Comparison of Postoperative Bleeding in Total Hip and Knee Arthroplasty Patients Receiving Rivaroxaban, Enoxaparin, or Aspirin for Thromboprophylaxis

Background: Guidelines recommend the use of multiple pharmacologic agents and/or mechanical compressive devices for prevention of venous thromboembolism, but preference for any specific agent is no longer given in regard to safety or efficacy. Objective: To compare postoperative bleeding rates in patients receiving enoxaparin, rivaroxaban, or aspirin for thromboprophylaxis after undergoing elective total hip arthroplasty or total knee arthroplasty. Methods: This retrospective cohort analysis evaluated patients who received thromboprophylaxis with either enoxaparin, rivaroxaban, or aspirin. All data were collected from the electronic medical record. The primary outcome was any postoperative bleeding. Results: A total of 1244 patients were included with 366 in the aspirin, 438 in the enoxaparin, and 440 in the rivaroxaban arms. Those who received aspirin or enoxaparin were less likely to experience any bleeding compared to those patients who received rivaroxaban (P <.05). There was also a lower rate of major bleeding in these groups, but the differences were not significant. Conclusions: Aspirin and enoxaparin conferred similar bleeding risks, and both exhibited less bleeding than patients who received rivaroxaban.

Background: The Regulation of Coagulation in Orthopedic Surgery to Prevent Deep Venous Thrombosis and Pulmonary Embolism (RECORD) 1 to 4 trials compared rivaroxaban 10 mg daily with commonly used doses of enoxaparin and demonstrated similar rates of VTE and bleeding. Objective: To evaluate bleeding events between patients who received enoxaparin or rivaroxaban for prevention of venous thromboembolism (VTE) following total hip arthroplasty (THA) or total knee arthroplasty (TKA). Methods: Retrospective cohort that compared patients undergoing THA and TKA who received enoxaparin (enoxaparin) with those who received rivaroxaban (rivaroxaban) and also with those who received enoxaparin in the RECORD 1 to 4 trials (enoxaparin RECORD). The primary outcome was any postoperative bleeding, defined as a composite of major and clinically relevant nonmajor bleeding based on the definitions in the RECORD 1 to 4 trials. Results: There was a lower rate of any postoperative bleeding (2.2% vs 6.8%, P = 0.004) in patients who received enoxaparin compared with rivaroxaban, and bleeding rates between the enoxaparin group and the enoxaparin RECORD groups were similar (2.2% vs 2.5%, P = 0.085). Major bleeding in the enoxaparin group (0.2%) was not significantly different from that in the rivaroxaban group (1.4%, P = 0.12) or the RECORD group (0.2%, P = 0.93). Clinically relevant nonmajor bleeding was also lower in the enoxaparin group compared with the rivaroxaban group (2.0% vs 5.5%, P = 0.012). Conclusions: The use of enoxaparin for VTE prophylaxis following THA and TKA was associated with a lower rate of the primary outcome (any postoperative bleeding) compared with the use of rivaroxaban in a similar cohort of patients.

The Perceptions of Orthopaedic Surgeons and Patients on Opioid Reduction After Total Joint Replacement

With an increasing prevalence of osteoarthritis, total knee (TKA) and hip (THA) arthroplasty are the second-and third-most common surgeries in Canada. Although these procedures improve pain and function for a majority of patients, some patients report persistent postoperative pain. Opioids are conventionally used for these patients even though they are associated with addiction, falls, overdose, and death. Recently, many strategies have been proposed to decrease reliance on opioids after TKA and THA, including opioid-free and opioid-reduced multimodal protocols for pain management. Our findings demonstrate that Canadian patients’ receptivity to opioid-free or reduced postoperative protocols is associated with their perception of the efficacy and safety of opioids compared to non-opioid alternatives, and current opioid use. More patients are open to opioid-reduced postoperative care as they perceive that pain will be intolerable without opioids. This overlaps with many Canadian surgeons’ perception that opioids cannot be completely eliminated from postoperative pain management regimens and that patients expect an intolerable level of pain after surgery, warranting opioid use. This highlights a need for enhanced patient education on the safety and efficacy of opioids and alternatives, while managing patients’ expectations of postoperative pain control. Most Canadian and Dutch surgeons reported that they prescribe opioids to nearly all of their patients postoperatively. However, our cross-sectional study indicates that 40% of Canadian patients would be open to receiving no opioids postoperatively. In contrast, Japanese surgeons believed that