The imbalance in the production of globin chains can result in ineffective erythropoiesis, insufficient production of hemoglobin, hemolysis and anemia of varying degree. Beta thalassemia is more common in countries bordering the Mediterranean Sea, reflecting the participation of these peoples in the formation of the Brazilian population. The predominant mutations in Brazil are the IVS-I-1, IVS-I-6, IVS-I-110 and CD 39, which are associated with different clinical conditions and are mostly regionally specific. Objective: To characterize the clinical, hematological and molecular adults with beta thalassemia of the University Hospital Cantídeo Walter and followed at a referral center for Hematology of the state of Ceará (Hemoce). Methods: We analyzed 22 individuals with beta thalassemia, 7 intermediate and 15 minor, in both sexes, from February 2008 to September 2009. Clinical data and laboratory tests: blood count, levels of Hb A2 and Hb F, serum iron, total capacity and latent iron binding (CTLFe, CLLFe), ferritin and transferrin saturation index (IST) were obtained from medical records, diagnosis. About 5 mL of venous blood was collected in tubes containing EDTA anticoagulant for molecular study. The analysis of mutations was performed using the technique of chain reaction mediated by allele specific polymerase (PCR-AE), where we analyzed the following mutations: IVS-I-1, IVS-I-6, IVS-I-110 and CD 39. Statistical analysis was carried out in software R (version 2.7.0) and the level of significance was 5%. Results: Of 22 patients studied, 15 were patients with β thalassemia minor and seven intermediate. The age ranged 18-68 years with a mean of 44.7 years. 18.2% male and 81.8% female. The mutations were characterized in 68.2% of cases, which had the most frequent IVS-I-6, followed by the codon 39. The mutation IVS-I-1 was found in one patient and IVS-I-110 was not found. There was no significant clinical differences between the hematological and biochemical parameters. There was a discrepancy between phenotype and genotype in some patients, but no significant difference between mutations and clinical manifestations. Conclusions: The results of this study reinforce the dominance of the mutation IVS-I-6 in northeastern Brazil. Are recommended further studies to investigate the co-inheritance with α-thalassemia in these patients to justify the discrepancy between genotypes and phenotypes. / Introdução: A beta talassemia é um grupo de distúrbios, cada um resultando de um defeito genético, na velocidade de síntese de uma ou mais cadeias globínicas da hemoglobina (Hb). A desproporção na produção das cadeias globínicas pode resultar em eritropoese ineficaz, produção insuficiente de Hb, hemólise e anemia de grau variado. A beta talassemia é mais frequente nos países banhados pelo mar Mediterrâneo, refletindo a participação desses povos na formação da população brasileira. As mutações predominantes no Brasil são a IVS-I-1, IVS-I-6, IVS-I-110 e o CD 39, as quais estão associadas a diversos quadros clínicos e são, em sua maioria, regionalmente específicas. Objetivo: Caracterizar o perfil clínico, hematológico e molecular dos indivíduos adultos com beta talassemia do Hospital Universitário Walter Cantídeo, em acompanhamento no centro de referência de Hematologia e Hemoterapia do estado do Ceará (HEMOCE). Metodologia: Foram analisados 22 indivíduos portadores de beta talassemia, sendo 7 intermediária e 15 menor, de ambos os sexos, no período de fevereiro de 2008 a setembro de 2009. Os dados clínicos e laboratoriais: hemograma, níveis de Hb A2 e de Hb F; ferro sérico; capacidade total e latente de ligação do ferro (CTLFe, CLLFe), ferritina e índice de saturação da transferrina (IST), foram obtidos dos prontuários, ao diagnóstico. Cerca de 5 mL de sangue venoso foi coletado em tubo contendo o anticoagulante EDTA para o estudo molecular. A análise das mutações foi realizada por meio da técnica da reação em cadeia mediada pela polimerase alelo específico (PCR-AE), onde foram analisadas as seguintes mutações: IVS-I-1, IVS-I-6, IVS-I-110 e o CD 39. As análises estatísticas foram desenvolvidas no software livre R (versão 2.7.0) e o nível de significância estabelecido foi 5%. Resultados: Dos 22 pacientes estudados, 15 eram portadores de β talassemia menor e sete intermediária. A idade variou de 18 a 68 anos, com média de 44,7 anos. 18,2 % do sexo masculino e 81,8% do sexo feminino. As mutações foram caracterizadas em 68,2% dos casos, que teve como mais frequente a IVS-I-6, seguida do códon 39. A mutação IVS-I-I foi caracterizada em um paciente e a IVS-I-110 não foi encontrada. Não houve diferença clínica significante entre os parâmetros hematológicos e bioquímicos. Houve discrepância entre o fenótipo e o genótipo em alguns pacientes, porém não houve diferença significativa entre as mutações e as manifestações clínicas. Conclusões: Os resultados do presente estudo reforçam o predomínio da mutação IVS-I-6 no nordeste do Brasil. Recomendam-se estudos posteriores para investigação da co-herança com a α talassemia nesses pacientes para justificar a discrepância entre genótipos e fenótipos.

