Sjuköterskors erfarenheter av munhälsa hos äldre personer : En systematisk litteraturstudie / Registered nurses´experiences of oral health among elderly people : A systematic review

Bakgrund: Munhälsan har betydelse för den allmänna hälsan och välbefinnandet. Munhälsoproblem ökar ofta med stigande ålder, men trots att detta nedprioriteras munvården i omvårdnaden av äldre personer. Nedsatt kognition och funktionsförmåga försämrar äldre personers möjlighet att självständigt sköta sin munhälsa.  Sjuksköterskor är ansvariga för omvårdnaden, men upplever sin roll i omvårdnaden av personer med nedsatt munhälsa som oklar. Syftet: Syftet med den systematiska litteraturstudien var att sammanställa befintlig litteratur om sjuksköterskors erfarenheter av munhälsa hos äldre personer. Metod: En systematisk litteraturstudie med kvalitativ ansats baserad på nio vetenskapliga artiklar inhämtade från CINHAL och PubMed genomfördes. Statens beredning för medicinsk och social utvärdering (SBU) granskningsmall användes för relevans- och kvalitetsbedömning. För analysen av artiklarnas resultat användes metoden kvalitativ innehållsanalys av Forsberg och Wengström. Resultat: Resultatet presenteras i tre teman: munhälsa och munvård – viktigt men eftersatt; Interaktion med patienterna; avsaknad av riktlinjer, samverkan och dokumentation. Slutsats: Sjuksköterskor prioriterar inte munvård hos äldre personer. Orsaken till detta är brist på kunskap och färdigheter i ämnet, oklara riktlinjer och att de inte har en aktiv roll i munvårdsarbetet. / Background: Oral health has an impact on general health and wellbeing. Oral health problems are often increasing with aging and although this is well known oral care is still not prioritized in nursing among elderly. Cognitive and physical impairment deteriorates older persons ability to take care of their oral health independently. Registered nurses are responsible for the nursing care but experience their role in nursing among elderly with poor oral health to be unclear.Aim: The aim of the systematic review was to describe registered nurses´ experiences in oral health among elderly.Method: A systematic review with a qualitative approach based on nine scientific articles obtained from databases CINAHL och PubMed. The review templet of the Swedish agency for medical and social evaluation was used for the assessment of relevance and quality. Qualitative content analysis by Forsberg and Wengström was used for the analysis of the result of the articles. Results: The results are presented in three themes: oral health and oral care – important but neglected; interaction with patients; lack of guidelines, teamwork and documentation.Conclusion: Registered Nurses don´t prioritize oral care among elderly. The reason for this is lack of knowledge and skills in the subject, unclear policies and that they don´t take an active part of oral care nursing.

Evaluating the Performance of Swedish-Registered Actively Managed Emerging Market Equity Mutual Funds

We calculate the alpha of a survivorship bias-free sample of Swedish-registered actively managed emerging market equity mutual funds (with at least 10 years of return data), using long-short and long-only versions of several asset pricing models: the CAPM, the Fama-French three-factor model, the Carhart four-factor model, the Fama-French five-factor model and a six-factor model that adds the momentum factor to a modified Fama-French five-factor model. We find that our sample of mutual funds has statistically significant negative alpha (on a 5% level) using the CAPM, the Fama-French three-factor model and the long-only Fama-French three-factor model and non-statistically significant negative alpha for all other asset pricing models. It is reasonable to assume that our sample overestimates the performance of the universe of Swedish-registered actively managed emerging market equity mutual funds, so it is reasonable to assume that the universe of Swedish-registered actively managed emerging market equity mutual funds likely has negative alpha. We also find that our sample of mutual funds has statistically significant factor loadings on the market factor (positive load), the SMB factor (negative load) and the CMA factor (negative load). Our asset pricing models explain 97% to 98% of the mutual fund returns. The distributions of alphas and SMB factor loadings are fairly normally distributed, but the other factor loadings are not normally distributed.