opioids are unnecessary for managing postoperative pain. Variations observed among orthopaedic surgeons in Canada, the Netherlands, and Japan can be attributed to differences at the surgeon-level (individual practices and beliefs), patient-level (patient characteristics and preferences), and system-level (regulatory frameworks and healthcare systems). Further research is required on surgeon-centered approaches to mitigating opioid use, focusing on education and guidelines/policies for opioid prescribing. / Thesis / Master of Science (MSc) / Opioid analgesics are routinely prescribed to manage pain after total knee and hip replacement surgery. However, opioids are not typically more effective than alternatives and are associated with addiction, overdose, and death. This thesis aims to understand the perceptions of patients and orthopaedic surgeons on opioid use after total knee and hip replacement surgery. The findings demonstrate that more patients are open to receiving opioid-reduced surgery compared to opioid-free surgery, with receptivity being associated with patients’ perceptions of opioid efficacy and safety and current opioid use, highlighting a need for improved patient education. Additionally, orthopaedic surgeons identified challenges and facilitators to postoperative opioid reduction in six key areas: opioid prescribing practices, patient factors, collaborative care, policies/guidelines, surgeon education and training, and personal perceptions/beliefs. Compared to Canadian and Dutch surgeons, Japanese surgeons heavily relied on non-opioid medications as they believed that opioids are unnecessary for managing postoperative pain.

INTRODUÇÃO: O diagnóstico microbiológico das infecções em artroplastias é de fundamental importância para a definição da estratégia de uso dos antimicrobianos. As culturas microbiológicas convencionais apresentam elevados índices de falso-negativos, em especial, nas infecções crônicas, em que é frequente a presença do biofilme aderido ao implante. A utilização de amostras deste biofilme, viáveis à cultura, a partir de seu descolamento do implante pela técnica de sonicação, tem mostrado aumento da sensibilidade em publicações recentes. O objetivo deste estudo foi comparar os resultados das culturas microbiológicas de fragmentos de tecido periprotético, realizadas em meio sólido, àquelas obtidas pelo cultivo do líquido oriundo da sonicação do implante removido, semeado, inicialmente, em frascos de hemocultura e, posteriormente, em meio sólido. MÉTODOS: Neste estudo de análise descritiva, prospectivo e comparativo, 30 pacientes com diagnóstico de infecção em artroplastias de joelho ou quadril, com mais de 90 dias de história, tiveram seus implantes cirurgicamente removidos e foram coletadas seis amostras do tecido periprotético, de locais previamente determinados, para a realização de cultura microbiológica em meios sólidos. Simultaneamente, os implantes foram submetidos ao processo de sonicação e o material resultante foi injetado em frascos de hemocultura BD Bactec e submetidos ao processo de cultura automatizada. Todas as amostras foram pesquisadas quanto à presença de bactérias aeróbias, anaeróbias, micobactérias e fungos, e os resultados comparados por meio de análise estatística, em busca da superioridade de um método sobre o outro. Como objetivo secundário, buscou-se analisar quais os pontos de coleta do tecido periprotético teriam maior sensibilidade em suas culturas. RESULTADOS: Não houve diferenças estatisticamente significantes da amostra em relação ao gênero, patologia articular primária, tipo de artroplastia, localização do implante ou lateralidade. Em 17 casos (56,7%), houve uso de antimicrobianos no período de 15 dias que antecederam a retirada do implante. O método de cultura do fluido de sonicação mostrou sensibilidade de 86,7% e foi superior, de modo estatisticamente significante (P < 0,001), em relação à cultura dos fragmentos periprotéticos, cujos resultados tiveram sensibilidades entre 26,7 e 53,3%. O uso de antibioticoterapia recente não interferiu de modo estatisticamente significante na sensibilidade da cultura do líquido oriundo da sonicação. (P = 0,113). Quanto ao objetivo secundário, a coleta de fragmentos da membrana periprotética mostrou maior sensibilidade, estatisticamente significante, na comparação com três dos demais cinco pontos de coleta (P < 0,05). CONCLUSÕES: A cultura microbiológica do líquido obtido por sonicação dos implantes removidos de pacientes com diagnóstico de infecção periprotética e semeado inicialmente em frascos de hemocultura mostrou ter sensibilidade superior, estatisticamente significante, à cultura convencional de fragmentos do tecido periprotético semeados em meios sólidos. A cultura microbiológica da membrana periprotética mostrou ter maior sensibilidade em relação à maioria dos outros sítios que tiveram fragmentos