A ocorrência dessas síndromes no Brasil tem sido estudada ao longo do tempo, entretanto, por causa das limitações nas metodologias utilizadas em muitas pesquisas, os dados de prevalência e diversidade são incompletos. Além disso, como conseqüência da composição étnica do nosso país, miscigenada por elementos de origens européia, africana, asiática e indígena, e desigualmente distribuída, a incidência da talassemia pode variar de região para região. Este estudo tem como objetivo mostrar caracteríticas clínicas, bioquímicas e moleculares das síndromes talassêmicas em população do estado de Pernambuco. Foram estudados 117 pacientes não relacionados, sendo 11 diagnosticados com talassemia ß maior, 36 talassemia ß intermediária e 70 com a interação S/ß talassemia, acompanhados na Fundação HEMOPE. A caracterização molecular foi feita por técnicas de PCR e seqüenciamento. A caracterização dos alelos ß talassêmicos revelou 15 diferentes mutações, sendo que 4 delas correspondem a 84,3% dos alelos ß talassêmicos, nas seguintes proporções: 54,9% IVS-I-6 (T-»C), 15,2% IVS-I-5 (G-»C), 7,9% códon 39 (C-»T) e 6,1% IVS-I-1 (G-»A). Foram encontradas outras mutações raras, pela primeira vez descritas no Brasil: IVS-II-849 (A-»G), poli-A (T-»C), - 29 (A-»G), Códon 30 (A-»C), IVS-I-2 (T-»C), IVS-II-837 (T-»G), Códons 106/107 (+G), além das mutações IVS-I-5 (G-»A) e -88 (C-»T). Uma nova mutação foi descrita: códon 12 (-C). Foram encontrados 30 pacientes homozigotos IVS-I-6 (T-»C) e tal mutação mostrou uma forte associação com o haplótipo VI. A mutação IVS-I-5 (G-»C) esteve relacionada ao haplótipo I, diferentemente do encontrado em outras populações (haplótipo VII) em que esta mutação é prevalente, podendo sugerir uma origem diferente para esta mutação no Brasil. A deleção -a3.7 kb esteve presente em 4,2% dos pacientes homozigotos para talassemia ß e em 15,7% dos S/ß tal, enquanto a aaaanti 3.7kb foi encontrada em 5,7% dos S/ß tal. O polimorfismo XmnI esteve presente em 6,1% dos pacientes com ß tal homozigota e em apenas 1,4% dos S/ß tal. Com relação ao alelo UGT1A1, foi visto que pacientes com genótipos (TA)7 / (TA)7 e (TA)7 / (TA)8 apresentaram maiores níveis de bilirrubina não conjugada e parecem ter maior probabilidade de desenvolver colelitíase. No Brasil, embora a composição étnica da população seja heterogênea, as mutações mais comuns são de origem Mediterrânea; Pernambuco pode ser considerado uma exceção já que cerca de 16,0% da população talassêmica possui mutações de origem Asiática e 10,8% são de origem africana. A diversidade de mutações e suas respectivas freqüências diferem do encontrado em outras populações brasileiras já estudadas, como a do Sudeste, onde 4 mutações também respondem pela maioria (97%) dos casos, porém a mutação predominante é a ß039 (C-»T) / Abstract: ß Thalassemia is a frequent inherited disease of the Hb molecule found in the majority of populations and distributed worldwide. The prevalence of this syndrome in Brazil has been studied for a long time, but due to limitations in the methodology of some research, data on prevalence and clinical diversity are not fully complete. Furthermore, as a consequence of our ethnic composition, mixed with an uneven distribuition of European, African, Asian and Indian native populations, the incidence of thalassemia may vary greatly from one region to another. This study aims to show clinical features, and also biochemical and molecular data of our thalassemia syndromes in a population setting of thalassemia patients in the State of Pernambuco. We studied 117 non-related patients, 11 of them with ß thalassemia major, 36 ß with thalassemia intermedia and 70 with the S/ß thalassemia interaction, followed regularly at the Fundação HEMOPE ¿ Recife - Brazil. Molecular characterization was performed by PCR techniques and DNA sequencing. Characterization of ß thalassemia alleles showed 15 different mutations, 4 of which corresponded to 84.3% of the ß thalassemia alleles, in the following proportions: 54.9% IVS-I-6 (T-»C), 15.2% IVS-I-5 (G-»C), 7.9% codon 39 (C-»T) and 6.1% IVS-I-1 (G»A). Other rare mutations were found and for the first time in Brazil, such as: IVS-II-849 (A-»G), poly-A (T?C), -29 (A?G), codon 30 (A-»C), IVS-I-2 (T-»C), IVS-II-837 (T-»G), codons 106/107 (+G), as well as IVS-I-5 (G-»A) and -88 (C-»T) mutations. A new mutation is also described: codon 12 (-C). We found 30 homozygous patients for the IVS-I-6 (T-»C) mutation, which showed a strong association with the ß gene haplotype VI. The IVS-I-5 (G-»C) mutation was related to the ß gene haplotype I, differently from that found in other populations studied (ß haplotype VII) where it is more prevalent, suggesting a different origin for this mutation in Brazil. The -a3.7 kb deletion was present in 4.2% of ß thalassemia homozygous patients and in 15.7% of the S/ß thalassemia patients, whereas the aaaanti3.7kb was found in 5.7% of them. The XmnI polymorphism was present in 6.1% of the homozygous ß thalassemia patients and in only 1.4% of the S/ß thalassemia individuals. In relation to the UGT1A1 allele, we found that patients with genotypes (TA) 7 / (TA) 7 and (TA) 7 / (TA) 8 showed higher levels of non-conjugated bilirrubin and seem to be more prone to developing gall stones. In Brazil, although the ethnic composition is markedly heterogeneous, the most common mutations are of Mediterranean origin; Pernambuco may be a curious exception to this, since around 16.0% of the thalassemia population have mutations of Asian origin and 10.8% of African origin. In conclusion, the diversity of the mutations found and their frequencies greatly differ from those found in other Brazilian populations studied previously, for example in the Southeast where only four thalassemia mutations are responsible for the majority (97%) of the thalassemia cases, although the commonest mutation is the ß039 (C-»T) / Mestrado / Ciencias Basicas / Doutor em Clínica Médica

Nas talassemias ß ocorre a supressão total ou parcial da produção de cadeias ß. O estado homozigótico da maioria das variantes genéticas da talassemia ß produz o quadro clínico da talassemia maior. Esses pacientes apresentam acentuada anemia e necessitam de transfusões sanguíneas regulares para sobreviverem. Os indivíduos heterozigotos para a talassemia ß apresentam, com raras exceções, apenas discreta anemia. Existem ainda alguns quadros clínicos não tão graves quanto à forma homozigótica clássica, geralmente não dependem de transfusões, que são denominados de talassemia intermediária. Os genes responsáveis pela síntese das cadeias da ß globina estão organizados em um cluster localizado no braço curto do cromossomo 11. Quase 200 alelos de talassemia ß já foram caracaterizados. Destes, 125 representam mutações pontuais em regiões funcionalmente importantes do gene, uma de particular interesse ao nosso estudo, é a mutação Cd 39 (C--T). Este trabalho teve como objetivo analisar o perfil global de expressão gênica de células CD 34+ de pacientes com talassemia ß, portadores da mesma mutação genética (Cd 39), mas com evoluções clínicas distintas, maior e intermediária, pelo método de SAGE (Serial Analysis of Gene Expression). Foram gerados 2718 trancritos únicos para o perfil de talassemia ß intermediária e 3052 para o perfil de talassemia ß maior, os quais foram classificados como genes identifcados, no matches, ESTs e outras sequências preditas e anotadas. As expressões de 14 genes foram quantificadas pela reação em cadeia da polimerase em tempo real nas amostras de células CD34+ dos pacientes talassêmicos intermediário e maior, com intuito de validar os resultados obtidos pelo SAGE. As expressões foram concordantes em 57,14% dos genes (ABCB10, APEX1, APOC1, EYA3, HMBS, OAZ1, SRGN, TAGLN2) e discordantes nos demais 42,85% (EIF5a, GRIN2C, HMGB1, NAE1, PCBP2, RAD23B). Quando ambos os perfis foram comparados entre si, 42 transcritos foram ditos como diferencialmente expresso (p<0,05). A análise funcional comparativa dos 42 transcritos diferencialmente expressos foi realizada de acordo com o Gene Ontology Consortium afim de obtermos a classificação funcional destes transcritos. Em conjunto, os resultados podem colaborar na identificação de transcritos importantes, que possam auxiliar na melhor compreensão da fisiopatologia desta doença e desempenhar papéis moduladores no fenótipo em talassemia ß / Abstract: Thalassemia syndromes are a heterogeneous group of hereditary diseases in which there is a reduction in the synthesis of one or more hemoglobin chains. In ß-thalassemia there is a reduction or total suppression of the expression of ß globin genes. The homozygous state of most ß-thalassemia genetic variants produces the clinical evolution of thalassemia major. These patients present severe anemia and require regular blood transfusions in order to survive. Heterozygous individuals present, with exceptions, discret anemia. There are some clinical evolutions that are not as severe as the classic homozygous form, which are often blood transfusion independent, named ß-thalassemia intemedia. The gene responsible for the synthesis of ß the globin chain is arranged in a cluster located on the short arm of cromossome 11. Nearly 200 ß-thalassemia alleles have been characterized. Of these, 125 represent mutations in functionally-important regions of the gene. A nonsense mutation, ie base substitution, which introduces a premature stop codon, destroying the normal reading and interfering in mRNA translation, is of particular interest to our study, especially the Cd 39 mutations (C--T). The aim of this study was to evaluate the global gene expression pattern of CD34+ culture cells from patients with ß-thalassemia, carrying the same genetic mutation (Cd 39) but with different phenotypes (major and intermedia), using Serial Analysis of Gene Expression (SAGE). Two SAGE profiles were gerated: INT and MAIOR. Comparison of the 2718 an 3052 distinct tags from the INT and MAIOR profiles, represented by identified trnacripts and novel tags, 42 tags demonstrated with a differential expression at a statistically significant level (p<0,05) and corresponded to known genes, ESTs or no matches. The expression of 14 genes was further investigated by the real-time polymerase chain reaction in cells cultured from patients with ß-thalassemia intermedia and major, with the purpose of to validating the results obtained by the SAGE method. Similar expressions were seen in 57.14% (ABCB10, APEX1, APOC1, EYA3, HMBS, OAZ1, SRGN, TAGLN2) and discordant expressions were seen in 42.85% (EIF5a, GRIN2C, HMGB1, NAE1, PCBP2, RAD23B). The functional classification was performed according to the Gene Ontology Consortium and a total of a 42 transcripts were submitted to functional classification. Together, our results may contribute to identify important transcripts that may play roles in modulating phenotype in ß-thalassemia and to better understand the pathophysiology of this disease / Mestrado / Ciencias Basicas / Mestre em Clinica Medica

