La rationalisation en douceur : sociologie des indicateurs qualité à l’hôpital / A soft rationalization : a sociology of quality indicators in French hospitals

Depuis les années 1990, le secteur hospitalier français est affecté par des réformes néo-managériales visant à renforcer le contrôle étatique sur les organisations de soin et sur les activités médicales. La thèse interroge ces transformations à la lumière du déploiement d'indicateurs qualité dont l'objet est de quantifier, mettre en transparence et améliorer la qualité des prises en charge. De quelle manière ces dispositifs d'évaluation affectent-ils la régulation institutionnelle de l'hôpital, l'organisation des établissements et l'autonomie des professionnels ? La thèse saisit l'action publique dans son caractère organisé, instrumental et cognitif, à partir d’une méthodologie qui croise entretiens auprès des responsables institutionnels, analyse de sources écrites et enquêtes dans quatre établissements hospitaliers. Cette approche permet de retracer le processus de construction des instruments, d'analyser leurs propriétés techniques et cognitives et de caractériser leurs usages sociaux. Ces indicateurs ont émergé dans un contexte institutionnel marqué par les infections nosocomiales et les palmarès hospitaliers. Construits à partir de savoirs hybrides, travaillés et légitimés par de multiples acteurs intermédiaires entre l’État et les professionnels, ils ont été généralisés prudemment, mais massivement, au cours des années 2000. En s'institutionnalisant, ils instillent formalisation, contrôle, traçabilité et auditabilité à l'hôpital. En déplaçant le regard des effets visibles et contraignants des instruments de tarification vers ces mécanismes plus discrets, la thèse montre comment l'évaluation de la qualité rationalise en douceur les bureaucraties professionnelles. / Since the late 1990s, the French hospital sector faces neomanagerial reforms aiming at strengthening state control over care organizations and medical activities. The thesis examines these contemporary transformations through the deployment of quality indicators measuring the quality of medical care, in order to enhance transparency and improve quality. How do these evaluation devices affect the institutional regulation of healthcare, the organization of hospitals and the professional autonomy of medical practitioners? The thesis describes the organized, instrumental and cognitive dimensions of public action, through a qualitative methodology crossing interviews with institutional leaders, analysis of written sources and investigations in four hospitals. It traces the career of these instruments, analyzes their technical and cognitive properties and characterizes their diverse social uses. These indicators have emerged in an institutional context deeply affected by nosocomial infections and hospital rankings. They are made of hybrid knowledge. Multiple intermediaries between state and professional actors shaped and legitimized them. They have been carefully but massively disseminated in the 2000s. Through their institutionalization, they instill formalization, control, traceability and auditability in hospitals. By moving the focus from the visible and constraining effects of payment instruments to these more discrete mechanisms, the thesis shows how quality evaluation softly rationalizes professional bureaucracies.

