Perioperativ omvårdnad som skapar trygghet och lindrar oro: En intervjustudie med patienter som har genomgått sköldkörtelkirurgi

Bakgrund: Under den korta perioden som den perioperativa vården varar ska sjuksköterskor skapa en tillitsfull relation, uppmärksamma behov, lindra oro och förmedla känsla av trygghet. Personcentrerad vård har visat sig vara ett effektivt verktyg för detta. Genom att studera upplevelser av vården hos patienter som har genomgått en sköldkörteloperation kan vården anpassas för att möta patienternas individuella behov. Syfte: Studiens syfte var att utforska och beskriva hur patienter som genomgått en sköldskörteloperation upplever den perioperativa omvårdnaden. Metod: En kvalitativ studie. Semi-strukturerade individuella intervjuer genomfördes med åtta patienter som nyligen genomgått sköldkörtelkirurgi. Datan analyserades genom systematisk textkondensering. Resultat: Studiens resultat visar på att känsla av oro och trygghet genomsyrar upplevelsen av den perioperativa processen hos patienter som har genomgått sköldkörtelkirurgi. Omvårdnaden som patienterna erhåller bidrar till att skapa trygghet och lindra oro. Ett gott bemötande av hjälpsam, lugn och omhändertagande vårdpersonal som ser patienternas behov bidrar till att skapa känslan av trygghet. Patienternas oro kan lindras genom tydlig och individanpassad information som ges under rätt omständigheter. En tillitsfull relation mellan vårdtagaren och vårdgivaren har en positiv inverkan på patienterna och är en viktig faktor för att lindra den perioperativa oron. Slutsats: Kunskap om omvårdnadsåtgärder som skapar trygghet och lindrar oro är betydelsefull och kan bidra till en mer personcentrerad vård. En förändring måste ske och omvårdnaden måste ges ett större utrymme, i första hand för patienternas skull. Strävan efter att minimera lidande och öka tryggheten i samband med den perioperativa vården ska vara en självklarhet. / Background: During the short period of time of the perioperative care, the nurses needs to create a trustful relationship, observe needs, ease anxiety and mediate feeling of safety. The person-centred care had proven to be a good tool to achieve that. By studying the experiences of care in patients who had undergone thyroid surgery, the care can be fitted to encounter the individual needs in patients. Aim: The aim of this study was to explore and describe how patients who had undergone thyroid surgery experience the perioperative care. Method: A qualitative study. Semi structured individual interviews were conducted with eight patients who recently had undergone thyroid surgery. Data were analysed using systematic text condensation. Results: Study results shows that the feeling of anxiety and safety permeates the experience of the perioperative process in patients who had undergone thyroid surgery. The nursing care from calm and caretaking personnel contributes to create safety and to ease suffering. Patients’ anxiety can be eased through clear and individually fitted information that is given under the right circumstances. Trustful relationship between the caretaker and the caregiver is an important factor to ease the perioperative anxiety. Conclusions: Knowledge about nursing actions that creates safety and eases anxiety is meaningful and can contribute to a more person-centred care. A change must take place and the nursing care must be given more space, primarily for the patient's sake. The striving to minimize suffering and increase the security associated with perioperative care should be a matter of course.

Erfarenheter av aktivitet och delaktighet hos personer med förvärvad hjärnskada : En kvalitativ innehållsanalys av en kollektivt skriven blogg / Experiences of people with acquired brain injury regarding activity and participation : A qualitative content analysis of a collectively written blog

Bakgrund: I ett globalt perspektiv är förvärvade hjärnskador en stor utmaning för samhället i form av kostnader för sjukvård och rehabilitering. Personer som drabbas av förvärvad hjärnskada behöver stöd för att själva ta ansvar för sin träning och hitta meningsfulla aktiviteter tillsammans med andra som en del av en hälsosam livsstil. Syfte: Syftet med studien är att beskriva erfarenheter av aktivitet och delaktighet hos personer som drabbats av förvärvad hjärnskada som skildras i en kollektivt skriven blogg. Metod: Materialet i den aktuella studien utgörs av bloggtexter som skrevs mellan åren 2017 och 2019 inom ramen för ett samarbetsprojekt och låg öppet publicerade på internet. Bloggen skrevs av en grupp personer med förvärvad hjärnskada. Texterna analyserades med Malteruds systematiska textkondensering. Resultat: Analysen resulterade i tre kategorier; Vikten av att hålla igång; som beskriver upplevelser och erfarenheter av fysisk aktivitet och träning efter hjärnskadan och hur aktiviteterna behövde anpassas efter de nya förutsättningarna. Att träna lite hela tiden i vardagen; som beskriver deltagarnas sätt att träna medvetet i alla aktiviteter som tillhör vardagslivet, att träna lite hela tiden. Hur man närmar sig samhället på nytt; som beskriver erfarenheter gällande delaktighet i samhällets olika delar och vikten av stöttning från människor i samma situation. Slutsats: Resultatet indikerar att fysisk aktivitet, träning och delaktighet i samhället kan vara viktigt efter en förvärvad hjärnskada. Vidare kan det vara betydelsefullt att dela erfarenheter med andra, få stöd av andra och lära sig av andras erfarenheter. Det är även viktigt att samhällsfunktioner och miljöer i det offentliga rummet är tillgängliga för alla. / Background: In a global perspective, acquired brain injury is a major challenge for society in the form of costs for healthcare and rehabilitation. People with acquired brain injury need support in order to take responsibility for their own health and incorporate exercise as part of a healthy lifestyle. Aim: The aim of the study was to describe the experiences of people with acquired brain injury, with regards to activity and participation, as depicted in a collectively written blog. Methods: The material in the current study consists of collectively written blog texts written between 2017 and 2019 by people with acquired brain injury. The material included 35 blog texts and was part of a larger research study. The texts were analyzed with Malterud's Systematic Text Condensation. Results: The analysis resulted in three categories; The importance of being physical active; describes experiences of physical activity and training after brain injury and how the activities needed to be adapted to the new conditions. To exercise a little all the time in everyday life; describes the participants' way of exercising consciously in all activities included in everyday life, to exercise a little all the time. How to re-approach society; describes experiences regarding participation in the various parts of society and the importance of support from people in the same situation. Conclusions: The results indicate that physical activity, training and participation in society are important after brain injury. Furthermore it is meaningful to share experiences with, get support from and learn from other people with acquired brain injury. It is also important that community functions and environments in public spaces are available for everyone.

