Influência da dose única de morfina peridural na normalização das espirometrias e pressões arteriais de oxigênio de pacientes submetidos à colecistectomia aberta

Os objetivos do presente estudo, na evolução pós-operatória de colecistectomias, foram: a) verificar a presença de distúrbios ventilatórios restritivos e, caso ocorram, estabelecer o período em que os mesmos se tornam mais intensos, bem como determinar sua intensidade; b) averiguar o tempo necessário para obtenção de espirometrias consideradas normais; c) verificar a presença de alterações gasométricas, relacionadas à PaO2 e, uma vez confirmada, determinar o tempo de normalização dessa variável; d) correlacionar a presença de distúrbios ventilatórios restritivos com alterações na PaO2; e e) acompanhar a evolução pós-operatória das variáveis CVF e VEF1. Método: Em um estudo experimental do tipo ensaio clínico randomizado duplo-cego, 45 pacientes foram distribuídas em três grupos, GL, GA e GAM, cada qual com 15 componentes e submetidas a colecistectomias. O grupo GL foi operado pela via laparoscópica, enquanto GA e GAM, pela via aberta subcostal. O protocolo das anestesias foi idêntico para os três grupos, exceto para o grupo GAM, que recebeu morfina peridural. As enfermas realizaram espirometrias e gasometrias de sangue arterial, no pré-operatório, no dia seguinte ao procedimento e, desde então, a cada dois dias, até a obtenção de um teste espirométrico e gasométrico normal, quando se interrompia a realização de novos exames. As variáveis CVF e VEF1 foram analisadas, separadamente, até o momento de suas normalizações. A hipótese de igualdade de médias entre os grupos foi verificada utilizando-se a ANOVA. Quando os resultados foram estatisticamente significativos, realizava-se o teste de Tukey. Já a hipótese de igualdade de médias entre um mesmo grupo, antes e após uma determinada intervenção, foi verificada por meio do teste t-Student emparelhado. Um valor de p < 0,05 foi considerado estatisticamente significativo. Resultados: Comparando as variáveis espirométricas no pré X pósoperatório imediato: a) entre os três grupos: para CVF GL X GA [p < 0,001] e GL X GAM [p = 0,002]; para redução percentual da CVF GL X GA [p = 0,000] e GA X GAM [p = 0,027]; para VEF1 GL X GA [p < 0,001], GL X GAM [p = 0,013]; b) mesmos grupos entre si: GL para CVF [p = 0,020] e VEF1 [p = 0,022]; GA para CVF [p = 0,000] e VEF1 [p = 0,000]; e GAM para CVF [p = 0,007] e VEF1 [p = 0,001]. Assim, todos os grupos apresentaram distúrbios ventilatórios leves, mais acentuados no pós-operatório imediato e mais intenso no grupo GA, menos no grupo GAM e mínimos no grupo GL. Quando se compararam os valores espirométricos pós-operatórios, com aqueles pré-calculados [preditos] como normais, o grupo GL passou a ser considerado normal sob o aspecto espirométrico. As espirometrias, no grupo GL, apresentaram-se normais no primeiro exame pós-operatório; no grupo GA, a normalização ocorreu em até sete dias de pós-operatório; e, no grupo GAM, em até três dias. Não foram detectadas diferenças significativas relacionadas ao valor da PaO2, no pós-operatório, entre pacientes de um mesmo grupo, muito embora tenha havido queda no valor dessa variável, nos três grupos. No pós-operatório imediato, o GL apresentou maior PaO2, cujas reduções mais acentuadas, nos três grupos, foram observadas neste período, coincidindo com os menores valores das variáveis espirométricas, representadas pela CVF e o VEF1, que a partir de então, nas medidas posteriores, apresentaram valores sempre crescentes. Conclusões: Pode-se concluir que as menores disfunções ventilatórias ocorreram nas pacientes operadas pela via laparoscópica, e que a morfina peridural reverte, parcialmente, o distúrbio ventilatório pós-operatório de colecistectomia aberta representadas pela CVF e o VEF1, que a partir de então, nas medidas posteriores, apresentaram valores sempre crescentes. Conclusões: Pode-se concluir que as menores disfunções ventilatórias ocorreram nas pacientes operadas