Sífilis em gestantes e o tratamento do parceiro sexual

Introdução: A sífilis em gestantes é um problema de saúde pública, com casos crescentes a cada ano. O tratamento do parceiro sexual da gestante com sífilis, é de suma importância, pois a falta de tratamento deste pode invalidar todas as medidas de controle instituídas durante o cuidado pré-natal. Objetivo: Descrever a ocorrência de tratamento do parceiro sexual e avaliar fatores maternos que favorecem a realização do tratamento do parceiro sexual das gestantes com sífilis gestacional atendidas no Hospital Fêmina (HFE). Método: Estudo transversal descritivo onde foram descritos os casos de pacientes com diagnóstico de sífilis gestacional atendidas no Serviço de Obstetrícia do HFE no período de 01 de janeiro de 2007 a 31 de dezembro de 2014, e seus respectivos parceiros. A coleta de dados foi realizada através dos dados encaminhados pelo Serviço de Controle de Infecção Hospitalar do Hospital Fêmina à Vigilância em Saúde do Município de Porto Alegre em sífilis, em conjunto com os prontuários das pacientes estudadas. O projeto teve aprovação do Comitê de Ética em Pesquisa do Grupo Hospitalar Conceição com protocolo de número 47914815.2.0000.5530. Resultados: Foram identificados 771 casos de sífilis em gestantes, e desses 570 não tinham informações sobre o tratamento do parceiro sexual da gestante. Dos 201 casos de gestantes com informações sobre o tratamento do parceiro sexual, 25 (12,44%) parceiros foram adequadamente tratados. Na análise univariada comparando gestantes com parceiros tratados para sífilis e não tratados, identificaram-se características associadas à ocorrência de tratamento adequado do parceiro em relação a mulheres que apresentaram sífilis gestacional: a) mais de oito anos de estudo (p=0.022), b) acompanhamento pré-natal adequado (p=0.010) e diagnóstico da sífilis no pré-natal (p=0.003). Conclusão: Escolaridade, diagnóstico precoce de sífilis, e a realização de pré-natal adequado parecem ser fatores determinantes para o adequado tratamento do parceiro e prevenção da transmissão vertical da doença. / Introduction: Syphilis in pregnant women is a public health problem, with increasing cases each year. Treatment of the sexual partner of the pregnant women with syphilis is very important because the lack of treatment may invalidate all control measures imposed during prenatal care. Objective: to describe the occurrence of treatment of the sexual partner and evaluate maternal factors that favor the realization of the treatment of the sexual partner of pregnant women with gestational syphilis treated in Hospital Fêmina. Method: This is a cross-sectional descriptive study, in which were described the cases of patients diagnosed with gestational syphilis in the Obstetrics Department of Hospital Fêmina, and their partners, from January 1st 2007 to December 31st 2014. Comunicação de diagnóstico de HIV a parceiros sexuais na perspectiva do cuidado em saúde / Disclosure of HIV infection to sexual partners in the comprehensive care context

