Legitimerad vårdpersonals erfarenhet av ersättningsmodellen inom vårdval Halland i förhållande till individer med intellektuell funktionsnedsättning.

The care choicesystem and its replacementmodel were introduced in Region Halland in 2007 with the aim of contributing to good quality, continuity, high accessibility but also increased freedom of choice in primarycare for the individual. The aim of this study was to illustrate the experience of qualified healthcare professionals in primary care in Region Halland had regarding the replacementmodel within care choiceselection in relation to individuals with intellectual disabilities. Qualitative survey responses from healthcare professionals in primarycare were analyzed according to the Grounded Theory method. The result identified a main category, a theme, "care needs of individuals with intellectual disabilities are not fully met" and two subcategories "flaws in replacementmodel" and "complex care needs". The theme and its subcategories are described in a table that illustrates the clinical experience of the healthcarestaff regarding how care needs are met for individuals with intellectual disabilities in relation to the replacementmodel. It can be noted that the clinical effects that the replacementmodel in the healthcaresystem generated in terms of continuity and accessibility for individuals with intellectual disabilities have largely been negative. The districtnurse with a leading role in nursing, organization and leadership holds the core competence important to utilize in the pursuit of developing and sustainable care. Healthcare needs to master the responsibility that lies with them so that the care needs of individuals with IF can be fully met. Further research is needed in this area as a basis for the development of the replacementmodel and its clinical significance. / Vårdvalssystemet och dess ersättningsmodell infördes i Region Halland år 2007 med syfte att bidra till god kvalitét, kontinuitet, hög tillgänglighet men även ökad valfrihet inom primärvården för regioninvånarna. Syftet med denna studie var att belysa vilken erfarenhet legitimerad vårdpersonal inom primärvården i Region Halland hade angående ersättningsmodellen inom Vårdval i förhållande till individer med intellektuell funktionsnedsättning. Kvalitativa enkätsvar från vårdpersonal yrkesverksamma inom primärvården analyserades enligt Grounded Theory. Resultatet vilket identifierats utgörs av kärnkategorin, temat, vårdbehov för individer med intellektuell funktionsnedsättning tillgodoses ej fullt ut samt två underkategorier ersättningsmodellen brister och komplext vårdbehov. Temat och dess underkategorier belyser vårdpersonalens kliniska erfarenheter av hur vårdbehov tillgodoses för individer med intellektuell funktionsnedsättning i relation till ersättningsmodellen. Konstateras kan att de kliniska effekter vilka ersättningsmodellen inom vårdvalssystemet genererat gällande kontinuitet och tillgänglighet för individer med intellektuell funktionsnedsättning till stor del utfallit negativt. Distriktssköterskan med ledande roll inom omvårdnad, organisation och ledarskap innehar kärnkompetens viktig att tillvarata i strävan efter utvecklande och hållbar vård. Hälso- och sjukvården behöver bemästra det ansvar som åligger dem så att även vårdbehov hos individer med IF kan tillgodoses fullt ut. Vidare forskning behövs inom området som grund för utvecklandet av ersättningsmodellen och dess kliniska betydelse.

Optimization of Magnetic Resonance Diffusion Tensor Imaging for Visualization and Quantification of Periprostatic Nerve Fibers

Prostatectomy, surgical resection of the whole prostate is a common treatment for high- risk prostate cancer. Common side effects include long-time urinary and or erectile dysfunction due to damage inflicted to periprostatic nerves. The aim of this study was to identify an optimal magnetic resonance diffusion tensor imaging protocol for visualization and quantification of these nerves, as pre-surgery visualization may help nerve-sparing surgery. Both scanner filter, parameters for accelerated scan techniques, diffusion-related acquisition parameters and post- processing tractography parameters were investigated. Seven healthy volunteers were scanned with a state-of-art 3 T MRI scanner with varying protocol parameters. Diffusion data were processed and analysed using Matlab and Explore DTI. The resulting protocol recommendation included a normalized scanner filter, a parallel imaging acceleration factor of 2, partial Fourier sampling of 6/8, a right-left phase encoding direction, a b-value of 600 s/mm2, monopolar gradient polarity with applied eddy current correction, four acquisitions of 12 diffusion- sensitizing gradient directions, and a reverse phase encoding approach for correction of geometrical image distortions induced by static field inhomogeneity. For post-processing tractography, the recommended parameters were a lower limit for fractional anisotropy of 0.05, a minimum tract length of 3 centimetres and a maximum turning angle between voxels of 60 degrees. The limited parameter range that was tested and the low number of volunteers can be regarded as limitations to this study. Future work should address these issues. Furthermore, feasibility of periprostatic nerve tracking with the optimized protocol should be tested in a patient study.