Na presente tese de doutorado, apresentamos nossas investigações acerca do Mestrado Nacional Profissional em Ensino de Física (MNPEF) enquanto dispositivo de política pública de formação continuada de professores. O desenvolvimento de nossa pesquisa foi fundamentado na articulação dos quadros conceituais da Sociologia da Ação Pública e da Filosofia da Linguagem do Círculo de Bakhtin. Nessa perspectiva, nosso esquema geral de investigação consistiu na análise bakhtiniana de uma complexa cadeia de enunciados oriundos dos modos de regulação que delimitamos no interior do MNPEF, notadamente, a regulação de controle e a regulação autônoma. Assim, concentrados nos primeiros 21 Polos Regionais (PR) do MNPEF criados em 2013, nosso objetivo foi estudar os modos de regulação desse programa, descrevendo e analisando a existência dos modos de regulação de controle - estruturas concebidas para coordenar e orientar as ações dos atores sociais - e dos modos de regulação autônoma, relativos à forma como esses atores convivem e (re)ajustam essas orientações. O corpus de nossa pesquisa foi construído, por um lado, pautado na regulação de controle e uma cadeia de documentos referentes tanto ao contexto específico do MNPEF, quanto o contexto amplo da pós-graduação nacional, e, por outro lado, no âmbito da regulação autônoma, por trabalhos de conclusão oriundos das primeiras turmas e por entrevistas realizadas com professores que atuam em distintos PR. Nossas investigações apontam o MNPEF como um dispositivo de política pública multirregulado, balizado sobretudo pelas regulamentações da Coordenação de Aperfeiçoamento de Pessoal de Nível Superior (CAPES) para os cursos de Mestrado Profissional em Ensino (MPE) e pelos órgãos administrativos nomeados pela Sociedade Brasileira de Física (SBF), entidade coordenadora do programa. Ao olharmos para o MNPEF como uma ação pública, identificamos a coexistência de modos de regulação de controle e de regulação autônoma. Contudo, não identificamos nenhum elemento que potencialmente ilustre ações conjuntas no âmbito nacional do MNPEF, mas apenas alguns indicativos de ações conjuntas que influenciam exclusivamente o contexto local dos PR, caracterizando, assim, processos de microrregulação local. / In this work, we show investigations about the Mestrado Nacional Profissional em Ensino de Física (MNPEF) as a public policy for formation of Physics teachers. Le foncier, entendu comme des relations entre les humains à propos de la terre, est au cœur de la répartition des pouvoirs, particulièrement dans les sociétés à dominantes agraires. Encadrer sa gestion à travers la formulation d'une politique publique signifie altérer les rapports entre l'État et les pouvoirs locaux, et la manière dont l’État entend construire un pouvoir de contrôle sur les hommes et le territoire. À travers l'exemple de l'Ouganda sous le régime du National Resistance Movement (NRM), cette thèse en science politique analyse la manière dont une politique publique foncière se fabrique en interaction avec les structures de pouvoir. Nous touchons ici aux rapports entre polity, politics et policy. Appréhendée comme une activité politique, la fabrication d’une politique publique foncière en contextes africains n’est pas qu’une affaire d’État. Elle est une source de légitimité politique pour les acteurs intégrés au processus. Son instrumentalisation peut profiter à un changement de rapports de force. Ce processus commence à partir de la fabrication des énoncés de problème et aboutit à la négociation d'une solution. Cette recherche inductive se fonde sur des méthodes de recherche qualitative : observations participantes, entretiens semi-structurés, recours à la littérature grise et aux archives. Elle est le fruit d'un travail de terrain de quatre ans en Ouganda. Cette thèse innove d'un point de vue théorique en intégrant l'approche discursive et pragmatique de l'action publique à l'approche structurationniste. Elle lie cette approche à la notion « d'historicité de l’État importé ». Ce cadre théorique permet d'étudier les mouvements d'ordre et de désordre de la société qu'engendre la formulation des énoncés de problème et de solution. Elle fournit aussi un apport empirique détaillé à l'étude de la politique foncière en Ouganda. Cette thèse contribue ainsi à l'étude de la démocratisation en Afrique, de celle du foncier en Afrique, et de celle des politiques publiques en contextes africains. / Land tenure, defined as the set of relations among humans that determine their interaction with land, lies at the heart of power struggles, especially in agrarian societies. Governing land management through public policy means changing power relations between the state and the local institutions that exercise social control. Using Uganda under the regime of the National Resistance Movement (NRM) as its case study, this thesis in political science explores the manner a public policy concerning land is constructed through the interactions among various power structures. We are dealing here with the interactions among polity, politics and policy. Examined as a political activity, the construction of a public policy concerning land in African contexts goes beyond a matter of concern for the state alone. This activity is a source of political legitimacy for those actors participating in the process. Its instrumentalisation can lead to changes in power relations. This process starts with the construction of the problem and ends with the negotiation of a solution. This inductive research is based on qualitative research methods : participant observation, semi-structured interviews, analysis of grey literature and of archives. It is based on four years of field work in Uganda. This thesis innovates theoretically as it integrates the discursive and pragmatic approach of public policy to structuration theory. It ties this approach to the notion of « historicity of the imported state ». This theoretical framework allows us to study the ordering and disordering of society that are triggered by the formulation of problems and solutions. It provides a detailed empirical study of public policy concerning land in Uganda. This thesis contributes, more generally, to the study of democratisation in Africa, land tenure in Africa and public policy in African contexts.