A denominada ¨resposta brasileira à epidemia de aids¨ é exemplar pelas suas políticas e iniciativas públicas, incitando continuamente a emergência de desafios à atenção à saúde das pessoas vivendo com HIV. Um deles diz respeito a uma questão polêmica, ainda que pouco debatida no cenário nacional: a comunicação de diagnóstico de HIV aos parceiros sexuais de pessoas soropositivas. Por um lado, essa questão evoca a responsabilidade pelo controle da cadeia de transmissão do HIV. Por outro, há a preocupação com o bem estar psicossocial do paciente em suas relações afetivo-sexuais, conjugais e familiares. O objetivo do presente trabalho foi apreender, através dos discursos de sujeitos envolvidos nessa prática, como essas distintas lógicas - de uma perspectiva coletiva, informada pelos pressupostos da Saúde Pública e de uma tônica no indivíduo e suas singularidades - são operadas e articuladas nas práticas de comunicação de diagnóstico de HIV a parceiros sexuais, em serviços de saúde especializados em DST/aids, do Município de São Paulo. Trata-se de estudo qualitativo, orientado pelos discursos de profissionais de saúde, de membros da Comissão Nacional de Aids, de técnicos em vigilância epidemiológica e pacientes, captados através de grupos, focal e educativo, e de entrevistas individuais em profundidade, com roteiro semi-estruturado, entre 2007-2008. A base teórica para interpretação dos discursos combina aportes conceituais sobre a Esfera Pública e a Teoria da Ação Comunicativa (Habermas), e sobre o Cuidado (Ayres). A comunicação de diagnóstico de HIV aos parceiros sexuais apresenta-se como uma questão que, embora importante, não alcança esferas de debates públicos com os diferentes atores sociais envolvidos. Na prática, a lógica epidemiológica não se materializa nem é almejada sob a forma de ações clássicas da vigilância epidemiológica, como a busca de comunicantes. São valores implicados em seus pressupostos, contrastados com aqueles das esferas privadas e íntimas, que parecem tornar problemática a comunicação de diagnóstico de HIV ao parceiro sexual. Dentre as estratégias utilizadas pelos profissionais, oscila-se entre a ameaça (de cunho policialesco) e a cumplicidade (que trata a comunicação como um problema íntimo). O principal foco dessas iniciativas é minimizar o estigma que cerca o portador do vírus. Da amplitude das interpretações e proposições que consideram o estigma nas práticas de saúde, vislumbra-se que a sua ativa problematização nas situações concretas da atenção pode ser um caminho possível e prático para o seu enfrentamento, por meio da criação de repertórios argumentativos. Esse modo peculiar de fazer face ao estigma pode deslocar as perspectivas, higienista e de regulação jurídica, para horizontes normativos técnica, ética e politicamente relevantes para integrar comunicação de diagnóstico de HIV ao parceiro sexual e atenção à saúde das pessoas vivendo com HIV. / The Brazilian response to the AIDS epidemic outstands for its policies and public initiatives, continuously stimulating the challenges for the healthcare of people living with HIV to come out. One of them refers to a polemic issue, yet little debated in the national agenda: the HIV disclosure to sexual partners of HIV positive people. This question claims for the responsibility over HIV transmission control. On the other hand, the patients psychosocial well-being in their sexual-affective, marital and family relationships raises concern. The objective of the present study was to understand, based on the discourses of the subjects involved in this practice, how these distinctive logics the collective perspective based on Public Health concepts and the emphasis on the individual and their singularities are managed in the practices of disclosing HIV infection to sexual partners in STD/AIDS specialized clinics in the city of São Paulo. A qualitative study was carried between 2007-2008 out with health professionals, members of Brazils AIDS Program, surveillance technicians and patients who participated in focus and education groups and answered individual in-depth interviews, both oriented by a semi-structured guide. Public Sphere/ Communicative Action Theory (Habermas) and Comprehensive Care (Ayres) were the theoretical framework of the analysis. HIV disclosure to sexual partners constitutes an issue that, despite relevant, does not reach the public debate domains with the different social actors involved. In practice, the epidemiology logic is neither materialized, nor desired in terms of classic surveillance practices, such as the contact tracing. The contrast of assumptions between public and private spheres seems to be a barrier to HIV disclosure to the sexual partner. The strategies adopted by professionals range from threatening (acting as if they were the police) to complicity (the disclosure is managed as an intimate matter). The main focus of these initiatives is to minimize the stigma towards the seropositive person. From the great variety of interpretations and propositions about stigma in health practices, the constitution of argumentative repertoires that actively question the stigma in concrete situations may be a possible and practical way of facing it. This peculiar way of approaching stigma may change hygienist and law regulated perspectives into normative horizons that are technical, ethical and politically relevant to integrate HIV disclosure to sexual partners and care of people living with HIV.

