Medication utilization, adherence and use of relief agents among Texas medicaid patients with persistent asthma

Asthma is a prevalent chronic disease with high health care utilization and excessive costs. Adherence to asthma long-term controller medications is one of the key drivers to improve asthma management. The purpose of this study was to investigate how patient characteristics and medication regimens, including adherence, impact asthma-related outcomes, which was represented by the use of quick-relief medications: oral corticosteroids (OCSs) and short-acting [beta]2-agonists (SABAs). Texas Medicaid prescription claims from July 1, 2008 to August 31, 2010 were retrospectively analyzed. Patients aged 5-63 years with a primary diagnosis of asthma (ICD-9 code 493) and four or more prescription claims for any asthma medication in one year (persistent asthma) were included. The primary outcomes were adherence to asthma long-term controller medications estimated by Proportion of Days Covered (PDC) and asthma control defined by the number of OCS and SABA claims. A total of 32,172 patients were included in the study. The majority of the patients were on monotherapy (58.9%), with leukotriene receptor antagonists (LTRAs) being most commonly prescribed (65.6%). Among patients on combination therapy (41.1%), fixed dose combinations of inhaled corticosteroids (ICSs) plus long-acting β-agonists (LABAs) were most commonly prescribed (51.9%). Mean (±SD) adherence to controller therapy was 32.2% (±19.7). Patients on LTRAs were 7.7% more adherent compared to patients on ICS therapy, and patients on fixed dose combination therapy were 4.5% more adherent compared to patients on concurrent therapy, while controlling for covariates (age, gender, race, number of non-study medications) (p<0.0001). The likelihood of having 6 or more claims for SABA in one year was significantly higher for adherent (PDC≥50%) patients (p<0.0001). However, the likelihood of OCS use was significantly higher for nonadherent (PDC<50%) patients compared to those who were adherent (p<0.0001). In conclusion, adherence to long-term controller medications was low among patients with asthma. Healthcare providers should emphasize adherence to controller therapy among patients with asthma. Being adherent is critical in prevention of asthma exacerbations requiring OCS use. As frequent SABA use signals poor asthma control, healthcare providers and patients (both adherent and nonadherent) should be educated/re-educated about SABA inhaler use. / text

The Effects of Bariatric Surgery on Medication and Health Services Utilization Among Members From a Large Health Benefits Company

The main objectives of this dissertation were to examine the effects of bariatric surgery on medication and health services utilization among a cohort of Commercial and Medicare insured members from a large health benefits organization in the U.S.. A total of 1,492 members with morbid obesity underwent gastric bypass (n=785) or gastric banding (n=707) procedure between January 2005 and June 2008. Administrative claims databases were accessed and three data files including a member file, a medical file and a pharmacy file were merged at the member level. Non-parametric Wilcoxon signed rank tests revealed that the average number of all prescription claims were significantly lower during the 12 months post-surgery, compared to the 12 months pre surgery (p<0.0001). Moreover, McNemar’s Chi Square analyses showed that after the surgery, there was a statistically significant (p=<0001) decline in the proportion of members utilizing antihypertensives, antidiabetics and antihyperlipidemics. Our results also showed that the average number of prescription claims for each of these medication groups significantly declined during the 12-month post-surgery period, among members who had at least one prescription for these medications before the surgery (p<0.0001). Logistic regression modeling revealed that members who underwent bypass procedures were more likely to discontinue antihypertensives (OR=2.04; 95% CI= 1.30-3.23) , antihyperlipidemics (OR=3.25; 95% CI 1.96-5.40) and antidiabetics (OR=1.89; 95% CI 1.13–3.08) post-surgery than members who underwent banding procedures. In terms of medical services utilization, our results showed a significant decline in the average number of medical claims for all outpatient services overall from the 12 months pre to the 12 months post-surgery (p<0.0001). In contrast, the average number of medical claims for emergency room and inpatient hospitalizations were significantly increased from the pre to the post-surgery period (p<0.01). Logistic regression modeling revealed that the type of bariatric surgery was a significant positive predictor for inpatient hospitalizations post-surgery (OR =2.33; 95% CI= 1.76-3.08; p<0.0001) but not for emergency room visits (OR=1.23; 95% CI 0.97–1.56). The implications of the findings from a managed care perspective are discussed, along with limitation and future directions.

Aim Musculoskeletal disorders are a major public health problem in most developed countries. As a main cause of chronic pain, they have resulted in an increasing prescription of opioids worldwide. With regard to the situation in Germany, this study aimed at estimating the prevalence of musculoskeletal diseases such as chronic low back pain (CLBP) and hip/knee osteoarthritis (OA) and at depicting the applied treatment patterns. - Subject and methods German claims data from the InGef Research Database were analyzed over a 6-year period (2011–2016). The dataset contains over 4 million people, enrolled in German statutory health insurances. Inpatient and outpatient diagnoses were considered for case identification of hip/knee OA and CLBP. The World Health Organization (WHO) analgesic ladder was applied to categorize patients according to their pain management interventions. Information on demographics, comorbidities, and adjuvant medication was collected. - Results In 2016, n = 2,693,481 individuals (50.5% female, 49.5% male) were assigned to the study population; 62.5% of them were aged 18–60 years. In 2016, n = 146,443 patients (5.4%) with CLBP and n = 307,256 patients (11.4%) with hip/ knee OA were identified. Of those with pre-specified pain management interventions (CLBP: 66.3%; hip/knee OA: 65.1%), most patients received WHO I class drugs (CLBP: 73.6%; hip/knee OA: 68.7%) as the highest level. - Conclusion This study provides indications that CLBP and hip/knee OA are common chronic pain conditions in Germany, which are often subjected to pharmacological pain management. Compared to non-opioid analgesic prescriptions of the WHO I class, the dispensation of WHO class II and III opioids was markedly lower, though present to a considerable extent.