Caracteriza??o molecular e laboratorial da talassemia beta e da intera??o hemoglobina s/talassemia beta

The inheritance of beta thalassemia is characterized by the existence of heterozygous individuals, compound heterozygotes, homozygotes and those with coinheritance of beta thalassemia allele and other thalassemias and/or hemoglobin variants. The aim of this study was to perform molecular and laboratory characterization of beta thalassemia in heterozygous and homozygous individuals and in those with coinheritance of S beta thalassemia. A total of 48 individuals were included (35 heterozygotes, 4 homozygotes and 9 S beta thalessemia carriers) referred to the Integrated Laboratory of Clinical Analyses of the Federal University of Rio Grande do Norte (UFRN) and the Hematology Ambulatory Facility of the Dalton Barbosa Cunha Hemocenter (Hemonorte Natal, Brazil). Peripheral blood samples form each patient underwent the following laboratory examinations: erythrogram, hemoglobin electrophoresis at alkaline pH, measurements of Hb A2, Fetal Hb and serum ferritin. DNA was extracted using the illustra blood genomicPrep Mini Spin Kit and molecular characterization was performed by the PCR/RFLP technique, which involves digestion with specific restriction enzymes for IVS-1 nt 1 (G?A), IVS-1 nt 6 (T?C) and codon 39 (CAG?TAG) mutations. Of the 35 heterozygotes, 37.1% showed IVS-1 nt 6 mutation, 42.9% IVS-1 nt 1 and 20% were carriers of other mutations not identified by the technique used. The four homozygous patients presented with the IVS-1 nt 6 mutation, while 66.7% of the individuals with S beta thalassemia had the IVS-1 nt 1 mutation. Codon 39 was not detected in any of the patients investigated. Of the thallasemic alleles found, 40.4% were IVS- 1 nt 1, 40.4% IVS-1 nt 6 and 19.2% were not identified. Laboratory data showed that the heterozygotes exhibited microcytosis and hypochromia, evidenced by MCV ranging from 57 to 75fL and MCH from 15.9 to 23.6 pg. Hemoglobin A2 varied between 3.7 and 7.2%. The homogygotes also showed reduced MCV and MCH and elevated HbA2.. Comparison of laboratory data between heterozygous individuals with IVS-1 nt 1 and IVS-1 nt 6 mutations showed that heterozygotes for the IVS1-1 mutation had significantly lower mean MCV and MCH (p = 0.023 and 0.007, respectively) and significantly higher hemoglobin A2 (p < 0.001) when compared to heterozygotes for the IVS-1 nt 6 mutation. PCR/RFLP was useful in identifying the presence or absence of IVS-1 nt 6, IVS-1 nt 1 and codon 39 mutations in most of the patients investigated here. This is the first study conducted in the state of Rio Grande do Norte, Brazil aimed at identifying beta thalassemia mutations and represents an important contribution to the knowledge regarding the molecular profile of beta thalassemia in our country / A talassemia beta ocorre devido ? diminui??o (?+, ?++, ?silent) ou aus?ncia (?0) de s?ntese de cadeias beta de globina e ? decorrente de v?rios mecanismos que provocam o defeito gen?tico. Dentre os 35 heterozigotos, 37,1% apresentaram a muta??o IVS-1 nt 6, 42,9% a IVS-1 nt 1 e 20% eram portadores de outras muta??es n?o identificadas com a t?cnica utilizada. Os quatro pacientes homozigotos apresentaram a muta??o IVS-1 nt 6, enquanto 66,7% dos indiv?duos com intera??o HbS/talassemia beta tinham a muta??o IVS-1 nt 1. A c?don 39 n?o foi detectada em nenhum dos pacientes investigados. Dentre os alelos talass?micos encontrados, 40,4% eram IVS-1 nt 1, 40,4% eram IVS-1 nt 6 e 19,2% n?o foram identificados. Em rela??o aos dados laboratoriais, os heterozigotos apresentaram microcitose e hipocromia evidenciada pelo VCM variando de 57 a 75fL, e o HCM, entre 15,9 a 23,6 pg. A hemoglobina A2 variou de 3,7 a 7,2%. Os homozigotos tamb?m apresentaram VCM e HCM reduzidos e HbA2 elevada. Ao comparar os dados laboratoriais entre os indiv?duos heterozigotos para as muta??es IVS-1 nt 1 e IVS-1 nt 6 observou-se que os heterozigotos da muta??o IVS1-1 apresentaram valores m?dios de VCM e HCM significativamente menores (p = 0,023 e 0,007, respectivamente) e hemoglobina A2 significativamente mais elevados (p < 0,001) quando comparados aos heterozigotos da muta??o IVS-1 nt 6. A t?cnica de PCR/RFLP se mostrou ?til para a identifica??o da presen?a ou aus?ncia das muta??es IVS-1 nt 6, IVS-1 nt 1 e ?0 c?don 39 na maioria dos pacientes investigados na pesquisa. A maior expectativa de vida dos pacientes com talassemia maior, as repetidas