En vissnande blomma : Upplevelsen av att leva med ALS

Amyotrofisk lateralskleros (ALS), är en neurologisk sjukdom som drabbar årligen omkring 200 personer i Sverige. Sjukdomen angriper det motoriska nervsystemet där nervcellerna gradvis dör och som i sin tur leder till en successiv försvagning av kroppens muskler. Längre fram i sjukdomsförloppet blir både armar och ben totalt förlamade, därefter förlorar patienten även tal och sväljförmågan medan känslor och tankar förblir opåverkade. Överlevnadstiden efter att diagnosen har ställts är mindre än fem år. Grundorsaken till sjukdomen ALS är ännu okänd och det har fortfarande inte hittats något botemedel mot denna sjukdom. Syftet med studien är att beskriva patienters upplevelser av att leva med sjukdomen ALS. En kvalitativ studie, baserad på narrativ analys användes som metod till arbetet. För att besvara syftet, analyserades självbiografiska böcker. I studiens resultat framkom upplevelsen av både lidande och välbefinnande som en del i patienternas vardag samt upplevelse av förlorat kontroll över den egna kroppen. Patienterna plågades av tanken på att deras liv hade begränsats och att de numera var beroende av andras hjälp för att klara sig genom sin vardag. Något som ökade deras välbefinnande och fick patienterna att sträva mot sjukdomen, var all stöd och hjälp som familjen bidrog med. Att fortsätta med tidigare sysselsättningar så länge kroppen orkade, fick patienterna att må bättre och gav en känsla av välbefinnande. För att kunna utföra en god specifik omvårdnad är det viktigt att sjuksköterskan i möte med patienterna är respektfull och empatisk. / Program: Sjuksköterskeutbildning

Patientens upplevelse av att leva med ALS

Uppsatsen behandlar patientens upplevelse av att leva med sjukdomen Amyotrofisk Lateralskleros (ALS). ALS är en neurologisk sjukdom som orsakas då motoriska neuron i hjärnan och ryggmärgen successivt förloras och dör. Då den har ett snabbt förlopp med successiv försämring och slutligen dödlig utgång är det en mycket allvarlig sjukdom att drabbas av. Då neuron dör försvagas muskulaturen för att slutligen helt gå förlorad. Detta leder således till totalförlamning av kroppen. Patienten dör oftast till följd av kvävning då andningsmuskulaturen slutar fungera. Tidigare forskning visar att patienter mår dåligt av sin diagnos med exempelvis ångest, oro för närstående och rädsla inför sjukdomen och döden som faktorer. Syftet med uppsatsen är att beskriva patienters upplevelse av att leva med diagnosen ALS. För att finna lämplig information valdes att göra en innehållsanalys av biografier skrivna av ALS drabbade personer. Vad som framkommit i uppsatsen är att de författare som skrivit biografierna förhåller sig till sjukdomen relativt likvärdigt. Vid analysen framkom fyra teman: Sorg över sjukdomen, Glädje över livet, Skuld och skam och Känslan av förlorad kontroll. De drabbades ambivalens till närstående diskuteras liksom hur glädjen kan framkomma samtidigt som sorgen är närvarande. / Program: Sjuksköterskeutbildning

Amyotrofisk lateralskleros (ALS) är en sjukdom som leder till förlamning och slutligen döden. Den kan debutera i förhållandevis unga år men bryter vanligtvis ut efter 50 års ålder. Orsaken till ALS är okänd och det finns idag inget botemedel utan det är symtomen som behandlas. När en person drabbas av ALS påverkas inte bara kroppen utan hela dennes liv. ALS är en sjukdom som leder till ovanliga etiska ställningstaganden. Syftet med arbetet är att beskriva patienters upplevelser av att leva med ALS. En analys av självbiografier gjordes för att få insikt i detta. Resultatet visar att personer med ALS upplever att kroppen förändras, de upplever ett lidande, ett välbefinnande och de försöker finna svar på varför de fått sjukdomen samt förbereder sig