de tecido periprotético pesquisados. / INTRODUCTION: Microbiological diagnosis in periprosthetic infection is of fundamental importance to define the most appropriate antimicrobial strategies. Conventional microbiological cultures have high rates of false negatives, especially in chronic infections, in which there is often the presence of biofilm attached to the implant. The use of samples of viable culturable biofilm taken from the detachment of the implant by sonication technique has been shown to increase the sensitivity in recent studies. The objective of this study was to compare the results of microbiological cultures of periprosthetic tissue fragments, made in a solid medium, to those obtained through the cultivation of the liquid coming from the sonication of the removed implant initially seeded in blood culture bottles and later in solid medium. METHODS: Using descriptive, prospective and comparative analysis, thirty patients with a diagnosis of infected knee or hip arthroplasty, with more than ninety days of history, had their implants surgically removed. Six periprosthetic tissue samples, collected at predetermined places, were used for microbiological culture on solid media. Simultaneously, the implants were subjected to the sonication process, and the resulting material was injected into vials of BD Bactec blood cultures and subjected to an automated culture process. All samples were screened for the presence of aerobic bacteria, anaerobes, mycobacteria and fungi and the results compared by statistical analysis to find the superiority of one method over the other. As a secondary objective, this study sought to analyze which of the periprosthetic tissue collecting points would have greater sensitivity in their cultures. RESULTS: There were no statistically significant differences in the sample as related to gender, primary joint pathology, type of arthroplasty, implant location or laterality. In seventeen cases (56.7%), antimicrobials were used within the 15-day period leading up to the removal of the implant. The sonication culture fluid showed a sensitivity of 86.7% and was higher, statistically significant (P < 0.001) in relation to the culture of periprosthetic fragments, where results displayed sensitivities between 26.7 and 53.3%. The use of recent antibiotic therapy did not affect the sensitivity of the liquid coming from the sonication culture, which was not statistically significant (P = 0.113). As for the secondary objective, the collection of periprosthetic membrane fragments showed higher sensitivity, statistically significant (P < 0.05), as compared to three of the remaining five collecting points. CONCLUSIONS: The microbiological culture liquid obtained by sonication of the implants removed from patients with diagnosis of periprosthetic infection and initially seeded in blood culture bottles was shown to have superior sensitivity, statistically significancy, as compared to conventional culture of the periprosthetic tissue fragments seeded on solid media. The microbiological culture of the periprosthetic membrane seems to be more sensitive compared to most other sites that had periprosthetic tissue fragments surveyed

Total Shoulder Arthroplasty : clinical and radiological studies on the implant positioning and fixation

Shoulder arthroplasty surgery has shown remarkable progress during the last few decades. A number of factors affect postoperative range of motion, pain and prosthetic durability. Among these factors, the length of the lever arm and joint stability is the ones that can be altered by the selected prosthetic component. It is uncertain how much of the normal anatomy needs to be re-established. Stemless prostheses with total reliance on metaphyseal fixation were introduced in France in 2004 (TESS, Zimmer Biomet). The goals were to avoid stem-related complications. Stemless implants have other potential benefits, including the ability to restore shoulder anatomy. Study I: This is a prospective cohort study of 49 patients with one of two versions of the TESS prosthesis (anatomical or reverse) with clinical and radiological follow-up ranging from 9–24 months. The TESS prosthesis showed short-term results that were comparable with other shoulder prosthetic systems. Study II: This is a prospective comparative non-randomised study of 37 patients (40 shoulders) who underwent TESS reverse shoulder arthroplasty (RSA) with a follow-up ranging from 15–66 months. We found a significant improvement in functional outcome and reduction of pain in both stemmed and stemless groups. Glenoid overhang influenced the occurrence of scapular notching (SN). Study III: This is a radiological study showing that CT had a good reliability and reproducibility in estimating LHO. Study IV: This is a prospective radiological study of 69 patients (70 shoulders) with primary osteoarthritis (OA) who had undergone stemless total anatomical shoulder arthroplasty (TSA). This study showed that stemless anatomical TSA could be useful in restoring shoulder anatomy. Study V: This is a prospective study of 44 patients with OA who had undergone stemless anatomical TSA with a clinical and radiological follow up ranging from 12 – 50 months. Our study showed that LHO reconstruction close to the anatomy of a healthy contralateral shoulder improved shoulder function. Stemless anatomical TSA help to restore LHO. Increasing LHO may have a negative effect on shoulder function at three months but had no effect at 12 months. The main conclusions of this thesis are: 1. TSA (anatomic and reverse) using stemless humeral components is reliable if bone quality is adequate. The complication rate is comparable with other shoulder prosthetic systems. 2. Glenoid overhang decreased complications in RSA. 3. LHO measurement on AP radiographs is less reliable and underestimates the distance when compared with CT. 4. Stemless TSA could be of help in reconstructing shoulder anatomy. 5. Shoulder reconstruction close to the anatomy of a healthy contralateral shoulder improves shoulder function. / Axelprotes kirurgi har visat avsevärd utveckling under de senaste decennierna. Ett antal faktorer påverkar postoperativt rörelseomfång, smärta och proteshållbarhet. Bland dessa faktorer utgör längden av hävarmen och ledstabilitet de faktorer som kan ändras genom val av proteskomponent. Det är osäkert om den normala anatomin måste återupprättas. Oskaftad protes med eliminering av humerusstamm och tillit till metafysär fixering introducerades i Frankrike år 2004 (TESS, Zimmer Biomet). Målen var att undvika stam relaterade komplikationer. Oskaftat implantat har andra potentiella fördelar, inklusive möjligheten att återställa axelnsanatomi. Vår studie visade att LHO rekonstruktion till den friska axeln förbättrar axelfunktion. Oskaftat implantat kan vara av hjälp till att återställa LHO. Ökad LHO kan ha en negativ effekt på axelnsfunktion vid tre månader, men denna effekt påvisade ej vid 12 månader. De viktigaste slutsatserna i denna avhandling är: 1. Oskaftad total axel artroplastik (anatomisk och omvänd) är tillförlitlig om benkvalitén är god med komplikationer som var jämförbar med andra axelprotessystem. 2. Glenoid overhang minskar komplikationer vid omvänd axelartroplastik. 3. LHO mätningen på röntgen är mindre tillförlitlig och underskattar avståndet jämfört med CT. 4. Oskaftad axelprotes skulle kunna vara till hjälp för att rekonstruera axelnsanatomi. 5. O risco de infecção varia de 1 a 5%, e acarreta grande impacto negativo na qualidade de vida do paciente, além de envolver custos expressivos. O diagnóstico e tratamento precoce da infecção permite um tratamento mais rápido e efetivo, o que pode melhorar o resultado clínico final. Sabe-se que germes patogênicos podem contaminar o campo cirúrgico durante a cirurgia, apesar de rigorosa técnica asséptica, mas existe debate na literatura sobre a existência de benefício em identificar tais germes já na artroplastia primária em pacientes não infectados. Embora a coleta de múltiplas amostras para cultura seja prática bem estabelecida em procedimentos de revisão, permanece controversa a coleta de culturas em artroplastias primárias. Diversas bactérias podem contaminar o campo operatório durante a cirurgia, mas somente seriam capazes de causar infecção aquelas que contaminarem os planos anatômicos mais profundos. O objetivo deste estudo foi investigar se há valor prognóstico na realização de cultura de amostras profundas de tecido ósseo e de partes moles durante a realização da artroplastia total primária do quadril, buscando correlação entre a positividade das culturas e de infecção periprotética subsequente. MÉTODOS: Estudo retrospectivo, utilizando informações obtidas do prontuário médico e do sistema informatizado de consulta de exames laboratoriais. Foram colhidas amostras profundas (osso femoral, acetabular e cápsula) de 426 casos consecutivos de artroplastia total primária eletiva do quadril. Os pacientes foram seguidos por no mínimo três anos e foram identificados os pacientes que evoluíram com infecção. O perfil microbiológico das culturas nas cirurgias primárias foi comparado com aquele dos pacientes infectados. Adicionalmente, os prontuários foram revisados para a identificação de fatores de risco associados a maior risco de infecção periprotética. RESULTADOS: 54 cirurgias (12,6%) apresentaram culturas positivas. 