Assessment of respiratory risks associated with exposure to particulate matter (PM2.5) in informal waste pickers in a landfill site in KwaZulu-Natal

Submitted in fulfillment of the requirements for the Degree of Master of Technology Degree : Environmental Health, Durban University of Technology, Durban, South Africa, 2015. / Introduction Informal waste picking has both economic and environmental benefits, however there are many health risks associated with this activity. This cross sectional descriptive study assessed the respiratory health risks and dust (PM2.5) exposure among informal waste pickers operating at the New England landfill site in the Msunduzi Municipality. Data was collected from 102 informal waste pickers using a validated questionnaire adapted from the British Medical Research Council and American Thoracic Society. Our study population comprised of 66.4°/o women with a mean age of 36.7 years. Women reported greater use of safety shoes and gloves compared to men. Very few participants used dust masks or respirators. Results showed a high prevalence of chronic cough (57o/o), wheeze (51°/o) shortness of breath (31.6°/o) and cough with phlegm (29.0°/o) among all participants. Among those who reported having ever smoked, there was a significantly higher prevalence of cough with phlegm (45.2°/o p=0.02) and wheeze (73.1°/o, p= 0.05). Similarly, there was a bivariate association between current smoking and wheeze (p=0.02) and between ever smoked and wheeze {p< 0.05). A random sample of 28 waste pickers was chosen from the 102 participants for personal sampling. The levels and distribution of time weighted average to PM2.5 were calculated during an 8 hour working period. Average personal sampling levels of PM2.5 were very high compared to environmental levels. Personal sampling results showed a maximum exposure of 431 J.,Jg/m 3 and a mean of 187.4 J.,Jg/m 3 compared to a maximum level of 27.9 J.,Jg/m 3 and a mean of 16.5 J.,Jg/m 3 for environmental sampling. Mean personal sampling results significantly exceeded recommended eight hour time weighted average for South African National Standard for Occupational Exposure Limit (Republic of South Africa OHS ACT 85 of 1993), World Health Organisation and United State Environmental Protection Agency guidelines for PM2.5, while the environmental mean was within guidelines. This study showed that waste pickers are exposed to high levels of PM2.5 while working at the landfill site. Limited or no use of personal protective equipment increases risk of dust exposure and adverse respiratory effects. Socio-cultural perceptions of nursing and its influence on the recruitment and retention of males student nurses in Nursing Education Institutions (NEIs), KwaZulu Natal Province

Nursing is a female dominated profession; making it difficult for men in the profession to excel in their caring capacities as nurses. This study aimed at identifying and describing male and female nurses' insights into and perceptions of socio-cultural influences on the recruitment and retention of men in the nursing profession and also explores their experiences in providing intimate care to patients of opposite gender. The study adopted a qualitative research methodology: 16 male and 11 female nursing students were purposively sampled and were interviewed using semi-structured questions. Themes of nursing seen as women's work; low status; stigma; caring and helping others were developed. Feelings of embarrassment and discomfort; fear and refusal of care were experienced when providing intimate care to patients of opposite gender; this has led male participants to develop strategies to protect themselves from sexual accusations. The Nursing Education Institutions (NEIs) have insufficient toilets for male nursing students; there is a lack of male role models and feminine pronouns are used when relating to professional nurses. Evidently socio-cultural perceptions of nursing enforce a negative image. The difficulties experienced by male and female nurses when providing intimate care and lack of male-friendliness in NEIs were discussed. Quantifying indigenous forest change in Dukuduku from 1960 to 2008 using GIS and remote sensing techniques to support sustainable forest management planning