Coercive and Deceptive Predictors of Sexual Risk: The Moderating Role of Self-esteem

Risky sexual practices can lead to concerning public health issues, including sexually transmitted infections (STIs) and unintended pregnancy. Coercive or deceptive behaviors by one’s partner to engage in risky practices may be one factor contributing to sexual risk. This study examined experiences of sexual risk coercion and deception, including partner sexual infidelity, coerced condom nonuse, and fear of negative partner reaction to condom request, as predictors of engagement in sexual risk behaviors, including condom use, safer sex communication, and lifetime number of sexual partners. Self-esteem was examined as a moderator. College students (N = 216) were recruited through the ETSU Sona System to complete self-report surveys via the REDCap survey platform. Using SPSS, linear regression analyses and PROCESS moderation analyses were performed. In analyses of covariance, gender, race/ethnicity, and sexual orientation made no significant contributions to the models. Partner sexual infidelity significantly predicted lifetime number of sexual partners (F(1, 210) = 11.042, p = .001, β = 3.088, SE = .929), R2 = .050. Self-esteem was found to be a significant moderator of this relationship (F(1, 197) = 8.759, p = .0035). Fear of negative partner reaction to condom request significantly predicted lifetime number of sexual partners (F(1, 213) = 4.930, p = .027, β = 2.609, SE = 1.175), R2 = .023. Future research should continue to examine the psychosocial determinants of sexual behaviors, as increased understanding will inform more effective sexual risk intervention to reduce HIV, other STIs, and unplanned pregnancy among college populations.

Number of Sexual Partners Predicting Self-Esteem, Sexual Satisfaction, and Sexual Self-Efficacy

<p>The present study predicted that gender, relationship status, sexual orientation, and sexual debut would have an impact on number of sexual partners, which would then affect self-esteem, sexual satisfaction, and sexual self-efficacy. Through an online survey, participants anonymously responded to statements related to these concepts in order to test relationships among demographics and the number of sexual partners as well as among the number of sexual partners and self-esteem, sexual satisfaction, and sexual self-efficacy. Overall, men were much more influenced by their relationship status and number of sexual partners than women, implying that the men are still affected by the sexual double standard through the praise they receive for having many sexual partners.</p>

A training programme for professional nurses to support patients in disclosing HIV-Positive status to sexual partners at selected public hospitals in Limpopo Province, South Africa