på att möta döden. När en patient med ALS vårdas är det viktigt att sjuksköterskan kan sätta sig in i dennes livssituation, det är även viktigt att tänka på att den sjuke får tid och rum för att göra sig förstådd och se till att han/hon får de hjälpmedel som behövs för att klara av vardagen så bra som möjligt. Det finns en risk att sjuksköterskan misstolkar den svårt sjuke patienten, vilket kan leda till ett ökat lidande. När personalen inte tar den sjukes symtom, smärta och oro på allvar känner sig patienten förödmjukad. Detta kan undvikas genom att tänka på hur, vad och på vilket sätt information delges patienten. När patienter får diagnosen amyotrofisk lateralskleros (ALS) ställs hela tillvaron på sin spets. Då ALS är en kronisk sjukdom kommer döden närmare för varje dag. Hela sjukdomsförloppet innebär en förändrad livsvärd och ett outhärdligt lidande för patienten och dennes anhöriga. Fastställande av diagnos tar oftast lång tid och väntan är lång för patienten vilket kan resultera i kunskapssökande på annat håll. Vissa patienter har redan en misstanke om vilken sjukdom de drabbats av innan läkaren fastställt diagnosen. Som sjuksköterska är det viktigt att möta patienten i lidande och visa medkänsla i vårdandet. Syftet med studien är att belysa den ALS sjuka patientens vård och hur det upplevs att bli vårdad. Metoden är en kvalitativ innehållsanalys och baseras på tre självbiografier vilka utgår från upplevelsen att leva med ALS. I resultatet beskrivs upplevelsen av vården och att bli vårdad ur patientens perspektiv. Metoddiskussionen beskriver för- och nackdelar med metodvalet, där vi även kritiskt granskat oss själva. Resultatdiskussionen belyser upplevelser och områden som framkommer i resultatet, där vi reflekterar över sjuksköterskans arbete och hur det ser ut i verkligheten samt knyter an med vårdvetenskaplig forskning. / Program: Sjuksköterskeutbildning

Investigation of the cell- and non-cell autonomous impact of the C9orf72 mutation on human induced pluripotent stem cell-derived astrocytes

Amyotrophic lateral sclerosis (ALS) is a late onset neurodegenerative disorder characterised by selective loss of upper and lower motor neurons (MNs). Recently, the GGGGCC (G4C2) hexanucleotide repeat expansion in chromosome 9 open reading frame 72 (C9orf72) has been identified as the most common genetic cause of ALS, highlighting the importance of studying the pathogenic mechanisms underlying this mutation. Accumulating evidence implicates that ALS is a multisystem and multifactor disease. Specifically, non-neuronal cells, astrocytes in particular, are also affected by toxicity mediated by ALS-related mutations, and they can contribute to neurodegeneration, suggesting astrocytes as a key player in ALS pathogenesis. Here, a human induced pluripotent stem cells (iPSCs)-based in vitro model of ALS was established to investigate the impact of the C9orf72 mutation on astrocyte behaviour—both cell- and non-cell autonomous. Work in this study shows that patient iPSC-derived astrocytes recapitulate key pathological features associated with C9orf72-mediated ALS, such as formation of G4C2 repeat RNA foci, production of dipeptide repeat (DPR) proteins and reduced viability under basal conditions compared to controls. Moreover, C9orf72 mutant astrocytes in co-culture result in reduced viability and structural defects of human MNs. Importantly, correction of the G4C2 repeat expansion in mutant astrocytes through targeted gene editing reverses these phenotypes, strongly confirming that the C9orf72 mutation is responsible for the observed findings. Altogether, this iPSC-based in vitro model provides a valuable platform to gain better understandings of ALS pathophysiology and can be used for future exploration of potential therapeutic drugs.