L’invention d’une identité régionale : la Bretagne et le livre (1945-2014) / Developing a Regional Identity : Brittany and the book (1945-2014)

À partir d’une approche socio-historique, la thèse propose une analyse des mécanismes de fabrication d’une politique du livre en Bretagne. En restituant les logiques sociales à l’œuvre dans le processus de naturalisation d’une politique publique de la culture, de la période de la Libération à la période actuelle, l’étude entend retracer les configurations successives de négociations et de luttes – entre les agents de l’Etat et les agents sociaux qui composent l’espace régional (notamment au sein des champs éditorial et politique) – pour le monopole de la définition d’une « identité régionale ». Dans cette perspective, « le livre » constitue un instrument légitime de production et de promotion de perceptions antagonistes de « la culture ». De sorte que le retour sur les conditions sociales et historiques d’émergence d’une catégorie d’intervention publique révèle les usages différenciés du « livre » et les enjeux politiques de « la culture », en les rapportant aux trajectoires individuelles et collectives des agents investis dans l’espace des mouvements sociaux bretons, en fonction d’une conjoncture nationale de prise de conscience régionale et de remise en cause de l’ordre symbolique établi. Il conduit par ailleurs à repérer le travail de représentation politique des institutions régionales consistant à homogénéiser une politique culturelle et le sens public qui en découle. De l’incorporation d’une indignité culturelle au renversement de l’ordonnancement légitime du monde social, l’histoire de la politique du livre en Bretagne met ainsi au jour les rapports de force engagés, avec et contre l’Etat, pour l’institutionnalisation de la région. / From a sociohistorical perspective, this thesis analyses the different developments that lead to the creation of a political identity through the use of the book in Brittany (France). By restoring the social logistics at work in the process of institutionalisation of a cultural policy, from the Liberation (1945) up to the present day, this study aims to retrace the path of the successive struggles – between State bodies and local bodies (particularly editors and politics) – in monopolizing the definition of a “regional identity”. In this regard, “the book” constitutes a legitimate instrument in the production and promotion of opposing views of “the culture”. Consequently, looking back on the social and historical conditions of the emergence of a public intervention highlights the different ways the book is used and also the political stakes of “the culture”. That must be put in parallel with both individual and collective paths of the bodies invested in the Breton social movements, which are taking place in a nationwide growth of a regional awareness and challenge of the established symbolic order. It is also necessary also to notice the work of the regional institutions in their political representation in order to homogenize a cultural policy and the public meaning that follows. From the acceptance of a cultural unworthiness to the overhauling of the legitimate order of the social field, the history of the book policy in Brittany highlights the power struggles engaged with and against the State for the institutionalisation of the region.

Les récits contrastés de la « migration environnementale » : élaboration, usages et effets sur l'action publique / Contrasting “environmental migration” narratives : a transnational comparison of their development, uses and effects in public action