transfusões de concentrado de hemácias como parte do tratamento, podem ocasionar maior deposição de ferro nos órgãos, e consequentemente, as comorbidades. Dessa forma, dentre as comorbidades endócrinas, temos o diabetes mellitus como uma das principais, sendo necessário conhecer o panorama em nossa realidade. O estudo tem como objetivos determinar a prevalência do diagnóstico de diabetes em pacientes com talassemia maior, caracterizar e comparar os pacientes com talassemia maior, e diabetes mellitus, segundo as variáveis sociodemográficas e clínicas.Trata-se de um estudo descritivo e transversal, realizado em um centro de referência no tratamento de talassemia do interior paulista. A amostra foi constituída por 31 pacientes com talassemia maior. Para a coleta de dados utilizou-se um instrumento subdividido em duas partes. Os dados sociodemográficos e clínicos foram obtidos por meio de entrevista dirigida e os resultados de exames laboratoriais pelo prontuário eletrônico do paciente, no período de junho a agosto de 2015. Os dados foram digitados e importados para o programa SPSS for Windows, versão 17.0 e submetidos à análise estatística descritiva. O projeto foi aprovado pelo Comitê de Ética em Pesquisa da Escola de Enfermagem de Ribeirão Preto da Universidade de São Paulo (CEP/EERP-USP), sob Protocolo nº 41912415.3.0000.5393. Dos 31 pacientes com talassemia maior, 5 (16,1%) tinham diabetes mellitus. Em relação as variáveis sociodemográficas, não houve diferenças na distribuição dos pacientes entre os sexos, a maioria eram solteiros e cursaram até o ensino médio completo. A idade variou de cinco a 48 anos, com média de 24,9 anos de idade, a maioria recebia até 10 salários mínimos, eram estudantes e possuíam carteira de trabalho assinada. No que tange às variáveis clínicas, temos que o tratamento utilizado predominante foi o quelante oral deferasirox, e naqueles pacientes com diabetes, além do deferasirox, a maioria utilizava a insulina. O esquema transfusional predominante foi o de 15 a 22 dias. O índice de massa corpórea foi classificado como eutrófico e a pressão arterial, considerada ótima para a maioria dos pacientes. Quanto aos achados dos exames laboratoriais, os pacientes com diabetes e talassemia apresentaram valores alterados de glicemia de jejum e transaminase, já os pacientes sem diabetes apresentaram valores alterados de ferritina sérica. No que se refere aos achados dos exames de imagem, nenhum paciente com diabetes e Talassemia maior apresentou massa óssea adequada, o que reforça a importância de seu monitoramento. Destaca-se o aumento de sobrecarga cardíaca para os pacientes com diabetes e talassemia. Para os pacientes com talassemia e sem diabetes, a maioria apresentou grave sobrecarga de ferro hepático na ressonância magnética do fígado, enquanto para a maioria dos pacientes com diabetes foi considerado normal. Dessa forma, conhecer as características clínicas dos pacientes com talassemia maior e diabetes permite subsidiar a assistência de enfermagem qualificada e contribuir com a saúde dos pacientes com condições crônicas. Contudo, os nossos achados corroboram com as taxas de prevalência de diabetes em pacientes com talassemia maior encontradas na literatura nacional e internacional / The longer life expectancy of patients with thalassemia major, repeated red blood cell transfusions as part of the treatment, may lead to increased iron deposition in the organs, and consequently, comorbidities. Thus, among the endocrine comorbidities, we have diabetes mellitus as one of the main ones, being necessary to know the panorama in our reality. The aim of the study was to determine the prevalence of diabetes in patients with thalassemia major and to characterize and compare patients with thalassemia major, and diabetes mellitus, according to sociodemographic and clinical variables.This is a descriptive and cross-sectional study, carried out in a reference center in thalassemia treatment of São Paulo countryside. The sample consisted of 31 patients with thalassemia major. For data collection, an instrument was divided into two parts. Of the 31 patients with thalassemia major, 5 (16,1%) had diabetes mellitus. Regarding the sociodemographic variables, there were no differences in the distribution of the patients between the sexes, most of them were single and enrolled in high school. The age ranged from five to 48 years, with a mean of 24,9 years of age, the majority received up to 10 minimum wages, were students and had a work contract. Regarding the clinical variables, we have that the predominant treatment was the oral chelator deferasirox, and in those patients with diabetes, in addition to deferasirox, the majority used insulin. The predominant transfusion regimen was 15 to 22 days. Body mass index was classified as eutrophic and blood pressure was considered optimal for most patients. Regarding the laboratory findings, patients with diabetes and thalassemia had altered values of fasting glycemia and transaminase, whereas patients without diabetes had altered values of serum ferritin. Regarding imaging exams findings, no patient with diabetes and major thalassemia presented adequate bone mass, which reinforces the importance of their monitoring. The increase in cardiac overload for patients with diabetes and thalassemia stands out. For patients with thalassemia and without diabetes, the majority had severe hepatic iron overload in the magnetic resonance imaging of the liver, while for most patients with diabetes it was considered normal. Thus, knowing the clinical characteristics of patients with thalassemia major and diabetes allows subsidizing qualified nursing care and contributing to the health of patients with chronic conditions. However, our findings corroborate the prevalence rates of diabetes in patients with thalassemia major found in the national and international literature