Inicialmente, um Modelo Digital de Superfície normalizado (MDSn) é gerado através da diferença entre o Modelo Digital de Superfície (MDS) e o Modelo Digital de Terreno (MDT), obtidos a partir da nuvem de pontos LASER. Em seguida, o MDSn é segmentado para a obtenção dos polígonos que representam objetos altos da cena. Esses polígonos são projetados na imagem para restringir o espaço de busca para a segmentação da imagem em regiões. Esse processo possibilita a extração de polígonos na imagem que representem objetos altos. O processo de identificação de contornos de telhados, em meio aos objetos altos detectados na imagem, na etapa anterior, é realizado através da otimização de uma função de energia estabelecida com base em MRF que modela propriedades específicas de contornos de telhados de edifícios. No modelo MRF são utilizados tanto os polígonos extraídos da imagem quanto os extraídos dos dados VLA. A função de energia é otimizada pelo método Algoritmo Genético (AG). O método proposto nesse trabalho foi avaliado com base em dados reais - imagens aéreas de alta resolução e dados VLA. Os resultados obtidos na avaliação experimental mostraram que a metodologia funciona adequadamente na tarefa de extrair os contornos de telhados de edifícios. A função de energia proposta associada ao método de otimização AG diferenciou corretamente os contornos de telhados de edifícios dos demais objetos altos presentes nas cenas. Os contornos de telhados extraídos apresentam boa qualidade, o que é evidenciado por meio dos índices de completeza e correção obtidos pela avaliação numérica. Com base nos índices médios obtidos para cada experimento, têm-se as médias de completeza e correção para os experimentos iguais a 90,96% e 98,99%, respectivamente. Os valores máximos de completeza e correção são de 99,19% e 99,94%, respectivamente, e os valores mínimos de 78,08% e 97,46%, respectivamente. Os menores valores de completeza estão associados às áreas de oclusão por vegetação e presença de sombras. / This paper proposes a method for the automatic extraction of building roof contours through a combination of Airborne Laser Scanner (ALS) and photogrammetric data, and Markov Random Field (MRF). Initially, a normalized digital surface model (nDSM) is generated on the basis of the difference between the digital surface model and the digital terrain model, obtained from the LiDAR point cloud. Then the nDSM is segmented to obtain the polygons representing aboveground objects. These polygons are projected onto image to restrict the search space for image segmentation into regions. This process enables the extraction of polygons in the image representing aboveground objects. Building roof contours are identified from among the aboveground objects in the image by optimizing a Markov-random-field-based energy function that embodies roof contour specific properties. In the MRF model are used both polygons extracted from image and from ALS data. The energy function is optimized by the Genetic Algorithm (GA) method. The method proposed in this work was evaluated based on real data - high-resolution aerial images and ALS data. The results obtained in the experimental evaluation showed that the methodology works adequately in the task of extracting the contours of building roofs. The proposed energy function associated with the GA optimization method correctly differentiated the building roof contours from the other high objects present in the scenes. The extracted roof contours show good quality, which is evidenced by the indexes of completeness and correctness obtained by numerical evaluation. Based on the mean indexes obtained for each experiment, the average completeness and correctness for the experiments were equal to 90.96% and 98.99%, respectively. The maximum completeness and correctness values are 99.19% and 99.94%, respectively, and the minimum values are 78.08% and 97.46%, respectively. A fim de esclarecer o efeito do estresse hídrico sobre as características anatômicas e fisiológicas da soja, bem como a seletividade de herbicidas, o presente trabalho teve como objetivo examinar as alterações morfológicas e enzimáticas das cultivares de soja: MG / BR 46 Conquista (convencional) e a BRS Valiosa (transgênica) por meio de resposta a diferentes manejos de água no solo, em dois estádios vegetativos de desenvolvimento. As plantas foram submetidas a diferentes manejos de água no solo, nos estádio de desenvolvimento