Understanding key challenges in health and social care integration in Scotland : principal stakeholders' perspectives

This study explores the challenges faced by Principal Stakeholders in achieving Scotland's strategic approach to health and social care integration. This is a developing area for academic literature, limited to, but not devoid of the experiences of integration of specific public sector areas and the experience of other countries that have implemented an integrated approach. The research studies the views and opinions of 12 Chief Officers from three areas of public sector provision; the Scottish Government, Heath Boards and Local Authorities to elicit their initial fears and concerns, approaches and development. This qualitative research approach provided a richness and depth of data which is presented within the thesis. The evidence gathered informed the design of the influencing factors and a conceptual model for health and social care integration and importantly for future integration models which may be applied to future projects. It is concluded from this study that health and social care integration while challenging, is achievable through a planned approach when considering the key influencing factors. The factors, are flexible depending on the stage of integration and the priorities of the Integration Board, and therefore should be used accordingly. The development of the factors would not have been achievable without the considerable input of the participants involved. The key contributions of this research span across academic literature, research methodology and professional practice. The study contributes to the current research considerations by developing an approach based on the work undertaken by the professionals and learning from the unintended consequences of the experiences of such early intervention. Finally, the influencing factors and conceptual model are offered as evidence of the research undertaken as a contribution to practice. The value to academics, practitioners and the wider public service and agencies involved in integration lies in the frameworks contextual factors and ultimately the flexibility of application.

<p>The purpose with our essay was:</p><p>-          To study how personal assistants understand their occupation role.</p><p>-          To describe the attitude of personal assistants to the right of self-determination of their care receivers.</p><p> </p><p>We have used a qualitative research method called phenomenografic.</p><p> </p><p>The empirical material was based on four interviews that served as the data collection methods. The interviewees were four women from two different municipalities of Middle Sweden. The interviewed personal assistants work with adult people who have mental disabilities.</p><p> </p><p>As a result we came up with eight categories which showed that personal assistants, despite some difficulties, perceive their careers as crucial for society’s well being. The phenomenon of self-determination was much easier to describe than to put to practice. Teater och utvecklingsstörning : En studie av Ållateatern

This dissertation is an inductive, qualitative study of a theatre programme with persons with intellectual disabilities. The aims of the dissertation were: • to explore the experiences had by the actors as expressed through their descriptions, images and narratives, • to explore the context in which these descriptions, images and narratives take place and, • to conceptualise the experiences had by the actors. In the project, the following three qualitative methods were combined: qualitative interviews, participant observations and research circles. The theoretical framework is mainly built on three theoretical standpoints - social constructionism; the theoretical perspective that describes disability as a balance between deviance and normality; and cultural analysis. The analysis shows that the theatre and artistic programme are experienced as something new, interesting and qualitatively different from their earlier experiences. Their daily activities in the theatre are organized around and influenced by an artistic rationale. The work, the physical environment, and relations with the leaders of the theatre are different from the traditional social care setting (daily work places, group homes etc.) in which the actors are socialized. The analysis also shows a tension, in the theatre practice, between disability policy and artistic aims. On one hand, the programme has to consider the consequences of disability and disability ideology in their daily work. On the other hand, the focus of the theatre is on artistic aims and on the actors’ skills as producers of theatre and art. The social care setting, of which the theatre is a part, emphasises deviance (the problems and shortcomings of the actors) whereas the theatre emphasises normality (the artistic skills of the actors). Through their participation in the theatre company, the actors have gained a new role - the role of actors and producers of theatre and art, and not only the role of clients in the social service system. The actors also experience ambivalence from their social surroundings, such as in the theatre arena where the audience applauds their acting, whereas in other arenas they experience dissociating interaction. Both roles, being a client and being a theatre actor, are integrated in the self-understanding of the actors. In this way, the actors’ self-images can be described as a balance between deviance (intellectually disabled) and normality (theatre actor). Through their participation in the theatre, they also gained fellowship among other actors and between the actors and the leaders. This is analyzed in terms of affinity grouping. That is, the theatre can be understood as a community where the members share common interests and goals. Through the processes and acts in this community, the actors develop confidence and the strength to stand up for themselves. Against this background, parallels are discussed to disability culture and disability arts.