Talassemias são as mais comuns desordens monogenéticas em humanos; são caracterizadas pela presença de anemia microcíticas e hipocrômicas que resultam da redução ou ausência na síntese de uma ou mais cadeias globínicas. No Brasil a beta-talassemia tem importante significado clínico e sua ocorrência pode ser explicada como conseqüência da grande mistura étnica. A beta-talassemia é extremamente heterogênea e mais de 200 mutações têm sido descritas causando diferentes graus de anemia. Essas mutações são regionalmente específicas e cada país possui seu grupo de alterações característico. No estado homozigoto são classificadas clinicamente como major e no estado heterozigoto como intermédia e minor. Os pacientes com talassemia intermédia podem não apresentar sintomas clínicos, mas constatam-se características laboratoriais específicas. A avaliação dos índices hematológicos, dosagens dos parâmetros bioquímicos e dos valores das hemoglobinas fetal e HbA2 em combinação com o entendimento das possíveis interações gênicas são os procedimentos recomendados para o diagnóstico laboratorial. O objetivo deste estudo foi determinar a presença de mutações em pacientes que utilizam a Divisão de Laboratório Central do Hospital das Clínicas da Faculdade de Medicina da Universidade São Paulo (DLC-HC-FMUSP) e que apresentavam perfil laboratorial sugestivo para beta-talassemia. Estudou-se a região do gene da globina em onde estão descritas as mutações mais freqüentes para nosso meio através da técnica de PCR, e posterior seqüenciamento para identificação das mutações. As mutações observadas na população avaliada (N=40), em ordem crescente de freqüência foram: Codon 39 (CT), IVS-I-110 (GA), IVS-I-1(GA), IVS-I-5 (GC), IVS-I-6 (TC), IVS-I-Exon-II (GA), Exon-II-IVS-II (GA), Stop Cd 6 (GT), Stop Cd15 (GA), Del C Cd 44 e 5UTR+20. Os primeiros quatro genótipos representaram 82,5% das mutações observadas neste trabalho. O uso combinado dos índices hematimétricos, parâmetros bioquímicos e avaliação das características morfológicas das hemácias demonstraram eficiência no rastreamento de pacientes portadores de beta-talassemia, assintomáticos A técnica do PCR e posterior processo de seqüenciamento demonstraram elevada eficiência na determinação das alterações moleculares no grupo de pacientes avaliados. / Thalassemias are the most common monogenic disorders in humans; are characterized by hypochromic and microcytic anemia arising from the reduction or absence in the synthesis of one or more chains globínicas. In Brazil, beta-thalassemia has important clinical significance and its occurrence can be explained as a result of the large ethnic mix. The beta-thalassemia is extremely heterogeneous and more than 200 mutations have been described causing varying degrees of anemia, these mutations are regionally specific and each country has its characteristic group of amendments. In the homozygous state are clinically classified as major and the heterozygous state as intermediate and minor. Patients with thalassemia can not present interim clinical symptoms, but have characteristics specific laboratory. The evaluation of hematologic indices, strengths and biochemical parameters of fetal hemoglobin and HbA2 values in combination with an understanding of possible interactions genes, are the recommended procedures for laboratory diagnosis. The purpose of this study was to determine the presence of mutations in patients who use the Division of Central Laboratory of the Hospital of the Faculty of Medicine of the University Sao Paulo (DLC-HCFMUSP) who presented suggestive profile laboratory for beta-thalassemia. It was studied the region of the gene of beta globin where they are frequently described the changes to our region through the PCR technique, and subsequent sequencing to identify the mutations. Of the 40 patients evaluated were found the following changes: Codon 39 (CT), IVS-I-110 (GA), IVS-I-1 (GA), IVS-I-5 (GC), IVS-I-6 (TC), IVS-I-Exon-II (GA), Exon-II-IVS-II (GA), Stop Cd 6 (GT), Stop Cd15 (G A), Del Cd 44 and 5UTR+20. The combined use of hematologic indices, biochemical parameters and evaluation of morphological characteristics of red blood cells demonstrated efficiency in the tracking of patients with beta- thalassemia, asymptomatic. The technique of PCR and subsequent process of sequencing demonstrated high efficiency in determining the molecular changes in the group of patients evaluated.