vegetativo V2 e V4. Foram realizadas avaliações de condutância estomática, área foliar, medidas de anatomia foliar e caulinar. Também foram avaliados os mecanismos de resposta ao estresse antioxidante pelas medições de, peroxidação lipídica, teor de H2O2, prolina, superóxido dismutase (SOD), catalase (CAT) e ascorbato peroxidase (APX), além de avaliar o comportamento dos herbicidas fomesafen a 250 g i.a. ha-1 e chlorimuron-ethyl 20 g i.a. ha-1 em plantas de soja submetidas a diferentes restrições hídricas. A cultivar BRS Valiosa RR apresentou melhor resposta adaptativa morfológica e anatômica quando submetida a uma restrição hídrica moderada de até (-0,07 MPa) quando ocorrida nas primeiras semanas de desenvolvimento das plantas. A atividade da SOD, CAT e APX das plantas transgênicas apresentaram um intenso aumento no estádio V4, enquanto que a cultivar convencional não superou a condição de estresse nesse período. Estas respostas determinaram que a BRS Valiosa (transgênica) apresentou uma resposta tardia que aumentou a sobrevivência a longos períodos de escassez de água. A escassez hídrica reduz a fitointoxicação visual quando submetida à pulverização dos herbicidas fomesafen e chlorimuron-ethyl. Constatou-se que o fomesafen e chlorimuron-ethyl proporcionaram um efeito negativo na infecção pelos rizóbios, assim como o déficit hídrico reduziu drasticamente a fixação biológica de nitrogênio e o crescimento das plantas de soja. Quando as plantas de soja foram submetidas a uma condição de escassez hídrica moderada (-0,07 MPa) a cultivar convencional teve o seu desenvolvimento mais afetado em comparação com a cultivar transgênica. Evidenciou-se, ainda, que a cultivar transgênica mostrou ter uma maior capacidade em sustentar a fixação biológica de nitrogênio quando submetida a uma condição de escassez hídrica moderada (-0,07 MPa) em comparação com a cultivar convencional. / The drought is the most limiting and severe abiotic stress for agricultural crops, because it triggers a number of physiological adaptations and anatomical modifications in the plants as an adaptive process in response to the water deficit and which regulates the reduction of water losses from the plant to the atmosphere. In order to know the effect of water stress on the anatomical and physiological characteristics of soybean, as well as the selectivity of herbicides, the present work had as objective to examine the morphological and enzymatic alterations of soybean cultivars: MG / BR 46 Conquista (conventional) and BRS Valiosa (transgenic) by means of response to different water management in the soil, in two stages of vegetative development. The plants were submitted to different soil water management, in the vegetative development stage V2 and V4. Stomatal conductance, foliar area, and leaf and stem anatomy measurements were performed. The antioxidant stress was measured by lipid peroxidation, H2O2 content, proline, superoxide dismutase (SOD), catalase (CAT) and ascorbate peroxidase (APX). The spraying of fomesafen at rate 250 g a.i ha-1 and chlorimuron-ethyl at rate 20 g a.i ha-1 in soybean plants under different water restrictions was also evaluated. BRS Valiosa RR has a better adaptive morphological and anatomical performance in response to a moderate water restriction up to (-0.07 MPa) when it occurs in the first weeks of plant development. SOD, CAT and APX activity of transgenic plants showed an intense increase in the V4 stage, while the conventional cultivar did not overcome the stress condition in this period. These responses appoint that BRS Valiosa (transgenic) has a late response, which increases survival at long periods of water scarcity. Drought reduces visual phytointoxication when sprayed with the herbicides fomesafen and chlorimuron-ethyl. It was found that the spraying of the herbicide fomesafen and chlorimuron-ethyl has a negative effect on rhizobia infection, as well as the water deficit drastically reduced the biological nitrogen fixation and the growth of the soybean plants. When soybean plants were submitted to a moderate drought condition (-0.07 MPa) the conventional cultivar had its development more affected compared to the transgenic cultivar. It was also evidenced that the transgenic cultivar showed to have a greater capacity to support the biological fixation of nitrogen when submitted to a condition of moderate drought (-0.07 MPa) in comparison to the conventional cultivar.