16 casos (3,8%) evoluíram com infecção, dos quais cinco haviam apresentado cultura positiva na cirurgia primária. Os germes isolados nos pacientes infectados não coincidiram com os identificados na cirurgia índice. A taxa de infecção foi de 9,3% nos pacientes com cultura positiva e 3% naqueles com cultura negativa (p < 0,05), com um \"odds ratio\" de 3,34 (IC95% 1,09-10,24). Pacientes submetidos a alguma cirurgia prévia no quadril apresentaram taxa de infecção de 8,5%, sendo que tal percentual foi de 2,9% nos pacientes sem cirurgia prévia (p < 0,05). O único fator de risco associado a uma maior positividade de culturas foi o tabagismo (10,5% vs 23,6%, p < 0,05). CONCLUSÃO: Dentre os pacientes submetidos à artroplastia total primária do quadril, aqueles com cultura positiva apresentaram maior risco de desenvolvimento de infecção periprotética (IPP). Pacientes submetidos a algum procedimento cirúrgico prévio no mesmo quadril apresentaram maior risco de IPP / INTRODUCTION: The total hip arthroplasty is one of the most frequent procedures in orthopedics. The risk of infection usually varies from 1% to 5% and has a great negative impact on patients\' quality of life, while also involving significant costs. The diagnosis and early treatment of infection allows a faster and more effective treatment, which can improve the final clinical result. It is known that pathogenic germs can contaminate the surgical theater, despite strict aseptic technique, but there is debate in the literature regarding the benefit of identifying such germs during the primary arthroplasty in non-infected patients. Although the obtainment of multiple samples for culture is a well-established practice in revision procedures, the obtainment of cultures in primary arthroplasties remains controversial. Several bacteria can contaminate the operative field during surgery, but only those that contaminate the deeper anatomical layers should be capable of causing infection. The aim of this study was to investigate if there is prognostic value in the culture of deep samples of bone and soft tissues during primary total hip arthroplasty, seeking a correlation between the positivity of the cultures and subsequent periprosthetic infection. METHODS: This was a retrospective study, using information obtained from medical records and a computerized system for consultation of laboratory tests. Deep samples (femoral bone, acetabular bone and capsule) were collected from 426 consecutive cases of elective primary total hip arthroplasty. Patients were followed up for at least three years and patients who had developed infection were identified. The microbiological profile of cultures in the primary surgeries was compared to that of the infected patients. In addition, the medical records were reviewed for the identification of risk factors associated with a higher risk of periprosthetic infection. RESULTS: 54 surgeries (12.6%) presented positive cultures. 16 cases (3.8%) developed infection, of which five had presented a positive culture in the primary surgery. Isolated germs in infected patients did not coincide with those identified in the index surgery. The infection rate was 9.3% in patients with positive culture and 3% in those with negative culture (p < 0.05), with an odds ratio of 3.34 (95% CI 1.09-10.24). Patients who had a previous hip surgery had an infection rate of 8.5%, and this percentage was 2.9% in patients with no previous surgery (p < 0.05). The only risk factor associated with a higher risk of culture positivity was smoking (10.5% vs. 23.6%, p < 0.05). CONCLUSION: Among patients undergoing primary total hip arthroplasty, those with a positive culture had a higher risk of developing periprosthetic infection (PPI). Effet de l’obésité sur la récupération fonctionnelle de la marche après prothèses totales de hanche et de genou : analyse biomécanique et clinique / Obesity and functional gait recovery after total hip and total knee arthroplasties : clinical and biomechanical analysis

Introduction : L’influence de l’Index de Masse Corporelle (IMC) sur la récupération des paramètres de la marche après prothèse totale de hanche (PTH) et de genou (PTG) est méconnue. Peu d' étude se sont intéressées à l'influence de l’IMC sur la récupération des amplitudes articulaires actives à la marche (AAA) ou de la vitesse de marche après PTH et PTG. Notre objectif était d’évaluer cet impact en préopératoire et en post-opératoire par une évaluation clinique et biomécanique. Notre hypothèse était que l’obésité (IMC>30kg/m2) serait un facteur péjoratif de récupération des paramètres de marche.Matériel et méthodes : Après avoir effectué une revue systématique de la littérature sur la récupération fonctionnelle après prothèses totales du membre inférieur (PTH et PTG) afin de nous assurer de l’originalité de notre travail et de sa pertinence nous avons réalisé 2 études prospectives indépendantes : en incluant 76 PTH chez des patients coxarthrosiques d’une part et 79 PTG chez des patients gonarthrosiques d’autre part. Tous les patients bénéficiaient d’une analyse quantifiée de la marche en préopératoire et en post-opératoire (à 6 mois pour les PTH et à un an pour les PTG). Un groupe de témoins sains appariés suivait le