Influência da ovariectomia no processo de reparo de enxerto ósseo autógeno em bloco na mandíbula : estudo histológico em ratas adultas /

Orientador: Álvaro Francisco Bosco / Resumo: O objetivo do presente estudo foi avaliar, por meio de análise histológica, a influência da ovariectomia (depleção de estrógeno) no processo de reparo de enxertos ósseos autógenos em bloco. Para isso, foram utilizadas 36 ratas fêmeas, com idade de 12 meses, divididas em grupo controle (GC) e grupo experimental (GE) de 18 animais cada. As ratas do GE foram submetidas à cirurgia de ovariectomia (Ov), enquanto as ratas do GC foram submetidas a simulação do mesmo procedimento cirúrgico, sem a remoção dos ovários (Si). Transcorridos 30 dias da Ov ou Si, todos os animais receberam enxerto ósseo autógeno em bloco na mandíbula, na região próxima ao ângulo, tendo como área doadora o osso parietal da calvária. Os animais de cada grupo foram divididos em subgrupos de 6 animais, os quais foram submetidos à eutanásia nos períodos de 7, 14 e 28 dias, após a realização do enxerto ósseo. Aos 7 dias, a interface enxerto-leito receptor no GC mostrou-se preenchida por um típico tecido de granulação, com discreta atividade de angiogênese na porção central, ao passo que no GE, havia ainda a presença de coágulo sanguíneo e um tecido de granulação em organização. Aos 14 dias, no GC, a interface apresentou-se parcialmente preenchida por matriz óssea neoformada, estabelecendo uma união do enxerto ao leito receptor, enquanto que no GE, a maior parte da interface encontrou-se preenchida por um típico tecido de granulação com nítida angiogênese e discreta atividade osteogênica, com neoformação de matriz óssea em algumas áreas. Aos 28 dias, o enxerto no GC apresentou-se histologicamente integrado ao leito receptor, com acentuada atividade osteogênica, ao passo que no GE a interface apresentou-se parcialmente preenchida por matriz óssea neoformada, encontrando-se também áreas de tecido conjuntivo entre o enxerto e o leito receptor... (Resumo completo, clicar acesso eletrônico abaixo) / Abstract: The aim of this study was to investigate, by means of histological analysis, the influence of ovariectomy (estrogen depletion) on the repair process of autogenous block bone grafts. 36 female adult rats, age twelve months, were assigned to two groups (n=18): control group (CG) and experimental group (EG). The rats in the EG had an ovariectomy (Ovx) whereas those in the CG underwent the same surgical intervention but did not have their ovaries removed ("Sham"). Thirty days after Ovx or "Sham" all the animals received autogenous block bone grafts in the mandible, in the region near the mandibular angle. The donor area was the parietal bone. The animals of each group were euthanized at 7, 14 and 28 days postoperative. On the 7th postoperative day, the graft-recipient site interface in the CG appeared filled by a typical granulation tissue with discreet angiogenic activity in the central portion whereas the EG still exhibited a blood clot and an organizing granulation tissue. On the 14th postoperative day, the interface in the CG was partially filled with newly formed bone matrix establishing a union between the graft and the recipient site. The majority of the interface in the EG was filled by a typical granulation tissue with evident angiogenic activity, discreet osteogenic activity and newly formed bone matrix in some areas. On the 28th postoperative day, the graft in the CG appeared histologically integrated with the recipient site and exhibited pronounced osteogenic activity. On the other hand, the interface in the EG appeared partially filled by newly formed bone matrix with areas of connective tissue observed as well. Análise comparativa in vitro da citotoxicidade dos ácidos hialurônicos de alto e baixo peso molecular em enxertia óssea

This technology requires a carrier for the cells, where hyaluronic acid, a natural glycosaminoglycan, appears to have suitable characteristics to carry out this role. It exerts various biological functions depending on its molecular weight, which can be classified as high (> 1000 kDa), low (from 50 to 1000 kDa) or very low (<50 kDa). Furthermore, there is the hyaluronidase, an enzyme that degrades hyaluronic acid into smaller fragments, influencing the cellular response. Thus, this study aimed to evaluate in vitro the influence of the enzyme hyaluronidase (HYAL) on the high hyaluronic acid (HA-HMW) and low molecular weight (HA-LMW) and their potential cytotoxicity on NIH - 3T3 cells. The MTT assay was performed analyzing cell viability of the following groups: (G1) Cells + HA-HMW; (G2) Cells + HA- LMW; (G3) Cells + HA-HMW + HYAL; (G4) Cells + HA-LMW + HYAL; (G5) Cells + HA-HMW + hydroxyapatite; (G6) Cells + HA-LMW + hydroxyapatite; (G7) Cells + HA-HMW + HYAL + hydroxyapatite; (G8) Cells + HA-LMW + HYAL + hydroxyapatite; (G9) Cells (positive