Understanding exposure to pharmacogenetically actionable opioids in primary care

Indiana University-Purdue University Indianapolis (IUPUI) / Pharmacogenetic testing has the potential to improve pain management through addressing wide interindividual variations in responses to pharmacogenetically actionable opioids, ultimately decreasing costly adverse drug effects and improving responses to these medications. A recent review of pharmacogenomics in the nursing literature highlighted the need for nurses to more fully embrace the burgeoning field of pharmacogenomics in nursing research, clinical practice, and education. Despite the promise of pharmacogenetic testing, significant challenges exist for evaluating outcomes related to its implementation, including oversimplification of medication exposure, the complexity of patients' clinical profiles, and the characteristics of healthcare contexts in which medications are prescribed. A better understanding of these challenges could enhance the assessment and documentation of the benefits of pharmacogenetic testing in guiding opioid therapies. This dissertation is intended to address the challenges of evaluating outcomes of pharmacogenetic testing implementation and the need for nurses to lead pharmacogenomic-related research. The dissertation purpose was to advance the sciences of nursing, pain management, and pharmacogenomics through the development of a typology of common patterns of medication exposure to known pharmacogenetically actionable opioids (codeine & tramadol). A qualitative, person-oriented approach was used to retrospectively analyze six months of electronic health record and pharmacogenotype data in 30 underserved adult patients. An overarching typology with eight groups of patients that had one of five opioid prescription patterns (singular, episodic, switching, sustained, or multiplex) and one of three types of medical emphasis of care (pain, comorbidities, or both) were identified. This typology consisted of a description of multiple common patterns that compare and contrast salient factors of exposure and the emphasis of why individuals were seeking care. Furthermore, in an aggregate descriptive analysis evaluating key clinical profile factors, these patients had complex medical histories, extensive healthcare utilization, and experienced significant polypharmacy. These findings can aid in addressing challenges related to the implementation of pharmacogenetic testing in clinical practice and point to ways in which nurses can take the lead in pharmacogenomics research. Findings also provide a foundation for future studies aimed at developing medication exposure measures to capture its dynamic nature and identifying and tailoring interventions in this population.

Syftet med denna litteraturstudie var att undersöka tillgänglig litteratur beträffande taktil beröring som alternativ smärtbehandlingsmetod samt vilka effekter taktil beröring har på patientens smärtupplevelse och för vårdgivande personal. Metoden som användes var Goodmans modell för evidensbaserad litteraturgranskning. Tio artiklar granskades enligt kriterier av Polit och Beck (2006) samt Willman et al (2006). Resultat visar vikten av beröring som metod för minskning av smärta, samt andra effekter som förbättrad sinnestämning och ökad livskvalitet hos patienter. Positiva och negativa effekter av beröringsbehandling för vård-personalen har tagits upp i några inkluderade studier. / The purpose of this literature study was to examine available literature regarding tactile touch as an alternative pain treatment method, and also the effects of tactile touch on the patients´ pain experience and to the care giving personnel. The method used was Goodman´s model for evidence based literature examination. Ten articles were examined following the criteria described by Polit and Beck (2006) and Willman et al (2006). The result shows the importance of tactile touch as a method for reducing pain and other effects as improved mood and life quality in patients. Some positive and negative effects by tactile touch treatment to the care giving personnel are also considered in some of the examined studies.

Fördelar och risker med smärtlindring vid akut buksmärta innan läkarbedömning - en systematisk litteraturstudie

Bakgrund: Akut buksmärta är en av de vanligaste sökorsakerna på en akutmottagning. Trots det är smärtlindringen till denna patientkategori omdiskuterad då smärtlindring innan läkarbedömning anses äventyra patientens hälsa och riskerar att maskera potentiellt livshotande tillstånd trots att forskning de senare årtiondena visat på motsatsen. Syfte:  Syftet med litteraturstudien var att undersöka fördelar och risker med smärtlindring administrerad av sjuksköterska innan läkarbedömning vid akut buksmärta. Metod: Systematisk litteraturstudie som granskat kvantitativa artiklar i ämnet med företrädesvis randomiserade kontrollerade studier beskrivna. De granskade artiklarna har sökts upp i medicinska databaser med hjälp av den strukturerade sökmetoden PICO. Artiklarnas resultat har granskats med innehållsanalys. Resultat: Totalt 12 vetenskapliga artiklar inkluderades och utgjorde resultatet i litteraturstudien. I resultatet framkom tre kategorier och en underkategori som belyste smärtskattning och smärtlindring, biverkningar och komplikationer, påverkan på diagnosbeslut och patientbelåtenhet. Slutsats: Tidig smärtlindring med opioider och paracetamol smärtlindrar patienten adekvat och försenar inte klinisk bedömning och efterföljande diagnosbeslut av läkare vid akut buksmärta.