Darbe atlikta teorinė ir empyrinė senyvo amžiaus asmenų, gyvenančių socialinės globos įstaigose, socialinių problemų analizė. Iškelta hipotezė, kad senyvo amžiaus asmenų, gyvenančių Šiaulių apskrities ilgalaikės socialinės globos įstaigose, socialinės problemos yra įtakojamos jų socialinių poreikių patenkinimo/nepatenkinimo. Anketinės apklausos metodu atliktas tyrimas, kurio tikslas –išaiškinti ir apibūdinti pagrindinius senyvo amžiaus žmonių, gyvenančių socialinės globos namuose, socialinių problemų bruožus. Atlikta statistinė ir kokybinė duomenų analizė. Tyrime dalyvavo 128 senyvo amžiaus asmenys, gyvenantys Šiaulių apskrities ilgalaikės socialinės globos įstaigose ir 4 įstaigų vadovai. Empirinėje dalyje nagrinėjamos Šiaulių apskrities ilgalaikės socialinės globos įstaigose gyvenančių senyvo amžiaus asmenų socialinės problemos ir jas įtakojantys veiksniai. Svarbiausios empirinio darbo išvados teigia, kad senų žmonių socialines problemas Šiaulių apskrities ilgalaikės socialinės globos įstaigose įtakoja netektys ir pokyčiai jų gyvenime, iššaukti persikėlimo gyventi į globos namus. Socialinės problemos daugiausiai sietinos su paties seno žmogaus ir jį supančios socialinės aplinkos sąveikos įtakotu nepakankamu socialinių poreikių tenkinimu. Didžiausią poveikį senyvo amžiaus asmenų socialinių problemų kilimui daro nepatenkinti saugumo, savarankiškumo ir savirealizacijos socialiniai poreikiai, pasireiškiantys menkesniu fiziniu, socialiniu ir psichologiniu saugumu... Case Managements spridning i kommunala organisationer : En organisatorisk förändring inom socialt arbete / The diffusion of Case Management in Swedish municipalities : An organizational change of Social Work

The aim of this study was to comprehend and explain the idea and function of Case Management and its diffusion in Swedish municipalities. More specifically its aim was to investigate factors for the diffusion of Case Management and its differences to similar professional roles. The study was based on interviews with four managers at various levels and two case managers representing two municipalities in Sweden. Five themes were identified that were especially interesting when discussing the function of Case Management, its differences to similar professional roles and its diffusion in Swedish municipalities. The themes were: shortcomings in the welfare system, the functions of Case Management, differences to similar professional roles, descriptions of Case Management and factors for the diffusion. The analysis was based on organizational institutionalism and translation, as well as previous research on Case Management and the diffusion of ideas. As a conclusion Case Management was described as a revolutionizing method that would import evidence based social work in the organization. That, and the obvious effort to adapt and integrate Case Management in the institutionalized metods and values of Swedish welfare organizations were factors for the diffusion. Case Management also turned out to be a similar method to the already established profession personligt ombud, wherefor Case Management can be seen as a relaunch of personligt ombud.

Mental health care roles and capacities of non-medical primary health and social care services: an organisational systems analysis

Top-down, centralised approaches to reform of mental health services implemented over the past 15 years in Australia have failed to achieve the widely shared aim of comprehensive, integrated systems of care. Investment to date has focused on the development and integration of specialist mental health services and primary medical care, and evaluation research suggests some progress. Substantial inadequacies remain however in the comprehensiveness and continuity of care received by people affected by mental health problems, particularly in relation to social and psychosocial interventions. Intersectoral collaboration that includes the diverse range of non-medical primary health and social care services is one of the most fundamental remaining challenges facing mental health system reform.

It also intends to show how changes in the practices of care to the homeless influence directly in their daily lives, specifically in the ways they organize themselves in the city. We highlight bancas - a term which defines, in Carlos and in other cities of São Paulo s State, groups of homeless people which are designed in certain places of the city. We have chosen the bancas as the locus of analysis here due to the fact that it is the most common way of living on the streets among the homeless in the researched city. Also, it gives a visibility to these groups in the urban space, obligating the creation of new forms of public management to this situation. As a mythological approach, we developed an ehtnograpy on Social Care Institutions and on the streets. Also, we performed in-depth interviews with care workers. The use of ethnography allowed us to develop analytical constructs such as Circulation and Settlement , which permitted us to conduct investigations about the changes in care practices in this population, giving the contemporary device of public management on the issue of the homeless. / O objetivo desta dissertação é apresentar como têm sido produzidos e gerenciados, em contexto contemporâneo, os moradores de rua. Pretende-se demonstrar que as mudanças ocorridas nas práticas de atendimento aos moradores de rua influenciam diretamente no seu cotidiano, especificamente nas maneiras como se organizam na cidade. Destacamos as bancas, termo que designa em são Carlos e outras cidades do interior paulista, agrupamentos de moradores de rua que se formam determinados lugares de cidade. A escolha das bancas como lugar lócus de análise deve a maneira mais comum de viver nas ruas entre os moradores de rua no município pesquisado, além da visibilidade que tais agrupamentos provocam no espaço urbano, ensejando formas de gerenciamento público do problema que representam. Para a consecução da pesquisa destacamos o método etnográfico, realizado em instituição de Assistência Social (Centro POP) e nas ruas, entrevistas com técnicos e trajetória profissional do gestor-pesquisador. O recurso à etnografia possibilitou a elaboração de constructos analíticos Circulação e Fixação, que permitem conduzir as investigações sobre as transformações nas práticas de atendimento dessa população, originando o dispositivo contemporâneo de gestão pública da questão dos moradores de rua.