control); (G10) Cells + hypochlorite (negative control). The cell viability for each group were as follows: (G1) 76,0%, (G2) 63,3%, (G3) 67,4%, (G4) 76,5%, (G5) 57,6%, (G6) 60,8%, (G7) 64,4%, (G8) 74,3%, (G9) 100% e (G10) 12,4%.It was found that all groups had significantly higher cell viability compared to the negative control. And although there was no significant difference between experimental groups, only the G1, G4 and G8 groups did not differ significantly from the positive control. Therefore, HA-HMW had better viability in the absence of enzyme hyaluronidase, although HA-LMW provided greater cell viability in the presence of the enzyme. Results indicated that the low-weight hyaluronic acid might be applicable for tissue engineering grafting techniques. / Nas últimas décadas, técnicas envolvendo engenharia tecidual têm sido reportadas na literatura como opção alternativa aos enxertos autógenos convencionais para correção de defeitos ósseos e posterior instalação de implantes dentários. Estas buscam basicamente um material que apresente as mesmas propriedades biológicas do osso autógeno sem seus fatores limitantes. Tal tecnologia necessita, entre outros componentes, de um scaffold para as células, onde o ácido hialurônico, um glicosaminoglicano natural, parece ter características adequadas para exercer este papel. Ele exerce funções biológicas diferentes dependendo do seu peso molecular, que pode ser classificado em alto (> 1000 kDa), baixo (de 50 a 1000 kDa) ou baixíssimo (< 50 kDa). Além disso, no organismo humano há a presença da hialuronidase, enzima que degrada o ácido hialurônico em fragmentos menores, elementos estes que possivelmente podem influenciar a resposta celular. Desta forma, este estudo buscou avaliar, in vitro, a influência da enzima hialuronidase (HIAL) sobre o ácido hialurônico de alto (AH-APM) e baixo peso molecular (AH-BPM), bem como seu possível potencial citotóxico sobre células da linhagem NIH-3T3.Para tal, foi realizado o teste de MTT analisando a viabilidade celular dos seguintes grupos: (G1) Células + AH-APM; (G2) Células + AH-BPM; (G3) Células + AH-APM + HIAL; (G4) Células + AH-BPM + HIAL; (G5) Células + AH-APM + Hidroxiapatita; (G6) Células + AH-BPM + Hidroxiapatita; (G7) Células + AH-APM + HIAL + Hidroxiapatita; (G8) Células + AH-BPM + HIAL + Hidroxiapatita; (G9) Células (controle positivo); (G10) Células + hipoclorito (controle negativo). A viabilidade celular média de cada grupo foi: (G1) 76,0%, (G2) 63,3%, (G3) 67,4%, (G4) 76,5%, (G5) 57,6%, (G6) 60,8%, (G7) 64,4%, (G8) 74,3%, (G9) 100% e (G10) 12,4%. Os resultados indicaram que todos os grupos apresentaram viabilidade celular significativamente superior ao controle negativo. E apesar de não haver diferença significativa entre os grupos experimentais, somente os grupos G1, G4 e G8 não diferiram do controle positivo. O AH-APM apresentou melhor viabilidade na ausência da enzima hialuronidase, enquanto que o AH-BPM proporciona uma melhor viabilidade celular na presença desta enzima.

Reparo do defeito ósseo na trepanação craniana com enxerto ósseo autólogo : estudo comparativo morfológico e tomográfico

A cranioplastia para a correção dos defeitos causados pelo trépano cirúrgico é um dos problemas não adequadamente solucionados em neurocirurgia. As depressões deixadas pelo trépano neurocirúrgico nas craniotomias é uma queixa freqüente dos pacientes no pós-operatório. A extensa gama de materiais aloplásticos que tem sido propostos e utilizados para este fim é um bom exemplo desta dificuldade. O pó de osso, que é comumente usado nessas reconstruções, sofre um alto grau de reabsorção, mas até o momento, essa absorção não tinha sido mensurada, nem comparada com a do osso cortical. Outro grande desafio não solucionado até o momento é a dificuldade para acessar o desenvolvimento biológico de ossos transplantados em humanos, impossibilitando assim a quantificação dos resultados. A avaliação do local de reconstrução pela inspeção e pela palpação é um método grosseiro de aferição e permite apenas uma análise qualitativa subjetiva. Nossa insatisfação com o resultado estético das cranioplastias, além da falta de medidas objetivas das falhas ósseas, nos levou a testar a utilização do enxerto ósseo autógeno da lâmina interna da calota craniana para a resolução estético-funcional das deformidades causadas pelo trépano neurocirúrgico. O pó de osso foi recolhido durante a trepanação e o fragmento ósseo foi retirado, com uma trefina, especialmente confeccionada pelo autor para esse fim Após oito meses, realizamos tomografia de crânio com reconstrução óssea. Os locais reconstruídos com pó de osso e com fragmento ósseo foram delimitados e tiveram a densidade óssea aferida mediante tomografia computadorizada de crânio, procedimento ainda não utilizado na prática clínica neurocirúrgica. Os resultados foram expressos em unidades Hounsfield (HU). As