Pain : psychological measurement and treatment

This research was executed as three separate studies. Study 1 focused on the perception of pain and the semantic aspects of pain. Study 2 focused on the measurement of acute pain and mood states. Study 3 focused on the psychological treatment of cancer pain. In Study 1 a group of 66 Setswana-speaking adults were required to describe what they saw, what happened, and what would be the outcome with respect to three visually presented pain scenes using The Pain Apperception Test (PAT) A qualitative analysis of their responses shows that pain is experienced as an all encompassing experience affecting all aspects of their lives, such as the physical, emotional, social, and economic. This was found to be true, irrespective of gender and age with the exception of economic issues. A qualitative analysis of their responses to the Pain Eliciting Incidents Questionnaire (PEIQ) reveals that the Setswana pain descriptors are classifiable according to the three dimensions of pain namely, the sensory-discriminative, affective-motivational, and cognitive-evaluative. Sludy 2 applied the Profile of Mood States (POMS) preoperatively to a group of 58 female laparotomy (gynaecological) patients. These patients were also tested post-operatively with the Visual Analogue Scale (VAS) and the Wisconsin Brief Pain Questionnaire (WBPQ) as pain measures. The pain measures were taken at no medication and at the peak of medication. Factor analysis could not confirm the validity of the six POMS scales. These scales also did not show correlations with post-operative pain. Correlations between the pain measures showed acceptable reliability and validity of the VAS and the WBPQ. In Study 3 three groups of 15 cancer patients each, suffering from chronic pain, were treated over a period of two weeks with either cognitive behavioural therapy plus medication, reassurance therapy plus medication, or medication only. Comparison of before and after treatment pain measures showed that both cognitive behavioural therapy and reassurance therapy had a beneficial effect. Follow-up results three months later showed that the beneficial effect of reassurance therapy did not persist. Η παρουσία σοβαρού ισχαιμικού πόνου πριν από τον ακρωτηριασμό κάτω άκρου πολύ συχνά συνοδεύεται από την ανάπτυξη πόνου φάντασμα. Ερευνήσαμε αν η εντατική περιεγχειρητική αναλγησία μειώνει την ανάπτυξη πόνου φάντασμα 6 μήνες μετά τον ακρωτηριασμό. Μεθοδολογία: Σε συνολικά 65 ασθενείς πραγματοποιήθηκε προγραμματισμένος ακρωτηριασμός κάτω άκρου. Οι ασθενείς τυχαιοποιήθηκαν σε 5 ομάδες αναλγησίας: (1) Οι ασθενείς της ομάδας Επι/Επι/Επι έλαβαν περιεγχειρητικά επισκληρίδια αναλγησία και επισκληρίδιο αναισθησία; (2) Οι ασθενείς της ομάδας PCA/Επι/Επι έλαβαν προεγχειρητικά ενδοφλέβια αναλγησία ελεγχόμενη από τον ασθενή (Patient Controlled Analgesia-PCA), μετεγχειρητικά επισκληρίδιο αναλγησία και επισκηρίδιο αναισθησία; (3) Οι ασθενείς της ομάδας PCA/Επι/PCA έλαβαν περιεγχειρητικά ενδοφλέβια PCA και επισκληρίδιο αναισθησία; (4) Οι ασθενείς της ομάδας PCA/ΓΑ/PCA έλαβαν περιεγχειρητικά ενδοφλέβια PCA και γενική αναισθησία (ΓΑ); (5) Οι ασθενείς της ομάδας ελέγχου έλαβαν συμβατική αναλγησία και ΓΑ. Η επισκηρίδιος αναλγησία ή η ενδοφλέβια PCA ξεκινούσε 48 ώρες προεγχειρητικά και συνεχιζόταν για 48 ώρες μετεγχειρητικά. Τα αποτελέσματα της κλίμακας πόνου VAS (Visual Analogue Scale) και της κλίμακας πόνου McGill καταγραφόταν περιεγχειρητικά και στον 1 και 6 μήνες. Trial registration: Clinical Trials.gov, number NCT00443404 / Severe preamputation pain is associated with phantom limb pain (PLP) development in limb amputees. We investigated whether optimized perioperative analgesia reduces PLP at 6-month follow-up. Methods: A total of 65 patients underwent lower-limb amputation and were assigned to five analgesic regimens: (1) Epi/Epi/Epi patients received perioperative epidural analgesia and epidural anesthesia; (2) PCA/Epi/Epi patients received preoperative intravenous patient-controlled analgesia (PCA), postoperative epidural analgesia, and epidural anesthesia; (3) PCA/Epi/PCA patients received perioperative intravenous PCA and epidural anesthesia; (4) PCA/GA/PCA patients received perioperative intravenous PCA and general anesthesia (GA); (5) controls received conventional analgesia and GA. Epidural analgesia or intravenous PCA started 48 h preoperatively and continued 48 h postoperatively. The results of the visual analog scale and the McGill Pain Questionnaire were recorded perioperatively and at 1 and 6 months. Results: At 6 months, median (minimum–maximum) PLP and P values (intervention groups vs. control group) for the visual analog scale were as follows: 0 (0–20) for Epi/Epi/Epi (P _ 0.001), 0 (0–42) for PCA/Epi/Epi (P = 0.014), 20 (0–40) for PCA/Epi/PCA (P = 0.532), 0 (0–30) for PCA/GA/ PCA (P = 0.008), and 20 (0–58) for controls. The values for the McGill Pain Questionnaire were as follows: 0 (0–7) for Epi/Epi/Epi (P = 0.001), 0 (0–9) for PCA/Epi/Epi (P = 0.003), 6 (0–11) for PCA/Epi/PCA (P = 0.208), 0 (0–9) for PCA/GA/PCA (P = 0.003), and 7 (0–15) for controls. At 6 months, PLP was present in 1 of 13 Epi/Epi/Epi, 4 of 13 PCA/Epi/Epi, and 3 of 13 PCA/GA/PCA patients versus 9 of 12 control patients (P=0.001, P=0.027, and P=0.009, respectively). Residual limb pain at 6 months was insignificant. Conclusions: Optimized epidural analgesia or intravenous PCA, starting 48 h preoperatively and continuing for 48 h postoperatively, decreases PLP at 6 months. Perceptions of surgical nurses regarding the post-operative pain management of patients after total hip or knee replacement surgery