Ce travail de thèse cherche à interroger l’importance, fluctuante, prise par la question des « migrations environnementales », et sa constante re-problématisation. Il s’inscrit dans une perspective de sociologie de l’action publique et entreprend une comparaison transcalaire, entre le plan international et l’échelon national malien. Cette recherche montre que, dans un contexte d’incertitudes politiques et scientifiques multiples, l’élaboration de récits d’action publique relatifs aux « migrations environnementales » permet de dramatiser des enjeux qui dépassent le strict cadre des effets des changements environnementaux sur les déplacements de population, et de les construire en problèmes relevant de l'action publique.Trois récits discordants ont été élaborés dans les arènes internationales : le récit des « réfugiés », le récit des « personnes déplacées par les catastrophes naturelles » et le récit de « la migration comme adaptation ». Leur multiplication s’explique, d’une part par leur visée instrumentale, et d’autre part par les constants efforts de reformulation mis en œuvre par des entrepreneurs politiques afin de les inscrire dans les référentiels dominants. En résultent des récits en décalage avec les résultats des travaux académiques consacrés au lien entre environnement et déplacement. A l’échelon malien, le récit des « réfugiés » s’insère dans un discours plus général venant justifier des mesures de lutte contre les changements environnementaux et climatiques, nouvelle priorité des agences d’aide. Les récits élaborés au plan international sont ainsi sélectionnés et articulés avec des enjeux locaux afin d’être adaptés aux logiques et normes de fonctionnement de l’action publique locale. / This research work aims at questioning the variety of narratives used to describe “environmental migration” and the shifting importance given to the matter. Building on a political sociology of ‘public action’ perspective, it undertakes a transnational comparison between international arenas and Malian politics and policies. This study shows that, in a context of multiple political and scientific uncertainties, “environmental migration” narratives are used to add dramatic tension to issues that go beyond the subject of environmentally linked migration, and to construct them as public problems.Three conflicting narratives have been elaborated in the international arenas: the “refugee” narrative, the “natural disaster induced displacement” narrative and the “migration as adaption” narrative. Their multiplication can be explained by their instrumental purpose and by the constant reformulation efforts undertaken by political entrepreneurs in order to include them in the prevailing cognitive and normative frames. As a result, these narratives appear to be out of step with academic findings that insist on the complex causal relationship linking environment to displacement. In Mali, the “refugee” narrative is the one predominantly used by ‘public action’ actors but it is part of a larger discourse focused on climate change, which is used to legitimise development aid flows. The narratives elaborated at the international level are thus selected and adapted to local cognitive and normative frames and agenda setting dynamics.

Action publique sans problème public reconnu : la lutte contre les discriminations ethno-raciales en France dans les années 2000

Cette thèse porte sur la lutte contre les discriminations ethno-raciales en France de 1998 à fin 2011. L'analyse s'inscrit dans une approche constructiviste et culturelle des problèmes publics. Elle s'appuie sur des documents, des enquêtes et des observations à différentes échelles d'intervention : européenne, nationale et locale. La prise en charge politique et institutionnelle des discriminations ethno-raciales en France est d'abord étudiée. Impulsée par l'agenda européen, celle-ci se caractérise par la faiblesse du portage politique, une euphémisation de la question raciale et en parallèle une territorialisation de l'action publique. La lutte contre les discriminations, menée sous le sceau de l'expérimentation, essentiellement dans le domaine de l'emploi, se traduit par un outillage conséquent et un évitement du débat politique. Dans un second temps, la thèse analyse la carrière du problème des discriminations dans une commune. Les stratégies pour construire une « culture de la non-discrimination » s'avèrent fragiles / This thesis focuses on the fight against ethno-racial discrimination in France between 1998 and 2011. The analysis uses a constructivist and cultural approach to public problems. It is based on documents, enquiries and observations from various sources of European, national and local level. We first study the political and institutional approaches to the problem of ethno-racial discrimination in France. Driven by the European agenda these approaches are characterized by a lack of political support, a playing-down and understatement of racial issues, along with a territorialisation of public action. The fight against discrimination, led on a basis of experimentation essentially in the field of employment, results in an abundance of tools and an avoidance of public debate. The thesis goes on to monitor the “career” of the discrimination problem as experienced in one municipality. It highlights the strong disownment of the problem and demonstrates that local public action is above all a struggle for the recognition of the problem of ethno-racial discrimination. The analysis then focuses on the strategies put in place to legitimize the existence of such acts of discrimination: a witnessing policy to help minorities formulate their discriminatory experiences, and a legalistic strategy aimed at piercing political discourse and encouraging the sharing of responsibilities. But the right to non-discrimination, faced with strong competition, is unable to found a « symbolic order » recognized by all. In the end, the weaknesses of strategies aimed at building a « culture of non-discrimination » are brought to light.

