Efeitos do fosfato ou silicato de cálcio no substrato dentinário erodido e nas propriedades do sistema adesivo autocondicionante Clearfil SE Bond /

Orientador: Milton Carlos Kuga / Resumo: O objetivo do estudo foi avaliar o efeito de agentes remineralizantes à base de fosfato ou silicato de cálcio em formar precipitados e obstruir túbulos dentinários e seus efeitos sobre o colágeno da dentina erodida. Assim mesmo, avaliar seu efeito na resistência de união, formação de camada hibrida e penetrabilidade dentinária do sistema adesivo autocondicionante Clearfil SE Bond. Material e métodos: Trezentos espécimes foram obtidos do segmento cervical de incisivos bovinos e previamente submetidos à erosão dentinária. Os espécimes foram distribuídos em 5 protocolos (n=30): NP, Desensibilize NanoP; RD, MI Paste Plus RecaldentTM; NR, Regenerate NR-5TM; KF, Desensibilize KF 2% e CO, sem tratamento. Cada um dos protocolos foi aplicado no total de 4 sessões, com intervalos de 7 dias. Duzentos espécimes (n=20) foram diariamente submetidos ao desafio ácido, por imersão em suco de laranja, por 5 minutos e, posteriormente mantidos em saliva artificial. Na sequencia, cem espécimes foram analisados em microscopia eletrônica de varredura para avaliar a formação de precipitados sobre a superfície dentinária (500x) e obter o número de túbulos dentinários abertos (2,000x), e em microscopia EDX para obter resultados qualitativos sobre a composição do precitado, e avaliados por meio de microscopia de luz polarizada, com coloração picrosirius red, para qualificar a morfologia do colágeno da dentina após a conclusão do tratamento. Ability of Caries Detection Methods to Determine Caries Lesion Activity

Indiana University-Purdue University Indianapolis (IUPUI) / Background: Non-cavitated caries lesions form due to acid diffusion and demineralization of enamel subsurface with an intact surface layer (SL). Caries lesions progress when the outcome of demineralization and remineralization processes over time is net mineral loss. Lesions that continue to demineralize are called active, while those that display no evidence of further demineralization are called inactive. Micro-computed-tomography (µCT) analysis provides objective non-destructive measurements of the thickness of the surface layer (SL) and severity of caries lesions. Aims: 1) To investigate if visual/tactile suspected active non-cavitated early white spot lesions present a thinner surface layer than inactive ones; 2) To investigate if there is an association between the thickness of the surface layer (SLT) and caries activity, as determined by QLF during dehydration (△QD); 3) To determine lesion severity by comparing lesion volume and maximum depth correlation with △Q value at 15 s from QLF during dehydration. Materials and Methods: Thirty extracted human premolars exhibiting non-cavitated approximal white spot early lesions stored in 0.1.-percent thymol/4C and treated with 5.0-percent NaOCl/30 min were included in the study. Fifteen active and 15 inactive lesions were determined by visual/tactile examinations by consensus of two experienced examiners. Roughness measurements (Ra) were acquired using non-contact optical profilometry. Two-dimensional minimum (2D-min), maximum (2D-max), average (2D-avg) SL and three-dimensional (3D) analyses, volume and depth of lesions were determined from µCT image analysis. A series of fluorescence images were acquired at baseline (hydrated), at 1 s, at 5 s, at 10 s and at 15 s by QLF. During image acquisition, surfaces were dehydrated with continuous-compressed-air. △Q and △Q/s (△QD) were calculated. Data were analyzed using two-sample t-tests and Pearson correlation coefficients (p < 0.05). Results: Surface roughness of active and inactive lesions was not significantly different (p > 0.08). Overall lesion volume and depth in dentin were significantly larger in active lesions (p = 0.022, p = 0.009). SL thickness of active and inactive lesions was not significantly different (2D = 0.121, 3D = 0.080, 2D-avg = 0.446, 2D-min = 0.197, 2D-max = 0.122). △QD at 1s was significantly larger for active lesions (p = 0.046). ΔQ at 15 s of dehydration had a moderate positive association with lesion volume (r = 0.56). △QD had a weak negative association with SL thickness (2D-avg) and (2D-min). Conclusions: 1) Active and inactive non-cavitated lesions show no difference in SL thickness; 2) QLF during dehydration (△QD) does not correlate well with SL thickness; 3) ΔQ at 15 s of dehydration correlates moderately well with lesion volume and is consistent with caries activity assessed by visual/tactile examination.