pela via laparoscópica, e que a morfina peridural reverte, parcialmente, o distúrbio ventilatório pós-operatório de colecistectomia aberta. _________________________________________________________________________________ ABSTRACT / Purpose: Operations on the upper abdomen may cause ventilatory dysfunctions in post-operative. The objectives of the present study concerning the post-operative evolution of cholecystectomies were a) to check for the presence of restrictive ventilatory disturbances, and if they occur, to determine the period when they are most intense as well as to determine their gravity; b) to determine how much time is necessary for spirometries to return to normal; c) to check for the presence of PaO2-related gasometric alterations and if detected, to determine how long it takes for this variable to return to normal; d) to correlate the presence of restrictive ventilatory disturbances with alterations in PaO2 and finally e) to follow separately the post-operative evolution of the forced vital capacity [FVC] and forced expiratory volume in one second [FEV1] variables. Methods: In an experimental study of the double-blind randomized clinical trial type, 45 cholecystectomy patients were divided into three groups of 15 components each, GL, GA and GAM. Patients in GL were operated on laparascopically, while those in GA and GAM were submitted to the conventional subcostal approach. The anesthesia protocols were identical except for group GAM, which received peridural morphine. Patients had spirometries and arterial blood gasometries done in the preoperative phase and in post-operative, on the day following the procedure and every two days thereafter, until normal spirometric and gasometric tests were obtained, at which point the exams were discontinued. The variables FVC and FEV1 were analyzed separately until their return to normal. The hypothesis of equality of means among the groups was tested using ANOVA variance analysis. When the results were statistically significant, the Tukey test was carried out. The hypothesis of equality of means in the same group, before and after a particular intervention, was tested using a paired t-Student test. P < 0.05 was considered statistically significant. Results: Comparing the spirometric variables pre- vs. post-operative a) among the three groups: for FVC GL X GA [p < 0.001] and GL X GAM [p = 0.002]; for reduction percentage in FVC GL X GA [p = 0,000] and GA X GAM [p = 0,027]; for FEV1 GL X GA [p < 0.001], GL X GAM [p = 0.013]; b) same groups with themselves: GL for FVC [p = 0.020] and FEV1 [p = 0.022]; GA for FVC [p = 0.000] and FEV1 [p = 0,000] and GAM for FVC [p = 0.007] and FEV1 [p = 0.001]. Thus, all of the groups presented light ventilatory disturbances, more accentuated in early post-operative and more intense in group GA, less in group GAM and minimal in group GL. When post-operative spirometric values were compared to the predicted [pre-calculated] normal values, group GL could be considered normal from the spirometric point of view. Group GL showed normal spirometries in the first early post-operative exam, in group GA normalization occurred after up to seven days of post-operative, and in group GAM, it occurred within three days of post-operative. No significant differences related to the value of PaO2 were detected in the post-operative of same-group patients, although there was a decrease in the value of this variable in all three groups. GL presented the largest PaO2 in early post-operative, which was the period during which the largest reductions of PaO2 in the three groups were observed, coinciding with the lowest values of the spirometric variables, represented by FVC and FEV1, which from this point in later readings presented increasing values. Conclusions: It can be reported that the fewest ventilatory dysfunctions occurred in patients operated on laparascopically and that peridural morphine partially reverts post-operative ventilatory disturbance after open cholestectomy.