reconstruções foram avaliadas por dois especialistas diferentes, cegados para o estudo, que atribuíram uma nota de 0 a 10 para a aparência estética dos locais reconstruídos. Analisamos a correlação entre a nota atribuída pelos avaliadores com a densidade medida pela TC. Foram reconstruídos 108 orifícios de craniotomia nos 23 pacientes estudados, sendo 36 orifícios reconstruídos com fragmento circular da lâmina interna (33,3%) e 72 com pó de osso (66,6%). A densidade média das reconstruções com fragmento circular foi 987,01 ± 172,6 HU, enquanto a de pó de osso foi 464,46 ±197,66 HU. Essa diferença foi estatisticamente significativa (p< 0,001, teste t de Student, < 0,05). A nota média atribuída às reconstruções pelos avaliadores foi de 9,5 para ao fragmento ósseo e 5,7 para o pó de osso, (p< 0,001, teste t pareado < 0,05). Não ocorreram complicações durante o seguimento de até 25 meses. Ao final do estudo, pode-se concluir que o fragmento de osso autólogo da lâmina interna é superior ao pó de osso para reconstrução dos orifícios de trepanação. A absorção do fragmento ósseo é menor e sua característica estética é superior. O custo nulo, bem como a ausência de morbidade do sítio doador, habilita o auto-enxerto como a primeira escolha para a correção dos defeitos causados pelo trépano cirúrgico. / The cranioplasty for the correction of defects caused by surgical trepan is one of the problems that is not properly solved in neurosurgery. Frequently, the patients complain during postoperative period about the depressions left by the neurosurgical trepan in the craniotomy. The extensive gamma of aloplastic materials that has been considered and used for this end is a good example of this difficulty. The bone dust, frequently used in these reconstructions, suffers a high reabsorption degree, but until the moment, this absorption had not been mesured, nor compared to the one of the cortical bone. Another great challenge that has not been solved yet until the moment is the difficulty to access the biological development of transplanted bones in human beings, something that disabled the results quantification. The place reconstruction evaluation through inspection and palpation is a coarse method of gauging and allows only a subjective qualitative analysis. Our insatisfaction with the cranioplasties aesthetic result and the lack of objective measures of the bone imperfections made us test the use of bone autogenous graft of the internal calvarial bone blade as a aesthetic-functionary resolution for the deformities caused by neurosurgical trepan. Twenty three adult patients submitted to the surgery due to varied causes (ragged aneurism, not ragged aneurism, arteriovenous malformation and benign neoplasia) had had the trepanation orifices reconstructed with autologous bone dust or a autologous bone circular took from the internal blade of the bone segment removed for the intracranium procedure. In the same pacient, the two types of reconstruction had been carried through. The bone dust was collected during trepanation with a trephine especially confectioned by the author for this end. After eight months, we have carried through a tomography of the cranium that had bone reconstruction. The places reconstructed with dust of bone and with bone fragment had been delimited and they had had the bone density gauged by means of cranium computerized tomography, procedure still not used in the practical neurosurgical clinic. The results were expressed in Hounsfield units (HU). The reconstructions were evaluated by two different specialists, blinded for the study, who attributed a grade from 0 to 10 for the aesthetic appearance of the reconstructed places. We analyzed the correlation between the grade attributed for the appraisers with the density measured for the CT. 108 orifices of craniotomy in the 23 studied patients were reconstructed, among those 36 orifices were reconstructed with circular fragment from the internal blade (33,3%) and 72 with bone dust (66,6%). The mean density of the reconstructions with circular fragment was 987.01 ± 172,6 UH, while the one of bone dust was 464,46 ±197,66 UH. This difference was significant in a statistical way (p< 0.001, paired t-test < 0,05). The average grade attributed to the reconstructions by the appraisers was of 9,5 to the bone fragment and 5,7 to the bone dust, (p< 0.001. paired t-test < 0,05). It had not occurred complications during the pursuing of up to 25 months. In the end of the study, it can be concluded that the autologous bone fragment of the internal blade is superior to the dust of bone for the reconstruction of trepanation orifices. The absorption of bone fragment is smaller and its aesthetic characteristic is superior. The null cost, as well as the absence of morbidade of donor place, qualifies the selfgraft as the first choice for the correction of the defects caused by surgical trepan.

