Bemötande inom primärvård för patienter med psykisk sjukdom : En kvalitativ litteraturöversikt

Introduktion Det förekommer att sjukvårdspersonal återspeglar samhällssynen som råder kring personer med psykisk sjukdom, där okunskap kan leda till stigma. Hälso- och sjukvårdslagen framhåller rätten till god vård och hälsa för alla, men i dagsläget finns inte nationella register beträffande psykisk ohälsa i primärvården, trots att det är dit de flesta patienter vänder sig. Upplevelsen av vårdmötet är av vikt då det kan bli avgörande för vidare vård. Syfte Att undersöka hur patienter med psykisk sjukdom upplever bemötande i primärvården. Metod Detta är en litteraturöversikt med deskriptiv design och kvalitativ ansats, baserat på tio kvalitativa originalartiklar valda från vetenskapliga tidskrifter funna via sökmotorn PubMed. Resultat Fyra kategorier framkom, holistisk syn, fördomar, kommunikation, samt kontinuerlig relation. Att inte bli tagen på allvar eller trodd på, bristande engagemang, samt nedsatt kommunikation mellan personal och vårdformer liksom mellan sjukvårdspersonal och patienter ledde till ökad osäkerhet. Förebyggande arbete, fast och erfaren vårdkontakt liksom avsatt tid var uppskattat då det gav känsla av meningsfullhet. Slutsats Patienter med psykisk sjukdom önskade bli bemötta holistiskt, där både psykiska och somatiska åkommor ses. Då områden som kunskap, engagemang, kommunikation och hol­istiskt synsätt ingår i sjuksköterskans kompetensbeskrivning, är litteraturöversiktens fynd viktiga insikter för omvårdnad som sjuksköterskan bör ta hänsyn till / Introduction It happens occasionally that healthcare staff reflect society’s views on people with severe mental illness, where lack of knowledge easily turns into stigmatization. The Health and Medical Care Act emphasizes patients’ right to good care and health for all, yet there is currently no national register regarding mental illness in primary care, despite that is where most patients turn to. The patient’s experience of that encounter is of importance as it can be crucial for further care.  Aim To describe how patients with severe mental illness experience the encounter from health care staff in primary care. Method Literature review, with descriptive design and qualitative approach, based on ten qualitative articles selected from scientific journals through the search engine PubMed. Results The result developed into four categories, holistic view, prejudice, communication, and continuous relationships. Not to be taken seriously or believed in, lack of engagement and coordination among staff as well as between various forms of healthcare organizations and patients led to increased feelings of uncertainty. Preventive care, experienced and a fixed care contact, along with designated time was appreciated as it gave a sense of meaning. Conclusion Patients with severe mental illness desired a holistic approach, where both mental and somatic ailments are seen. Areas such as knowledge, commitment, communication and a holistic approach are included in the nurse's competence description. The findings of the literature review are important attainments for nursing that the nurse should take into account.

Development of Manufacturing Systems for Nanocrystalline and Ultraine Grain Materials Employing Indexing Equal Channel Angular Pressing

Nanotechnology offers significant opportunities in providing solutions to existing engineering problems as well as breakthroughs in new fields of science and technology. In order to fully realize benefits from such initiatives, nanomanufacturing methods must be developed to integrate enabling constructs into commercial mainstream. Even though significant advances have been made, widespread industrialization in many areas remains limited. Manufacturing methods, therefore, must continually be developed to bridge gaps between nanoscience discovery and commercialization. A promising technology for integration of top-down nanomanufacturing yet to receive full industrialization is equal channel angular pressing, a process transforming metallic materials into nanostructured or ultraine grained materials with significantly improved performance characteristics. To bridge the gap between process potential and actual manufacturing output, a prototype top-down nanomanufacturing system identified as indexing equal channel angular pressing (IX-ECAP) was developed. The unit was designed to capitalize on opportunities of transforming spent or scrap engineering elements into key engineering commodities. A manufacturing system was constructed to impose severe plastic deformation via simple shear in an equal channel angular pressing die on 1100 and 4043 aluminum welding rods. 1/4 fraction factorial split-plot experiments assessed significance of five predictors on the response, microhardness, for the 4043 alloy. Predictor variables included temperature, number of passes, pressing speed, back pressure, and vibration. Main effects were studied employing a resolution III design. Multiple linear regression was used for model development. Initial studies were performed using continuous processing followed by contingency designs involving discrete variable length work pieces. IX-ECAP offered a viable solution in severe plastic deformation processing. Discrete variable length work piece pressing proved very successful. With three passes through the system, 4043 processed material experienced an 88.88% increase in microhardness, 203.4% increase in converted yield strength, and a 98.5% reduction in theoretical final grain size to 103 nanometers using the Hall-Petch relation. The process factor, number of passes, was statistically significant at the 95% confidence level; whereas, temperature was significant at the 90% confidence level. Limitations of system components precluded completion of studies involving continuous pressing. Proposed system redesigns, however, will ensure mainstream commercialization of continuous length work piece processing.