même protocole. La vitesse de la marche, l’AAA de hanche et de genou, ainsi que des scores fonctionnels et de qualité de vie étaient évalués, pour tous les paramètres le gain était calculé. Les patients étaient répartis en 2 groupes : patients non-obèses (G1) : IMC<30 et patients obèses (G2) : IMC≥30 dans chacune des cohortes. Des t-tests appariés étaient utilisés pour évaluer (1) les différences pré- et post-opératoires entre G1 et G2, puis les différences de gain entre G1 et G2. Enfin une régression linéaire uni- et multivariée recherchait les associations entre l’IMC, la vitesse de la marche et les AAA.Résultats Pour les 2 cohortes : en préopératoire, la vitesse de la marche et les AAA étaient significativement plus bas chez les obèses. En post-opératoire la vitesse de la marche et les AAA étaient significativement plus bas pour tous les patients opérés d’une PTG ou d’une PTH comparativement au groupe témoin. En pré-opératoire les obèses étaient plus symptomatiques. Une différence significative était retrouvée entre G1 et G2 concernant l’amélioration de la douleur en faveur des obèses (sur le gain de WOMAC douleur pour les PTG et d’EVA pour les PTH). Pour tous les autres paramètres les gains n’étaient pas différents entre G1 et G2. La régression uni- et multivariée ne montrait pas de relation significative entre la vitesse de la marche, les AAA et l’IMC.Conclusion : L’IMC n’a pas d’effet délétère direct sur la récupération des paramètres de marche après une PTH ou une PTG. En effet malgré une symptomatologie pré-opératoire plus forte les patients obèses présentent une amélioration comparable en termes de gain aux patients non-obèses. Cependant les paramètres de marche post-opératoire après une PTH ou une PTG demeurent significativement plus bas que ceux des témoins et ce quel que soit l’IMC des patients opérés, sans récupération ad integrum clinique ou biomécanique de leur fonction de la hanche ou du genou. Ainsi l’obésité semble présenter un effet similaire sur la récupération des paramètres de marche après une PTH ou une PTG. / Introduction : Body Mass Index (BMI) impact on gait recovery after total hip and total knee arthroplasty (THA and TKA) remains misunderstood. Few studies have analyzed the influence of BMI on the recovery of hip and knee range of motion (ROM) during gait or walking speed after THA and TKA. Our objective was to assess this impact preoperatively and post-operatively using both 3D gait analysis and clinical assessment. Our hypothesis was that obesity (BMI> 30kg /m2) would be a negative factor in the recovery of gait parameters.Material and methods: After conducting a systematic review of the literature on functional recovery after total prostheses of the lower limb (THA and TKA) to ensure the originality of this work and its relevance we conducted 2 independent prospective studies: including 76 THA in hip osteoarthritis patients on the one hand and 79 TKA in knee osteoarthritis patients on the other hand. All patients had a quantified gait analysis of preoperatively and postoperatively (at 6 months for THA and 1 year for TKA). A control group of matched healthy people followed the same protocol. The gait speed, ROM hip and knee, as well as functional and quality of life scores were evaluated, for all parameters the gain was calculated. Patients were divided into 2 groups in each cohort: non-obese patients (G1): BMI <30kg/m2 and obese patients (G2): BMI≥30kg/m2. Paired t-tests were used to evaluate the pre- and post-operative differences between G1 and G2, then the differences in gain between G1 and G2. Finally, a uni- and multivariate linear regression sought associations between BMI, gait speed, and ROM.Results: In both cohorts: preoperatively, gait speed and ROM were significantly lower in obese patients. Postoperatively, walking speed and ROM were significantly lower for all patients undergoing TKA or THA compared with the control group. Preoperatively obese patients were more symptomatic. A significant difference was found between G1 and G2 regarding pain improvement in favor of the obese (on the gain of WOMAC pain for TKA and analogic pain scale for THA). For all other parameters the gains were not different between G1 and G2. Uni- and multivariate regression showed no significant relationship between gait speed, ROM and BMI.Conclusion: BMI has no direct negative effect on recovery of walking parameters after THA or TKA. Indeed despite a higher preoperative symptomatology obese patients show a comparable improvement in terms of gain to non-obese patients clinically and biomechanically. However, postoperative gait parameters after THA or TKA remain significantly lower than those of controls, regardless of the BMI of patients without full clinical or biomechanical recovery. Though, obesity appears to have a similar effect on the gait parameters recovery after THA or TKA.