In these situations, The use of phytoremediation species may be an alternative in the degradation of these molecules, minimizing the carryover risks. The first step in establishing if some species can be used as a phytoremediation, is check that it has tolerance to the product, for later verify its remedial effect. Against the foregoing, The present master's thesis aimed to identify plant species capable of phytoremediation the diclosulam herbicide, elucidating the biological mechanism of phytoremediation used by plants. For this, 3 different experiments were performed. In the first experiment, conducted at the Dow Agrosciences Experiment Station, it was selected, among the species Arachis pintoi, Brachiaria brizantha, Brachiaria decumbens, Canavalia ensiformis, Cajanus cajan e Crotalaria juncea, those that shows tolerance to the diclosulam herbicide. In the second experiment, conducted in the Universidade Federal Rural do Rio de Janeiro, it was evaluated the efficiency of the previously selected plant species to remediate soil contaminated with the diclosulam herbicide, using cucumber as a bioindicator plant. In the third experiment, conducted at Centro de Energia Nuclear na Agricultura (CENA/USP), it was verified if the tolerance mechanism of the two species that presented the greatest phytoremediation potential occurs due to the phenomena of absorption and / or translocation of the herbicide by the plant. The doses of diclosulam herbicide tested in the first and second experiments were 21, 42, 63, e 84 g ha-1 + control (without presence of the herbicide); And in the third experiment, the doses that were tested was 42 g ha-1 + control (without presence of the herbicide). The species Arachis pintoi, Canavalia ensiformis, Cajanus cajan e Crotalaria juncea showed a tolerance to the diclosulam herbicide at the doses tested, and the species Canavalia ensiformis, Cajanus cajan e Crotalaria juncea were efficient in the dissipation/degradation of these molecules in the soil, it is pointed out that the last two species were those that presented the greatest remedial effect. Using the 14C radiolabeled herbicide in its moleculares structure, could be inferred through the mass balance and observation of the radioimagens of the species Crotalaria juncea e Cajanus cajan that the phytoremediative action performed by these species is, probably, through phytostimulation. In addition, there is an anatomic / metabolic barrier of diclosulam translocation in the collar of these species, which gives them tolerance to this herbicidal molecule. / Herbicidas residuais, em alguns casos, podem afetar culturas subsequentes. Nestas situa??es, o uso de esp?cies fitorremediadoras pode ser uma alternativa na degrada??o destas mol?culas, minimizando o risco de carryover. O primeiro passo para estabelecer se uma esp?cie pode ser utilizada como fitorremediadora, ? verificar se a mesma apresenta toler?ncia ao produto, para posteriormente verificar o seu efeito remediador. Diante do exposto, a presente disserta??o de mestrado teve por objetivo identificar esp?cies vegetais capazes de fitorremediar o herbicida diclosulam, elucidando o mecanismo biol?gico de fitorremedia??o empregado pelas plantas. Para isso foram realizados 3 experimentos distintos. No primeiro experimento, realizado na Esta??o Experimental da Empresa Dow Agrosciences, foi selecionado, dentre as esp?cies Arachis pintoi, Brachiaria brizantha, Brachiaria decumbens, Canavalia ensiformis, Cajanus cajan e Crotalaria juncea, aquelas que apresentem toler?ncia ao herbicida diclosulam. No segundo experimento, realizado na Universidade Federal Rural do Rio de Janeiro, avaliou-se a efici?ncia das esp?cies vegetais, previamente selecionadas, em remediar solo contaminado com o herbicida diclosulam, utilizando o pepino como planta bioindicadora. No terceiro experimento, realizado no Centro de Energia Nuclear na Agricultura (CENA/USP), foi verificado se o mecanismo de toler?ncia das duas esp?cies que apresentaram maior potencial fitorremediador ocorre devido aos fen?menos de absor??o e/ou transloca??o do herbicida pela planta. As doses do herbicida diclosulam testadas no primeiro e segundo experimento foram 21, 42, 63, e 84 g ha-1 + controle (sem presen?a do herbicida); E no terceiro experimento foram testadas as doses 42 g ha-1 + controle (sem presen?a do herbicida). As esp?cies Arachis pintoi, Canavalia ensiformis, Cajanus cajan e Crotalaria juncea apresentaram toler?ncia ao herbicida diclosulam nas doses testadas, sendo as esp?cies Canavalia ensiformis, Cajanus cajan e Crotalaria juncea eficientes na dissipa??o/degrada??o dessas mol?culas no solo, posto que as duas ?ltimas esp?cies foram as que apresentaram maior efeito remediador. Utilizando o herbicida radiomarcado com 14C em sua estrutura molecular, p?de-se inferir atrav?s do balan?o de massa e observa??o das radioimagens das esp?cies Crotalaria juncea e Cajanus cajan que a a??o fitorremediadora exercida por essas esp?cies ?, provavelmente, atrav?s da fitoestimula??o. Ademais, existe uma barreira anat?mica/metab?lica de transloca??o do diclosulam no coleto dessas esp?cies, o que lhes confere toler?ncia ? essa mol?cula herbicida.