Enxertos ósseos liofilizados e impactados humano e bovino em revisão de artroplastia total de quadril

O presente estudo é uma coorte não concorrente em 63 pacientes (66 quadris) submetidos à revisão de artroplastia total de quadril (RATQ), com enxerto ósseo liofilizado moído e impactado. Foi realizado no Serviço de Ortopedia e Traumatologia do Hospital de Clínicas de Porto Alegre (HCPA), no período de maio de 1997 a setembro de 2003. O objetivo do estudo foi comparar clínica, radiográfica e cintilograficamente a capacidade de osteointegração dos enxertos ósseos liofilizados humano e bovino impactados em RATQs cimentadas e não-cimentadas. Os pacientes foram divididos em dois grupos: o Grupo 1 (n=35) foi composto pelos que receberam enxerto ósseo liofilizado de origem humana e o Grupo 2 (n=31) por aqueles que receberam enxerto de origem bovina. O tempo médio de seguimento foi de 33 meses. Os enxertos ósseos purificados e liofilizados foram produzidos pelo Banco de Tecidos do HCPA. A análise clínica baseou-se no escore de Merle, d’Aubigné e Postel; a radiográfica, nos critérios de radioluscência, densidade, formação de trabeculado ósseo, migração dos componentes e floculação, formulando-se um escore radiográfico de osteointegração (EROI). Foram escolhidos, aleatoriamente, como forma complementar de análise por imagem, 35 (53%) pacientes assintomáticos para realizarem cintilografia óssea com Tecnécio, sendo 17 (48,5%) do Grupo 1 e 18 (51,5%) do Grupo 2. Não foram encontradas diferenças clínicas, radiográficas ou cintilográficas relevantes entre os grupos, obtendo-se em torno de 85% de integração do enxerto, tanto no componente acetabular quanto femoral. Estes resultados são comparáveis aos relatados na literatura com o uso de enxerto alogênico congelado e confirmam a adequacidade do uso de enxertos liofilizados de origem bovina e humana em RATQ. / This is a non-concurrent cohort trial of 63 patients (66 hips) submitted to revision total hip arthroplasty (RTHA) using impacted freeze-dried cancellous bone grafts. The study was carried out in the Orthopedics and Traumatology Unit of Hospital de Clinicas de Porto Alegre (HCPA) from May 1997 to September 2003. The main purpose of the study was to compare clinical, radiographic and scintigraphic graft incorporation capability between human and bovine freeze-dried bone grafts. Patients were divided into two groups: Group 1 (n=35) was composed of those receiving human grafts and Group 2 (n=31) of those receiving grafts of bovine source. The median follow-up was 33 months and no death or major complications occurred in this series. The grafts were purified and freeze-dried at the Tissue Bank of the HCPA. The clinical analysis was based on the score of Merle, d’Aubigné and Postel; and the radiographic analysis on the criteria of radiolucency, density, trabecula formation, migration and “cotton flock” appearance by using these parameters, isolated or combined in an established score for radiographic bone incorporation (SCORBI). Thirty-five (53%) asymptomatic patients, 18 (51%) and 17 (49%) from Groups 1 and 2 respectively, were randomly selected to be assessed by 99mTc scintigraphy, as a complementary form of image analysis. No clinical, radiographic or scintigraphic differences were observed between the groups and both groups showed around 85% of graft integration both in the acetabular and in the femoral component. These results are comparable to those reported in the literature with the use of deep-frozen grafts. Therefore, bovine and human freeze-dried grafts can be safely and adequately used in RTHA.

A proposta neste estudo foi avaliar por meio de um estudo biomecânico utilizando o sistema fotoelástico, diferentes protocolos de angulações dos implantes posteriores convencionais e comprimento do cantilever. Os implantes de diâmetro 3,5 x 15 mm de comprimento, dispostos de forma que em 3 modelos os implantes foram instalados com angulação de 15º e em outros 3 modelos com angulação de 35º. Os modelos fotoelásticos foram submetidos a teste de forças verticais sobre os implantes e cantilever. As franjas de forças formadas durante os testes foram avaliadas por meio de um polaroscópio de forma qualitativa e em seguida foi realizada a leitura das regiões de maior concentração de tensão. Foram avaliadas tensões submetidas aos implantes, sendo este dividido em tensão total, tensão cervical e tensão apical para cada grupo analisado. Não houve diferença estatisticamente significativa quando comparado os implantes nas diferentes angulações em relação à incidência de tensão total, tanto em cantilever, quanto em pilar e oclusal. Houve diferença estatística significativa em relação à incidência de força no cantilever e no pilar somente quando é analisada a região apical do implante. Em relação à incidência de força na oclusal houve diferença estatisticamente