Estes exames, com exceção do PFNI, são dispendiosos e complexos. Para utilização na Medicina do Trabalho e mesmo na prática clínica diária, faz-se necessário um método de mensuração simples, rápido, de baixo custo e confiável. Atualmente, os estudos tendem a correlacionar métodos que reúnem estas características, com aqueles métodos já bem documentados. Objetivos: comparar os resultados do PFNI com achados da Escala Analógica Visual (EVA), antes e após o uso de vasoconstritor nasal em indivíduos assintomáticos. Em um segundo momento, avaliar o PFNI entre indivíduos saudáveis e com queixas nasais obstrutivas. Forma de estudo: Artigo 1: Estudo de coorte em que a variável em questão é a ralação entre o PFNI e a EVA para obstrução nasal antes e após vasoconstrição nasal em indivíduos saudáveis; Artigo 2: Estudo transversal, em que a variável estudada é o comportamento do PFNI em uma população de indivíduos sem e com queixas nasais obstrutivas. Métodos: Dissertação realizada em formato alternativo composta de dois artigos. No primeiro foi feito uma correlação entre o resultado do PFNI e EVA antes e após vasoconstrição nasal de uma amostra de 60 sujeitos saudáveis. No segundo artigo foram avaliados 78 sujeitos com e sem sintomas nasais obstrutivos. Ambos estudos tiveram alocação de sujeitos de forma não randomizada. Resultados: No teste de regressão linear simples, o PFNI mostrou resultados significantes para obstrução nasal, rinorréia, prurido, espirros e lacrimejamento (p<0,001) e não apresentou significância para desvio de septo nasal (p=0,561). Encontrou-se uma correlação positiva entre PFNI e a pesquisa através da EVA (p<0,001). Houve significância estatística entre os valores encontrados no PFNI com rinite alérgica e idade (p=0,005 e p=0,023 respectivamente). O valor médio encontrado para EVA pré vasoconstrição foi de 4,1 e 2 após a vasoconstrição (variação de 44%). Em relação ao PFNI, a média encontrada foi de 151l/mim e de 178l/mim antes e após a vasoconstrição (variação de 20%). Conclusão: O PFNI mostrou ser um método confiável para detecção das alterações da patência nasal, tanto por causas obstrutivas quanto inflamatórias, com um nível de significância estatística aceitável. Apresentou forte correlação com a EVA, que é um método subjetivo muito utilizado para mensuração da obstrução nasal. É simples, fácil de manusear, pouco dispendioso e reprodutível. Por estas características descritas, pode ser usado como exame de triagem em Medicina do Trabalho. Entretanto, estudos com amostras maiores serão necessários para a normatização e sua utilização em massa / Abstract: For the objective assessment of nasal patency are used to acoustic rhinometry, rhinomanometry and nasal inspiratory peak flow (PFNI). These tests, except PFNI, are expensive and impractical. For use in occupational medicine and even in daily clinical practice, it is necessary a method for measuring simple, fast, inexpensive and reliable. Currently, studies tend to correlate methods that meet these characteristics, with other methods already well documented. Objectives: compare the results with findings PFNI EVA before and after using a nasal vasoconstrictor. In a second step, to evaluate the PFNI between healthy and obstructive nasal symptoms for allergic rhinitis. Study design: Article 1: A cohort study in which the variable is to determine the relationship between EVA Visual Analogue Scale) and PFNI for nasal obstruction before and after using a nasal vasoconstrictor. Article 2: A cross-sectional study in which the variable is the behavior of PFNI in a population of individuals. Methods: Essay held in alternate format consists of two articles. In the first article, was made a correlation between the result of PFNI and VAS before and after nasal vasoconstriction of a sample of 60 subjects. In the second article, was involve 78 subjects with and without symptoms of rhinitis. Both studies had allocation of subjects in a non-randomized. Results: In simple linear regression, the PFNI showed significant results for nasal obstruction, rhinorrhea, itching, sneezing and watery eyes (p<0.001). There was also a correlation between the presence of nasal septal deviation and PFNI (p=0.561). We found a positive correlation, the Spearman test between PFNI and research through the VAS (p<0.001). There was no statistical significance between the values found in PFNI with allergic rhinitis and age (p=0.005 and p=0.023 respectively). The mean value for VAS pre vasoconstriction was 4.1 and 2 after vasoconstriction. This represented a 44% variation between measurements. Regarding the values of PFNI, the average found in the pre measure vasoconstriction was 151l/min and 178l/min after vasoconstriction, an increase of 20%. At the moment pre vasoconstrictor, the change of a point on the average VAS represents a decrease of 3.8% in the average PFNI. In the post, each one point increase in the average value of EVA, corresponds to a decrease of 4.5% on average PFNI. Conclusion: The PFNI proved to be a reliable method for detecting larger studies are needed for a