The purpose of this study was to explore and describe the surgical nurses’ perceptions about the management of post-operative pain and strategies employed in management of pain in patients who have undergone total hip or knee replacement surgery in King Abdulaziz Medical City, Riyadh, Kingdom of Saudi Arabia. Qualitative, exploratory-descriptive research design was applied. Purposive nonprobability sampling was used and data collected by means of audio-recorded semistructured individual interviews. Data saturation was reached after interviewing twenty surgical nurses. Themes and categories emerged from adopting Creswell’s (2013) “data analysis spiral”. One of the key findings was that patients reportedly experience moderate to severe pain during the first three days after surgery, before it is controlled on mild to moderate levels or before the patient is pain free. Conclusions were drawn, and one of the major ones was that multimodal strategies are employed by the surgical to manage post-operative pain. Recommendations were also made from findings of this study, and one key recommendation was that expatriate nurses to have access to Arabic speakers to overcome language barriers. / Health Studies

L’invalidité attribuable à la douleur représente un problème important en raison de ses coûts personnels, financiers et sociétaux. L’effort scientifique mène à l’identification des facteurs de risque pour l’évolution de la douleur vers un état qui mine la capacité de la personne affligée à vaquer à ses occupations fondamentales. Cet effort met en relief le rôle déterminant que jouent les facteurs psychosociaux à chaque stade de l’évolution vers l’invalidité en raison de la douleur. Parmi les facteurs mis en cause, se trouvent les difficultés psychologiques (dépression, anxiété, somatisation, trouble de la personnalité, catastrophisme et évitement de l’activité), l’insatisfaction au travail et le contexte de réclamation. Forts de cette connaissance, les pays industrialisés se dotent de lignes directrices pour la prise en charge de la douleur aiguë dans le but de réduire les coûts, tant pour la personne que pour la société. Vingt ans après la parution des premiers guides de pratique, et la publication subséquente de dizaines d’autres guides véhiculant essentiellement les mêmes informations, les médecins peinent toujours à appliquer les recommandations. À partir des données probantes issues de la littérature scientifique, le présent ouvrage propose une synthèse critique des résultats pour pousser la réflexion et faire avancer la démarche dans le sens d’une réduction des coûts personnels, financiers et sociétaux. / Disability due to pain is a significant problem owing to its personal, financial and societal costs. Scientific efforts are aimed towards the identification of the risk factors for the development of pain leading to a state which undermines the afflicted person’s ability to attend to his/her basic functions. These efforts highlight the key role that psycho-social factors play in each stage of the progression towards disability due to pain. Among the influencing factors are psychological difficulties (depression, anxiety, somatization, personality disorder, catastrophization and activity avoidance), dissatisfaction at work and the compensation context. In light of this awareness, industrialized countries have instituted policies for the management of acute pain with the aim of reducing costs, both for the patient and for society. Twenty years after the appearance of the first clinical practice guidelines and the subsequent publication of numerous other guides propounding essentially the same information, physicians are still struggling to apply the recommendations. Relying on the evidence from the scientific literature, this study proposes a critical synthesis of the results to provoke thought and to advance the process towards a reduction in the personal, financial and societal costs of pain.
