Hälsosamtal utifrån ett personcentrerat perspektiv -skolsköterskors erfarenheter

Sammanfattning Bakgrund: Skolsköterskan ska arbeta hälsofrämjande och sjukdomsförebyggande. Detta görs till stor del genom att samtala med elever om mående och hur hälsa kan förbättras och sjukdom förebyggas. Studier visar att skolsköterskan ofta styr hälsosamtalet vilket försvårar elevens interaktion och skolsköterskans personcentrerade förhållningssätt utmanas.  Syfte: Syftet med studien var att beskriva skolsköterskans erfarenheter av att genomföra hälsosamtal med grundskoleelever utifrån ett personcentrerat perspektiv.  Metod: En kvalitativ intervjustudie med deduktiv ansats tillämpades, utgångspunkt var den personcentrerade modellen. Genom ändamålsenligt urval intervjuades 13 kvinnliga skolsköterskor via digital plattform. Data analyserades med kvalitativ innehållsanalys på manifest nivå.  Resultat: Resultatet presenterades i tre kategorier: Partnerskap vid hälsosamtal i skolan, elevens berättelse om sin hälsa och skolsköterskans dokumentation. Skolsköterskan behövde skapa förutsättningar för att elevens berättelse skulle framträda och vara en aktiv lyssnare som bjöd in eleven till delaktighet i partnerskapet. Det var viktigt med ett bra bemötande där samtalet skedde på elevens villkor. Dokumentationen skulle genomsyras av elevens berättelse som dokumenterades med hänsyn och med elevens delaktighet.  Slutsats: Studien ökade förståelsen för vilka förhållningsätt, egenskaper och förutsättningar som främjade skolsköterskans personcentrerade arbete vid hälsosamtal. Om ett personcentrerat förhållningssätt eftersträvades kunde elevens förtroende vinnas. Skolsköterskan kunde då lättare hjälpa eleven att förbättra sin hälsostatus och guida eleven till en hälsosam livsstil. / Summary Background:  The school nurse shall work on health promotion and disease prevention. This is  mainly done by talking to pupils about well-being and how health can be improved and disease prevented. Studies show that the school nurse often controls the health dialouges, which makes it difficult for the pupils to interact and the person-centered perspective challenged. Aim: The aim of the study was to describe school nurses’ experiences of conducting health dialogues with primary school pupils from a person-centered perspective.  Method: A qualitative interview study with a deductive reasoning was applied, based on the person-centered model. Through purposive sampling, 13 female school nurses’ were interviewed by video conferencing. Data was analyzed with qualitative content analysis at a manifest level. Results: The results were presented in three categories: Partnership in school health dialouges, pupil narratives about thier health and school nurses’ documentation. The school nurse needed to establish conditions for the pupil's story to emerge and be an active listener who invited the pupil to participate in the partnership. It was important to have a good reception, where the conversation took place on the pupil's terms. The documentation would be permeated with the pupil's narrative, which was documented with consideration and participation. Conclusion: The study increased the understanding of which approach, characteristics and conditions that promoted the school nurse's person-centered work when she implemented health dialogues. If a person-centered approach is sought, the pupil's trust can be gained. The school nurse could then more easily help the pupil improve their health status and guide the pupil to a healthy lifestyle.

Bakgrund  Fysisk aktivitet under skoltid är av stor vikt då barn och ungdomar rör sig allt mindre. Skolans elevhälsa, däribland skolsköterskor, arbetar med främjandet av fysisk aktivitet i skolan. Studier kring hur en fysioterapeut kan verka inom elevhälsan är idag begränsade. Hur skolsköterskor uppfattar att en fysioterapeut kan bidra i skolhälsoarbetet var därför av intresse att undersöka. Syfte  Att undersöka skolsköterskors uppfattning om behovet av en fysioterapeut inom skolverksamheten, samt undersöka vad de anser har betydelse för att främja fysisk aktivitet i skolan.  Metod Studien var av kvalitativ design. Fem skolsköterskor intervjuades genom enskilda semistrukturerade intervjuer. Kvalitativ innehållsanalys användes för bearbetning av data.   Resultat  Utifrån insamlade data identifierades nio kategorier med sammanlagt trettiotvå underkategorier. Kategorier om behovet av en fysioterapeut inom skolverksamheten var ”Efterfrågad kompetens”, ”Hänvisar till extern kompetens” och ”Osäkerhet kring fysioterapeutens roll”. Kategorier om främjandet av fysisk aktivitet var ”Individanpassat”, ”Hemsituationen påverkar”, ”Gemensamma organiserade aktiviteter”, ”Aktivitetsfrämjande miljöer”, ”Gemensamt förhållningssätt” och ”Utmaningar att förhålla sig till”.  Konklusion  Samtliga skolsköterskor ger uttryck för att det finns ett behov av en fysioterapeut, samtidigt framkommer det att de är osäkra kring hur rollen skulle se ut inom skolverksamheten. Främjandet av fysisk aktivitet i skolan ansågs påverkas av skolans organisation, personal, föräldrar och elever. Utmaningar i arbetet med fysisk aktivitet beskrivs bero på tidsbrist och bristande kunskaper inom området. / Background  Physical activity during school hours is of great importance since children and adolescents move less and less. The school health services, including school nurses, is working to promote physical activity in school. Studies who aim to examine how a physiotherapist can contribute to the school health services are limited. To examine how school nurses perceive the need for a physiotherapist in the school health services was therefore of interest to investigate.  Purpose To examine how school nurses perceive the need for a physiotherapist in the school health services, as well as examining what they consider to play a significant role in promoting physical activity in school. Method  The study was of qualitative design. Five school nurses were interviewed through individual semi-structured interviews. Qualitative content analysis was used to analyzing the data.   Result A total of nine categories and thirty-two subcategories was identified from the collected data. Categories about the need for a physiotherapist were “Requested expertise”, “Refers to external competence”, “Uncertainty about the role of the physiotherapist”. Categories about the promotion of physical activity were “Individualized”, “Home situation affects”, “Organized activities”, “The environment affects physical activity”, “Common approach” and “Challenges to relate to”.  Conclusion  The school nurses perceive that there is a need for a physiotherapist within the school health services. At the same time, it appears that they are a bit unsure about the physiotherapists’ role. The promotion of physical activity in school is considered to be affected by the schools’ organization, school staff, pupils and parents. The work with physical activity in school has certain challenges which is described to depend on shortage of time and lack of knowledge in the area.