The effect of acid etching on remineralization of incipient caries lesions : a micro-ct study

Indiana University-Purdue University Indianapolis (IUPUI) / Etching of enamel caries lesions has been demonstrated to enhance remineralization. However, this effect reaches a plateau after a period of time. This study aimed at investigating the effectiveness of additional acid etching on remineralization. Forty 1 mm × 2 mm human enamel blocks with chemically induced artificial incipient lesions were used. Ten specimens were randomly selected at the end of demineralization for transverse microradiography (TMR) analysis. The remaining specimens were then divided into three groups (n = 10). Group A was remineralized by a pH cycling system with 1100 ppm sodium fluoride for 20 days. In group B, the specimens were etched with 35-percent phosphoric acid for 30 s and then remineralized. Group C was remineralized by same procedure as group B plus and given an additional acid etch after 10 days of remineralization. Mineral density was measured by x-ray microtomography (µ-CT). The volumetric mineral content [VM (µm3×105)] was determined between 91 and 0-wt%. The µ-CT % mineral recovery (%) was calculated using the formula 100×(remineralize VM - demineralization VM) / (sound VM - demineralization VM). One-hundred-μm sections of demineralized and remineralized specimens were used to assess the mineral loss (IML: vol%×µm) and lesion depth (µm) using TMR. The three groups showed no significant difference in mineral change or mineral content for µ-CT or TMR lesion depth. The TMR IML showed a significant difference between the demineralized specimens and the three remineralized groups. The correlation between TMR IML and TMR lesion depth was 0.66 (p < 0.0001). The µ-CT percent mineral recovery from demineralization was correlated with neither TMR IML nor TMR lesion depth. When evaluated with µ-CT, the twice-acid-etched group presented lower mineral gain values than the group etched only once with acid. Also, the twice-etched group presented lower mineral gain and greater TMR IML compared with the non-acid etch group. TMR images revealed reduction of surface layer in the acid-etched groups, especially in the twice-etched group, in which significant reduction or loss of surface layer occurred. Based on these results, we conclude that additional acid etching with 35-percent phosphoric acid does not enhance remineralization compared with a single application of acid etching. We believe that the viable existence of the surface layer is essential for remineralization of the lesion. Further investigations into the accuracy of µ-CT to detect minute mineral changes in incipient caries lesions are probably needed.

