need to guide school principals and school stakeholders to make every educational effort to improve the current low matriculation pass rates in the ECP. Relevant recommendations and a suggested model to rectify the situation are proposed to ensure that all learners receive the quality of education they deserve. / Faculty of Education / D.Ed. In this research project the overcrowding problem in South African prisons will be analysed as well as the influence it had on the rehabilitation of offenders. The problem of overcrowding in South Africa's prisons is very clear when the approved accommodation of a prison is compared with the actual number of prisoners incarcerated. Prison sentences are a favoured form of punishment throughout the world. More and more people seek alternatives to incarceration in order to reduce prison overcrowding. The time to finalize criminal cases, releases on bail, the abolishment of minimum sentences and better utilization of community based sentences are some of the issues which can reduce overcrowding. One of the concepts that was implemented to establish rehabilitation within prisons was Unit Management. The number of prisoners incarcerated makes it difficult for Unit Management to be effective. In some cases prisoners are kept under inhumane conditions which is a violation of the Constitution of South Africa. Currently the morale of members of Correctional Services is very low and this has an influence on the way in which they treat prisoners. These members are supposed to be an example to prisoners but instead their bad behaviour has a negative influence on prisoner's discipline. Correctional Services goal is to make every member a rehabilitator but overcrowding will first have to be reduced. Individual attention to prisoners is one of the building blocks of Unit Management but overcrowding of prisons creates a member/prisoner ratio which is far too high to be effective with rehabilitating prisoners. Community based sentences can also be utilized more frequently by the courts. It can better rehabilitation possibilities and through this the objectives of punishment are met. In the past the focus of South African Law was on the criminal. With the Correctional Services Act (Act 111/1998) this focus was shifted to the victim as well. Both these parties can now come together and discuss the offence as well as the influence it had on the victim. International and national resources on the application of Restorative Justice both in Corrections and as pre-sentence option had been studied. The background of the penal system and the development of Restorative Justice as part of the Criminal Justice System are investigated by looking at ancient practices, indigenous and modern practices. The study intends to draw on experience from other countries, while at the same time advocate for the development of a uniquely South African model or practice. The role of the most important stakeholders, namely victim, offender and community is emphasized with reference to the challenges that offenders face in terms of reintegration and the lack of resources in communities to enable them to honour Restorative Justice agreements. / Criminology / M. The impact and constitutionality of delayed trials on the rights of a suspect or accused person during criminal proceedings

The Constitution of the Republic of South Africa guarantees every person a fair trial; the right to a fair trial right trial must begin and conclude within a reasonable time and without undue delay. Internationally the same guarantees and protections are available to unconvicted suspects. However, the South African criminal justice system lacks behind internationally and falls short of promoting these guarantees. Investigation was done on delays in commencing and finalising trials in light of constitutional provisions, the consequence and the impact of the delay with discussion on prison conditions and overcrowding with reference to the Constitutiton, legislation and case law. Delayed trial, prison overcrowding and poor prison conditions are still an issue in South Africa and there needs to be positive change to enforce and practice prescribed directives.