Market Risk: Exponential Weightinh in the Value-at-Risk Calculation

When measuring market risk, credit institutions and Alternative Investment Fund Managers may deviate from equally weighting historical data in their Value-at-Risk calculation and instead use an exponential time series weighting. The use of expo-nential weighting in the Value-at-Risk calculation is very popular because it takes into account changes in market volatility (immediately) and can therefore quickly adapt to VaR. In less volatile market phases, this leads to a reduction in VaR and thus to lower own funds requirements for credit institutions. However, in the ex-ponential weighting a high volatility in the past is quickly forgotten and the VaR can be underestimated when using exponential weighting and the VaR may be un-derestimated. To prevent this, credit institutions or Alternative Investment Fund Managers are not completely free to choose a weighting (decay) factor. This article describes the legal requirements and deals with the calculation of the permissible weighting factor. As an example we use the exchange rate between Euro and Polish zloty to estimate the Value-at-Risk. We show the calculation of the weighting factor with two different approaches. This article also discusses exceptions to the general legal requirements.

Il existe plusieurs filtrations intéressantes définies sur la sous-algèbre de Cartan d'une algèbre de Lie simple complexe issues de contextes très variés : l'une est la filtration principale qui provient du dual de Langlands, une autre provient de l'algèbre de Clifford associée à une forme bilinéaire invariante non-dégénérée, une autre encore provient de l'algèbre symétrique et la projection de Chevalley, deux autres enfin proviennent de l'algèbre enveloppante et des projections de Harish-Chandra. Il est connu que toutes ces filtrations coïncident. Ceci résulte des travaux de Rohr, Joseph et Alekseev-Moreau. La relation remarquable entre les filtrations principale et de Clifford fut essentiellement conjecturée par Kostant. L'objectif de ce mémoire de thèse est de proposer une nouvelle démonstration de l'égalité entre les filtrations symétrique et enveloppante pour une algèbre de Lie simple de type A ou C. Conjointement au résultat et Rohr et le théorème d'Alekseev-Moreau, ceci fournit une nouvelle démonstration de la conjecture de Kostant, c'est-à-dire une nouvelle démonstration du théorème de Joseph. Notre démonstration est très différentes de la sienne. Le point clé est d'utiliser une description explicite des invariants via la représentation standard, ce qui est possible en types A et C. Nous décrivons alors les images de leurs différentielles en termes d'objects combinatoires, appelés des chemins pondérés, dans le graphe cristallin de la représentation standard. Les démonstrations pour les types A et C sont assez similaires, mais ne nouveaux phénomènes apparaissent en type C, ce qui rend la démonstration nettement plus délicate dans ce cas. / There are several interesting filtrations on the Cartan subalgebra of a complex simple Lie algebra coming from very different contexts: one is the principal filtration coming from the Langlands dual, one is coming from the Clifford algebra associated with a non-degenerate invariant bilinear form, one is coming from the symmetric algebra and the Chevalley projection, and two other ones are coming from the enveloping algebra and Harish-Chandra projections. It is known that all these filtrations coincide. This results from a combination of works of several authors (Rohr, Joseph, Alekseev-Moreau). The remarkable connection between the principal filtration and the Clifford filtration was essentially conjectured by Kostant. The purpose of this thesis is to establish a new correspondence between the enveloping filtration and the symmetric filtration for a simple Lie algebra of type A or C. Together with Rohr's result and Alekseev-Moreau theorem, this provides another proof of Kostant's conjecture for these types, that is, a new proof of Joseph's theorem. Our proof is very different from his approach. The starting point is to use an explicit description of invariants via the standard representation which is possible in types A and C. Then we describe the images of their differentials by the generalised Chevalley and Harish-Chandra projections in term of combinatorial objects, called weighted paths, in the crystal graph of the standard representation. The proofs for types A and C are quite similar, but there are new phenomenons in type C which makes the proof much more tricky in this case.