Background: Death can be viewed and understood through different perspectives and occurs in different ways after different trajectories. Death can occur in the hospital setting where the registered nurse has a leading role in the nursing of dying patients. It is the registered nurses’ responsibility that the given care has dignity and is respectful of the patient’s integrity. Registered nurses need an awareness of the feelings they may experience when patients die and apply different coping strategies in order to cope with the pressure that encounters with death can result in. Purpose: Was to describe registered nurses’ experiences of caring for patients who die in hospitals. Method: This literature review used scientific articles with qualitative method that were analysed in five steps according to Friberg (2017a) and was formed with an inductive approach. Result: After analysis of 12 scientific articles two main categories were presented: Emotional impact and The importance of personal and professional development with a total of five subcategories. Conclusion: Registered nurses experienced difficulties and challenges when caring for dying patients in the hospital setting and experienced emotional and professional effects as well as effects on their everyday life. Limitations within oneself and outer circumstances was perceived to limit the registered nurses from providing good care. Coping strategies could be applied in order to cope with these difficulties. / Bakgrund: Döden kan ses och förstås genom olika perspektiv och inträffar på olika sätt efter varierande förlopp. Dödsfall kan inträffa på sjukhus där sjuksköterskor har en ledande roll i omvårdnaden av patienter som avlider. Sjuksköterskor ansvarar för att given omvårdnad sker med värdighet och att patienters integritet respekteras. Sjuksköterskor behöver vara uppmärksamma på de egna känslor som kan uppkomma vid patienters dödsfall och tillämpa olika copingstrategier för att kunna hantera den belastning som möten med döden kan innebära. Syfte: Var att beskriva sjuksköterskors upplevelser av att vårda patienter som avlider på sjukhus. Metod: I litteraturöversikten användes vetenskapliga artiklar av kvalitativ metod som analyserades i fem steg enligt Friberg (2017a) och utformades med ett induktivt förhållningssätt. Resultat: Efter analys av 12 vetenskapliga artiklar presenterades två huvudkategorier: Emotionell påverkan och Betydelsen av personlig och professionell utveckling med totalt fem subkategorier. Slutsats: Sjuksköterskor upplevde svårigheter och utmaningar kring att vårda patienter som avlider på sjukhus och upplevde känslomässig, vardaglig och professionell påverkan. Inre begränsningar och yttre omständigheter upplevdes hindra sjuksköterskor från att ge god vård till patienter som avlider. Sjuksköterskor kunde tillämpa olika copingstrategier för att kunna hantera de svåra situationerna.

Bakgrund: Covid-19 upptäcktes i Kina i slutet av 2019. Symtomen karaktäriseras av en luftvägsinfektion, men symtomen kan variera. Viruset spreds snabbt, vilket ledde till att sjuksköterskornas risk för att smittas av viruset ökade. Att studera sjuksköterskornas erfarenheter är relevant inför kommande pandemier. Syfte: Syftet med studien var att undersöka sjuksköterskors erfarenheter av att vårda patienter med Covid-19. Metod: En litteraturstudie med induktiv design baserad på åtta kvalitativa empiriska vetenskapliga artiklar från databaserna Cinahl, PubMed och Scopus. Resultat: Innehållsanalysen resulterade i fem kategorier med totalt nio subkategorier. Dessa kategorier är Brister, Arbetsförhållanden, Sjuksköterskan och samhället, Försämrad psykisk hälsa och Positiva erfarenheter. Konklusion: Med utbildningar inom akutvård skulle vården bli mer säker och belastningen för de mer erfarna sjuksköterskorna skulle minska. Arbetsgivaren kan bygga upp ett lager med skyddsutrustning inför kommande pandemier och erbjuda psykosocialt stöd till sjuksköterskorna som vårdar patienter med Covid-19. Vidare forskning som kan genomföras är till exempel att undersöka sjuksköterskornas, som vårdat patienter med Covid-19, psykiska hälsa efter att pandemin är slut för att se hur den långsiktigt påverkat deras psykiska hälsa. / Background: Covid-19 was detected in China at the end of 2019. Symptoms are characterized by a respiratory infection, but symptoms may vary. The virus spread rapidly, which led to an increase in nurses' risk of contracting the virus. Studying the nurses´experiences is relevant for future pandemics. Aim: The aim of this study was to investigate registered nurses´experiences of caring for patients with Covid-19. Methods: A literature review based on eight qualitative empirical scientific articles retrieved from the databases Cinahl, PubMed and Scopus. Results: The content analysis resulted in five categories with a total of nine subcategories. These categories are Insufficiency, Working conditions, The nurse and society, Deteriorated psychologial health and Positive experiences. Conclusion: With training in emergency care, the given care would be more secure and the burden on the more experienced nurses would be reduced. The employer can build up a supply of protective equipment for upcoming pandemics and offer psychosocial support to nurses who take care of patients with Covid-19. Further research that can be carried out is, for example, to examine the mental health of nurses who have cared for patients with Covid-19 after the end of the pandemic to see how it has affected their mental health in the long term.