Modélisation et analyses physiopathologiques de la Sclérose Latérale Amyotrophique liée à l'ubiquiline 2 à l'aide de vecteurs AAV10 / Modeling and pathophysiological analysis of the ubiquilin-2-linked Amyotrophic Lateral Sclerosis with AAV10 vectors

La Sclérose Latérale Amyotrophique (SLA) est une maladie neurodégénérative incurable caractérisée par la perte progressive des motoneurones. Récemment, des mutations dans le gène UBQLN2, codant pour l'ubiquiline 2, ont été identifiées dans des formes héréditaires de SLA et de SLA avec Démence Fronto-Temporale (SLA-DFT). Ces mutations induisent la formation d'agrégats positifs à l'ubiquiline 2 dans la moelle épinière de patients atteints de SLA et dans le cerveau de patients atteints de SLA-DFT, ainsi que dans d'autres formes familiales et dans des formes sporadiques de la maladie. En raison de l'implication de l'ubiquiline 2 dans la dégradation protéique intracellulaire, son rôle semble être déterminant dans la pathogénèse des différents types de SLA. Afin d'étudier les mécanismes physiopathologiques de la maladie et d'envisager des solutions thérapeutiques, nous avons créé un modèle murin de SLA et de SLA-DFT liées à l'UBQLN2 qui récapitule l'ensemble des caractéristiques associées à la maladie, suite à l'injection intracérébroventriculaire de vecteurs viraux associés à l'adénovirus (AAV) de sérotype 10, renfermant l'ADNc muté de l'ubiquiline 2. De plus, nos analyses de spectrométrie de masse ont révélé une perte d'interaction de l'ubiquiline 2 mutée avec une protéine indispensable pour le maintien du cytosquelette des neurones, l'?-spectrine. Enfin, nous avons observé que le Bleu de Méthylène augmente la quantité d'inclusions engendrées par la surexpression de l'ubiquiline 2 mutée. Cette étude a donc permis d'obtenir un paradigme solide pour l'étude de l'implication de l'ubiquiline 2 dans la SLA et pourra servir à l'élaboration de stratégies thérapeutiques efficaces. / Amyotrophic Lateral Sclerosis (ALS) is an incurable neurodegenerative disease characterized by progressive loss of motor neurons (MNs). Recently, mutations in the UBQLN2 gene, encoding ubiquilin 2, have been identified in hereditary forms of ALS and ALS with FrontoTemporal Dementia (ALS-FTD). These mutations induce the formation of ubiquilin-2-positive aggregates in the spinal cord of patients with ALS and in the brain of patients with ALS-FTD, as well as in other familial forms and in sporadic forms of ALS and ALS-FTD. The role of ubiquilin 2 seems to be crucial in the pathogenesis of the different types of ALS, due to its involvement in proteostasis. To understand the physiopathology of the disease and to envision therapeutic strategies, we have generated an ubiquilin 2-linked ALS and ALS-FTD mouse model which recapitulated disease associated phenotypes, after intracerebroventricular (ICV) injection of Adeno Associated Virus serotype rh10 (AAV10) vectors, containing the cDNA encoding mutated ubiquilin 2. Furthermore, mass spectrometry analysis revealed a loss of interaction between the mutant protein and one protein necessary for cytoskeleton maintenance of neurons, the α-spectrin. Finally, we have observed that Methylene Blue increased the number of inclusions in mice expressing the mutant ubiquilin 2. In conclusion, this study presents a solid paradigm to study the involvement of ubiquilin 2 in ALS and can be used for the development of effective therapeutic strategies.