proper understanding of the method and its widespread use. changes in nasal patency by both inflammatory and obstructive causes, with an acceptable level of statistical significance, simple, easy to handle, inexpensive and reproducible. Esses distúrbios são capazes de levar as condições que comprometem a qualidade de vida desses sujeitos trazendo agravo a sua saúde física e excluindo-o do convívio social. Objetivo: Foi investigar se há correlação nas variáveis da composição corporal com a função com a força muscular respiratória em sujeitos portadores da Síndrome de Down. Material e Método: Foi realizado um estudo analítico transversal com 26 sujeitos portadores da Síndrome de Down com idade 26,72 ± 7,14 anos; peso 77,2 ± 3,5 kg, sendo 14 sujeitos do gênero feminino e 12 do gêneros masculino. Foi realizada a espirometria através do espirômetro para a obtenção do volume expirado forçado no primeiro segundo (VEF1), capacidade vital forçada (CVF) e pico de fluxo expiratório (PFE); a manovacuometria através do manovacuômetro para a mensuração da pressão inspiratória máxima (PImáx) e pressão expiratória máxima PEmáx) e para a avaliação do percentual de gordura corporal (%GC) foi utilizado o Dual Energy X-Ray Absormetry (DXA). O tratamento dos dados utilizou uma estatística descritiva dos dados quantitativos média (tendência central) e desvio padrão (dispersão dos dados). Foi realizada uma análise de normalidade das amostras para verificar a sua distribuição em relação às variáveis, e foi aplicado o teste de normalidade de Shapiro Wilk, considerando a diferença significativa de p < 0,05. Na inferência estatística foi aplicado o teste paramétrico (coeficiente de correlação de Pearson) para verificar se houve correlação entre as variáveis da função pulmonar (capacidade vital forçada, volume expirado forçado no primeiro segundo, pico de fluxo expiratório) força muscular respiratória (pressão inspiratória máxima e pressão expiratória máxima) com o percentual de gordura corporal (%GC). Resultados: foi encontrada correlação negativa para as variáveis de %GC e VEF1 em ambos os gêneros sem diferença significativa para os homens (p < 0,5244) e com diferença altamente significativa para as mulheres (p < 0,0045); na variável de %GC e CVF a correlação também foi negativa em ambos os gêneros com diferença significativa de p < 0,0137 para as mulheres e sem diferença significativa para os homens de p < 0,4910; na variável de PFE a correlação também foi negativa, sem diferença significativa para ambos os gêneros, nas mulheres p< 0,3165 e nos homens p< 0,7165; na variável de %GC e PImáx, houve correlação negativa para as mulheres sem diferença significativa com p< 0,7871 e correlação positiva para os homens sem diferença significativa de p< 0,8307; nas variáveis de %GC e PEmáx a correlação foi negativa e sem diferença significativa para ambos gêneros, nas mulheres com p< 0,9070 e nos homens p< 0,11369. Conclusão: sujeitos portadores da Síndrome de Down apresentam %GC mais elevado frente aos valores de referência para a população brasileira sem a Síndrome de Down, que pode estar atribuído as alterações inerentes a essa população associado a um estilo de vida sedentário. Os mesmos sujeitos apresentam correlação negativa para as variáveis de VEF1, CVF, PFE e PEmáx, possivelmente pela diminuição da complacência pulmonar, maior resistência ao fluxo aéreo e pela interferência direta que a obesidade exerce sobre a mecânica do diafragma e caixa torácica, diminuindo seus volumes e capacidades. Nas variáveis de %GC e PImáx, houve diferença de correlação entre os gêneros: negativa para as mulheres possivelmente pelo valor mais elevado de %GC em relação aos homens. A correlação foi positiva para os homens podendo o resultado estar relacionado a um % GC mais próximo do valor de referência nesse gênero. _______________________________________________________________________________ ABSTRACT / Introduction: Overweight and respiratory malfunctions are frequently found in Trysomy 21 bearers. Such disturbances are potentially able to lead to conditions which endanger the quality of life of affected individuals, thus aggravating their physical health and excluding them from social interaction. Purpose: the purpose of this academic essay was to investigate the possible correlation involving body composition, pulmonary function and respiratory muscle strength in Trysomy 21 bearers. Material and Method: a transversal analytical study was carried out involving twenty six Tryssomy 21 bearers who aged 26,72 + 7,14 and weighed 77,2 + 3,5 kg, whose genders were, respectively, 14 females and 12 males. A spirometry was conducted by means of a spirometer in order to obtain the forced expiratory volume in one second (FEV1), as well as the forced expiratory vital capacity (FVC) and peak expiratory flow (PEF). Pressure manometry, by means of a manometer, was also conducted in order to enable measuring both the maximal inspiratory pressure (IPmax) and the maximal expiratory pressure (EPmax). A Dual Energy X-Ray Absorptiometry (DXA) was used to evaluate the percentage