As metaloproteinases da matriz (MMPs) foram identificadas na saliva, na placa dental, na dentina e no cemento. Este trabalho teve como objetivos: Estudo I (I) - avaliar a expressão de MMP-2 e MMP-9 presentes na saliva total e parotidiana e no fluido gengival crevicular (FGC) de pacientes portadores e não portadores de lesões cervicais não cariosas (LCNC); Estudo II (II) - investigar se a presença de MMP-8 e -9/TIMPs poderia influenciar a remineralização de lesões artificialmente criadas na superfície radicular, com ou sem desgaste por abrasão. Os métodos utilizados foram: (I) Coleta de amostras de saliva e do FGC de 32 pacientes, com (n=16) e sem LCNC (n=16). A atividade gelatinolítica das MMPs foi avaliada através de análise zimográfica e Western Blot. (II): Espécimes de dentina humana radicular foram obtidos. O grupo controle G1(10) não sofreu nenhum tratamento. Os demais segmentos radiculares foram desmineralizados G2(60). O Grupo A não foi submetido à escovação e o Grupo B foi submetido à abrasão por escovação em uma máquina de escovação simulada. G2(10) foi apenas desmineralizado, G3(10) desmineralizado e remineralizado, e os Grupos G4(10), G5(10), G6(10), G7(10) foram desmineralizados e remineralizados em presença de tampão neutro, TIMP, MMP-8 e -9, MMP-8,-9 e TIMP, respectivamente. Para a análise elemental, as concentrações de Ca+2, P, Mg+2 assim como a relação molar Ca/P e Mg/Ca foram determinadas através de uma sonda eletrônica para microanálise (EPMA). A análise qualitativa por retrodispersão (BSE) foi realizada para demonstrar a distribuição global da densidade mineral. Os resultados (I) mostraram que a principal gelatinase presente, tanto na saliva total quanto no FGC, é a proMMP-9. Na saliva secretada pela glândula parótida, não foram detectadas bandas indicando a presença de gelatinases. Os resultados do estudo (II) indicaram que os espécimes escovados apresentaram maior conteúdo de Ca+2 a 20µm e maior conteúdo de Mg+2 a 30 e 50µm. Em presença de TIMPs, ocorreu uma redução do conteúdo de Ca+2 a 20µm. Para os espécimes não escovados, em todas as profundidades, as amostras incubadas com MMPs apresentaram maiores valores de Ca+2. Portanto, pode-se concluir que (I) a comparação entre pacientes com e sem LCNC mostrou não haver diferença estatisticamente significante quanto à atividade gelatinolítica; (II) quando não inibidas pelos TIMPs, as MMPs degradaram o colágeno completamente desmineralizado na superfície radicular, permitindo melhor recalcificação na superfície subjacente. Esse fenômeno foi também facilitado pela abrasão por escovação. / Matrix metalloproteinases (MMPs) have been identified in saliva, plaque, gingival crevicular fluid (GCF), dentin and cementum. Study (I) aimed at evaluating the presence and quantity of gelatinases MMP-2 and MMP-9 in total and parotid saliva and in GCF (GCF) of subjects with and without NCCL. Study (II) aimed at investigating whether the presence of matrix metalloproteinase (MMP)-8 and - 9/TIMPs would influence the remineralization of artificial root lesions with and without mechanical wear. (I) Total stimulated saliva, parotid saliva, and GCF from patients with (n=16) and without NCCL (n=16) were collected and assessed for gelatin zymography and for western immunoblot analysis. (II) Human root segments from Group A (n=35) were not brushed and from Group B (n=35) were subjected to machine-controlled brushing, simulating mechanical wear. Specimens from Group 1 (control, n=10) were left untreated. Group 2 (n=10), was just demineralized; Group 3 (n=10) was demineralized and remineralized. The other samples G4 (n=10), G5 (n=10), G6 (n=10), G7 (n=10) were subjected to remineralization with HEPES buffer, tissue inhibitor of matrix metalloproteinase-2 (TIMP-2), activated MMP-8 and MMP-9 and activated MMP-8, MMP-9 and TIMP-2, respectively. Ca+2, P, Mg+2 concentrations as well as Ca/P and Mg/Ca molar ratios were determined through an Electron Probe Microanalyser (EPMA). (I) Densitometric analysis revealed that the main gelatinase was proMMP-9. No statistically significant difference was observed for MMP-2 and MMP-9 levels, separately. In parotid saliva, gelatinolytic activity was very low or absent. Western immunoblots revealed that, while little immunoreactivity was detected for MMP-2, there was positive immunoreaction for MMP-9, both in total saliva and in GCF. Gelatinases do not seem to originate from parotid gland. (II) The results indicated that the brushed specimens presented higher Ca+2 levels at 20 µm and higher Mg+2 content at 30 and 50 µm. Ca+2 content at 20 µm decreased in the presence of TIMPs. For the non-brushed specimens, in all depths, samples incubated with MMPs showed highest Ca+2 values. It can be concluded that (I) the main gelatinase present in the oral cavity is MMP-9. No significant differences were found in total gelatinolytic activity among NCCL+ and NCCL- patients. (II) When not inhibited by TIMPs, MMPs degraded the completely demineralized collagen in the root surface, allowing for better recalcification in the deeper areas. This phenomenon was also facilitated by the brushing procedure.

In vitro enamel subsurface lesions : characterization and treatment / Lésions carieuses de l'émail in vitro : caractérisation et traitement