significativa somente na região cervical. Quando comparado à incidência de forças no cantilever nos dois tipos de angulações, o implante 35º apresentou melhor distribuição de forças na região apical, o mesmo ocorreu para incidência de forças na oclusal da região cervical do implante de 35º. Concluiu-se que o implante com angulação de 35º mostrou-se com melhor distribuição de forças quando comparado com o implante de angulação de 15º que apresentou maior concentração de tensão na região apical / Abstract: Introduction: The implants conventional with angulation are used in treatment with dental implants into bone in the jaws heavily absorbed so that there is no need to use bone grafts to reconstruct the same. Objective: To evaluate using a biomechanical study (photoelastic system) using protocols of the angulation different in the posterior implants conventional and length of the cantilever. Materials and Methods: We made 6 models in photoelastic resin from a skull human prefabricated. In these models have been installed 12 implants with diameter 3.5 x 15 mm long cylindrical cone. The implants were placed so those in three models were fitted with implants of angle of 15 ? and other three with models implants of 35 ?. Photoelastic models were subjected to test of vertical forces on the cantilever of the implants. The fringes forces formed during the tests were evaluated through a polariscope qualitatively and then were carried out reading the regions of greatest stress concentration. Was further assessed the total area of tension that the implant was submitted, which was divided into the total stress, tension neck and apical voltage for each group analyzed. Results: No statistically significant difference when compared the implants in different angles in relation to the incidence total tensile strength in both in relation the cantilever, abutment and occlusal. There was a statistically significant difference in the incidence of force in the cantilever when the region of pillar is analyzed only in the apical region of the implant. There was statistically significant to the incidence of occlusal force only in the cervical region. When compared to the effect of forces on the cantilever in both types of angles, the implant 35 ? showed better distribution of forces in the apical region, the same occurred in the incidence of occlusal forces on the neck of the implant of 35 ?. O carreador utilizado foi o gel de polissacarídeo, que é biocompatível, de baixa toxicidade e apresenta características físicas e químicas adequadas para a função de enxertos. Objetivos: Avaliar as propriedades do gel de polissacarídeo de melaço, como carreador das Proteínas Ósseas Morfogenéticas (BMP) na reparação de defeitos críticos na calvária de ratos. Métodos: Trinta e seis ratos machos adultos Wistar foram distribuídos em quatro grupos experimentais, um grupo controle positivo e um grupo controle negativo, com seis animais cada grupo. Foi realizado defeito crítico de 11 mm na calvária dos animais sob condições de assepsia e anti-sepsia. Nos animais experimentais foram enxertados: gel+matriz orgânica do osso liofilizado cortical (gen-ox-org®)+proteína morfogenética do osso (genpro®) um grupo com membrana biológica de origem bovina (genderm®) e um grupo sem membrana; gel + osso bovino inorgânico liofilizado esponjoso (gen-ox-inorg®) + proteína morfogenética do osso (genpro®) um grupo com membrana biológica de origem bovina (genderm®) e um grupo sem membrana; o grupo controle positivo utilizou o enxerto com osso orgânico e membrana, sem gel, o grupo controle negativo com confecção do defeito e sacrifício imediato. O sacrifício ocorreu 180 dias após a cirurgia. As cabeças foram fixadas em formaldeído a 10% tamponado. Para obtenção das imagens foi utilizado um aparelho tomógrafo iCATVision (cone-bean 3D), que permitiu, após reconstrução das imagens e tratamento com Photoshop 10.0 CS3, o cálculo das áreas radiolúcidas. Os defeitos residuais foram medidos com paquímetro; a partir da metade do defeito foi realizada secção da calota e utilizado o segmento caudal para análise histológica qualitativa, após serem corados com hematoxilina e eosina. Foi realizado estudo histomorfometrico para avaliar a intensidade da angiogênese. Resultados: Os grupos que utilizaram o gel+osso bovino inorgânico liofilizado esponjoso (gen-ox-inorg®)+proteína morfogenética do osso (genpro®), o grupo com membrana biológica de origem bovina e o grupo sem membrana foram os que apresentaram menor área radiolúcida, melhor reparação na observação qualitativa histológica, com diferença significativa a 5% comparada ao controle positivo na análise do número de artérias e veias. Conclusão: Os resultados sugerem que o gel de polissacarídeos, associado ao osso liofilizado inorgânico, pode ser usado como carreador eficiente para a BMP na estimulação da cicatrização óssea.