É originária da região do Mediterrâneo porém, atualmente apresenta-se amplamente distribuída pelo mundo devido ao fluxo gênico pela migração das populações. No Brasil, os tipos de talassemia mais prevalentes são as talassemias alfa e beta e apresentam número variável de indivíduos portadores devido ao alto grau de miscigenação da população. As formas graves de talassemia beta são facilmente diagnosticadas entretanto, as formas mais suaves muitas vezes são interpretadas e tratadas como anemia ferropriva. O presente estudo teve como objetivo caracterizar as formas talassêmicas do tipo beta e verificar os interferentes na expressão do fenótipo como a possível co-herança com hemocromatose hereditária. Através da associação de análise hematimétrica, metodologias clássicas e análise por HPLC analisamos 332 amostras de sangue com suspeita de talassemia beta. Um total de 70 amostras foram identificadas como portadores de hemoglobinas normais (AA), 145 portadores de talassemia beta heterozigota (BTH) e 117 portadores de talassemia alfa/beta (ABT). A análise estatística por regressão linear entre as metodologias clássicas e HPLC para quantificação de hemoglobina A2 e Fetal para os três grupos foram estatisticamente significativas. A análise molecular por PCR-ASO para identificação dos mutantes para hemocromatose hereditária mostraram 11,76% de mutação C282Y e 70,58% para H63D no grupo de talassemia beta e 25% de mutação C282Y e 75% H63D para o grupo de talassemia alfa/beta. Esses resultados evidenciam a necessidade da associação de metodologias para o correto diagnóstico da talassemia beta, bem como caracterização molecular para hemocromatose... / The beta-thalassemia constitutes a heterogeneous group of genetic disorders of hemoglobin synthesis. It has been one of the most common monogenic diseases identified and studied for many decades. Its origins are from Mediterranean region, although nowadays it is spread for the whole world due to gene flow of migration populations. In Brazil, the most prevalent types of thalassemia are the alpha-thalassemia and beta-thalassemia. They present a variable number of individual carriers due to high degree of miscigenation. The serious forms of beta-thalassemia are easily identified, but the milder forms are many times diagnosed and treated as iron deficiency anemia. The objective of the present study was to characterize the thalassemic forms and verify the interferents in the expression of the phenotype as the possible co-inheritance with hereditary hemochromatosis. It was analysed, utilizing hematimetric analysis, classic methodologies and HPLC analysis, 332 blood samples suspect of beta-thalessemia. The total of 70 samples were identified as carriers of normal hemoglobins (AA), 145 carriers of beta-thalassemia heterozigote (BTH) and 117 carriers of alpha/beta-thalassemia (ABT). The statistic analysis by linear regression between classic methodologies and HPLC for quantification of hemoglobin A2 and Fetal hemoglobin were statistically differents and significants. The molecular analysis by PCR-ASO to identify the mutants with hereditary hemochromatosis showed 11,76% of mutation C282Y and 70,58% to H63D in the beta-thalassemia group and 25% of mutation C282Y and 75% of H63D in the alpha/beta- thalassemia group. These results prove the necessity of association of methodologies in order to achieve the correct diagnosis of beta-thalassemia, as well as the molecular characterization of hemochromatosis, due to the fact of its possible co-inheritance with beta-thalassemia in the vast genotypic diversity found in Brazil.