Når virkeligheten strammer grepet : Overdoser hos beboere i lavterskel botiltak i Oslo: ansatte forteller. / When reality kicks in : employee stories about overdosing by residents of low threshold housing facilities in Oslo

Målet med studien er å undersøke og beskrive ansatte i lavterskel botiltak sine erfaringer med overdoser hos beboere. Gjennom å belyse den ofte tause kunnskapen de ansatte i lavterskeltiltak har om hva som karakteriserer en overdosesituasjon, er studiens hensikt å systematisere og presentere denne erfaringskunnskapen, og gjennom det kunne bidra til en kvalitetsforbedring av den tjenesten som ytes til beboerne.Datainnsamlingen ble gjort gjennom fokusgruppeintervjuer med ansatte på lavterskel botiltak. Flere av kodegruppene som ble arbeidet frem i analysen var tema i begge fokusgruppene. Temaene var overganger, tilgang, å legge lokk på det vanskelige, å dempe kriser og relasjonen til andre. Disse ble igjen rekontekstualisert til «min virkelighet» og «den rusfrie virkelighet,» sammenfattet som «når virkeligheten strammer grepet.»Hovedfunnet i studien er at ansatte opplever at det er når virkeligheten strammet grepet, når rusens dempende slør ikke kan holde virkelighetens krav på avstand, at risikoen for overdoser oppstår. Dette taler for viktigheten av gode relasjoner til hjelpeapparatet, og at de ansatte kjenner til hva som beveger seg i den enkelte beboers liv. / Objectives: This study aimed to investigate and describe how employees working in low threshold housing facilities experience overdosing by facility residents. We also aimed to enlighten the often silent employee knowledge about the characteristics of an overdose situation, thus enhancing the quality of resident services.Materials and Methods: Data was collected through focus group interviews with employees working in low-threshold housing facilities. Many of the code terms identified through data analysis were common to both employees and residents (e.g., “transitions,” “access,” “putting a lid on the difficulties,” “attenuating crises,” and “relation to others.” We recontextualized these codes to “my reality” and “drug-free reality,” or “when reality kicks in.”Results: Our study determined that employees recognize the risk of overdosing as the point at which reality kicks in (i.e., when the attenuating effect of the drugs can no longer keep reality at distance).Conclusion: This study shows that employees are fully aware of residents’ lives and activities. Therefore, policies that maintain good employee relationships with and within the different social and health services may contribute significantly to overdose prevention. / <p>ISBN 978-91-86739-07-2</p>

Medikaliseringen av den svenska omskärelsedebatten : en religionssociologisk studie om förskjutningen av ”den religiösa andre.”

The question regarding male circumcision in Sweden became a relevant topic within the public debates during the last three years. Due to the Swedish political party, Centerpartiet’s decision to strive for a ban on male circumcision, this debate surfaced after the 28th of September. Participation in the public debate came from different directions and perspectives on male circumcision. In this thesis I focus on different actors and their public opinions in debate articles published between the years of 2018-2019. The aim of this thesis is to examine how critical arguments are constructed from perceptions of a decreasing significance of religion within late modern societies. This results in a motivation to ban the performance of male circumcision without medical indication. By using the qualitative text analysis of systematic text condensation this thesis closer categorizes the arguments into the three themes of “children’s bodily integrity”, “the delimitation of medicalization” and “the religious other”. In addition to this, the thesis also looks into the argument from the analytical framework on argumentation analysis in order to distinguish “normative” and “factual” arguments in the debate. To look closer at the constructions of the critical arguments within the debate this thesis uses the theoretical framework of Sara Ahmed (2011). From a phenomenological inspired perspective this thesis further looks into the argument’s correlation and construction of a collective body. This in particular is the construction of the secular- in relation to the religious body. Constructions of the religious body allows for secular ideas to circulate around national symbols and principles, which results in an exclusion of religious motives and perspectives. The thesis further sheds light on the cultural imbedded “medical gaze”, from the theoretical perspective on medicalization of Karin Johannisson (2013). This thesis aims to examine the reproduction of the medical superiority within debates on questions regarding the human body, like the one on male circumcision. The thesis illustrates the complexity of religious visibility within the Swedish context. Although the critical arguments reject the religious motives of male circumcision, the debate increases the religious visibility in the public debates.
