Children's Attention Deficit/Hyperactivity Disorder self-help books and the politics of correction

AD/HD is a prevalent medical diagnosis given to 3-7% of children in British Columbia. Since the diagnosis’ inception in 1902, children’s behaviour has been described in similar ways, but labels to define it have continuously changed, reflecting the diagnosis’ mutability and connection to shifting discourses of normativity. An analysis of moments in the text of 13 children’s self-help books illuminates that the process books refer to as correction is actually a disciplinary process exercised in children’s social relations, which guide them to act according to socially constructed notions of normative behaviour. I draw two conclusions from my research: (a) the correction of AD/HD-diagnosed children is a political process supported by a complex network of power relations and (b) diagnosed children’s lives are emmeshed in practices of disciplinary power that establish, and maintain, their state of being normalised.

Delivering Acceptance and Commitment Therapy (ACT) for mental health disorders across group and guided self-help formats : a meta-analysis and randomised controlled trial

Background: Acceptance and Commitment Therapy (ACT) has shown promise as an effective intervention in the treatment of mental health disorders. In the last decade, the delivery of ACT has expanded to include various formats (e.g. groups, self-help, online and phone apps). Further research is needed to evaluate whether such delivery formats are a viable extension of ACT. Furthermore, the existing evidence base of certain alternative delivery formats have yet to be reviewed. This thesis portfolio sought to contribute to this area of research. Methods: A systematic review of the literature was conducted to investigate the efficacy of group-based interventions for mental health disorders using ACT. Five databases were systematically searched, manual searches were conducted and corresponding authors were contacted. Studies which used a randomised-controlled design, with adult samples and investigated group-based ACT interventions for mental health disorders were included. A meta-analysis of the included studies was conducted for post-intervention and follow-up data. In the empirical study, an ACT manual was trialled using a randomised-controlled design to investigate the efficacy of using ACT in a guided self-help context. Participants with anxiety/depression were randomly assigned to receive either the ACT intervention or treatment as usual (TAU). Those in the ACT group were posted an ACT manual and received two telephone calls. Outcome measures were analysed after the six-week intervention. Results: From the meta-analysis, 18 randomised-controlled trials were identified, 14 of which focussed on anxiety and depression. The findings suggest that ACT-based groups have a large effect on symptom reduction when compared to non-active comparisons at post-treatment and a moderate effect when compared to non-active comparisons at follow-up. Additionally, there was a small effect in favour of ACT when compared to active treatment controls at post-treatment and equivalent effects when comparing ACT to active treatment controls at follow-up. Similar effects were found when separately comparing the 14 studies which focussed primarily on anxiety and depression. The empirical study revealed that guided self-help was found to be no more effective in improving quality of life or reducing psychological distress than the TAU group. However, such results should be interpreted with caution as the small sample size and high attrition rate indicates that further research with larger samples and follow-up are needed before strong conclusions can be made. Conclusions: The findings of this research indicate that group-based ACT interventions may be a suitable alternative delivery format for service providers in the provision of common mental health disorders, particularly anxiety and depression. Further research is needed before any strong conclusions can be made regarding the efficacy of guided self-help for anxiety/depression.

An interpretative phenomenological analysis of HIV positive individuals’ experiences of being in a support group

People who have been diagnosed HIV positive often experience distress and anxiety due to uncertainties pertaining to the implications of an HIV positive status. Research has shown that support groups have always been a way for people to cope with the distress and stressful circumstances associated with health conditions such as HIV. This research investigated the role of face to face support groups in the lives of those living with HIV. The primary focus of this research project is to provide an in-depth exploration of HIV positive individuals‟ experiences of being in a support group. The study aims to explore the positive and negative experiences of being in an HIV support group and aims to gain an understanding of the role support plays in the lives of those living with HIV. A qualitative research design was used to explore the above mentioned aim. Semi-structured interviews were conducted with five individuals, (three women and two men) who had experienced HIV support groups. The interviews were transcribed and then analysed according to the principles of interpretative phenomenological analysis (IPA). The findings of this study revealed five super-ordinate themes, which revealed not only the positive and negative experiences of being in a support group, but also the challenges that the participants‟ experienced before joining the group. Getting a sense of the participants‟ experience before joining the group allowed the researcher to get a better understanding of how useful or not the support group has been in helping them deal with the challenges of living with HIV. The themes included: „struggling to survive after diagnosis‟, „struggling to cope: adopting negative coping skills‟, „experiencing a turning-point: a will to survive‟, „attending support group: a sweet experience‟ and lastly, „attending support groups: a bitter experience‟. Findings suggest that for these participants, the advantages outweighed the disadvantages of being in a support group. Therefore this study suggests that face to face support groups are a viable and even necessary option for support. These findings support previous research and literature in regards to the importance of social support in the form of support groups in effectively assisting HIV positive people in their journey to adjust to the consequence of living with HIV.