Poucos estudos na literatura correlacionam possíveis efeitos deletérios dessas mutações, quando em heterozigose. A coerança com mutações em genes, não localizados no cluster do gene da β-globina, podem modificar o fenótipo talassêmico. O estresse oxidativo gerado pelo aumento de ERO é um dos mecanismos responsáveis pala aceleração da apoptose celular. Há evidências que o excesso de cadeias alfa, mesmo em pequenas quantidades, seja suficiente para iniciar a geração de ERO e, portanto, estresse oxidativo em beta talassêmicos heterozigotos. Nas formas homozigotas ou com dupla heterozigose os pacientes apresentam anemia grave, transfusão dependente, e a sobrecarga de ferro é uma das mais frequentes e graves complicações nesses indivíduos. Na forma heterozigota a ocorrência de sobrecarga de ferro é rara, porém alterações no perfil do ferro são observadas. O objetivo desse trabalho foi analisar a influência dos níveis de ferritina sérica, das mutações no gene da β-globina, das mutações no gene HFE, da atividade das enzimas de detoxificação GST e CAT e dos polimorfismos das GSTs na expressão fenotípica de indivíduos beta talassêmicos heterozigotos. Foram avaliados 158 portadores de beta talassemia heterozigota diagnosticados previamente por metodologias clássicas. Todos adultos, de origem caucasóide, residentes na região Central do Estado de São Paulo e acompanhados em serviços públicos ou privados da região. O grupo controle foi composto de 24 adultos, caucasóides e residentes na mesma região. O perfil... / Beta thalassemia may occur due to mutations β 0 or β +, according to absence or reduction in the production of beta globin chains, respectively. Few studies have been done correlating possible deleterious effects of these mutations, when in heterozygous. Co-inheritance with mutations in the genes, not located in the cluster of β-globin gene may modify the phenotype of the thalassemic. Stress oxidative generated by the increase of ROS is one of the mechanisms responsible for the acceleration of cell apoptosis. There are evidences that alpha chains in excess, even in small quantities, are sufficient to start ROS generation and thus oxidative stress in heterozygous beta thalassemic. In homozygous form or genetic heterozygous compound present severe anemia, transfusion dependent and iron overload is one of the complications more frequent and severe in these patients. In the heterozygous form the presence of iron overload is rare, but alterations in the profile of iron are observed. The aim of this study was to examine influence of the levels of serum ferritin, the mutations in the β-globin gene, the mutations in the HFE gene, activity of enzymes detoxification, GST and CAT, and polymorphisms of GSTs in the expression phenotypic of individuals heterozygous beta thalassemic. We evaluated 158 patients with beta thalassemia heterozygous previously diagnosed by classic methodologies. They were all adults, Caucasian origin, residents in the Central Region of São Paulo state and accompanied in public or private services of the region. Elementos de regulação e microRNAs envolvidos na modulação dos níveis de hemoglobina fetal em indivíduos portadores de beta-hemoglobinopatias

As análises in silico para o rastreamento dos fatores de transcrição foram realizadas comparando as sequências dos genes HBG1 e HBG2 de espécies de primatas do Velho e do Novo Mundo com as sequências da espécie Homo sapiens. O rastreamento in silico dos microRNAs preditos à regulação dos genes da γ-globina foi realizado a partir da utilização de bancos de dados públicos de microRNA. Para as análises de expressão dos microRNAs selecionados foram analisadas 38 amostras de sangue periférico, divididas em cinco grupos de estudo: grupo controle, anemia falciforme em uso ou não de hidroxiureia (HU), e talassemia beta na presença e ausência do alelo β0. Todas as amostras foram submetidas aos testes clássicos de diagnóstico de hemoglobinopatias, quantificação das frações de globinas por cromatografia e análise molecular para a genotipagem dos indivíduos com anemia falciforme (PCR-RFLP) e talassemia beta (PCR-AE e sequenciamento genômico), além da caracterização dos haplótipos βS e β-Tal (PCR-RFLP). Some of the genetic elements that act in the regulation of Hb F expression are transcription factors and microRNAs. The aim of this study was to use phylogenetic footprinting to screen transcription factors that have binding sites in the γ-globin genes' noncoding regions, and miRNAs candidates to modulating Hb F levels, also evaluating the expression of microRNAs selected from in silico analyses in different groups with distinct profiles of Hb F expression. In silico analysis to screening of transcription factors were performed using bioinformatics tool VISTA, comparing the sequences of HBG1 and HBG2 genes in species of the Old and New World primates with the Homo sapiens sequences. The screening of predicted microRNAs to regulation of γ-globin genes was performed using public databases of microRNA. For the expression analyses of the selected microRNAs, we analized thirty-eight peripheral blood samples, divided into five groups: control group, sickle cell anemia using or not hydroxyurea (HU), and beta-thalassemia with and without allele β0. All samples were tested to hemoglobinopathies by classical diagnostic tests, quantification of globin fractions by chromatography and molecular analyses for genotyping of sickle cell anemia (PCR-RFLP) and beta-thalassemia (PCR-AE and genomic sequencing), and the characterization of haplotypes βS and β-Tal (PCR-RFLP).