Cette thèse analyse les relations de pouvoir qui se jouent autour des enjeux environnementaux (gestion de l'eau et changement climatique) dans le processus de construction étatique de l'Autorité palestinienne, entité gouvernementale sous régime d'aide et marquée par l'occupation israélienne. Elle mobilise la sociologie de l'action publique, la political ecology et les science and technology studies afin d'étudier les processus multi-échelles de co-construction de l'ordre social et de l'environnement à travers les savoirs, la définition des problèmes et les politiques adoptées pour y répondre. Cette recherche analyse les liens entre l'aide internationale, le développement, l'environnement et la volonté de consolidation du pouvoir de l'Autorité palestinienne. Elle se penche sur la circulation et l'hybridation des savoirs et des solutions d'action publique. En dehors de l'action des décideurs nationaux et internationaux, d'autres acteurs (ONG, organisations humanitaires) interviennent et mobilisent d'autres représentations des problèmes environnementaux et des solutions à apporter en interactions avec leurs représentations du territoire et du conflit. Cette thèse se fonde sur une série d'entretiens et de discussions informelles, la littérature grise sur le sujet et de nombreuses observations participantes. / This thesis analyses the power relations involved in environmental issues (water management and climate change) and the process of state building of the Palestinian Authority, an entity dependent on international aid and under israeli occupation. This thesis mobilises sociology of public action, political ecology and science and technology studies in order to examine the multi-level processes of co-construction of social order and environment through knowledges, problems definition and public policies adopted to tackle them. This research analyses the interactions between international aid, development and environment and the objective of empowerment of the Palestinian Authority. It focuses on the circulation and hybridisation of knowledge and public policy solutions. Beyond national and international decision makers, other actors such as NGOs or humanitarian organisations participate and mobilise other representations of environmental problems and solutions in relation with their representations of the territory and the conflict. This thesis is based on a series of interviews, informal discussions, grey literature dealing with the subject and observational work.

L'industrie automobile à l'épreuve des voitures électriques : entre changement et continuité / The automobile industry struggling the electric cars

Cette thèse a pour objectif de participer à l’étude du changement des industries contemporaines et d’appliquer des outils de la science politique à l’analyse de l’économie. À travers l’étude du cas de la voiture électrique, nous nous interrogeons sur les causes des évolutions de l’industrie automobile. Notre hypothèse est que la construction du marché des voitures électriques entre 2008 et 2013 n’a pas produit la « Révolution » annoncée par les acteurs sociaux. Au contraire, elle a permis la reproduction des structures et des hiérarchies industrielles existantes. Pour le démontrer, ce travail tente de comprendre non seulement comment la question du développement des voitures électriques a été érigée au rang de problème public, mais aussi comment ce « problème » s’est institutionnalisé comme un cadre consensuel d’action des constructeurs automobiles. Par ce biais, nous mettons en lumière l’importance des croyances et des représentations collectives sur l’activité économique, dans l’optique de déconstruire le mythe de la « Seconde Révolution automobile ». En outre, notre travail montre que la science politique, et plus précisément l’analyse des politiques publiques et la sociologie de l’action publique, peut apporter des outils analytiques heuristiques et des réponses concrètes au fonctionnement des industries et du capitalisme contemporain. Par cette voie, cette thèse entend apporter une contribution aux questions que se posent sociologues, économistes et politistes sur le changement économique et les processus qui en sont à l’origine. / This thesis aims to generate knowledge upon change within contemporary industries and apply tools from political science to the analysis of the economy. Through the case study of the electric car market, the focus is on the causes of the transformation of the automobile industry. The central claim made is that the social construction of the electric car market between 2008 and 2013, did not produce the “Revolution” announced by the actors involved in this industry. On the contrary, it has facilitated the reproduction of existing industrial structures and hierarchies. To demonstrate this claim, the thesis has sought to understand how the issue of the development of electric cars has been elevated to the status of public problem by several States around the world. From there, we then sought to discover how this “problem” has become a consensual strategic framework for automakers and a key issue within the automobile industry. Throughout this research, we highlight the impact of beliefs and collective representations upon economic activity, and deconstructed the myth of the “Second Automobile Revolution”. Furthermore, our work shows that political science, specifically the sociology of public action, can supply heuristic analytical tools and practical answers to the understanding of contemporary industries and capitalism. This thesis is thus a contribution to questions asked by sociologists, economists and political scientists about economic change