Para tanto, selecionam-se e descrevem-se dez obras representativas desse discurso. Neste trabalho, analisa-se o discurso de autoajuda para as adolescentes, considerando-se os seguintes planos discursivos: tema, intertextualidade, ethos discursivo e cenas de enunciação. O interesse pela análise dessas quatro dimensões discursivas justifica-se porque se entende que tais planos participam de modo bastante significativo da construção da(s) identidade(s) das adolescentes. Considerando-se aspectos da materialidade linguística do corpus, a análise revela a heterogeneidade dos temas desenvolvidos pelo discurso de autoajuda para as adolescentes e das relações intertextuais que mantém com outros textos em sua materialidade linguística, bem como a dos ethé discursivos e a das cenografias instauradas nas obras analisadas. Além disso, identificam-se quatro identidades adolescentes femininas que são constituídas nas obras: a da adolescente cristã devota, a da adolescente “fashionista”, a da adolescente geek feminista e a da adolescente “comum”. Por fim, a análise semântica de determinados planos discursivos das obras conduz a proposição de um sistema de restrições semânticas comum a essas obras, o que permite a postulação de uma formação discursiva específica: a autoajuda para as adolescentes. / This research aims to investigate which is or what are the image(s), the stereotype(s) and the identity(ies) of female adolescent that are projected in self-help works directed at female adolescents based on the theoretical-methodological subsidies of Discourse Analysis of French line, specifically in Maingueneau's reflections on concepts such as interdiscourse, global semantics, intertextuality, discursive ethos and scenes of enunciation. For this purpose, we selected and described ten representative works of this discourse. In this work, the self-help discourse for female adolescents is analyzed, considering the following discursive plans: theme, intertextuality, discursive ethos and scenes of enunciation. The interest in the analysis of these four discursive dimensions is justified because it is understood that such plans participate quite significantly in the construction of the female adolescent's identity(ies). Considering aspects of linguistic materiality of the corpus, the analysis reveals the heterogeneity of the themes developed by the self-help discourse for female adolescents and of the intertextual relations that it maintains with other texts on its linguistic surface, as well as of the discursive ethé and of the scenographies in the works analyzed. In addition, we identified four feminine adolescent that are constituted in the works: that of the devout Christian teenager, that of the fashionable teenager, that of the geek feminist adolescent and that of the "ordinary" teenager. Finally, the semantic analysis of certain discursive plans of works leads to the proposition of a common system of semantic restrictions to these works, which allows the postulation of a specific discursive formation: self-help for female adolescents. / En esta pesquisa se investiga(n) cuál(es) es(son) la(s) imagen(es), el(los) estereotipo(s) y la(s) identidade(s) de la adolescente que se proyectan en obras de autoayuda dirigidas al público adolescente femenino de acuerdo con los aportes teórico-metodológicos del Análisis del Discurso de línea francesa, más específicamente con las reflexiones de Maingueneau acerca de conceptos como el de interdiscurso, el de semántica global, el de intertextualidad, el de ethos discursivo y el de escenas de enunciación. Para eso, se seleccionan y se describen diez obras representativas de ese discurso. En este trabajo, se analiza el discurso de autoayuda para las adolescentes, considerando las siguientes propiedades discursivas: tema, intertextualidad, ethos discursivo y escenas de enunciación. El interés por el análisis de estas cuatro dimensiones discursivas es justificado porque se entiende que ellas participan de modo bastante significativo en la construcción de la identidad de las adolescentes. El análisis del corpus revela la heterogeneidad de los temas desarrollados por el discurso de autoayuda para las adolescentes, de las relaciones intertextuales que mantiene con otros textos en su materialidad lingüística, de los ethé discursivos y también de las escenografías instauradas en las obras analizadas. Además, se identifican cuatro identidades adolescentes femeninas que están constituidas en las obras: la de la adolescente cristiana devota, la de la adolescente aficionada a la moda, la de la adolescente geek feminista y la de la adolescente "común". Por fin, el análisis semántico de determinadas dimensiones discursivas de las obras conduce a la proposición de un sistema de restricciones semánticas común a esas obras, lo que permite la postulación de una formación discursiva específica: la autoayuda para las adolescentes.