The Impact of a Rural School-Based Health Center on Students and Their Families in Sneedville, Tennessee: A Case Study.

The purpose of this study was to describe the impact that the school-based health center in Sneedville, Tennessee had on students who attended Hancock County High School. The study documented how the school-based health center affected students, families, and the community of Sneedville. Case study methodology was used to determine if the school-based health center impacted school attendance, mental and emotional health, sexual practices, and overall quality of life for students and their families. Data were collected through 25 interviews with health center stakeholders. The methodology included working with the staff at the school-based health center to identify the key players and key center users over the seven years of the center's existence. An interview protocol was designed for each of the seven groups that were interviewed. Data were collected from current and former students, parents of current and former students, school counselors, teachers, and the school nurse. Students who attended Hancock County High School as well as their parents were impacted by their direct access to primary comprehensive health care services. Benefits to parents included a reduction of time in missed work and lower medical care costs for students without health insurance. The center promoted and improved school attendance while distributing valuable health education information to students and parents. Because the city of Sneedville has only one medical center and no hospital, the school-based health center served the rural residents well. Beginning as one of only three such centers in the United States, the school-based health center endured and evolved into a full-service, comprehensive health care provider. The center is located in an isolated region of Northeast Tennessee; the location added to the center's impact on the residents. This study could provide a model for rural communities seeking ways to serve the health care needs of youth. In this time of economic instability and educational accountability, this school-based health center appeared to impact student care, thus allowing students to be more capable and ready to learn.

Validar um instrumento de avaliação de promoção de saúde no ambiente escolar que possa contemplar essas ações de forma ampla, a partir da percepção dos gestores e que possa ser aplicado nas diferentes realidades do país. Estudo de validação, amostra por conveniência de 247 escolas públicas e privadas de quatro munícipios brasileiros, sendo dois deles na região Sul e dois na região Nordeste do país, no ano de 2016. O instrumento avaliado é composto por 60 questões, divididas em três dimensões: pedagógica, estrutural e relacional, com respostas categóricas dicotômicas (sim ou não). Para a análise foram utilizadas matrizes de correlações tetracóricas, sendo realizadas a análise fatorial exploratória (AFE) e a análise fatorial confirmatória (AFC) do instrumento. Ao final da análise tetracórica para validação de construto permaneceram no instrumento 31 itens. Na AFE a comunalidade (h2) apresentou valores acima do mínimo recomendado (0,3) em todos os itens. Na AFC os modelos apresentaram valores próximos e superiores aos esperados de um bom ajustamento. Os resultados sugerem que o instrumento se mostrou valido e que as dimensões e fatores encontrados ao final das análises foram satisfatórios para avaliação de promoção de saúde nas escolas. / The health promotion at school encourages children and adolescents to assume behaviors and skills for life, through decision-making and critical thinking. However, health promotion initiatives at school environment needs monitoring and evaluation. To validate a health promotion evaluation instrument at school environment, which can contemplate these actions broadly, from the perception of managers and that could be applied in the different country’s realities. Validation study, convenience sample of 247 public and private schools of four Brazilian municipalities, being two of them in the southern region and two in the northeastern region of the country, in the year 2016. The evaluated instrument consists of 60 questions, divided into three dimensions: pedagogical, structural and relational, with dichotomous categorical answers (yes or no). Arrays of tetrachoric correlations were used for the analysis, with exploratory factor analysis (EFA) and the confirmatory factor analysis (CFA) of the instrument being conducted. At the end of the tetrachoric analysis for construct validation, 31 items remained in the instrument. In the EFA the commonality (h2) presented values above the recommended minimum (0.3) in all items. In the CFA the models presented values near and above the expected for a good adjustment. The results suggest that the instrument proved to be valid and that the dimensions and factors found at the end of the analyses were satisfactory for the evaluation of health promotion at schools.