Thesis (MSc)--Stellenbosch University, 2013. / ENGLISH ABSTRACT: This study aimed to understand how Dukuduku Forest in Kwa-Zulu Natal has changed from 1960 to 2008 and whether the change in political regimes, during and post apartheid eras might have contributed to changes in forest extent. To achieve the aims, the following analyses were made: - Qualitative and quantitative spatial analyses of forest change; - Analyses of the correspondence of change with political changes in the country; - Assessment of perception of people living in the Dukuduku forest area. The Dukuduku land cover was mapped from aerial photos using ArcGIS 9.3 to determine whether or not there has been a significant change in the area from 1960-2008, in response to resource use pressures and to come up with the strategic sustainable management plan from the results found. Five aerial photographs were used to determine the changes in land cover from the year: 1960, 1970, 1992, 2005 and 2008. The Land cover types were classified into four classes, Indigenous Forests, Plantation Forests, Water Bodies and Other (open areas, cultivated land, and all the human disturbed and transformed land). The percentage of cover per class was compared across the years to determine overall change in land cover and the rate of change per year was also calculated. The results from the study showed that: - Natural Forest increased by 11% (700 ha), at the rate of 20.56 hectares per year between 1960 and 1992, which is the apartheid era. Between 1992 and 2008, the democratic era, the forest decreased by 34.4% (2472.31ha), at the rate of 168 hectares per year. - The Dukuduku forest community gains resources (timber and grass for construction, art, firewood, medicinal plants, grazing of livestock and food) from the forest. The people are willing to contribute in protecting the forest only if the governing authorities would include them in decisions made, as the NFA demands Participatory Forest Management, but which does not currently exist in Dukuduku. / AFRIKAANSE OPSOMMING: Hierdie studie ondersoek die verandering van die Dukuduku woud in Kwa-Zulu Natal vanaf 1960 tot 2008, en vernaamlik of die verandering in politieke regimes tydens en in die postapartheid eras tot verandering bygedra het in die woud se vorm. Om hierdie doelwitte te breik is die volgende analises gedoen: - Kwalitatiewe en kwantitatiewe ruimtelike analises van woudverandering; - Analises van die korrelasie tussen hierdie fisiese omgewingsverandering en politieke verandering in die land; - Analise van die persepsie van mense wat in die Dukuduku woudgebied woon. Die Dukuduku gronddekking is gekarteer met behulp van lugfotos, waarvoor ArcGIS 9.3 gebruik is om te bepaal of daar noemenswaardige verandering in die gebied plaasgevind het van 1960 tot 2008, in reaksie op hulpbrongebruike, en om ‘n volhoubare bestuursplan gestel voor wat op die bevindinge gebaseer is. Towards the development of a mycoinsecticide to control white grubs (Coleoptera: Scarabaeidae) in South African sugarcane