Vinte e nove coelhos machos (Oryctolagus cuniculus) tiveram fragmentos ósseos corticais de 8mm de diâmetro removidos com trefina da metáfise proximal tibial que foram transplantados para uma área adjacente após a realização de um dos seguintes tratamentos: G-EDTA (desmineralização com EDTA pH neutro a 24% na forma de gel por 3 minutos das superfícies ósseas de contato); G-EDTA-P (o mesmo tratamento de G-EDTA e perfuração do leito receptor); G-P (perfuração do leito receptor) e G-C (nenhum tratamento adicional ao transplante). Noventa dias após as cirurgias, os animais foram eutanasiados para obtenção de blocos ósseos contendo os enxertos e preparo de corpos de prova para ensaio mecânico de determinação da resistência à tração e módulo de elasticidade. Os valores encontrados para resistência à tração foram: G-C = 2.0861.200 MPa; G-EDTA = 2.0330.976 MPa; G-EDTA-P = 1.9770.795 MPa e G-P = 1.8260.800 MPa. Entretanto, não houve diferença estatisticamente significante entre os grupos (p=0.819). Os valores médios do módulo de elasticidade foram: G-C = 38.8022.72 MPa; G-EDTA-P = 26.2414.17 MPa; G-EDTA = 25.119.89 MPa e G-P = 17.517.94 MPa, sendo identificada diferença estatística apenas entre o G-C e os demais. Concluiu-se que a desmineralização por EDTA das superfícies ósseas de contato em enxertos autógenos em bloco do tipo onlay não interfere na resistência à tração da interface independentemente da perfuração do leito receptor e que tanto a desmineralização como a perfuração aumentam a deformação dos corpos de prova sugerindo um melhor comportamento elástico nesses grupos. / Cortical bone fragments of 8mm of diameter were removed with trephine burr from the proximal tibial metaphysis of twenty nine male rabbits (Oryctolagus cuniculus) and transplanted to an adjacent area after one of the following treatments: G-EDTA (demineralization of the contacting bone surfaces with EDTA pH 24% during 3 minutes); G-EDTA-P (the same treatment of G-EDTA and perforation of the receiving host bed); G-P (perforation of the receiving host bed) and G-C (no additional treatment to the transplant). Ninety days post-operatively, the animals were euthanized in order to attain bone blocks with the grafts. The specimens were prepared to mechanical assay for tensile strength and Young modulus. The values found for tensile strenght were: G-C = 2.086±1.200 MPa; G-EDTA = 2.033±0.976 MPa; G-EDTA-P = 1.977±0.795 MPa and G-P = 1.826±0.800 MPa. These differences, therefore, were not statically significant (p=0.819). The Young modulus average values for the specimens were: G-C = 38.80±22.72 MPa; G-EDTA-P = 26.24±14.17 MPa; G-EDTA = 25.11±9.89 MPa and G-P = 17.51±7.94 MPa. There were statistically significant differences only between the G-C and the other groups. The results showed that EDTA demineralization of the contacting bone surfaces in onlay autogenous bone grafts does not interfere in the tensile strenght at the interface independently of the perforation of the host bed. On the other hand, demineralization as well as perforation increased the deformation of the specimens suggesting a better elastic behavior for these groups.

O alcoolismo é considerado uma doença crônica, trazendo prejuízos para saúde do indivíduo. O tecido ósseo também sofre sérios danos com o consumo crônico do álcool, pois induz defeitos na mineralização e redução da espessura do osso medular e cortical. Devido à grande procura por tratamentos reconstrutivos para lesões e perdas ósseas, os enxertos xenógenos vêm sendo uma boa opção para o tratamento regenerativo. Em vista da procura de pacientes que fazem consumo crônico do álcool e apresentam perdas ósseas, necessitando de enxertos xenógenos, este trabalho teve como objetivo comparar, por meio de análise histomorfológica e histomorfométrica, a ação de dois biomateriais, o Bio-Oss® e o OrthoGen®, no processo de reparo de defeitos ósseos em tíbia de ratos submetidos ou não ao alcoolismo experimental. Foram utilizados 80 ratos machos (Rattus norvegicus) da linhagem Wistar, com 60 dias de idade, separados aleatoriamente em quatro grupos experimentais contendo 20 animais cada: Grupo Água Bio-Oss® (GAB), Grupo Etanol Bio-Oss® (GEB), Grupo Água OrthoGen® (GAO) e Grupo Etanol OrthoGen® (GEO). Os animais dos grupos GEB e GEO foram submetidos à adaptação gradativa ao álcool e depois permaneceram a 25% por 90 dias. Após esse período, todos os animais foram submetidos à cirurgia experimental. Foi realizada uma cavidade cirúrgica de aproximadamente 3 mm de diâmetro na epífise proximal da tíbia e nos animais dos grupos experimentais GAB e GEB se fez o preenchimento com o biomaterial Bio-Oss®. Os animais dos grupos experimentais GAO e GEO receberam o preenchimento da cavidade com o biomaterial OrthoGen®. Decorridos os períodos de 10, 20, 40 e 60 dias pós-cirúrgico, cinco animais de cada grupo, por período, foram eutanasiados. Os resultados histomorfológicos demostraram que os animais dos grupos GAB e GEB apresentaram células sanguíneas e tecido conjuntivo nos períodos de 10 e 20 dias, e nos períodos de 40 e 60 dias ocorreu a formação de novo osso, sendo que o grupo GEB apresentou um maior retardo na formação quando comparado ao GAB. Nos grupos GAO e GEO o início da formação de tecido ósseo ocorreu nos períodos de 40 e 60 dias, com um retardo na formação quando comparado ao biomaterial Bio-Oss®. Na análise histomorfométrica, comparando os grupos que receberam a dieta líquida não alcóolica com biomaterias diferentes (GAB e GAO) ocorreu diferença significante em formação óssea em todos os períodos sendo que o grupo Água Bio-Oss® apresentou uma maior formação óssea. Nos grupos que receberam a dieta alcóolica, com