(Nursing Science)) --University of Limpopo, 2019 / The ability of professional nurses to support and motivate people living with HIV and AIDS to disclose to sexual partner continue to be a challenge based on the social, economic, psychological and ethical circumstances that surround the process to do so. A qualitative research study using semi-structured interviews with a schedule guide whereby probing questions were used to elicit more data. Two focus groups with 6 professional nurses were also conducted to explore and describe professional nurse’s knowledge and practices as they support People Living With HIV and AIDS (PLWHA) to disclose their positive status to sexual partners. The interviews were carried out in five (5) district hospitals of Limpopo Province, South Africa. Dickoff, James, and Wiedenbach (1968) practice theory guided the study. Study findings revealed that professional nurses do understand the concept but however it is difficult for them to translate and link learning and understanding in the clinical area. Social, economic, psychological and ethical dilemmas pose a challenge for professional nurses to support individuals to disclose to sexual partners. Professional nurses refer individuals with challenges to psychologists and social workers, but however, they do not receive referral back on the outcome. The absence of support groups and link with community-based groups to support PLHWA reduce efforts to improve knowledge on benefits of disclosure to communities led by PLHWA including reduction of stigma and discrimination associated with the diagnosis. The study therefore, recommends a training programme that links the South African Nursing Council statutes, including that of World Health Organization and Department of Health with teaching and learning methods that clarify and simulate real clinical situation to enhance the translation of this policy in the real-life situation. The training programme further suggests a link with community base structures led by PLWHA to enhance disclosure of HIV positive status to a sexual partner, reduction of stigma and discrimination associated with the diagnosis and reduce the incidence of HIV among people living with HIV. INTRODUÇÃO: No Brasil, os doadores de sangue são submetidos à triagem clinica antes da doação através de um questionário padronizado que segue recomendações do Ministério da Saúde. Apesar de não ser obrigatório, os serviços de hemoterapia brasileiros costumam perguntar aos candidatos sobre o número de parceiros sexuais nos doze meses que precederam aquela doação de sangue. Os candidatos que referem um número de parceiros acima do limite permitido em cada hemocentro são recusados na triagem clínica pré-doação. Este estudo analisa as características demográficas, o número de parceiros heterossexuais e marcadores sorológicos em 689.868 doações de três hemocentros brasileiros, participantes do REDS-II, entre 1 de julho de 2007 a 31 de dezembro de 2009. MÉTODOS: Os doadores foram classificados de acordo com o número máximo declarado de parceiros sexuais nos últimos doze meses permitidos em cada hemocentro. Os valores de corte para Belo Horizonte, Recife e São Paulo são dois, três e seis parceiros, respectivamente. Foram realizados os testes de qui-quadrado e regressão logística a fim de examinar associações entre características demográficas, número de parceiros sexuais em doze meses e taxas de marcadores sorológicos individuais e globais positivas para o vírus da imunodeficiência adquirida (HIV), vírus linfotrópico humano (HTLV) tipo 1 e 2, hepatite B, hepatite C e sífilis. RESULTADOS: Doadores de primeira vez, jovens e com maior nível educacional foram associados a maior número de parceiros sexuais recentes, assim como o gênero em São Paulo e Recife (p < 0, 001). Marcadores sorológicos globais, para HIV e sífilis foram associados com maior número de parceiros em São Paulo e Recife (p < 0, 001), mas não em Belo Horizonte. Na análise de regressão logística, o número de parceiros sexuais foi associado com marcadores sorológicos positivos [razão de chance ajustada (AOR) 1,2-1,5], especialmente no HIV (AOR 1,9-4,4). Em conclusão, o número de parceiros sexuais nos doze meses antes da doação de sangue, foi associado com positividade para HIV e taxas globais de marcadores sorológicos para doenças transmissíveis por transfusão. A associação não foi consistente entre os centros, tornando difícil definir um valor de corte uniforme para todos os hemocentros brasileiros. Estes achados corroboram que o uso da informação dos contatos heterossexuais recentes é um importante critério de inaptidão e de melhora na segurança transfusional no Brasil / INTRODUCTION: In Brazil, blood donors undergo medical screening before donation through a standardized questionnaire that follows recommendations from the Ministry of Health. Although not required, most of the Brazilian blood centers routinely ask candidates about the number of sexual partners in the twelve months preceding that blood donation. Candidates who refer a number of partners over the limit allowed in each blood center are refused at the predonation clinic screening. This study analyzes the demographic characteristics, the number of heterosexual partners and serological markers in 689,868 donations from three Brazilian blood center, participants in the REDS-II, from 1 July 2007 to December 31, 2009. METHODS: Donors were classified according to the stated maximum of the number of sexual partners in the last twelve months, allowed at each blood center. The cutoff values for Belo Horizonte, Recife and Sao Paulo are two, three and six partners, respectively. We conducted the chi-square and logistic regression to examine associations between demographic characteristics, number of sexual partners in twelve months and rates of individual and global serological markers positive for human immunodeficiency virus (HIV), human lymphotropic virus (HTLV) type 1 and 2, hepatitis B, hepatitis C and syphilis. RESULTS: First time donors, young and better educated were associated with increased number of recent sexual partners, as well as gender in São Paulo and Recife (p < 0.001). Global serological markers for HIV and syphilis were associated with greater number of partners in Sao Paulo and Recife (p < 0.001), but not in Belo Horizonte. In logistic regression analysis, the number of sexual partners was associated with positive serological markers [adjusted odds ratio (AOR) 1.2 to 1.5], especially HIV (AOR 1.9 to 4.4). In conclusion, the number of sexual partners in the twelve months before blood donation was associated with HIV positivity and overall rates of serologic markers for transfusion-transmissible diseases. The association was not consistent among the Brazilian blood centers, making it difficult to set a uniform cut off value for all blood banks in Brazil. These findings confirm that, the use of information from recent heterosexual contacts is an important criterion of disability and improvement in transfusion safety in Brazil