Les taches blanches sont liées à l'hypominéralisation sous la surface de l'émail et sont la 1ère étape du développement de la carie dentaire. La détection précoce des caries dentaires naissantes avant qu'elles n'atteignent le stade de la cavitation offre une opportunité pour des soins dentaires efficaces. Pour réaliser les objectifs de cette étude qui sont de renforcer l'idiome des soins minimalement invasifs, nous avons identifié un modèle de cycle de pH modifié qui permet d’imiter les conditions intra-buccales qui conduisent à la formation de lésions de type tache blanche, dans un court laps de temps. Des techniques optiques non invasives, comme la microscopie Raman confocale et la microscopie multiphotonique, ont été utilisées dans cette étude pour détecter de petits changements dans la composition chimique de l'émail in vitro.De plus, la technique de nano-indentation a été appliquée pour étudier les changements dans les propriétés mécaniques de l'émail et les relier à ceux affectant sa composition chimique après l'induction des caries, afin d‘ajouter de la spécificité chimique-mécanique aux informations sur les lésions sous-surface de l'émail. Les résultats obtenus démontrent un grand potentiel pour les techniques examinées, fournissant une base pour des applications intéressantes dans le diagnostic clinique de différentes pathologies dentaires. Pour résoudre le problème du traitement des lésions primitives sans intervention chirurgicale, l'efficacité de la crème GC Tooth Mousse et du dentifrice contenant de la nano-hydroxyapatite «KAREX» dans l'amélioration de la reminéralisation de l'émail déminéralisé par la localisation du phosphate de calcium amorphe à la surface de la dent a été inspectée. L'étude a indiqué le manque de preuves fiables soutenant l'efficacité des agents reminéralisants dans le traitement des taches blanches. Ce travail doit être poursuivi par d'autres études in-vitro et par des études cliniques. / White spot lesion is the subsurface hypomineralization of enamel indicating the 1st stage of dental caries development. Early detection of incipient dental caries before it reaches the stage of cavitation offers an opportunity for effective dental care. The objectives of this study were to strengthen the idiom of minimally invasive treatment. In order to achieve the specified goals; we have identified a modified pH cycling model that can mimic the intraoral conditions leading to white spot lesions formation in a short time. In addition, non-invasive optical techniques, such as confocal Raman microscopy and multiphoton microscopy were used in this study to detect small changes in the enamel chemical composition in vitro.Furthermore, nano-indentation technique was used to detect the changes in the mechanical properties of enamel and relate them to those affecting its chemical composition after caries induction in order to add chemico-mechanical specificity in providing important information about subsurface lesions in enamel. The obtained results demonstrate a great potential for the examined techniques, providing a basis for interesting applications in the clinical diagnosis of various pathological conditions in dentistry. To treatment the incipient carious lesions non-invasively, the effectiveness of GC Tooth Mousse cream and nHA containing-dentifrice "KAREX" in the remineralization of demineralized enamel through localizing amorphous calcium phosphate at tooth surface has been inspected. The study indicated a lack of reliable evidence supporting the efficacy of remineralizing agents in the treatment of white spot lesions. Within the limitations of this study, further laboratory studies together with clinical research are therefore required to increase the available knowledge on this prevalent subject.

Programa de Pós-Graduação em Geociências- Geoquímica, Niterói, RJ / Os oceanos exportam o CO2 atmosférico para o fundo na forma de carbono orgânico particulado (COP) através da sedimentação de partículas planctônicas: detrito, fitoplâncton e pelotas fecais (PF). Em Cabo frio, sudeste da costa Brasileira, os eventos de ressurgência da ACAS ricas em nutrientes possibilitam o aumento na produtividade de comunidades fito e zooplanctônicas, COP e PFs. As PFs exportam carbono (C) para o fundo marinho e seu aporte para o fluxo de COP é variável, afetado pela natureza da rede trófica e a remineralização na coluna de água. O estudo do fluxo de COP é realizado mediante armadilhas de sedimentação que coletam partículas em intervalos pré-programados na parte externa da plataforma continental de Cabo Frio. Um fator de interferência no cálculo de COP é o zooplâncton ou swimmer (SW) que entra ativamente nas armadilhas de sedimentos através da migração vertical, causando uma superestimação no fluxo de partículas. As coletas de SWs e PFs foram realizadas com duas armadilhas de sedimentação em 50 e 100 m de profundidade, equipadas com 13 garrafas coletoras. Os intervalos de coleta foram de 5-7 dias por garrafa durante outono e inverno de 2011 e verão 2012. O material coletado nas garrafas foi passado através de peneiras de 1 mm e 500 μm de malha para separar os SWs. As PFs foram triadas a partir de uma aliquota da amostra peneirada. Os SWs e PFs foram separados, classificados e seu teor de C foi analisado. O fluxo de COP foi entre 0,2 e 4,4 mg C m-2 dia-1 sendo superior durante o verão, épocas de maior produtividade primária, produto de uma maior intrusão da ACAS na camada fótica. Os SWs representaram 63-94 % do C coletado na armadilha e foram constituídos principalmente por copépodos e pterópodos. O fluxo da maioria dos grupos de SWs aumentou no verão, produto de uma maior disponibilidade de alimento e diminuíram com a profundidade. O fluxo de C de PFs representou uma média de 1,5 % do fluxo de COP na maioria dos intervalos o que sugere a predominância de processos de remineralização na coluna d‟água. As variáveis medidas, não explicam os aumentos de PF em dois intervalos do inverno (até 56 % do COP), portanto se verifica uma alta dinâmica física das massas d‟água no ponto de coleta do sistema de ressurgência de Cabo Frio. A alta presença de SWs e o baixo fluxo de PFs descrevem uma área de baixa produtividade e altos processos de remineralização / The oceans export atmospheric CO2 to deep in the form of particulate organic carbon (POC) through the sedimentation of planktonic particles: detritus, phytoplankton and fecal pellets (FP). At Cabo Frio, southeast of Brazil margin coast, upwelling events of rich nutrients waters from South Atlantic Central Water (SACW) allow an increase in productivity of phyto and zooplankton communities, POC and FPs. The FPs can export carbon (C) to depth and their contribution to the POC flux is variable and is affected by the configuration of the food chain and remineralization in the water column. The study of the POC flux its made by sedimentation traps, that collect particles in pre-programmed time intervals on the outer continental shelf of Cabo Frio. The swimmers (SW) are zooplankton that interference on POC flux calculation. The SWs enter actively into the sediment traps through vertical migration, causing an overestimation of particle flux. The sampling of FPs and SWs were performed with two sediment traps with 13 sampler bottles at 50 and 100 m depth. The sampling intervals were 5-7 days per bottle during fall and winter of 2011 and summer of 2012. The collected material in the bottles was sieved with 1 mm and 500 μm sieves to separate SWS. The FPs were picked from an aliquot of the sieved sample. SWs and PFs were separated, sorted and their C content was analyzed. The POC flux oscillated between 0.2 and 4,4 mg C m-2 day-1 being higher during the summer, this is a season with higher primary production due to a higher frequency of intrusions of SAWC in the photic layer. The SWs accounted for 63-94 % of C collected in the trap and were composed mainly of copepods and pteropods. The flux of most groups of SWs increased during summer product of mayor food availability and decrease with depth. The C flux of PFs represents an average of 1.5 % of COP in the most of intervals, which suggests predominance of remineralization processes in the water column. The measured variables did not explain the increase of FPs in two intervals of winter (45-56 % of POC), therefore there is a high physical dynamics of water masses at the collection point in the Cabo Frio upwelling system. Dégradation de la matière organique dissoute de haut poids moléculaire par les communautés procaryotiques des zones méso- et bathypélagique