Severe Weather Parameters and their Effectiveness on Forecasting Tropical Cyclone Induced Tornadoes

ropical cyclone-induced tornadoes (TCIT) exacerbate the devastation that landfalling tropical cyclones have on the United States. This research applied machine learning techniques in conjunction with midlatitude severe weather parameters to create an artificial intelligence (AI) capable of predicting TCIT occurrence. Severe weather diagnostic variables were collected at thousands of gridpoints from the North American Regional Reanalysis (NARR) to characterize the environments within tropical cyclones between 1991 and 2011. A support vector machine (SVM) was generated in various configurations to obtain the most effective AI. This approach revealed many parameters that were ineffective at predicting TCITs (primarily those utilizing the effective inflow layer). In addition, the most highly configured AI were capable of predicting TCIT occurrence with a Heidke Skill Score around 0.48.

Therapist-aided self-help early intervention program for severe weather fears and phobias

Because extreme weather events are relatively common in the Southeastern U.S., and the current treatments for phobias can be time-consuming and costly, it is important to find an effective early intervention program for those individuals who are at risk of developing severe weather phobia (SWP). Participants (N = 12) were randomly assigned to the experimental or modified control condition. Participants from both conditions participated in the therapist-aided self-help early intervention program. However, the modified control condition completed posttest measures approximately 3 weeks after baseline, before beginning the intervention. The intervention was most effective at reducing subjective fear and avoidance behavior related to severe weather events. Additionally, participants’ subjective views of their fears were no longer categorized as excessive, or unreasonable, nor did they avoid or endure severe weather with intense anxiety or distress after the three-week early intervention.

Syftet med studien är att beskriva forskningen som finns avseende informationsflöde, erfarenheter och upplevelser av anhöriga som grupp när en närstående drabbas av en allvarlig, livsomvälvande eller livshotande olycka eller katastrof. Studien har gjorts genom en scopingreview där fem databaser har genomsökts för att få en överblick av relevant forskning. Resultat påvisar vikten att information kring deras närstående når anhöriga i ett tidigt skede för att minska onödig stress. I arbetet med anhöriga visar det sig att socialarbetare bör ha en plats i krishanteringsarbetet då de ofta har utbildning och erfarenheter av att jobba emotionellt och stärkande. När en allvarlig olycka eller katastrof sker har det visat sig att även anhöriga påverkas. När anhöriga inte har förmågan att på egen hand ta sig igenom krisen bör det finnas krisinterventioner att sätta in. Interventionen bör vara individuellt anpassad och kan därmed behövas långsiktigt. / The purpose of this paper is to examine the needs close family and relatives have of information and support when a serious accident or disaster occurs. This was done through a scoping review. Five databases were searched to summarize relevant research. The results indicate how important it is with information to close family and relatives in an early stage to minimize their negative stress. Social workers should have a given place with the trauma team since they usually have the training and skills to work emotionally and supportive with people. When a severe accident or disaster occurs, it is proven that close family and relatives are also affected. A crisis intervention should be available when they don’t have the resources to cope with the crisis on their own. Crisis interventions should be individually adapted and might be needed fora long time.

Equal-channel angular pressing (ECAP) is one of the severe plastic deformation (SPD) to produce ultra-fine grain (UFG) material, and its principle and microstructural developments. The majority of papers on SPD materials have been devoted to the face centered cubic (FCC) structure materials such as Al, Cu and Ni. The UFG of high alloy ECAP processing has been difficult up to now, but we were successful in this study. Fe-20%Cr steel with extremely low C and N has different slip behavior from the FCC. A survey of potential level of burden experienced by South African caregivers of children with severe intellectual disability