Current Practices in Residential Treatment of Co-Occuring Eating Disorders and Type 1 Diabetes

Eating Disorder-Diabetes Mellitus Type 1 (ED-DMT1) refers to individuals who have type 1 diabetes and a co-occurring eating disorder. The aim of this study was to identify current treatment practices for individuals with ED-DMT1 in a residential eating disorder treatment setting. Clinical nutrition managers (CNMs) at 18 residential eating disorder treatment facilities were interviewed about treatment practices for patients with type 1 diabetes. Four themes were identified through qualitative case study analysis: 1) nutrition interventions for patients with diabetes, 2) medical diabetes management, 3) interdisciplinary diabetes team, and 4) CNM's assessment of diabetes care. The majority of CNMs interviewed reported utilizing an exchange- based meal plan, which is well suited for patients with type 1 diabetes. Dietitians described the use of diabetes technology (e.g., insulin pumps and continuous glucose monitors) and described a gradual advancement of responsibility and autonomy with portioning food at meals and snacks and managing diabetes care. The dietitian is heavily involved in providing diabetes education along with the medical and/or nursing team. The majority of CNMs interviewed demonstrated knowledge deficits related to diabetes management and expressed the need and desire for increased education for themselves and other staff members in order to provide the highest quality of care.

Frontline RNs knowledgeable in the strategic objectives of their organization made a difference in the selection of an electronic medical record business partner for a large, complex healthcare system. Their impact was significant because of the chief nurse executive's personal articulation of the organization's strategic goals and of her investment in their education. These factors provided the frontline RNs with a foundational base of knowledge about a variety of electronic medical record systems. The preparation and exposure enabled the frontline RNs to make a valuable contribution to the selection of an electronic medical record business partner. The RNs were a major force in affecting philosophical change from the organization's original pursuit of "best-of-breed" interfaced systems to a fully integrated, "best-of-class" vendor business partner. The learning experiences of the frontline RNs are explored to answer the following question: Why must frontline RNs play a key role in this process?

Bakgrund: Att arbeta i ambulans kräver ett öppet sinne och en bred klinisk blick samt en förmåga att kunna ta beslut i akuta situationer. Vilket kan vara en utmaning för en grundutbildad sjuksköterska att utföra en patientsäker vård då grundutbildningen inte motsvarar de höga förväntningarna i ambulanssjukvården. Syfte: Syftet med studien är att beskriva grundutbildade sjuksköterskors erfarenheter av att arbeta inom ambulanssjukvården.  Metod: Examensarbetet genomfördes som en litteraturöversikt och baserades på 15 vetenskapliga artiklar som har använts i resultatet. Artiklarna inhämtades från databaserna CINAHL och PubMed och var publicerade mellan 2005–2020.  Resultat: Litteraturöversikten resulterade i tre huvudkategorier: Den grundutbildade sjuksköterskans behov av ökad kompetens, Arbetet är utmanande för den grundutbildade sjuksköterskan samt Den grundutbildade sjuksköterskans känsla av att vara ny. Grundutbildade sjuksköterskor inom ambulanssjukvården upplever att de saknar viktiga erfarenheter och tillräcklig kompetens som kan leda till försenad behandling. De uttrycker även en osäkerhet vid arbete som utförs på egen hand vilket påverkar arbetet. Grundutbildade sjuksköterskor känner oro inför det stora ansvaret. Det framkommer även att vara ny inom ambulanssjukvården innebär en känsla av ensamhet.  Slutsats: Litteraturöversiktens resultat visar att grundutbildade sjuksköterskor som saknar erfarenhet inom ambulanssjukvården upplever osäkerhet och bristfälligt stöd,  vilket grundar sig i begränsad kunskap. Fördjupade kunskaper genom specialistutbildning skulle kunna öka tryggheten och förståelsen hos den grundutbildade sjuksköterskan. En för låg kompetens i kombination med bristande erfarenhet kan resultera i otillräcklig vårdkvalitet. / Background: Working in an ambulance requires an open mind and a broad clinical eye, as well as an ability to make decisions in emergency situations. Which can be a challenge for a registered nurse to perform patient-safe care as the education does not meet the high expectations in ambulance care. Aim: The aim of the study is to describe the registered nurses' experiences of working in ambulance care. Method: The thesis was conducted as a literature review based on 15 scientific articles. The articles were collected from databases CINAHL and PubMed and were published between 2005-2020. Result: The literature review resulted in three main categories: The registered nurse needs for increased competence, Work is challenging for the registered nurse and The registered nurse feeling of being new. Registered nurses in ambulance care feel that they lack important experience and sufficient competence that can lead to delayed treatment. They also express insecurity when working on their own, which affects the work. Registered nurses are concerned about the great responsibility. It also appears that being new in ambulance care means a feeling of loneliness. Conclusion: The results of the literature review show that registered nurses who lack experience in ambulance care experience uncertainty and inadequate support, which is based on limited knowledge. In-depth knowledge through specialist training could increase the security and understanding of the registered nurses. Too little competence in combination with lack of experience can result in insufficient quality of care.