Utilisation des bases de l'Assurance Maladie pour l'analyse de l'utilisation et de la sécurité des anticoagulants oraux dans la fibrillation auriculaire / Patterns of use and safety of direct oral anticoagulants in patients with nonvalvular atrial fibrillation : findings from the French healhcare databases

En France, depuis 2012, les anticoagulants oraux directs (AOD), indiqués dans la prévention des accidents vasculaires cérébraux chez les patients avec fibrillation auriculaire (FA) non valvulaire, sont une alternative aux antivitamines K (AVK) pour lesquels une sous-prescription et un défaut d’observance ont été largement décrits. L’objectif de cette thèse était, dans le cadre du programme de travail du Département études de santé publique de la Caisse nationale de l’Assurance maladie, d’étudier à partir des bases de données médico-administratives (BDMA) françaises l’utilisation et la sécurité du traitement anticoagulant oral (ACO) en situation réelle de soins chez les patients avec FA. Un premier travail a permis de construire un algorithme pour identifier l’indication FA chez les patients débutant un ACO et pour lesquels aucun diagnostic de FA n’était retrouvé dans les BDMA. Un deuxième travail a porté sur l’évaluation de l’adhésion au traitement ACO chez les nouveaux utilisateurs : au cours de l’année suivant l’initiation du traitement, au moins un patient sur trois arrêtait son traitement, dabigatran ou rivaroxaban, et la persistance à ces traitements n’était pas meilleure que celle des AVK. Un troisième travail a décrit une amélioration de la couverture anticoagulante chez les patients avec FA entre 2011 et 2016. Cependant, un patient sur trois était encore sans traitement ACO en 2016 et de potentiels mésusages à l’instauration du traitement AOD ont été identifiés, dont un signal de sous-dosage. Enfin, une analyse en symétrie de séquences a suggéré que l’initiation des AOD serait associée à la survenue rare d’atteintes toxiques aiguës du foie et à la survenue plus fréquente de troubles gastrointestinaux. Avec la description de la population et de l’utilisation rejointes des traitements ACO en France chez les patients avec FA, ces résultats encouragent la poursuite de la surveillance des effets indésirables non hémorragiques des AOD et l’amélioration de leur utilisation. / Direct oral anticoagulants (DOAC) were gradually introduced since 2012 in France for stroke and systemic embolism prevention in patients with nonvalvular atrial fibrillation (AF), as a more convenient alternative to vitamin K antagonists (VKA) for which underprescribing and high rates of discontinuation have been frequently reported. As part of the work programme of the Department of Studies in Public Health, French National Health Insurance, the aim of this dissertation was to assess the patterns of use and safety of oral anticoagulant (OAC) therapy in real-life setting using the French healthcare databases. First, an algorithm was developed to identify AF in outpatients initiating OAC and for whom no diagnosis of AF was found in the French claims data. Second, 1-year dabigatran and rivaroxaban adherence rates were estimated in nonvalvular AF patients and 1-year non-persistence rates were compared versus VKA. At least 1 in 3 dabigatran or rivaroxaban new users was found to be non-adherent to treatment. Treatment persistence among dabigatran or rivaroxaban new users was not found to be better versus VKA therapy. Third, OAC therapy use was found to have increased following in France between 2011 and 2016 but remained suboptimal with 1 in 3 patients with AF not treated by OAC therapy. Several situations of inappropriate use of DOAC were identified including potential undertreatment by inappropriate dosing. Finally, a sequence symmetry analysis suggested that DOAC therapy is associated with rare but severe liver injury and more frequent gastrointestinal disorders. A low risk of kidney injury with DOAC therapy can also not be excluded. These findings advocate further investigation of the potential risk of DOAC underdosing at initiation and the continuous monitoring of the non-bleeding adverse events of DOAC therapy.

Background About 9 % of all children in Germany are born preterm. Despite significant improvements of medical care, preterm infants are at a greater risk to develop short and long term health complications. Negative consequences of preterm birth include neurodevelopmental disabilities, behavioral problems or learning disorders. Most data on effects of prematurity are derived from single or multi-center studies and not population-based. Since some of the long term problems of preterm delivery are associated with a disturbed parent-child interaction originating in the neonatal period, several intervention programs became available aiming to strengthen the early parent-child relationship. However, there is insufficient knowledge regarding the psychosocial and socioeconomic impact of these interventions. Prior to introducing them into routine care, those effects have to be rigorously evaluated. The population-based cohort study EcoCare-PIn (Early comprehensive Care of Preterm Infants—effects on quality of life, childhood development, and healthcare utilization) will investigate the following primary research questions: 1) What are the short- and long-term consequences of preterm birth with regard to parental stress, parent-child relationship, childhood development, quality of life and healthcare utilization including costs? 2) Does early family-centered psychosocial care prevent the hypothesized negative consequences of preterm birth on the above mentioned outcomes? Methods/Design EcoCare-PIn examines the research questions by means of a linkage of a) pseudonymized administrative individual-level claims data from the German statutory health insurance AOK PLUS on approximately 140,000 children born between 2007 and 2013 in Saxony, and b) primary data collected from the parents/caregivers of all very low birth weight (<1,500 g; n = 1,000) and low birth weight infants (1,500 to 2,500 g; n = 5,500) and a matched sample of infants above 2,500 g birth weight (n = 10,000). Discussion In Saxony, approximately 50 % of all individuals are insured at the AOK PLUS. The linkage of patient-level administrative and primary data is a novel approach in neonatal research and probably the only way to overcome shortcomings of studies solely relying on one data source. The study results are based on an observation period of up to 8 years and will directly inform perinatal healthcare provision in Saxony and Germany as a whole.