Introduction: Caregivers of a child with severe intellectual disability (CWSID) often embody the sole provider of care for their CWSID. Due to the severity of the difficulties their CWSID may experience, these caregivers pose a variety of challenges when caring for their children. The demands placed on the caregiver could lead to high levels of caregiver burden which can influence the quality of child-caregiver relationship. This study evaluated the relationship between caring for a CWSID and the level of burden by using the Burden Scale for Family Caregivers. Additionally, the investigation aimed to identify potential risk factors faced by South African caregivers of CWSID that could possibly increase levels of caregiver burden. Lastly, the study proposed intervention practices that took into account both the potential risk factors identified, as well as the level of caregiver burden experienced to aid childcare. Method: Data were utilised from 218 South African caregivers of CWSID from all over the country. The study applied a quantitative approach and made use of a survey available online and paper based. The questionnaire comprised of two parts. Section A was a demographic questionnaire aimed at identifying possible risk factors to increase levels of burden and Section B covered the Burden Scale for Family Caregivers. Descriptive and inferential statistics were used for analysis. Results: The majority of caregivers (67%) identified family as their main or only form of support structure. The study did not find any obvious risk factors that could indicate a relation between the level of burden and the different demographic factors. It was found that caregivers of CWSID experience moderate to severe levels of caregiver burden due to caring for their CWSID. Conclusion: As caregivers of CWSID experience severe levels of caregiver burden, the results of the present study indicate that caregivers experience this burden in various areas of living influencing their quality of life. It is recommended that a caseworker be assigned to each CWSID to decrease caregiver burden. THE RELATIONSHIP BETWEEN EMPLOYMENT STATUS AND NONVOCATIONAL OUTCOMES FOR PERSONS WITH SEVERE MENTAL ILLNESS ENROLLED IN VOCATIONAL PROGRAMS: A LONGITUDINAL STUDY

Indiana University-Purdue University Indianapolis (IUPUI) / The primary purpose of the current study was to determine the relationship between employment and the nonvocational functioning of people with severe mental illness in a prospective 24 month study, as a partial replication of another study. An employment typology was utilized that was comprised of participants at four employment levels: no work (those who did not work the entire study period), minimal work (those who worked 24 weeks or less in competitive and/or noncompetitive, paid employment), paid work (those who worked 24 or more weeks, the majority of which was spent in noncompetitive, paid employment), and competitive work (those who worked 24 or more weeks, the majority of which was spent in competitive employment). Both objective and subjective outcomes were addressed, including quality of life, symptoms, psychiatric hospitalizations, social networks, and residential status. Participants consisted of persons with severe mental illness (most with a schizophrenia-spectrum disorder) who were receiving employment services at a large, urban psychiatric rehabilitation center. Mixed effects regression modeling and logistic regression were used to analyze the date. Results indicate that the competitive work group experienced an accelerated improvement in negative symptoms across time and was less likely to have psychiatric hospitalization days as compared with the no work group. The paid work group showed an accelerated improvement in social network scores across time as compared with the no work, minimal work, and competitive work groups. The full sample demonstrated improvements across the study period regardless of employment typology status in the areas of overall quality of life, financial quality of life, cognitive symptoms, and social networks. In conclusion, this study adds to the growing body of evidence indicating that work, particularly periods of extended competitive work may lead to important benefits in clinical and social domains, although threats to internal validity that could not be ruled out preclude a causal link from being established. Future research is needed to further investigate this relationship, particularly in regards to those outcomes that are not well understood, such as residential status.

Using an Electronic Visual Activity Schedule for Students with Severe Disabilities to Independently Complete Life Skills and Increase Communication Skills.

The question of the quality of life, such as the ability to complete the routine skills independently has been widely debated in the special education field. However, perspectives have not adequately addressed the various outcomes of electronic visual activity schedule via iPad (First Then App) on assisting the individual with severe disabilities completing routine skills independently. This study aims to investigate the effects of using electronic visual activity schedules with special attention to learning routine skills and communication skills for students with severe disabilities. The following questions will be examined during the research period: What is the effect of visual activity schedules via an iPad First Then App, on the percentage of steps completed for routine tasks for students with severe disabilities? 2. What is the level of perceived student engagement when using the app vs. typical instruction? 3. What is the effect of the First Then App on the communication skills when completing daily life skills and school routines? 4. What value do the teacher and students place on the use of an electronic VAS to teach daily life and school routines? The study will take place in a suburban elementary classroom for a student with significant disabilities, and who match the inclusion criteria.The data will be collected via single-subject multiple baselines across skills (ABAB design). Participating student will be asked to complete routine skills without (baseline data) and with (intervention data) the technology-based intervention. The least to most intrusive prompts will be consistent with the First Then the application, and the positive reinforcement will be included to prevent behavioral challenges. A member of the research team will collect the interobserver agreement and procedural fidelity. As a result, the researchers will expect to see the independent variable, the app with systematic instruction, to have an impact on the dependent variable, overall skill acquisition of the targeted life skill and increases in overall communication. Also, We expect to have a functional relation between the independent variable and dependent variable. This paper sheds new light on the rarely acknowledged issue of using electronic visual activity schedules for students with severe disabilities to learn routine skills and communication skills.