of body fat (%BF). Data processing was conducted by means of a descriptive statistics of quantitative data – average (central tendency) and standard deviation (data dispersion). Also, a normality analysis of samples was carried out in order to check their distribution in relation to the variables, as well as the Shapiro Wilk normality test, considering the significant difference of p< 0,05. At the statistical inference, the parametric test (Pearson Correlation Coefficient) was applied to verify whether there had been a correlation among variables of pulmonary function (Forced Vital Capacity, forced expiratory volume in one second, peak expiratory flow, maximal inspiratory pressure and maximal expiratory pressure), respiratory muscle strength, maximal inspiratory pressure (IPmax) and maximal expiratory pressure (EPmax) with the percentage of body fat (%BF). Results: a negative correlation was found for the variables of %BF and FEV1 in both genders without a significant difference for men (p < 0,5244) and a highly significant difference for women (p < 0,0045); as to the variables of %BF and FVC, the correlation was also negative in both genders, with a significant difference of p < 0,0137 for women and without a significant difference for men (p < 0,4910); concerning the variable of PEF, the correlation was also negative, without significant difference for both genders, that is, p< 0,3165 for women and p< 0,7165 for men; as to the variables of %BF and IPmax, there was a negative correlation for women, without significant difference (p< 0,7871), and a positive correlation for men, without significant difference (p< 0,8307); as to the variables of %BF and EPmax, the correlation was negative and without a significant difference for both genders, namely, p< 0,9070 for women and p< 0,11369 for men. Conclusion: individuals who bear the Tryssomy 21 have higher %BF comparing with reference values related to the brazilian population without any mental deficiency, which might be attributed to inherent alterations of such population, as well as a sedentary lifestyle. The same individuals have negative correlation for variables of FEV1, FVC, PEF, and EPmax, possibly due to the decrease of lung compliance, the higher resistance to airflow and the direct interference exerted by obesity on the mechanics of the diafhragm and the chest box, diminishing their volumes and capacities. As to the variables of %BF and IPmax, there was a correlation difference between both genders: negative for women, possibly due to the higher value of % BF in relation to men; and positive for men, result which might be related to a value of % BF closer to the reference value for the gender.

Para isso foi utilizado um sistema de análise cinemática tridimensional de movimentos (Dvideow) para obter a descrição das coordenadas 3D de 30 marcadores fixados sobre referências anatômicas na caixa torácica e abdômen durante a ventilação. As medidas de volume foram calculadas a partir de um modelo geométrico de representação do tronco que o subdivide em 8 dodecaedros irregulares correspondendo aos volumes parciais do tronco (tórax superior, TX-S; tórax inferior, TX-I; abdômen superior, AB-S e abdômen inferior, AB-I). Para avaliação da metodologia foram realizados três experimentos; um compara os volumes obtidos através da videogrametria com os volumes de ar trocados entre os pulmões e o meio externo, medido através da espirometria; o segundo experimento analisa a reprodutibilidade do método proposto. Um terceiro experimento foi realizado para verificar a contribuição dos compartimentos para a variação do volume total no tronco. Para tanto foram analisados os volumes do tronco de 13 voluntários em duas manobras ventilatórias distintas, volume corrente (VC) e volume máximo (VM). Na comparação entre a videogrametria e espirometria, os resultados mostraram um coeficiente de correlação de 0.9938 entre as medidas de ar obtido pela espirometria e a variação do volume do tronco medida pela videogrametria. Contudo, os valores absolutos diferiram em até 0.6 litros na inspiração profunda. No teste de reposicionamento de marcadores encontrou-se uma variabilidade da ordem de 3% do volume total médio. Na análise da contribuição dos compartimentos para o volume total do tronco foi observada uma contribuição maior do tórax em relação ao abdômen nas duas manobras estudadas. Na análise dos compartimentos TX-S, TX-I, AB-S e AB-I os compartimentos TX-I e AB-S tiveram participação mais significativa que o TX-S, enquanto que o AB-I teve uma contribuição negativa para o volume total na manobra de VC. Na manobra de VM os resultados demonstraram uma contribuição semelhante entre os compartimentos TX-S, TX-I, AB-S, enquanto o AB-I, apresentou oposição de fase em alguns voluntários. De maneira geral os resultados evidenciaram sinais correlacionados entre as variações de volume do tronco e o sinal ventilatório e a videogrametria demonstrou ser um método eficiente na análise da contribuição dos compartimentos para a variação de volume total no tronco / Abstract: This work has the