We live in a fast, fluid and dynamic world, where the inherited certainties of tradition and first modernity give place to uncertainties, risks and unpredictability. Changes are socioeconomic, technological and cultural involving new forms of individual thinking, feeling, acting, and living. Such changes occur singularly in working life, accompanying the changes in the capitalist production mode. The new stage of post-industrial capitalism produces new ideologies and orders of discourse that strongly challenge subjectivities. These ideologies and orders of discourse are materialized and disseminated through self-help manuals and other hybrid types, which tend to favor certain representations of work, the company and the employee, and also to inculcate certain ways of being and acting inside and outside of the workplace. This research investigated how this type of literature provides resources that claim to enable the individual to cope with the new work configuration nowadays and with the existential dilemmas arising from this new scenario. Assuming here the language as a social practice, our main objective was to investigate how the self-help books are related to the sense of uncertainty that characterize the contemporary experience of work and other spheres of life. For this we use the approach of Critical Discourse Analysis (CDA) in order to identify the main ideological investments of self-help speech, as presented in two books of the genre: The Monk and the executive, James Hunter (2004) and Be the leader of yourself, Augusto Cury (2004). The analysis revealed that the works studied tend to disseminate certain representations of current problems (for example, unstable and precarious work, the difficulties of leadership) problems as personal and subjective nature, the exclusive purview of the individual. Both take issues and social problems to the sphere of interiority, proposing theories about how the mind works and intervention techniques for personal development and for changing behaviors and habits. To represent these problems as issues of interiority and interpersonal relationships, the works tend to blame the person for the solution of such problems and the pursuit and achievement of your success and happiness. In according to a consumer society depoliticized and fragile in criticizing the system and the social and economic macrostructures, the discourse of self-help drive its "critical" to the individual. This individual feels forced to turn to himself and to interfere on their own attitudes, feelings and behaviors to create change and achieve success. The discourse of self-help in this context meets the new demands and requirements of the existing capitalist order: people engaged in constant self-examination and self-regulation and willing to reorganize while facing uncertainty and risk that are part of their lives. The self-help manuals contribute to the formation of new subjectivities and the inculcation of the "new spirit of capitalism", with new ways of being, acting and interacting that adapt to new labor realities and to the capitalist order in force. / A sociedade contemporânea é marcada por grandes e constantes mudanças. Vivemos num mundo veloz, fluido e dinâmico, onde as certezas herdadas da tradição e da primeira modernidade dão lugar às incertezas, aos riscos e à imprevisibilidade. São mudanças socioeconômicas, tecnológicas e culturais que implicam novas formas de o indivíduo pensar, sentir, agir, viver. Tais mudanças ocorrem de modo singular na vida laboral, acompanhando as transformações no modo de produção capitalista. O novo estágio do capitalismo pós-industrial produz novas ideologias e ordens de discurso que interpelam fortemente as subjetividades. Essas ideologias e ordens de discurso são materializadas e disseminadas em manuais de autoajuda e outros gêneros híbridos, que tendem a favorecer certas representações do trabalho, da empresa e do trabalhador, e também a inculcar certos modos de ser e agir dentro e fora do âmbito laboral. Esta pesquisa investigou como esse tipo de literatura fornece recursos que pretendem habilitar o sujeito a lidar com a nova configuração do trabalho hoje e também com os dilemas existenciais decorrentes desse novo cenário. Assumindo aqui a linguagem como prática social, nosso objetivo geral foi investigar como os livros autoajuda estão relacionados com o sentimento de incerteza que marcam a experiência contemporânea do trabalho e de outras esferas da vida. Para tanto utilizamos o enfoque da Análise de Discurso Crítica (ADC) com o intuito de identificar os principais investimentos ideológicos do discurso de autoajuda, tal como se apresentam em dois livros do gênero: O monge e o executivo, de James Hunter (2004) e Seja líder de si mesmo, de Augusto Cury (2004). A análise revelou que as obras estudadas tendem a disseminar certas representações de problemas atuais (por exemplo, o trabalho precário e instável, as dificuldades da liderança) como problemas de natureza pessoal e subjetiva, da exclusiva alçada do indivíduo. Ambas deslocam as questões e problemas sociais para a esfera da interioridade, propondo teorias sobre o funcionamento da mente e técnicas de intervenção para o desenvolvimento pessoal e para a mudança de comportamentos e hábitos. Ao representarem esses problemas como questões da interioridade e das relações interpessoais, as obras tendem a responsabilizar a pessoa pela solução de tais problemas e pela busca e alcance de seu sucesso e felicidade. Afim a uma sociedade de consumo despolitizada e frágil na crítica ao sistema e às macroestruturas sociais e econômicas, o discurso da autoajuda dirige sua “crítica” ao indivíduo. Este se vê obrigado a voltar-se para si mesmo e intervir em suas próprias atitudes, sentimentos e condutas para gerar mudança e alcançar êxito. O discurso da autoajuda, nesse contexto, atende às novas demandas e exigências da ordem capitalista vigente: de pessoas empenhadas no constante autoexame e auto-regulação e dispostas a se reorganizarem diante da incerteza e do risco que fazem parte de sua vida. Os manuais de autoajuda contribuem para a constituição de novas subjetividades e para a inculcação do “novo espírito do capitalismo”, com novos modos de ser, agir e interagir que se adequam às novas realidades laborais e à ordem capitalista em vigor.