Documenting the History of Oxygen Depletion in Lake St. Croix, Minnesota, Using Chironomidae Remains in the Sedimentary Record

Lake St. Croix is a natural impoundment located at the southern end of the St. Croix River. Land use changes since European settlement (c. 1850) have resulted in nutrient runoff, eutrophication, and periodic oxygen depletion in the hypolimnion of Lake St. Croix. Establishing sound lake management practices requires knowledge of historical conditions obtained through paleoecological studies. Remains of non-biting midges (Insecta: Diptera Chironomidae) in lake sediments have been shown to be reliable indicators of past hypolimnetic oxygen conditions. Cores from two sub-basins in the lake were collected in 2006. Midge analysis indicated that shifts in species assemblages correspond to the times of land use change. Chironomus and Procladius, which are tolerant of low oxygen levels, increased in relative abundance as land use changes adversely impacted the St. Croix River’s watershed. Volume-weighted hypolimnetic oxygen concentrations were estimated using a transfer function developed for southern Ontario. Mean post-settlement chironomid reconstructed average volume-weighted hypolimnetic oxygen values were 0.73 mg/L lower than mean pre-settlement values for sub-basin 1, near Prescott, WI and 0.45 mg/L lower for sub-basin 3, near Lakeland, MN. These results indicate that oxygen depletion has occurred in the lake since the time of European settlement, and are supported by increases in the relative abundance of eutrophic midge bioindicators and the decrease in relative abundance of bioindicators of less productive conditions since the 1850s. This study, in conjunction with other historical and paleoecological studies of Lake St. Croix, provides historical data for setting management goals and strategies for Lake St. Croix.

A Quantitative Framework for Constructing a Multi-Asset CTA with a Momentum-Based Approach

Commodity Trading Advisors (CTAs) have gained popularity due to their abilities to generate an absolute return strategy. Little is known about how CTAs work and what variables are important to tune in order to create a profitable strategy. Some investors use CTA-like strategies to leverage their portfolio and create positive returns in times when the spot market is falling. The report is written for Skandinaviska Enskilda Banken and aims to give the bank and readers an understanding on how changes of parameters in a CTA strategy change the outcome of it with focus on three main measurements: Sharpe ratio, drawdown and total return.  The foundation of CTAs is that they rely on signals from some given sets of assets and make investments decisions solely based on them. CTAs can be rule-based with a binomial signal, or they can use a continual signal, like in the report. The thesis aims to recreate a CTA using a continuous momentum signal and with the signal, invest accordingly. Some different variables were tested, most importantly the report focuses on the weights of the assets and investigates if the momentum signal is good as it is or if a risk parity weighting is needed on top of the signal in order to generate a return that matches the expectations of a low drawdown and a high Sharpe ratio.  Beyond the weight allocation, different lookback periods of both the signal and weight were tested. A shorter lookback generated a quicker return that was more sensible to short trends on the market. Which in some cases was profitable but it also lost more of it accumulated return when the trend was "false". The equally weighted signal that only takes the trend into account when allocating the weights of the assets was more volatile it its returns and benefited from a long signal. The CTA results presented can only be seen as an index since it is rebalanced every rebalancing point, the frequency of those points was examined and the strategy was performing well if rebalanced once a week or once a month, every day and once a year did not yield a better result.  As expected, the CTA benefits from trend on the market, no matter the direction of it. The best periods for the CTA were when the market was very volatile, mainly 2008 and 2022. When there is no clear trend, the CTA reacts too slowly and often loses money. One important conclusion is that the CTA never should be used as an investment strategy on its own, rather as a hedging strategy that allocates a fraction of a total long-only portfolio.