Ce travail a pour objectif principal l'étude des interactions entre les compartiments procaryotiques des zones méso- et bathypélagique avec les différentes fractions du carbone organique dissout (DOC) océanique. Des mesures d'assimilation de monomères (3H-Glucose), de dégradation de molécules complexes (3H-EPS et HMW-DOC), et de production hétérotrophe procaryotique (PHP/3H-Leucine) ont été réalisées le long de la colonne d'eau en Mer Méditerranée Nord Occidentale (Golfe du Lion, sites DYFAMED, ANTARES) et en Océan Atlantique Nord-Est (site PAP). Au cours des ces études, toutes les mesures réalisées au-delà de 1000 m de profondeur ont été effectuées dans des conditions in situ de haute pression hydrostatique (> à 10 MPa, HP) et comparées à des mesures réalisées sous pression atmosphérique (0,1 MPa, ATM). Cette double mesure détermine le rôle de la pression hydrostatique sur les activités microbiennes profondes via un rapport Pe pour pressure effect (= mesure HP / mesure ATM). Les résultats démontrent que les activités microbiennes mesurées en conditions HP sont plus importantes qu'en conditions ATM en période de stratification des eaux, (Pe moyen de 4,01, n=120), et confirment la capacité des procaryotes du domaine océanique profond à dégrader des molécules organiques complexes. Par ailleurs et à une échelle cellulaire, les populations métaboliquement actives du milieu profond dégradent les 3H-EPS à une vitesse 6 fois plus rapide que leur homologue de surface, indiquant la capacité des procaryotes autochtones profonds à dégrader des molécules plus complexes en conditions de haute pression. / This main objective of this work is the study of interactions between prokaryotic compartments of meso-and bathypelagic zones with different size classes of dissolved organic carbon (DOC). Several measurements of monomers assimilation (3H-Glucose), of complex molecules degradation (3H-EPS and HMW-DOC) and prokaryotic heterotrophic production were realized through the water column of NW Mediterranean Sea (Gulf of Lion, DYFAMED and ANTARES station) and NE Atlantic Ocean (PAP site). During these studies, all measurements realized below 1000 m depth, were carry out under in situ condition of hydrostatic pressure (> 10 MPa, HP) and compared to their decompressed counterpart measurements, realized at atmospheric pressure (0.1 MPa, ATM). These coupled measurements determine the role of hydrostatic pressure on deep sea microbial activity following the Pressure effect (Re) ration (=HP measurement / ATM measurement). The results show that microbial activities measured under HP condition during stratified water period, were more important than those measured under ATM condition (mean Pe = 4.01, n=120), and confirm the abilities of deep sea prokaryotes to degrade complex organic molecules. Moreover, the cell-specific activity of deep sea prokaryotes in 3H-EPS degradation are 6 time more active than the surface, indicating the ability of autochthonous deep sea prokaryotes to degrade complex molecules under high conditions of pressure.