In the KwaZulu-Natal (KZN) Midlands North region of South Africa, the importance and increased prevalence of endemic scarabaeids, particularly Hypopholis sommeri Burmeister and Schizonycha affinis Boheman (Coleoptera: Melolonthinae), as soil pests of sugarcane, and a need for their control was established. The development of a mycoinsecticide offers an environmentally friendly alternative to chemical insecticides. The identification of a diversity of white grub species, in two Scarabaeidae subfamilies, representing seven genera were collected in sugarcane as a pest complex. Hypopholis sommeri and S. affinis were the most prevalent species. The increased seasonal abundances, diversity and highly aggregated nature of these scarabaeid species in summer months, suggested that targeting and control strategies for these pests should be considered in this season. Increased rainfall, relative humidity and soil temperatures were linked to the increased occurrence of scarab adults and neonate grubs. Beauveria brongniartii (Saccardo) Petch epizootics were recorded at two sites in the KZN Midlands North on H. sommeri. Seventeen different fluorescently-labelled microsatellite PCR primers were used to target 78 isolates of Beauveria sp. DNA. Microsatellite data resolved two distinct clusters of Beauveria isolates which represented the Beauveria bassiana senso stricto (Balsamo) Vuillemin and B. brongniartii species groups. These groupings were supported by two gene regions, the nuclear ribosomal Internal Transcribed Spacer (ITS) and the nuclear B locus (Bloc) gene of which 23 exemplar Beauveria isolates were represented and sequenced. When microsatellite data were analysed, 26 haplotypes among 58 isolates of B. brongniartii were distinguished. Relatively low levels of genetic diversity were detected in B. brongniartii and isolates were shown to be closely related. There was no genetic differentiation between the two sites, Harden Heights and Canema in the KZN Midlands North. High gene flow from swarming H. sommeri beetles is the proposed mechanism for this lack of genetic differentiation between populations. Microsatellite analyses also showed that B. brongniartii conidia were being cycled from arboreal to subterranean habitats in the environment by H. sommeri beetles. This was the first record of this species of fungus causing epizootics on the larvae and adults of H. sommeri in South Africa. The virulence of 21 isolates of Beauveria brongniartii and two isolates of B. bassiana were evaluated against the adults and larvae of S. affinis and the adults of H. sommeri and Tenebrio molitor Linnaeus (Coleoptera: Tenebrionidae). Despite being closely-related, B. brongniartii isolates varied significantly in their virulence towards different hosts and highlighted the host specific nature of B. brongniartii towards S. affinis when compared to B. bassiana. Adults of S. affinis were significantly more susceptible to B. brongniartii isolates than the second (L2) or third instar (L3) grubs. The median lethal time (LT₅₀) of the most virulent B. brongniartii isolate (C13) against S. affinis adults was 7.8 days and probit analysis estimated a median lethal concentration (LC₅₀) of 4.4×10⁷ conidia/ml⁻¹. When L2 grubs were treated with a concentration of 1.0×10⁸ conidia/ml⁻¹, B. brongniartii isolates HHWG1, HHB39A and C17 caused mortality in L2 grubs within 18.4-19.8 days (LT₅₀). Beauveria brongniartii isolate HHWG1 was tested against the L3 grubs of S. affinis at four different concentrations. At the lowest concentration (1×10⁶ conidia/ml⁻¹), the LT₅₀ was 25.8 days, and at the highest concentration (1×10⁹ conidia/ml⁻¹) the LT₅₀ dropped to 15.1 days. The persistence of B. bassiana isolate 4222 formulated on rice and wheat bran and buried at eight field sites in the KZN Midlands North was evaluated by plating out a suspension of treated soil onto a selective medium. All eight field sites showed a significant decline in B. bassiana CFUs per gram of soil over time, with few conidia still present in the samples after a year. Greater declines in CFUs were observed at some sites but there were no significant differences observed in the persistence of conidia formulated on rice or wheat bran as carriers. Overall, poor persistence of B. bassiana isolate 4222 was attributed to suboptimum temperatures, rainfall, which rapidly degraded the nutritive carriers, attenuated fungal genotype and the action of antagonistic soil microbes. Growers’ perceptions of white grubs as pests and the feasibility of a mycoinsecticide market were evaluated by means of a semi-structured questionnaire. The study showed that the reduced feasibility of application, general lack of potential demand for a product, high cost factors and most importantly, the lack of pest perception, were factors which would negatively affect the adoption of a granular mycoinsecticide. Growers however exhibited a positive attitude towards mycoinsecticides, and showed all the relevant attributes for successful technology adoption. It is recommended that because B. brongniartii epizootics were recorded on target pests which indicated good host specificity, dispersal ability and persistence of the fungus in the intended environment of application; that a mycoinsecticide based on this fungal species be developed. What will likely increase adoption and success of a mycoinsecticide is collaboration between various industries partners to increases market potential in other crops such as Acacia mearnsii De Wild (Fabales: Fabaceae).

The impact of negative influences facing children with physical disabilities in rural areas

The purpose of this study was to investigate the impact of negative influences facing children with physical disabilities living in rural areas. Children with physical disabilities living in rural areas of South Africa have to deal with many negative influences in addition to their disability. The main theme is that all children in rural areas face negative influences, but these are far worse for the children with physical disabilities. Societal and educational exclusion and poverty continue to place these children at a disadvantage despite South Africa’s policies that promote inclusion and equality. These rights are central to a meaningful democracy yet; research indicates that children with physical disabilities remain in a hopeless situation in the rural areas. To escape their plight inclusive education is pivotal. It is argued that the government’s capacity to deal with all the negative influences is limited and this therefore necessitates community involvement. Bronfenbrenner’s Bio-ecological approach was used both to investigate the impact of the negative influences facing children with physical disabilities as well as providing a framework that tackles the barriers that prevent this marginalised group of children having an equal opportunity to education. A qualitative and ethnographic study was undertaken to investigate these issues. This entailed the researcher living in four rural areas in KwaZulu-Natal, and collecting data through community participation, observations, questionnaires and interviews as well as photographs and drawings collected from the children. Data was analysed and poverty and exclusion clearly impacted not only on the children with physical disabilities, but their parents/caregivers. It became clear to the researcher that unless this group of children received early intervention to prepare them for education they would not be able to be included in formal education from Grade 1 but would need to begin their schooling in a specialised and exclusive environment. A framework for intervention based on Bronfenbrenner’s model was formulated that involved community involvement on the micro and mesosystemic levels. This framework is unique in that it will provide intensive early intervention for children with physical disabilities with the explicit aim of preparing them for inclusion and at the same time giving their parents/caregivers an opportunity to escape their plight of poverty.Every child in South Africa according to our Constitution and other policies is guaranteed equality. This equality needs to be provided to children with physical disabilities before they attend formal schooling and end up floundering never having had the opportunity to lift themselves out of the cycle of poverty that they face. Contemporary models for understanding these negative impacts and exclusion suggest that an important outcome of rehabilitation services is to optimise children with physical disabilities participation in the home, school and community life. Bronfenbrenner’s Model is based on the premise that disability involves an interaction between features of the child and features of the environment that can be adapted to promote educational inclusion and thus reduce the cycle of poverty. / Psychology of Education / D. Commuting to school in semi-rural KwaZulu-Natal: characteristics, causes and consequences