biomateriais diferentes (GEB e GEO), ocorreu diferença em formação óssea nos períodos de 20, 40 e 60 dias, onde o biomaterial Bio-Oss® apresentou maiores médias. Comparando os grupos que receberam o mesmo biomaterial, mas com dieta líquida diferente (GABXGEB e GAOXGEO), evidenciou-se diferença significante em formação óssea em todos os períodos (exceto GAOXGEO; 10 dias), sendo que os grupos que receberam dieta líquida apenas com água apresentaram melhores resultados. Conclui-se que o biomaterial Bio-Oss® mostrou-se superior na neoformação óssea quando comparado ao Ortoghen®, e que a dieta alcoólica interferiu de forma negativa no processo de reparação. / Alcoholism is considered a chronic disease, which brings harm to an individuals health. Bones also suffer serious damage resulting from the chronic consumption of alcohol, since it induces defects in the mineralization and reduction of the thickness of the cortical and the cancellous bone. Xenografts have been a good option for regenerative treatments in response to the high search for reconstructive treatments for bone losses and lesions. Given the demand of patients who make chronic use of alcohol and present bone loss, needing xenografts, the aim of this study was to compare, by means of a histomorphological and histomorphometric analysis, the action of the biomaterials Bio-Oss® and OrthoGen® in the repair process of bone defects in the tibia of rats submitted or not to experimental alcoholism. Eighty male Wistar rats (Rattus norvegicus) aged 60 days were randomly separated into four experimental groups with 20 animals each: Bio-Oss® Water Group (GAB), Bio-Oss® Ethanol Group (GEB), OrthoGen® Water Group (GAO) and OrthoGen® Ethanol Group (GEO). The animals in the GEB and GEO were submitted to gradual adaptation to alcohol and later kept at 25% for 90 days. After this period, all animals were submitted to experimental surgery. A surgical cavity with approximately 3 mm of diameter was made in the proximal tibial epiphysis and in animals from the experimental groups GAB and GEB it was filled with the Bio-Oss® biomaterial. Animals from the experimental groups GAO and GEO had their cavity filled with the OrthoGen® biomaterial. After 10, 20, 40 and 60 days post-surgery, five animals in each group, per period, were euthanized. The histomorphological results showed that the animals from the GAB and GEB groups presented blood cells and connective tissue in the 10 and 20 days periods, and in the periods of 40 and 60 days there was formation of new bone, with the GEB group presenting greater delay in formation when compared to the GAB. In the GAO and GEO groups, bone formation started in the periods of 40 and 60 days, with a delay in formation when compared to the Bio-Oss® biomaterial. In the histomorphometric analysis, comparing the groups that received nonalcoholic liquid diet with different biomaterials (GAB and GAO), there was a significant difference in bone formation in all periods, with the Bio-Oss® Water Group presenting greater bone formation. In the groups that received an alcoholic diet, with different biomaterials (GEB and GEO), there was a difference in bone formation in the periods of 20, 40 and 60 days, in which the Bio-Oss® biomaterial presented greater means. Comparing the groups that received the same biomaterial, but with a different liquid diet (GABXGEB and GAOXGEO), there was a significant difference in bone formation in all periods (except for GAOXGEO; 10 days), in which the groups that received a liquid diet with only water presented better results. In conclusion, the Bio-Oss® biomaterial presented better performance in bone neoformation when compared to the Ortoghen®, and the alcoholic diet affected negatively the repair process.

Avaliação clínica, histológica e imunológica de enxertos ósseos alógenos fresco-congelados utilizados como técnica na preservação de rebordo alveolar pós-extração / Preservation of alveolar bone in extraction sockets using fresh-frozen bone allograft: a clinical, histological and immunological study

O osso alógeno fresco-congelado (FFBA, do inglês fresh-frozen bone allograft) é uma alternativa para os procedimentos cirúrgicos de enxerto ósseo, principalmente na preparação do rebordo alveolar para a instalação de implantes osseointegráveis. No entanto, existem alguns paradigmas que envolvem a relação entre resposta do sistema imunológico à aloantígenos presentes no enxerto e o seu comportamento clínico. Procurando entender essa relação, o FFBA foi avaliado como enxerto para preservar o rebordo alveolar pós-extração. Os resultados mostraram que embora tenha ocorrido uma redução estatisticamente significante na altura, espessura e volume do rebordo entre a avaliação inicial e final, essa redução não foi clinicamente significante, permitindo a instalação de implantes osseointegráveis. Em adição, as análises histológicas sugerem um bom comportamento do enxerto, com ausência de reação do tipo corpo estranho e formação de novo osso em todos os sítios analisados. Ao analisar o comportamento da resposta imune, os resultados mostraram que a injeção intradérmica de aloantígenos presentes no FFBA, não induziu uma reação de hipersensibilidade tardia nos pacientes após 4 meses do enxerto. Além disso, os monócitos do sangue periférico (PBMCs) dos pacientes não proliferaram frente aos aloantígenos in vitro. No entanto, os dados também demonstraram que os aloantígenos aumentam a produção de IL-2 e IFN-, mas não alteram a produção de IL-4 e IL-10, por PBMCs dos pacientes. Ao avaliar a relação entre a produção dessas citocinas e o comportamento clínico do enxerto, os dados mostram que existe uma correlação