School health services, health promotion and health outcomes: an investigation of the Health Promoting Schools approach as supported by school nurses

Health promotion interventions in schools have grown in popularity and have demonstrated varying degrees of effectiveness on the health of the school and its individuals. The School Based Youth Health Nurse (SBYHN) Program introduced in 1999 by Queensland Health into state secondary schools supports and encourages use of the Health Promoting Schools (HPS) approach in addressing health issues, in addition to providing individual health consultations to the school community. This Program is unique in that a health service is entering into the education system with a role of supporting implementation of a comprehensive approach to addressing health issues. The study investigates how SBYHNs support the implementation of the HPS approach in the secondary school setting and explores the health outcomes for the school community. A statewide survey of SBYHNs examines the variety of health promotion and HPS work being undertaken within schools and identifies key implementation and practice issues. Qualitative case studies of three schools further investigates the barriers faced by nurses in supporting HPS implementation, and explores the perceived outcomes of implementing the HPS approach that have begun to emerge within the school community. Results found that nurses have the capacity to support the implementation of whole-of-school health promotion, with the presence of enablers influencing the comprehensiveness with which schools address health issues or decide to adopt the HPS approach. The study also indicated several outcomes of nurse and school-supported, comprehensive school health promotion across three major areas corresponding with the HPS framework (curriculum, teaching and learning; school organisation, ethos and environment; partnerships and services) and the addition of outcomes in specific health issues. Implications for future developments in health promotion-orientated, school health service interventions and research into the evidence of effectiveness of the HPS approach are discussed.

Bakgrund: Elevhälsans medicinska insats ska tillsammans med övriga elevhälsan arbeta främst hälsofrämjande och förebyggande men ska även erbjuda eleverna enkla sjukvårdsinsatser. I skollagen och den vägledning som finns för elevhälsan finns ingen närmare beskrivning av vad enkla sjukvårdsinsatser innebär. Syfte: Syftet med denna studie var att belysa skolsköterskors uppfattning av vad enkla sjukvårdsinsatser innebär. Metod: Studien har en kvalitativ ansats. Åtta skolsköterskor intervjuades och intervjumaterialet analyserades med fenomenografisk metod. Resultat: Studien resulterade i fyra beskrivningskategorier: Egna tolkningar, Förväntningar, Förutsättningar och Bedöma nivå på insatserna. Konklusion: Eftersom det i styrdokumenten saknas motivering för och beskrivning av de sjukvårdande insatserna i skolan uppfattade skolsköterskorna i studien att det var upp till dem själva att tolka innebörden av enkla sjukvårdsinsatser. De uppfattade även att sjukvårdsinsatserna styrdes av de förväntningar som fanns från omgivningen och de förutsättningar som fanns att bedriva sjukvård i skolans kontext. De hade tankar om huruvida enkla sjukvårdsinsatser var någonting skolsköterskan utifrån sin kompetens skulle ägna sig åt. Författarna har utifrån studiens resultat kommit till slutsatsen att vidare samtal och diskussion behöver föras kring elevhälsans sjukvårdande uppdrag. Då detta område till stora delar är outforskat behöver vidare studier göras. Samtal, diskussioner och vidare forskning kan möjliggöra att vägledande policy och riktlinjer utarbetas på området. / Background: The school health services, together with school-based interprofessional teams should work primarily for health promotion and prevention but will also provide students with simple health care efforts. In the Swedish education act and in the guidance for school-based interprofessional teams do not provide a detailed description of what simple medical care measures mean. Aim: The aim of the study was to illustrate school nurses perception of simple care measures. Method: The study had a qualitative approach. Eight school nurses were interviewed and the interview material was analyzed by a phenomenological method. Results: The study resulted in four description categories: Making one’s own Interpretations, Expectations, Prerequisites and Assessing Level of Efforts. Conclusion: As there is no motivation for and description of the health care interventions in school in the governing documents, the nurses in the study considered it up to themselves to interpret the meaning of simple medical care interventions. They also perceived that health care efforts were governed by the expectations that existed from the environment and the conditions for healthcare in the school context. They had thoughts about whether simple medical care activities were in fact something the school nurse should deal with on the basis of his / her skills. Based on the results of the study, the authors have concluded that further dialogs and discussions needs to be conducted regarding the school health care services mission. As this area is largely unexplored, further studies need to be done. Conversations, discussions and further research can enable guidance policies and guidelines to be developed in this area.