objective of proposing and evaluating a methodology to calculate the trunk volume variation during human respiration. Thereunto a three-dimensional movement kinematical analysis system (Dvideow) was used to obtain the 3D coordinate description of 30 markers attached over anatomical references in the rib cage and abdomen during breathing. The volume measures were calculated from a geometrical representation model for the trunk, which subdivides it in 8 irregular dodecahedrons, corresponding to partial volumes of the trunk. To evaluate the methodology, three experiments were done: one that compares the trunk volumes obtained by videogrammetry with the volume of air exchanged between the lungs and external medium, measured by spirometry; and a second analyzes the reproducibility of the proposed method. A third experiment was executed to verify the compartments contributions to the total trunk volume variation examining this variation in 13 volunteers in two distinct ventilatory maneuvers, tidal volume and maximum volume. In the comparison between videogrammetry and spirometry, results exhibit a correlation coefficient of 0.9938, however, absolute values differ up to 0.6 liters in deep inspiration. In the reproducibility test, it was determined a variability around 3% of the mean total volume. In the compartment contribution analysis, a greater contribution from the rib cage relative to the abdomen was observed in the two studied maneuvers. In the analysis of the subdivision of rib cage and abdomen compartments, the lower rib cage and the upper abdomen had a more significant participation that the upper rib cage, while the lower abdomen had a negative contribution to the total volume during tidal volume maneuvers. In the maximum volume maneuver, results demonstrate a similar contribution for the upper ribcage, lower ribcage and upper abdomen, while the lower abdomen, presented negative contribution. In the overall, results reveal correlated signals between the trunk volume variation and the ventilatory signal, and videogrammetry proved to be an efficient method for the analysis of compartment contribution for total trunk volume variation / Mestrado / Biodinamica do Movimento Humano / Mestre em Educação Física

O método consistiu em: projetar uma malha de marcas circulares sobre a superfície anterior e posterior do tronco; registrar as superfícies do tronco através de quatro câmeras de vídeo durante a respiração; segmentar as imagens e rastrear as marcas projetadas através de técnicas de pré-processamento de imagens, operações morfológicas e algoritmos de detecção; estabelecer uma correspondência entre as marcas nas seqüências de imagens registradas pelas câmeras; reconstruir as coordenadas 3D das marcas; reconstruir a superfície do tronco a partir da nuvem de pontos desordenada usando o método QHULL. Medidas de variação de volume e perímetro do tronco ao longo do ciclo respiratório e mapas de contorno foram usados para analisar a superfície reconstruída. A acurácia da medida de volume foi avaliada através da comparação entre a medida de volume de um manequim de tronco masculino obtida por imersão e através da metodologia proposta. Um exemplo de aplicação da análise foi realizado com dois sujeitos com formas e tamanhos de tronco distintos, um do sexo feminino com escoliose idiopática severa e um do sexo masculino sem deformações na coluna vertebral. A variação do volume do tronco foi comparada com a variação do volume de ar trocado durante o ciclo respiratório medido através da espirometria. O método foi capaz de reconstruir com alto nível de detalhamento a superfície do tronco possibilitando a identificação de estruturas anatômica e a diferenciação da forma do tronco dura ate as fases do ciclo respiratório. A acurácia relativa na determinação do volume foi de 2,9%. A variação do volume do tronco durante a respiração apresentou um sinal coerente com o ciclo respiratório e apresentou altos valores de correlação (r=0.988) quando comparado com o volume de ar trocado. A análise da superfície através de mapas de contorno mostrou ser uma ferramenta promissora na identificação de deformações da superfície. Podemos concluir que a proposta do uso de um método baseado em de projeção de luz foi adequada para reconstrução e análise da superfície do tronco durante a respiração / Abstract: This study aimed to propose and evaluate a method based on light projection for trunk surface reconstruction and analysis during respiration. The method consisted of: to project a grid of circular markers on the anterior and posterior surface of the trunk; to register the trunk surfaces by four video cameras during respiration; to segment the video stream and track the projected markers using pre-processing techniques, morphological operators and detection algorithms; to establish a correspondence between the markers in the sequences of images recorded by the cameras; to reconstruct the 3D coordinates of the markers; to reconstruct the trunk surface from cloud of unorganized points using the QHULL method. Measures of volume and perimeter along