The Umnini Tribal Authority is a poor, semi-rural area in KwaZulu-Natal, governed by a local chief. This study set out to determine school commuting patterns in relation to school choice and socio-economic status (SES) in the area. Mixed methods were used, involving a parental questionnaire survey, interviews, focus groups and a field audit with photographs. Overall, almost all learners enrolled in the seven 'no-fee' state primary (four) and high schools (three) under study were found to be Black African, IsiZulu speakers, and generally ‘extremely poor’ to ‘very poor’. Most households are headed by single mothers who have completed high school but are unemployed. Most households rely on government social grants to some extent. Most fathers were either absent, poorly educated to uneducated, and far less likely to be employed than the mothers. Children living with both parents, especially where the father was tertiary educated, and the mother employed, fell into the ‘less poor’ and ‘better off than most’ categories. In terms of school choice affordability, good teachers, good school management and proximity to home were the main drivers of enrolment. SES seems to have an impact on the amount of money spent on school lunches and school expenses, although there were some concerning exceptions. Primary school learners walked to school, but many parents pay for monthly transport. Some use the government-subsidised bus. High school learners tended to have longer and less safe journeys to school and back than primary school learners. This is especially true for boy learners, who are targets for criminals, drug addicts and, even worse, school bus drivers. Several challenges face all these learners en route to school every day: crime, unsafe and poor road conditions, roadworthy vehicles, long journeys and rough terrain. Primary school learners often get lost or left behind by their transport drivers. Fortunately, their teachers are actively involved in ensuring their safety and well-being – far more so than their parents, who seldom even know the name of the transport driver, let alone whether the vehicle is appropriate, roadworthy or licenced. High school learners were more likely to live in ‘extremely poor’ to ‘very poor’ households, a possible indicator that financially better-off parents are sending their high school children to schools outside of the area. It is recommended that the local tribal authority, the provincial government, the schools, parents and drivers need to work in unison to make the school commute simpler and safer. / Environmental Sciences / M. The system of care for persons suffering from mental retardation is in a state of upheaval. Considering the mentally handicapped as holistic beings, this study explored and assessed the needs of those residing in the community of District 22 (sub-district 222), KwaZulu-Natal, thus facilitating the planning of care and care facilities for these individuals to enable them to function optimally in the community. To accomplish this purpose, specific objectives were formulated. A quantitative, exploratory and descriptive study based on Maslow's hierarchy of needs theory was carried out, using 167 respondents. The study revealed that despite being mentally retarded, they were still regarded as valuable members of the community, their needs did not differ from the needs of others in the rest of the world and that mental retardation is still rated low in the prioritization of health problems, hence the lack of resources and support needed for the rehabilitation of such persons. / Health Studies / M.A. (Health Studies)

The impact of the principal's instructional leadership on the culture of teaching and learning in the school

Recently it has been noticed that there is a marked decline in the culture of teaching and learning (COLT) in sortie of the schools in the KwaZulu Natal province. An investigation was undertaken to check whether the instructional leadership of school principals was a contributory factor to this decline. A literature study and an empirical investigation was done to identify if any relationship existed between the instructional leadership ofthe principal and COLT in schools. From the findings it became apparent that such a relationship did exist. Schools where principals neglected such instructional leadership roles were more susceptible to perpetuate the erosion of the culture of teaching and learning (COLT), whereas principals who do fulfil their instructional tasks tend to alleviate the erosion of COLT.