estatisticamente significante entre a produção de IL-2 in vitro e a redução (em %) da altura do rebordo alveolar, embora essa redução não tenha sido clinicamente significante. De fato, a presença de aloantígenos no FFBA não é suficiente para sua contraindicação como material de enxertia. / The fresh-frozen bone allograft (FFBA) is an alternative to surgical procedures of bone grafts, mainly in the preservation of alveolar ridge prior the installation of osseointegrated implants. However there are paradigms that surround the relation between immune response to alloantigens present inside the graft and the clinical response of the graft. An attempt to understand this relationship, the FFBA was evaluated as a graft to preserve the alveolar ridge post-extraction. The results show a statistically significant reduction in height, thickness and volume of the ridge between the initial and final examination, however this reduction was not clinically significant. The ridge preservation allowed implant installation and osseointegration. In addition, histologic analysis suggests a good performance of the graft with no foreign body reaction and formation of new bone at all sites. In analyzing the behavior immune response, the results showed that stimulation with alloantigens present in bone allograft induced no delayed hypersensitivity reaction in vivo. Additionally, periphery blood mononuclear cells (PBMC) from patients no proliferate in response to alloantigens in vitro. However, the data also demonstrated that the alloantigens increase IL-2 and IFN- production, but no IL-4 and IL-10 production, by PBMCs from patients. When evaluate the relation between the cytokines production and clinical parameters, the results demonstrate that there statistically significant correlation between IL-2 production in vitro and ridge height changes (%), although this clinical parameter is not clinically significant. In fact, the alloantigens in FFBA are not sufficient for its contraindications as grafting material.

O tratamento das lesões ósseas cavitárias com autoenxerto ainda é considerado o padrão-ouro. Seu uso, entretanto, é limitado, devido principalmente à baixa disponibilidade e à morbidade do sítio doador. Os enxertos ósseos sintéticos são estudados com o objetivo de superar as limitações decorrentes da retirada de autoenxerto ou da disponibilidade de bancos de tecidos. O vidro bioativo é um material sintético osteoindutor, osteocondutor e antibacteriano, à base de sílica e cria um arcabouço para o crescimento ósseo. Objetivo: comparar o vidro bioativo com o autoenxerto, com relação às características histomorfométricas. Métodos: foi realizado um estudo experimental prospectivo caso-controle em animais, para comparar o vidro bioativo com o autoenxerto com relação às características histomorfométricas. Oito coelhos foram submetidos a uma cirurgia em que um defeito cavitário foi criado em ambos os fêmures proximais; de um lado, o defeito ósseo criado foi preenchido com os grânulos de vidro bioativo; do outro, o defeito ósseo foi preenchido com o autoenxerto retirado do lado contralateral. Os lados foram randomizados. Catorze dias após a cirurgia os animais foram sacrificados. Resultados: a análise histológica revelou que a neoformação óssea entre os dois grupos foi equivalente, e que a contagem de osteoblastos foi superior nos fêmures tratados com vidro bioativo. A contagem de osteócitos, por outro lado, foi menor. A semelhança na intensidade de neoformação óssea coincide com o encontrado na literatura. As diferenças de contagem celular são concordantes com o mecanismo de ação do vidro bioativo: aumento do turnover ósseo, estímulo dos osteoblastos e retardo na sua diferenciação para osteócitos. Conclusão: o vidro bioativo promove neoformação óssea semelhante ao autoenxerto neste modelo animal de defeito ósseo cavitário. Nível de evidência III, estudo caso-controle / Autograft is still considered the gold standard in the treatment of cavitary bone lesions. The low availability and donor site morbidity, however, limits its use. Synthetic bone grafts have been studied in order to overcome the limitations of autograft or allograft. Bioactive glass is an osteoinductive, osteoconductive and antibacterial silica-based synthetic material. It promotes the creation of a scaffold for bone growth. Objectives: to compare bioactive glass and autograft regarding their histomorphometric characteristics. Methods: the author conducted a prospective case-control experimental study on animals, to compare the histomorphometric characteristics of bioactive glass versus autograft. Eight rabbits underwent surgery in which a cavitary defect was created in both proximal femurs. On one side, the bone defect was filled with bioactive glass granules and on the other, the defect was filled with autograft grafted from the contralateral side. The sides were randomized. Fourteen days after surgery, the animals were euthanized. Results: histologic analysis revealed that bone neoformation was equivalent among the two groups and the osteoblasts cell-count was higher in the femurs treated with bioactive glass. The osteocytes cell-count, however, was lower. The similarity in bone formation between both groups was also found in the literature. The differences in cell-count of osteocytes and osteoblasts are in accordance with the mechanism of action of the bioactive glass: increase in bone turnover, stimulation of osteoblasts and inhibition of their differentiation into osteocytes. Conclusion: bioactive glass is similar to autograft regarding bone neoformation, in this animal model of cavitary bone defects. Level of evidence III, case-control study