the respiratory cycle and contour maps were used to analyze the reconstructed surface. The accuracy of volume measurement was assessed by comparing the volume of a male torso mannequin obtained by water displacement and through the proposed method. An application example of the analysis was conducted with two subjects with different shapes and sizes of different body, a female with severe idiopathic scoliosis and a male without spine deformation. The change in trunk volume was compared with the air exchanged volume variation during the respiratory cycle measured by spirometry. The method was able to reconstruct with high level of detail the trunk surface allowing the identification of anatomical structures and differentiation of the trunk shape during the phases of the respiratory cycle. The relative accuracy in determining the volume was 2.9%. The variation of the trunk volume during respiration showed a coherent signal with the respiratory cycle and showed high correlation (r2 = 0,983) when compared with the volume of air exchanged. The analysis of the surface through contour maps proved to be a promising tool in the identification of the surface deformations. A metodologia proposta foi avaliada através de dois experimentos: 1) Comparação entre a medida de volume de um manequim de tronco masculino obtida por imersão (M!) e através da metodologia proposta (MO); 2) Avaliação da aplicabilidade da metodologia para o cálculo do volume do tronco durante a manobra respiratória de capacidade vital. Os resultados mostram que .a metodologia foi capaz de medir e estabelecer uma correspondência automática de mais de duas mil marcas o que possibilitou a reconstrução de uma superfície com alta resolução. No primeiro experimento, a MI foi de 38,39 litros contra 37,34 litros da MO (acurácia relativa de 3%). Os resultados do segundo experimento mostraram que a metodologia foi capaz de identificar um sinal coerente com o ciclo respiratório em manobra de capacidade vital realizado pelo sujeito. Sendo assim, podemos concluir que a metodologia baseada em projeção de luz e reconstrução de superfície proposta é adequada e acurada para medir os volumes do tronco durante a respiração / Abstract: This work proposes a video-based method for measuring the trunk volumes during breathing using projected light and surface reconstruction. The method consists of: a) Projecting a grid of circular markers on the anterior and posterior trunk surface; b) Registering each surface using a pair of digital video cameras; c) Segmenting and tracking of the markers using image processing tools; d) Establishing the correspondence between marks in the video sequences registered by different cameras; e) Reconstructing the 3D coordinates; f) Reconstructing the surface; g) Calculating the trunk volume. The evaluation of the method was based on twoexperiments. 1) Comparison of the volume of a trunk model (mannequin) by immersion and using the proposed method (1M). 2) Analysis of the applicability of the method to measure a subject trunk volumes during vital capacity respiratory manoeuvre (OM). The results related to the methodological aspects showed that the method was able to measure and establish the correspondence automatically of more than two thousand points that this provides a very detailed anterior and posterior trunk surfaces. In the first experiments, the 1M were 38.39 liters against 37.34 liters obtained by OM (relative accuracy of 3%). The ana1ysis of the experiment 2 revealed the capability of the method identifies signals coherent with the respiratory cyc1es in vital capacity manoeuvres. Compõe-se de dois estudos: um referente ao movimento respiratório (MR) e outro, relativo ao comportamento cardiovascular (CC). Para isso, foram organizados dois grupos: um grupo controle, constituído por onze (n=11) voluntárias não praticantes de yoga, com idade média 66 anos ('+ OU ¿' 7anos) e que realizaram atividade física regularmente; e outro grupo yoga, formado por onze (n=11) voluntárias praticantes de yoga ( experiência média 16 '+ OU ¿' 10 anos), com idade média 66 anos ('+ OU ¿' 5 anos). As voluntárias foram submetidas a duas situações: na primeira realizaram 12 respirações máximas voluntárias, na posição em pé; na segunda concentraram-se em imagens mentais sugeridas com indução dos estados emocionais, na posição supina. Os métodos utilizados foram os seguintes: a análise cinemática tridimensional para reconstrução da superfície anterior do tronco em movimento, referente ao estudo MR, e análise psicofisiológica das mudanças da freqüência cardíaca e da pressão arterial sistólica e diastólica, referente ao estudo CC. It was comprised of two studies: 1 ¿ respiratory movement (RM); 2 ¿ cardiovascular behavior (CB). In order to achieve this, two groups were organized: a control group, composed of eleven (n=11) volunteers who do not practice yoga, and with a mean age of 66 ('+ OU ¿' 7years); and another yoga group, composed of eleven volunteers (n=11) who practice yoga (mean experience of 16 '+ OU ¿' 10 years), and mean age of 66 ('+ OU ¿' 5 years).
